Background: Physiotherapists work in a health care situation of constant rising complexity and rapid change and demands for accountability, of careful examination from both internal and external sources. New generations of emerging physiotherapists require more than a solid foundation of clinical skills. They require a proper educational foundation that is reinforced with good attributes, attitude and skills that will help them in building their profession as well as their professional practice. Clinical Teaching is very important in preparing Physiotherapists for working with patients and it has long been recognized as a necessary part of Physiotherapy education. Context & Purpose of the study: The present study included 200 participants from 1st year there were around 52 students with average score of 106.63, 2nd year with around 55 students with average score of 106.13, 3rd year with around 22 students with average score of 107.9, 4th year with 71 students with average score of 104.39. Results: The result shows mean value of 105.75 and SD value of 12.80654. The result concluded that the Perception of Undergraduate Physiotherapy Students towards Clinical Attributes and Clinical Teaching is Good according to the gradings. Conclusion: The present study concluded that the Perception of Undergraduate Physiotherapy Students of Dr. A. P. J. Abdul Kalam College of Physiotherapy is Good. Overall, Undergraduate Physiotherapy Students rated the clinical teaching attributes of their teachers highly, and they were also satisfied and felt positively challenged during their clinical rotations. Abstract:
Objective: Level of plasma cholinesterase and its association with clinical manifestations. Methods: This is a prospective study and observation study of 50 cases of organo phosphorus poisoning compound admitted in casualty and intensive care department. In present study we were evaluated plasma cholinesterase levels and correlate with clinical symptomatology in organo phosphorus poisoning, also included patient requiring ventilator support in respiratory failure patients. Prior approval for the study and the protocol was obtained from the institution ethical committee. In present study, serum cholinesterase estimation was repeated on after 6 hours, 12 hours, 24 hours, 48 hours, 96 hours and 7th day of stay in intensive care unit. The serum cholinesterase activity was measured by Genx cholinesterase butyryl thiocholinemethod (quantitative determination of cholinesterase in human serum). Conclusion: In present study 50 cases of organophosphorus compound poisoning admitted to our tertiary care centre. Commonest age group 21 to 30 years and 31 to 40 years. Serum cholinesterase is a useful marker for predicting clinical outcome in op poisoning as marked reductions are associated with increased need for ventilator support, atropine requirement, duration of hospital stay and outcome. Serum cholinesterase levels correlated well with clinical manifestation, severity of poisoning, ventilator support required or not and outcome.

A 35-year-old patient, a native of Uzbekistan, had shortness of breath and swelling of the lower extremities at the 35th week of the fourth pregnancy. She gave birth at the 39th gestational week naturally. Symptoms progressed within 3 months after delivery: ascites and hydrothorax appeared. According to echocardiography, dilation of the heart chambers was detected, the left ventricular ejection fraction was 30%. A diagnosis of peripartal cardiomyopathy (CMP) complicated by CHF third degree was established. During 10 days of treatment, the manifestations of HF decreased significantly.

Abstract:
The endocannabinoid system (ECS), discovered in the 1990s, is a system involved with maintaining cellular homoeostasis by down-regulating the damaging inflammatory responses and upregulating regenerative processes. Tetrahydrocannabinol (THC), Cannabidiol (CBD), tetrahydrocannabivarin (THCV) and cannabidivarin (CBDV) are all phytocannabinoids that have novel therapeutic effects on hair growth through the ECS receptors on hair follicles. These cannabinoids are fat-soluble and poorly absorbed past the epidermis, but topical application easily reaches hair follicles where CBDV, THCV and CBD act as partial or full CB1 antagonists and agonists of vanilloid receptor-1 (TRPV1) and vanilloid receptor-4 (TRPV4). All these ECS receptors relate to hair follicle function. THC on the other hand has the opposite effects decreasing hair shaft elongation and matrix production. A summary of the pre-clinical and clinical cannabinoid research is reviewed. The studies confirm that for androgenetic alopecia (AGA) the method of action is different from and synergistic with current hair regrowth therapies. Blocking the CB1 receptor on the hair follicle has been shown to result in hair shaft elongation and matrix production via keratinocytes, in addition, the hair follicle cycle (anagen, catagen, and telogen phases) is controlled by TRPV1. The effects of CBD on hair growth are dose dependent and higher doses may result in premature entry into the catagen phase via a different receptor known as TRPV4. CBD has also been shown to increase Wnt signaling, which causes dermal progenitor cells to differentiate into new hair follicles and maintains anagen phase of the hair cycle. Two recent six-month duration clinical trials of adults with androgenetic alopecia (AGA) have revealed an average 93.5% increased hair count with CBD alone, and 164% increased hair count with hemp extract high in CBD, THCV, CBDV and menthol. A current study is underway to look at punch biopsies of the areas of hair regrowth in patients treated with a combination of CBD, THCV isolate, menthol and caffeine. Another study is underway looking at using hexahydrocannabinol (HHC) a hydrogenated version of THC to decrease facial hair growth as a cosmetic effect

