Abstract:
Le phéochromocytome est une tumeur peu fréquente de la médullosurrénale. Les signes cliniques sont liés à une dysendocrinie induite par la sécrétion excessive et souvent pulsatile de catécholamines, mais également à l’invasion locale et à distance du processus tumoral. Ces signes sont très variés et souvent peu spécifiques et intermittents; le plus fréquemment sont présents de la faiblesse, de l’abattement, des phases de tachypnée/d’halètement et des malaises épisodiques. L’hypertension artérielle systémique peut être majeure mais évolue souvent par poussées et n’est donc pas toujours présente en consultation. Les anomalies observables lors du bilan sanguin de routine et à l’analyse d’urine sont peu spécifiques mais peuvent permettre d’exclure d’autres hypothèses. Le diagnostic repose sur la mise en évidence par imagerie d’une masse surrénalienne associée à la démonstration d’une production excessive de catécholamines par la mesure des concentrations sanguines et/ou urinaires de leurs métabolites, métanéphrine et normétanéphrine. Ces dosages étant encore peu disponibles en routine, le clinicien est souvent amené à réaliser un diagnostic d’exclusion en éliminant l’hypothèse d’une tumeur surrénalienne sécrétant d’autres hormones, par des tests endocriniens. Lorsque le doute persiste après ces examens, l’opportunité d’une ponction à l’aiguille fine échoguidée de la lésion pour analyse cytologique peut être considérée.

Abstract:
Le syndrome de Cushing est une endocrinopathie fréquente chez le chien. Il touche principalement les animaux âgés et certaines races sont prédisposées. Les signes cliniques, peu spécifiques, sont une polyuro-polydipsie, une polyphagie, une distension abdominale et des troubles cutanés. Plusieurs dosages biologiques peuvent permettre de renforcer une suspicion clinique comme, entre autres, la modification de l’activité de l’enzymologie hépatique. Les tests fonctionnels utilisés possèdent des qualités intrinsèques imparfaites et ainsi il est nécessaire de sélectionner les animaux au préalable afin d’augmenter la probabilité pré-test de ces derniers. L’interprétation des résultats est à réaliser à la lumière des éléments épidémiologiques, cliniques et biologiques. Après avoir confirmé un hypercorticisme, il est important d’en déterminer l’origine (hypophysaire ou surrénalien) afin de proposer la prise en charge thérapeutique la plus adaptée et de déterminer le pronostic. Il est notamment possible de se baser sur les résultats de tests dynamiques, le dosage de l’ACTH endogène et/ou sur des examens d’imagerie.

The Chinese medicinal herbs Radix Isatidis and Viola yedoensis Makino have been suggested to possess antiviral activity. This study tests whether these and other Chinese and Western herbal medicinal formulas can modulate the immune functions involving virus-suppression in BALB/c mouse. We first confirmed the extract from Viola yedoensis Makino, but not from Radix Isatidis, the traditional Chinese medicine (TCM) formula Chui-Uren-Chien (CUC), or a Western homeopathic medicinal drink Método Canova, could inhibit the replications of herpes simplex virus-1 and enterovirus 71 in the human neuroblastoma SK-N-SH cell line. Subsequently, the same herbal extracts and drink underwent toxicity and immunomodulatory tests on mice of 5–7 weeks old. After 8 weeks of feeding different herbal medicinal formulas, no hepatic or renal toxicity was noted in any tested animal; whereas among the immune function evaluations, only the mice treated with CUC extract were found to be associated with significant increases (p < 0.05) in both the level of plasma IgG and the percentage of monocyte in blood mononuclear cells as well as the activation of macrophage Raw264.7 cells for nitric oxide production, suggesting its role in modulating the non-specific immune response. Analyses using protein arrays showed CUC was the most potent herbal medicinal formula eliciting fluctuations in plasma cytokine and chemokine concentrations. Taking all experimental data together, we conclude Chui-Uren-Chien possesses immunomodulatory capability in mouse, but none of the herbal medicinal formulas tested here are involved in strengthening antiviral immunity.

Abstract:
Des tissus post mortem (du cortex cérébral, de l’hippocampe, du cervelet, des ganglions trigéminaux et des glandes salivaires), fixés dans du formaldéhyde et inclus dans de la paraffine, de 25 chiens ont été obtenus dans un laboratoire de diagnostic vétérinaire du Nigeria et soumis à des tests de recherche de l’ARN et de l’antigène du virus de la rage. L’ARN génomique et l’ARN messager (ARNm) du virus de la rage codant pour la glycoprotéine ont été détectés dans les tissus par la technique d’hybridation in situ (HIS) en utilisant des sondes d’ARN à simple brin marquées au 3H. L’antigène du virus de la rage a été mis en évidence par immuno-marquage avec de la peroxydase et le complexe avidine-biotine. L’ARNm viral a été détecté dans les tissus en utilisant des sondes d’ARN marquées à la digoxigénine. Le diagnostic histopathologique par coloration à l’hématoxyline-éosine a révélé des corps de Negri dans les tissus de 8 (32 p. 100) chiens ; 5 (20 p. 100) autres chiens ont présenté des modifications inflammatoires dues à des encéphalites virales sans corps de Negri. L’antigène a été détecté chez 11 (44 p. 100) des chiens examinés, soit dans les tissus des 8 chiens chez lesquels des corps de Negri ont été observés, de 2 des 5 chiens ayant présenté des modifications inflammatoires et d’un chien sur les 12 dont l’histopathologie était négative. L’ARN génomique et l’ARNm ont été détectés dans les tissus de tous les chiens où se trouvait l’antigène, et la distribution des signaux colorant pour les deux méthodes dans les neurones et dans les acini des glandes salivaires a été similaire. L’ARNm était plus abondant que l’ARN génomique et les signaux radioactifs étaient distribués de manière diffuse dans les périkaryons et les processus dendritiques des neurones, et dans les acini et les canaux salivaires. L’ARNm a également été trouvé en abondance dans les neurones et dans les acini avec des sondes marquées à la digoxigénine ; la méthode était plus simple. Par ordre décroissant de quantités, les marqueurs de l’antigène et de l’ARNm étaient les plus nombreux, venaient ensuite ceux de l’ARN génomique et enfin ceux des corps de Negri et d’histopathologie. La méthode de coloration par immunoperoxydase et la technique HIS utilisant des sondes marquées à la digoxigénine peuvent permettre d’effectuer des recherches sur la rage et d’en établir le diagnostic, en particulier dans les pays en développement où cette maladie continue de poser un problème de santé publique de grande importance.

