Abstract:
In order to establish a worldwide standard of care for management of bariatric patients, the American Association of Clinical Endocrinologists (AACE), The Obesity Society (TOS), and the American Society for Metabolic and Bariatric Surgery (ASMBS) published clinical practice guidelines (CPG) for perioperative management of bariatric surgery patients in 2008. The CPG, which were updated in 2013, have been endorsed by professional bariatric societies worldwide. This chapter summarizes guidelines based on the updated CPG, with a focus on prevention and management of complications of bariatric surgery. The chapter outlines the guidelines for perioperative care of bariatric patients and specific management of perioperative complications using a system- or disease-oriented approach. The guidelines are designed to assist practitioners in patient management; however, practitioners should practice bariatric surgery based on current knowledge, experience, latest literature reviews, and patients’ needs in order to achieve optimal patient care.

Abstract:
Multiple documents have been published regarding the use of radionuclide imaging which attempt to outline the applications of SPECT and PET cardiac imaging and to offer guidance in not only how these techniques are performed but also when these tests should be used. These guidance documents, which include position papers, consensus documents, white papers, clinical practice guidelines (CPG), and appropriate use criteria (AUC) serve a number of purposes and are applicable to various health care providers. The focus of this chapter is on CPG and AUC, as these provide the basis for care optimization and reimbursement decisions and are the most widely accepted documents regarding radionuclide-imaging utilization. Throughout this book, each chapter has focused on the best practice of nuclear cardiology, emphasizing methods that will not only improve image quality but also provide significant clinical value. This chapter will emphasize the selection of patients for radionuclide imaging and focus on how best to use these resources to improve the outcome of patients with known or suspected heart disease.

Abstract:
Evaluation of the obese patient requires identification of all comorbidities and health conditions, including underlying cardiac and pulmonary conditions that could have a negative outcome on noncardiac surgery. Once comorbidities have been recognized, aggressive optimization of these medical conditions can provide improved outcomes after bariatric surgery. Estimating medical risk can be performed by several validated classification systems. The preoperative checklist and clinical practice guidelines (CPG) were updated in 2013. The CPG recommendations for preoperative evaluation of the bariatric surgery patient include lab testing, nutritional screening, endocrine assessment, and cardiopulmonary assessment, including sleep apnea screening. The CPG suggest an extensive multidisciplinary team approach to the preoperative bariatric surgery patient. Finally, the medical evaluation includes an algorithm for a seven-step approach to the preoperative visit. Also recommended for evaluation of the morbidly obese patient is an algorithm that uses a five-step approach after a comprehensive history and physical exam and lab testing.

Abstract:
Although there is currently insufficient evidence to support meditation as a first-line treatment for posttraumatic stress disorder (PTSD), the evidence base for meditation used adjunctively in the management of PTSD and related psychiatric comorbidities is rapidly expanding. The 2010 Veterans Administration/Department of Defense (VA/DoD) clinical practice guideline (CPG) for management of PTSD states that mind–body approaches may be considered adjunctive treatment for hyperarousal symptoms. Although several reviews support the conclusions reflected in the CPG, others suggest meditation interventions may be more useful in managing PTSD than originally speculated. Meditation may help reduce intrusive memories, avoidance, and anger; and increase self-esteem, pain tolerance, energy, and ability to relax and cope with stress. One comparative effectiveness review concluded that mindfulness meditation is beneficial in reducing psychological stress consequences, such as depression, pain, and mental health-related quality of life.
