Abstract:
The increasing usage of social networking sites, such as Facebook has changed the way that corporations and customer interact with each other. New ways of customer-corporation interactions evolve constantly, enabling customers to participate and co-create value with the corporations. An organisation that has taken this a step further is Skånetrafiken, which uses Facebook as a channel for its customer service. This thesis focuses in the interactions that occur between Skånetrafiken and its customers, which in this case takes place on Facebook.   The purpose of this thesis is to explore the what, how and why customer and corporation interaction occur on Skånetrafiken's Facebook page. In order to approach the phenomenon of co-creation, we conducted a case study as a research design and strategy. Data collection methods we used were observations on Skånetrafiken’s Facebook page, an interview and a focus group discussion.     The findings indicate that there must be an engagement for the co-creation process to begin and that the co-creation process is continuous rather than a fixed model. It further turned out that different types of interactions occur, not only customer-to-corporation, but also customer-to-customer and customer-to-employee interactions. The limitations of the study imply that the findings cannot be generalised on all organisations since only one organisation have been examined. This thesis further gives a practical implication by presenting an applicable framework for co-creation on Facebook. Additionally, few if any have previously studied phenomenon of co-creation on Facebook, which contributes to the original value of the thesis.

Abstract:
Scholars have long been interested in the complex relationships between personality and motivation. However, much of their understanding has been limited to The Big Five personality factors (namely, Openness, Conscientiousness, Extraversion, Agreeableness, and Neuroticism), and a proliferation of motivation constructs emanating from a large number of different theories and sub-theories. This study adds to the body of personality psychology and motivation science literature by examining the relationships between The Big Five personality factors and The MUSIC Model of Academic Motivation (MMAM) components (i.e., eMpowerment, Usefulness, Success, Interest, and Caring). The MMAM is comprised of five components that summarize the many instructional implications derived from motivation theories and research to provide instructors with a holistic, conceptual understanding of them. The results of this study may provide evidence as to whether or not the personality traits of college students influence their academic motivation beliefs. To obtain data, I surveyed college students using two self-report instruments. The first instrument, the Big Five Inventory (BFI), measured college students on The Big Five personality factors. The second instrument, the MUSIC Model of Academic Motivation Inventory (MMAMI), measured the academic motivation beliefs of college students as related to the MMAM. Data were obtained from 305 college students at a single university in southwest Virginia enrolled in an online course. Exploratory Factory Analysis (EFA) and Confirmatory Factor Analysis (CFA) using Structural Equation Modeling (SEM) were used to answer the following research question: To what extent do the Five Factor Model factors relate to the MMAM components? Due to the lack of substantiated knowledge regarding the relationships between The Big Five personality factors and the MMAM components, specific hypotheses were not generated. The measurement models for the Big Five personality factors and the MMAM fit well. However, the structural model, in which the Big Five factors were modeled to predict the MMAM components, did not adequately fit the data for these college students. Yet, there were a number of significant pathways between The Big Five personality factors and the MMAM components. Openness and Conscientiousness had a significant, negative impact on eMpowerment, while Extraversion had a significant, positive impact on eMpowerment, Usefulness, and Interest. Additionally, Agreeableness had a significant, positive impact on Usefulness, Success, Interest, and Caring, while Neuroticism had a significant, negative impact on Success. Findings indicated that personality factors can relate to or predict academic motivation. In other words, students' academic motivation beliefs are, to a certain degree, influenced by some of their personality traits, and these differences in traits may manifest themselves in the classroom. In the future, researchers could examine the extent to which students with different personality traits display varying preferences as to the types of pedagogical methods or strategies that motivate them academically.

Abstract The aim of this study was to understand the meaning of caring as experienced by Arab Muslim nurses within the context of Arab culture. A qualitative approach using ethnographic methodology based on the approaches of Geertz (1973), Fitzgerald (1997) and Davies (1999) was used to develop a description that embeds the phenomena of the nurses’ meaning of caring within the cultural context. Good and Good’s (1981) meaning–centred approach was used to interpret the nurse’s explanatory models of health, illness and healing that inform the caring experience. This study conveys the cultural worlds of Arab Muslim nurses from Saudi Arabia, Lebanon, Jordan and Egypt while caring for Arab Muslim patients in Saudi Arabia. Data were collected over a four year period (2004-2007). Arab Muslim nurses have a religiously informed explanatory model where health is spiritual, physical and psycho-social well-being. Spirituality is central to the belief system where spiritual needs take priority over physical needs as a distinctive care pattern. The professional health belief system blends into the nurses’ cultural and religious belief system, forming a culturally distinct explanatory health beliefs system. This finding suggests that in non-Western health contexts, professional models are not dominant but incorporated into nurses’ indigenous worldviews in a way that makes sense within the culture. Caring is based on shared meanings between nurse and patient. Caring is an act of spiritualty and an action by the nurse to facilitate his or her own spirituality and that of the patient. In turn, the nurse receives reward from Allah for caring actions. A distinct ethical framework based on principles of Islamic bio-ethics guides the nurses in their caring. This research provides the missing link between Western professional nursing systems and Arab Muslim nurses’ caring models and contributes to the development of a caring model that is relevant to, and reflective of, Arab cultural and Islamic religious values. This caring model can provide direction for nurse education and the provision of care to Muslim patients, whether in Arab cultures, Islamic societies or with immigrant Muslim populations. In addition, it provides the basis for an Islamic nursing identity and a beginning point for improving the moral status and image of nursing in the Middle East.

Abstract:
Sharing economy businesses have increasingly began to inhibit the competitive landscape of many different industries. Their growth can be primary connected to general digitalization developments. At its core, the sharing economy concept is built around efficiently using excess resources. In this thesis, it is argued that sharing economy businesses should be considered disruptive innovations, partly because they present incumbent firms with significant challenges. While the sharing economy business concept facilitates effective peer-to-peer exchange, and in the process provides new possibilities of employment, customer-centric innovation and market development, it can also be tied to a decreasing demand for goods and services offered by incumbents in the same marketplace. Examples of businesses built around the sharing economy concept include – but are not limited to – Airbnb, Vrbo, Uber, Lyft, Zilok, and Hygglo. Prior to the beginning of this research project, very little research regarding potential strategic countermeasures for incumbent firms to employ against sharing economy competitors had been conducted. As a result, this thesis serves the purpose of investigating the impact of sharing economy businesses from the perspective of incumbents and theorizing about potential responses. In order to facilitate this purpose, the hospitality industry was chosen as a representative of an industry that has experienced sharing economy business entrances and sharing economy business competition to a researchable extent. Through this thesis and its accompanying qualitative data collection process, several challenges that are presented to incumbents as a result of sharing economy business entrances were identified. Additionally, a focus was placed on evaluating both tried and prospective strategic measures, using existing theory and insights from the data collection to discover the key aspects needed for the development of potential responses to sharing economy businesses. A similar process was undertaken regarding business models, as the crucial and core elements of a perceived successful business model needed to be identified. The findings generated by the data collection process were then analyzed using a thematic analysis, centered on the themes of: Strategic Management in the Hospitality Industry; Current Trends in the Hospitality Sector; Fundamentals of a Successful Hotel Business; and Future Strategy Suggestions and Industry Prospects. After conducting the three identification and analyzation stages, the findings were discussed in the penultimate chapter of the thesis, where the findings’ relation to existing theory was explored. Following the discussion, the possibility to answer the main research question of this thesis appeared. After discussing our findings, our analysis, and the connection to existing theory, we were able to conclude that incumbent firms can successfully compete with sharing economy businesses. Specifically, they could ensure this through strategic changes and business model innovations built around efficiently exploiting the incumbents’ internal strengths and advantages that are currently not easily imitable by sharing economy businesses. As such, they should look to create, deliver, and communicate unique value to its customers, and suggestions on how this could be achieved by both incumbent hospitality managers and others are provided.

Abstract:
The purpose of this DNP project was to determine the feasibility of implementing a nurse practitioner led interprofessional collaborative management model of caring for patients with complex medical conditions who are at high risk for ED and hospital readmission. The target of the feasibility study was an accountable care organization (ACO) in Idaho. The ACO assumes greater financial risk for providing care to a population that includes Medicare Advantage patients - dual insured Medicare/Medicaid patients. The care management teams are currently led by physicians. The members of the population that suffer most from multiple chronic conditions often encounter barriers to accessing high quality primary care, in particular when transitioning between different levels of care. Interprofessional collaborative team based care coordination can address medical and social issues that can affect a patient’s ability to achieve/maintain wellness. The literature suggests that nurse practitioners are ideally suited to lead those teams Approval was given by leadership in the ACO to accomplish a study to determine the feasibility of successfully implementing an innovative NP led interprofessional collaborative care management model: the AEIØOU Bundle of Care Practices. Principles of qualitative descriptive methodology, using content analysis, were applied to explore the responses provided at individual interviews by thirteen key stakeholders. The data collected were not intended to be generalized, but rather to evaluate the potential for implementation of a new model of interprofessional collaborative care within the ACO. Findings suggest that implementation of this model is feasible within the ACO. Common themes uncovered include: (a) change is challenging, (b) coordinated patient care aligns with organizational goals, (c) success requires cost analysis, a comprehensive business plan, buy-in from primary care physicians, and a pilot program, and (d) strong support among all participants for NP and RN home visits was notable.

