OBJECTIVES: Describe the characteristics and research interests of Community Health Centers (CHC) pharmacists and pharmacies in a pharmacy based practice based research network (PBRN). METHODS: Pharmacy directors of eight Arizona CHC pharmacies were initially contacted by telephone and asked to participate in a survey. The survey was then sent to these directors via email and a second telephone conversation was set up to re-­‐administer the survey and gather the answers to the questionnaire. The questionnaire consisted of demographic, clinical and practice related questions targeted at identifying areas of interest for research in developing a community pharmacy practice based research network (PBRN). RESULTS: The data regarding the demographics and characteristics of the Arizona CHC pharmacies, pharmacists, and patients showed that only two pharmacies served more than 300 patients per day and dispensed more than 400 prescriptions per day. The data found that an average of 46% of the patients that went to these pharmacies did not consider English as their primary language and that an average of 49% of the patients were Hispanic/Latino. The data collected regarding the clinical interests of these pharmacies showed that asthma had the highest level of clinical interest among the eight CHC pharmacies (average rank = 3.1). For the public health interests of the CHC pharmacies, patient adherence/compliance was ranked the most important (average rank = 3.1). In regards to internal practice site interests job satisfaction was the most highly ranked interests among these CHC pharmacies (average rank = 3.1). CONCLUSIONS: The data suggests asthma, job satisfaction and patient compliance/adherence are the top areas of interest in the clinical, internal worksite, and public health sectors. The data collected from this study will help to establish a pharmacy based PBRN in Arizona and provide a starting point in terms of research topics that will be explored. The establishment of an Arizona pharmacy based PBRN is very important since it will provide cohesiveness between research and community based practice of Community Health Centers in Arizona and is a step in the right direction in terms of growth of these centers.

Abstract:
Community-based participatory research (CBPR) is an increasingly popular form of public health research. However, little is known about the application of CBPR and the levels of involvement for partners in specific phases of the partnership. This phenomenological study addressed the application of CBPR from the perspectives of 7 academic researchers and 6 community members experienced in CBPR. Arnstein's ladder of citizenship participation and the community coalition action theory provided the framework for the study. Semi-structured interviews addressed participants' levels of involvement in the CBPR process, as well as challenges, concerns, successes, and recommendations for improvement. Interview transcripts were analyzed by identifying recurrent themes relevant to the experience of being a CBPR partner. These themes were then used to develop descriptions of their experience. Results indicated that participants knew the term CBPR and had experienced it, but not all participants understood the depth of CBPR and how much bargaining power they could have for their community. Sustainability of partnerships and programs was a major concern. Ethical problems were also raised regarding the long-term commitment to projects and the need for CBPR partnership evaluation. Results may be used to strengthen awareness of the principles of CBPR to advance culturally tailored public health interventions.

Abstract:
Community-based participatory research (CBPR) is a research approach that benefits from the expertise of community members being involved in the research along all stages of a project (Israel et al., 2003). CBPR is often utilized with marginalized populations in order to amplify a community's voice on important issues in their lives (Bastida, Tseng, McKeever, & Jack, 2010; Minkler & Wallerstein, 2008). In the past, persons with disability have been excluded from research in order to protect them from exploitation. This practice of exclusion undermines opportunities for persons with disabilities to be independent and make decisions that are important for themselves and their communities. Exclusion also limits the generation of new knowledge that can benefit them (McDonald & Keys, 2008). Through involvement on a CBPR project, persons with disabilities are given the opportunity to become empowered within the context of the project (Atkinson, 2004; Oden, Hernandez, & Hidalgo, 2010). This study examined empowerment definitions, evolution of empowerment definitions, and facilitators and barriers to community partners with and without disabilities becoming empowered through their work on a CBPR project. Overall, community partners' definitions of empowerment related to individual and setting-level characteristics. Individual-level empowerment was defined as self-efficacy, self-esteem, control over decision-making, and disability rights advocacy. Facilitators to empowerment within the CBPR process were promoting inclusion, promoting an accessible partnership, sharing of power within and between groups, and actively sharing and gaining knowledge within and between groups. Inaccessible communication, inaccessible language, and lack of project ownership were identified as possible barriers to empowerment. In most cases, empowerment definitions remained stable across one's work on this project, but there were instances of positive change in the lives of some community partners who expressed being empowered through the partnership. CBPR provides an opportunity for persons with developmental disabilities to be included in the research processes as well as possibly gain important qualities throughout, such as empowerment. This study situated the individual's empowerment beliefs and behaviors within the CBPR setting, identifying both facilitators and barriers, and provides support that a CBPR process can be empowering for community partners. Future research in collaboration with community partners should continue to focus on empowerment in all stages of the research project, local collaborations, and continued diversity of community engagement in research. Engaging in a formal reflection process and documenting the process for other researchers to learn from diverse barriers and facilitators to empowerment is encouraged.

Abstract:
Communities arc dynamic, historically determined, and complex, and the topic itself has concerned social scientists for some time (Durkhcim, 1964, Weisenfeld, 1996). However, there has been an intrinsic problem in previous research arising from the ambiguity of the concept of the term 'community' itself, specifically the contradictions concerning its essential meaning. The aim of this study was to determine whether there are other ways of thinking about community without taking on any unwanted connotations of previous conceptualisations from past research. The research was based on the social constructivist paradigm and qualitative methodology was employed. Conceptualisations of the term community were surveyed among 16 participants using semi-structured in-depth interviews. Results were analysed using analytic induction methodology. Participants identified seven interrelated concepts: geographic attachment to place, communality, social interaction, active involvement and participation, family, sense of belonging, and transience. From this research, understanding of the term 'community' has been shown to have far reaching implications, which involve influencing the assumptions underlying community development initiatives and programs promoting social change. Furthermore, a sociopsychological understanding of community can help to facilitate the intentional creation of community when and where it is needed.

