Il disturbo narcisistico di personalità negli ultimi anni sta assumendo sempre più rilevanza al di là della diagnosi categoriale espressa dalle correnti classificazioni come il DSM-5. Infatti, mentre la diagnosi categoriale pone in primo piano la grandiosità, trattare il narcisismo come dimensione permette di tener conto della sua eterogeneità, identificando sottotipi che presentano diversi sintomi, gravità e impostazione del trattamento. In questo lavoro trattiamo il tema del narcisismo vulnerabile, caratterizzato da un'estrema sensibilità al giudizio, tendenza a sperimentare vergogna e colpa, e da comportamenti caratterizzati da inibizione ed evitamento. L'analisi di un caso di una persona affetta da narcisismo vulnerabile permetterà di descrivere come affrontare gli stati mentali di colpa e depressione, i comportamenti di evitamento sociale e di inibizione dei desideri. L'intervento terapeutico viene descritto utilizzando i trascritti di alcune sedute individuali in cui vengono applicate le procedure terapeutiche della terapia metacognitiva interpersonale (TMI).

Abstract:
The aim of the study was to develop an orthopedic implant coating in the form of vancomycin-loaded collagen/hydroxyapatite layers (COLHA+V) that combine the ability to prevent bone infection with the ability to promote enhanced osseointegration. The ability to prevent bone infection was investigated employing a rat model that simulated the clinically relevant implant-related introduction of bacterial contamination to the bone during a surgical procedure using a clinical isolate of Staphylococcus epidermidis. The ability to enhance osseointegration was investigated employing a model of a minipig with terminated growth. Six weeks following implantation, the infected rat femurs treated with the implants without vancomycin (COLHA+S. epidermidis) exhibited the obvious destruction of cortical bone as evinced via a cortical bone porosity of up to 20% greater than that of the infected rat femurs treated with the implants containing vancomycin (COLHA+V+S. epidermidis) (3%) and the non-infected rat femurs (COLHA+V) (2%). The alteration of the bone structure of the infected COLHA+S. epidermidis group was further demonstrated by a 3% decrease in the average Ca/P molar ratio of the bone mineral. Finally, the determination of the concentration of vancomycin released into the blood stream indicated a negligible systemic load. Six months following implantation in the pigs, the quantified ratio of new bone indicated an improvement in osseointegration, with a two-fold bone ingrowth on the COLHA (47%) and COLHA+V (52%) compared to the control implants without a COLHA layer (27%). Therefore, it can be concluded that COLHA+V layers are able to significantly prevent the destruction of bone structure related to bacterial infection with a minimal systemic load and, simultaneously, enhance the rate of osseointegration.

Purpose: This article is devoted to scientific and theoretical research of modern system of principles for ad-ministrative procedural law in updated conditions of the Ukrainian state development, based on the needs of qualitative provision and realization of legal rights, freedoms and interests of natural and legal persons, in the sphere of public and legal relations. Main content: The author analyzed the administrative law regulations, and the doctrine of administrative proce-dural law as for determining nature and significance of the basic principles, which define the nature, structure, social orientation, and basic properties of legal regulation of this branch of law, and which on the other side create appropriate organizational and functional conditions for administrative procedural activities. Methodology: Materials and methods for research were based on the analysis of documentary sources. The basis is the dialectical method of cognition of the facts of social reality, on which the formal legal and compa-rative legal approaches are largely based. Conclusions: The principles of administrative procedural law can be divided into those that directly reflect the specifics and content of this branch of law, determine its features, purpose, objectives and intention, as well as administrative procedural principles, i.e., basic principles enshrined in administrative procedural law, which do not undergo significant changes, nor determine the nature and content of the activities of all subjects of administrative procedural legal relations.

