Abstract:
Background: Many surgeries can be performed with spinal anesthesia (SA) or general anesthesia (GA). However, there are only limited and confounding data available regarding costs and anesthesia-related times. Hip or knee replacement are common orthopedic surgeries that can be performed using SA or GA without differences regarding mortality or morbidity. Observational studies have suggested that spinal anesthesia may be associated with lower risks of death, delirium and major medical complications and with shorter lengths of stay in the hospital than general anesthesia. The aim of this study was to assess and compare the effects of spinal versus general anesthesia on postoperative outcomes in patients undergoing orthopedic surgery. Material & Methods: This was a comparative observational study and was conducted in the Department of Anesthesiology of Holy Family red crescent Medical College Hospital, Dhaka, Bangladesh during the period from March,2021 to March,2023. In this study we included 200 patients undergoing orthopedic surgery. The patients were randomly divided into two groups – Group A (Patients who were given general anesthesia) & Group B (Patients who were given spinal anesthesia). Results: In total 200 patients from both the groups completed the study. In our study we found majority (44.5%) of our patients were aged 60-69 years and most of our patients were female (56%) compared to male (44%).The mean age of our patients was 61.73 ± 7.92 years. The mean BMI was 31.67±3.24 kg/m.2Among all patients ,48% had mild systemic disease and followed by 31.5% had severe systemic disease. Majority (43.5%) of our patients had hypertension, 31% had diabetes. Vomiting was found 47% in group A on contrary only 23% had vomiting in spinal group. We found the mean anesthesia induction time was significantly higher in spinal group. Anesthesia time was lower in spinal group while PACU time was higher in group B. Time duration of surgery was significantly lower in spinal group. After 24 hours, spinal group showed less pain score than general anesthesia group. Conclusion: In our study, we found that SA is associated with less fixed and variable costs and lower postoperative pain scores during the stay in the PACU. Therefore, SA is a more reasonable alternative to GA in the immediate postoperative period for patients undergoing hip or knee replacement. When compared to general anaesthesia, spinal anaesthesia provides better operating circumstances, better postoperative pain control, and faster postoperative recovery.

Background and Objectives: Induction of anesthesia is a critical part of anesthesia practice. Sudden hypotension, arrhythmias, and cardiovascular collapse are threatening complications following injection of induction agent in hemodynamically unstable patients. Present prospective randomized study is designed to compare propofol and etomidate for their effect on hemodynamics in patients undergoing general anesthesia. Methods: Hundred ASA I and II patients of age group 18-60 years scheduled for elective surgical procedure under general anesthesia were randomly divided into two groups of 50 each receiving propofol (2 mg/kg) and etomidate (0.3 mg/kg) as an induction agent. Vital parameters at induction, laryngoscopy and thereafter recorded for comparison. Results: Demographic variables were comparable in both the groups. Patients in etomidate group showed little change in mean arterial pressure (MAP) and heart rate (HR) compared to propofol (p > 0.05) from baseline value. Conclusions: This study concludes that etomidate is a better agent for induction than propofol in view of hemodynamic stability.

Abstract In this article, I describe the internal structure, surface distribution, and scope possibilities for comparative phrases in Syrian Arabic. Comparative phrases may show surface displacement from their scalar associate in this language, subject to certain restrictions. The restrictions on surface displacement match those on scope construal, suggesting that scope construal involves covert displacement. Further, the restrictions on displacement are shown to be at work in ‘Comparative Deletion’, an ellipsis process operative in clausal comparatives. Comparative Deletion is shown to suspend barriers to movement involved in the semantic derivation of clausal comparative constructions, as reported previously for English. However, I show that attempts in the literature to reduce the barrier-suspending effect of Comparative Deletion to ellipsis in general do not extend to Syrian Arabic. Rather, the Arabic facts suggest that the suspension of constraints on movement under Comparative Deletion is unique to comparative constructions.

It is possible to derive a general formula for constant‐velocity, two‐dimensional dip moveout (DMO). This serves to unify the many published forms of DMO. Known results for common‐offset and shot profile DMO are special cases of the general formula. The analysis is based on the dip‐corrected NMO equation, and thus is a kinematic DMO theory. Using a logarithmic stretch of the time axis, efficient fast Fourier transform (FFT) common‐offset DMO algorithms can be derived. In the published versions, log variables are introduced into the NMO equation. It can be shown that the resulting impulse response departs from the DMO ellipse, which means that some dips have been improperly handled. A new log‐stretch formulation (exact log DMO) which preserves the DMO ellipse can be derived by starting with the analytic impulse response, rather than the NMO equation. Tests on field data indicate that exact log DMO handles shallow, steeply dipping events in agreement with Hale’s DMO, while the other log‐stretch algorithms degrade these events. In situations where constant‐velocity DMO is viable, exact log DMO can be used to create a Hale‐quality image at FFT speed.