Abstract:
Calls for more creative teaching and learning in classrooms are often matched by increasingly stringent accountability measures. Negotiating the creativity/accountability paradox is difficult for teachers, who are often isolated as they interpret, design, and deliver curriculum in their classrooms. This dissertation introduces and refines a 7-stage process called the Community Creative Collective (3-C) designed to generate solutions to three problems that derive from this paradox. First, narrowing of curriculum inhibits the ability of teachers to generate creative teaching and learning. Second, factors, including time constraints and teacher training, limit teachers' ability to develop the creative habit. Third, inclusion of family and community members as co-creators of curriculum provides a potential source of creative curriculum development. Three research questions guide the exploration of the process: 1. How does the 3-C process allow teachers and community members to collaboratively generate creative teaching and learning opportunities for their students? 2. What are the distinguishing features of this collaborative curricular process? 3. How does such a process impact teachers' interpretations of their role as interpreters, designers and deliverers of curriculum? Using a Design Based approach, these questions investigate the process as it was implemented in a 5th-grade classroom. The first question uses a case study methodology to trace the development of the 3-C process as it was developed and implemented. Findings demonstrated that communication at multiple stages impacted the generation of creative ideas. The second question uses qualitative data from documents, interviews, audio and video recordings and observations to extrapolate some of the distinguishing feature of the process. Key features included the Ideational Speed Dating (ISD) process for idea-generation, the 3-C process as a peak flow experience and the impact of parent and community expertise to generate creative classroom content. The third question uses interviews with the participating teacher to examine the impact of the 3-C process on her interpretation of her role in the classroom. The process influenced her view on family and community involvement, providing space through which tensions can be resolved and creative engagement can flourish. Finally refinements for future iterations are discussed in addition to implications for future research.

Natural resource managers are facing increasing challenges as environmental degradation accelerates and the need to integrate a broad spectrum of community experiences into management decisions is increasingly recognised. To help meet these novel challenges, this study explores how professional researchers and communities can combine their skills and learn to work in partnerships to achieve shared management goals. Community-based research involves people as citizen scientists, whereby citizens actively participate in research on local issues. The inclusive nature of community-based research has the ability to produce auxiliary benefits uncommon in conventional research. These include the development of social capital and social learning as the practice of citizen science empowers communities with new skills, knowledge and social networks, thus building capacity within communities to take an effective role in natural resource management. Community-based research also has the potential to enrich the range of management options available by increasing the breadth of accessible knowledge. However, despite much rhetoric about democratising science, little is known about the practice, value and problems of involving citizens as collaborators in natural resource management research projects. This thesis presents the findings from a comparative survey of the attitudes to community-based research held by 'citizen' scientists, on the one hand, and 'expert' scientists and natural resource managers, on the other. It also draws upon a multi-site case study, set in a diverse urban-rural catchment, where an integrated research program was established for different natural resource management agencies to work with each other and community groups to develop research protocols so that community groups could participate in assessing the health of catchment areas. This involved scientists, natural resource managers and community education/extension officers working with established community groups to develop and trial modified scientific methods for the environmental monitoring of catchment and estuarine areas. This inter-agency/community project was continued as a case study site into the second and third years of research and was augmented in the second and third years by focusing on two of the initial community groups as second and third case study sites in their own right. Synthesis of both survey and case study analysis reveals that, despite resource and attitudinal barriers, community-based research can ensure access to local knowledge and increased relevance of research. In addition, many participants most valued the increased feeling of connection towards their local environment and community. I argue that citizen/expert collaboration is key to successful community-based research and best achieved in an atmosphere of mutual respect where all participants are seen as co-researchers. However, participatory intentions are unlikely to be acted upon without sufficient opportunity. Thus, the process of research must be re-defined from that associated with positivist science to include a greater range of participants and activities in an adaptive manner. This more inclusive and reflective approach seems most likely to ensure the quality and utility of research data, the knowledge sharing and social learning, and the enjoyable atmosphere that underpin successful citizen/expert interactions. Certainly, the ability to draw upon and create social capital is vital. The integration of these findings enabled the development of guidelines for effective collaboration between citizens and experts when addressing catchment management issues and undertaking participatory research.

Abstract:
Despite a significant increase in the demands for teachers’ professional development, the work environments of teachers have not developed at a comparable rate. Due to research on inequity in teacher work environment, the purpose of this case study was to explore the role of empowerment in the early childhood education (ECE) workforce, using Community Based Participatory Research (CBPR) as a framework. This project was done with 5 educators in a for‑profit ECE center. Over the course of five weekly sessions, the educators completed questionnaires, interviews, and engaged in weekly focus groups geared towards providing the teachers with an opportunity to share their experiences and collaborate on solutions for change in their work environment. Overall, descriptive statistics of the quantitative data did not demonstrate an increase in empowerment over the course of the project. However, the six emerging themes (i.e., Frustration with Center Operations and Corporate, Empowerment, Communication, Emotional and Physical Well‑Being, Teacher Unity, and Teacher Training and Education) provided important insights into the nature of teacher empowerment in the ECE setting, which informed lessons learned and future directions for research.

Abstract:
Governance is a response to a recognition that traditional forms of decision-making have become inadequate to address complex societal and health problems generated by significant social and global changes (Chhotray & Stoker, 2009). The contributions of scientific and technical knowledge towards solving these complex problems have also been recognized as insufficient (Jasanoff, 2007). Community-based research (CBR) is an approach to research which is designed to make use of the knowledge of community and university members and their participation and collaboration ―in all phases of the research process, with a shared goal of producing knowledge that will be translated into action or positive change for the community‖ (Lantz, Israel, Schulz & Reyes, 2006, p. 239). However, although the contributions of lay knowledge have been acknowledged, how governance or collaborative decision-making is arranged in the context of community-based research is not well described in the literature. In order to address this knowledge gap, a study was undertaken in which in-depth interviews were conducted with community and university members of Canadian CBR collaborations to determine their governing experiences. Results are reported in a thesis by research papers. The first paper focuses on describing the governance structures that CBR collaborations used. In the second paper, the nature and content of problems which occurred in governing CBR collaborations, point to the importance of theory for conceptualizing and solving governance problems. To develop a theory of participation in governance of community-based research, the third paper uses Arnstein‘s theory of participation to propose a grounded theoretical basis for implementing participation in governance of CBR collaborations (Arnstein, 1969). Governance is a means of organizing, shaping and steering a course of decision-making. Governance is a critical component in the organization of knowledge production. Study and theory of governance in community-based research may help in improving understanding and implementation of a critical population health practice.

