Abstract:

Four to five per cent of the Western population suffers from chronic diarrhoea (defined as the passage of >3 stools per day, for >4 weeks), with irritable bowel syndrome (IBS) being the commonest cause in 20–40-year-old patients. It is the commonest reason for referral to secondary care gastroenterology clinics. The list of possible causes of chronic diarrhoea is long but, in the absence of rectal bleeding, loss of weight, or abnormal blood tests, it is unlikely to be due to a serious illness. Laboratory investigations should include serum glucose, electrolytes, renal and liver tests, full blood count, thyroid tests, a coeliac antibody test, C-reactive protein (CRP) measurement to check for systemic inflammation, faecal fat and elastase estimation to check pancreatic exocrine function, faecal microscopy, and culture, although this is insensitive for giardiasis. In young patients with typical features of IBS, these laboratory investigations can be abbreviated to include only glucose, electrolytes, the coeliac antibody test, CRP measurement, and thyroid tests. Endoscopic examination of the large and small intestines is generally only required where there is a suspicion of coeliac disease, chronic giardiasis, microscopic colitis, inflammatory bowel disease, or colorectal cancer. A therapeutic trial of metronidazole for giardiasis is justified where this seems a likely diagnosis.

Abstract:

Infrastructure services in energy, transport, water, and telecommunications services underpin the wealth of modern nations. Yet inefficiencies abound. In developing nations hundreds of millions of people lack access to modern infrastructure services. Globally, as much as 40 percent of expenditures on infrastructure may constitute waste, equivalent to some 1 to 2 percent of global GDP. Natural monopoly features and sunk costs provide incentives for the parties to infrastructure ventures to play ransom games. Particularly in developing economies prices are often well below cost. Hence investors shy away and access remains limited. Government involvement in project choice and implementation may lead to ‘white elephants’ and mismanagement. Where head-to-head competition can be introduced, such as in modern telecommunications systems, the syndrome can be kept in check. Yet where such competition is not feasible, policymaking and inevitable price and quality regulation remain a challenge, requiring patient effort at arm’s-length from day-to-day political pressures.

Renegade physician Paracelsus compared St. John’s Day dances to earthquakes, epileptic tremors, and tics. This ecosophical and vitalist concept, according to which all sorts of bodies echo one another’s shaking motions, countered long-held academic prejudice against witchcraft; neither choreomaniacs nor witches were subject to supernatural forces. Rather, the ‘vital spirits’ caused limbs, like branches, to shake. What’s more, dancing was now thought to cure dancing, and municipal authorities keen to keep a Strasbourg dancing mania in check employed guards to help wear dancers out—while exorcism associated religious, municipal, and medical experts. The translatio or passage from collective to individual disorder, epitomized in St. Vitus, now patron saint of all dance maniacs, continued throughout the eighteenth and nineteenth centuries, as neurologists’ theories of chorea, epilepsy and insanity aligned popular carousing with individual quaking motions. Choreomania came to signal the epidemic proliferation of what Giorgio Agamben has styled purposeless gesture.

Abstract:

Since the communautarisation of the Schengen acquis, the EU is meant to build a system of ‘integrated border management’ (IBM) to help ensuring the administration of migratory flows ‘at all their stages’. The idea is that effective entry control cannot be based solely on checks at the external borders of the Member States but ‘must cover every step taken by a third country national from the time he begins his journey to the time he reaches his destination’. EU entry/pre-entry controls thus comprise a series of extraterritorial measures carried out abroad. This chapter describes this evolution in detail. It traces the origins and development of IBM, covering institutional, constitutional, as well as legal and political changes to the present day. The recognition that the ‘strengthening of European border controls should not prevent access to protection systems by those people entitled to benefit under them’ is introduced also at this stage, providing the starting point to the entire research.

Abstract:

Cynicism is usually seen as a provocative mode of dissent from conventional moral thought, casting doubt on the motives that guide right conduct. When critics today complain that it is ubiquitous but lacks the serious bite of classical Cynicism, they express concern that it can now only be corrosively negative. The Function of Cynicism at the Present Time takes a more balanced view. Re-evaluating the role of cynicism in literature, cultural criticism, and philosophy from 1840 to the present, it treats cynic confrontationalism as a widely employed credibility check on the promotion of moral ideals—with roots in human psychology. Helen Small investigates how writers have engaged with Cynic traditions of thought, and later more gestural styles of cynicism, to recalibrate dominant moral values, judgements of taste, and political agreements. The argument develops through a series of cynic challenges to conventional moral thinking: Friedrich Nietzsche on morality; Thomas Carlyle vs. J. S. Mill on the permissible limits of moral provocation; Arnold on the freedom of criticism; George Eliot and Ford Madox Ford on cosmopolitanism; Bertrand Russell, John Dewey, and Laura Kipnis on the conditions of work in the university. The Function of Cynicism treats topics of present-day public concern: abrasive styles of public argument, debasing challenges to conventional morality, free speech, moral controversialism, the authority of reason, and the limits of that authority, nationalism and resistance to nationalism, and liberty of expression as a core principle of the university.