Abstract:
There is a variety of anatomical deformities of the posterior pelvic compartment that associate functional disorders of the anorectum. The most common are: anterior rectocele, internal rectal prolapse (recto-anal intussusception), sigmoidcele and enterocele and overt rectal prolapse. There are a number of procedures employed in the treatment of mentioned disorders. One of them is laparoscopic prosthesis ventral colpo-rectopexy. Current evidence on this procedure is rather limited, because quality of studies is of low level and comparative trials are very few. Nevertheless, it is well documented that this procedure for mentioned is a safe procedure with very low morbidity, one death reported so far, and low conversion-to-open approach rate. Complications associated with the synthetic prosthesis (erosion or sepsis) are rare, and use of biological mesh does not seem to offer any advantage. Conversion is very low and usually as a result of previous pelvic surgery. The procedure is safe with low morbidity, short hospital stay and very low recurrence rate of overt rectal prolapse. Functional outcomes, namely constipation, obstructed defecation syndrome, incontinence and sexual dysfunction decrease in rates postoperatively.

Abstract:
Lo scopo del presente studio è stato quello di chiarire i fattori di rischio della rinoliquorrea a seguito di un approccio transfenoidale e di discuterne la prevenzione e il trattamento. Abbiamo revisionato retrospettivamente 474 casi consecutivi di adenoma ipofisario trattati con 485 procedure chirurgiche per via transfenoidale da Gennaio 2008 a Dicembre 2011 nel nostro dipartimento. Abbiamo analizzato l’incidenza di fuoriuscita di liquor cefalorachidiano intraoperatoriamente e nel postoperatorio, e la riuscita di varie strategie di riparazione. Abbiamo riscontrato fuoriuscita di liquor intraoperatoriamente in 85 casi (17.9%) e postoperatoriamente in 13 casi (2.7%). Sette dei 13 pazienti con rinoliquorrea postoperatoria non avevano mostrato fuoriuscita di liquor intraoperatoriamente; tre di questi pazienti avevano adenomi secernenti ADH. Dei rimanenti 6 pazienti con fuoriuscita di liquor sia intra che postoperatoria, 2 erano stati trattati per prolattinoma gigante e invasivo e 2 erano già stati sottoposti in passato a chirurgia trasnfenoidale. In 8 pazienti la fuoriuscita è stata risolta mediante puntura lombare, drenaggio lombare, riposo in posizione semi-reclinata o altri trattamenti conservativi. Due casi sono stati trattati mediante schiuma di gelatina e colla di fibrina utilizzando un approccio transfenoidale e due con grasso autologo e ricostruzione del pavimento della sella utilizzando un approccio transnasale endoscopico. Dopo essere stato sottoposto a due tentativi di riparazione per via transasale, un paziente è stato trattato con successo mediante un ulteriore drenaggio subaracnoideo. In conclusone le procedure che fanno uso di schiuma di gelatina, colla di fibrina e impianti di grasso autologo sono efficaci ai fini del trattamento della rinoliquorrea postoperatoria in pazienti sottoposti a chirurgia transfenoidale. Quando viene rilevata una perdita di liquido cefalorachidiano in corso di chirurgia transfenoidale, un’appropriata ricostruzione del pavimento della sella e un follow up a lungo termine sono necessari.