Abstract:
In an effort to begin filling a gap in the scientific literature about community-based food systems and promote food environment changes to increase dietary quality, this dissertation focuses on community-based food systems research with two different populations: Head Start families in southwest Virginia and Heifer International Alternative Spring Break (ASB) college student participants at Virginia Tech. Fresh Produce, Fresh Start (FPFS) tested the effectiveness of a local, fresh fruit and vegetable delivery program on dietary intake and purchasing patterns of Head Start families. FPFS utilized a one-group double pre-test post-test was conduced on two occasions at each of the two sites. Measures collected included: food-use questionnaire, height and weight, food security questionnaire, 24-hour recall, program process evaluation, and food receipts. Fifty-one of 67 (76%) of eligible participants were recruited. For dietary recalls completed (n = 29, 57%), significant increases were found in intake of vitamin A, vitamin C, fiber, vegetable, and fruit and vegetable (FV) servings combined, based upon paired t-tests (p <.05). Based on receipt data (n = 22, 43%), the proportion of fresh produce purchases significantly increased for both number of items and monetary costs, based upon a Wilcoxon Signed-Ranks Test (p < .05). A second study was conducted to examine the impacts of the Heifer Alternative Spring Break (ASB) programs on Virginia Tech studentâ s attitudes, motivations, diet, and behaviors regarding community-based food systems before and after a one-week intensive program. The design was quasi-experimental, using a one-group pre-test, post-test, and follow-up. Participants completed questionnaires, dietary recalls, and journals. Forty-three of 43 (100%) of eligible participants enrolled in the study. Although increases in local and organic foods were observed, there were no significant changes in dietary quality. The intervention showed significant increases in local and organic food consumption for individuals that consumed less than 50% of their calories from local and organic foods at baseline based upon a paired t-test (p < .05). The findings from these studies offer evidence of the capacity for community-based food systems to change dietary patterns, across different populations.

Tangible and intangible heritage values of cultural landscapes are becoming lost or transforming under the threat of rapidly changing landscapes. Researcher-oriented documenting methods are missing significant meanings of landscapes for local communities. Community-based methods can reveal both tangible and intangible heritage of landscapes without missing important values for local communities. This dissertation study proposed a community-based mixed method research to reveal and document cultural heritage or other values from the perspective of local community members in the case study area of Findikli in Rize/Turkey. Findikli's cultural landscape is under the threat of rapidly changing landscape due to newly introduced agriculture practice - tea production. To reveal lost or transformed tangible and intangible heritage meanings of the Findikli's cultural landscape, multiple community-based research methods were used for collecting data from local residents as well as those with family or community connections to the area. Community workshops, individual and group interviews, and surveys gathered information on the social and cultural relationships, as well as locations of past and present agricultural activities, land uses and built structures. Analysis of family and community photographs and aerial imagery, as well as community produced land use and cognitive maps helped place these in spatial relationship to the landscape. Results of this dissertation study made contributions to case study area with a rich archive of Findikli's traditional tangible and intangible landscape elements, and to cultural landscape studies with a method of discovering traditional cultural heritage and landscape values under the threat of change and a guidance to document them with the community-based methods to increase quality and quantity of information.

Background: Community gardens have existed in America since the late 1800s and have served multiple purposes from food subsidies to neighborhood beautification. The use of community gardens has grown in popularity and has been recommended as a way to encourage healthy eating habits in youth. Though the health benefits of having a diet high in fruits and vegetables is well known, youth in the United States do not meet recommendations for fruit and vegetable intake. Under-consumption of fruits and vegetables is problematic in youth, as eating habits are established in childhood. Community gardens have been successfully used to improve access, self-efficacy, preference, and consumption of fruits and vegetables. However, few published community garden studies have focused on low socioeconomic youth. The Dan River Partnership for a Healthy Community (DRPHC) was developed according to community-based participatory research (CBPR) principles. With a mission to reduce obesity using healthy lifestyle initiatives, community gardens are an evolving DRPHC initiative. Objective: To evaluate the feasibility (i.e., demand, acceptability, implementation, and limited-effectiveness testing) of a 10-week experiential theory-based gardening and nutrition education program targeting youth living in two public housing sites in the Dan River Region. Methods: Using pre- and post-program questionnaires/interviews, demand and acceptability were measured among youth, parents and site leaders. Implementation was measures via field notes and attendance. Limited-effectiveness was measured among youth using a pre-post design. Three researchers independently coded the qualitative transcripts, met to resolve disagreements, and built consensus through discussion of the codes. Similarly, field notes were reviewed and evaluated for reoccurring themes regarding barriers, facilitators, and other observations. For the quantitative measures, descriptive statistics were used to summarize the variables and Cronbach's alphas used to assess the reliability of each scale at baseline. Overall effects were tested with repeated measures ANOVA. An intent-to-treat analysis using the last observation carried forward method was used. A critical value of .05 was used for significance testing. A standard equation for reporting effect sizes on a single-group, pre-post study design is also reported. Results: Program enrollment included 43 youth, primarily African American. The positive demand and acceptability findings indicate the potential of the program to be used and suitable for the youth, parents, and site leaders. Field notes revealed numerous implementation facilitators and barriers. Youth weekly attendance averaged 4.6 of 10 sessions. Significant improvements (p<0.05) were found for some (e.g., FV asking self-efficacy, overall gardening knowledge, knowledge of MyPlate recommendations), but not all limited-effectiveness measures (e.g., willingness to try FV, FV eating self-efficacy). Study Implications: This study addresses recommendations for utilizing CBPR in community garden efforts and builds on community identified research priorities of the DRPHC. Results demonstrate the feasibility of a gardening and nutrition program targeting youth in public housing. Lessons learned are being used to adapt and strengthen the program for future efforts targeting FV behaviors. Findings will be shared with local community stakeholders and used to adapt and strengthen the program for future efforts in the Dan River Region targeting of fruit and vegetable behaviors.

