A previously unknown photograph of the old Emanuel Freud with his youngest daughter Bertha inspires an attempt to salvage what can be rescued of these lost lives and reminds us that history is always incomplete. Emanuel's appearances in Freud's dreams exemplify the dream-like aspects of the photographic image, which momentarily highlights particular aspects of the sitter while neglecting others. Examining photographs for documentation involves a reappraisal of the language of historical evidence.

Abstract:
The alkenone– p CO 2 methodology has been used to reconstruct the partial pressure of ancient atmospheric carbon dioxide ( p CO 2 ) for the past 45 million years of Earth's history (Middle Eocene to Pleistocene epochs). The present long-term CO 2 record is a composite of data from multiple ocean localities that express a wide range of oceanographic and algal growth conditions that potentially bias CO 2 results. In this study, we present a p CO 2 record spanning the past 40 million years from a single marine locality, Ocean Drilling Program Site 925 located in the western equatorial Atlantic Ocean. The trends and absolute values of our new CO 2 record site are broadly consistent with previously published multi-site alkenone–CO 2 results. However, new p CO 2 estimates for the Middle Miocene are notably higher than published records, with average p CO 2 concentrations in the range of 400–500 ppm. Our results are generally consistent with recent p CO 2 estimates based on boron isotope-pH data and stomatal index records, and suggest that CO 2 levels were highest during a period of global warmth associated with the Middle Miocene Climatic Optimum (17–14 million years ago, Ma), followed by a decline in CO 2 during the Middle Miocene Climate Transition (approx. 14 Ma). Several relationships remain contrary to expectations. For example, benthic foraminiferal δ 18 O records suggest a period of deglaciation and/or high-latitude warming during the latest Oligocene (27–23 Ma) that, based on our results, occurred concurrently with a long-term decrease in CO 2 levels. Additionally, a large positive δ 18 O excursion near the Oligocene–Miocene boundary (the Mi-1 event, approx. 23 Ma), assumed to represent a period of glacial advance and retreat on Antarctica, is difficult to explain by our CO 2 record alone given what is known of Antarctic ice sheet history and the strong hysteresis of the East Antarctic Ice Sheet once it has grown to continental dimensions. We also demonstrate that in the Neogene with low CO 2 levels, algal carbon concentrating mechanisms and spontaneous biocarbonate–CO 2 conversions are likely to play a more important role in algal carbon fixation, which provides a potential bias to the alkenone– p CO 2 method.

Abstract:
The abuse of methamphetamine, locally known as Yaba locally, has increased in Bangladesh recently. The study was designed to determine the proportion of co-morbidities, in terms of physical, psychiatric and other substances, and family history of substance use and other psychiatric disorders among methamphetamine abusers in Bangladesh.This was a cross-sectional study. Data were collected from the available medical documents of a private hospital dedicated for the management of substance abusers in Dhaka, Bangladesh. Information of the individuals admitted in the hospital during 1 January, 2014 to 31 December, 2015 due to substance related disorders having history of using methamphetamine within one month of hospitalization were enrolled in the study. Completed data of 115 individuals were taken and data analysis was performed using Statistical Package for Social Sciences (SPSS) version 24.Most (91.3%) of the respondents were male. Mean age of the respondents was 24.6 (±5.8) years. Half of the respondents (50.4%) belonged to the age group 21-30 years. Most (89.6%) of them resided in urban area and was Muslim (94.8%). Majority (52.2%) was unmarried. Regarding education status, majority (34.8%) completed graduation. About one-third (33.9%) were currently unemployed. All the methamphetamine users had used other substances. Among the other co-morbid substances, nicotine was the substance used by most (94.8%) respondents, followed by cannabinoids (56.5%) and opiates (38.3%). Among the respondents about one-third (33.9%) had current physical co-morbidities. Co-morbid psychiatric disorders were present among 29.6% of the respondents. Among the respondents, more than one-fourth (27.8%) had family history of substance use, 20.9% had family history of other psychiatric illnesses. All the methamphetamine users had used other substances. A substantial proportion of methamphetamine users had physical and other psychiatric comorbidities and family history of substance and other psychiatric disorders. Abstract Aims Tricuspid regurgitation (TR) was long forgotten until recent studies alerting on its prognostic impact. Cardiac output (CO) is the main objective of heart mechanics. We sought to compare clinical and echocardiographic data of patients with TR from inclusion to 1-year follow-up according to initial CO. Methods and results Patients with isolated secondary TR and left ventricular ejection fraction (LVEF) ≥40% were prospectively included. All patients had a clinical and echocardiographic evaluation at baseline and after 1 year. Echocardiographic measurements were centralized. The patients were partitioned according to their CO at baseline. The primary outcome was all-cause death. Ninety-five patients completed their follow-up. The majority of patients had normal CO (n = 64, 67.4%), whereas 16 (16.8%) patients had low-CO and 12 (12.6%) had high-CO. right ventricular function was worse in the low-CO group but with improvement at 1 year (30% increase in tricuspid annular plane systolic excursion). LVEF and global longitudinal strain were significantly worse in the low-CO group. Overall, 18 (19%) patients died during follow-up, of which 10 (55%) patients had abnormal CO. There was a U-shaped association between CO and mortality. Normal CO patients had significantly better survival (87.5% vs. 62.5% and 66.67%) in the low- and high-CO groups, respectively, even after adjustment (heart rate 2.23 for the low-CO group and 9.08 for high-CO group; P = 0.0174). Conclusion Significant isolated secondary TR was associated with 19% of mortality. It is also associated with higher long-term mortality if CO is abnormal, suggesting a possible role for evaluating better and selecting patients for intervention.