The purpose of this study was to explore Team Support, an in-school youth development program designed to facilitate high school student-athletes' acquisition of personal and social responsibility by using physical activity. The first research question sought to determine the fidelity of Team Support to Hellison's Personal and Social Responsibility model, the model it purported to implement. The second research question assessed if Team Support was perceived to be an appropriate intervention for teaching personal and social responsibility to the student-athlete participants. For the first research question, a protocol adherence rating scale was created to gather quantitative and qualitative data on approximately 110 student-athletes and 19 advisors. For the second research question, qualitative data were gathered through 2 focus groups of 8 student-athletes each, 12 individual interviews with student-athletes, 9 individual interviews with advisors, and participant observer notes to explore student-athletes and advisors' perceived social, emotional, and academic outcomes of student-athletes' participation in Team Support. The study employed descriptive statistics and thematic analyses to answer its research questions. The results of the investigation yielded short term and long term implications to urban youth, sport practitioners, school counselors, users of Hellison's model, school administrators, and other educators.

Abstract:
When patients and families come to the emergency department seeking medical attention, they come in with many mixed emotions and thoughts. The fast paced, rapid turnover of patients and the chaotic atmosphere may leave patients who visit the emergency department with the perception that staff is uncaring. The purpose of this project was to implement a patient care delivery model, relationship-based care, in the emergency department. The model is comprised of several caring theories including Jean Watson's model of human care and Kristen Swanson's middle range theory of caring. The main goals of the project were to help staff enhance the patient and caregiver interaction, strengthen co-worker relationships, and gain appreciation of the importance of self-care. The intervention was an educational workshop about the relationship-based care model. Eight participants were consented, given a preassessment survey, educated about the model, and then given a postassessment survey. Prior to education, 83% of participants believed strongly that patients and families need to feel cared for during an emergency department visit; this increased to 100% posteducation. Perception about the importance of coworkers' relationships being trusting went from 38% to 50% and the importance of caring for one's self increased from 63% to 100%. It was recommended that the model be implemented in all emergency departments and all staff educated in its use as a way to promote social change through intentional focus on caring in every patient interaction.

The priorities many nursing homes give to physical care often supersede consideration for leisure arrangements and resources with the effect that the range of activities and engagement opportunities for residents are limited. An inactive lifestyle in nursing homes can compromise the residents’ quality of life and their psychosocial wellness through boredom, diminished morale and reinforced dependency. A low level of engagement also exposes residents to a greater risk of developing cognitive impairment and accentuates feelings of loneliness and isolation. Given the demographic trend of an ageing population together with a growing demand for nursing homes in Singapore, this research addresses the current lack of research on lifestyles of nursing home residents and arrangements to promote their personal well-being. In this thesis, I investigate the effect of a participatory visual arts programme on the personal well-being of residents in a Singapore nursing home. The study explores the ways in which well-being is afforded through participatory arts activities and the role an artist can take in relation to human caring. To facilitate evaluation and reflection on my arts-health practice, I brought my arts-health practice into a novel dialogue methodologically with the action-research case study approach of social science. I refer to this hybrid approach as ‘critical arts-health practice’. The empirical data of the study prompted exploration of the link between vitality and participatory arts activities. Participatory arts activities are found to revitalise the sensory, physical, cognitive, emotional, social capacities of older adults and promote self-actualisation. Drawing on Gilles Deleuze’s assemblage theory, I conceptualised the Arts-Health assemblage as a way of understanding the processes through which participatory arts activities contribute to the participants’ well-being, as both dynamic and as involving multiple interrelated elements. I argue that the broad concerns of arts-health practice can be defined as combining individualised attention to the participant, well-being outcome, and ensuring the quality of the environment and activities for participatory arts. I argue that attentiveness to these dimensions will promote a more effective and caring arts-health practice. Lastly, the central importance of these dimensions within an arts-health practice emphasises that the arts-health practitioner is first and foremost a caring artist.

Abstract:
The original contribution of this qualitative study is that it sketches the front-line of the contested domain of infant feeding choices by exploring the under-researched experiences and perceptions of mothers who actively choose to bottle-feed their babies. Twelve bottle-feeding mothers in Western Australia participated in open-ended, in-depth interviews. The interview findings were further explored using participant observations of relevant hospital practices and critical, hermeneutic re-readings of midwifery's professional and policy documents. The thesis argues that bottle-feeding is marginalised by the midwifery profession that currently defines its practices in accordance with policies such as the Baby Friendly Hospital Initiative. Bottle-feeding mothers described experiencing negative, antagonistic encounters with midwives moralising about infant feeding choices, pressuring them to choose breastfeeding, and obstructing rather than supporting their infant feeding choice. The bottle-feeding choice is often marginalised in the literature by the deployment of nebulous concepts about parental emotions, most notably so-called "mother-child bonding". However, the mothers presented an identifiable "bottle-centric" perspective by which they considered bottle-feeding the best choice to support their priorities of overall life-style organisation, avoidance of discomfort and anxiety, and optimal family inter-relationship dynamics. Notably, the bottle-centric perspectives described breastfeeding as a threat to those goals and values. This study also identifies a previously unexplored phenomenon of the mothers' concept of “father bonding" with its symbolic meanings of bottle-feeding for the mother desiring the father's emotional involvement with the baby. A speculative theory of an evolved socio-biologic concept of "Father bonding/bondage" is forwarded. Theoretical dimensions of current health belief models, especially that underlying the BFHI, cannot adequately recognise or service this client group's needs. Bottle-feeding mothers intelligently engage in health decision-making processes when deciding their infant feeding options, considering a broad range of factors to optimise their family relationships. However, the mothers' value systems are diametrically opposed to those of the midwifery profession. Midwifery's approach to limiting information and practical educational demonstrations of bottle-feeding is a key aspect of marginalisation. The mothers strongly criticised bias in antenatal classes, poor support for their own training in bottle-feeding, experiences of learning by making mistakes, and some mothers afforded more credibility to advice from sources outside midwifery. These alarming findings appear to result from WHO/UNICEF anti-marketing principles mistakenly being generalised to educational functions in the BFHI policy. Variations in the mothers' levels of satisfaction with their experiences in hospitals depended upon whether facilities suited or inconvenienced their needs, how efficiently hospital administrative procedures upheld their choice, and staff attitudes towards them. Major differences appeared related less to public or private sector differences and more to how well facilities were oriented towards either bottle~ feeding or breastfeeding. Exemplary support should be provided to both breast- and bottle-feeding client groups but will require a more sophisticated approach. Practical suggestions for developing and disseminating more timely and relevant information and support for bottle-feeding, and suggested research projects to expand the present study's findings are forwarded. The thesis proposes a "Caring Options- Responsive" model of Midwifery services suited to fully informing and respectfully supporting clients in their choice.

The Design and Implementation of a Relationship-Based Care Delivery Model on a Medical- Surgical Unit by Paula A. Rodney MSN, California University of Pennsylvania, 2011 BSN, University of Virginia, 1979 Project Submitted in Partial Fulfillment of the Requirements for the Degree of Doctor of Nursing Practice Walden University April 2015   Patient satisfaction and clinical outcomes have become important issues in healthcare since the introduction of the Value Based Purchasing Program. Patient satisfaction, as measured by Hospital Consumer Assessment of Healthcare Providers and Systems (HCAHPS) survey, was declining and hospital-acquired pressure ulcers (HAPU), falls, and catheter-associated urinary tract infections (CAUTI) were rising on the pilot unit. The purpose of this non-experimental correlational design quality improvement project was to combine information from focus groups, a content analysis of the literature on Kristen Swanson's theory of caring, and relationship-based care, to develop and implement a relationship-based care delivery model. An additional aim was to determine its impact on patient satisfaction and the reduction of HAPU, falls, and CAUTI. The model was designed and implemented by a team consisting of bedside care providers, leaders, an educator, and a student facilitator. The components of the model included scheduling for continuity of care, whiteboards, seated bedside report, hourly rounding, a nurse advocate, and 5 focused minutes of attention per shift. Descriptive statistics were used to determine the mean change in HCAHPS scores before and after implementation of the model, and revealed improvements in dimensions of communication with nursing by 13.2%, responsiveness by 12.5%, overall rating of care by 14.5%, and willingness to recommend by 8.7%. The result of audits of the pilot unit's medical records indicated a reduction in falls by 3, HAPU by 2, and CAUTI by 2 from August, the baseline month. As a result of these findings the model will be implemented on all inpatient nursing units. The target audience for this project includes nursing leaders, educators, and bedside providers with interest in patient-centered care and staff empowerment.

