Abstract:
In the Unites States, approximately 24% of people undergoing primary total knee or total hip arthroplasty (TKA, THA) are chronic opioid users pre-operatively. Few studies have examined the incidence of opioid use prior to TKA/THA and whether it predicts outcomes post-surgery in the Australian context. The aim was to determine: (i) the proportion of TKA and THA patients who use opioids regularly (daily) pre-surgery; (ii) if opioid use pre-surgery predicts (a) complication and readmission rates to 6-months post-surgery, (b) patient-reported outcomes to 6-months post-surgery.A retrospective cohort study was undertaken utilising linked individual patient-level data from two independent databases comprising approximately 3500 people. Patients had surgery between January 2013 and June 2018, inclusive at Fairfield and Bowral Hospitals.Following data linkage, analysis was completed on 1185 study participants (64% female, 69% TKA, mean age 67 (9.9)). 30% were using regular opioids pre-operatively. Unadjusted analyses resulted in the following rates in those who were vs were not using opioids pre-operatively (respectively); acute adverse events (39.1% vs 38.6%), acute significant adverse events (5.3% vs 5.7%), late adverse events: (6.9% vs 6.6%), total significant adverse events: (12.5% vs 12.4%), discharge to inpatient rehab (86.4% vs 88.6%), length of hospital stay (5.9 (3.0) vs 5.6 (3.0) days), 6-month post-op Oxford Score (38.8 (8.9) vs 39.5 (7.9)), 6 months post-op EQ-VAS (71.7 (20.2) vs 76.7 (18.2), p<0.001), success post-op described as “much better” (80.2% vs 81.3%).Adjusted regression analyses controlling for multiple co-variates indicated no significant association between pre-op opioid use and adverse events/patient-reported outcomes.Pre-operative opioid use was high amongst this Australian arthroplasty cohort and was not associated with increased risk of adverse events post-operatively. Further research is needed in assessing the relationship between the amount of pre-op opioid use and the risk of post-operative adverse events.

Abstract:
Over sixty years ago, Walter Headlam identified Ecclesiazusae 960–76 as a paraclausithyron, or song sung by an excluded lover from the street to his beloved within. In 1958, however, C. M. Bowra suggested that the whole of Eccl. 952–75 was actually the sole surviving example of a previously unrecognized genre of Greek lyric poetry, the informal love duet. The thesis has been widely accepted, and is adopted by Rossi, Henderson and Silk, as well as by the Oxford editor, Ussher, who rejects Headlam's identification explicitly. Only Zimmermann has failed to embrace Bowra's interpretation wholeheartedly, although he offers no detailed discussion of the passage. In fact, Eccl. 952–75 is not evidence for a lost lyric genre, but a sophisticated literary parody, carefully designed as an elaborate poetic comment on the larger action of the play in which it appears. Bowra's ‘love duet’ is a critical fantasy, whose fictional existence only serves to obscure the real purposes and humour of this Aristophanic love-song.

Objective of the Paper: to demonstrate the observation of the long-term consequences of postresection short bowel syndrome in a 13-year- old boy. Key Рoints. Short bowel syndrome (SBS) is characterized by a low absorption capacity of the small intestine as a result of extensive resections, which is manifested by malabsorption, malnutrition and digestive disorders. Observation of a 13-year-old child with a history of postresection short bowel syndrome 10 years after acquiring intestinal autonomy (independence from parenteral nutrition) illustrates the long-term consequences SBS: megaloblastic B12-deficient anemia with neurological symptoms, delayed physical development, persistent anastomositis. Conclusion. In children with a history of short bowel syndrome, after acquiring intestinal autonomy, some risks remain that significantly disrupt the patient's health. Such patients need long-term food insurance and supervision by a multidisciplinary team of specialists. Aim: To evaluate the quality of bowel preparations concerning the Boston Bowel Preparation Score (BBPS) and to analyze the administration, ease, palatability, and side effects of practices. Study design: Cross-sectional study Place and Duration: this study was conducted in Liaquat University of Medical and Health Sciences Jamshoro from October 2021 to March 2022 Methodology: The present study incorporated 132 patients who visited the hospital for a colonoscopy. Patients above 18 years of age were included, whereas patients with surgical procedures were excluded from the study. Participants were evaluated through pre-designed proforma for demography, dietary recommendations, side effects, procedure indications, bowel preparations, and patient ease of the bowel preparations. BBPS was used to access the bowel preparation. A score ≥ 5 was regarded as satisfactory. Results: Out of 132 patients, 73 (55.3%) were males. Seventy-one (53.8%) patients received Polyethylene glycol-based bowl preparation, and sixty-one (46.2%) patients received sodium phosphate-based preparation. Chronic constipation was the most typical cause of colonoscopy. Common side effects were vomiting (12.1%), nausea (1.5%), and bloating (3%). More than half of them responded that the preparations were palatable. A satisfaction rate of 80.3% was observed in patients with both preparations. BBPS of above five was observed in 109 (82.6%) patients. Conclusion: Polyethylene glycol and sodium phosphate preparations have been widely used in clinical practices for cleansing the colon. Environmental factors, particularly diet, are considered central to the pathogenesis of the inflammatory bowel diseases (IBD), Crohn’s disease and ulcerative colitis. In particular, the Westernization of diet, characterized by high intake of animal protein, saturated fat, and refined carbohydrates, has been shown to contribute to the development and progression of IBD. During the last decade, soybean, as well as soy-derived bioactive compounds (e.g., isoflavones, phytosterols, Bowman-Birk inhibitors) have been increasingly investigated because of their anti-inflammatory properties in animal models of IBD. Herein we provide a scoping review of the most studied disease mechanisms associated with disease induction and progression in IBD rodent models after feeding of either the whole food or a bioactive present in soybean.

