Abstract:
The purpose of this study is to test the utility of Talcott Parsons' AGIL Model, i.e., Adaptation, Goal Attainment, Integration, and Latency (Pattern Maintenance) in evaluating the program effectiveness of a community mental health center (CMHC). The model provided a conceptual framework for the selection of appropriate variables. The dependent variable in this study is the overall evaluation of the CMHC as measured through the perception of community leaders. Fourteen hypotheses were constructed to identify and test the relationship among the AGIL criteria and the use of a selected set of independent variables. Data for this study were collected from primary and secondary sources. Secondary data were obtained from the Texas Department of Mental Health and Mental Retardation in Austin and the CMHC center in Eton. Primary data were collected through personal interviews of general community leaders and influential persons in health-related activities in the community. The selected independent variables included the scope of leadership, the attitude towards this community, socio-economic status, knowledge of the CMHC, and the commitment and involvement in the CMHC. Data indicated that Parsons' criteria for evaluating the CMHC's program were comprehensive and related to each other both positively and negatively. Among the selected independent variables, the type of leader was found to be the best predictor of program evaluation of this CMHC. Overall, generalized community leaders were more defensive and favorable to the CMHC's program compared with the specialinterest leaders. The leaders also differed in their emphasis of the AGIL criteria. The generalized community leaders were conservative in emphasizeing the evaluative criteria of adaptation, integration, and pattern maintenance; the special-interest leaders gave more emphasis to the goal attainment function of CMHC. It was concluded that Parsons' AGIL model was useful for evaluating a CMHC. The variant direction of relationship among AGIL criteria indicated differences in the perception of community leaders in the evaluation of the CMHC.

Abstract:
This research explores good practice with people on CTOs - via a case study of one area mental health service in Victoria. The emancipatory values of Social Work were used to guide the investigation, thereby ensuring the involvement of consumers and their families or carers. Critical Social Work theory provided an important theoretical base for the research, and both critical theory and pragmatism supported the methodology. A mixed methods approach was undertaken. This included a cluster analysis of 164 people on CTOs. Three clusters emerged from the exploratory cluster analysis. These clusters, labelled ‘connected’, ‘young males’ and ‘chaotic’ are discussed in relation to their particular characteristics. The results from the cluster analysis were used to inform the recruitment of four people on CTOs who were the central focus of case studies that represented the different clusters. Semi-structured group interviews were also undertaken to enhance the triangulation of data collection and analysis. This resulted in 29 semi-structured interviews with multiple informants, including consumers, family/carers, case managers, doctors, Mental Health Review Board members and senior managers. The data analysis was guided by a general inductive approach that was supported by the use of NVivo 7.
Five principles, and the processes required to enable them, emerged from the qualitative data: 1) use and develop direct practice skills, 2) take a human rights perspective, 3) focus on goals and desired outcomes, 4) aim for quality of service delivery, and, 5) enhance and enable the role of key stakeholders. These principles are discussed and then applied to the case studies in order to consider their potential relevance to practice within a diverse community of CTO recipients. The application of the principles identified two further findings: 1) that the principles are interdependent, and 2) the relevance of the principles varies depending on the characteristics of the consumer. The two most important findings to emerge from this thesis are that: 1) people on CTOs, their family/carers, and service providers are a diverse community of people who have a range of problems, needs and preferences in relation to either being on a CTO or supporting someone on a CTO; and 2) the implementation of CTOs is influenced by social and structural issues that need to be considered in developing any recognition or understanding about what represents good practice. Recommendations relating to each of the principles are made, along with identification of future research questions. A particular focus is whether application of the principles will enable improvements in practice on a range of measures, including reducing the use of CTOs, and the experience of coercion by consumers.

ABSTRACT This dissertation has the following specific aim: to measure outcomes of social work emergency certificates produced by a Mobile Crisis Team to determine the effectiveness of these certificates at linking clients to services. Linkage to services is a programmatic goal and is achieved by ensuring clients receive adequate crisis and follow up services as a result of Emergency Certificates. In doing so, risks of homicide, suicide and grave disability are mitigated. Under its administrative umbrella, Mobile Crisis Team operates a police-social work collaboration known as Crisis Intervention Team, and both are administered by the State of Connecticut Department of Mental Health and Addiction Services in southeastern Connecticut. The data comes from 233 consecutive cases that involved Emergency Certificate's to transport clients to the local emergency department for psychiatric evaluation and treatment. Data was gathered post-hoc by reviewing copies of each specific certificate and cross-referencing those certificates with an agency risk management report and an electronic data base that stores demographic data on all clients. Descriptive, bivariate and multivariable analysis, such as cross tabulations and binary logistic regression, were used to analyze the data in this study. Additionally, the chi-square automatic interaction detector (CHAID) was used to construct outcome trees to describe subgroups of interest. This research is a continuation of previously published research on the various permutations and outcomes of mobile crisis programs, and contributes two unique programmatic features: the use by social workers of a legally proscribed coercive tool to send people to the emergency department for assessment, and the effectiveness of police-social work collaborations compared to social work only referrals. Overall, linkage occurs in over 80% of cases, with significant predictors of linkage identified as substance abuse, Hispanic ethnicity, criminal justice involvement, suicide, affective disorders, and insurance status. Implications for program evaluation, future research and limitations of the study are also discussed

