Several tools are available to map brain electrical activity. Clinical applications focus on epileptic activity, although electric source imaging (ESI) and electroencephalography-coupled functional magnetic resonance imaging (EEG–fMRI) are also used to investigate non-epileptic processes in healthy subjects. While positron-emission tomography (PET) reflects glucose metabolism, strongly linked with synaptic activity, and single-photon-emission computed tomography (SPECT) reflects blood flow, fMRI (BOLD) signals have a hemodynamic component that is a surrogate signal of neuronal (synaptic) activity. The exact interpretation of BOLD signals is not completely understood; even in unifocal epilepsy, more than one region of positive or negative BOLD is often observed. Co-registration of medical images is essential to answer clinical questions, particularly for presurgical epilepsy evaluations. Multimodal imaging can yield information about epileptic foci and underlying networks. Co-registering MRI, PET, SPECT, fMRI, and ESI (or magnetic source imaging) provides information to estimate the epileptogenic zone and can help optimize surgical results.

Abstract: Patients with head and neck cancer face multiple, often severe psychological and functional problems associated with the diagnosis and treatment of their disease including alterations in or loss of human voice, disruptions in speech production, and deterioration of swallowing ability. These functional changes significantly compromise a patient’s body image and can occur as a result of the disease as well as its primary treatment, whether surgery, radiation therapy (RT), or both. Adjuvant treatments that include RT generally increase functional deficits and associated body image disturbance. Therefore, a thorough understanding of the functional effects associated with the given treatment modality or combination of modalities must be conveyed adequately to the patient. A multidisciplinary team approach and ongoing communication among clinicians is critical to successful outcomes. This approach includes the delivery of psychosocial interventions to facilitate coping with functional changes during active treatment and into cancer survivorship.

Ideas matter. Constitutional jurisprudence decisions reflect overarching intellectual trends in society. The Buckley Constitution reflects the influence of modern individualistic libertarianism in contemporary American society. Some prominent authors have glimpsed more inclusive approaches to free speech. For example, renowned First Amendment theorist Alexander Meiklejohn sought to illustrate an inclusive approach to freedom of speech in his timeless metaphor of a town meeting. This chapter begins by outlining several wrong turns that the campaign finance cases have taken which render an inclusive approach impossible. One involves equating spending money with speaking. Spending money to speak is at most a combination of speech and action. Moreover, monetary limitations on political campaigning are similar to content neutral time, place, and manner restrictions. Some authors concerned with the distribution of speech rights have overtly offered a more egalitarian free speech approach. They include Jürgen Habermas, Bruce Ackerman, and Ronald Dworkin.

The core principle of hybrid imaging is based on the fact that it provides information beyond that achievable with either data set alone. This is attained through the combination and fusion of two datasets by which both modalities synergistically contribute to image information. Hybrid imaging is, thus, more powerful than the sum of its parts, yielding improved sensitivity and specificity. While datasets for integration may be obtained by a variety of imaging modalities, its merits are intuitively best exploited when combining anatomical and functional imaging, particularly in the setting of evaluation of coronary artery disease (CAD) as this combination allows a comprehensive assessment with regard to presence or absence of coronary atherosclerosis, the extent and severity of coronary plaques, and the haemodynamic relevance of stenosis. In clinical practice, the combination of CT coronary angiography (CCTA) with myocardial perfusion studies obtained by single-photon emission computed tomography (SPECT) and by positron emission tomography (PET) has been well established. Recent literature also reports on the feasibility of combining CCTA with cardiac magnetic resonance imaging. Finally, recent advances in CCTA and SPECT imaging have led to a substantial reduction of radiation exposure, now allowing for comprehensive morphological and functional diagnostic work-up by cardiac hybrid SPECT/CCTA imaging at low radiation dose exposures ranging below 5 mSv.

Psychosis has probably affected humans since the start of humanity itself, although the construct of schizophrenia is a relatively new phenomenon, dating back to the nineteenth century. Work by Emil Kraepelin and Eugen Bleuler helped consolidate ideas about psychotic disorders, setting the stage for both clinical care and neuroscience research in subsequent centuries. Phenomenologically, psychotic symptoms range from “positive” symptoms (delusions, hallucinations), to “negative” symptoms (avolition, affective blunting), to “disorganization” symptoms (disorganized speech and behavior), which all combine to produce functional deficits. Different psychotic disorders have different combinations of symptoms, which can combine with mood and anxiety symptoms to affect functioning problems in unique ways. These symptoms can be recognized fairly reliably in individuals. Understanding the inner experience of psychosis can help improve patient-centered care.