OBJECTIVES: To identify and describe practical incentives and barriers for community health center pharmacists in adopting a practice based research network (PBRN) that facilitates sustainable collaboration. METHODS: Directors of pharmacy at Community health centers listed as members of Arizona Association of Community Health Centers (AACHC), with on-site pharmacies, were contacted via telephone. During initial contact an IRB approved script was used to recruit the pharmacy director’s participation, at which time the subject’s disclaimer form was read and an appointment for a future phone interview was scheduled. Phone Interviews were conducted using a standardized questionnaire, and all results were manually recorded on a standardized data collection form. Data collected included, site specific information including the: educational background of the pharmacy director, and his or her perceived inducements and barriers to participating in a pharmacy based PBRN with the University of Arizona. RESULTS: Phone interviews were completed by 8 directors of pharmacy, 4 women (50%) and 4 men (50%). A total of 5 participants (62.5%) had a BS degree, 2 (25%) had PharmD degrees and 1 (12.5%) had both as BS and a PharmD degree. The mean length of time in current position was 5.56 yrs (SD= 4 yrs.). 75% of the participants indicated that they considered working with the University of Arizona (UofA) as an inducement, the same number of participants felt that their staff and practice as a whole would also consider it an inducement. Overall participants indicated that both their personal (75%) and staff‘s (87.5%) motivation to improve the pharmacy profession was considered an inducement, as well as their opportunity for professional growth (75%). All of the participants (100%) indicated they did not have adequate staffing to support research at this time and therefore felt it was a barrier to participation. When asked about resources as a whole, including staff, time and technology 87.5% of the participants felt this was a barrier. Other common barriers were; anticipated time requirements (75%), current schedule/time allowances (75%), staff’s outside commitments (75%). Out of the 8 participants only 2 (25%) are currently participating in PBRNS at this time, 3(37.5%) have research ideas that they are interested in working on, and 3(37.5%) indicated that they were not currently participating nor did they have any current interests. The major themes identified as inducements to participation were patient benefit, time/staffing involvement, and professional growth. CONCLUSIONS: The most common barriers to participating in a PBRN were: working with the UofA, motivation to improve the profession of pharmacy and the opportunity for professional growth. The most common inducements were staffing, current resources, anticipated time requirements, current schedules and outside commitments.

To understand the reasons for the increase in recent years of the Bosnian population in Bowling Green, Kentucky and their adaptation problems as refugees in their host country, this study focused upon the Bosnian community in Bowling Green and addressed what the role of their challenges is in the shaping of refugees’ new life in their host country. Extensive literature review helped to emerge that for an understanding of the situation of the refugees, their interaction in the host country is more meaningful topic for research. This study employed qualitative research methods, drawing from existing empirical studies addressing resettlement in the context of the informants’ wartime experiences. Initially, the researcher approached patrons at Bosnian restaurants and worshipers at local mosques to find Bosnian people. Snowball sampling used to identify Bosnian refugees living in the Bowling Green community. Twenty-five in-depth interviews were conducted for needs assessment and issue identification. The interviews were transcribed and analyzed in an attempt to understand the difficulties of adaptation among Bosnian refugees living in Bowling Green. A qualitative case study approach was chosen because it was the most effective way to gain knowledge of refugees’ experiences and perceptions in the context of the societies in which they resettled. Findings revealed that interviewed group struggled with mostly language and employment challenges to integration. Social support was provided through organizations that included Americans aided integration and the families resettled before as they provided significant support is directing resettlement. Banki’s (2004) and Jacobsen’s (2001) indicators of refugee integration were used to in order to determine to the extent to which this sample of Bosnian refugees are integrated into their host county.

The lack of feedback of research results to research participants, or the lack of reflection on the feedback process where it does occur, is problematic. Some fundamental issues appear to inform the feedback process, these being the approach adopted by the researchers to research and the uses envisaged for the research and feedback. In this paper, we reflect on feedback sessions conducted with research participants and local community in a community-based mental health programme in a rural area of South Africa. The results of a comprehensive needs assessment were discussed in four different meetings. This paper reflects upon, amongst others, practical/logistical issues, attendance and non-attendance, the issue of language, group dynamics, challenges from participants, and social and cultural constructions of knowledge. It is concluded that when feedback of research results is seen as part of an on-going process of programme development and engagement with various stakeholders, it is a potential point of growth on a number of levels.