Introdução: Com o processo de desinstitucionalização psiquiátrica e a consequente reestruturação dos serviços de assistência em saúde mental, tem sido cada vez mais enfatizada a necessidade de avaliação dos equipamentos que compõem a Rede de Atenção Psicossocial. Diante da complexidade que envolve os sistemas de saúde e saúde mental, a avaliação é considerada como essencial para a garantia de qualidade dos serviços. A Organização Mundial de Saúde (OMS) destaca a necessidade de incorporar a avaliação dos resultados do tratamento como prática contínua e permanente, utilizando os dados da avaliação para melhorar a qualidade da assistência prestada. Objetivo: Este estudo tem por objetivo avaliar quantitativamente a estrutura, o processo de trabalho, a organização da atenção em saúde mental e o resultado da atenção em saúde mental oferecida pelo Centro de Atenção Psicossocial II em álcool e outras drogas (CAPSad) do município de Botucatu. Método: O estudo foi realizado em um CAPSad do município de Botucatu. Para a coleta dos dados utilizou-se de quatro instrumentos (coordenador, trabalhador, usuário e familiar) compostos pela Escala de Satisfação SATIS-Br, Escala de Mudança Percebida (EMP) e Escala de Atitudes Frente ao Álcool, Alcoolismo e Alcoolista (EAFAAA) e questões sobre os dados sociodemográficos, estrutura do serviço, recursos humanos e materiais e uso de álcool e outras drogas. Os dados foram processados de forma descritiva pelo Programa Statistical Package for Social Sciences (SPSS), versão 20,0 para Windows®. Resultados: A amostra foi de 01 coordenador, 19 trabalhadores, 25 usuários e 15 familiares. Foram obtidos níveis elevados de satisfação para usuário e familiares, e níveis intermediários de satisfação para trabalhadores com relação aos aspectos avaliados no serviço. Em relação à percepção de mudança teve-se percentuais elevados tanto para usuários quanto para familiares. Na questão de melhora global, 100% dos familiares perceberam a melhora em geral na vida de seu familiar, desde o início do tratamento, sendo que para os usuários 88% perceberam a ocorrência de melhora em seu tratamento. Evidenciou-se atitudes negativas desses trabalhadores frente ao alcoolismo, ao alcoolista e à bebida alcoólica, pois a maioria das médias dos fatores permaneceu na zona intermediária. Os resultados apontam a necessidade de investimentos em capacitações e cursos de atualização para os profissionais e melhora de pontos específicos em relação a infraestrutura do serviço. Como este estudo foi realizado em um único serviço de saúde mental, não pode ser generalizado para outros serviços.
Introduction: With the psychiatric deinstitutionalization and consequent restructuring of the mental health services, it is increasingly emphasized the need for evaluating services from the Network for Psychosocial Care. Given the complexity involved in health systems and mental health, evaluation is considered essential for quality assurance of the services. The World Health Organization (WHO ) emphasizes the need to incorporate the assessment of treatment outcomes as a continuous and ongoing practice, using assessment data to improve quality of care. Objective: This study aims to evaluate quantitatively the structure, work process, the organization of mental health care and outcomes of mental health care provided by the Psychosocial Care Center II for alcohol and other drugs (CAPSad) from the city of Botucatu . Method: The study was conducted in a CAPSad in Botucatu . To collect the data four instruments (coordinator , worker, family and user) were administered to the participants. Those instruments were composed by \" Satisfaction Scale - SATIS - BR \" Perceived Change Scale (EMP) and the Scale of Attitudes Towards Alcohol , Alcoholism and alcoholic (EAFAAA). The instruments were also composed by sociodemographic data, service structure, human and material resources and the use of alcohol and other drugs. Descriptive analysis of the data was performed using the software Statistical Package for Social Sciences (SPSS) version 20.0 for Windows® . Results: The sample consisted of 01 coordinator, 19 workers, 25 users and 15 relatives. We obtained high levels of users and family satisfaction, and intermediate levels of satisfaction for employees with respect to the evaluated service. Regarding the perceived change, the data showed high percentages of change for both users and for family. On a global question about improvements, 100% of the families noticed an improvement in the overall life of their relatives, when comparing to the beginning of treatment, and 88% of the users noticed the occurrence of improvement in their treatment. The data showed a tendency toward negative attitudes of these workers towards alcohol, alcoholism and alcoholics because for most means of factors remained in the intermediate zone. The results highlight the need for investment in training and extension courses for professionals and the need to improve specific points regarding the service infrastructure. As this study was conducted in a single mental health service, this results cannot be generalized to other services.