Le trilostane (Vetoryl®) est un inhibiteur de la synthèse du cortisol indiqué dans le traitement de l’hypercorticisme d’origine hypophysaire ou surrénalienne chez le chien. Son mode d’action repose sur l’inhibition de la synthèse du cortisol et, dans une moindre mesure, de la synthèse de l’aldostérone. Compte tenu de la grande variabilité de réponse et de pharmacocinétique de cette molécule, la surveillance des animaux traités au trilostane doit être fréquente et la dose doit être régulièrement ajustée. Le suivi thérapeutique repose essentiellement sur la clinique et la réalisation régulière de tests de stimulation à l’ACTH. Lors d’une absence de réponse à ce test, le traitement doit être arrêté. En revanche, lorsque les résultats de ce test sont dans les normes ou montrent une cortisolémie trop élevée, l’augmentation de la posologie doit reposer sur l’évaluation clinique de l’animal et la persistance de signes cliniques compatibles avec un hypercorticisme. Même si le trilostane est généralement bien toléré, un hypocorticisme réversible peut parfois apparaître au cours du suivi. Des troubles du ionogramme (hyperkaliémie et hyponatrémie) liés à la baisse de la concentration d’aldostérone circulante sont parfois observés.

Abstract:
Searches for parity violation in hadronic systems started soon after the evidence for parity violation in β-decay of 60 Co was presented by Madame Chien-Shiung Wu and in π and μ decay by Leon Lederman in 1957. The early searches for parity violation in hadronic systems did not reach the sensitivity required and only after technological advances in later years was parity violation unambiguously established. Within the meson-exchange description of the strong interaction, theory and experiment meet in a set of seven weak meson-nucleon coupling constants. Even today, after almost five decades, the determination of the seven weak meson-nucleon couplings is incomplete. Parity violation in nuclear systems is rather complex due to the intricacies of QCD. More straight forward in terms of interpretation are measurements of the proton-proton parity-violating analyzing power (normalized differences in scattering yields for positive and negative helicity incident beams), for which there exist three precision experiments (at 13.6, at 45, and 221 MeV). To-date, there are better possibilities for theoretical interpretation using effective field theory approaches. The situation with regard to the measurement of the parity-violating analyzing power or asymmetry in polarized electron scattering is quite different. Although the original measurements were intended to determine the electro-weak mixing angle, with the current knowledge of the electro-weak interaction and the great precision with which electro-weak radiative corrections can be calculated, the emphasis has been to study the structure of the nucleon, and in particular the strangeness content of the nucleon. A whole series of experiments (the SAMPLE experiment at MIT-Bates, the G0 experiment and HAPPEX experiments at Jefferson Laboratory (JLab), and the PVA4 experiment at MAMI) have indicated that the strange quark contributions to the charge and magnetization distributions of the nucleon are tiny. These measurements if extrapolated to zero degrees and zero momentum transfer have also provided a factor five improvement in the knowledge of the neutral weak couplings to the quarks. Choosing appropriate kinematics in parity-violating electron-proton scattering permits nucleon structure effects on the measured analyzing power to be precisely controlled. Consequently, a precise measurement of the ‘running’ of sin 2θw or the electro-weak mixing angle has become within reach. The [Formula: see text] experiment at Jefferson Laboratory is to measure this quantity to a precision of about 4%. This will either establish conformity with the Standard Model of quarks and leptons or point to New Physics as the Standard Model must be encompassed in a more general theory required, for instance, by a convergence of the three couplings (strong, electromagnetic, and weak) to a common value at the GUT scale. The upgrade of CEBAF at Jefferson Laboratory to 12 GeV, will allow a new measurement of sin 2θW in parity-violating electron-electron scattering with an improved precision to the current better measurement (the SLAC E158 experiment) of the ‘running’ of sin 2θW away from the Z0 pole. Preliminary design studies of such an experiment show that a precision comparable to the most precise individual measurements at the Z0 pole (to about ±0.00025) can be reached. The result of this experiment will be rather complementary to the [Formula: see text] experiment in terms of sensitivity to New Physics.

