Full textThe concept of sexual health was institutionalized by the World Health Organization (WHO) in the 1970s, when controversies between psychoanalysis and cognitive-behaviourism were growing. The new working definition proposed by the WHO in the early 2000s endorsed the cognitive-behavioural approach articulated to utilitarianism, by firmly embedding it in the continuation of its concept of health as a state of well-being. This concept thus combines an epistemological dimension with a moral and social dimension.This research work aims to shed light on the contemporary uses of the concept of sexual health by tracing its genesis, which far precedes its institutionalization by the WHO. We thus go back to its first occurrences in the first half of the 19th century, when health refers mainly to the absence of disease.We adopt a method of historical epistemology, with work on the texts of the time in the Anglo-American medical field and in the field of medicine on the European continent. While it is common to question the concepts of the medical and health fields, particularly those related to sexuality, from a social perspective that highlights their normative and biopower dimensions, this work aims to study the concept of sexual health by conducting an internal analysis of the fields in which the concept has been forged and deployed. Without contesting the relevance of the socio-political approach, the methodological choice that acknowledges its internal rationality makes it possible to better emphasize what can be irreducibly normative in the concept of sexual health.The latter is therefore analysed according to the scientific model in which it emerges, namely vitalist hygienist physiology. This genesis will shape its history until today, while a sexology that makes it its object, and a mechanistic sexual medicine that also refers to it, are in opposition. The history of biological epistemologies sheds light not only on the genesis and uses of the concept of sexual health, but also on its relationship to the field of sexual pathology. The opposition of epistemological models results in different, if not adverse, therapeutic approaches to the conception and treatment of sexual problems. According to the WHO, sexual health is not reduced to the absence of sexual diseases, but is holistic and aims to cover the mental and social aspects as well, in a preventive and educational approach in the area of public health. This last point also underlines the ethical burden, from a eudemonic perspective, conferred on it by its hygienic uses since the 19th century.This work thus allows at the same time: 1) to place the concept of sexual health in a longer history than that which usually begins after the Second World War; 2) to enrich the historiography of sexology, for which the concept of sexual health has become major in the 21st century; 3) to question the history of sexuality as developed by Michel Foucault; 4) to shed light on the relationships between the normal and the pathological that are at stake in the concept of sexual health and in the controversies between psychodynamics and cognitive-behaviourism; 5) to examine the idea that contemporary uses of this concept would contribute to the medicalization of sexuality

Full textOf interest of everyone, health is not an easy concept to figure out. Although there is no universal definition of health, its various uses and meanings allow us to specify its biological, psychic and social dimensions. In animal health, the biological dimension remains the most studied, there is therefore no consensual definition of international animal health. So how to understand the health of marine shellfish? A living being can be functionally described in physiopathology by monitoring biomarkers under laboratory conditions. In men and many animals, "normal" biomarker values have been proposed; these have been determined by carrying out measurements or observations in samples of healthy populations, the rigorous selection of these populations and samples being a major issue. This approach has long been based on the idea that a living being is in "good health" when it is in the "norm", or otherwise expressed, that its biomarker values are part of the distribution of biomarker values obtained in a population in "good health". Different stages then led to changing the terminology of "normal" or usual values, sometimes called medical constants, to prefer those of reference values and reference intervals. The definitions of these terms and their determination have been the subject of numerous publications and international standards which constitute the scientific body of the theory of reference values. After choosing biomarkers of interest in biological health in the Pacific oyster Crassostrea gigas (first issue), to establish healthy oyster populations (second issue), is it possible in a first study to determine reference intervals of these biomarkers in a reference sample group (third issue), then to verify that they provide elements of hypotheses on the health, by carrying out complementary studies of comparison with the observed values of biomarkers in Pacific oysters experimentally infected by pathogens, OsHV-1 (ostreid herpesvirus type 1) on the one hand, Vibrio aestuarianus on the other (fourth issue)?

Between philosophy (of medicine) and psychoanalysis, our thesis project intends to outline and synergize the meanings of health as a term and an administrative field (part I), a philosophical concept inscribed in a fifty-year old debate (part II) and a notion having a latent expression in the heart of the psychoanalytic clinic (part III). Within this multidisciplinary framework, our doctoral project counters the widespread rejection of the concept of health in psychoanalysis. This project gravitates around an axis of theoretical-clinical and epistemological questioning : to what extent and how does the outline of a Canguilhemian definition of health recapture the hypothesis of the Freudian unconscious ?

Full textThe law n° 2002-303 of March 4th 2002, relative to the rights of the patients and to the quality of the health system marks a decisive and debated bend in the maïeutics of sanitary democracy in France. Sanitary democracy is not any more only considered under the angle of the priviledge of the citizen's participative rights in public debate and in decision-making in sanitary matters, but also under the broader angle of the defense of the rights of the "sick person". In that way, the law of March 4th 2002, based on the concept of sanitary democracy, intends to bring to the foreground both a movement of promotion of the rights of the patients (sanitary democracy in the broad sense) and a political project (sanitary democracy in the strict sense). The study bases itself on the dialectics of both conceptions of sanitary democracy such as suggested by the law of March 4th 2002, relative to the rights of the patients and to the quality of the health system. Having raised a semantic rigorous lack on behalf of the legislator of 2002, the study tries to seize the reach and the impact of the highlighted terms of "sanitary democracy", such as they are stipulated. Within the singular colloqium by the law of March 4th 2002, that is under the angle of four main ideas : trust, justice, transparency and responsibility. Then the study focuses again on the essence of the political concept of sanitary democracy, namely the participation of the citizens, that is the sample group of laymen and experts, in debating and decision-making in sanitary matters, especially through public debater, consensus civic conferences. Which allow the confrontation of both experts and laymen' s points of views. All things considered, is the user of the health system becoming a "new citizen" ?

Full textThe evaluation of quality of the patients' life is central in health, especially in Public Health research programs and policies. Many instruments are developed and discourses on health related quality of life grow in number and diversity. Several approaches and accounts of the concept coexist. However, this growth and diversity is synonymous with enduring doubts about the validity of the existing concepts of health related quality of life. In order to understand and maybe dissolve the confusion, we have first to agree on a common framework of analysis for the concept of quality of life. But, to our knowledge, this framework of analysis is not available. And yet this framework is necessary in order to explicit, compare and test the theoretical choices that determine the concept of quality of life, and most of all of the good quality of life, underlying the measuring instruments. It is indeed the conceptualization of value, of what is good in life, which is at stake. But this aspect is concealed: the approach of quality of life in health is generally but mistakenly conceived as value free or neutral. This concealment probably explains why a common framework of analysis of the existing concepts of health related quality of life is not available. From an understanding of the logic of value specific to quality of life, we try to develop such a framework of conceptual analysis

Apart from this twofold process, the main thing lacking to medecine, taken as a solution to health problems in a given community, will always be "efficiency". The present approch can be dealt with by provinding upgrades for rudimentary health care, synthesizing roles and duties, in order to give back to the communites the paternity of their health system