Este estudo trata da avaliação de um Centro de Atenção Psicossocial (CAPS), um serviço supostamente funcionando nos moldes apregoados pela Reforma Psiquiátrica Brasileira, a qual, atualmente, na sua vertente assistencial, é marcada pela implantação de serviços substitutivos ao hospital psiquiátrico. A proposta da avaliação centra-se no microespaço e no cotidiano do serviço e ocorre mediante a participação da equipe, usuários e familiares. Trata-se de uma avaliação qualitativa, mediante a qual se busca apreender a dinâmica do serviço, a forma como os atores interagem e os sentidos que constroem em relação à própria prática; uma avaliação que também possa ser dispositivo, permitindo, mediante um processo participativo, que grupos de interesse ampliem a possibilidade de intervir na realidade do serviço, e que possam ser sujeitos, uma vez que em metodologias tradicionais estão excluídos. A Avaliação de Quarta Geração, desenvolvida por Egon G. Guba e Yvona S Lincoln juntamente com o Método Paidéia, desenvolvido por Gastão Wagner de Sousa Campos, foram norteadores do processo teórico-metodológico da pesquisa. Os instrumentos de coleta de dados foram entrevistas com equipe, usuários e familiares e observação. As questões que emergiram no processo avaliativo foram agrupadas em três núcleos temáticos: dimensões do objeto de trabalho, equipe, práticas e os meios de trabalho. Foi constatado que, mesmo que o serviço pareça ser um substitutivo para o atendimento no hospital psiquiátrico, tanto fatores externos, relacionados às políticas locais de saúde mental, quanto fatores internos, relacionados ao cotidiano do serviço, impedem que isso se efetive.
This study is about the evaluation of a Center of Psychosocial Care (CAPS), a service supposed to work according to the patterns divulged by the Brazilian Psychiatric Reform which, currently, in its assistance trend, is marked by the implantation of services intended to replace the psychiatric hospital. The evaluation proposal is centered in the micro space and in the day-to-day of the service and is performed by means of the participation of the team, the users and family members. It is about a qualitative evaluation, by means of which one searches to grasp the dynamics of the service, the way how the actors interact and the directions that they construct in relation to the practice itself; an evaluation that can also device, allowing, by means of a participation process, that interest groups broaden the possibility of intervening in the reality of the service and that they may become subjects, considering that in traditional methodologies, they are excluded. The Evaluation of Fourth Generation, developed by Egon G. Guba and Yvona S. Lincoln, together with the Paidéia Method, developed by Gastão Wagner de Sousa Campos, guided the theoretical-methodological process of the research. The instruments for the collection of data were interviews with the team, users and family members as well as observation. The issues that emerged over the evaluation process were grouped in three central themes: dimensions of the work object, the team, practices and the working means. It has been noticed that, even though the service seems to be a replacement for the attendance in the psychiatric hospital, both external factors, related to the local policies of mental health, and internal factors, related to the day-to-day of the service, prevents it from becoming effective.

Abstract:
A partir do redirecionamento do modelo assistencial no Brasil emerge a necessidade de discussão da reinvenção de práticas alinhadas com o cenário de transformações. As práticas agora devem se desenvolver no sentido de superar o isolamento das grades, dos muros e das chaves, do apagamento do sujeito, da supressão de sintomas e das personalidades dos sujeitos em sofrimento psíquico. Diante disso, é que este estudo tem como objetivos avaliar boas práticas em saúde mental na atenção psicossocial no contexto de um município da região Sul do Brasil. Para tanto, organizou-se como uma pesquisa avaliativa construtivista, com enfoque responsivo, a medida que desenvolve um consenso critico entre os grupos de interesse. Está baseada na Avaliação de Quarta Geração, proposta por Guba e Lincoln (2011), que emerge como opção metodológica e se articula com o referencial teórico das \"Boas Práticas em Saúde Mental Comunitária\", proposto por Thornicroft e Tansella (2010). Este é apresentado de modo a identificar boas práticas na saúde mental, com base na articulação de 3 eixos indispensáveis e indissociáveis: a ética, como princípio fundamental que deverá orientar o planejamento, a assistência e a avaliação dos serviços. A evidência deve embasar as intervenções e serviços. E a experiência como uma evidência do que vem sendo produzido nas experiências locais e regionais de organização da rede de serviços. Como resultado do processo as práticas de Acolhimento, Projeto Terapêutico Singular e Transferência de Cuidados emergiram para discussão. O Acolhimento torna-se concreto nas ações da Porta Aberta, e significa escuta qualificada no momento em que a necessidade emerge, além de representar a oferta de atenção de qualidade baseada no processo relacional que fortalece o estabelecimento de vínculos e a construção de projetos de vida. O Projeto Terapêutico Singular apresenta-se como uma boa prática, a medida que é capaz de dar resposta singularizada e individualizada às necessidades do sujeito, de modo a oportunizar como resultado uma pessoa capaz de andar na vida. É definido de forma compartilhada entre equipe, usuário e família, segundo os objetivos terapêuticos para cada sujeito, e utiliza o técnico de referência, as mini equipes, as oficinas terapêuticas, os grupos terapêuticos, as assembleias, enfim, ofertas e propostas que permitem ao sujeito retomar o envolvimento com os espaços da cidade, no caminho da produção de vida. Com relação à Transferência de Cuidados, esta possibilita ao sujeito que vive o processo não só circular em uma rede de serviços, mas, acima de tudo, buscar, com suporte dela, sua emancipação. Para tanto, investe em práticas que estimulem a autonomia dos sujeitos, por meio de instrumentos como o Acompanhamento Terapêutico, Grupo de Apoio à Alta, espaços de decisões e discussões, estímulo à busca e resolução de problemas, atividades de reconhecimento dos espaços da cidade, transporte, cultura e lazer. Além disso, investe nas ações com a Atenção Básica, a partir do Apoio Matricial com processos de formação continuada com as equipes, e mapeamento da situação da saúde mental nos territórios. Ao final deste estudo, é possível concluir que o modelo de atenção psicossocial demonstrou potencialidade de produzir boas práticas na atenção à saúde mental, e que estas tem transformado a vida dos sujeitos em sofrimento psíquico, bem como tem auxiliado no processo de reconquista de espaços sociais
The redirection of the care model in Brazil raises the need to discuss the reinvention of practices in tune with the scenery of changes. The practices must now be developed in order to overcome the isolation of bars, walls and keys, the deletion of the subject, the suppression of symptoms and the personalities of individuals in psychological distress. Given this, the present study aims to evaluate best practices in mental health in psychosocial care in the context of a city in Southern Brazil. To do so, it is organized as a constructivist evaluation research, with a responsive approach, as it develops a critical consensus among stakeholdres. It is based on the Fourth Generation Evaluation, proposed by Guba and Lincoln (2011), which emerges as a methodological option and articulates with the theoretical framework of \"Best Practice in Community Mental Health\" proposed by Thornicroft and Tansella (2010). This one is presented in order to identify best practices in mental health based on the articulation of three indispensable and inseparable pillars: ethics, as a fundamental principle that should guide the planning, assistance and assessment of services; evidence, which must base the interventions and services; and experience, as an evidence of what is being produced in the local and regional experience of organizing network services. As a result of the process, Welcoming practices, Singular Therapeutic Project and Transfer of Care emerged for discussion. The Welcoming becomes concrete in the actions of the Porta Aberta project, and it means qualified hearing at the moment the need arises, besides representing the offer of quality care based on the relationship process that strengthens the bonds and the construction of life projects. The Singular Therapeutic Project is presented as best practice, as it is able to give individualized responses to the needs of the subject in order to create, as a result, a person who is able to walk through life. It is set in a shared manner among staff, user and family, according to the therapeutic goals for each subject, and it uses the reference technician (ou professional), the mini teams, therapeutic workshops, therapeutic groups, assemblies, in other words, any offers and proposals which allow the subject to resume the engagement with the spaces of the city, on the way to production of life. Regarding the Transfer of Care, it enables the person who lives the process not only to have access to a network of services but, above all, to seek their emancipation through its support. To do so, it invests in practices that encourage the subjects\' autonomy, using instruments such as the Therapeutic Monitoring, Patient Discharge Support Group, spaces for decisions and discussions, motivation for searching and solving problems, recognition activities of city areas , transport, culture and leisure. In addition, it invests in actions with the Basic Health Care, based on the matrix support with processes of continuing education with the teams and mapping of the mental health situation in the territories. At the end of this study, the conclusion was that the psychosocial care model showed capability of producing best practices in mental health, and these have changed the lives of individuals in psychological distress, and have helped in the process of conquering social spaces again