Abstract:

The national security and civil liberties tensions of the World War II mass Japanese American internment (incarceration) link 9/11 and the 2015 Paris-San Bernardino attacks to the era in America darkened by accelerating discrimination against and intimidation of those asserting rights of freedom of religion, association, and speech, and one marked by increasingly volatile protests against racial and religious discrimination. This book discusses the broad civil liberties challenges posed by these past-into-the-future linkages, highlighting pressing questions about the significance of judicial independence for a constitutional democracy committed both to security and to the rule of law. First, the book portrays the present-day significance of the Supreme Court’s discredited yet never overruled 1944 Korematsu decision—a decision later found in the coram nobis cases to be driven by the government’s presentation of “intentional falsehoods” and “willful historical inaccuracies” to the Court. Second, the book implicates prospects for judicial independence in adjudging harassment, exclusion, and incarceration disputes in contemporary America and beyond. Third, and even more broadly for security and liberty controversies, the book engages the American populace in shaping law and policy at the ground level by placing the courts’ legitimacy on center stage. It addresses how critical legal advocacy and organized public pressure targeting judges and policymakers—realpolitik advocacy—at times can foster judicial fealty to constitutional principles while promoting accountability of the elective branches. Finally it addresses who we are as Americans and whether we are genuinely committed to a checks-and-balances democracy governed by the Constitution.

Abstract:

Most scientists now agree that some sixty-five million years ago, an immense comet slammed into the Yucatan, detonating a blast twenty million times more powerful than the largest hydrogen bomb, punching a hole ten miles deep in the earth. Trillions of tons of rock were vaporized and launched into the atmosphere. For a thousand miles in all directions, vegetation burst into flames. There were tremendous blast waves, searing winds, showers of molten matter from the sky, earthquakes, and a terrible darkness that cut out sunlight for a year, enveloping the planet in freezing cold. Thousands of species of plants and animals were obliterated, including the dinosaurs, some of which may have become extinct in a matter of hours. In Impact, Gerrit L. Verschuur offers an eye-opening look at such catastrophic collisions with our planet. Perhaps more important, he paints an unsettling portrait of the possibility of new collisions with earth, exploring potential threats to our planet and describing what scientists are doing right now to prepare for this awful possibility. Every day something from space hits our planet, Verschuur reveals. In fact, about 10,000 tons of space debris fall to earth every year, mostly in meteoric form. The author recounts spectacular recent sightings, such as over Allende, Mexico, in 1969, when a fireball showered the region with four tons of fragments, and the twenty-six pound meteor that went through the trunk of a red Chevy Malibu in Peekskill, New York, in 1992 (the meteor was subsequently sold for $69,000 and the car itself fetched $10,000). But meteors are not the greatest threat to life on earth, the author points out. The major threats are asteroids and comets. The reader discovers that astronomers have located some 350 NEAs ("Near Earth Asteroids"), objects whose orbits cross the orbit of the earth, the largest of which are 1627 Ivar (6 kilometers wide) and 1580 Betula (8 kilometers). Indeed, we learn that in 1989, a bus-sized asteroid called Asclepius missed our planet by 650,000 kilometers (a mere six hours), and that in 1994 a sixty-foot object passed within 180,000 kilometers, half the distance to the moon. Comets, of course, are even more deadly. Verschuur provides a gripping description of the small comet that exploded in the atmosphere above the Tunguska River valley in Siberia, in 1908, in a blinding flash visible for several thousand miles (every tree within sixty miles of ground zero was flattened). He discusses Comet Swift-Tuttle--"the most dangerous object in the solar system"--a comet far larger than the one that killed off the dinosaurs, due to pass through earth's orbit in the year 2126. And he recounts the collision of Comet Shoemaker-Levy 9 with Jupiter in 1994, as some twenty cometary fragments struck the giant planet over the course of several days, casting titanic plumes out into space (when Fragment G hit, it outshone the planet on the infrared band, and left a dark area at the impact site larger than the Great Red Spot). In addition, the author describes the efforts of Spacewatch and other groups to locate NEAs, and evaluates the idea that comet and asteroid impacts have been an underrated factor in the evolution of life on earth. Astronomer Herbert Howe observed in 1897: "While there are not definite data to reason from, it is believed that an encounter with the nucleus of one of the largest comets is not to be desired." As Verschuur shows in Impact, we now have substantial data with which to support Howe's tongue-in-cheek remark. Whether discussing monumental tsunamis or the innumerable comets in the Solar System, this book will enthrall anyone curious about outer space, remarkable natural phenomenon, or the future of the planet earth.