Abstract:
Background and Purpose. To improve the utility of the Berg Balance Scale (BBS), the aim of this study was to develop a short form of the BBS (SFBBS) that was psychometrically similar (including test reliability, validity, and responsiveness) to the original BBS for people with stroke. Subjects and Methods. A total of 226 subjects with stroke participated in this prospective study at 14 days after their stroke; 167 of these subjects also were examined at 90 days after their stroke. The BBS, Barthel Index, and Fugl-Meyer Motor Test were administered at these 2 time points. By reducing the number of tested items by more than half the number of items in the original BBS (ie, making 4-, 5-, 6-, and 7-item tests) and simplifying the scoring system of the original BBS (ie, collapsing the 5-level scale into a 3-level scale [BBS-3P]), we generated a total of 8 SFBBSs. Results. The distributions of scores for all 8 SFBBSs were acceptable but featured notable floor effects. The 4-item BBS, 5-item BBS, 5-item BBS-3P, and 7-item BBS-3P demonstrated good reliability. The subjects’ scores on the 6-item BBS, 6-item BBS-3P, 7-item BBS, and 7-item BBS-3P showed excellent agreement with those on the original BBS. The 6-item BBS-3P and 7-item BBS-3P exhibited great responsiveness. Only the 7-item BBS-3P demonstrated both satisfactory and psychometric properties similar to those of the original BBS. Discussion and Conclusion. The 7-item BBS-3P was found to be psychometrically similar to the original BBS. The 7-item BBS-3P, compared with the original BBS, is simpler and faster to complete in either a clinical or a research setting and is recommended. [Chou CY, Chien CW, Hsueh IP, et al. Abstract Background Respiratory viruses are readily detectable in hematopoietic cell transplant (HCT) recipients in the molecular diagnostic era. The association of respiratory virus infections with acute and chronic airflow obstruction (AFO) is poorly defined. Methods HCT recipients were prospectively followed with weekly handheld spirometry and symptom questionnaires through 1 year after HCT. Weekly multiplex PCR testing for 11 respiratory viruses was performed through day 100 post-HCT and every 3 months and with respiratory symptoms thereafter. Standard pulmonary function testing occurred at recommended intervals. Cox proportional hazard models were used to correlate longitudinal symptomatic respiratory tract viral infections with AFO phenotypes, defined as 2- or 4-week decline (↓) of 1 second forced expiratory volume (FEV1) &gt;10% by handheld spirometry; late AFO (FEV1/forced vital capacity [FVC] &lt; lower limit normal predicted and FEV1 decline &gt;10% from baseline at 3 years; or bronchiolitis obliterans syndrome (BOS; FEV1 &lt;75%, FEV1/FVC &lt; 0.7, and FEV1 ↓ &gt;10% from baseline) by 3 years after HCT; late AFO and BOS were assessed by standard pulmonary function testing. Results Overall, 7,091 PCR tests were performed in 471 patients; 70% of patients had ≥1 respiratory virus detected. Among 437 patients who survived &gt;4 weeks, decline of FEV-1 for 2 or 4 weeks, late AFO or BOS occurred in 11.9%, 7.1%, 15.6%, and 3.9%, respectively. In adjusted Cox models, human metapneumovirus (HMPV), influenza virus A/B, and parainfluenza virus 1–4 (PIV) upper tract infections (URI) were associated with 2 and 4 weeks FEV-1 decline (Figure 1). Late AFO and BOS were only significantly associated with RSV- or HMPV-related URI (Figure 2). Lower respiratory disease (LRD) due to HMPV (adjusted HR 11.1, P = 0.02) was associated with a 2- and 4-week FEV-1 decline. Conclusion Development of AFO after HCT is common. Respiratory viruses are significantly associated with both short-term airflow decline and long-term airflow obstruction. Interventional strategies that target multiple viruses are warranted. Aircraft noise is the chief source of the airport and its peripheral areas. CCAR imposes restriction on specific categories of aircraft and requires more strict measurements of the noise. A 45-year-old man visited our clinic with a painless swelling of the left scrotum and an ulcer as chief complaints. A hard and indurated mass was palpable with ulcerating foci that were proximal and distal, measuring 3 × 2 cm and 2 × 1 cm respectively and about 2 cm apart. Laboratory data were normal except for an elevated erythrocyte sedimentation rate (ESR), and white blood cell (WBC) differential showed neutropenia and lymphocytosis. A diagnosis of left testicular tumor was made and the patient had a left orchidectomy with fistulectomy. Histopathology results showed a stratified squamous epithelium with tuberculous granuloma and necrotic caseation. Patient is currently on anti-tubercular medication. The rarity of this condition makes these findings important to report.

American chief executives are popularly believed to play a crucial leadership role in the legislative process, but this role is not well understood by scholars. This article develops a model of executive leadership success in the legislative arena based on personal, institutional, political, and economic factors and tests that model with data on the legislative priorities of the 50 state governors in 1993–94. Institutional features of the executive, which have been the focus of much scholarship, are found to have little effect on a governor's legislative success. On the other hand, the nature of the legislature is quite important here, with more professional legislatures actually encouraging executive leadership. A governor's personal characteristics, political party, and a state's economy also influence a governor's legislative success. These findings point to the importance of examining chief executives in the context in which they work if we are to understand their role as legislative leader.

