Abstract:
The ability to characterize time-varying reservoir properties, such as the state of stress, has fundamental implications in subsurface engineering, relevant to geologic sequestration of [Formula: see text]. Stress variation, here in the form of changes in pore fluid pressure, is one factor known to affect seismic velocity. Induced variations in velocity have been used in seismic studies to determine and monitor changes in the stress state. Previous studies conducted to determine velocity-stress sensitivity at reservoir conditions rely primarily on laboratory measurements of core samples or theoretical relationships. We have developed a novel field-scale experiment designed to study the in situ relationship between pore-fluid pressure and seismic velocity using a crosswell continuous active-source seismic monitoring (CASSM) system. At the Cranfield, Mississippi, [Formula: see text] sequestration field site, we actively monitored seismic response for five days with a temporal resolution of 5 min; the target was a 26 m thick injection zone at approximately 3.2 km depth in a fluvial sandstone formation (lower Tuscaloosa Formation). The variation of pore fluid pressure was obtained during discrete events of fluid withdrawal from one of the two wells and monitored with downhole pressure sensors. The results indicate a correlation between decreasing CASSM time delay (i.e., velocity change for a raypath in the reservoir) and periods of reduced fluid pore pressure. The correlation is interpreted as the velocity-stress sensitivity measured in the reservoir. This observation is consistent with published laboratory studies documenting a velocity ([Formula: see text]) increase with an effective stress increase. A traveltime change ([Formula: see text]) of 0.036 ms is measured as the consequence of a change in pressure of approximately 2.55 MPa ([Formula: see text]). For [Formula: see text] total traveltime, the velocity-stress sensitivity is [Formula: see text]. The overall results suggest that CASSM measurements represent a valid technique for in situ determination of velocity-stress sensitivity in field-scale monitoring studies.

Abstract:
The present study investigates stability and motion of low angle dislocation boundaries in an array of precipitates. The model considers discrete dislocations and precipitates that are treated as impenetrable particles. Peach-Koehler forces, which originate due to the combined effect of dislocation-dislocation interactions and the applied stress, act the individual dislocations on. Both, the dislocation glide and the dislocation climb at elevated temperatures are taken into account. Results of the numerical study suggest that a critical applied shear stress (CASS) always exists which separates stable and unstable low angle boundary configurations. Varying particle size, interparticle spacing and density of dislocations in the boundary cause changes of the CASS that are systematically investigated. It is shown that the CASSs can considerably differ from the standard Orowan stress controlling the equilibrium of an isolated dislocation in a given microstructure. This result underlines the importance of long-range dislocation interactions that influence the high temperature strength of the precipitation-hardened alloys.

Abstract:
Background Computer-aided surgical simulation (CASS) has redefined surgery, improved precision and reduced the reliance on intraoperative trial-and-error manipulations. CASS is provided by third-party services; however, it may be cost-effective for some hospitals to develop in-house programs. This study provides the first cost analysis comparison among traditional (no CASS), commercial CASS, and in-house CASS for head and neck reconstruction. Methods The costs of three-dimensional (3D) pre-operative planning for mandibular and maxillary reconstructions were obtained from an in-house CASS program at our large tertiary care hospital in Northern Virginia, as well as a commercial provider (Synthes, Paoli, PA). A cost comparison was performed among these modalities and extrapolated in-house CASS costs were derived. The calculations were based on estimated CASS use with cost structures similar to our institution and sunk costs were amortized over 10 years. Results Average operating room time was estimated at 10 hours, with an average of 2 hours saved with CASS. The hourly cost to the hospital for the operating room (including anesthesia and other ancillary costs) was estimated at $4,614/hour. Per case, traditional cases were $46,140, commercial CASS cases were $40,951, and in-house CASS cases were $38,212. Annual in-house CASS costs were $39,590. Conclusions CASS reduced operating room time, likely due to improved efficiency and accuracy. Our data demonstrate that hospitals with similar cost structure as ours, performing greater than 27 cases of 3D head and neck reconstructions per year can see a financial benefit from developing an in-house CASS program.