Abstract:
The article presents data on the improved technique of vaginal hysterectomy (GE) with the use of medications and technical support in case of genital prolapse. The objective: was to determine the clinical effectiveness of preventive measures, the improved technique of vaginal GE in case of complete loss of the uterus with the use of drugs and technical support. Patients and methods. The main group included 21 women with total uterine prolapse, who underwent vaginal GE without uterine appendages according to the procedure we improved, and in the control group – 17 women, who underwent vaginal GE with a standard procedure. In the two groups after the vaginal GE, anterior and posterior colpoperineoraphy was performed with levatoroplasty. Results. We have improved the technique of conducting vaginal GE with the use of medications (hydroparreparation of the solution with 0.4 mg of terlipressin) and technical support (use of the radiofrequency scalpel and argon-plasma tissue coagulation), according to our study, significantly reduces the likelihood of intraoperative complications and improves postoperative period with respect to reparative Processes and a decrease in the frequency of hemorrhagic and pyoinflammatory diseases after surgery Skogen intervention. Conclusion. The clinical study determined the effectiveness of a comprehensive surgical technique for the complete loss of the uterus and prevention of possible complications of surgical intervention. Abstract:
Oxidized starch, an additive used in paper manufacturing and products for construction industry, is usually produced using harmful oxidant, such as hypochlorites or periodates. In this study, a simple and efficient eco-friendly laboratory and industrial procedures for starch oxidation were developed. The procedure involves application of small amounts of more environmentally friendly oxidant, hydrogen peroxide, a novel special metal complex catalyst such as copper(II) citrate and copper(II) ricinoleate and biobased plasticizers. Optimization procedure, with respect to the quantity of hydrogen peroxide and temperature in the presence of iron(II) sulphate catalyst, was performed by using the response surface methodology. Compa-rative analysis of the use of the other catalysts that is copper(II) sulphate, copper(II) citrate and copper(II) ricinoleate, indicated copper(II) citrate as the catalyst of choice. Improvement of starch is achieved using three plasticizers: ricinoleic acid (RA), diisopropyl tartarate, as well as epoxidized soybean, linseed and sunflower oils. The effects of hydrogen peroxide and catalyst concentrations, as well as the reaction temperature in the presence of naturally based plasticizers on the physicochemical, thermal and morphological properties of oxidized starch are presented. According to the results obtained in initial experiments, the optimal industrial process is based on the use of copper(II) citrate (0.1 %) as a catalyst and RA (3 %) as a plasticizer.

Abstract:
Il lavoro di ricerca si focalizza sui modelli di giustizia e sulla devianza in adolescenza. Obiettivo è quello di mettere a fuoco alcuni elementi cruciali che intervengono come fattori protettivi e fattori di rischio nel comportamento adolescenziale. Nello specifico è indagato il ruolo svolto da variabili familiari, dalla giustizia procedurale, dalle emozioni e dal bisogno di riparazione. I contributi empirici utilizzano metodologie diverse: partendo da un lavoro esplorativo e passando attraverso la validazione di uno strumento di misurazione della giustizia procedurale il lavoro giunge alle analisi confermative su un modello teorico. Il primo studio qualitativo indaga le narrazioni e i significati attribuiti, da parte di adolescenti autori di reato, a tematiche familiari, emotive e di giustizia. Il secondo offre un contributo alla validazione italiana della Family Procedural Justice Scale (Fondacaro, Jackson, & Luescher, 2002), uno strumento self-report volto a misurare la giustizia procedurale in famiglia. Il terzo si occupa di proseguire nella direzione generata dagli studi precedenti e, attraverso analisi confermative sul modello teorico proposto, evidenzia il ruolo protettivo della percezione di giustizia procedurale in famiglia, della legittimità parentale, del bisogno di riparazione e delle emozioni di vergogna reintegrativa e colpa, mentre individua come fattore di rischio l’associazione a pari devianti.
The research focuses on the models of justice and delinquency in adolescence. The primary goal is to focus on crucial elements that act as protective and risk factors in adolescent behavior. Specifically, the role played by family variables, procedural justice, emotions and reparation is investigated. The empirical contributions use different methodologies: starting from an exploratory work and going through the validation of a procedural justice measuring instrument, the research comes to confirmatory analysis on a theoretical model. The first qualitative study explores the narratives and meanings attributed by adolescents offenders to family, emotional and justice issues. The second is a contribution to the Italian validation of the Family Procedural Justice Scale (Fondacaro, Jackson, & Luescher, 2002), a self-report instrument designed to measure procedural justice in the family. The third study continues in the direction generated by previous studies and, through confirmatory analysis on the proposed theoretical model, highlights the protective role of family procedural justice, parents legitimacy, need for reparation, reintegrative shame and guilt, whereas the association to deviant peers is identified as a risk factor.