Abstract Background: Recent guidelines regarding estrogen (ER) and progesterone (PR) receptor testing from the American Society of Clinical Oncology and College of American Pathologists defined a new reporting category of ER-low positive breast cancer for tumors with 1-10% ER expression by immunohistochemistry (IHC). The clinical implications of ER-low positivity are incompletely understood, especially in invasive lobular carcinoma (ILC), the second most common type of breast cancer. Given the rarity of low-ER positivity in ILC, we evaluated tumor features and outcomes associated with a spectrum of ER/PR positivity in a monoinstitutional ILC cohort. Methods: We analyzed cases of stage I-III ILC with available IHC reports. Based on prior published categories in ILC, we classified ER as low, medium, or high as defined by ER staining of 10–69%, 70–89%, and ≥90% respectively. PR negative, low, and high tumors were defined by 0%, &lt; 20%, or ≥20% staining respectively. We used chi-squared tests, t-tests, and Cox proportional hazards models in Stata 16.1 to evaluate associations between ER/PR categories including clinicopathologic variables and event-free survival (EFS). Results: Of 744 cases, 24 (3.2%) were ER negative and 10 (1.3%) were ER-low positive as defined by 1-10% positive staining. 713 remaining cases had ER positivity ≥ 10% and comprised the cohort categories of ER low, medium, and high for this study (11.2%, 15.0%, and 73.8% respectively). In 751 cases with PR data, 122 (16.2%) were PR negative, 145 (19.4%) were PR low and 483 (64.3%) were PR high. ER high status was significantly associated with older age (mean 56.7, 56.7, and 60.6 years in ER low, medium, and high respectively, p=0.0005). ER low was associated with PR negative and low status (42.3% were PR neg/low and ER low, versus 37.4% with ER medium and 29.9% in ER high, p=0.045), and more likely to have overexpression of HER2 (9.7%, 9.0%, and 2.9% ER low, medium, high, respectively, p=0.002). ER low tumors were more likely to be grade 1 than ER medium or high (41.8%, 29.8% and 24.5% respectively, p=0.025), and have positive surgical margins (39.4%, 35.9% and 23.9% respectively, p=0.002). ER status was not associated with Ki67, stage, body mass index (BMI), lymphovascular invasion, lobular carcinoma in situ (LCIS), pleomorphic histology, local therapy, or chemotherapy use. In contrast, PR high was significantly associated with younger age (57.6 versus 63.5 years in PR low, p&lt; 0.0001). PR low was associated with HER2 overexpression (8.6% versus 3.2% in PR high, p=0.002). PR low cases were more likely to present at higher stages (15.8% stage III versus 10.1% stage III in PR high, p=0.032), to be pleomorphic (16.8% versus 8.2%, p&lt; 0.001), and to receive chemotherapy (30.8% versus 23.1%, p=0.022) but were less likely to have associated LCIS (64.0 versus 74.2%, p=0.004). PR status was not associated with Ki67, BMI, lymphovascular invasion, local therapy, or surgical margins. In a Cox proportional hazards model adjusting for age, stage, grade, pleomorphic histology, and chemotherapy use, ER category was not associated with EFS but both PR negative and PR low status each had significantly worse EFS compared to PR ≥20% (HR 3.5, 95% CI 1.8-6.7, p&lt; 0.001 for PR negative, and HR 2.0, 95% CI 1.1-3.5, p=0.015 for PR low). The estimated cumulative 5-year EFS for patients with ER low, medium, and high tumors was 87.1%, 93.4%, and 90.1% respectively. The estimated cumulative 5-year EFS for patients with PR negative, low, and high tumors was 78.9%, 90.2%, and 92.7% respectively. Conclusions: Using ILC-specific categories for ER expression, we found associations between ER category and clinicopathologic variables but not with EFS. In contrast, PR negative and low status was associated with worse EFS. These findings highlight the importance of exploring the spectrum of ER/PR activity within ILC, a classically strongly hormone receptor-positive tumor type, using more quantitative methods. The river chief system (RCS) is an autonomous environmental policy implemented by local governments in China that incorporates environmental responsibilities into the performance evaluation. Although existing literature suggests that RCS can reduce water pollution, the impact of RCS on energy efficiency has not been assessed. Therefore, this paper compiles data on industrial enterprises and industrial pollution in China from 2003 to 2013 and empirically examines the impact of RCS on green total factor energy efficiency (GTFEE) by using a multiple difference-in-difference approach. The results show that RCS significantly enhances firms’ GTFEE, and a series of tests confirm the robustness of the findings. Second, we further explore how RCS affects GTFEE, the mechanism tests conclude that the RCS improves GTFEE mainly through optimizing energy structure and promoting technological innovation. Third, compared with small firms, exporters, and firms in non-heavy polluting industries, the RCS has a greater effect on improving the GTFEE of large firms, non-exporters, and firms in heavily polluting industries. This study provides new and novel ideas for emerging countries to improve environmental policies and achieve sustainable development.

The purpose of this study is to investigate the impact of women in politics on the risk of a coup d’état. Previous research indicates that the relationship between female political leaders and security is dependent on the office she holds. Subsequently, we expect female legislators to have a different influence than a female chief executive on the likelihood of a coup. We argue that a higher level of female representation reduces the risk of a coup d’état. However, we assert that a female chief executive has a different effect and increases coup risk. Using data covering 160 states over the years 1952 to 2009, our empirical tests provide support for our expectations. All else being equal, increased levels of women in parliament lead to a substantial drop in coup likelihood. However, the argument that a female chief executive will be more coup prone is not fully supported in our findings.

Introduction: Emergency physicians must maintain a broad differential when seeing patients in the emergency department (ED). Occasionally, a patient may have an undiagnosed, life-threatening medical condition not related to the presenting chief complaint. It is imperative to review all ordered laboratory tests and any available previous laboratory values to assess for any abnormalities that may warrant further evaluation. Case Report: This case report is regarding the missed diagnosis of acute leukemia and subsequent disseminated intravascular coagulation in a 27-year-old male who presented to multiple EDs with the unrelated chief complaint of finger ring entrapment. This patient ultimately succumbed to his illness. Conclusion: When evaluating patients in the ED, it is important to review any prior available test results for abnormalities, even if the results do not specifically correlate with the chief complaint. Emergency physicians must remain vigilant to avoid missing a critical diagnosis.

Background: Recent changes in healthcare delivery have necessitated residency education reform. To adapt to these changes, graduate medical education can adopt a chief resident-led clinical curriculum. Chief residents are ideal clinical instructors, as they are recent graduates who have excelled in their residency programs. To effectively use the limited time available for education, chief residents can implement active learning techniques. We present a chief resident-led, small-group, problem-based curriculum for teaching first-year internal medicine residents, and provide preliminary data supporting the efficacy of this approach.Methods: The seminar consisted of 11 4-week modules. Week 1 was a team-based crossword competition. Weeks 2-4 were small-group, problem-based clinical reasoning sessions taught by chief residents. The program was evaluated via pre- and post-module multiple-choice tests. Resident satisfaction data were collected via self-reported, anonymous surveys.Results: Preliminary results revealed a statistically significant increase from pre-test to post-test score for 9 of the 11 modules. The chest pain, fever, abdominal pain, shock, syncope, jaundice, dizziness, anemia, and acute kidney injury modules achieved statistical significance. Additionally, resident satisfaction surveys show that this teaching approach was an enjoyable experience for our residents.Discussion: Our chief seminar is an evidence-based, clinical reasoning approach for graduate medical education that uses active learning techniques. This is an effective and enjoyable method for educating internal medicine residents. Because of its reproducibility, it can be applied throughout residency education.