ENGLISH ABSTRACT : It has been suggested that the complex needs of children with neurodevelopmental disorders are best addressed with a family-centred approach. As this approach has been increasingly incorporated into healthcare systems over recent years, most children with disabilities are now able to remain at home as opposed to living in institutions or group homes. Many parents have thus been forced to adopt the role of a primary caregiver for their child that has been diagnosed with a disability or chronic condition. This study aimed to investigate the experiences of caregivers for children with cerebral palsy in rural communities of the Western Cape, with a specific focus on the barriers and facilitators that they encounter. An exploratory qualitative design was employed and 15 individuals (aged 27-62), who were identified as caregivers for a child with cerebral palsy through purposive sampling were interviewed. Thematic analysis was used to analyse and generate themes from the semi-structured interviews that were conducted with participants. A number of barriers to caring emerged including the personal consequences of caregiving, difficulty adjusting to caregiving duties, environmental conditions, lack of access to healthcare services, lack of respite services, and negative perceptions towards disability. Various facilitators to caring were also identified, namely personal coping methods, personal transformation, social support, relationship with one’s child, community resources, child’s crèche, and financial assistance. The five levels of the Social Ecological Model were used to conceptualise the discussion of these findings and links were made to the existing literature on the caregiver experience in the developing context. In this process it was found that participants often encountered similar barriers and facilitators to caregivers for individuals with other types of conditions, such as HIV/AIDS, autism, mental illness, cancer, and neurological disorders. This could suggest that caregivers residing in developing countries share similar experiences, regardless of the condition of the care receiver. Although participants received support from a number of sources that aided them with their caregiving duties, it is evident that they still encountered gaps in the provision of a number of important services, including lack of information from healthcare professionals, lack of community programs to reduce stress and promote empowerment, and lack of disability-friendly facilities. It is vital that caregivers have access to these services to ensure their child’s well-being as well as their own. There is thus a need to explore how these services can be made more accessible to caregivers in rural communities.
AFRIKAANSE OPSOMMING : Daar word voorgestel dat die komplekse behoeftes van kinders met neuro-ontwikkelingsversteurings die beste aangespreek word deur ‘n familie-gefokusde benadering. Aangesien hierdie benadering meer geredelik geïnkorporeer is in die gesondheidsorg sisteem oor die afgelope paar jaar is meeste kinders met gestremdhede nou in staat om by die huis te kan bly in plaas daarvan om in inrigtings of groepshuise te woon. Baie ouers is dus geforseer om die rol as primêre versorger aan te neem vir hulle kind wat gediagnoseer is met ‘n gestremdheid of kroniese toestand. Die vyf vlakke van die Sosiale Ekologiese Model is gebruik om die bespreking van hierdie bevindinge te konseptualiseer en verbintenisse is gemaak met bestaande literatuur wat handel oor die versorger se ervaring in die ontwikkellende konteks. In hierdie proses is gevind dat deelnemers dikwels dieselfde hindernisse en fasiliteerders ondervind as versorgers van individue met ander tipe toestande, soos MIV/VIGS, outisme, geestesiektes, kanker en neurologiese versteurings. Dit kan moontlik beteken dat versorgers wat in ontwikkellende lande woord dieselfde ervarings deel, ten spyte van die toestand van die persoon wat die versorging ontvang. Alhoewel deelnemers ondersteuning van ʼn aantal bronne ontvang het wat hulle help met hulle versorgingspligte, is dit duidelik dat hulle steeds gapings ervaar in die voorsiening van ʼn aantal belangrike dienste, insluitende ʼn tekort aan inligting van gesondheidsorg werkers, ʼn tekort aan gemeenskapsprogramme om stres verligting en bemagtiging te bevorder en ʼn tekort aan gestremdheid-vriendelike fasiliteite. Dit is baie belangrik dat versorgers toegang het tot hierdie dienste om die welstand van hulle kind en hulself te verseker. People with neurocognitive disorders (NCD) condition called dementia are mainly taken care by family members. The number of people suffering from neurocognitive disorders is estimated to increase. Providing care for the person with NCD can be stressful and demanding which is why effective interventions to support caregivers are needed. Occupational therapy practitioners have unique knowledge of the person, environment and occupation and how those factors influence on individuals occupational performance. Occupational therapy ́s interventions to support caregivers with NCD have mainly concentrated on problems in the behaviour of person with NCD. The aim of this scoping review was to identify why some caregivers are managing better as a caregiver before any interventions have been done. What are those aspects of Person (P), Occupation (O) and Environment (E) that facilitate occupational performance (OP) when taking care for a person with NCD. PEO-model by Law et. al (Law et al., 1996) was utilized to formulate research question, in search words and in summarizing the results. The search from Ageline, Cinahl, Psycinfo and PubMed databases was done in January 2018 and went back 10 years. Results found 23 relevant articles for inclusion in the scoping review. Facilitating factors emphasized the supports of relationship seen as an overlap of caregivers and individuals with NCD on occupational performance. Facilitating personal factors of resilience, finding gains in caregiving, self -efficacy and religiosity were found to have an impact on care that the caregiver provides. Facilitative aspects of occupations highlighted importance of daily occupations as a source of well-being for both caregiver and person with NCD. In conclusion, facilitating aspects of relationships, personal factors and engagement in activities and hobbies supported occupational performance of both the caregiver and a person with NCD. Taking these factors into consideration could help target interventions more precisely to meet caregivers needs.

This critical race qualitative research study examines the perspectives of Chicanas\os regarding their educational experiences. Critical race theory in education has been critical in the effort to bring a deeper understanding of the racism that is experienced in American schools by Chicanas\os and other children of color. This study examines the intersectionality of American education; the Chicana\o social, political and historical experiences; and racism.This study is informed by theoretical frames from the disciplines of critical race theory, Latino critical race theory and their educational implications, new racism, Chicana/o authentic caring, and critical pedagogy. These theories expose inequality and injustice that adhere in American schools, and they help me understand that Chicana/o students, their parents and their communities are constructors of knowledge and facilitators of critical transformation.The study triangulates qualitative data through two critical components: interviews and an archival evaluation of the academic impact of the Social Justice Education Project and its Critically Compassionate Intellectualism (CCI) model of transformative education. The interview component consists of one open-ended focus group interview and one open-ended interview. In the archival segment, I evaluate informal open-ended student interviews, end of the year progress reports, post-program surveys, and achievement and graduation data.These data indicate that racism remains a key variable within the educational experiences of Chicanas\os students in SUSD schools. Additional findings indicate that the student cohorts that participate in the Social Justice Education Project and experience the CCI model of transformative education have a higher AIMS pass rate and higher graduation rates than those students cohorts that do not experience both the Social Justice Education Project and its CCI model.Given these findings, the study proposes that educational leaders demonstrate the political will that is needed to discover and implement multiple forms of critical transformative educational praxis. In addition, the need for more research that centers the voices of students and that focuses on racism and the Chicana\o contemporary experience.

