Abstract:
A survey was conducted among specialized users in order to establish the level of knowledge and diffusion of four informative tools: Cancerlit, PDQ, Clinprot, Cancergram. A questionnaire was distributed to 975 graduate researchers working in four national cancer research institutes in the north (2), center (1) and south (1) of Italy, as well as to the students of the Postgraduate School of Oncology (Oncology 2nd) of the “La Sapienza” University of Rome. Respondents were 486 (49.8%): only 167 (34.4%) declared to know Cancerlit, 56 (11.5%) PDQ, 10 (2%) Clinprot and 39 (8%) Cancergrams. Moreover, there is a time gap of nearly 6 years between the availability on-line of these tools and the information of its existence by the users. Most of the respondents were in favor of inserting courses on the use of information tools into the University curriculum and of training in the use of the tools.

Abstract:
Background. Colorectal cancer (CRC) is one of the most common cancers in the world, identification of biomarkers for early detection of CRC represents a relevant target. The present study aims to determine serum peptidome patterns for CRC diagnosis.Methods. The present work focused on serum proteomic analysis of 32 health volunteers and 38 CRC by ClinProt Kit combined with mass spectrometry. This approach allowed the construction of a peptide patterns able to differentiate the studied populations. An independent group of serum (including 33 health volunteers, 34 CRC, 16 colorectal adenoma, 36 esophageal carcinoma, and 31 gastric carcinoma samples) was used to verify the diagnostic and differential diagnostic capability of the peptidome patterns blindly. An immunoassay method was used to determine serum CEA of CRC and controls.Results. A quick classifier algorithm was used to construct the peptidome patterns for identification of CRC from controls. Two of the identified peaks at m/z 741 and 7772 were used to construct peptidome patterns, achieving an accuracy close to 100% (>CEA,P<0.05). Furthermore, the peptidome patterns could differentiate validation group with high accuracy.Conclusions. These results suggest that the ClinProt Kit combined with mass spectrometry yields significantly higher accuracy for the diagnosis and differential diagnosis of CRC.