Comprehending the ongoing (re)building process of the substitutive psychosocial model to the nursing home care model for people suffering from mental disorders, it is worthwhile to know the healthcare dynamic of a substitutive service, in order to identify any issues within the team, which could interfere with the quality of care they provide. The aim of this study was to identify the working process of professionals at a Type III Psychosocial Care Center (PCC). This is an interventional study using a qualitative approach, that is based on the problem-based method, known as Arco de Maguerez. It consists of five stages: reality observation, key points, theorizing, problem-solving hypothesis, and application to reality. The development of the study was based on lightweight technology sources mediated by group technique. In the first stage, strong and weak points of the service were identified, followed by a discussion of issues related to the patient, family, teamwork, territory, and management. From this, the strengths and weaknesses of the staff and service were discussed. Following this, the participants emphasized the importance for discussion on the difficulty of integration and coordination of the Psychosocial Care Network. In the next step, problems regarding key points in question were expressed and this guided the active search in the literature for theorizing, which would guide in developing possible solution options. Therefore, permanent health education to perform the matricial and approaches to disclose the PCC territory were debated, and subsequently strategies were developed and applied to reality. Through the research process, the strengths of the team that would help them to overcome their shortcomings were identified. While some restraining forces that precluded this movement were also identified. In this context, the awareness-making process and the participants’ willingness to embrace change were evident throughout the study. This highlights the acceptance of the proposal of the interventional research that is based on the problematization of the service. It is evident that the spaces validity for the exchange of practices and knowledge, reflection-action-reflection, are imperative to the production of knowledge, which supports professionals for the qualification of care in mental health.
Entendendo o processo contínuo de (re)construção do modelo psicossocial substitutivo ao modelo de assistência asilar a pessoas com sofrimento ou transtornos mentais, considera-se fundamental conhecer a dinâmica assistencial de um serviço substitutivo, com vistas a delinear junto à equipe situações problemas que interferem na qualidade do cuidado que oferecem. A pesquisa teve o objetivo de conhecer os processos de trabalho dos profissionais de um Centro de Atenção Psicossocial tipo III. Estudo do tipo de intervenção, de abordagem qualitativa, direcionada pela metodologia problematizadora do Arco de Maguerez, que compreende cinco etapas sucessivas: Observação da Realidade, Pontos-chave, Teorização, Hipóteses de Soluções e Aplicação à Realidade. Seu desenvolvimento foi norteado utilizando-se essencialmente recursos das tecnologias leves, mediadas por técnicas grupais. Na etapa inicial, foram elencados pontos frágeis e fortes do serviço, seguidos da discussão de questões relativas ao usuário, família, equipe, território e gestão. A partir desse momento discutiram-se os entraves e pontos propulsores da equipe e do serviço. Na sequência, os participantes elegeram como ponto prioritário de discussão a dificuldade de integração e articulação da Rede de Atenção Psicossocial. A etapa seguinte privilegiou-se a problematização dos pontoschaves em questão, que orientaram a busca ativa na literatura para a teorização que orientaria as possíveis hipóteses de soluções. Assim, a educação permanente em saúde para realizar o matriciamento e formas de divulgação do CAPS no território foi debatida e, posteriormente, estratégias de ações foram aplicadas à realidade. No caminhar do processo de pesquisa, constataram-se forças propulsoras da equipe para o enfrentamento de suas dificuldades e também algumas forças restritivas que impediam este movimento. Neste contexto, os processos de tomada de consciência e de mudança dos participantes ficaram evidentes ao longo do estudo, salientandose a aceitação da proposta da pesquisa intervencionista, fundamentada na problematização do cotidiano do serviço. É perceptível a validez dos espaços de trocas de práticas e saberes, reflexão-ação-reflexão, para a produção de conhecimento que subsidiem os profissionais para a qualificação do cuidado em Saúde Mental.