Background: It is predicted that China will have the largest number of cases of dementia in the world by 2025 (Ferri et al., 2005). Research has demonstrated that caring for family members with dementia can be a long-term, burdensome activity resulting in physical and emotional distress and impairment (Pinquart & Sorensen, 2003b). The establishment of family caregiver supportive services in China can be considered urgent; and the knowledge of the caregiving experience and related influencing factors is necessary to inform such services. Nevertheless, in the context of rapid demographic and socioeconomic change, the impact of caregiving for rural and urban Chinese adult-child caregivers may be different, and different needs in supportive services may therefore be expected. Objectives: The aims of this research were 1) to examine the potential differences existing in the caregiving experience between rural and urban adult-child caregivers caring for parents with dementia in China; and 2) to examine the potential differences existing in the influencing factors of the caregiving experience for rural as compared with urban adult-child caregivers caring for parents with dementia in China. Based on the literature review and Kramer.s (1997) caregiver adaptation model, six concepts and their relationships of caregiving experience were studied: severity of the care receivers. dementia, caregivers. appraisal of role strain and role gain, negative and positive well-being outcomes, and health related quality of life. Furthermore, four influencing factors (i.e., filial piety, social support, resilience, and personal mastery) were studied respectively. Methods: A cross-sectional, comparative design was used to achieve the aims of the study. A questionnaire, which was designed based on the literature review and on Kramer.s (1997) caregiver adaptation model, was completed by 401 adult-child caregivers caring for their parents with dementia from the mental health outpatient departments in five hospitals in the Yunnan province, P.R. China. Structural equation modelling (SEM) was employed as the main statistical technique for data analyses. Other statistical techniques (e.g., t-tests and Chi-Square tests) were also conducted to compare the demographic characteristics and the measured variables between rural and urban groups. Results: For the first research aim, the results indicated that urban adult-child caregivers in China experienced significantly greater strain and negative well-being outcomes than their rural peers; whereas, the difference on the appraisal of role gain and positive outcomes was nonsignificant between the two groups. The results also indicated that the amounts of severity of care receivers. dementia and caregivers. health related quality of life do not have the same meanings between the two groups. Thus, the levels of these two concepts were not comparable between the rural and urban groups in this study. Moreover, the results also demonstrated that the negative direct effect of gain on negative outcomes in urban caregivers was stronger than that in rural caregivers, suggesting that the urban caregivers tended to use appraisal of role gain to protect themselves from negative well-being outcomes to a greater extent. In addition, the unexplained variance in strain in the urban group was significantly more than that in the rural group, suggesting that there were other unmeasured variables besides the severity of care receivers. dementia which would predict strain in urban caregivers compared with their rural peers. For the second research aim, the results demonstrated that rural adult-child caregivers reported a significantly higher level of filial piety and more social support than their urban counterparts, although the two groups did not significantly differ on the levels of their resilience and personal mastery. Furthermore, although the mediation effects of these four influencing factors on both positive and negative aspects remained constant across rural and urban adult-child caregivers, urban caregivers tended to be more effective in using personal mastery to protect themselves from role strain than rural caregivers, which in turn protects them more from the negative well-being outcomes than was the case with their rural peers. Conclusions: The study extends the application of Kramer.s caregiving adaptation process model (Kramer, 1997) to a sample of adult-child caregivers in China by demonstrating that both positive and negative aspects of caregiving may impact on the caregiver.s health related quality of life, suggesting that both aspects should be targeted in supportive interventions for Chinese family caregivers. Moreover, by demonstrating partial mediation effects, the study provides four influencing factors (i.e., filial piety, social support, resilience, and personal mastery) as specific targets for clinical interventions. Furthermore, the study found evidence that urban adult-child caregivers had more negative but similar positive experience compared to their rural peers, suggesting that the establishment of supportive services for urban caregivers may be more urgent at present stage in China. Additionally, since urban caregivers tended to use appraisal of role gain and personal mastery to protect themselves from negative well-being outcomes than rural caregivers to a greater extend, interventions targeting utility of gain or/and personal mastery to decrease negative outcomes might be more effective in urban caregivers than in rural caregivers. On the other hand, as cultural expectations and expression of filial piety tend to be more traditional in rural areas, interventions targeting filial piety could be more effective among rural caregivers. Last but not least, as rural adult-child caregivers have more existing natural social support than their urban counterparts, mobilising existing natural social support resources may be more beneficial for rural caregivers, whereas, formal supports (e.g., counselling services, support groups and adult day care centres) should be enhanced for urban caregivers.

Abstract:
Aim: The overall aim of the thesis was to describe the experiences of awake patients during surgery under regional anesthesia. In addition, the aim was to develop a model for intraoperative care that can support and enhance patients’ well-being during the intraoperative period.   Methods: Study I was a patient interview study guided by a reflective lifeworld approach. In study II a philosophical reflection of the findings from study I was carried out. In study III a hermeneutic approach inspired by Ricoeur and Gadamer was used in order to interpret video recorded material. In study IV a hermeneutic approach inspired by Gadamer was used to synthesize the findings in studies I-III transformed into an intraoperative caring model.  Overall main findings: The analysis shows that being awake during surgery can be compared with walking a tightrope because of ambiguous feelings. The proximity and presence of the nurse anesthetist (NA) anchors the patient in the present and strengthens the patient’s feeling of trust. The temporary disruption in the relationship between the body and the world due to regional anesthesia means that the patient’s being in the world is exposed to revolutionary experiences. Gaps between the patient’s experiences and the situation can be bridged over when the NA acts as the patient’s bodily extension and links the patient as a subject to the world in the intraoperative situation. From the patient’s perspective this calls for the NA’s proximity and genuine presence in the ‘intraoperative caring space’. When the NA’s performance of his/her professional duties clashes with the patient’s existential being in the intraoperative situation the need of present presence from the NA is crucial. Conclusions: The findings contribute to knowledge development about intraoperative care and raise awareness that care for the awake patient cannot be performed on formal routines that might disregard the uniqueness of each patient’s situation. The model can be used as a tool to encounter awake patients’ existential needs in the intraoperative situation and to further enlighten NAs about the possible impact of their proximity, interaction and communication behavior in the delivery of intraoperative nursing care.

Carding is an essential pre-spinning process whereby masses of dirty tufted fibres are cleaned, disentangled and refined into a smooth coherent web. Research and development in this `low-technology' industry have hitherto depended on empirical evidence. In collaboration with the School of Textile Industries at the University of Leeds, a mathematical theory has been developed that describes the passage of fibres through the carding machine. The fibre dynamics in the carding machine are posed, modelled and simulated by three distinct physical problems: the journey of a single fibre, the extraction of fibres from a tuft or tufts and many interconnecting, entangled fibres. A description of the life of a single fibre is given as it is transported through the carding machine. Many fibres are sparsely distributed across machine surfaces, therefore interactions with other neighbouring fibres, either hydrodynamically or by frictional contact points, can be neglected. The aerodynamic forces overwhelm the fibre's ability to retain its crimp or natural curvature, and so the fibre is treated as an inextensible string. Two machine topologies are studied in detail, thin annular regions with hooked surfaces and the nip region between two rotating drums. The theoretical simulations suggest that fibres do not transfer between carding surfaces in annular machine geometries. In contrast to current carding theories, which are speculative, a novel explanation is developed for fibre transfer between the rotating drums. The mathematical simulations describe two distinct mechanisms: strong transferral forces between the taker-in and cylinder and a weaker mechanism between cylinder and doffer. Most fibres enter the carding machine connected to and entangled with other fibres. Fibres are teased from their neighbours and in the case where their neighbours form a tuft, which is a cohesive and resistive fibre structure, a model has been developed to understand how a tuft is opened and broken down during the carding process. Hook-fibre-tuft competitions are modelled in detail: a single fibre extracted from a tuft by a hook and diverging hook-entrained tufts with many interconnecting fibres. Consequently, for each scenario once fibres have been completely or partially extracted, estimates can be made as to the degree to which a tuft has been opened-up. Finally, a continuum approach is used to simulate many interconnected, entangled fibre-tuft populations, focusing in particular on their deformations. A novel approach describes this medium by density, velocity, directionality, alignment and entanglement. The materials responds to stress as an isotropic or transversely isotropic medium dependent on the degree of alignment. Additionally, the material's response to stress is a function of the degree of entanglement which we describe by using braid theory. Analytical solutions are found for elongational and shearing flows, and these compare very well with experiments for certain parameter regimes.

Bakgrund: Forskning pekar på att det kommer ske en fortsatt ökning av barn med psykisk ohälsa de kommande åren. Sjuksköterskor kommer således att möta fler med barn diagnosen depression. Studier kring sjuksköterskors erfarenheter av att vårda barn med depression är sparsam och mer kunskap kring omvårdnaden av denna sårbara grupp är betydelsefull. Syfte: Att beskriva sjuksköterskors erfarenheter av omvårdnad av barn med depression inom barn – och ungdomspsykiatrisk öppenvård.  Metod: Studien har en kvalitativ design med induktiv ansats. Som datainsamlingsmetod användes semistrukturerade intervjuer som genomfördes med sex sjuksköterskor inom barn-och ungdomspsykiatrisk öppenvård. Resultatet analyserades med kvalitativ innehållsanalys. Resultat: Det framkom; Att sjuksköterskans roll handlar om att vara en mellanhand, Att beakta att barnet alltid finns i ett sammanhang, Att skapa en relation till barnet och Att hantera egna känslor. Omvårdnaden går inte att beskriva på ett tydligt sätt, trots att sjuksköterskan träffar dessa barn och deras familjer dagligen. Slutsatser: Sjuksköterskans roll och betydelse skulle behövas synliggöras då den idag kan anses vara otydlig. Det finns en kompetens bland specialistsjuksköterskor som inte alltid tas i anspråk för att främja barnets återhämtning från depression.
Background: Science shows that we will see a continuing increase of children with mental health problems in the coming years. As a consequence of this nurses will be meeting a larger number of children suffering from depression. Studies regarding nurses’ experiences of caring for children suffering from depression are sparse. More knowledge is required and will be meaningful in the future care of this vulnerable patient group. Aim: To describe nurses’ experiences of caring for children with depression within child and adolescent outpatient psychiatric care.  Method: This study has a qualitative design with an inductive approach. As a data collecting method, semi- structured interviews were used which were conducted with six nurses in child and adolescent psychiatric outpatient care. The result was analyzed with qualitative content analysis. Results: The results show that; the nurse’s role is about being an intermediary, to take into account that the child always exists in a specific context, to create a relationship to the child and to manage one's own emotions. Conclusions: The nurse's role and significance would need to be made more visible as it today can be considered unclear. Specialist nurses have significant competences that is not always used in the best way to promote the child's recovery from depression.