Abstract:
O carcinoma epidermóide de pênis (CEP) representa 95% das neoplasias penianas e afeta quase sempre pacientes não circuncidados estando muitas vezes associado à falta de higiene local adequada e à fimose. No Brasil a sua incidência é de 2,7 % porém em algumas áreas do país pode chegar a 17% dos casos diagnosticados por ano. O tumor pode ocorrer em qualquer parte do órgão sexual masculino e o tipo de estadiamento empregado é controverso. A classificação de Broders é a mais utilizada. Estudos sugerem a relação entre o desenvolvimento do carcinoma de pênis com a infecção por HPV (Papiloma Vírus Humano). O método de avaliação dos linfonodos inguinais permanece controverso sendo difícil a diferenciação entre linfadenomegalia inflamatória reacional e metastática. O exame físico não é um preditor confiável do comprometimento linfonodal pois pacientes com linfonodos palpáveis podem não apresentar metástases. Há poucas publicações sobre os mecanismos moleculares envolvidos na gênese e progressão do CEP. Apesar de vários marcadores terem sido avaliados, atualmente a aplicação clínica destes é limitada. A maior parte dos marcadores estudados requer procedimentos invasivos para obtenção do tecido tumoral. Existe a necessidade de encontrar através de uma técnica pouco invasiva marcadores tumorais circulantes capazes de diferenciar portadores de CEP com e sem envolvimento metastático. Neste tipo de neoplasia, a descoberta de biomarcadores que avaliem o prognóstico é relevante, pois o exame físico não é um indicador confiável do comprometimento linfonodal e da sobrevida.Os objetivos foram 1) revisar e discutir a epidemiologia, a etiologia, os diversos tipos de abordagem cirúrgica e as controvérsias no tratamento cirúrgico do câncer de pênis 2) investigar através da plataforma ClinProt/ MALDI / TOF a presença de marcadores plasmáticos capazes de discriminar indivíduos saudáveis de pacientes afetados por carcinoma epidermóide de pênis (CEP) 3) avaliar a importância destes marcadores na evolução da doença. Foram coletados e analisados pela plataforma ClinProt / MALDI / TOF o plasma de 36 indivíduos saudáveis e 25 pacientes com CEP invasivo, submetidos a tratamento cirúrgico entre junho de 2010 e junho de 2011, nos serviços de urologia do Instituto Nacional de Câncer e do Hospital Mário Kröeff (Rio de Janeiro). Nossos resultados apontaram para um conjunto de dois peptídeos (A = m / z 1897,22 + -9 Da e B = m / z 2021,99 + -9 Da) que foram capazes de diferenciar pacientes com CEP de indivíduos controles. Esses peptídeos foram posteriormente identificados como fragmentos C3 e C4 A/B do sistema complemento. A validação cruzada, utilizando toda casuística apresentou 62,5% e 86,76% de sensibilidade e de especificidade, respectivamente, com uma alta sensibilidade (100%) e especificidade (97%) nos pacientes que morreram pela doença. Além disso, os pacientes com envolvimento ganglionar obtiveram uma sensibilidade e uma especificidade de 80 % e 97%, respectivamente. Ficou demonstrado que à medida que a doença progride mais subexpressos está o conjunto de peptídeos quando comparados com indivíduos saudáveis. Estes resultados podem ser úteis como ferramentas para a avaliação do prognóstico destes pacientes.
Squamous cell carcinoma of the penis (SSCP) represents 95% of penile cancers. It affects mostly uncircumcised patients and is often associated with lack of adequate local hygiene and phimosis. In Brazil, the incidence is 2.7% but in some areas of the country can reach 17% of diagnosed cases of cancer per year. The tumor can occur in any part of the sexual organ and the type of staging to be used is controversial. The Broders classification is more often used to classify tumors. Studies suggest the relationship between the development of penile carcinoma and HPV infection (Human Papilloma Virus). The evaluation method of inguinal lymph nodes remains controversial and it is difficult to differentiate inflammatory reaction from metastatic lymphadenopathy. Physical examination is not a reliable predictor of lymph node involvement since patients with palpable lymph nodes can not present metastases. There are few publications about the molecular mechanisms involved in the genesis and progression of the SSCP. Although several markers have been evaluated, currently the clinical application of these is limited. Most of the markers studied require invasive procedures for obtaining tumor tissue. There is a need to find through a minimally invasive technique circulating tumor markers able to differentiate SSCP patients with and without metastatic involvement. In this type ofmalignancythediscovery of biomarkers that assessthe prognosisis relevant since physical examination is not a reliable predictor oflymph node involvementand survival. The objectives of this study were: 1) to review and discuss the epidemiology, etiology, different types of surgical approach and controversies in the surgical treatment of penile cancer 2)to investigate via the platform ClinProt / MALDI / TOF presence of plasma markers able to discriminate healthy subjects from patients affected by squamous cell carcinoma of the penis (SCCP) 3) to evaluate the importance of these markers in disease progression. Between June 2010 and June 2011, plasma samples from 36 healthy subjects and 25 patients with penile carcinoma who underwent surgical treatment in the UrologyServicesofNational Cancer InstituteandMarioKröeffHospital were collected and analyzed by the ClinProt/MALDI/TOF platform. Our results found a cluster of 2 peptides (A=m/z 1897.22 +-9 Da and B=m/z 2021.99 +-9 Da that was able to discriminate patients from controls subjects. These peptides were further identified as C3 and C4 A/B fragments from complement system. Cross validation analysis using the whole casuistic showed 62.5% and 86.76% of sensitivity and specificity, respectively with a very high sensitivity (100%) and specificity (97%) for SCCP patients that have died by disease. Moreover, patients with lymph node involvement present a sensitivity and specificity of 80% and 97%, respectively. The results showed that as the disease progresses more under express are the cluster comparing with healthy subjects. These results may be useful as prognostic toll.

The PhD thesis is focused on the evaluation of different Mass Spectrometry approaches for the study of the proteome of biological fluids and tissues. In detail, the ClinProt technology has been applied to amniotic fluids and urines respectively, to evaluate potential biomarkers for the preterm premature rupture of the membranes (pPROM) and to invetsigate molecular mechanisms of kidney adaptation to hypobaric hypoxia conditions at high and very high altitude. MALDI Imaging Mass Spectrometry (IMS) has been applied for the study of tissues. In detail, this part of the work evaluated the use of the detergents to enhance sensitivity and number of peaks detected for protein IMS analysis.

Abstract:
Il progetto di questa ricerca è stato rivolto allo studio e alla caratterizzazione del peptidoma serico di soggetti sani e pazienti con carcinoma renale, così come alla diagnosi differenziale delle forme di tumore renale maligno da lesioni benigne. A questo proposito sono stati purificati, mediante tecnica ClinProt che utilizza microparticelle magnetiche funzionalizzate, il siero di un ampio numero di pazienti affetti da ccRCC e soggetti controllo, estendendo lo studio anche a pazienti affetti da non-ccRCC. Le analisi di profiling peptidico sono state condotte mediante SM MALDI-TOF seguita da elaborazione dei profili spettrali per all’individuazione di clusters con potenziale diagnostico. Inoltre si è cercato di identificare proteine/peptidi con alterata espressione serica. A questo scopo sono state effettuate analisi mediante tecnologia nLC-ESI MS/MS e MALDI-TOF MS/MS. I risultati del "profiling" tramite MALDI-TOF sono stati verificati utilizzando un approccio proteomico diverso basato su una quantificazione relativa label-free in nLC-ESI MS/MS.