This thesis presents a case-based qualitative evaluation of a community-based memory service for people with dementia and carers, informed by an advisory group including people with dementia and carers. Drawing on theoretical frameworks of critical and narrative gerontology, interviews with service users, carers and community mental health team staff generated rich accounts of experiences of using and delivering the memory service. Data from people with dementia and carers were analysed thematically and built on work by Willis et al. (2009) to generate quality indicators for dementia care. Analysis of data from CMHT staff drew on a framework of collective leadership devised for use in the NHS by the Center for Creative Leadership and The King's Fund (2014a). Findings reveal rich insights into: i) the everyday challenges faced by people with dementia and carers, ii) the positive contributions they can bring to the evaluation and development of services for people with dementia and carers and iii) the important role of collective leadership in the provision of high quality services for people with dementia. The thesis extends the current knowledge base relating to people with dementia and carers by developing particular understandings of how they can contribute to the evaluation and development of memory services.

Apesar das substâncias psicoativas serem consumidas há milhares de anos, nas últimas décadas a questão de seu consumo abusivo e dependência tem se configurado como uma demanda de saúde pública, diante de um cenário marcado pela acelerada transição socioeconômica que está ocorrendo em vários países. Este estudo tem como objetivo a avaliação das práticas oferecidas nos Centros de Atenção Psicossocial para Usuários de Álcool e Outras Drogas (CAPSad) do ponto de vista dos familiares dos usuários. Métodos: pesquisa avaliativa transversal, com análise quantitativa de dados coletados em CAPSad do município de São Paulo, utilizando a Escala de Satisfação (Satis-BR) e a Escala de Mudança Percebida (EMP), já validadas e utilizadas anteriormente em estudos avaliativos. Além das escalas, dados acerca da estrutura do serviço, dados sociodemográficos, de acesso, e de caracterização da participação do familiar no tratamento do usuário, foram analisados, com condução de modelo de regressão linear múltipla, no qual evidenciou-se a mudança percebida como variável preditiva de melhor nível de satisfação com os serviços, bem como a correlação negativa das seguintes variáveis com índices de mudança percebida: residir nas zonas leste e norte da cidade e ter renda familiar mensal entre 2 e 4 salários mínimos. O fato de ter parentesco não nuclear, como ser tio ou sobrinho, com o usuário, já teve associação positiva com a mudança percebida. São necessários mais estudos com este tipo de serviço para melhor explorar essas dimensões relacionadas à qualidade da atenção nesta temática, contribuindo para avaliar o impacto das novas políticas e da Rede de Atenção Psicossocial.
Despite psychoactive substances have been consumed for thousands of years, in recent decades, the issue of their abuse and dependence has been configured as a public health demand, against a backdrop marked by rapid socioeconomic transition that has been occurring in several countries. The object of this study is the evaluation of practices offered at Psychosocial Care Centers for Users of Alcohol and Other Drugs (CAPSad) based on the point of view of the families of their users. Methods: evaluative research, by quantitative method, to analyze data collected at CAPSad located in the city of São Paulo, using the Satisfaction Scale (Satis-BR) and the Perceived Change Scale (EMP), already validated and previously used in evaluation studies, will guide the focus of analysis. In addition to the scales, data on the service structure, sociodemographic data, access, and characterization of the family\'s participation in the user´s treatment, were analyzed, with the lead of the multiple linear regression model, in which it revealed a change perceived as a predictive variable higher level of satisfaction with the services and the negative correlation of these variables with perceived change in rates: living in the eastern and northern areas of the city and considering a monthly income between 2 and 4 minimum salaries. The fact that no nuclear kinship, as being an uncle or nephew, with the user, has had a positive association with the perceived change. Further studies are needed with this type of service to better explore these dimensions related to quality of care in this theme, helping to assess the impact of new policies and Psychosocial Care Network.

The University of Zululand Community Psychology Centre (CPC) is a joint project of the Department of Psychology, Educational Psychology and Industrial Psychology. It was established in response firstly to the University's need to provide relevant training for its post-graduate psychology students and secondly to meet the need within the Zululand community for affordable psychosocial and psycho-educational services. The Centre works in partnership with other local Zululand Mental Health centres. The Centre is accredited with the Professional Board for Psychologists of the Health Professions Council of South Africa (HPCSA). This board ensures that institutions providing psychological services observe ethical codes of conduct and good practice (HPCSA, 2002). The present research was motivated by the fact that the CPC functioning has never been evaluated in terms of service delivery and relevance. The desire to know whether the CPC serves the purposes for which it was established, and to identify areas that need improvement, is a strong motivation. This helps to identify its viability, effectiveness, and value for the University of Zululand and the community served by the University. The aims of the study were to evaluate the CPC in terms of clients' perceptions with regard to effectiveness of the centre in meeting their needs and improving their psychological well-being. An accidental or convenient sample was used. An invitation to volunteer was extended to clients who happened to come for the CPC services. Volunteers completed a questionnaire^ which included a biographical inventory needs analysis questionnaire and RyfTs Scale (1995) of psychological well-being. Participants were pre and post-tested on the variable of psychological well being. The study concluded that the CPC still serves the purpose for which it was established. It is still relevant to the demand of the community. The results revealed that pre and post testing was associated with a significant improvement in total psychological well-being as with special reference to personal growth and positive relations with others.

