Abstract:
This chapter describes strategies for designing programme evaluation for communication skills training courses. It draws on the communication literature to demonstrate evaluation approaches including use of control groups, validated instruments, and observation methods. The logic model is introduced as a tool to ensure that evaluation is aligned with programme plans. Kirkpatrick’s four levels of programme evaluation are used to analyse training outcomes. Kirkpatrick’s Level 1 evaluates learners’ reaction to training. Level 2 evaluates changes in the learners’ attitudes, such as self-efficacy, knowledge and skills, including assessment by standardized patients. Level 3 assesses change in communication behaviours in the context of patient care, and Level 4 measures changes in patient outcomes, including patient satisfaction. Examples of each Kirkpatrick level are provided in this chapter, along with their strengths and limitations.

Abstract:
This chapter reviews contemporary approaches to teaching and assessing communication skills at the medical school level, explicating considerations for effective communication in the context of oncology and primary care. Primary approaches for training involve small-group teaching with either role play or interviews with simulated patients. The SEGUE Framework for Teaching and Assessing Communication Skills is a widely used conceptual framework that is flexible enough for diverse situations, yet sufficiently specific to afford practical guidance. Several aspects of communication in oncology and palliative care make communication particularly challenging and important: (i) physician role; (ii) illness severity; (iii) interdisciplinary teams; (iv) dying and death; (v) family involvement. Accordingly, this context presents predicaments requiring advanced communication skills, which augment the basic skills needed to accomplish tasks outlined in the SEGUE Framework. These include addressing: (i) bad news; (ii) prognosis/uncertainty; (iii) goals of care/palliative care; (iv) conflict management; (v) family meetings; (vi) teamwork.

Abstract:
In this chapter, we describe some of the challenges present in healthcare communication in Arab countries, including: disclosure of diagnosis; working with families; and language barriers. We then focus specifically on the efforts being made in the multicultural state of Qatar to improve communication skills in healthcare. We describe the curricular approach with medical students at Weill Cornell Medical College in Qatar, with examples of challenges faced surrounding gender and culture issues. We then describe the communication skills training programme for residents, fellows, and practising physicians at Hamad Medical Corporation, the public healthcare system. Challenges in Arab countries for healthcare communication result predominantly from multicultural populations and from cultural differences with the West. This chapter explores how these clinicians can face and transcend these challenges via communication skills training.

Abstract:
This chapter provides historical overview of communication skills knowledge in cancer care. Topics discussed include the role communication plays in classical accounts of medicine, medical education, the Lipkin model, American Academy on Communication in Healthcare, and Cancer Research Campaign Studies in the United Kingdom. Most of the history of communication skills knowledge and teaching derives from work and studies done in general medicine, or further afield, rather than in cancer care. This chapter includes such material because much of our communication knowledge and skill is generic, crosses specific applications and content areas like cancer care, and because the most useful conceptual frameworks and approaches began elsewhere and have only partially been rendered cancer specific.