Introduction. Meckel diverticulum-is a residue of not completely reduced yolk duct. Among the occurring complications of Meckel’s cuticle in the adult population, acute intestinal obstruction prevails. In this case, the most common intussusception and inversion of the intestines. The occurrence of small bowel obstruction due to phytobesoar in the Meckel diverticulum is a rare condition and has been described in the literature in isolated cases. Materials and methods. The patient of 29 years entered the surgical Department with complaints of abdominal pain of a permanent nature, violation of the discharge of stool and gases, vomiting up to 4 times. Sick for about 2 days. Fluoroscopy of the abdomen showed multiple arches with small bowel fluid levels, single bowl kloybera. The man was operated on urgently with a diagnosis of acute mechanical small bowel obstruction. Results. During the operation, Meckel’s phytobezoar diverticula was detected, which caused acute small bowel obstruction, for which the fragmentation of the bezoar was performed with its relegation to the cecum. The postoperative period proceeded without complications, the patient was discharged on the 10th day after surgery. After 6 weeks, the patient underwent laparoscopic diverticulectomy as planned. Discharged for 5 days. Conclusion. A clinical case of treatment of a patient with phytobezoar Meckel diverticulum complicated by acute mechanical small bowel obstruction is presented. The use of delayed diverticulectomy can reduce the risk of anastomosis failure in acute intestinal obstruction, and if proper diet is observed, it reduces the risk of recurrent intestinal obstruction before re-planned hospitalization.

Background. Small bowel obstruction is a common and dangerous surgical emergency which is associated with high morbidity and mortality if not managed appropriately and timely. Objective. To determine the causes and management outcome of small bowel obstruction in Nekemte Referral Hospital, Nekemte, Ethiopia. Method. Institution-based retrospective cross-sectional study design was used. Three-year data (from January 1, 2014, to December 30, 2016) were collected from July 1 to August 30, 2017. Data were collected from medical records and checked for any inconsistency, coded, and entered into SPSS version 20 for analysis. Descriptive, binary, and multivariate logistic regression analyses were used. On binary logistic regression analysis, variables with p ≤ 0.25 were selected as a candidate for multivariate logistic regression analysis. The level of statistical significance was set at p ≤ 0.05 . Results. With 100% response rate, records of 211 patients with small intestinal obstruction were retrieved for analysis. One hundred thirty-seven (64.9%) were males. The commonest cause of small bowel obstruction was adhesion (35.1%). More than a quarter (26.5%) participants developed postoperative complications, and wound infection was the commonest postoperative complication (49.2%). A majority (84.8%) of patients improved and were discharged, and the rest 15.2% of patients died. Sex (AOR = 3.98, 95% CI: 1.51–10.52), duration of illness before surgical intervention (AOR = 4.4, 95% CI: 1.69–11.45), level of hematocrit (AOR = 4.25, 95% CI: 1.56–11.57), types of intestinal obstruction (AOR = 3.73, 95% CI: 1.09–12.64), and length of hospital stay (AOR = 4.69, 95% CI: 1.82–12.07) were independent predictors of the management outcome of patients with small bowl obstruction. Conclusion. Small bowel obstruction is a commonly encountered surgical emergency. Adhesion, small bowel volvulus, and intussusception were the leading causes of small bowel obstruction, respectively.