NISAA was established in 1994. It is one of the first organisations dealing with women abuse in Lenasia and the surrounding areas. The organisation has great potential in becoming a recognised institution in South Africa. Women who are in distress, have no where to go and are left abandoned have a shelter with NISAA. In the short period of its existence NISAA has strived hard to uplift and assist women. Counselling and treatment is offered on a daily basis by professionals and trained personnel. However, many needs still have to be met and research has shown us that not nearly enough is being done for those who are being abused, be it wife abuse or child abuse. The processs of research is presented in five stages and covers, the problem statement, the literature survey, research methodology, analysis, conclusions and recommendation. The survey of the literature is very important in this study. South Africa has become epitomised by a rapidly changing social order, swiftly unfolding political development and an unprecedented fluidity in the thinking of future opinions. The attitudes and perceptions of domestic violence and violence in general needs to be changed. The chapter on research methodology stresses the importance of feminist organisations. Feminist organisations' role is vital in any study on woman abuse, as research on woman abuse began with these organisations. Methodology in this study also focuses on the qualitative nature of battering, research and the relevant themes surrounding battering. These themes include woman abuse, intervention. treatment and shelters. Other important aspects discussed is coding of data, research design, sampling and interviewing procedures. The analysis and results shows that NISAA has made a significant contribution to battering of women. From the results of this study, NISAA has become an important structure and service in the South African society. The services NISAA should expand on include, a safe and positive crisis help, as well as promoting the rights of all victims. Important to the growth of NISAA is alleviating the funding problem. Other relevant findings discussed are attitudes and perceptions of women abuse and services that encourage and discourage the use of NISAA. These findings compliments. Gishen's (1993) study on the Power Organisation. Recommendations made to NISAA are based on the findings of this research. This research hopefully will expose abuse, help women to achieve recognition in its fight against abuse and in its quest for the empowerment of women. The research was also intended as an educative process. This study entails the physical and psychological abuse of women which has become a global problem. The problem needs to be addressed, understood, analysed and treated. An effective solution must be found.

Assertive Community Treatment (ACT) represents an effective treatment for individuals with severe mental illness. Though studies estimate that as many as half of all people with severe mental illness are parents, little is known about consumers receiving ACT services who are parents. Thus, the purpose of the present study was to 1) estimate the prevalence of parent ACT consumers, 2) identify current ACT team policies and practices for treating parent consumers, and 3) examine the perspective of parent consumers served by ACT teams. Quantitative and qualitative data were collected and analyzed via two separate studies. In study 1, eighty-two ACT providers from 76 teams across the United States and Canada were surveyed to determine the prevalence of parent ACT consumers, ACT team policies for identifying the parental status of consumers, treatment services available for parent consumers, and provider attitudes about parent consumers. Providers estimated roughly 21.6% of ACT consumers were parents. Less than half of providers (46.3%) reported formally asking about parental status during intake and only 20.7% providers belonged to ACT teams that provide special programs/services designed for parent consumers. The majority of providers (75.6%) reported negative or mixed attitudes about parents with severe mental illness. In study 2, seventeen parents with severe mental illness being served by ACT teams were interviewed about parenting, the relationship between parenting and severe mental illness, parenting needs, and suggestions for improved treatment services for parents. All parents were able to identify at least one positive aspect of parenting and most parents (76.5%) also identified negative aspects of parenting. Loss of custody emerged as a significant parenting problem, with the majority of parents (88.2%) experiencing loss of custody at least once. Given the difficulties of being a parent and having to manage a severe mental illness, parents expressed interest in several parent-focused treatment services, including family therapy, parenting skills, communication skills training, resources/finances for children, and social support groups with peers. When asked about overall satisfaction with ACT services, most participants with adult children (87.5%) reported having no unmet parent-related needs and high satisfaction (4.63 out of 5) with ACT services, whereas parents with young children (77.8%) reported having numerous unmet parenting needs and low satisfaction (3.78 out of 5) with ACT services. Thus, the age of participants’ children was a significant factor, indicating that the ACT treatment model may not be adequately serving parents of young, dependent children. Overall, findings suggest the need for more attention and focus on parent consumers, including formal identification of consumers’ parental status and improved parent-related treatment services and support

This study looks at two community mental health facilities. The one setting is that of a state aided organisation, while the other is a non-government organisation (NGO). These two settings are contrasted in terms of how they conceptualise the concept 'community', their physical settings and facilities, and the activities and processes at each setting. The differences in the day-to-day operational processes, and activities according to their respective philosophies - psychiatric medical model and ecological model - are explored and captured from the participants through utilising qualitative data gathering methods such as interviews, observations and the personal experiences of the researcher. The information obtained from each participant in both settings reflect how they think, feel and behave towards their work. This information contributes to an understanding of how community mental health clinics operate. Finally the recommendations are of how work could be done differently, making them both more community orientated.

Chronic diseases have increased dramatically in Australia and around the world over the past decade, causing pain, suffering, disability, psychosocial problems, early mortality and economic and public health crisis. However, many chronic diseases and conditions could be prevented with better evidence based and community based health promotion strategies. Guided by a philosophy of idealism, the aim of this thesis was to develop a community centred health promotion strategy to assist the improvement of health and the prevention of chronic disease in an Australian context. More specifically, the research was concerned with exploring the potentiality of statistical or epidemiological evidence and community collaboration as pathways to chronic disease prevention and improvement of health at an individual, community and system level. The research utilised a praxis paradigm and action research design over three stages. Stage One included in depth quantitative analysis of health and epidemiological data and addressed the question: What is the current evidence/knowledge about health status, determinants and inequalities in Victorian communities and the broader Victorian and Australian context? Stage Two involved qualitative participatory action research methods to engage a small group of community members from the Brimbank region of Melbourne (Victoria, Australia) in the community governance of health promotion and disease prevention. The questions addressed were: What are the benefits of community based health promotion and prevention? What ideas for health promotion action does the community have to offer? Stage Three involved a minor evaluation of the strategy as a whole and addressed the question: In what ways, can health evidence and community involvement in health promotion contribute to better health outcomes? Stage One identified various determinants that impact on health status and result in inequalities. Stage Two revealed six major benefits for community based health promotion and prevention and generated a number of useful ideas for health promotion action in the community. Stage Three showed positive evaluations by the participants and identified numerous indicators of success of the health promotion strategy as a whole.

