Abstract CLINICAL PROBLEMS ARE often complex. Problems of great complexity are usually associated with a commensurately greater degree of difficulty with respect to the decision making process. This is most certainly true regarding the management of cervical spondylosis. Usually, clinicians make clinically appropriate decisions. However, more often than realized, suboptimal decisions may be made. Therefore, an assessment of the types of errors regarding clinical decision making are worthy of consideration. In this article, a scheme for decision making regarding the management of cervical spondylosis is presented.

Abstract:
Background: Clinical decisions are a nurses' daily clinical practice skill that needs to be constantly evolving in order to provide quality patient care. Purpose: To investigate and compare the clinical decisions made by nurses in Greece and Slovakia. Materials and Methods: Q-methodology was used and eight modified scenarios were given to the nursing staff. Using power analysis, a total of 100 nurses that worked in public hospitals in Greece (nG = 50) and Slovakia (nS = 50) successfully completed the study. Results: In the scenarios with more medical interventions, Greek nurses showed higher scores in the scenarios of tachycardia, bradycardia and cardiopulmonary resuscitation. In contrast, Slovak nurses scored higher individual scores in all four scenarios (anxiety - difficulty sleeping –dyspnea - vomiting) with more nursing interventions (p<0.05). Conclusions: Greek nurses overall had higher individual scores in the scenarios with more medical interventions, probably due to the medically-centered philosophy of nurses, the lack of protocols and autonomy to provide patient care.

Abstract:
Shared decision making (SDM) is a process by which physicians and patients jointly participate in choosing to pursue one of several alternatives in a clinical decision. It is most relevant for decisions that involve significant potential harms and benefits with tradeoffs and uncertainty. This paper provides a state-of-the-art review about SDM covering its concept, value, implementation and application in emergency management and communication. SDM is valuable in the process of making decisions which patients may benefit most from, thus resulting in satisfying patient-centered outcomes. Although SDM can be challenging to incorporate into clinical practice, it is likely to become a useful tool of communication in future.