Mental health is a major child public health issue in Sweden. The overall aims of this thesis are to explore girls’ and boys’ perspectives on mental health and on mental health-promoting initiatives, and to deepen the knowledge of disclosure and support for ado-lescents at risk of abuse and neglect. Four studies are included in the thesis, and a qualitative descriptive design was chosen. In three of the studies (Studies I, II, and III), adolescents in Sweden were interviewed individually or in focus-groups. In the fourth study (IV) officials and politicians in nine municipalities in Sweden were interviewed, and documents were analysed in a multiple-case-study design. In study I the adolescents were asked about the mental health-promoting dialogue with the school nurse. The results reveal that what the adolescents found important were trustiness, attentiveness, respectfulness, authenticity, accessibility, and continuity. The adolescents also had certain views on what issues to talk about in the health dialogue; physical and psychological issues should be included. In Study II, the adolescents perceived “mental health” to be an emotional experience with positive and negative aspects of internal and relational feelings. Family, friends, and school were regarded as important determinants of mental health by the adolescents. Neither girls nor boys thought that there were any major differences in mental health between girls and boys, but did think they were subject to different expectations. In Study III the results show that adolescents with families with alcohol problems are unsure whether to disclose their home situation to an adult; the adolescents seem to make a risk assessment when looking for trustworthy adults. It is a disclosure process. Friends are confidants and supportive, and sometimes facilitators for contacting adults, when support from adults is needed. Study IV show that even small grants to municipalities for children-at-risk projects lead to more activities for these children and adolescents. But children or adolescents were not involved in the planning or decision-making of the activities. The studies in this thesis show that most important thing for adolescents’ mental health is the relation between adolescents and adults, foremost parents, and between adolescents and friends. In addition, gender and age, adolescents’ perspec¬tives and par-ticipation, and society’s support, including the implementation of the UN Convention on the Rights of the Child, have an impact on adolescents’ mental health, both for ado-lescents in general and for adolescents at risk of abuse and neglect. The results are viewed in relation to the bioecological model, to illustrate how all levels in society influence mental health among adolescents, on an individual and a population plane. The findings have implications for adults: to learn more about adolescents and puberty, and about the home situation for children and adolescents at risk of abuse and neglect; to listen to suggestions from children and adolescents; to include friends in support to adolescents at risk of abuse and neglect; and to include girls and boys in all matters concerning them.

An ageing population is a challenge faced by many developed and developing countries. By understanding the needs in health and well-being of the elderly governments can ensure policies and services are as efficient as possible, in order to provide the best care amidst growing demand. The thesis analyses categorical data from the English Longitudinal Study of Ageing (ELSA) and Brazil’s National Household Sample Survey (PNAD) to form measurement models for aspects of health, economic well-being and subjective well-being in the elderly, in England and Brazil. Applying structural equation modelling (SEM), a measurement model for health and economic well-being in both countries was constructed using categorical variables from the survey data, as well as for subjective well-being in England. For health the variables represented self-reported morbidity and functional status, while for economic well-being variables represented housing quality and durables owned within the household. Psychometric measures of satisfaction with life, quality of life, loneliness and depression constituted subjective well-being. Based on empirical evidence from exploratory factor analysis (EFA), the latent structure of each aspect was defined. Multilevel structural equation modelling was applied to the PNAD to capture the hierarchical structure, whereby individuals were clustered by household then the households were clustered by sector and the sectors clustered by municipality. Meanwhile, the longitudinal dynamic of the ELSA allowed for a multivariate latent growth modelling, using multiple indicators to model the trajectories of subjective well-being in multiple aspects. Health and economic well-being were positively associated in both countries. Metabolic health was a factor that was identified in both England and Brazil, while musculoskeletal health was identified only in Brazil. In England, economic well-being had two separate factors relating to the inside and outside of the household. Health and household quality significantly influenced subjective well-being, but not neighbourhood quality, with better health and economic wellbeing associated with greater subjective well-being. In the multilevel SEM a different structure of latent constructs was identified for the health of the sectors of elderly individuals, and the variables had different priorities. In both countries, health was significantly better for men, but worse for those of non-white race and with older age. Regional disparities were also present in health and economic well-being. In Brazil, economic well-being was worse with older age, in rural areas, male gender and non-white race. In England, economic well-being was better for older age with no difference between the genders and races. Subjective well-being was persistently better for men and those that were married, while being persistently worse for those of non-white race or divorced/separated marital status. Those that were widowed recovered from initially lower subjective well-being. Marital status was highly influential to the subjective well-being of the elderly with significant impact from becoming divorced or widowed and benefits from getting remarried.

Within the space of a few decades, young people's understandings and experiences of nature have changed radically. Today, many young people are aware of the global threats to the environment - but their physical contact, their intimacy with nature, is diminishing, leading it is argued to a "nature deficit disorder". In order to minimise the risk of nature deficit disorder, this study seeks to understand how behaviour change models, innovative environmental education approaches, and an ethic of care may influence young people's environmentally caring behaviours. The main research question is: What are the factors that influence young people's environmentally caring behaviours in Indonesia? This question is addressed through three sub-research questions: 1) What can we learn from behaviour change models about young people's environmentally caring behaviours?, 2) How do environmental education approaches influence young people's environmentally caring behaviours?, and 3) What added value do ethic of care principles bring to better understanding young people's environmentally caring behaviour?

Att vara en lidande människa kan innebära att individen känner sig kränkt och integriteten kan hotas. Syfte: Syftet med litteratur studien är att få en ökad förståelse för hur Roys anpassningsmodell kan integreras i personers upplevelser av att ha genomgått en benamputation. Metod: den här litteraturstudien har genomförts med inspiration av Evans analys metod integrerad med Roys anpassningsmodell av vårdvetenskapliga artiklar som sökts i olika databaser. Resultat: utifrån personers upplevelser av amputation integrerat med Roys anpassningsmodell blev resultatet att fyra teman kunde skapas som bygger på Roys anpassningsmodell. Slutsats: Roys anpassningsmodell kan användas som en modell för sjuksköterskor vid omvårdnad av personer som genomgått en amputation.

Försoning sker inom människan och skapar en möjlighet att förenas med svåra livsöden. Syftet med litteraturstudien var att belysa vad begreppet försoning är och hur det gestaltas i omvårdnad. I resultatet redovisas försoningens olika dimensioner inom människan. Försoning kan gestaltas som ett mänskligt accepterande, som en mänsklig förmåga att återta kontrollen, som ett mänskligt hopp, som en mänsklig förändring och som en mänsklig process. En modell presenteras i resultatet för att visa försoningens olika dimensioner som tillsammans bildar den inre kärnan den oändliga försoningen. De olika dimensionerna av försoning inom människan är i ständig rörelse. Begreppet försoning är inte ett välbeforskat begrepp inom vårdvetenskapen, utan det råder kunskapsluckor kring försoning i omvårdnad. Om mer evidens tas fram om begreppet försoning i omvårdnad, kan sjuksköterskor få redskap att i vårdprocessen förstå och främja försoning hos patienter.
The nursing scientific concept of reconciliation creates an opportunity for people to experience an emotional satisfaction in life. Reconciliation takes place inside the human and creates an opportunity to unite with difficult life situations. The purpose of this study was to explain what reconciliation is and how it takes shape in nursing. The results explain reconciliations different proportions inside the human. Reconciliation can be represented as a human acceptance, as a human's ability to regain control, as a human hope, as a human change and as a human process. A model in the result presents the different proportions of reconciliations. Together the different proportions form the infinite of reconciliation that together creates the inner core of reconciliation. The different proportions of reconciliation are constantly in motion. There is missing evidence for reconciliation in nursing science. If more sciences performed, nurses can get implements for the care process to understand how reconciliation takes shape in human and how nurses can promote reconciliations for patients.