This working paper analyzes the financial integration level of the recent CASSH acronym formed by the countries Australia, Canada, Hong Kong, Singapore and Switzerland, which have strong economies and homogeneous profiles in a social, demographic and financial context, being classified by UN as countries with very high human development. By analyzing the presence of common trends and cycles associated to the market indices most representative of CASSH stock exchanges, during the period between January 1998 and November 2010, using the methodological technique developed by Vahid and Engle (1993), it is evident that, during global economic stability periods the stocks of these economies are more influenced by trends than by cycles, being determined more by economic fundamentals, while in the crisis periods there is a higher influence of cycles, assuming the financial risk factors greater relevance in the composition of the indices returns. It is possible to identify that financial markets analyzed have distinct longterm scenarios governed by four common trends, two leading to a positive trajectory, one to a pessimistic scenario and the other negative initially, but after the 2007 crisis, it recovers following a positive trajectory. It is notorious that they react differently to short-term shocks and with different intensities, mainly due to the behavior of Canadian cycle that correlates negatively with the others and with the common cycle. Through Granger causality test, the common trend pessimist can only be provided by the Swiss index, while the index of Hong Kong appears as the only one able to predict the common cycle.
Este artigo analisa o nÃvel de integraÃÃo financeira do recente acrÃnimo CASSH, formado pelos paÃses AustrÃlia, CanadÃ, Hong Kong, Singapura e SuÃÃa, os quais possuem economias fortes e perfis homogÃneos sob um contexto social, demogrÃfico e financeiro, sendo classificados pela ONU como paÃses com desenvolvimento humano muito elevado. Resumo: Objetivos: avaliar a transmissão vertical (TV) em gestantes portadoras de hepatite B crônica, em um serviço universitário. Sujeitos e Método: foram analisadas as sorologias para hepatite B de todas as gestantes atendidas no serviço entre 2000 e 2005, identificando-se as HbsAg +; nessas foi realizado levantamento de prontuários, avaliando a presença do marcador de replicação viral (HbeAg positivo), imunoprofilaxia neonatal e taxa de TV. Análise de dados: foi avaliada a proporção de casos com HbsAg+ e nessas a presença do HbeAg. Para as portadoras de hepatite B, analisaram-se características clínicas e epidemiológicas através de frequências simples e a presença de TV. Resultados: entre 2000 e 2005 foram rastreadas para hepatite B no CAISM 5638 mulheres; dessas 28 (0,5%) apresentavam HbsAg+, definindo-se como portadoras crônicas. Não se encontrou nenhuma com replicação viral (HbeAg+). A idade média foi de 25 anos, com escolaridade média de sete anos, sendo 57% de brancas. O número de gestações médio foi de dois, sendo 52% de nulíparas. A categoria de exposição foi ignorada em 20; em quatro a via foi a sexual, em duas por TV e em duas por uso de drogas. A média de Idade gestacional ao parto foi de 38 semanas, com uma taxa de cesárea de 42%. O peso médio ao nascimento foi de 3094g e todos os recém-nascidos apresentaram boas condições de vitalidade e receberam imunoprofilaxia neonatal (vacina e imunoglobulina específica) nas primeiras horas de vida. Não houve TV. Conclusões: Nas gestantes atendidas no período, a prevalência de hepatite B crônica foi de 0,5%. Todas as crianças receberam imunoprofilaxia neonatal nas primeiras horas de vida e não ocorreu nenhum caso de TV, reforçando que para as gestantes sem replicação viral, as medidas de imunoprofilaxia neonatal protegeram a totalidade de seus recém-nascidos
Abstract: The purpose of this paper is to evaluate mother-to-child transmission of chronic hepatitis B in a university hospital. Subjects and methods: Hepatitis B serologic studies were pooled from all pregnant women referred to this prenatal service from 2000 to 2005. HBsAg positive patients were selected and, for those, clinical, laboratory and epidemiologic data were analyzed, including presence of HBeAg marker, immunoprophylactic procedures for the newborn and mother-to-child transmission rates. Data analysis: HBsAg carriers were characterized for clinical and epidemiologic factors associated with mother-to-child transmission. Results: Between 2000 and 2005, 5638 pregnant women were referred to high-risk prenatal care at our facility; of these, 28 women (0,5%) were HbsAg+ ¿ defined as chronic Hepatitis B virus (HBV) carriers. None of these were seropositive for HBeAg. Mean age was 25 years with a mean of 7 years of formal education and 57% were white; 52% were nulliparous. Exposure to hepatitis B virus was ignored in 20 women, sexual in 4, from mother-to-child transmission in 2 and associated with drug use in 2. Mean gestational age at delivery was 38 weeks with cesarean delivery in 42% of women. Mean weight at birth was 3094g and all newborns presented with good vitality and received immunoprophylactic procedures. There were no cases of mother-to-child transmission. Conclusion: Among all pregnant women seen at this tertiary high risk prenatal care facility between 2000 and 2005, chronic HBV infection was detected in 0,5% of patients. All newborns received immunoprophylaxis during the first hours after delivery and no case of mother-to-child transmission was detected. Our findings support that, among pregnant chronic HBV carriers without serologic evidence of active viral replication, immunoprophylactic measures are effective in preventing mother-to-child transmission in all instances