Abstract:
Fluid overload and chronic hypertension are the most common indications for diuretics. The diuretic response varies between different types and depends on underlying renal function. In patients with congestive heart failure, diuretics appear to reduce the risk of death and worsening heart failure compared with placebo, but their use in acute decompensated heart failure is questionable. Diuretics are also widely used in chronic kidney disease to prevent or control fluid overload, and treat hypertension. In acute kidney injury, there is no evidence that they improve renal function, speed up recovery, or change mortality. In patients with chronic liver disease and large volume ascites, paracentesis is more effective and associated with fewer adverse events than diuretic therapy, but maintenance treatment with diuretics is indicated to prevent recurrence of ascites. Mannitol has a role in liver patients with cerebral oedema and normal renal function. The use of diuretics in rhabdomyolysis is controversial and restricted to patients who are not fluid deplete. In conditions associated with resistant oedema (chronic kidney disease, congestive heart failure, chronic liver disease), combinations of diuretics with different modes of action may be necessary. Diuresis is easier to achieve with a continuous furosemide infusion compared with intermittent boluses, but there is no evidence of better outcomes. The role of combination therapy with albumin in patients with fluid overload and severe hypoalbuminaemia is uncertain with conflicting data.

This chapter examines how wartime and Red Scare repression expanded into a general cultural war on “Bolshevik” causes, individuals, and organizations targeted by the Anticommunist Spider Web during the 1920s. It considers a combination of federal, state, and local ordinances that effected political repression, suppressed free speech and economic liberty, and promoted Americanization in formal education settings led by the Ku Klux Klan and the American Legion. The chapter demonstrates how this climate of repression also led to the collapse of progressivism and impeded social welfare initiatives, gave rise to an amendment designed to make constitutional change virtually impossible, and resulted in the demise of the Roosevelt administration's Federal Theatre Project (FTP). It shows that the Spider Web members and their supporters created a repressive infrastructure of blacklists, witch hunts, loyalty oaths, and compulsory patriotism. In the process, the Spider Web strengthened its influence not only on the doctrine of anticommunism but also on the nation's political culture.

Xenophon’s morality contains surprisingly realistic elements, and his peculiar combination of ethics and politics highlights Thucydides’ own intense interest in the morality and piety of his characters as they struggle with the claims of natural necessity. Moral agency is shown to depend on morale. The perception of necessity sometimes destroys, sometimes raises morale.

This chapter examines the language of speeches given at International Mr. Leather, an annual competition among gay leathermen. After giving an overview of gay leather subculture and leather contests, the chapter examines the relationship between nationalist discourse and leatherman discourse during the International Mr. Leather contest. Leathermen involved in the contest combine language that indexes a radical “sexual outlaw” identity with traditional forms of nationalist discourses associated with their home country and with the idea of a global gay “leather nation.” The combination of these two distinct types of discourse is a form of indexical disjuncture that normalizes leather sexual practices by aligning them with conservative political stances. In addition, the use of nationalism serves in the construction of leather masculinity through links to military discourses.

This chapter illustrates how the creation of illusive categories and perceptions through the use of religious metaphor, among other rhetorical tools, culminated in the inevitable dichotomy in the way the world perceived Osama bin Laden. It thus conceptualizes bin Laden's discourse as a set of discursive illusions, in which the dual faces created of and by him turn out to be two sides of the same coin. Drawing on a combination of analytical tools, which include the historical approach, membership categorization analysis, and discourse as metaphor, the chapter analyzes a selection of speeches by Osama bin Laden and George W. Bush in an attempt to illustrate how both parties use almost identical forms of discourse in order to produce diametrically opposed conceptualizations of reality. It illustrates how Osama bin Laden played the role of both the evil terrorist and the brave champion of Islam through the creation of discursive illusions.

The book is devoted to the research and development of methods for combining computational structures for reconfigurable signature-based information protection tools for computer systems and networks in order to increase their efficiency. Network security tools based, among others, on such AI-based approaches as deep neural networking, despite the great progress shown in recent years, still suffer from nonzero recognition error probability. Even a low probability of such an error in a critical infrastructure can be disastrous. Therefore, signature-based recognition methods with their theoretically exact matching feature are still relevant when creating information security systems such as network intrusion detection systems, antivirus, anti-spam, and wormcontainment systems. The real time multi-pattern string matching task has been a major performance bottleneck in such systems. To speed up the recognition process, developers use a reconfigurable hardware platform based on FPGA devices. Such platform provides almost software flexibility and near-ASIC performance. The most important component of a signature-based information security system in terms of efficiency is the recognition module, in which the multipattern matching task is directly solved. It must not only check each byte of input data at speeds of tens and hundreds of gigabits/sec against hundreds of thousand or even millions patterns of signature database, but also change its structure every time a new signature appears or the operating conditions of the protected system change. As a result of the analysis of numerous examples of the development of reconfigurable information security systems, three most promising approaches to the construction of hardware circuits of recognition modules were identified, namely, content-addressable memory based on digital comparators, Bloom filter and Aho–Corasick finite automata. A method for fast quantification of components of recognition module and the entire system was proposed. The method makes it possible to exclude resource-intensive procedures for synthesizing digital circuits on FPGAs when building complex reconfigurable information security systems and their components. To improve the efficiency of the systems under study, structural-level combinational methods are proposed, which allow combining into single recognition device several matching schemes built on different approaches and their modifications, in such a way that their advantages are enhanced and disadvantages are eliminated. In order to achieve the maximum efficiency of combining methods, optimization methods are used. The methods of: parallel combining, sequential cascading and vertical junction have been formulated and investigated. The principle of multi-level combining of combining methods is also considered and researched. Algorithms for the implementation of the proposed combining methods have been developed. Software has been created that allows to conduct experiments with the developed methods and tools. Quantitative estimates are obtained for increasing the efficiency of constructing recognition modules as a result of using combination methods. The issue of optimization of reconfigurable devices presented in hardware description languages is considered. A modification of the method of affine transformations, which allows parallelizing such cycles that cannot be optimized by other methods, was presented. In order to facilitate the practical application of the developed methods and tools, a web service using high-performance computer technologies of grid and cloud computing was considered. The proposed methods to increase efficiency of matching procedure can also be used to solve important problems in other fields of science as data mining, analysis of DNA molecules, etc. Keywords: information security, signature, multi-pattern matching, FPGA, structural combining, efficiency, optimization, hardware description language.