Abstract The odd-skipped (odd) gene, which was identified on the basis of a pair-rule segmentation phenotype in mutant embryos, is initially expressed in the Drosophila embryo in seven pair-rule stripes, but later exhibits a segment polarity-like pattern for which no phenotypic correlate is apparent. We have molecularly characterized two embryonically expressed odd-cognate genes, sob and bowel (bowl), that encode proteins with highly conserved C2H2 zinc fingers. While the Sob and Bowl proteins each contain five tandem fingers, the Odd protein lacks a fifth (C-terminal) finger and is also less conserved among the four common fingers. Reminiscent of many segmentation gene paralogues, the closely linked odd and sob genes are expressed during embryogenesis in similar striped patterns; in contrast, the less-tightly linked bowlgene is expressed in a distinctly different pattern at the termini of the early embryo. Although our results indicate that odd and sob are more likely than bowl to share overlapping developmental roles, some functional divergence between the Odd and Sob proteins is suggested by the absence of homology outside the zinc fingers, and also by amino acid substitutions in the Odd zinc fingers at positions that appear to be constrained in Sob and Bowl.

The small bowel tumor diagnosis is often late, in the course of this affection, which is explained by the low number of tumors and crude symptomatology (abdominal pain, wight loss, queasiness, vomit, ocult bleeding of gastro-intestinal tract). There is no unique investigation method of the small bowel for patient suspect of SMT. Choices are either X-ray (CT-scan, enteroclysis etc.) or endoscopic (upper endoscopy, wireless video endoscopy etc.). It has not been decided upon the best strategy or the series of investigations. The patient usually requires full imagistic explorations; laparotomy being sometimes useful in the selection of a positive diagnosis.

Plant-derived protease inhibitors (PI), such as Bowman-Birk inhibitors and Kunitz-type inhibitors, have been suggested to negatively affect dietary protein digestion by blocking the activity of trypsin and chymotrypsin in the human gastrointestinal system. In addition, some PIs may possess proinflammatory activities. However, there is also scientific evidence on some beneficial effects of PIs, for example, gut-related anti-inflammatory and chemopreventive activities in vitro and in vivo. Some PIs are sensitive to processing and digestion; thus, their survival is an important aspect when considering their positive and negative bioactivities. The aim of this review was to evaluate the relevance of PIs in protein digestion in humans and to discuss the potential of PIs from whole foods and as purified compounds in decreasing symptoms of bowel-related conditions. Based on the reviewed literature, we concluded that while the complex interactions affecting plant protein digestibility and bioavailability remain unclear, PI supplements could be considered for targeted purposes to mitigate inflammation and gastric pain.

Abstract:
A two-year-old male German short-haired pointer was presented with a 1.5-year history of intermittent small-bowel diarrhoea. Inflammatory bowel disease (chronic lymphocytic-plasmacytic gastritis, enteritis and colitis) was diagnosed on the basis of histological examination of biopsies obtained on repeated endoscopy and by exclusion of other possible causes. Warthin-Starry silver staining of stomach mucosa revealed the presence of gastric spiral organisms. The evidence of L. intracellularis was provided by a positive nested polymerase chain reaction in one biopsy of duodenal mucosa and in one rectal smear. In 5 blood sera collected over a period of 8 months the IgG antibodies to L. intracellularis were found by an indirect fluorescent antibody test. Treatment with oral prednisone led only to a temporary improvement.

Abstract:
Gallstone ileus is common in elderly female population. To obtain a diagnosis of gallstone ileus is a challenge requiring clinical and radiological assistance. It’s a rare cause of intestinal obstruction, accounts approximately 1-4%. Here we report a case of 56 years old lady presented with intestinal obstruction sign and symptoms. Per abdomen examination revealed generalised tenderness with sluggish bowel sound. Abdominal X-ray revealed prominent small bowel with presence of gas till rectum. CT abdomen noted intraluminal mass over distal small bowel loops mimickering intusseption. Exploratory laparotomy with small bowel enterotomy was performed. Intra-operative finding noted impacted gallstone measuring 2x3cm, 360cm from duodenal-jejunal flexure and 50cm from terminal ileum. Post-operative patient had speedy recovery and discharged home. Here we emphasize in elderly female patient presented with sign and symptoms of intestinal obstruction, diagnosis of gallstone ileus should be one of differential diagnosis.

Introduction: Feeding jejunostomy is a rather safe procedure. Major complications are rare, but can be serious, nevertheless. Case Report: We report a case of a 57-year-old male, diagnosed with epidermoid carcinoma of the larynx. The patient received 3 cures of induction chemotherapy before being lost to follow-up. One year later, the patient presented with significant dysphagia and benefited from a feeding jejunostomy utilizing the Witzel technique. Postoperative course was marked by abdominal tenderness, and hemodynamic and respiratory impairment. Lab tests showed a continuous elevation of markers of infection. Computed tomography (CT) scan revealed an extradigestive distal end of the jejunostomy tube, associated with a voluminous pneumoperitoneum, predominant around the tube end, keeping with a small bowel perforation. The evolution was marked by respiratory fatigue, and hemodynamic instability. The patient, unfortunately died subsequently, despite maximal supportive measures. Conclusion: Computed tomography (CT) scan is a helpful tool for the diagnosis of feeding jejunostomy’s major complications besides physical examination and biology. Limiting the indications of this surgical procedure and being attentive to surgical details may help reduce the risk of complications.