This study is based on the everyday lives as Alain Mabanckou describes them through the different characters of the novel Verre Cassé (2005). This novel describes the African society during the postcolonial period. In this society, leaders are struggling to stay in power as long as possible, while the people live in poverty and utter misery. Many people experience life without taking action. Everyone must fight for his survival. In this work, I will investigate how the life of ordinary citizens during the postcolonial period in African cities and the place of women in man's relationship is describe in Mabanckou’s novel. I focus on the link between postcolonialism and feminism. At the end of this work, I found that the Mabanckou’s novel Verre Cassé demonstrates that life of many citizens in African cities is still difficult despite the independence. These newly independent countries face many new problems like dictatorship, corruption and poverty. Many women prostitute themselves to meet their needs in this novel. Alain Mabanckou points out in his novel that men continue to assert their superiority over women in this postcolonial society.

Introdução - A Lei 9.656/1998 fundamenta o marco legal do setor privado da saúde no Brasil. Igualmente importante, a Lei 9.961/2000 criou a Agência Nacional de Saúde Suplementar (ANS) para fiscalizar e regulamentar a relação entre operadoras e beneficiários de planos. A partir de então houve um crescimento expressivo no número das ações judiciais, fenômeno denominado judicialização da saúde. Objetivo - Conhecer e analisar o montante e os motivos das ações judiciais relacionadas às coberturas assistenciais e avaliar o impacto econômico-financeiro em uma entidade de autogestão. Método Análise de banco de dados das ações judiciais do período 1998 a 2009. A base de dados constitui-se de 7.271 ações cíveis ativas e baixadas. Desse total foram selecionadas 3.569 relacionadas a coberturas assistenciais, cujos dados foram organizados em planilha eletrônica. Analisou-se ainda a população total e o impacto econômico-financeiro a partir de informações de balanço anual. Resultados Várias constatações importantes foram reveladas, como a evolução crescente do número de ações; o fato de a Bahia possuir 9,5 por cento da população assistida e responder por 33,4 por cento das ações totais; as gastroplastias aparecerem como o objeto mais importante 427 ações (12 por cento do total); o impacto econômico-financeiro atingindo em média 1 por cento da receita total, mas 32 por cento do resultado (superávit) do exercício de 2009, entre outros achados. Conclusão o marco legal foi determinante para os beneficiários recorrerem ao Judiciário por coberturas assistenciais. O fenômeno da judicialização da saúde impacta de maneira significativa a Autogestão estudada. Boa parte das decisões judiciais parece não observar critérios técnicos relacionados à homologação de novas tecnologias e aos protocolos médicos, o que vem ensejando uma atuação mais ativa do Conselho Nacional de Justiça no encaminhamento de questões relacionadas à saúde
Introduction - Law 9.656/1998 moved the legal framework of private health sector in Brazil. Equally important, 9.961/2000 Act created the National Agency for Supplementary Health (ANS) to oversee and regulate the relationship between providers and beneficiaries of plans. Since then there has been significant growth in the number of lawsuits, a phenomenon called the legalization of health. Objective - To investigate and analyze the amount and the reasons for lawsuits related to care coverage and to evaluate the economic and financial impact on a self-insured entity. Methods - Analysis of database of lawsuits in the period 1998-2009. The database consists of 7271 civil actions, active or downloaded. Of this total were selected 3569 related to care coverage, and data were organized into a spreadsheet. It was also analyzed the total population and the economic and financial impact from the annual balance sheet information. Results - Several important findings were revealed as a growing trend in the number of shares, the fact that Bahia has 9.5 per cent of the assisted population and account for 33.4 per cent of the total shares, the gastroplasty appears to be the most important object - 427 shares (12 per cent of total), the economic and financial impact means on average 1 per cent of total revenue, but 32 per cent of the result (surplus) for the year 2009, among other findings. Conclusion - the legal framework was crucial for the beneficiaries to avail themselves of judicial assistance coverage. The phenomenon of legalization of health impacts significantly the self study. Much of judicial decisions seem not to notice technical criteria related to approval of new medical technologies and protocols, which have demanded a more active role of the National Council of Justice in addressing health-related issues