The purpose of this research was to investigate the development and implementation of an online community of learners within the context of an intermediate and advanced level Italian as a second language university class using a design-based research approach. The online learning community of this study was developed to address some of the issues related to learning a second language in a context where learners only have limited opportunities to engage in collaborative social interaction and participate in meaningful and authentic activities with other speakers of the target language, such as more competent peers and native speakers. The development of the learning environment drew upon theories and principles derived from the literature, including Vygotsky’s sociocultural theory applied to second language learning, theories of situated and authentic learning, and principles that guide the development of online communities of practice and communities of learners. The learning environment comprised a course website, which provided a combination of different text-based synchronous and asynchronous Computer Mediated Communication (CMC) tools to support communication and collaboration within the learning community, two authentic tasks designed according to the defining elements of authentic tasks identified in the relevant literature, together with the support and assistance provided by selected native speaker facilitators. The research sought to investigate students’ views and opinions on the impact of each element of authentic tasks on their learning, the process and strategies employed by students to collaborate in a community of learners, the nature of students’ contributions to the CMC features provided in the course website, and the role of native speaker facilitators in assisting and supporting students’ collaborative work. The study was structured within a four-phased design-based research approach, which allowed the researcher to progressively test and refine the learning environment developed through a series of successive iterative implementations, and to develop a new set of design principles to guide the development and implementation of similar learning environments in other second language learning contexts. Data collected included individual and focus group interviews with students and facilitators, analysis of online interactions and messages, students’ reflective portfolios, and note taking and observations. Data were analysed using techniques of qualitative data analysis. Findings suggest that all the critical elements of authentic tasks determined from the literature on situated learning and authentic learning environments provided a useful frame of reference for the design of the two authentic tasks of this study. Each element provided valuable opportunities for student learning, particularly in relation to the development of target language communication skills and problem solving and project management skills, and had a positive impact on students’ motivation and engagement with the subject. The findings also highlighted a number of significant issues encountered by the collaborative groups as they completed the tasks, and the strategies employed by the different groups to solve them. Participants identified key principles that could lead to more effective and successful future collaborative work, supporting the view that they had gained a deep understanding of individual and collaborative learning processes. The findings also shed light on the nature and extent of students’ contributions to the CMC features and resources provided to support interaction and collaboration in the online learning community, and on the native speaker facilitators’ role in supporting students in the process of completing authentic collaborative tasks. The major implication of the research is that a learning environment which supports the development of an online community of learners through participation in authentic tasks in collaboration with other learners and native speakers of the target language can be effectively designed and implemented by second language educators to facilitate second language acquisition.

Practice-based research networks (PBRNs) are groups of health care practitioners who engage in translational research and quality improvement activities, with the overarching goal of improving patient care in primary care settings. The Appalachian Research Network (AppNET), a rural primary care PBRN, was created in 2009 and comprises 17 clinics in 16 rural communities in South Central Appalachia. Nationally, only 4 of 152 PBRNs registered with the Agency for Healthcare Research and Quality (AHRQ) place particular emphasis on community pharmacies and pharmacists in research efforts. Researchers at ETSU seek to integrate community pharmacies into AppNET, thereby establishing a novel interprofessional rural PBRN. The objective of this study was to assess pharmacist perceptions regarding practice-based research and interest in participating in AppNET. Barriers to participation in a PBRN, perceived benefits of participation, and practice-specific characteristics were also assessed. Contact information was obtained via telephone calls made to individual pharmacies in AppNET communities. Thereafter, paper-based surveys were mailed to 69 pharmacist contacts, along with a personalized cover letter and a stamped return envelope. A total of two mailings were used to recruit pharmacists to participate in the study. A response rate of 42% was obtained. Respondents were on average 44 years of age and had been licensed as a pharmacist for an average of 19 years. A large majority of respondents (86%) were very or somewhat interested in participating in AppNET. The majority of respondents felt that time constraints and workflow interruptions were the greatest barriers to participation. One hundred percent of respondents indicated that research on prescription drug abuse, medication adherence, and medication safety are very or somewhat applicable to their practice settings. Ninety-two percent felt that research on value-added services (e.g., immunizations, diabetes education) and medication therapy management was somewhat or very applicable to their practice. Overall, pharmacist respondents in AppNET communities indicated interest in research that benefits the care of their patients and interest in AppNET. Researchers are presently conducting a third recruitment attempt and will thereafter develop AppNET enrollment mechanisms that minimize barriers to participation of community pharmacies in practice-based research.

Furthermore, the study can be utilized by other interns, social worker and professionals alike as a form of an in-service on Hispanics perspective of mental health and how to overcome some of the barriers when servicing this population. This qualitative study used individual interviews as a resource to shed some light on themes from the perspective of a variety of mental health providers. Individual interviews were audio recorded via a digital recorder then transcribed to a Word file. A variety of themes on the subject matter emerged from participants responses.

Using the lens of a community-based childhood obesity intervention, it is possible to examine the role of non-profit organizations in community development and to deconstruct the “community” in community-based research and identify the many competing interests within a community. This contextual understanding includes how the community is formed, how a community’s agenda is set, and who will complete the tasks outlined in that agenda. In applied anthropological settings and public health interventions that are community-based, it is essential to understand the context of community and which community (or communities) the researcher is working with to ensure that the data you collect reflects the community you wish to impact. The data collection for this dissertation occurred across phases. In Phase One, the focus was on collecting baseline data for a childhood obesity intervention using participant observation, unstructured interviews, and a community canvassing survey conducted with community volunteers who collected data going door-t- door. A midcourse review of results led to a shift in the research focus from the evaluation of a community-based intervention to an analysis of how community is conceptualized, with its various competing interests, in this particular context. To examine community membership, agenda setting, and how the community seeks to achieve its goals, this project utilized participant observation, unstructured interviewing, and semi-structured interviewing. Phase One data revealed that the community had limited interest in a childhood obesity intervention; additionally, local and county level data was ambiguous about the actual need for such an intervention. As a result, Phase Two data was collected to shed light on the role of community. There are three actors that make up “the community” at this project’s research site: 1) long-term residents, 2) short-term residents, and 3) the non-profit service providers, who work in the community. The extent to which the service providers are members of the community is somewhat contested, and honorary membership may be exchanged for other forms of capital. The agenda in the research setting appeared to be set by the local non-profit service providers, but data collection showed the importance of long-term residents (and, to a lesser extent, short-term residents) in guiding the focus of the non-profit agencies. To accomplish the goals of the agenda set in the community, a group of women emerged as key actors. In this dissertation I use the termed “Wonder Women” to connote an archetype of a resident in this community context; these women are residents who are committed to the agenda of the community and, through volunteering, are tested for their ability to work often exorbitant hours to achieve the goals of the community. The Wonder Women are worked until a breaking point, at which time they typically leave their post as key players in the community. This research not only contributes to identifying and operationalizing the concept of “communities” in community-based research but presents a new cultural phenomenon: the emergence of “Wonder Women.” Further research into this phenomenon is required to determine if they are occurring elsewhere and to what extent. Moreover, this dissertation informs the work of non-profit organizations working in the United States. The importance of true community participation and ways to prevent volunteer burnout are emphasized in the lessons learned from the research.