Objective: The Bilancio Logopedico Breve (BLB)-swallowing subtest is a non-validated baseline Italian protocol widely used for clinical swallow examination (CSE). The aim of the study was to critically analyze the clinical utility of the items of BLB based on 5-years clinical experience in an intensive rehabilitation facility. Methods: In this retrospective study, 407 patients with different diagnoses were included. Clinical data, BLB assessment at admittance and discharge, signs of dysphagia during fiberoptic endoscopic evaluation of swallowing (FEES) and diet recommendations were extracted. A univariate analysis was performed to investigate the items associated with the decision of nutrition method and diet type, the presence of penetration or aspiration on FEES, and an improvement in diet type during the hospital stay. Results: Oral motor skills, dentition, voluntary cough, gag reflex, swallow trigger and signs of aspiration with liquid and/ or puree consistencies items showed significant associations with the decision of oral nutrition and/or to the type of oral diet, and with an evolution in the diet type. Gag reflex, swallow trigger and signs of aspiration with liquid and/or puree consistencies were significantly associated with penetration or aspiration at FEES. Pathological reflexes, lip sealing during swallowing, laryngeal excursion and oral residue items displayed poor associations with the clinical outcomes considered. Conclusion: The present findings support the use of BLB in the clinical practice as a guideline for CSE despite its limitations. Results should guide the revision of the protocol, preliminary to an advisable validation process of the tool.

Treatment options for mild to moderate COVID-19 is limited. N-acetylcysteine and bromhexine have antiviral activity and show potential as treatment options against SARS-CoV-2 infections. This study evaluates the in vitro antiviral effect of bromhexine (BMX) for SARS-CoV-2 and determines the efficacy of treatment with BMX in combination with N-acetylcysteine (NAC) to reduce clinical scores in patients with mild to moderate COVID-19. Upon evidence from preclinical studies, a single center randomized trial of BMX + NAC (ClinicalTrials.gov Identifier: NCT04928495) with 420 participants in total took place in Fortaleza, CE, Brazil. Out of the 420 participants 140 received placebo, 140 received NAC alone, and 140 received NAC + BMX. Patients were monitored for 10-14 days, where physicians recorded all signs and symptoms reported. Nasopharyngeal swabs and blood samples were collected for SARS-CoV-2 RNA testing during the first visit, as well as 3 and 10 days after. Blood samples were collected at first visit and 10 days after for immuno-inflammatory biomarkers measurements. Treatment with NAC+BMX reduced clinical scores and symptoms when compared to placebo group (2/26; 8% vs 7/18; 39%; p < 0.05). Fever (≥37.8°C) was reduced by NAC + BMX treatment when compared to treatment with NAC alone and placebo. This study was limited by a largely vaccinated population. Our analysis showed that BMX reduces SARS-CoV-2 infection in vitro. Clinical trial results suggested that combinatory treatment with NAC + BMX is beneficial in mild to moderate COVID-19.

Abstract:
Osteosarcoma is a malignant tumor of mesenchymal cells that have the ability to produce osteoid matrix or mineralized bone. They can involve any bone, but in the jaw region they are uncommon and account for about 6% of all cases. These tumors present as painful and progressively growing masses. Often a sudden, non-traumatic fracture is the first significant clinical symptom. This paper reports a clinical case of osteosarcoma in the body of the mandible. A 25-year-old male patient came to the clinic reporting an intra and extra-oral swelling in the left lower jaw, associated with pain and paresthesia, which lasted four months. The patient reported a previous history of mucoepidermoid carcinoma in the left retroauricular region treated with parotidectomy when he was six years old. The diagnostic hypotheses of Odontogenic Myxoma and Osteosarcoma were raised. Thus, an incisional biopsy was performed, which showed histopathological examination compatible with osteosarcoma. After that, it was decided to perform a left segmental pelvemandibulectomy with histopathological study that confirmed the diagnosis. The case report contributes to the knowledge of health professionals about the osteosarcoma of the mandible since many times this diagnostic hypothesis is not raised because it is a rare situation.