The career of this great huguenot sailor, born into the common people, is in keeping with the reign of Louis XIV. A book keeper on a ship of the west indies company, he becomes a merchant captain, getting involved in the slave trade in the Antilles. He is the founder of the first colonial French empire in Senegal. In 1686 he joins the king's navy and wins renown during the taking of St Christopher in 1689 and the liberation of Guadeloupe in 1691. As the governor of santo domingo, he leads his buccaneers against Jamaica in 1694, then against Cartagena of Indies in 1697. He allows the french side to have a hand in the sugar cycle which just like the illegal trade with the spaniards, is the origin of his fortune. He gets back to france in 1700 and negotiates the asiento with philippe v the following year. During the succession war in Spain, he defends the catholic king's american colonies three times, bringing back to Madrid the necessary amount of silver for supporting the effort of war. Resumo: Objetivo geral: Analisar os indicadores de qualidade da triagem auditiva neonatal realizada na unidade de terapia intensiva do CAISM/Unicamp, considerando dois protocolos distintos, assim como o diagnóstico audiológico. Objetivos Específicos: estudar as características dos neonatos nos grupos estudados; analisar a porcentagem de neonatos triados, assim como os indicadores de risco para surdez, considerando os grupos estudados; estudar o tipo de alta dos neonatos que não realizaram a triagem; estudar o momento que a triagem foi realizada, assim como seus resultados nos grupos estudados; analisar a relação entre os indicadores de risco para surdez presentes e o resultado da triagem auditiva neonatal; analisar o diagnóstico audiológico nos grupos estudados. Sujeitos e Métodos: Foram avaliados os neonatos internados na unidade de terapia intensiva e/ou cuidados intermediários do CAISM/Unicamp, que permaneceram por mais de 48 horas e nasceram no período de março/2011 a novembro/2012. Os neonatos foram reunidos em dois grupos: GI - utilizado o protocolo 1 onde os neonatos que falharam na triagem foram encaminhados para o diagnóstico e GII - utilizado o protocolo 2 onde os neonatos que falharam foram encaminhados para o reteste, quando confirmado o resultado foi encaminhado para o diagnóstico. Para realizar a triagem auditiva, utilizou-se o Potencial Evocado Auditivo de Tronco Encefálico - automático com estímulo tipo clique na intensidade de 35 dB. Procurou-se realizar a avaliação antes da alta hospitalar. Resultados: No GI-teste 84,7% realizaram triagem auditiva neonatal, no GII-teste 75,8%. Entre os neonatos triados, no GI-teste 81,4% passaram e no GII-teste 85,6%. No diagnóstico, 36,4% dos neonatos do GI possuem perda auditiva condutiva, neurossensorial ou neuropatia, já no GII 100% possuem perda auditiva neurossensorial ou condutiva. Os indicadores de risco com maior taxa de falha foram a anóxia neonatal e infecção congênita. Conclusão: O protocolo 2 mostrou-se fundamental nessa população, por reduzir significativamente o índice de encaminhamento de falso-positivo para o diagnóstico
Abstract: Objectives: Analyze the newborn hearing screening in a university hospital's neonatal intensive care unit (NICU), considering screening percentage, the newborn age at screening, as well as the reference rate for audiological diagnosis. Design: transversal study. Methods: This study comprised babies born from March 2011 to November 2012 who stayed hosted at the neonatal intensive care unit of an university hospital for more than 48 hr. Aiming to evaluate the newborns, it was used automated auditory brainstem response (AABR) with stimulation at 35 dB nHL levels, resulting in "pass" or "fail". The evaluation was planned to occur before hospital discharge. In GI group, newborns who showed FAIL status at hearing screening were submitted for audiological diagnosis. In GII group, newborns who showed FAIL status at hearing screening were submitted to retest; after a confirmation of that failure, they were submitted for diagnosis. Results: in GI-test, 84.7% were screened; in GII-test, 75.8%; and in GII-retest, 66%. In GI-test, 81.4% passed the screening; in GII-test, 85.6%; and in GII-retest, 76%. During the diagnosis phase, 36.4% of newborns screened from GI showed conduction disorders, neural hearing loss or auditory neuropathy/dyssynchrony; 100% of those screened from GII showed hearing loss. Conclusion: Some actions should take place to increase the hearing screening range. The retest showed essential to this population, since it significantly reduced the false-positive index for diagnosis