Safety is the chief consideration in recipient preparation of spermatogonial stem cell (SSC) transplantation in mammals, especially humans. In this study, we compared the safety of the SSC transplantation recipients that were prepared both by testes heat shock plus testes busulfan injection (heat shock+busulfan(t)) and by busulfan intraperitoneal injection (busulfan i.p.) only. Our results showed that heat shock+busulfan(t) treatment significantly ( p < 0.05) reduced mortality in mice and did not produce bone marrow cell toxicity. Furthermore, heat shock+busulfan(t) treatment directly damaged SSCs and exhausted almost all of the germ cells in the testis; the exhaustion of these cells is considered a key factor in the successful preparation of the recipients. Therefore, we used heat shock+busulfan(t) treatment to prepare recipients of SSC transplantation. Two months after SSC transplantation, the number and length of donor SSC-derived colonies in the testis of recipient in heat shock+busulfan(t) group was closed to that in busulfan i.p. group. Therefore, compared with busulfan i.p. treatment, heat shock+busulfan(t) treatment improved the safety of recipient preparation without reducing the efficiency of SSC transplantation. Two GFP-positive offspring were produced from 1 of the 20 recipients that had mated with female mice 72 days after SSC transplantation. In conclusion, heat shock with busulfan treatment is a safe method to prepare the recipient of SSC transplantation in mice.

To study the effect of temperature on the dynamic mechanical behaviors and fracture characteristics of thermally treated sandstone, compressive dynamic loading experiments were performed on a series of samples using a split Hopkinson pressure bar (SHPB). In the tests, the dynamic uniaxial compressive strength of the thermally treated sandstone was inversely proportional to the temperature treatment in the range of 200°C–1000°C, while the dynamic elastic modulus first increased and then gradually decreased after different temperature treatments. The results show that two classical mechanical types (i.e., Class I and Class II) are observed from the dynamic stress-strain responses of SHPB tests for thermally treated sandstone. By means of scanning electron microscopy (SEM), the microdifference of postloading microfracture characteristics in Class I and Class II behavior was identified. In Class I behavior, intercrystalline cracks (IE) are the chief form of cracks on the fracture surface of a specimen fractured by SHPB loading even though there some intracrystalline cracks may also be present. In contrast, Class II behavior results from the chief cracking type being intracrystalline cracks.

Corporate governance mechanisms highlight the invaluable role of the Chief Executive Officer (CEO) in the affairs of an organization. The nexus between CEO characteristics and firm performance is not mute in the literature and mixed findings have also been presented. This study deviates from previous studies by focusing on the impact of CEO attributes on growth opportunities of the firm. Accordingly, this paper analysed duality and nationality of CEO on firms’ growth opportunities against the backdrop of national culture and societal values. To this end, secondary data of 76 non-financial firms listed on the Nigerian Exchange Group from 2010- 2019 were analyzed. Various diagnostic tests for panel data unit root were conducted. The result of the robust panel data analysis revealed that both CEO duality and nationality have negative but insignificant relationship with firms’ growth opportunities. Clearly, the CEO attributes examined do not wield significant influence in diminishing growth opportunities.

In developing a menu of computerized performance tests for repeated-measures applications the metric properties of selected tests have been examined. Factors of chief concern have been stability and reliability, as well as the practical issue of the length of time it takes to achieve high levels of both. In this study, these factors, as well as predictive validity, are examined. 25 subjects were tested repeatedly (10 sessions) with 11 tests previously identified as “good” candidates for repeated-measures research in paper-and-pencil (marker test) versions. The 11 tests were administered concurrently in their traditional paper-and-pencil modes and newly implemented microcomputer-based versions, along with the Wechsler Adult Intelligence Scale (WAIS). Nine of the 11 microcomputer-based tests achieved stability. Reliabilities were generally high, with r ≥ .77 for 3 min. of testing. Cross-correlations of microbased tests with traditional paper-and-pencil versions suggest equivalency between the test constructs in the different media. Correlations between six of the microbased subtests and the WAIS identified common variance, and these might comprise an efficient short (6 min.) battery of tests.

The charge given to me by the organisers of the memorial meeting for Prof. Abdus Salam’s 90th birthday is to recall my personal impressions of him and review an aspect of the standard model (SM) physics related to my work. Salam was, first and foremost, a brilliant theoretical physicist whose work is still very much en vogue, currently being tested precisely by the experiments at the Large Hadron Collider (LHC). Salam was, however, equally effective as a scientific advisor to many institutions, such as IAEA and CERN, but also to the government of Pakistan as the chief scientific strategist. He was also an untiring advocate of scientific research and higher education in developing countries, which took a concrete form in the International Centre for Theoretical Physics (ICTP) in Trieste. I discuss these aspects of his scientific life seen from my perspective in the first part. In the second part of my talk, which may appear as a disjoint piece to the first, I summarise some selected topics in rare B-decays — the current flavour physics frontier. Experiments carried out over several decades are largely in agreement with the SM, thanks also to dedicated theoretical effort in their interpretation. However, this field is undergoing an anomalous phase in a number of key measurements, in particular reported by LHCb, triggering a very lively debate and model building. These anomalies, which I review here, are too numerous to be ignored, but none is individually significant enough to announce a breakdown of the SM.

PurposeThis paper aims to examine whether there is an association between the level of performance‐based incentives offered to CEOs and the composition of firms' boards of directors and the compensation committee.Design/methodology/approachUnivariate tests are used to test the relation between the level of performance‐based incentives and corporate governance structures. A logistic regression analysis is used to predict the probability of CEOs receiving low performance‐based incentives when various characteristics of firms' boards of directors and compensation committees exist.FindingsThe authors find the presence of CEO duality reduces the likelihood of lower levels of performance‐based incentives offered to CEOs. Additionally, the authors find CEOs are more likely to receive lower levels of performance‐based incentives when the majority of the compensation committee members serve on less than three other boards, and when the size of the board is less than or equal to nine members.Research limitations/implicationsThis study is limited by the fact that the sample may not be representative of the general population of companies in the US.Practical implicationsShareholders who desire to keep CEO compensation levels low may consider supporting the separation of the positions of CEO and Chairperson of the Board, as well as supporting limiting the number of other boards directors may serve, and reducing or keeping the size of the board to a maximum of nine members.Originality/valueThe authors have documented an association between board structure and CEO compensation. It appears that company boards are able to monitor and control the compensation level offered to CEOs.