There is an apparent lack of in-depth knowledge about the processes that may underlie the interaction between work and home life, and their relationships with employee health and well-being. Work and home has traditionally been considered as separate domains, but during the past decade of democracy, transformation developments (eg, Affirmative Action, Employment Equity) changed the nature of the labour market and economy. This facilitated the increase in the number of working single-parent, dual-earner families, and of women participating in the workforce, which in turned influenced the work-home interaction of employed individuals. Furthermore, various researchers regard burnout and engagement as important constructs to consider in the well-being of employees. The level of a person's wellbeing subsequently affects his/her functionality in both the work and home spheres. Thus, there is a need to identify ways which both the individual and the organisation can apply to increase personal well-being and the balance between work and home life. South Africa has 11 different national languages, and only 8,3% of the population actually speak English at home. Language differences should therefore be taken into account when administering questionnaires. Studies in South Africa generally report race, education, language, and understanding of English as the main factors which impact on construct and item comparability of psychometric tests. There is consequently an obvious need to translate research instruments before they are administered to individuals from different language groups. If language differences are not taken into account, invalid conclusions regarding the constructs under study could be made, with serious implications for culturally diverse settings such as in South Africa. The objectives of this research were to translate the Survey Work-Home Interaction Nijmegen (SWING), Maslach Burnout Inventory-General Survey (MBI-GS) and Utrecth Work Engagement Scale (UWES) into Afrikaans and Setswana, and to investigate the construct validity, construct equivalence and reliability of these instruments. Furthermore, differences between demographic groups regarding work-home interaction and well-being were investigated. Finally, a structural model was tested, which included job characteristics, negative and positive work-home interference (WHI) and well-being (burnout and engagement). A cross-sectional survey design was used. Random samples (N = 685) were taken from police stations in the North West province. Structural equation modelling (SEM) was used to determine the construct validity and equivalence of the measuring instruments and to test the structural model. The results indicated that work-home interaction can be described as a four-dimensional construct consisting of negative WHI, positive WHI, negative home-work interference (HWI), and positive HWI. This factor structure was equivalent across all three language groups and all the scales were reliable. A four-factor model was confirmed for burnout and included exhaustion, cognitive weariness, cynicism and professional efficacy. A one-factor model was found for engagement. Both translated instruments were found to be equivalent for the three language groups. Furthermore, a second order factor analysis revealed that the underlying structure of well-being consists of two negatively related and equivalent factors, namely burnout (exhaustion, cognitive weariness and cynicism) and an enlarged engagement construct (engagement and professional efficacy). Members reported more negative WHI than negative HWI, and more positive HWI than positive WHI. Statistically significant differences exist between demographic groups regarding work-home interaction based on language, gender, marital status. parental status and education. Statistically significant differences of wellness exist between demographic groups based on language and educational level. The results of the structural equation modelling revealed that job demands were directly and positively associated with negative WHI and burnout, while job resources were directly and positively associated with positive WHI and work engagement. This also indicates the partial mediating effect of WHI between job characteristics and wellness. In addition, a lack ofjob resources was associated with higher levels of burnout. Recommendations for future research were made.

Introdução: Os múltiplos e interativos efeitos protetores do aleitamento materno exclusivo (AME) na saúde e sobrevivência infantil justificam as recomendações universais para promover sua prática. Poucos são os estudos que avaliam a tendência do padrão do AME entre países. Objetivo: Analisar a mudança temporal do AME em cinco países da América Latina e Caribe (ALC) comparando dados das décadas de 1990 e 2000. Métodos: A dissertação é composta por um manuscrito, que avaliou dados de crianças de 0 a 6 meses incluídas nas amostras das pesquisas Demographic Health Survey conduzidas em Brasil, Colômbia, Haiti, Peru e República Dominicana. Foram estimadas as prevalências do AME e suas taxas anuais de variação ponderada, segundo país e ano de inquérito. A duração do AME foi estimada usando a análise de sobrevida de Kaplan-Meier, considerando a idade atual da criança como o tempo de sobrevida e o AME como variável binária, referente à situação da prática no momento da entrevista. As curvas de sobrevivência foram construídas por país, em cada década, e a comparação entre elas usou o teste log-rank. A mediana do tempo de amamentação foi calculada para cada variável independente e a relação entre essas variáveis e o desmame até os seis meses foi analisada pela técnica de regressão de Cox com modelo múltiplo. Resultados: A prevalência de AME aumentou em quatro dos cinco países estudados, com incremento ao ano mais marcante na Colômbia (11 por cento ) e no Haiti (17 por cento ). A duração mediana apresentou duas tendências de evolução: aumento com equidade na Colômbia e no Haiti, e estagnação com distribuição desigual entre os subgrupos populacionais da última década no Brasil, Peru e República Dominicana. No modelo múltiplo de regressão, variáveis de demografia e do perfil de uso dos serviços de saúde associaram-se à duração do AME. A residência em área rural foi a variável reiteradamente associada, de forma negativa no Brasil (HR=1,68; IC 95 por cento :1,06-2,67) e na Colômbia (HR=1,39; IC 95 por cento :1,03-1,87), enquanto que positivamente no Peru (HR=0,40; IC 95 por cento :0,19-0,83). Conclusão: O balanço da tendência do AME na ALC é positivo, embora não uniforme ao longo das duas décadas analisadas. Os achados sinalizam a necessidade de intervenções para a promoção do AME que levem em consideração a localização geográfica das famílias e a qualidade prestada nos serviços de saúde
Introduction: Multiple and interactive protective effects of exclusive breastfeeding (EBF) in health and child survival justify recommendations for promoting universal practice. There are few studies that assess the tendency of the pattern of EBF between countries. Objective: To analyze the temporal change of the AME in five countries in Latin America and Caribbean (LAC) comparing data from 1990 and 2000 decades. Methods: The dissertation consists of a manuscript, which evaluated data from children aged 0 to 6 months in the samples of the Demographic Health Survey conducted research in Brazil, Colombia, Haiti, Peru and the Dominican Republic. Were estimated the prevalence of exclusive breastfeeding and its weighted annual rates of change, according to country and survey year. The duration of EBF was estimated using survival analysis Kaplan-Meier method, considering the current age of the child as the survival time and EBF as binary variable, concerning the state of practice at the time of the interview. The survival curves were constructed for each country, in every decade, and the comparison between them used the log-rank test. The median duration of breastfeeding was calculated for each independent variable and the relationship between these variables and weaning at six months was analyzed using Cox regression model. Results: The prevalence of EBF increased in four of the five countries studied, increasing the most remarkable years in Colombia (II per cent ) and Haiti (17 per cent ). The median duration of evolution showed two trends: growth with equity in Colombia and Haiti, and stagnation with unequal distribution among the population subgroups of the last decade in Brazil, Peru and the Dominican Republic. In the multiple model of regression variables and the demographic profile of use of health services were associated with duration of EBF. The residence in a rural area was the variable consistently associated negatively in Brazil (HR = 1.68, CI 95 per cent : 1,06-2,67) and Colombia (HR = 1.39, CI 95 per cent : 1,03-1,87), while positively in Peru (HR = 0.40, CI 95 per cent : 0,19-0,83). Conclusion: The balance of the trend of EBF in LAC is positive, though not uniform throughout the two decades analyzed. The findings suggest the need for interventions for the promotion of exclusive breastfeeding taking into account the geographical location of families and provided quality health services

Esta tese propõe um modelo estratégico para o cluster calçadista do Cariri-CE, tendo como motivação identificar quais componentes estratégicas influenciam o desenvolvimento do Cluster, das 21 identificadas após uma Revisão Sistemática da Literatura. O trabalho, inicialmente, envolveu 8 empresas de diferentes portes da região estudada. Por intermédio de um método de pesquisa qualitativo verificou-se a adoção das 21 componentes estratégicas estas componentes, foram incorporadas no modelo estratégico proposto. Na sequência, utilizando-se uma amostra de 50 empresas do Cluster, foi aplicado um questionário aos dois principais gestores destas empresas. As análises estatísticas feitas permitiram dividir as empresas consultadas em 3 grupos, com características diferentes e a análise fatorial permitiu definir um modelo estratégico com 4 fatores de entrada e 4 fatores de saída. Identificou-se que, as empresas do Grupo 1 adotam estratégia de foco, mas não investem em diferenciação e qualidade dos seus produtos e possuem baixo desempenho econômico. As empresas do Grupo 2 possuem média superior em desempenho econômico as dos demais grupo devido à adoção da estratégia de foco e diferenciação representada pela qualidade. Já as empresas do Grupo 3 possuem médio desempenho econômico, buscando se diferenciar, mas não alcançam o desempenho econômico superior em comparação as empresas do Grupo 2. No geral, pode-se concluir que as empresas do Cluster calçadista do Cariri-CE não se diferenciam em relação as estratégias de custo; todas possuem igual preocupação com os custos envolvidos na sua operação, e que, principalmente, as estratégias de foco e diferenciação levam as empresas a obter bom desempenho econômico.
This research proposes a strategic model for the Cariri-CE footwear cluster, motivated by the need to identify among the 21 components identified in a Systematic Review of Literature what strategic components influence the development of the Cluster. At first the work involved 8 companies of different sizes in the studied area and a qualitative research method verified the adoption of the 21 strategic components into the proposed strategic model. After, a questionnaire was applied to the two main managers of 50 companies of the Cluster and by Statistical analyzes the companies was divided into three groups of different characteristics and the factor analysis defined a strategic model with 4 input factors and 4 exit factors. Companies of the Group 1 adopt a focus strategy, but do not invest in differentiation and quality of their products and have low economic performance. Companies of the Group 2 have a higher average in economic performance than the other groups due to the adoption of the focus and differentiation strategy represented by quality. Companies of Group 3, on the other hand, have a moderate economic performance, trying to differentiate themselves, but they do not achieve the superior economic performance in comparison with Group 2 companies. In general, it can be concluded that the Cariri-CE Cluster companies do not differ in relation to cost strategies; All are equally concerned about the costs involved in their operation, and that, in particular, focus and differentiation strategies lead companies to achieve good economic performance.