Abstract:
SSRIs are chemically distinct from traditional antidepressants such as tricyclic, tetracyclic, and monoamine oxidase inhibitors, but they share the same mechanism of action in that they selectively and potently inhibit serotonin neuronal reuptake while having no or very little effect on norepinephrine, acetylcholine, and histamine neuronal reuptake. As a result, when compared to other tricyclic and tetracyclic antidepressants, these drugs have fewer sedative, anticholinergic, and cardiovascular effects. Fluroxamine, fluroxamine, sertraline, indalpine, paroxetine, alproclate, femorxetine, and choroxamine are examples of SSRI drugs.

Abstract:
In this paper five case reports were presented and include growth of hypoplastic right ventricle following pulmonary valvotomy, intracavitary electrocardiography in the diagnosis of Ebstein’s anomaly of the left atrioventricular valve in congenital corrected transposition of the great arteries, growth phenomenon as a cause of kinking of the right pulmonary artery in Waterston anastomosis, the usefulness of pulmonary vein wedge angiography in the visualization of an obstructed pulmonary artery, false aneurysm of the right pulmonary artery as a new complication of the Waterston anastomosis. For each case report, clinical, chest x-ray, electrocardiogram, angiogram, and other pertinent findings were presented. This was followed by discussion of etiology, diagnosis, and treatment options, as appropriate.

Discussion of uniqueness of these findings and management options were reviewed.

Abstract:
In this report, five different case scenarios were presented. In the first case, successful use of propranolol to relieve severe right ventricular infundibular obstruction was illustrated which facilitated balloon pulmonary valvuloplasty. In the second case, successful percutaneous occlusion of an atrial septal defect with buttoned device in an infant weighing 3.6 kg was documented. In the next case, rare association tricuspid atresia with persistent truncus arteriosus was recorded. The fourth case illustrates first successful transcatheter closure of patent ductus arteriosus with buttoned device in a child. The final case presents an unusual presentation of coarctation of the aorta in an infant and effective management with balloon angioplasty in the critically ill infant. Each of these cases were unique unto themselves either documenting successful management of these pathologies with innovative techniques or recording rare congenital cardiac abnormalities.

Abstract:
Five interesting cases reported between 1992 and 1994 were reviewed. The first case documents successful treatment of chylothorax with balloon angioplasty of baffle obstruction following Mustard operation. The second case illustrates successful relief of supra-valvar pulmonary artery stenosis that developed after arterial switch procedure by balloon dilatation. In the third case, shock-like syndrome developed in an 11-day-old baby with severe aortic coarctation which was relieved by balloon angioplasty; the procedure was performed via the umbilical artery, sparing the femoral artery. The fourth case documents percutaneous occlusion of patent ductus arteriosus with a buttoned device in an adult patient; this was first case of using this device in an adult patient. The final case points to late clinical presentation of the origin of the anomalous circumflex coronary artery from the pulmonary artery several years following repair of aortopulmonary window in infancy. Surgical correction of this anomaly was successfully accomplished. The cases presented successful use of catheter or surgical interventional procedures to address uncommon types of cardiac problems.

Schwannoma is a benign, encapsulated, slow-growing and generally solitary tumour that arise from Schwann cells of the peripheral nerve sheath. Sinonasal schwannoma is a rare entity represent less than four percent of all tumors in the head and neck area. We present our clinical observations in a 57-year-old woman with a locally advanced sinonasal schwannoma, originating in the sphenoid sinus and infiltrating ethmoid cells, the nasal cavity and the left retrobulbar space. Histopathological and immunohistochemical examination confirmed the diagnosis. Differential diagnosis requires a broad immunohistochemical analysis that will differentiate schwannoma from other benign and malignant peripheral nerve sheath tumors. Our clinical observations show that the only effective treatment of patients with schwannoma is a radical tumor surgery, as it concerns an extremely radioresistance benign tumor.