Over the past nine years, the School of Public Health and its three divisions of Primary Health Care (PHC), Public Health (PH) and Family Medicine (FM) in the Health Sciences Faculty at the University of Cape Town (UCT) have placed medical students in communities for eight-week rotations. During this time they undertake a community-based epidemiology project, followed by a health promotion intervention, in collaboration with community partners. The purpose of the research project was, primarily, to explore the benefits, if any, of this model of teaching for community stakeholders.

Paradigms in rehabilitation and disability service delivery in economically developed countries are currently being challenged and reviewed. An analysis of rehabilitation and disability literature arising from these countries, identified a number of issues of concern. Utilising a systems framework, adapted from the work of Urie Bronfenbrenner, the analysis indicated that certain aspects of current paradigms, may have adverse impacts on people with disabilities. It was determined that new paradigms should be explored. An analysis of current trends of relevance to the disability sector, identified a number of important directions, particularly the significance of the community paradigm. Community Based Rehabilitation (CBR), a disability service delivery approach which has arisen in developing countries, was proposed as an approach which was consistent with the identified trends and the community paradigm, and which constituted a constructive response to the identified concerns. It was noted however, that CBR lacked a strong research base and that fundamental principles had not been clearly elucidated. Based on the current literature, a detailed description and analysis of CBR was undertaken, and strategies, benefits and limitations of the approach were documented. The description of the parameters of CBR resulted in the elucidation of an evolutionary process, and the identification of key principles. It was proposed that the defining concept of CBR is ‘engagement’ between people with disabilities and their local communities. This concept was seen as having greater import, beyond the traditional contexts in which CBR has traditionally been employed. The possible application of CBR to economically developed countries was considered at a theoretical level. In order to explore the potential of the notion of engagement, two multi-phase, qualitative studies were devised and conducted in South East Queensland. The inductive phase of the research, which involved both studies, resulted in the development of a model consisting of five bipolar axes. This ‘model for enhancing engagement’, described the process by which engagement between users of human services (specifically people with disabilities) and their local communities might be maximised. The subsequent deductive phase of the research consisted of an exploration of the potential utility of this model through the two studies. Within the limitations of the qualitative design, the research indicated that the model had practical utility in the current context. In order to confirm concepts within the model, and consider its congruence with the field of CBR, a final verificatory phase was employed. This phase drew data from other sources to provide a degree of confirmation of the concepts within the model. The primary outcome of the research was the development of the ‘model for enhancing engagement’ between people with disabilities and their local communities. This model was described and its potential application was considered at a conceptual level. Three subsidiary outcomes were also seen as contributions of the research. First, a descriptive and conceptual framework, based on the work of Urie Bronfenbrenner, developed and applied in the current studies may have further utility. Second, a detailed analysis of the CBR literature resulted in the documentation of an evolutionary process in CBR, the identification of key principles, and the proposal of the notion of engagement. Third, a comprehensive, multi-phase, qualitative research process devised for the research which meets requirements for rigour and effective data presentation.

Infants who experience multiple risk factors, such as preterm birth, developmental delay, and low socioeconomic status, are at greater risk for mental health problems. Mental health interventions for infants typically target infants from high-risk groups, and there is strong evidence that some intervention programs for infants can prevent long-term negative outcomes and promote long-term positive outcomes. Despite emerging research and federal initiatives promoting early intervention, minimal research has examined community-based mental health services during infancy. Improving the effectiveness and efficiency of routine care requires close examination of current practices. The current study characterized current usual care practices in infant mental health through a survey of mental health providers. Provider, practice, and client characteristics, provider use of intervention strategies and intervention programs, and provider attitudes toward and knowledge of evidence-based practices are described. Study findings are discussed in the context of previous usual care research. Implications and directions for future research are discussed.

This thesis focuses on strengthening financial management of community-based organisations (CBOs) in Papua New Guinea. Through the use of qualitative, arts-based, and participatory action research methodologies, the findings indicated that CBOs harnessed social capital and community relations to successfully implement their work. However, their relational engagements and needs were often not adequately captured in financial and organisational plans, leading to tensions between CBOs and donors. The findings contribute to literature on CBOs, capacity building and development partnerships by highlighting that strengthening financial management requires reciprocal understanding, sharing of power and responsibility and building mutual accountability between CBOs and donors.

Abstract:
Introduction: Community coalitions with public health-related missions are formal, semi-permanent, action-oriented partnerships comprised of community members, representatives of government agencies, policymakers, and academic partners. Despite their potential to promote sustainable change, coalitions have had mixed success in effecting long-term improvements in community health. There is a need to assist them in developing strategies for improving and sustaining their functionality. The purpose of this study was to improve understanding of the elements of coalition success and sustainability that are vital to health-related community-based participatory research. Methods: Although the literature describes coalition functions for effecting sustainable programs and policies, most research reports on individual programs only, rather than looking at the breadth of community coalitions and the commonalities that contribute to their effectiveness. Semi-structured interviews (N = 42) with academic and community partners followed by a grounded theory analysis of the data address this gap and provide theoretical underpinnings of factors sustaining coalition effectiveness. Results: Seven domains emerged: (1) Characteristics related to coalition structure and processes; (2) Partner characteristics, e.g., diversity, patience, flexibility, expertise; (3) Community characteristics, e.g., capacity, ownership; (4) Partnership dynamics and synergy; (5) Tangible benefits; (6) Available resources, and; (7) Project characteristics. In all, 70 elements representing these categories influenced coalition effectiveness over time. Discussion: Sustained effectiveness means continual improvement, ongoing development of skills and structures to support positive change, and expanded program or policy activities that benefit stakeholders. Whereas community researchers have yet to reach consensus on the universal elements of sustainability, this study expands knowledge of the factors contributing to coalition effectiveness beyond initial project implementation.