Abstract:
An experimental study on three types of roll damping devices—bilge keels, passive bilge fins, and paravanes—is described. These tests were conducted in the MIT Ship Model Towing Tank using scale models of a 76-ft single-chine trawler and a 119-ft double-chine trawler. The models were fitted with each damping device and excited in roll by a hull-mounted moment generator. Roll motions were measured at zero speed and at trawling and steaming speeds. Nondimensional damping ratios have been calculated and the effects on roll damping of each device are compared. The relationship of bilge keel and bilge fin area and aspect ratio to damping ratio are studied and some design considerations are presented. Practical aspects of each device type are also discussed.

La rage est une zoonose endémique dans les pays en développement et compte parmi les pathologies virales les plus virulentes et mortelles. Elle constitue une menace pour la sante publique et animale au Burkina Faso. La présente étude transversale a été conduite pour décrire l’épidémiologie de la rage et les connaissances, attitudes et pratiques (CAP) des populations à Ouagadougou. Elle a consisté en une collecte de données auprès des ménages et des structures techniques impliquées dans la prévention, le contrôle et la surveillance de la rage humaine et animale. L’enquête CAP a été réalisée auprès de 616 ménages. Elle a montré que la majorité des participants avaient entendu parler de la rage humaine (80,7 %) et animale (94,6 %), savaient qu’elle se transmettait essentiellement à la suite de morsure par un chien infecte (94,2 %) et étaient capables de citer correctement au moins deux voies de transmission de la maladie (65,7 %). En revanche, seuls 9,7 % des propriétaires de chiens affirmaient être capables de financer la vaccination antirabique canine annuelle. Les principaux canaux d’information et de communication sur la rage utilises par les participants étaient l’entourage (41,8 %), l’école (33,4 %) et les médias (24,8 %). Quant à l’étude épidémiologique rétrospective, qui s’est appuyée sur les documents disponibles au sein des structures nationales, elle a mis en évidence qu’en moyenne 4172 cas de morsures étaient enregistrées chaque année, que seules 31,7 % des personnes mordues avaient bénéficié d’une prophylaxie postexposition complète et que 68 % des animaux testes étaient positifs au virus rabique. Ces études confirment la présence du risque rabique dans le pays et montrent la nécessite de renforcer l’éducation et l’engagement communautaire ainsi qu’une étroite collaboration entre les secteurs de la santé animale et de la santé humaine pour une gestion intégrée des cas de morsures.

Abstract Abstract 897 Sickle cell disease is a complex process involving biophysical and biological phenomenon such as microvascular occlusion due to rigid sickle erythrocytes, hemolysis, and aberrant cellular interactions involving endothelial cells and sickle erythrocytes and leukocytes. Indeed, a key aspect of sickle cell pathophysiology is endothelial cell dysfunction. Cardiovascular research in recent years has shown that endothelial cells biologically respond to the local mechanical environment, particularly to the changes in the applied shear stresses (Chiu and Chien, Physiological Reviews, 2011). Interestingly, no studies investigating how the biophysical alterations in sickle cell disease may directly affect endothelial function have been published. The classic view has been that vaso-occlusion is simply due to sickled erythrocytes becoming stuck in microvasculature at low oxygen tensions leading to decreased blood flow and tissue ischemia. However, the mechanical aspects of sickle cell vaso-occlusion themselves, that is, the physical phenomenon of sickling erythrocytes tightly packed in an occluded blood vessel, may directly affect endothelial biology and lead to dysfunction. We hypothesize that these pathologic forces induced by sickling erythrocytes directly lead to dysfunction of endothelial cells, which are mechanosensitive, and contribute to sickle cell pathophysiology. However, these sickling-induced forces and their effects on endothelial cells have been difficult to measure, in part due to a lack of available tools. To that end, we have developed two microfluidic tools to assess the role of sickle-cell vaso-occlusion on endothelial cells. The first device is an in vitro microfluidic platform featuring microchannels the size of post-capillary venules (30 μm) with human endothelial cells cultured within and completely lining the entire inner surface of those microchannels (Figure 1A). This “microvasculature-on-a-chip” enables the visualization of blood cell-endothelial cell interactions during vaso-occlusion under a controlled hemodynamic environment and provides a platform to study the effect of vaso-occlusion on endothelial cells. To date we have characterized this “endothelialized” microfluidic device, showing that endothelial cells are confluent using anti-VE-cadherin immunostaining and adequately generate nitric oxide. Furthermore, we have flowed blood samples from patients with sickle cell disease and found that hydroxyurea treatment both reduces the number of occlusions and increases the mean velocity of the blood traveling through the device, as expected (Figure 1B–E). To decouple whether it is a biochemical or biophysical phenomenon that causes endothelial cell dysfunction during vaso-occlusion, a second micromechanical device was created to quantitatively measure the forces generated by sickling events. The device captures whole blood and will deform outward when forces are applied by the sickle erythrocytes as shown in Figure 2. The membrane above the sickle cells has been coated with 2 μm fluorescent beads which will change focus during deflection. Deflections of one or two beads indicates that a single sickle cell is locally applying force, whereas deflections of large numbers of beads indicates that the cells are collectively applying a pressure to the membrane. The device has been fully fabricated and loaded with blood cells. An accompanying experimental setup enabling the deoxygenation of the device coupled with microscopy has also been created and preliminary tests show successful deoxygenation of sickle erythrocytes from patients with hemoglobin SS disease and the Berkeley sickle cell mouse model. By combining insights gained from each device, future work will determine how the mechanical process of sickling and vaso-occlusion directly affect endothelial function and will lead to a new understanding of sickle cell pathophysiology. Sickle cell vaso-occlusion will be induced in the “endothelialized” microfluidic device while monitoring nitric oxide production and the upregulation of inflammatory markers, such as adhesion molecules and free radicals. The second device will provide quantitative numbers of forces produced by sickling erythrocytes, leading to experiments in which these forces are applied to endothelial cells while monitoring the same metrics. This is a case of a 68-year-old male who presented with a chief compliant of a testicular mass, which was discovered to be a metastatic lesion of undiagnosed renal cell carcinoma. A computed tomography scan revealed a large right renal mass and multiple pulmonary metastasis. Shortly after diagnosis, the patient was initiated on systemic therapy and received a cytoreductive nephrectomy. We discuss the details of this case as well as a pertinent review of metastatic renal cell carcinoma to the testes.