Resumo: O efeito de dentifrícios fluoretados na prevenção da cárie dentária na dentição decídua é concentração-dependente. No entanto, não há um modelo de ciclagens de pH validado em termos de dose-resposta ao fluoreto (F) para esmalte de dente decíduo humano. Assim, esse estudo validou um modelo de ciclagens de pH em termos de dose-resposta ao F para avaliar a desmineralização do esmalte de dente decíduo, e posteriormente, o modelo foi testado com dentifrícios comerciais com diferentes concentrações de F. Dois estudos independentes foram realizados e blocos de esmalte de dente decíduo (3 X 3 X 2 mm), selecionados pela dureza de superfície, foram usados em ambos. Para validar o modelo, blocos dentais (n=12/grupo) foram submetidos ao modelo de ciclagens de pH e tratados duas vezes ao dia com: água purificada (controle negativo) e soluções contendo 62,5; 125, 250 e 375 ?g F/mL. Essas concentrações de F foram escolhidas para simular a diluição (1:3 g/g) que ocorre na cavidade oral quando dentifrícios contendo 250, 500, 1000 e 1500 ?g F/g, respectivamente, são usados. O modelo durou 10 dias e os blocos permaneceram diariamente por 6 h em solução desmineralizante e 18 h em solução remineralizante a 37°C. Após a validação, o modelo foi testado com dentifrícios comerciais e blocos de esmalte (n=15/grupo) foram tratados com: dentifrício não fluoretado, 500 ?g F/g, como dentifrício de baixa concentração de F, 1100 e 1450 ?g F/g, como dentifrícios convencionais. A porcentagem de perda de superfície (%PDS) e área da lesão de cárie (?S) foi determinada. O efeito dose-resposta ao F foi analisado por regressão quadrática e o efeito do teste com dentifrícios pelo teste de Tukey. O modelo mostrou relação dose-resposta entre concentração de F e %PDS (R²=0,7047; p<0,01) e ?S (R²=0,4465; p<0,01). Em conclusão, o modelo proposto validado é capaz de diferenciar o potencial anticárie de dentifrícios de baixa concentração de F (500 ?g F/g) dos dentifrícios de concentração convencional (1000-1500 ?g F/g)
Abstract: The effect of fluoridated toothpaste on dental caries prevention in primary dentition is concentration-dependent. However, there is no pH-cycling model validated to assess the dose-response to fluoride (F) in human primary teeth enamel. Thus, this study validated a pH-cycling model in terms of dose-response to F to evaluate primary teeth enamel demineralization, and later, the model was tested with commercial toothpastes with different F concentrations. Two independent studies were conducted and primary teeth enamel slabs (3 X 3 X 2 mm), selected according to the surface hardness, were used in both. To validate the model, dental slabs (n=12/group) were submitted to the pH-cycling model and treated twice a day with: purified water (negative control) and solutions containing 62.5; 125; 250 and 375 ?g F/mL. These F concentrations were chosen to simulate the dilution (1:3 w/w) that occurs in the oral cavity when dentifrices containing 250, 500, 1000 and 1500 ?g F/g, respectively, are used. The model lasted 10 days and the slabs remained daily for 6 h in a demineralizing solution and 18 h in a remineralizing solution at 37°C. After validation, the model was tested with commercial toothpastes and enamel slabs (n=15/group) were treated with: no F toothpaste, 500 ?g F/g, as low F toothpaste, 1100 and 1450 ?g F/g, as standard toothpastes. The percentage of surface microhardness loss (%SMHL) and the carious lesion area (?S) were determined. The dose-response effect to F was analyzed by quadratic regression and the toothpaste effect by Tukey¿s test. The model showed dose-response relationship between F concentration and %SMHL (R²=0.7047; p<0.01) and ?S (R²=0.4465; p<0.01). When the pH-cycling model was used to evaluate the anticaries potential of commercial toothpastes, the mean (±SD) of %SMHL and ?S for the group treated with 500 ?g F/g was 36.6±8.0 and 6298.5±1221.3, respectively, which were significantly higher than those treated with 1100 (25.2±8.7 and 4565.7±1122) and 1450 ?g F/g toothpastes (24.2±5.2 and 2339.1±879.7), respectively. In conclusion, this validated model is able to differentiate the anticaries potential of toothpaste with low F concentration (500 ?g F/g) from those with standard concentrations (1000-1500 ?g F/g)

As medidas propostas no combate às cáries, apesar das dificuldades sócio-econômicas da comunidade e das disparidades regionais, têm produzido efeitos positivos em alguns locais. São comunidades integradas, participantes e motivadas que parecem estar fazendo a diferença nesta luta. Esses núcleos assistenciais, tidos como exemplares por sua racionalidade e produtividade, foram visitados e observados neste trabalho e deles subsídios importantes para futuros empreendimentos neste setor. Embora tenha-se encontrado alguns serviços mal estruturados, com coordenação omissa e produzindo aquém doas expectativas, não se pode assinalar que houve negligência por parte de seus funcionários e indiferença expressa dos responsáveis pela idealização e planejamento dos mesmos. Contudo, fica claro que onde tem-se pessoal treinado e motivado para as tarefas indicadas, obtém-se produtos e bens de qualidade. A late diagnosis of oral squamous cell carcinoma is related to high morbidity and mortality rates, as well as recurrence after treatment. Hence, there is an increasing interest in the validation of biological markers, chemoprevention strategies, and adjuvant treatment alternatives for combating oral cancer. The present Thesis evaluated the anticancer effect of cannabidiol (CBD) in a validated experimental murine model of oral carcinogenesis. Fifteen Fischer 344 rats were randomly divided into three groups of five animals each and subjected to a 7,12-dimethylbenzanthracene (DMBA) topical application on the ventral mucosa of their tongues, thrice a week. From the 2nd to the 12th week, they received intraperitoneally-administered vehicle (group 1), CBD 5 mg/kg (group 2), or CBD 10 mg/kg (group 3). After euthanasia, the tongues were dissected, processed and assessed by histological and immunohistochemical analysis. Group 2 and group 3 showed inhibition of severe oral epithelial dysplasia and carcinoma, and exhibited lower cell proliferation as compared to group 1. The null hypothesis was rejected when results showed statistical significance at a 0.05 level (confidence interval = 95%). In sequence are presented three manuscripts, first one regarding the original experiment and the two subsequent ones providing overall theoretical support.
O diagn?stico tardio do carcinoma espinocelular oral est? relacionado a um alto ?ndice de morbi-mortalidade e recorr?ncia ap?s o tratamento. Portanto, h? um crescente interesse na valida??o de marcadores biol?gicos que contribuam para o diagn?stico precoce, novas estrat?gias de quimiopreven??o e recursos adjuvantes para o tratamento dessa neoplasia maligna. A presente Tese centrou-se na avalia??o do efeito anticarcinog?nico do canabidiol (CBD) em um modelo experimental consagrado para a indu??o de altera??es epiteliais, com risco de malignidade, na mucosa oral de murinos. Para tanto, 15 ratos Fischer 344 foram aleatoriamente divididos em tr?s grupos, onde todos os animais tiveram o ventre de suas l?nguas expostas ao 7,12-dimetilbenzantraceno (DMBA) tr?s vezes por semana. Background: Early Childhood Caries (ECC) is one of the most common chronic illnesses among young children, affecting around 23% of children 2-5 years old, leading to pain, discomfort, and poor quality of life. It is a multi-factorial disease that develops through the combined effects of bacteria, tooth morphology, fermentable carbohydrates, time, and various social factors. Several studies have investigated the associations between dental caries and non-traditional factors acquired during the first years of life including; mode of delivery, breastfeeding, and Environmental Tobacco Smoke (ETS). However, the literature on these associations has been inconsistent. Objective: To investigate the unadjusted and adjusted associations between the presence of Early Childhood Caries (ECC) and Severe-ECC (S-ECC), and three non-traditional factors: breastfeeding, mode of delivery, and ETS, among children 1-5 years old. Methods: A cross-sectional design was employed, and a sample of 112 caregiver/ child dyads was recruited from the ongoing flow of patients at Temple University Maurice H. Kornberg School of Dentistry (TUKSoD). After consent, subjects completed a questionnaire and received a standard intra-oral examination and the American Academy of Pediatric Dentistry (AAPD) Caries-risk Assessment Tool (CAT) by a student doctor. The study was approved by the Temple University Institutional Review Board (Protocol # 23885). Chi-square tests, two-sample t-tests and bivariate logistic regressions were used to assess the unadjusted associations. Two multivariable logistic models were developed for ECC and S-ECC and included demographics, overall CRA, and the three non-traditional risk factors. Results: The prevalence of ECC and S-ECC were 61% and 30%, respectively. The following variables were significant in the unadjusted analysis for both ECC and S-ECC: child’s age, maternal educational attainment, overall AAPD CAT classification, sugary snacks per day, presence of plaque on child’s teeth and ETS. Exposure to ETS was associated with an increased adjusted odds ratio for ECC (aOR=5.39 [95% CI: 1.14-25.33], P=0.033), but not for S-ECC. Furthermore, C-section birth was associated with a decreased adjusted odds ratio for both ECC and S-ECC, respectively (ECC: aOR=0.132 [95% CI: 0.02-0.72], P=0.02; S-ECC: aOR=0.141 [95% CI: 0.026-0.748], P=0.021). With inclusion of the AAPD CAT, demographics, and the three non-traditional factors, the overall model accuracy at predicting ECC was 82.2%. Conclusions and Clinical Relevance: In this study of urban, predominantly African American, and low income children, ECC was found associated with two non-traditional factors, ETS and mode of delivery, suggesting that including them in CRA may improve prediction of future dental caries, and aid in the prevention and treatment of disease. Results from this study support the notion that ECC is a multi-factorial disease, and highlights the importance of adopting oral health education among caregivers.