Abstract:
This design-based research study explored whether Sense of Community was maintained while flexibility in the course was increased through an adoption of a unique blended learning model. Data collected in this study show a significant drop in the sense of connectedness score from a mean of 50.8 out of 66 to a mean of 39.68 in the first iteration. The score then began to gradually increase, reaching 50.65 in the third iteration. Results indicate that transitioning to a blended learning environment may be a suitable option to increase flexibility while maintaining a Sense of Community in a project-based course. Future research into specific aspects of course design such as maturity of design, age-level of participants, and context would further develop understanding in this area.

Abstract:
This paper describes a community coalition-university partnership to address health needs in an underserved US-Mexico border, community. For approximately 15 years, this coalition engaged in community-based participatory research with community organizations, state/local health departments, and the state's only accredited college of public health. Notable efforts include the systematic collection of health-relevant data 12 years apart and data that spawned numerous health promotion activities. The latter includes specific evidence-based chronic disease-preventive interventions, including one that is now disseminated and replicated in Latino communities in the US and Mexico, and policy-level changes. Survey data to evaluate changes in a range of health problems and needs, with a specific focus on those related to diabetes and access to healthcare issues-identified early on in the coalition as critical health problems affecting the community-are presented. Next steps for this community and lessons learned that may be applicable to other communities are discussed.

This thesis explores the complex discipline of community palliative care. Palliative patients suffer from a range of conditions, have complex, evolving clinical problems and receive care from a wide variety of health and social care professionals. Understanding these issues is central to effective service provision and maintaining continuity of care for patients and their carers. Current community palliative care provision in the UK is the result ofthe co-evolution of the emergent specialties of palliative and primary care over the past sixty years. A critical realist and practitioner based research approach is used in a multi-method study ofthe reasons for emergency admission ofpalliative patients into hospital and a qualitative study exploring the work and experiences of health care assistants prov~ding practical supportive care for palliative patients in their homes. Examining narratives of patients, carers and health professionals enables in depth exploration ofthe fundamental elements and contexts which define the inherent complexity ofthis area. Emergency admission ofpalliative patients represents a significant breach in continuity of care often resulting in disruption for both patient and health care services. The reasons for these admissions are many and complex. Relationships of power between different health professionals and between patients and health professionals exert a major influence on community palliative care provision and the process of admission. Health care assistants are relatively powerless and employed as basic carers, yet in community palliative care they undertake emotional labour, for which they draw on their personal resources. This aspect oftheir practice is crucial for patients and their carers; however it is largely unrecognised by formal health care services. Undertaking research in the context of a community palliative care team enables the development of a community of research practice. This provides an effective model for both developing an evidence base for community palliative care and the development of appropriate local services.

To date, traditional behavioral interventions have done little to reduce the prevalence and transmission of HIV among African American men who have sex with men (AAMSM), a highly at risk group. Some researchers theorize that the lack of success may be because these interventions do not address contextual factors among AAMSM. Community-based participatory research (CBPR) is one approach to research with the potential to lead to effective interventions in the future. CBPR is a collaborative, mixed-methods and multidisciplinary, approach to scientific inquiry, which is conducted with, and within, the community. The current study follows the CBPR approach to engage and develop a relationship with the African American communities in the Dallas/Fort Worth Metroplex. Contextual issues were discussed in order to identify emerging themes regarding HIV health related issues among AAMSM to provide the groundwork for continued CBPR research and future interventions with AAMSM in the Dallas/Fort Worth Metroplex. To accomplish this goal, researchers began the CBPR process by conducting interviews and focus groups with a sample of approximately 62 (34 from key informant interviews, 28 from focus groups [gender balanced]) AIDS service organization leaders and workers, advocates, medical doctors and community members with first-hand knowledge of HIV health issues in the AAMSM community. Transcripts of these interviews and focus groups were analyzed to identify emerging themes at the societal (religious doctrine, African American Culture, age-related norms and stigma), community (education, religious views/policy and community norms) and individual (disclosure, personal identity, sexual behavior/risk, accessing care and communication) levels. This data was used to create a holistic narrative report that will be used to direct the community advisory board (CAB) and guide future research and interventions.

While community gardens (CG) have emerged as a popular public health strategy to improve fruit and vegetable access and consumption, few studies provide evidence-based principles to inform the initiation and maintenance of CG. Grounded in Community-based Participatory Research and guided by the RE-AIM (Reach, Effectiveness, Adoption, Implementation, and Maintenance) framework, this mixed methods case study explores the potential public health impact of CG in the DRR. Six CG completed harvest logs during the 2011 season. Following the growing season, CG leader key informant interviews (N=6) and CG participant focus groups were conducted (N=21) using a semi-structured script, guided by RE-AIM dimensions. The five RE-AIM dimensions and associated components were used to develop a coding matrix and identify emerging themes. Three researchers coded the transcribed interviews using a deductive approach, which included coding raw data into meaning units. The six CG yielded 811 pounds of produce. The majority of focus group participants (95%) stated they would continue CG participation. From qualitative analysis, themes emerged such as increased the number of residents participating in CG, increased consumption of produce, key characteristics of successful CG leaders and locations, programs associated with CG, and adequate funding and resources necessary for maintenance. This study provides important insights to promote the potential public health impacts of CG in the DRR. Findings provide best-practice opportunities to promote the successful adoption, implementation, and maintenance of CG in similar communities.

Today’s emerging patient centered health movement is focused not on a specific condition or demographic, but rather on shifting the balance of power and enabling access to information to drive decision-making in healthcare. This takes place through electronic health records as well as more generalized sources. The uptake of social media is contributing to an innovation in patient centered healthcare: information and support on a global scale is coming not only from the formal healthcare system, but also within online social networks. Today, through computer-mediated interactions, patients are not only seeking information, they are curating and sharing information. Subsequently, patients are also creating information, establishing a novel ecosystem of engagement that has the potential to disrupt the current healthcare system.

