Full textThe purpose of this work was to identify the principal factors that can account for the inter-individual difference in the psychological adjustment of patients with severe burns that require hospitalization. For this study we used a multifactorial and dynamic model exploited in health psychology which is based on biopsychosocial theories. The results show on the one hand, that adjustment can be evaluated by a coherent pattern of somatic, emotional and behavioral criteria. On the other hand, the results show that the adjustment of burn patients to diverse medical and hospital stressors are not linearly dependant on the objective seriousness of the burns. Indeed, the other predictive variables (sociobiographical characteristics, premorbid psychological profiles, life events, information given to the patients) and moderating variables (cognitive : coping, locus of control, social support, and affective: trait-anxiety, psychical satisfaction) influence the stress-distress relationship

Full textDespite crucial role of trace elements in functions disturbed by thermal aggression (immunity, inflammation, oxidative stress, insulin system) and enhanced deficits, burned patient supplementation with trace elements keeps always restricted and unsettled. We used an animal experimental model to study Zn, Cu, Se and Cr status modifications since the first hours after burn injury and relation with oxidative stress and glucose/insulin system markers. Zn, Se and Cr status are impaired. The brain is protected, liver and muscle are deficit target tissues. Selenium status is the earliest modified, and if Se supplementation can restore it, it is not sufficient in correcting oxidative stress induced according to burn size. Other micronutrients, vitamin E, Zn and Cr seem involved. Our results suggest that burned patient supplementation has to be early started and to combine several micronutrients

Full textIn vivo, the skin regenerates continuously through the proliferation of basal stem cells. That can be obtained from a skin biopsy and may be grafted to regenerate skin. In vitro, we studied the regenerative keratinocyte capacities according to site donors, and we noted that keratinocytes from foreskin have the greatest proliferative and regenerative capacities, combined with expression of progenitor cell markers (p63 and keratin 19) and low expression of epidermal differentiation markers (involucrin and filaggrin). These results led us to use these preputial keratinocytes in suspension for graft in burned children. Apply directly cells with a suspension device, eliminates the use of a transfer membrane and a potential source of inefficiency. In our study, boys grafted with autologous keratinocytes isolated from foreskin leads to epithelialization with a high quality of scar. This technique is easily applicable to a large number of pediatric surgery teams. Our clinical success and the safety of the procedure in the operating room enable us to propose the kératinocyataire autologous transplantation as a standard procedure, which can be added to the therapeutic armamentarium in burns

Full textThe aim of this study was to evaluate the viability of cryopreserved human skin allografts. For this purpose, we developed the MTT assay on skin samples harvested from multi-organ and tissue donors. Then viability was assessed at laboratory reception, during storage at +4°C (at day 2 and day 7), and after cryopréservation and thawing, with two different transport media (Ringer Lactate versus RPMI) and two different cryoprotectants (Glycerol versus DMSO). Histological analysis was performed for each MTT assay. We also tried to correlate allograft viability with efficacy of coverage in a xenograft model (human/mice). Results indicate that when stored at +4°C, skin retains more viability with RPMI, whereas Glycerol and DMSO are equivalent cryoprotectant regardless of the transport medium. In our model of xenograft, cryopreserved skin was less rejectable than fresh skin. The use of cryopreserved skin does not change the potential for healing

Full textLewisite is an organic arsenical compound ranked among the vesicating agents whose antidote is the British Anti-Lewisite. This chemical warfare agent remains a current threat and generates damage ranging from erythema to vesication, larger or smaller depending on the dose received after skin contact. Only few data are available regarding the toxicity on human and are sometimes contradictory. Therefore, the relevance of the description of the symptoms and their chronology of apparition are limited. Current treatments of skin burns are not specific, only symptomatic and are closed to those used to treat thermal burns. In such context, we consider it important to gain better understanding of the injury mechanisms associated with skin exposure to lewisite. These understanding stages of the physiopathology aim at identifying potential targets for pharmacological interventions and are necessary and prerequisites for the establishment of new curative strategies. It is within this framework that our investigations are based on. An appropriate experimental approach was conducted using the SKH-1 hairless mouse. In the first place, the aim was to describe the time course of skin lesions development induced by exposure to saturating vapors of lewisite on the dorsal skin of the mouse. The injury process was monitored throughout the first 3 weeks after exposure in order to cover the different phases of wound healing. A long term monitoring throughout 27 weeks was also performed. The use of several methods of functional exploration has enabled the study of the skin color, the evaluation of the integrity of the barrier function and the biomechanical properties of the skin. These studies were completed by microscopic analyses of the injured tissues. In a second step, key targets of the lewisite skin toxicity were identified by genomic and biochemical approaches. The transcripts of the target genes were quantified by RT-qPCR in a 7-day kinetic after exposure. Data were completed by protein analyses involving complementary analytical methods (antibody array, western blot, gel zymography in a 14-day kinetic. Lastly, a combination of hydrosurgical debridement and matrix therapy was assessed and considered in order to treat skin lesions induced by lewisite

Full textTo enhance the production conditions for cultured epidermal autografts (CEA) for large burns, we sought to study the in vitro effect of a low oxygen level on epidermal growth. We tested this parameter on CEA grown on murin feeder cells (Green's method) as well as human feeder cells. We first could evidence that the optimal feeder density depended on the oxygen level. A feeder density made optimal at 20% 02 could prove inhibitory on keratinocyte growth at 3% 02. At their respective best feeder densities, low oxygen level (3%) led to an average 4,2 fold increase in keratinocyte yield for a same arrest day as compared with 20% culture. This effect proved to be stable on several successive passagings, showing the increase in proliferation did not take place at the expense of tell self renewal. Keratinocytes grown at a low oxygen level kept their ability to form a stratifying epidermis on an in vivo NOD/SCID mouse excisional model. In parallel, the increase in proliferation was also observed when keratinocytes were cultured on human feeder cells, bone marrow mesenchymal stroma' cells and dermal fibroblasts. This effect of a low oxygen tension on keratinocytes appears to be partly direct, as the growth rate of HaCat feederless keratinocytes was enhanced at 3% vs 20% 02. It is also partly an indirect effect, as conditioned medium from murin feeder cells cultured in hypoxia has a more pronounced positive effect on keratinocyte growth than its normoxic counterpart. These preliminary resuits could lead to the modification on the culture protocol currently in use for the majority of CEA grafts for large burns. The expected benefits for the patients, beyond slightly shortening culture time, would include salvaging abortive cultures and bringing less differentiated keratinocytes, a parameter linked with a decrease in fibrotic evolution on murin models