Abstract:
La ricerca prende l’abbrivio dalla constatazione della crescente importanza acquisita, nel panorama delle fonti penalistiche, dal fenomeno dell’autonormazione: prodotto del diritto penale post-moderno consistente nell’autoimposizione, da parte dei destinatari stessi della norma, di precetti comportamentali in chiave criminal-preventiva. Oltre al ruolo ambivalente del principio di legalità penale (effetto e causa, al contempo, del fenomeno qui preso in considerazione), l’interesse del penalista per l’approfondimento scientifico del fenomeno è sollecitato dal potenziale che quest’ultimo rivela come alternativa (sostitutiva o integrata) rispetto al diritto penale. Il primo capitolo è dedicato alla ricostruzione delle cause che hanno dato origine al fenomeno, all’uopo ripartite in due macro-categorie: (i) le cause di ordine generale, per l’enucleazione delle quali è stata condotta una ricerca che spazia nelle materie sociologiche, economiche e giusfilosofiche; (ii) le cause di natura giuridica, che sono state investigate considerando sia le manifestazioni comuni all’intero ordinamento giuridico, sia quelle specifiche della penalistica, in cui la crisi del principio della riserva di legge e il declino del diritto penale classico assumono un’importanza cruciale. Nel secondo capitolo, il focus dell’analisi si concentra sulla dimensione strutturale del paradigma autonormativo per come emerso nelle sue principali manifestazioni e nelle concettualizzazioni teoriche maturate soprattutto grazie all’approfondimento riservato al fenomeno della Self-Regulation dagli studiosi di area anglosassone. La paradigmatica dell’autonormazione viene scrutinata tanto nelle sue singole componenti costitutive statiche, quanto nei suoi moti dinamici come strategia regolatoria all’interno dell’ordinamento. La ricerca si sposta nel terzo capitolo dalla struttura alla funzione, con l’obiettivo di ricavare i criteri di politica-criminale strumentali all’impiego dell’autonormazione nel sistema penale. A tal fine, sono state esplorate le possibili relazioni interordinamentali di raccordo tra sistemi autonormativi e ordinamento statale, applicando una metodologia mutuata dall’impostazione di Santi Romano ma ambientata sul terreno del diritto penale e delle sue alternative. Nel quarto capitolo l’indagine si rivolge verso i più eminenti esempi di autonormazione manifestatisi nell’ordinamento italiano: i modelli organizzativi ex D. Lgs. 231 del 2001; i piani per la prevenzione della corruzione nella P.A.; le linee guida medico-chirurgiche per lo svolgimento delle attività sanitaria. Oltre a una disamina ricognitiva della disciplina di questi sub-sistemi normativi, i tre banchi di prova vengono scandagliati in chiave struttural-funzionalistica alla luce dei criteri di analisi illustrati nel secondo capitolo e ricavati nel terzo. Il capitolo 5 chiude il lavoro proiettando i risultati delle ricerche sul piano della teoria del reato, per verificare quale impatto abbia/possa avere l’autonormazione sulla dogmatica. Dopo aver passato in rassegna le possibili ricadute sulle diverse categorie penalistiche, la chiosa finale valorizza il potenziale del diritto riflessivo come candidato ideale per la concretizzazione della clausola di extrema ratio in materia penale. L’uso dell’autonormazione come strumento alternativo rispetto al diritto penale viene ritenuto, infatti, il profilo applicativo più promettente e degno di essere ulteriormente esplorato.
One of the crucial challenges of Criminal Law in the new millennium is to deal with the complexity of contemporary society. The traditional approach based on the State monopoly on criminal matters keeps abreast no longer with the scientific-technological sophistication and the rate of changes in criminal behavior in the era of globalization. In this scenario, we witness the rise of Self-Regulation as an auxiliary tool of crime prevention, whose main goal is to fill the vacuum and to compensate for the rapid obsolescence of state legislation. Compliance Programs, Anti-Bribery Plans, Clinical Guidelines are some of the elements of a diverse constellation of cases in which preventive measures, behavioral rules, surveillance, and sanctions are issued and enforced by a legislator who coincides with the recipient, and which is often a private actor. Nevertheless, the ambivalence of Self-Regulation lies in the fact that – in the face of some positive externalities promised – this paradigm could jeopardize some of the fundamental principles of Criminal Law. The aim of this work is to provide a critical analysis of such phenomenon in order to verify the compatibility of Self-Regulation with the Rule of Law and to assess its efficacy in deterring and detecting misconducts.