This dissertation explores an online health community, BCSM (which stands for “breast cancer social media”), established using Twitter, a largely public and searchable social media platform. Drawing primarily from social network theory, disruptive innovation, and ecological systems theory, this research identifies essential characteristics within the community that may inform future development and support for patient centered healthcare. To conduct this research, a blended approach of netnography – referring to the approach of ethnography applied to the study of online cultures and communities – and in-depth interviews with BCSM participants were employed.

Data collected via interviews and tweets using the hashtag #bcsm provide evidence of clinical support, emotional support, information sharing, and knowledge translation. Underpinning this activity is the opportunity to associate not only with peers, but also with individuals of varying roles (including patients, providers, advocates, researchers, and caregivers). As evidenced by the data collected, educational opportunities flow in both directions.

This work contributes to the larger corpus of health-related literature in the identification and naming of a significant community element that has seen little focused attention: cross-peer engagement, a term used to highlight the interaction amongst individuals of differing status, ability, or rank. This research also documents the formation of microspurs, defined as relationships that form as a result of community participation. These come in many forms and range from expanding a personal support network to participation in federal policy work. Findings suggest that the future of healthcare will not revolve around hospitals and bounded systems. Instead, patients will demand an expanded set of entry points for health information sharing, knowledge transfer, condition management, and general support.

Abstract:
This commentary aims to convey my publications on community based care and Quality of Life (QOL), in Learning Difficulties (LD), and Dementia care. My rationale through this journey aims to critique my earlier observation, assessments, and interview findings and theory, in research care practices and policy. I further argue for reconceptualising care and QOL research methodology, underpinned by phenomenological theory and research focussing on service user lived experience and open dialogue between care disciplines. My contributions to knowledge in LD focuses on service user life experiences and varying QOL in private sector care homes, and observation of differences in quality of interaction in day-centre settings. Life experiences in LD residential care significantly changes over time in relationships; opportunities and freedom of choice, comparing poorly with the general population. The LD residential care research and LD day-centre observation is critiqued drawing on Social Role Valorisation (SRV) theory, and objective and subjective QOL. My Dementia research compares traditional, enhanced traditional and social models of care by setting. The social model provides more choice, control, recreation, privacy and social interaction, than traditional and enhanced-traditional models. Further work shows significant gender differences in interactions with staff in Dementia care. A lack of implementation of policy concerning a normal community based life in LD, is noted in residential and day-centre care. The social model setting's policies and practice in Dementia care, is noted as more 'enlightened' and showing more positive interaction. Gender disparity findings in care interactions in Dementia is salient to practice and policy intervention, and my subsequent training program relates directly to practice by addressing quality of interaction training. My epistemic 'shift' escapes the 'natural sciences' method by proposing discursive and phenomenological enquiry into lived experiences of service users, in a practice and policy context of trans-disciplinary discourse promoting relational and compassionate care.

Like many American Indian tribes, the Coquille Indian Tribe of Oregon has endured long struggles to preserve its cultural traditions despite the impacts of colonization. Now, advancing climate change poses additional threats to indigenous ways of life. In recent decades, the Coquille have archived historical documents and photographs as a means to protect and assert their tribal sovereignty. There has also been a surge in photography within the Tribe to document contemporary tribal activities. Community photography may be a useful tool for the purpose of asserting tribal culture and self-determination in the face of a changing climate. Photovoice, a type of community photography in which photographs are combined with oral descriptions, may be particularly well suited for tribal purposes. This collaboration explores the value of community photography in general, and photovoice in particular, when used by the Coquille Indian Tribe in the context of climate change.

This thesis investigates the situated knowing-in-practice of locally-based community organisations, and studies how this practice knowledge is translated and contested in inter-organisational relations in the community services field of practices. Despite participation in government-led consultation processes, community organisations express frustration that the resulting policies and plans inadequately take account of the contributions from their practice knowledge. The funding of locally-based community organisations is gradually diminishing in real terms and in the competitive tendering environment, large nationally-based organisations often attract the new funding sources. The concern of locally-based community organisations is that the apparent lack of understanding of their distinctive practice knowing is threatening their capacity to improve the well-being of local people and their communities. In this study, I work with practitioners, service participants and management committee members to present an account of their knowing-in-practice, its character and conditions of efficacy; and then investigate what happens when this local practice knowledge is translated into results-based accountability (RBA) planning with diverse organisations and institutions. This thesis analyses three points of observation: knowing in a community of practitioners; knowing in a community organisation and knowing in the community services field of practices. In choosing these points of observation, the inquiry explores some of the relations and intra-actions from the single organisation to the institutional at a time when state government bureaucracy has mandated that community organisations implement RBA to articulate outcomes that can be measured by performance indicators. A feminist, performative, relational practice-based approach employs participatory action research to achieve an enabling research experience for the participants. It aims to intervene strategically to enhance recognition of the distinctive contributions of community organisations’ practice knowledge. This thesis reconfigures understandings of the roles, contributions and accountabilities of locally-based community organisations. Observations of situated practices together with the accounts of workers and service participants demonstrate how community organisations facilitate service participants’ struggles over social justice. A new topology for rethinking social justice as processual and practice-based is developed. It demonstrates how these struggles are a dynamic complex of iteratively-enfolded practices of respect and recognition, redistribution and distributive justice, representation and participation, belonging and inclusion. The focus on the practising of social justice in this thesis offers an alternative to the neo-liberal discourse that positions community organisations as sub-contractors accountable to government for delivering measurable outputs, outcomes and efficiencies in specified service provision contracts. The study shows how knowing-in-practice in locally-based community organisations contests the representational conception of knowledge inextricably entangled with accountability and performance measurement apparatus such as RBA. Further, it suggests that practitioner and service participant contributions are marginalised and diminished in RBA through the privileging of knowledge that takes an ‘expert’, quantifiable and calculative form. Thus crucially, harnessing local practice knowing requires re-imagining and enacting knowledge spaces that assemble and take seriously all relevant stakeholder perspectives, diverse knowledges and methods.
