Abstract:
The chapter is concerned with recognition and the most recent guidance on the management of commonly seen pregnancy complications. These include early pregnancy bleeding, antepartum haemorrhage, hyperemesis, obstetric cholestasis, multiple pregnancy, breech presentation, intrauterine growth restriction, thromboembolic disorders, and the principles of thromboprophylaxis. Each section describes the condition and factors that may lead to its development. Signs and symptoms are described, along with contraindications and aspects of treatment. Special considerations in the antenatal management plans are included.

Abstract:
The hallmark of HIV infection is immune dysregulation and immunosuppression. As the immune system deteriorates, inflammatory dermatoses, metabolic dysregulation, adverse drug reactions, opportunistic infections, and cutaneous malignancies become more common, atypical in presentation, and recalcitrant to therapy. Both acute and chronic skin complaints contribute significantly to reduced quality of life for HIV patients. The Centers for Disease Control and Prevention recommends that individuals between ages 13 and 64 years be tested for HIV at least once in their lifetime, with increased screening of high-risk individuals and testing based on symptoms. The presence of dermatoses uncommon in the general population but concentrated in the HIV population, or dermatoses strikingly recalcitrant to therapy, should warrant suspicion and testing for HIV.

Abstract:
Before the era of antiretroviral therapy (ART), cytomegalovirus (CMV) retinitis was the most common ocular opportunistic infection in patients with AIDS, occurring in up to 30% of patients. The incidence of CMV retinitis, its natural history, and the indications for and response to therapy depend in part on whether at-risk patients are taking ART. Symptoms of CMV retinitis are nonspecific and include floaters, light flashes, peripheral visual field loss, and blurred central vision. Patients taking ART, especially those with immune recovery, are at lower risk of CMV retinitis than are patients who remain profoundly immunosuppressed; thus, patients taking ART do not need to be screened as often.

Abstract:
Renal disease in persons with HIV has been a major cause of morbidity and mortality since the onset of the epidemic. HIV nephropathy (HIVAN) was the most common form of kidney disease initially seen, but in the post-antiretroviral therapy (ART) era it is much less common. Other renal conditions associated with HIV infection include immune complex disease and classic focal segmental glomerulosclerosis. The pathologic spectrum of renal disease in patients with HIV is extensive. Some conditions, including HIVAN, improve following treatment of the virus with ART. Acute kidney injury is much more common in HIV-infected patients and is associated with a sixfold increase in mortality. Patients with HIV are also much more likely to require renal replacement therapy, including dialysis and renal transplantation. ART may also contribute to renal disease in patients with HIV.