Des vaccins contre la fièvre Q ont été testés sur des chiens de races croisées en utilisant des cellules entières de Coxiella burnetii inactivées à la formaline dans la phase I (CEI) ou phase II (CEII) ou le résidu obtenu par extraction par chloroforme/méthanol (RCM) de cellules en phase I. Le vaccin CEI mélangé (1 : 1) à l'adjuvant incomplet de Freund (AIF) a provoqué des réponses immunitaires humorales aux antigènes des phases I et II, comme il a été mesuré par le test de microagglutination. Le vaccin RCM mélangé (1 : 1) à l'AIF a engendré des titres d'anticorps spécifiques aux antigènes de phases I et II plus élevés que le vaccin CEI. Le vaccin CEII a produit seulement des anticorps contre des antigènes de phase II. La durée d'un érythème et d'une induration après des tests dermiques avec des antigènes de Coxiella burnetii, fait penser à une immunité cellulaire. Les résultats des tests dermiques montrent que le vaccin RCM est le meilleur choix comparé aux vaccins CE, étant donné l'absence de formation tardive des granulomes par le premier. D'autres études seront nécessaires pour déterminer l'origine des réactions indésirables et pour évaluer l'efficacité des vaccins contre la coxiellose des chiens.

This study explicates and empirically tests the implication of gender in the C-Suite for corporate governance. In particular, we investigate the impact of the Chief Financial Officer’s (CFO) gender on informational asymmetry. The results document that firms hiring female CFOs experience an improvement in the level of transparency, represented by a reduction in the bid-ask spread and an increase in share turnover, relative to those hiring male CFOs. Additionally, the evidence shows that the impact of female CFO representation is more pronounced among firms with high agency costs. The results are robust to a series of robustness tests and even after including gender diversity of the board and of the C-Suite

Her sayıda yapmaya çalıştığım gibi bu sayıda da dergimize gönderilen makalelerin niteliğinin arttırılması amacıyla gözlemlediğim bazı noktalar üzerinde durmak istiyorum. Araştırmanın beklenen amacına ulaşabilmesi için geçerli ve güvenilirliği belirlenmiş olarak toplanan verilerin uygun çözümleme yöntemleri kullanılarak analiz edilmesi gerekmektedir. Örneğin, birbiriyle ilişkili iki ya da daha fazla bağımsız değişkenin bulunduğu bir araştırmada, grupların bağımsız değişkenler açısından ne derece farklılaştığını belirlemek için bağımsız gruplar t testi ya da varyans analizi yapmak yerine verileri manova ile analiz etmek daha uygun olacaktır. Ancak burada dikkat edilmesi gereken önemli bir nokta, parametrik testlerin kullanımında gerekli önkoşulların test edilme gereğidir. Bilindiği gibi parametrik istatistik tekniklerinin uygulanabilmesi bazı koşullara bağlıdır. Bu koşulların test edilerek verilerin uygunluğu analize başlamadan belirlenmelidir ve verilerin analizi kısmında bu koşulların sağlanıldığı bilgisine yer verilmelidir. Eğer parametrik testlerin koşulları sağlanamıyorsa uygun bir nonparametrik test seçilmelidir. Analiz edilen verilerin yorumlanmasında sadece olasılık değerlerini (p) kullanmak yeterli olmamaktadır. Zira olasılık (p) değeri verilerin toplanıldığı örneklem sayısına duyarlı bir değerdir. Ortalamalar arasındaki aynı farklılık küçük örneklemlerden elde edilen verilerin analizinde istatistiksel olarak anlamlı çıkmazken daha büyük bir örneklemden elde edilen verilerin analizinde anlamlı çıkabilmektedir. Bu nedenle farklılık ya da ilişkilerin anlamlandırılmasında etki büyüklüğünün hesaplanmasında yarar vardır. Etki büyüklüğü bağımsız değişken ya da değişkenlerin bağımlı değişkenler üzerindeki etkisini belirlemek için daha uygun ve etkili bir yöntemdir. Etki büyüklüğü değerleri veri analizi programlarında kolaylıkla hesaplanabilmektedir. Bu sayıda, hakem değerlendirme süreci tamamlanan ve daha önce doi numarası verilen altı makale yer almaktadır. Eğitim bilimlerinin çeşitli alanlarında yapılan bu çalışmaların siz değerli bilgi üretici ve tüketicilerine yararlı olmasını ve eğitim uygulamalarına katkı yapmasını diliyor, gelecek sayıda buluşmak dileğiyle saygılarımı sunuyorum.

Background: Communication failure during handover is the main cause of the patient's injury and is the root cause of 65% of the sentinel incident. Bad handover is assumed to have correlation with clinical diagnosis inaccuracies, delay in diagnosis, delay in laboratory tests, treatment errors, inconsistent or misinterpretation of results, duplication of tests, increase in complications of the Hospital.Purpose: This study aimed to determine the factors related to the quality of the patient's handover in inpatient room of the hospital.Methods: This research used a correlational analysis study with a cross sectional approach. Data retrieval use questionnaires and observation sheets. The number of samples on this study is 79 nurses taken in random sampling techniques. Statistical analysis used is a test of spearman rank.Results: The result of this research revealed that most of the respondents (69.6%) are diploma graduates, and 72.2% of respondents have a working time of < 5 years. Almost all respondents (93.7%) have good motivation, and 94.9% of respondents have good handover quality. The last educational variable (p = 0.388) and the nurse’s working time (P = 0.209) are not related to the quality of the handover, while the motivation (P = 0.000) and the implementation of the chief nurse’s supervision (P = 0.000) relate to the quality of the handover between the shifts in the inpatient room of the hospital.Conclusion of this study is the motivation and the implementation of the chief nurse’s supervision relate to the quality of the handover between the shifts in the inpatient room of the hospital. Hospital’s management is expected to raise the motivation of nurses in conducting handover and enhancing the supervision of the chief nurse to improve the quality of the patient's handover and minimize the errors in information exchange conducted by nurses

(1) Background: Toxoplasmic lymphadenitis (TL), caused by the protozoan Toxoplasma gondii, is a worldwide zoonosis. We report a case of TL in the head and neck region diagnosed using ultrasound (US)-guided fine needle aspiration cytology (FNAC), serological tests, and pathological findings. (2) Case Presentation: A 51-year-old female with a chief complaint of a left posterior neck mass that had been growing for approximately 2 weeks. TL was confirmed by histopathological examinations and serological tests. US-guided FNAC and en bloc resection of the lymph node were performed. The diagnosis was confirmed as TL in the neck. (3) Conclusions: We suggest that US-guided FNAC should be considered as the first-line test for assessing a tiny mass before a definitive treatment is chosen.

