Community care has lost its innocence. Critics and evangelists vie for television space and newspaper headlines. The Chief Medical Officer had to beg for clarification of a term that had developed too many meanings and an “ethos of virtue” (Acheson, 1985). How should it be defined? Can a hospital be part of the community? Does it mean care ‘in’ ‘by’ the community? The Department of Health and Social Security (DHSS) definition seems to be “the provision of alternatives to long-term institutional care for the chronically sick or those suffering from long-term handicaps or disabilities” (Hunt, 1985). Lord Trefgarne (1984) put the policy aim, more simply, as “to move out of hospital those people who do not really need to be there”.

Abstract:
Primary Health Care (PHC) has been promulgated for over two decades as a global strategy for ensuring basic health care for all people. PHC is characterized by equity, accessibility, availability of resources, social participation, intersectoral community action, and cultural sensitivity. While PHC can be discussed as philosophy or a process, it is critical that PHC be understood as a community focus in health care that differs from a primary care focus on individuals. Capturing PHC components in community-based interventions in order to advance the development of a rigorous research base requires a shift in thinking about what constitutes acceptable methods and evidence for evaluating changes in health care. To this end, the authors of this review discuss perspectives and available research that inform practice within multidisciplinary teams, highlight the importance of social discourse, and review participatory evaluation issues for achieving a working relationship with communities. Particular attention is focused on education for nurses’ roles in PHC activities within implementation models fostering community mobilization and development. An action plan is suggested as a means for situating discrete research activity within a PHC framework.

AbstractObjective: To measure the level of commitment by mental health care professionals within the Eastern Health Board to the concept of community mental health.Methods: An adapted version of the Baker-Schulberg Community Mental Health Ideology Scale was administered to a random sample of professionals (psychologists, psychiatrists, psychiatric nurses, community psychiatric nurses, social workers and occupational therapists).Results: Psychiatrists within this study had the lowest scores indicating the least level of commitment, and were also found to focus upon extrinsic issues, such as economic barriers, as possible obstacles to the development of a community mental health service in Ireland.Conclusion: Policy and decision making committees need to address the balance of the mental health care professionals represented on those committees.
