Abstract:
Ethosuximide is a first-generation antiepileptic drug characterized by few antiepileptic indications, with acceptable interaction profile in polytherapy. Ethosuximide has a good behavioural tolerability profile, but no approved indications or clinical uses in psychiatry. Behavioural adverse effects can sometimes occur following cessation of seizures and normalization of the EEG and resolve with discontinuation of Ethosuximide and seizure recurrence. Specifically, there are reports of psychosis (but no aggression-related behaviours) in adult patients treated with Ethosuximide. Since the psychotic symptoms tend to occur in clear time relation to the Ethosuximide treatment, it seems plausible that they are manifestations of alternative psychosis in the context of forced normalization.

Abstract:
Autism spectrum disorder (ASD) is an early onset and persistent condition defined by alterations in social communication and social interation alongside repetitive, restricted stereotypic behaviours and interests causing disabilities. Until recently, research on the co-occurrence of ADHD with ASD has been limited by DSM-IV criteria, allowing no dual diagnosis of these two neurodevelopmental disorders. Since the DSM-5 permits a double diagnosis of ADHD plus ASD, research on their comorbidity has substantially increased. In addition to shared and distinct aetiological factors, studies have revealed a high clinical impact of the combined symptomatology on individual outcomes. This chapter provides a selective overview of behavioural, cognitive, and biological findings as well as intervention strategies in combined ADHD/ASD phenotypes.

Abstract:
As sleep medicine advances, there is increasing demand on services including neurophysiology to investigate sleep disorders. This chapter classifies the sleep disorders according to the main symptom presenting to the clinician, including excessive daytime sleepiness, insomnia, patients sleeping at the wrong times due to a circadian rhythm disorder, and movements or behaviours in sleep. The clinical presentation of common sleep disorders in each category are outlined, including obstructive sleep apnoea, narcolepsy, restless leg syndrome, periodic leg movements disorder, circadian rhythm disorders, and non-rapid eye movement and rapid eye movement parasomnias. The chapter discusses the overlap of symptoms in different sleep disorders, and the importance of selecting appropriate sleep studies, and recognizes the pitfalls, both clinical and in interpretation of sleep studies. The difficulties in diagnosing narcolepsy and differentiating from other causes of excessive daytime sleepiness, and when to investigate parasomnias is explained.

Abstract:
This chapter takes the need to save ‘normal’ and avoid diagnostic inflation as a point of departure to reflect on what is normal, what is not, and what is in-between. It argues that ‘normal’ is a fallible and vulnerable comparative class to define mental disorders. Mental disorders must be distinguished from behaviours and mental states that reflect individual difference but not clinically significant psychopathology. Risk conditions, mild symptoms, and prodromal stages signify that a person is not yet or not severely diseased. Such notions of unclear cases and in-between states exhibit both vagueness and ambiguity. These considerations are used to suggest ways of limiting the (mis)uses of DSM in fields other than clinical psychiatry.

Abstract:
Cognitive and behavioural dysfunction in amyotrophic lateral sclerosis (ALS) occurs in up to half of patients with a spectrum from ALS with no cognitive or behavioural impairment to ALS with frontotemporal dementia (FTD). ~ 15% have a full blown ALS-FTD syndrome, while ~ 35% show milder and specific deficits on verbal fluency, executive and language functions and social cognition. Patients may show a behavioural syndrome that ranges from mild specific difficulties to changes that fulfil diagnostic criteria for behavioural variant-FTD. Apathy is the most prevalent symptom, but disinhibition, perseveration, loss of sympathy/empathy, and change in eating behaviour are also described. The importance of assessment is increasingly recognized. A distinction is made between brief assessment tools useful within ALS clinics and more extensive neuropsychological assessment by a qualified clinical neuropsychologist. Newly developed assessments specifically designed for ALS are available and will make valuable contribution to clinical care.