O grande desafio está em encontrar uma substância que elimine efetivamente a sensação dolorosa não permitindo sua recidiva. O objetivo deste estudo in vitro foi determinar a eficácia do Clinpro XT® na redução da permeabilidade dentinária e avaliar a resistência deste produto a desafios ácidos. O grau de obliteração dos túbulos dentinários e a rugosidade superficial após aplicação do produto, também foram avaliados. Foram utilizados 65 terceiros molares humanos íntegros para a preparação de 65 amostras que foram ligadas a um sistema de pressão hidráulico para mensurar a permeabilidade dentinária após os seguintes passos: (1) preparação da amostra; (2) tratamento com ácido fosfórico a 37% durante 30 segundos; (3) aplicação do Clinpro XT®; (4) primeiro; (5) segundo e (6) terceiro desafios ácidos, respectivamente. As amostras foram divididas aleatoriamente em 5 grupos (n = 13) de acordo com as diferentes soluções ácidas: refrigerante a base de cola, suco natural de limão, vinagre de vinho, vinho branco e energético. 10 terceiros molares adicionais foram utilizados para a preparação de 20 amostras para análise do grau de obliteração dos túbulos dentinários, através da microscopia eletrônica de varredura, e, rugosidade superficial, através de um rugosímetro. Pode-se concluir que Clinpro XT® foi eficaz na redução da permeabilidade dentinária e esse efeito se manteve após três desafios com diferentes substâncias ácidas. Além disso, o produto testado foi capaz de promover a obliteração dos túbulos dentinários e a redução da rugosidade superficial.

Abstract:
Objetivo: Apresentar uma revisão da literatura sobe os diferentes tratamentos restauradores em pacientes com Amelogénese Imperfeita. Materiais e Métodos: Realizou-se uma pesquisa bibliográfica, recorrendo à base de dados PubMed. Foram incluídos artigos publicados nos últimos 10 anos, nas línguas inglesa, portuguesa, espanhol ou francesa. A pesquisa resultou em 42 artigos. Resultados: Encontra-se descrita uma diversidade de tratamentos para os pacientes atingidos por Amelogénese Imperfeita, no entanto, não existem protocolos padrão por falta de evidência cientifíca. Conclusão: São necessários mais estudos clínicos randomizados que se foquem no tipo e severidade de Amelogénese Imperfeita para determinar a melhor intervenção, tal como, mais pesquisas para a elaboração de um diagnóstico mais preciso.
Objective: Present a literature review on the diferent restorative treatments Materials and Methods: Bibliographic research was undertaken using the PubMed database. Articles published within the last 10 years written in English, Portuguese, Spanish or French have been included. The literature review resulted in the analysis of 42 articles. Results: A range of treatments exist for patients affected by Amelogenesis Imperfecta, however, there is an absence of standard guidelines due to lack of evidence. Conclusion: More randomized clinical trials are needed focusing on the type and severity of Imperfect Amelogenesis to determine the best intervention, as well as more research to improve the accuracy of diagnosis.

Abstract:
On human dental enamel, the remineralisation efficiency of a subsurface enamel defect was evaluated using powder extracts from three marine skeletal species: Crassostrea madrasensis (oyster shell), Perna viridis (mussel shell), and Penaeus monodon (shrimp shell).Methods: The study included 60 freshly extracted human third molar teeth that were sectioned at the cement-enamel junction (CEJ). Later, each tooth coronal region was separated into two halves of enamel blocks, yielding 120 blocks that were exposed to the demineralization technique and categorised as follows: Subsurface demineralization + oyster shell powder extract form Group I. Group II: demineralization of the subsurface + mussel shell application Subsurface demineralization + shrimp shell powder extract (Group III). Group IV: subsurface demineralization plus clinpro application, followed by thirty days in artificial saliva. The intact enamel samples served as a positive control, whereas demineralized enamel samples served as a negative control. Using X-ray fluorescence spectroscopy, micro hardness testing, and energy dispersive x-ray spectroscopy,samples were evaluated for remineralisationThe samples were evaluated using X-ray fluorescence spectroscopy, energy dispersive x-ray spectroscopy and micro hardness testing for atomic analysis & surface microhardness. One-way ANOVA and the Tukey – Kramer multiple comparison tests were used for statistical analysis.