Abstract This study conducts failure tests using a simulated specimen to investigate the effect of thermal aging on the deformation and failure behaviors of system, structure, and components (SSCs) of nuclear power plants (NPPs) made of cast austenitic stainless steels (CASSs) under excessive seismic loads. Both unaged and thermally aged CF8A CASSs were used for the experiment, and the large cyclic loads in the form of displacement-control and load-control were applied at a quasi-static displacement rate. Displacement-controlled tests were performed at room temperature (RT) and 316°C and load-controlled tests were performed at RT. The results show that the deformation behaviors of aged CF8A CASS under both types of cyclic load are almost the same as those of unaged CF8A CASS. The thermal aging slightly promotes the failure of CF8A CASS under displacement-controlled cyclic loads, but the failure of specimen still occurs under the cyclic load levels several times higher than the load of the design basis earthquake. Under load-controlled cyclic loads, thermal aging retards the failure of CF8A CASS. Consequently, the thermal aging has no apparent negative effect on the deformation and failure behaviors of CASSs under large cyclic loads, even if it considerably changes the strength, ductility, and fracture toughness of CASSs.

This study conducted J-R fracture toughness tests using aged CF8A cast austenitic stainless steel (CASS) under dynamic and cyclic loading conditions at room temperature (RT) and 316°C and investigated the effect of seismic loading characteristics on the fracture behavior of age-related degraded material. For comparison, J-R fracture tests were also conducted on unaged CF8A CASS. CF8A CASS was made as a static casting, and it was aged thermally at 400°C for 175 days, which is equivalent to thermal aging at the operating temperature of nuclear power plants (NPPs) for 32 effective full power years (EFPYs). Monotonic J-R tests were conducted at both quasi-static and dynamic loading rates, and cyclic J-R tests were conducted at a quasi-static loading rate. The results showed that the fracture resistance of aged CF8A CASS under monotonic load was lower, by ∼35%, than that of unaged CF8A CASS, regardless of test temperatures and loading rates. The dynamic loading effect on fracture behavior was almost negligible for both unaged and aged CF8A CASSs at RT and 316°C. Cyclic loading reduced J-R fracture toughness of unaged CF8A CASS considerably at both test temperatures. Such a cyclic loading effect on fracture behavior was still observed from aged CF8A CASS. Thus, we conclude that the dynamic and cyclic loading effects on fracture behavior of CF8A CASS were not altered by thermal aging at the operating temperature of NPPs for 32 EFPYs.

Cast austenitic stainless steels (CASSs) are used in the cooling system of light water reactors (LWRs) for components with complex shapes, such as pump casings, valve bodies, coolant piping, etc. The CF grades of CASS alloys are the cast equivalents of 300-series stainless steels (SSs) and show excellent mechanical properties and corrosion resistance. In contrast to the fully austenitic microstructure of wrought SSs, CASS alloys consist of a dual-phase microstructure of delta ferrite and austenite and are vulnerable to thermal aging embrittlement. The service performance of CASS alloys is of concern after long-term exposure to high-temperature coolant. In this work, we studied the effects of thermal aging and ferrite content on the fracture resistance of CASS alloys. Crack growth rate and fracture toughness J–R curve tests were performed on aged and unaged CASS alloys in simulated light water reactor environments. The impact of thermal aging on the cracking susceptibility was investigated and the effect of ferrite content was evaluated. Significant embrittlement was observed in the CASS alloys after aging at 400°C. To understand the embrittlement mechanism, microstructural characterizations were performed with transmission electron microscope. The thermal aging produced G-phase precipitates and phase separation in the ferrite, but did not affect the microstructure of austenite. Consequently, the ferrite was hardened considerably after thermal aging while the hardness of austenite phase remained unchanged. The difference in hardness created a high incompatible strain at the interface between ferrite and austenite, leading to fracture at phase boundaries.