ABSTRACT This dissertation consists of two papers, the first one is a review and the second is an original paper. In the first article, the authors reviewed the literature on cancer and propolis, addressing experimental carcinogenesis, chemically induced by 9,10-dimethyl-1,2-benzanthracene (DMBA), the chemical composition of propolis and its chemopreventive effects. The aim of the research paper was to evaluate the?tissue response?of tongue mucosa of hamsters, subjected to daily and alternating application of ethanolic extract of propolis (EEP) of a commercial presentation. Materials and methods: 60 hamsters were divided into three groups, and two experimental periods, 13 and 20 weeks. The lateral edge of the tongue underwent daily and alternate topical application of DMBA 0.5% and 30% EEP (EEP group, n = 20), DMBA and 0.5% aqueous extract of propolis (EAP) (EAP group; n = 20), and DMBA 0.5%?saline (DMBA?group, n =?20). The occurrence of types of clinical and histological changes was analysed. The area and volume of clinical changes, the occurrence of structural (AE) and cytological (CA) changes of squamous epithelial tissue with atypia and the histological area of squamous cell carcinoma (SCC) were measured. For statistical analysis, ANOVA, chi-square and Kruskal-Wallis and Mann-Whitney test were applied. Results: For all variables, there were no significant differences when comparing the groups in the two experimental periods. In 13 weeks, a single lesion of SCC in EEP group was observed. At 20 weeks, the highest occurrence of SCC was also in the group EEP. Conclusion: the EEP, with an alcohol content of 30% favored tissue reaction of initiation and promotion of carcinogenesis by mechanisms not yet elucidated.

Modelo predictivo de caries en preescolares Mazahuas del Municipio de San José del Rincón, Estado de México.
Construir un modelo predictivo de caries en preescolares Mazahuas del Municipio de San José del Rincón, Estado de México. Métodos: La muestra fue seleccionada por el método de muestreo no-probabilístico por conveniencia, que incluyó 131 niños de 3 a 5 años de edad de ascendencia Mazahua. La examinación clínica fue realizada mediante el Sistema Internacional para la Detección y Evaluación de Caries (ICDAS). La información de las variables sociodemográficas, de hábitos dietéticos y de higiene oral se obtuvieron a través de una entrevista con los padres o cuidadores principales de los niños. Se construyó un modelo lineal automatizado para identificar los predictores. Resultados: La prevalencia de caries fue de 86.3% donde el 63.4% de los niños presentó Caries Temprana de la Infancia (CTI), mientras que sólo el 22.9% tuvo Caries Temprana de la Infancia Severa (CTI-S). El modelo mostró que el cpos depende de: número de veces que el niño está expuesto a alimentos con azúcares durante las comidas, nivel educativo del cuidador principal, presencia de materia alba, uso de azúcar o miel en el biberón para alimentar al niño. Conclusiones: El modelo predictivo mostró que el cpos fue mayor en los niños que se exponen más veces a azúcares durante las comidas, con presencia de materia alba y aquellos a quienes les dieron biberón y le agregaron azúcar o miel, mientras que fue menor en los niños cuyos cuidadores principales tienen un mayor nivel educativo. Alta prevalencia de caries no tratada.

Resumo: Esta tese propõe um modelo estratégico para o cluster calçadista do Cariri-CE, tendo como motivação identificar quais componentes estratégicas influenciam o desenvolvimento do Cluster, das 21 identificadas após uma Revisão Sistemática da Literatura. O trabalho, inicialmente, envolveu 8 empresas de diferentes portes da região estudada. Por intermédio de um método de pesquisa qualitativo verificou-se a adoção das 21 componentes estratégicas estas componentes, foram incorporadas no modelo estratégico proposto. Na sequência, utilizando-se uma amostra de 50 empresas do Cluster, foi aplicado um questionário aos dois principais gestores destas empresas. As análises estatísticas feitas permitiram dividir as empresas consultadas em 3 grupos, com características diferentes e a análise fatorial permitiu definir um modelo estratégico com 4 fatores de entrada e 4 fatores de saída. Identificou-se que, as empresas do Grupo 1 adotam estratégia de foco, mas não investem em diferenciação e qualidade dos seus produtos e possuem baixo desempenho econômico. As empresas do Grupo 2 possuem média superior em desempenho econômico as dos demais grupo devido à adoção da estratégia de foco e diferenciação representada pela qualidade. Já as empresas do Grupo 3 possuem médio desempenho econômico, buscando se diferenciar, mas não alcançam o desempenho econômico superior em comparação as empresas do Grupo 2. Abstract: This research proposes a strategic model for the Cariri-CE footwear cluster, motivated by the need to identify among the 21 components identified in a Systematic Review of Literature what strategic components influence the development of the Cluster. At first the work involved 8 companies of different sizes in the studied area and a qualitative research method verified the adoption of the 21 strategic components into the proposed strategic model. After, a questionnaire was applied to the two main managers of 50 companies of the Cluster and by Statistical analyzes the companies was divided into three groups of different characteristics and the factor analysis defined a strategic model with 4 input factors and 4 exit factors. Companies of the Group 1 adopt a focus strategy, but do not invest in differentiation and quality of their products and have low economic performance. Companies of the Group 2 have a higher average in economic performance than the other groups due to the adoption of the focus and differentiation strategy represented by quality. Companies of Group 3, on the other hand, have a moderate economic performance, trying to differentiate themselves, but they do not achieve the superior economic performance in comparison with Group 2 companies. In general, it can be concluded that the Cariri-CE Cluster companies do not differ in relation to cost strategies; All are equally concerned about the costs involved in their operation, and that, in particular, focus and differentiation strategies lead companies to achieve good economic performance

Os perciformes constituem o maior e mais diversificado grupo de peixes. Uma parcela consider?vel de suas esp?cies apresenta um marcante conservadorismo cromoss?mico exibindo um padr?o caracterizado por 2n=48a, NF=48, que tem sido apontado como uma condi??o basal para a Ordem. Blocos heterocrom?ticos reduzidos e RONs simples s?o caracter?sticos para este grupo de peixes. No entanto, n?o se encontra ainda bem estabelecido se este conservadorismo se deve, em parte, a dados provenientes de bandamentos cromoss?micos pouco resolutivos, como os convencionalmente utilizados nas caracteriza??es citogen?ticas das esp?cies marinhas ou a uma condi??o peculiar cariot?pica deste grupo. Visando clarificar os processos envolvidos no peculiar conservadorismo cromoss?mico observado nesta Ordem, cinco esp?cies da fam?lia Haemulidae foram submetidas ? variadas t?cnicas citogen?ticas como colora??o com Giemsa, bandamento C e impregna??o por nitrato de prata, bem como digest?o com enzimas de restri??o (AluI, TaqI, PstI e EcoRI), bandamento de replica??o pela incorpora??o do an?logo de base 5 BrdU, colora??o com os fluorocromos CMA3/MM e DAPI, double FISH com sondas para as subunidades ribossomais 5S e 45S, sendo tamb?m analisadas morfometricamente atrav?s de morfometria geom?trica (MG). Os dados obtidos permitiram identificar um alto grau de similaridade cariot?pica neste grupo independente do n?vel de resolu??o das t?cnicas utilizadas. As esp?cies Conodon nobilis, Pomadasys corvinaeformis, Haemulon aurolineatum, H. plumierii e H. steindachneri apresentaram uma macroestrutura cariot?pica comum composta por 2n=48a (NF=48), com RONs simples localizadas em um mesmo par cromoss?mico (24? par) nas esp?cies C. nobilis, H. aurolineatum H. plumierii e H. steindachneri e em outro par em P. corvinaeformis (18? par), considerando-se estas regi?es caracteres citotaxon?micos pouco importantes. O padr?o heterocrom?tico apresentou-se similar para todas as esp?cies, observando-se reduzidos blocos heterocrom?ticos detectados preferencialmente em regi?o centrom?ricas e em menor n?mero em regi?es pericentrom?ricas e telom?ricas. Contrastando com a reduzida diferencia??o cromoss?mica observada, as an?lises por MG indicaram uma consp?cua diferencia??o morfol?gica entre as esp?cies.