Comparative literature is unlike any other discipline. elsewhere—for example, in politics or religion—the comparative operates as a subdiscipline within a larger general discipline. The problem for comparative literature is that there is no general discipline of literature: institutionally, the discipline consists of nothing but the fragments of different languages. As a result, through a curious metonymic inversion, comparative literature has come to figure as the totalizing general discipline of which it should form a part. This is why it also seems to offer a natural home for the idea of Weltliteratur. Comparative literature promises the Utopian recreation of the lost amphora of literature as it stood before its fall into the clutches of the nation.

The article showing that the highest goal of each autonomous general legal discourse is the construction and implementation of its vision and mission as a special picture of the world – a conceptualized and structured model of an aspect or fragment of reality accessible through its research program and methods. Depending on different types of scientific rationality, this picture of the world can be considered as a subject, object or goal of a scientific discipline, but it continues to be the main question of the latter, even if it is not formulated as such. The claim to the right to construct and present one’s own picture of the world as common to all legal discourses has actually been and is a key point of competition and dominance between legal philosophy, legal theory and comparative jurisprudence (comparative law) during the last two centuries. The author argues that only comparative jurisprudence’s awareness of its own fundamental experience and continuity, available opportunities and valuable advantages in creating a picture of the world can bring it from a semi-marginal state to a well-deserved central status among general legal disciplines in the scientific and educational agenda. In addition, a realistic and inductive picture of the world of comparative jurisprudence is better for considering and modeling the existing and potential serious challenges of legal development. <p class="abstract"><strong>Background:</strong> The main aims of pre-anaesthetic medication are anxiolysis, analgesia, anti-emesis and reducing perioperative patient risk. Producing a state of amnesia for pre and post-operative events is desired by all. This study has been undertaken to evaluate the role of three of the benzodiazepines i.e. diazepam, lorazepam and midazolam during general anaesthesia, in providing anxiolysis, sedation and amnesia.</p><p class="abstract"><strong>Methods:</strong> The study included patients with ASA grade I and ASA grade II physical status of both sexes, age ranging between 18-60 years. Patients were divided into three groups of thirty patients each, every group receiving intramuscular injections of diazepam 0.1 mg/kg body weight, lorazepam 0.07 mg/kg body weight and midazolam 0.08 mg/kg body weight respectively; 45 minutes prior to induction of general anaesthesia. Anxiety assessment before premedication along with assessment of sedation after premedication was done.</p><p class="abstract"><strong>Results:</strong> Before premedication the mean values of pulse rate, blood pressure and respiratory rate were not significantly different among the three groups (p&gt;0.05). Maximum changes in these parameters were observed with Midazolam followed by lorazepam and diazepam. The dose of thiopentone used as inducing agent was also lowered significantly in case of midazolam (p&lt;0.05). One patient in midazolam group showed respiratory depression whereas four patients receiving lorazepam and diazepam showed delayed recovery and prolonged sedation, but the effects were self-limiting.</p><p class="abstract"><strong>Conclusions: </strong>Midazolam offers the maximum advantage in allaying anxiety and providing excellent sedation and amnesia during general anaesthesia and proves to be the most suitable premedicant before general anaesthesia.</p>

AbstractA comparative study of religion rests on comparison and generalization. Both require that the object of study, the subject matters of various religions have something in common - certain properties that warrant their juxtaposition in analyses. If they had nothing in common, "the study" of religion would not make any sense. But it seemingly does, thus "religions" presumably form a subject matter with certain regularities. Such regularities may be "emic universals" on the level of socio-cultural formations and they may be "etic universals" on the levels of the analysts' stock of general terms - and the two levels are connected. This article focuses mainly on the role of universals as general concepts in method and theory, i.e., on the status and use of etic level generalizations such as models, maps, metaphors that are constructed in order to explain and make sense, as general terms, of emic level entities, properties, functions, structures etc. The last part concerns the use of universals in four modes of comparison of material, cognitive and symbolic matters.