In Ghana, poor knowledge about mental disorders, compounded by negative attitudes developed from deep-rooted stigma, may be responsible for inappropriate behaviour towards people with mental disorders. The effects of negative societal reactions toward individuals with mental disorders and their families may be minimised considerably if people are exposed to credible and adequate information about mental disorders. The broad aim of this study was to evaluate the effectiveness of a mental health literacy programme on Ghanaian assembly members’ knowledge about and attitudes toward people with mental disorders. In addition, the study sought to evaluate the perspectives of the assembly members who participated in the programme about its usefulness and in what ways, if any, they used this acquired information in their routine dealings with people with mental disorders and their families. This was a mixed method, sequential explanatory design, comprising a cluster randomised controlled trial and a process evaluation. A mental health literacy programme, based on a problem-solving Story-bridge approach, underpinned by andragogy, was developed for the study. Ten district assemblies were randomised into intervention and control clusters. Overall, an equal sample of 70 assembly members in the intervention group received the programme while the control group received a plain language basic brochure about mental health issues. Data were collected from both groups at baseline and 12-week follow-up. In addition, a mixed method process evaluation was undertaken with the intervention group. Principal findings of the study indicated that the programme was somewhat effective in improving participants’ knowledge about and attitudes toward people with mental disorders. Overall, both clusters demonstrated an increase in their mean scores at follow-up; however, the intervention cluster demonstrated a greater improvement than the control cluster. Apart from the community mental health ideology outcome measure, which had a medium and statistically significant difference between the two groups in changes over the two time-points, all other outcome measures had small differences which were not statistically significant. The process evaluation highlighted the usefulness of the programme. Participants expressed value in the programme; improved knowledge, increased confidence and willingness to engage, and increased empathetic and non-judgemental attitudes toward people with mental disorders and their families. In addition, participants commended the delivery of the programme, and recommended ways to boost realisation of the programme objectives and education activities that could be introduced to improve community mental health literacy. Overall, the findings indicate that it is possible to improve people’s knowledge about and attitudes toward people with mental disorders and their families. The outcomes suggest that mental health education should be intensified to improve mental health literacy and reduce deep-rooted stigma. These improvements, in turn, may contribute to positive attitudes towards people with mental disorders and their families, enhance the quality of mental health service delivery, and decrease the treatment gap and burden of mental disorders in Ghana.

Abstract:
This research programme endeavours to develop, implement and evaluate an effective method of offering psychosocial support to vulnerable children. Vulnerability is defined by trained community members as including children who are experiencing especially difficult lives. The forms of difficulties experienced by the children has usually been a consequence of the HIV/AIDS pandemic, extreme poverty combined with other psychosocial risk factors, child abuse (especially child sexual abuse) and violence. This community based programme incorporates four phases of intervention, only two of which were the focus of summative evaluation. However, due to the integrated nature of the programme it was necessary to implement and document the various phases of the intervention programme: (i) community selection and mobilisation; (ii) the 5-day Sensitisation Programme (SP) sensitises adult community volunteers to the psychosocial needs of vulnerable children; (iii) the 15-session Structured Group Therapy Programme (SGTP) enables children to work through past adversities and to build resilience within small groups of peers in a programme where community volunteers served as apprentice facilitators under the supervision, guidance and ethical responsibility of qualified psychologists; (iv) community based initiatives to offer on-going of PSS activities to vulnerable children in each of the partnering communities. Nine partnering communities were selected, three township, periurban and rural communities. This programme was not effective in the informal settlements as it was not possible for these communities to place children as a priority. A qualitative summative evaluation of the SP took place using post workshop evaluation questionnaires, focus group discussions conducted by an independent researcher and an audit of the community based initiatives that developed as a result of participation in the SP. The SGTP was summatively evaluated using a 4-way Factorial design with one within-subject and three between-subject conditions: to investigate the age of the subjects, the geographic regions and gender variables. The 741 children formed five experimental and control conditions to conduct various combinations of the above-mentioned phases programmes and to adequately control for the many confounding variables. Pre- and post intervention assessments were conducted by trained community research assistants. The dependent variable measures were the Culture Free Self Esteem Inventory (Battle, 1992), the Trauma Symptom Checklist for Children (Biere & Elliot, 1997), the Reynolds Depression Scale for Children (Reynolds, 1989), the Social Support Scale (Beale Spencer, Cole, Jones, and Phillips Swanson, 1997) and the Connor's Parent Questionnaire Connors, 1998). Multivariate analysis evaluated the effectiveness of the various experimental and control conditions. The results indicate that the SGTP, run in combination with the SP, is an effective intervention strategy in that it alleviates symptoms of self-reported depression and other psychosocial manifestations of distress as well as decreased the number and severity of symptoms reported by primary caregivers, and leads to increased access to perceived social support. The SP and the SGTP conducted independently of each other have limited benefits and as such can be considered to be partially effective. The children who had formed part of the non-vulnerable control group felt left out of the programme and report an increase in symptomatology and decreased access to social support. While this community-based programme can be considered to be an effective method of therapeutic intervention and of offering psychosocial support to vulnerable children, further research is needed to consider the cost-effectiveness, the sustainability and ways in which those children who do not participate can still can benefit.

