Multistable figures offer an intriguing model for arbitrating conflicting positions. Moving back and forth between the different aspects under which something can be seen, one recognizes that mutually contradictory descriptions can be equally valid and that disputes over the correct account can be resolved without dissolving differences or establishing a higher synthesis. Yet, the experience of a gestalt switch also offers a model for radical conversions and revolutions – that is, for irreversible leaps to incommensurable alternatives foiling ideals of rational choice while providing the possibility and necessity of decision. Accentuating the temporal dimensions of multistable figures, this multidisciplinary volume illuminates the critical potentials and limits of multistability as a complex figure of thought.

Transposition complexes refer to hearts in which there is a reversal in the relationship between the ventricles and great arteries, i.e. there is ventriculoarterial discordance. Thus, the right ventricle gives rise to the aorta and supports the systemic circulation, whilst the left ventricle becomes the subpulmonary ventricle. There are two types of transposition: complete transposition of the great arteries (TGA) and congenitally corrected TGA. This chapter discusses complete TGA, including interarterial repair (Mustard or Senning operation), arterial switch operation, and Rastelli operation. It also covers congenitally corrected transposition of the great arteries (ccTGA), including atrioventricular (AV) and ventriculoarterial (VA) discordance.

Abstract:
The lower urinary tract has two main functions; the collection and low pressure storage of urine and periodical controlled elimination of urine at an appropriate time. In order to achieve continence during bladder filling and storage and produce efficient and effective bladder emptying, there is accurate coordination between opening and closing of the urethral sphincters and contraction of the detrusor smooth muscle. The process of micturition has two phases: the storage/filling phase and the voiding phase. The analogy for the transition between these two phases has been described as an on-off circuit, rather akin to flicking a light switch, between synchronous bladder contraction and urethral outlet relaxation, and vice versa. These phases are regulated by a complex, integration of somatic and autonomic efferent and afferent mechanisms that coordinate the activity of the bladder and urethra. This chapter provides an overview of our current understanding of these complex mechanisms.

The annual volume of bariatric surgery is growing, giving rise to an increase in complications requiring complex management, including revision. Bariatric revision procedures are also becoming increasingly necessary for weight-loss recidivism and patients at the extreme of obesity. This chapter outlines clinical management pathways used to address secondary bariatric surgery. It summarizes reasons for, and outcomes with, revision of a laparoscopic gastric band, vertical banded gastroplasty, sleeve gastrectomy, or Roux-en-Y gastric bypass. Surgical techniques used to manage weight regain or failed weight loss after bariatric surgery are also discussed. Finally, surgical solutions for bariatric surgery-induced malnutrition are described, particularly in the setting of biliopancreatic diversion, duodenal switch, or jejunoileal bypass. Overall, the chapter concludes that standardization of revisional procedures can have a significant patient impact, and guidelines must be evidence-based in order to ensure patient safety and success.

The kidney is central to acid–base homeostasis. Major processes are reabsorption of filtered bicarbonate, de novo synthesis of bicarbonate from ammoniagenesis, and net excretion of protons. The latter requires buffers such as ammonium, phosphate, citrate and other bases binding protons (so-called titratable acids). The proximal tubule is the major site of bicarbonate reabsorption and only site of ammoniagenesis. The thick ascending limb and the distal convoluted tubule handle ammonia/ammonium and complete bicarbonate reabsorption. The collecting duct system excretes protons and ammonium, but may switch to net bicarbonate secretion. The kidney displays a great plasticity to adapt acid or bicarbonate excretion. Angiotensin II, aldosterone and endothelin are involved in regulating these processes, and they induce morphological changes along the nephron. Inborn and acquired disorders of renal acid–base handling are caused by mutations in acid–base transport proteins or by dysregulation of adaptive mechanisms.

A theology aims to explicate what God is like, and a metatheology investigates more fundamental issues concerning how to structure such a project and where it should begin. Approaches that ignore this more fundamental investigation risk presupposing stances that do not withstand scrutiny and perhaps would never have been endorsed if considered directly. In addition, approaches that ignore the issue of fundamentality often switch from one set of assumptions to another without noticing the change in perspective that results, giving rise to a chance of incoherence and to an approach that is theoretically disorderly and thus failing to as systematic and elegant as we would like. This work begins with the more basic question of where to begin thinking about God, where it is best to start the project of theology, in a way that offers some hope of a defensible metatheory, from which a complete theology, displaying the kind of theoretical elegance and structure we find in our best scientific and philosophical theories, can be developed.

This chapter reconstructs how Nalca, a Mek language of the Trans-New Guinea phylum, has acquired gender markers and describes the non-canonical properties of this highly unusual gender system. Gender in Nalca is mainly assigned by two different defaults, phonological assignment is holistic, there is a gender switch depending on the syntax of the noun phrase, controller and target are adjacent, and gender has the function of case marker hosts. Gender in Nalca is only weakly entrenched in the lexicon and predominantly phrasal. It is argued that canonical gender is an attractor (a complex, diachronically stable structure with heterogeneous origins). A model of the gender attractor based on the notion of information transfer chain is developed. The rise of Nalca gender is an instance of system emergence where several diachronic processes, such as grammaticalization, reanalysis, and analogy, interact. Chains of rapid diachronic change are triggered by anomalies that entail other anomalies.

This article considers molecular electronics based on self-assembled monolayers. It begins with a brief overview of the nanofabrication of molecular devices, followed by a discussion of the electronic properties of several basic devices, from simple molecules such as molecular tunnel junctions and molecular semiconducting wires, to more complex ones such as molecular rectifying diodes. It also describes molecular switches and memories, focusing on three approaches called ‘conformational memory’, ‘charge-based memory’ and ‘RTD-based memory’ (RTD is resonant tunnelling diode). It shows that memory can be implemented from resonant tunnelling diodes following cell architecture already used for semiconductor devices. The article concludes with a review of molecular transistors.

Inversions are intrachromosomal structural rearrangements. The most common is the simple (or single) inversion. If the inversion coexists with another rearrangement in the same chromosome, it is a complex inversion. In an inversion, a segment of chromosome is switched 180 degrees. If this segment includes the centromere, this is a pericentric inversion; if not, it is a paracentric inversion. In principle, and almost always in practice, it is only the pericentric inversion that conveys an important genetic risk to carriers of the inversion: Their children may inherit a “recombinant” chromosome that would inevitably be imbalanced. This chapter considers these two type of inversions and discusses the degree of genetic risk that may—or may not—be associated.