AbstractIn this paper I first introduce Tomasello’s notion of thought and his account of its emergence and development through differentiation, arguing that it calls into question the theory bias of the philosophical tradition on thought as well as its frequent atomism. I then raise some worries that he may be overextending the concept of thought, arguing that we should recognize an area of intentionality intermediate between action and perception on the one hand and thought on the other. After that I argue that the co-operative nature of humans is reflected in the very structure of their intentionality and thought: in co-operative modes such as the mode of joint attention and action and the we-mode, they experience and represent others as co-subjects of joint relations to situations in the world rather than as mere objects. In conclusion, I briefly comment on what Tomasello refers to as one of two big open questions in the theory of collective intentionality, namely that of the irreducibility of jointness.

America's most northern history theme park has been located in Fairbanks, Alaska since 1967. This article focuses on the evolution of the Alaskaland/Pioneer Park: from a tourist attraction where Alaskan traditions of progress and boosterism ruled into a community park with a sincere concern for preserving the local past. Due to its origins, and in spite of decades of controversies, the park became an excellent example of ““partnership”” between public and private sectors which determine the park's profile and destination. The result is a popularized presentation of Fairbanks' early twentieth-century pioneer experience where historic authenticity and commercial activities co-exist harmoniously.

This article considers state-funded films in contemporary Vietnam and the legacy of the Republic of Vietnam (South Vietnam), which fell to communist forces in 1975. From a close reading of films produced on the thirtieth anniversary of the end of the war, the article deciphers complicated meanings about national identity, history, and gender. In this new political economic context, the possibilities for remembering the southern regime—including its people and veterans—remains open and closed. Through the framework of heteroglossia of history, the co-presence of competing viewpoints within cinematic texts points to the complexity of an ever-changing Vietnam.

History taking is crucial in the diagnostic process for vestibular disorders. To facilitate the process, systems such as TiTrATE, SO STONED, and DISCOHAT have been used to describe the different paradigms; together, they address the most important aspects of history taking, viz. time course, triggers, and accompanying symptoms. However, multiple (vestibular) disorders may co-occur in the same patient. This complicates history taking, since the time course, triggers, and accompanying symptoms can vary, depending on the disorder. History taking can, therefore, be improved by addressing the important aspects of each co-occurring vestibular disorder separately. The aim of this document is to describe a 4-step approach for improving history taking in patients with non-acute vestibular symptoms, by guiding the clinician and the patient through the history taking process. It involves a systematic approach that explicitly identifies all co-occurring vestibular disorders in the same patient, and which addresses each of these vestibular disorders separately. The four steps are: (1) describing any attack(s) of vertigo and/or dizziness; (2) describing any chronic vestibular symptoms; (3) screening for functional, psychological, and psychiatric co-morbidity; (4) establishing a comprehensive diagnosis, including all possible co-occurring (vestibular) disorders. In addition, pearls and pitfalls will be discussed separately for each step.

Abstract:
This article details Yaseen Aslam’s experience of organizing at Uber. Yaseen is the National General Secretary of UPHD (United Private Hire Drivers), a branch of the IWGB (Independent Workers Union of Great Britain). He is a co-claimant, with James Farrar, in the employment rights court cases against Uber in the UK. The article is the outcome of co-writing with Jamie Woodcock, presenting Yaseen’s first-person perspective. It builds on the method of workers’ inquiry and writing between workers and academics.

All museums do research into their collections. Some maritime museums, however, have started co-operations with adjacent universities and formed research bodies that initiated multidisciplinary research projects, as well as inaugurating PhD programmes and joining the transnational academic discourse. These initiatives are dependent on active promotion by research directors. Once these directors retire, their research initiatives are often jeopardised.

Abstract:
We report a case of a man with Asperger's syndrome, paraphilic behaviour and convictions for sexual offences. We describe his assessment within a secure mental health setting to determine issues of diagnosis, treatment and risk. We also highlight the difficulty in reducing the risk of further offending because of the apparent ineffectiveness of interventions for the small group with Asperger's syndrome and an offending history. Consequently, they are likely to face long periods in institutional settings.
