Kenny, Paul D. The Puzzle of Populism. Oxford University Press, 2017. http://dx.doi.org/10.1093/oso/9780198807872.003.0001.

Full text
Abstract:
This chapter sets out the puzzle at the center of the book: what explains the success of populist campaigners in India, Asia, and beyond? It summarizes the existing literature on populist success both in Latin America and Western Europe and argues that these explanations do a poor job of explaining Indian and Asian cases in particular. Populists win elections when the institutionalized ties between non-populist parties and voters decay. However, because different kinds of party systems experience distinct stresses and strains, we need different models of populist success based on the prevailing party–voter linkage system in place in any given country. The chapter then sets out the rationale for concentrating on explaining populist success in patronage-based party systems, which are common not only to Asia, but also to Latin America and Eastern Europe.

Abstract:
This chapter focuses on the political and institutional factors behind the implementation of UN Security Council resolution 1325. It illuminates two elements of the Women, Peace, and Security (WPS) Agenda: participation and protection. It argues that despite the WPS Agenda’s efforts, women continue to remain underrepresented in peace negotiations and post-conflict political settlements. Further, by concentrating solely on protecting women from sexual violence, and neglecting an analysis of gender inequality and its contribution to conflict-propensity, the WPS Agenda perpetuates a protectionist narrative. This is due to opposition to the participation agenda from developing country member-states, a lack of accountability systems, and a lack of a powerful advocate within the UN bureaucratic system. The chapter concludes with suggestions for a recently formed working group under resolution 2242 to utilize, in order to better enable women’s participation in peace and security processes.

Abstract:
This chapter presents a concise overview of the linguistic means that express categories of information structure (IS) in Slavic languages. Concentrating especially on (a) the realization of broad vs narrow focus (focus projection); and (b) the expression of given information, topic, and delimitation, the chapter covers intonation and ‘free’ word order—the two most widely discussed ways of IS encoding. It further presents less well studied IS-sensitive devices and phenomena in Slavic: clefts, predicate doubling, topic and focus particles, as well as the interaction of IS with the Slavic clitic systems, in particular, in the expression of positive and negative polarity (or verum) focus.

Abstract:
The book serves as a collection of articles concerning different approaches to the notion of ‘place’ in terms of contrastive studies. Out of eight articles, the first four concentrate on the place of conceptualization in languages. Next two contributions discuss the notion of ‘place’ from a more literal point of view, concentrating on the role of case and prepositional systems in the expression of place as well as on the concept of place from the pragmatic point of view, as discursive representation of places in Polish and American cultures is analysed. The third part of the book contains two articles focusing on language contact. Each of the above-mentioned contributions provides an inspiring approach to contrastive studies and further research.

Abstract:
This chapter examines the relationship between hopeful thinking and major depressive disorder. Hope is a positive psychology construct that comprises goals, agency thinking, and pathways thinking and has been associated with psychological and physical well-being and psychosocial outcomes. Depression is inversely correlated with hope and is characterized by a host of symptoms and psychological correlates, including feelings of sadness, negative self-talk, amotivation, and difficulties in problem-solving and concentrating. This chapter explores the empirical evidence regarding the relationship between hope and depression, including the relationship between the subcomponents of hope (i.e., pathways and agency thinking) and the biological (e.g., neural reward systems) and cognitive (e.g., executive functioning) correlates of depression. In addition, the evidence for hope as a viable route for remediating depressive symptoms is reviewed, and future directions are proposed.

Chau, Anthony T. Magnesium Toxicity. Edited by Matthew D. McEvoy and Cory M. Furse. Oxford University Press, 2017. http://dx.doi.org/10.1093/med/9780190226459.003.0058.

Full text
Abstract:
Magnesium sulfate is currently the drug of choice for prophylaxis against eclampsia in women with preeclampsia and first-line treatment for eclampsia. In this population, medication administration error is a frequent cause of magnesium toxicity. Symptoms of toxicity are linked to increasing serum concentrations, but routine monitoring is not recommended. Instead, deep tendon reflexes and respiratory rate are the most commonly monitored parameters. However, magnesium serum concentration should be monitored when magnesium toxicity is suspected or in patients at high risk of toxicity. The gastrointestinal and central nervous systems are usually affected first. As the serum concentration becomes extremely high, neuromuscular and cardiovascular effects may occur, leading to respiratory failure and cardiac arrest. In addition to standard advanced life support measures, calcium is the mainstay of treatment and in some cases forced diuresis and dialysis may also be considered.

