Abstract:
Patients and doctors often define constipation differently. The normal frequency of defaecation is once every 3 days to three times per day, and constipation may be defined as abnormally infrequent defaecation. A change in the normal pattern and frequency for the particular patient is pertinent. There are numerous causes of constipation, and most can be encountered in both primary and secondary care. In patients with chronic constipation without an evident cause, irritable bowel syndrome (IBS) is the cause in 59%, pelvic floor dysfunction in 25%, slow transit in 13%, and a combination of pelvic floor dysfunction and slow transit in 3%. Constipation affects twice as many women as men, with a higher prevalence in pregnant women. Prevalence is also greater in the elderly, affecting ~20% in the community.

Abstract:
Chronic functional constipation is a common problem in childhood. Without early treatment, the condition is likely to impact all aspects of the child’s life, including education and psychological well-being as well as physical growth and development. The key to successful management is early diagnosis and prompt treatment with an emphasis on practical management strategies with multidisciplinary support where needed. Conventional treatment relies on patient education, behavioural modification, and drugs. This chapter discusses the epidemiology, clinical assessment, indications for further investigation, management including behavioural strategies, and drug treatment.

Abstract:
Constipation is far more common in palliative care than diarrhoea and results not just from the use of opioids but also from the multifactorial effects of debility secondary to disease. Most palliative care patients will require regular administration of an oral laxative in a dose titrated against response, with the aim of avoiding the use of suppositories or enemas if possible as these are less liked. The lack of clear differences in laxative efficacy means that cost and patient choice are key factors in guiding treatment. Diarrhoea in palliative care most often results from excess laxative or from common infections that can be simply managed. Bowel shortening or diversion causes more resistant diarrhoea. Cytotoxic chemotherapy can lead to diarrhoea either as an adverse effect of treatment or from potentially life-threatening neutropenic colitis.