Nuclear cardiology is generally considered a clinical phenomenon of the past four decades. However, the field has its roots in earlier times. This chapter focuses on these historical roots as they have evolved into the present era. The initial application of radioisotopes to cardiac studies occurred in the mid-1920s. Ventricular function was evaluated in the 1960s and 1970s by first pass and equilibrium techniques. Myocardial stress perfusion imaging was first performed using potassium-43 and exercise in 1973. Stress imaging rapidly evolved thereafter with new tracers (thallium-201 and technetium-labeled agents) and from planar to SPECT approaches. Perfusion imaging rapidly proved its value diagnostically and in assessing prognosis. Infarct imaging reached its peak use in the 1970s but is now no longer employed. Advances in hybrid imaging, combining CT with radionuclide imaging has recently allowed attenuation correction as well as providing the combination of anatomic and physiologic data. PET myocardial perfusion studies have recently become a standard approach for evaluating perfusion, absolute coronary blood flow and coronary reserve. PET FDG studies of cardiac sarcoidosis have recently been established as a new approach for defining myocardial inflammation. New SPECT systems provide high sensitivity, high resolution studies, allowing for radiation dose reduction and high quality imaging studies.

Nuclear cardiac imaging is solidly based on many branches of science and engineering, including nuclear, optical and mathematical physics, electrical and mechanical engineering, chemistry and biology. This chapter uses principles from these scientific fields to provide an understanding of both the signals used, and the imaging system that captures these signals. Nuclear cardiology’s signals are the x-rays or ?-rays photons emitted from a radioactive tracer and its imaging systems are either single-photon emission computed tomography (SPECT) or positron emission tomography (PET) cameras. This combination has met with remarkable success in clinical cardiology. This success is due to the combination of sophisticated electronic nuclear instruments with a highly specific and thus powerful signal. The signal is as important as or more important than the imaging system. There is a misconception that cardiac magnetic resonance (CMR) cardiac computed tomography (CCT) and echocardiography are superior to nuclear cardiology imaging because of their superior spatial resolution. Yet, in detecting perfusion defects what is really necessary is superior contrast resolution. It is this superior contrast resolution that allows us to differentiate between normal and hypoperfused myocardium facilitating the visual analysis of nuclear cardiology perfusion images. Because these objects are bright compared to the background radioactivity, computer algorithms have been developed that allow us to automatically and objectively process and quantify our images. This chapter explains many of the important scientific principles necessary to understand nuclear cardiology imaging in general, i.e., how these sophisticated imaging systems detect the radiation emitted from the radiotracers.

Authoritarian states try to present a positive image of themselves abroad. They invest in foreign-facing media, retain public relations firms, and showcase their successes to elite and popular foreign audiences. But there is also a darker side to these efforts. Authoritarian states try to obscure or censor bad news about their governments and often discredit their critics abroad. In extreme cases, authoritarian states intimidate, physically attack, or even murder their opponents overseas. This book is about how authoritarian states manage their image abroad using both “promotional” tactics of persuasion and “obstructive” tactics of repression. They adopt these practices to enhance their internal and external regime security or, put differently, to make their world safe for dictatorship. To substantiate these arguments, the book uses a diverse array of data, including fieldwork and author interviews, cross-national data on extraterritorial repression, examination of public relations filings with the United States government, analysis of authoritarian propaganda, media frequency analysis, and speeches and statements by authoritarian leaders. It builds a new data set—the Authoritarian Actions Abroad Database—that uses publicly available information to categorize nearly 1,200 instances in which authoritarian states repressed their critical exiles abroad, ranging from vague threats to confirmed assassinations. It also selects three cases for closer examination to understand in more detail how authoritarian states manage their image abroad using combinations of promotional and obstructive tactics: China, Rwanda, and North Korea. The result is a new way of thinking about the international dimensions of authoritarian politics.