Inflammatory bowel disease (IBD) results from production of dysregulated gut mucosal immune response to luminal antigens. Scientific literature pertaining to Southern part of Indian is limited, especially with focus on demographic and clinical phenotypes of IBD. Hence, the present study is aimed at evaluating these factors in a geographic area of Uttar Karnataka in two types of IBD- Ulcerative colitis (UC) and Crohn’s disease (CD). Methodology: A retrospective study was done with 91 patients with IBD from 2009 to 2019. Patients were routinely started on treatment with local and systemic 5-aminosalicylic acid derivatives. Different line of therapies was administered to both UC and Crohn’s disease with different drugs or combination of drugs. Results: Of the 91 patients, 85 were having UC and 6 with CD. The mean age of patients with UC and CD was 41.8 years and 52.2 years; male predominance was observed in UC group of patients in contrast to CD group. UC patients were found with Extraintestinal manifestation i.e. Erythema nodosum (0.01%), Type 1 peripheral arthritis (0.01%), Type 2 peripheral arthritis (0.01%). A total of 6 UC patients required surgery for various causes, with the majority being males, including 2 of them who developed cancer. Conclusion: There was a predominance of patients with UC, especially in men. The age, origin and level of education may interfere with early diagnosis. This geographical region survey provides an opportunity for understanding possible etiopathogenetic factors associated with the disease and pattern of treatment.

The Trinity Peninsula Group (TPG) of northern Graham Land, a weakly metamorphosed thick sequence of predominantly quartz- and feldspar-rich greywacke, has tentatively been correlated with metasedimentary rocks exposed along the Bowman Coast of Graham Land (Stubbs 1968). The base of the TPG is not observed but the Bowman Coast rocks, here newly defined as the Bowman Coast Succession (BCS), is proximal to high-grade gneisses, which may represent the local basement. Recent geological mapping along the Bowman Coast has allowed a revision of the local geology (Fig. 1) and this note focuses on the relationship of the BCS with the adjacent gneisses.

Background: The diagnosis of inflammatory bowel disease (IBD) is based on a combination of endoscopic, clinical and biochemical investigations as well as cross-sectional imaging. The applications of cross-sectional imaging in IBD are manifold. Ultrasonography has emerged as an important imaging modality in the diagnosis of Crohn's disease (CD) as well as for monitoring disease progression and in the therapeutic response to CD and ulcerative colitis (UC). Key Messages: Ultrasonography is non-invasive, radiation free, cheap, easy to use and well tolerated and accepted by patients. Bowel ultrasonography can be used for the primary diagnosis of CD as it has a similar sensitivity and specificity like that of MRI and CT, particularly in the case of CD. Ultrasonography can also be used to monitor treatment response to therapy and to detect disease recurrence of CD as well as UC. In CD, ultrasonography can also be used to detect complications such as strictures as well as extramural complications, including abscesses and fistulas. Contrast-enhanced ultrasonography is a useful tool that might be helpful to detect certain indications in CD, in particular the differentiation between abscesses and inflammation. Conclusion: A variety of advantages of bowel ultrasonography over other imaging modalities suggest the more frequent use of this method to manage IBD patients in daily practice. Bowel ultrasonography should be a standard tool in IBD centers.

Daun mimba mengandung senyawa bioaktif diantaranya terpenoid, alkaloid dan flavonoid yang diduga memiliki efek dalam menurunkan kolesterol. Penelitian bertujuan untuk menguji efek pemberian ekstrak etanol daun mimba (Azadirachta indica A. Juss.) terhadap struktur ren tikus putih (Rattus norvegicus L.) yang diberi pakan tinggi lemak. Penelitian menggunakan Rancangan Acak Lengkap yang dibagi menjadi 6 kelompok dan 4 kali pengulangan, yaitu P0 (kontrol negatif), P1 (kontrol positif), P2 (simvastatin 8 mg), P3, P4 dan P5 (uji dosis ekstrak etanol daun mimba 75, 100 dan 125 mg). Variabel yang diamati yaitu bobot ginjal, diameter glomerulus, diameter kapsula Bowman dan tebal ruang Bowman. Hasil penelitian menunjukan tidak terdapat pengaruh bermakna (p>0,05) dari pemberian ekstrak etanol daun mimba terhadap bobot ginjal, diameter glomerulus, diameter kapsula Bowman dan tebal ruang Bowman pada ginjal tikus yang diberi pakan tinggi lemak. Kesimpulan penelitian ini yaitu ekstrak etanol daun mimba mampu memperbaiki struktur ginjal tikus hiperlipidemia.