AbstractObjectives:The New Mexico-1 Disaster Medical Assistance Team (NM-1 DMAT) has responded to more disasters due to hurricanes than disasters from any other type of event. To assess whether defined patient needs may be applied to future hurricanes, the patient needs after Hurricanes Andrew and Iniki were compared. The study question was, “Did patient medical needs differ after these hurricanes?”Methods:Design: Retrospective cohort review. Subjects: All patients evaluated by NM-1 DMAT following Hurricanes Andrew and Iniki. Observations: Age, past medical history, chief complaint, diagnosis, diagnostic tests, treatments, triage level, and disposition. Age was analyzed using Student's t-test, other data were analyzed using the chisquare test.Results:A total of 1,056 patients were evaluated. Age distributions did not differ between events. More patients had co-morbidities after Hurricane Andrew. The only difference in chief complaint was that more patients complained of “cold” symptoms following Hurricane Iniki. The only differences in diagnoses were for upper respiratory infections, which were diagnosed more often after Hurricane Iniki. There were no differences in the administration of tetanus toxoid, antibiotics, or analgesics. Patients evaluated after Hurricane Andrew had more diagnostic tests performed and a higher illness/injury acuity. The proportion of the total number of patients conveyed to a hospital did not differ.Conclusion:Patient needs were similar after Hurricane Andrew and Hurricane Iniki and may be applicable for predicting the needs of patients for future hurricanes.

This study aims to examine and obtain empirical evidence of the influence of female Chief Executive Officer (CEO), female Chief Financial Officer (CFO), female Board of Commissioners on accounting conservatism in all companies listed on the IDX in 2017-2019. The sample of this research is all companies listed on the Indonesia Stock Exchange (IDX) during the 2017-2019 period. The sample selection in this study using purposive sampling method, obtained 124 companies and multiplied during the year of the study, so that the total sample in this study was 372. The dependent variables used in this study were accounting conservatism, female CEO, female CFO, and female Board of Commissioners as independent variable. Tests in this study using analysis tools E-Views (Econometric Views) and for data analysis in this study using linear regression analysis. The results of this study indicate that female CEOs, female CFOs, and female Board of Commissioners have no significant effect on accounting conservatism

Riser system is a key equipment for offshore oil and gas development. When conducting riser design, fatigue failure mode is the chief one among the many failure modes which should be taken into account. To assess the fatigue performance of riser accurately, it is necessary to conduct fatigue tests. Resonant bending fatigue test is one effective method for fatigue tests of risers. In this paper, the principle of resonant bending fatigue test and test procedures are presented firstly, and then a finite element model using ABAQUS is created to simulate the resonant bending fatigue test, and the results from the finite element model are compared with the experimental results. The good agreements between the FEM results and experimental results verify the accuracy of the finite element model in this paper.

In the present paper, these hull forms are introduced, followed by a systematic presentation of comparative results illustrating the scale effects and the extrapolation diagrams, the influence of the block coefficient, the length to beam ratio, the type of stern, the shape of chine and the fore and aft trim on viscous resistance (form factor k), and wave-making resistance (wave-making resistance coefficient CW). The findings on the effects of some appendages such as the rudder, the heel of the rudder and the stern tube on the viscous resistance (form factor k), and the total resistance (total resistance coefficient CTs) are also illustrated. Finally, a general discussion of the results obtained from the resistance tests, wake measurement tests, and flow visualization tests are presented.

PurposeWe examine how the net share purchases of top executives of acquiring firms, specifically the Chief Executive Officer (CEO) and the Chief Operating Officer (COO), can impact shareholder perceptions of a merger and acquisition (M&A) around the announcement time.Design/methodology/approachRegression tests using the post-announcement cumulative returns as the dependent variables, and CEO and COO net purchases as independent test controlling for the net purchases of all other insiders, COO and CEO ownership, exercised options, unexercised exercisable options, merger type, pre-announcement firm size, past performance, industry growth, industry instability, year and industry fixed effects. The regression tests are used for various sub-samples (i.e. non-contemporaneous events, duality, operational complexity, economic conditions).FindingsWe find that overall shareholders value the COO's net purchases before the announcement but not those of the CEOs. If the COO is also the CEO, then executive buy-ins appear to have a negative reaction from the shareholders. When the firm has many business segments or when the announcement is made in an economic recession, the COO's net purchases do not have a positive influence on the shareholders.Originality/valueWe are the first to provide evidence that investors pay attention to the COO around M&A announcements. In the age of celebrity CEOs, who can instantaneously change the stock price with one press release, having another executive that can shape the opinion of investors can diversify the agency risk.

Abstract:
A 40-year-old male presented to our outpatient department with the chief complaint of a painless mass on his right testis with gradual size increase over the past two months. Physical examination and ultrasound revealed a firm and nontender mass both on the right and on the left testis. The only elevated biomarker was b-hcG (24,7 mIU/mL) and computer tomography (CT) did not reveal any pathology. Bilateral high orchiectomies were performed, without previous frozen storage of the sperm. Histology proved typical seminoma of the left testis and embryonal carcinoma of the right testis. He received two cycles of adjuvant combination chemotherapy with bleomycin, etoposide, and cisplatin. Six months after the operation no residual tumor or recurrence was observed.