Abstract Keywords: Lay counsellors, training and evaluation. The training of lay counselling is of great significance to the field of Counselling Psychology. Therefore,the Health Professions Council of South Africa (HPCSA) decided to allow for the registration of lay counsellor training courses. However, the criteria for registration need to be formulated. This study evaluated the Wits Crisis course for volunteer counsellors, via illumination of the perceptions of the trainees and a measurement of their self-efficacy and personal relations before and after the training. The study included qualitative and quantitative methodology. Past trained counsellors and trainees were interviewed and then assessed on Self-Efficacy and Personal Relations before and after training. A Wilcoxon Matched-Pairs Signed-Rank significance in self-efficacy or personal relations scores after training. However, the interviews of the participants' perceptions of the course and its impact on them supported the hypotheses. Overall, perceptions were positive. Negative perceptions were highlighted, as this information could add to the knowledge base of the program.

High prevalence of mental illness in South African and limited Mental Health Care (MHC) resources call for ways to supplement the overburdened system to meet the rehabilitative and supportive needs of service users as stipulated in the MHC Act no. 17 of 2002. Various psychological, social, and financial difficulties plaguing MHC users require holistic treatment intervention to help them cope with their conditions. This study explored the meaning of a Tshwane District community-based support group for MHC out-patients to help illuminate the value the group had for the members. A Social Constructionist epistemology informed the qualitative research approach. Data were collected by conducting semi-structured interviews and employing a collage-making method. The main themes illuminated by thematic analysis suggest that the group provided the members with a sense of belonging, mental and physical mobilisation and stimulation, a source of support, multiple learning experiences, and contributed to personal transformation and growth.

Abstract:
South Africa is a country steeped in decades of conflict and animosity. Apartheid and its consequences do not simply die: it has created a society struggling for survival. Against the backdrop of a country and its people still experiencing an extended social crisis, these struggles are socially constructed through various forms of aggressive, traumatic and violent behaviours such as crime victimisation. The resultant effect is that many South Africans are traumatised and require assistance to manage and deal with the impact of traumatic exposure. Counselling and psychological services within the South African context are limited, potentially expensive and often inaccessible to the poor. One way to address the needs of victims of crime and violence, is through the establishment of one-stop multidisciplinary crisis centres that specialise in short term crisis intervention service delivery. One such a crisis centre is the Sinoville Crisis Centre (SCC). The purpose of the study is to present an exploratory qualitative and participatory action research account of the SCC's endeavours and ongoing challenges in providing crisis intervention services as well as to serve as a guideline for future development. Research interviews with seven (7) SCC counsellors were complimented with a focus group interview. Subsequent conclusions were grounded in relation to relevant subject theory. Three (3) broad categories of recommendations are provided. Specific recommendations are levelled in relation to: * The SCC's crisis intervention models * The SCC's need to manage organisational change and loss; and * The SCC's role within the Victim Empowerment Programme.

Abstract:
Objective: This thesis aimed to advance the study of sustainability through the exploratory use of a conceptual framework for the investigation of a collaborative project. Methods: A qualitative case study design, utilizing document analysis and key informant interviews, was used to conduct a three-year follow-up of a collaborative mental health project. The study design and directed content analysis were informed by the Scheirer and Dearing (2011) conceptual framework. Results: The sustained outcomes identified by the six participating agencies included: institutionalized project components, maintained client benefits, continued collaboration and sustained attention to the issue. The sustainability of project components was associated with complex interactions between the innovation, organization, and community factors investigated. Conclusions: The study illustrated the importance of evaluating multiple aspects of sustainability to fully capture a project’s long-term effect. Complex systems theory was proposed to describe the interactions observed and as a direction for further development of the conceptual framework.

The system of care for persons suffering from schizophrenia is in a state of upheaval and the community in general is visibly frightened of an illness it knows little about except for the information acquired often from media reporting. Considering persons suffering from schizophrenia as holistic beings, this study explored and assessed the needs of these persons residing in the Mogoto Village, Zebediela District in the Northern Province. The primary purpose of the study was to explore and describe the holistic needs of persons suffering from schizophrenia thus facilitating the planning of care and care facilities for these individuals to enable them to function optimally in the community. To accomplish this purpose, specific objectives were formulated. A quantitative, exploratory and descriptive study based on the Nursing Theory for the Whole Person was carried out. Through purposive sampling 60 respondents were selected. A questionnaire was administered to the respondents. Data from questionnaires revealed that despite the diagnosis of schizophrenia, the respondents were still regarded as valuable members of the community by those who cared for them. There was also an indication that the needs of persons suffering from schizophrenia in the Mogoto Village did not differ from the needs of other persons in the rest of the world. Like any unique person they have specific needs. There was also an indication that mental illness is still rated low in the prioritisation of health problems, hence the lack of resources and support needed for the rehabilitation of persons suffering from schizophrenia, especially in the rural areas.

Considering the mentally handicapped as holistic beings, this study explored and assessed the needs of those residing in the community of District 22 (sub-district 222), KwaZulu-Natal, thus facilitating the planning of care and care facilities for these individuals to enable them to function optimally in the community. To accomplish this purpose, specific objectives were formulated. A quantitative, exploratory and descriptive study based on Maslow's hierarchy of needs theory was carried out, using 167 respondents. The study revealed that despite being mentally retarded, they were still regarded as valuable members of the community, their needs did not differ from the needs of others in the rest of the world and that mental retardation is still rated low in the prioritization of health problems, hence the lack of resources and support needed for the rehabilitation of such persons.