Full text
Abstract:
Successfully weaning patients from prolonged mechanical ventilation requires the varied expertise of a dedicated multidisciplinary care team. Traditionally, this care was provided in acute care hospitals, increasingly these patients are transferred to specialized weaning centres. These may improve patient outcomes by concentrating weaning expertise in a low-acuity environment and implementing protocols for liberation from mechanical ventilation. However, these centres might also worsen patient outcomes because they typically offer less intense nurse and physician staffing compared with traditional intensive care units. Generally, the clinical evidence is mixed, with the best studies suggesting that weaning centres offer similar outcomes as acute care hospitals, but at lower costs. Health systems also might stand to gain from dedicated weaning centres, because they can release intensive care unit beds for more acutely-ill patients. Many gaps remain in our understanding of which patients should be transferred to dedicated weaning centres, the optimal timing of transfer, and the best approach to care for patients in this highly specialized setting.

Full text
Abstract:
The optical interface between a light source and a detector is important. In most practical realizations the aim should be to collect the maximum light possible. Lens systems seldom do this efficiently, especially where the light source is diffuse. Underlying any attempt at concentrating or guiding light is subject to a fundamental limitation referred to as étendue (phase space cannot be squeezed). Light collection from small volume scintillators of high refractive index may approach 50 %, while collection from large-area scintillators is typically less than a few per cent. Incorporation of wavelength-shifting light guides and fibres leads to enhanced performance. Efficiency measurements by the author in terms of photoelectrons per keV are presented for selected configurations. Optical recycling derived from total internal reflection provides enhancement in effective quantum efficiency by a factor of up to 10. Concepts such as escape cones, adiabatic light guides, and trapped light are covered in detail.

Full text
Abstract:
Chapter 3 deals with the abiotic environment in high altitude lakes and streams worldwide. It searches for general patterns in relation to altitude, without overlooking the enormous variability, both temporally as well as at small and large spatial scales. Overall, environmental conditions in high altitude waters may be regarded as harsh. Indeed, certain features such as low temperature, low oxygen pressure, and high ultraviolet radiation (all a direct result of high altitude) as well as transparent and ion-poor waters with low nutrient concentrations are typical of high altitude aquatic systems. However, streams and lakes turbid from inflow of glacial meltwater loaded with glacial mineral flour and peatbogs with deep brown water from a high concentration of coloured dissolved organic matter are equally prominent. So, evidently, environmental conditions in aquatic systems are probably just as variable at high as at low altitudes.

Columb, Malachy O. Local anaesthetic agents. Edited by Michel M. R. F. Struys. Oxford University Press, 2017. http://dx.doi.org/10.1093/med/9780199642045.003.0017.

Full text
Abstract:
Local anaesthetic agents cause a pharmacologically induced reversible neuropathy characterized by axonal conduction blockade. They act by blocking the sodium ionophore and exhibit membrane stabilizing activity by inhibiting initiation and propagation of action potentials. They are weak bases consisting of three components: a lipophilic aromatic ring, a link, and a hydrophilic amine. The chemical link classifies them as esters or amides. Local anaesthetics diffuse through the axolemma as unionized free-base and block the ionophore in the quaternary ammonium ionized form. The speed of onset of block is therefore dependent on the pKa of the agent and the ambient tissue pH. Esters undergo hydrolysis by plasma esterases and amides are metabolized by hepatic microsomal mixed-function oxidases. Local anaesthetics are bound in the blood to α‎1-acid glycoproteins. Pharmacological potency is dependent on the lipid solubility of the drug as is the potential for systemic toxicity. The blood concentrations required to cause cardiovascular system (CVS) collapse and early central nervous system (CNS) toxicity are used to quantify the CVS:CNS toxicity ratio. Local anaesthetics also have the potential to induce direct neuronal damage. Intravenous lipid emulsion is used for the treatment of systemic toxicity but the scientific evidence is inconsistent. With regard to the pipecoloxylidine local anaesthetics, early evidence indicated that the S- was less toxic than the R-enantiomer. However, clinical research using minimum local analgesic concentration designs suggests that reduced systemic toxicity and motor block sparing is mainly explained by potency rather than enantiomerism.
