Abstract:
SUMMARYIn 1988, there were two outbreaks of legionellosis in Bolton Health District. Altogether 37 cases of Legionnaires' disease and 23 cases of non-pneumonic legionellosis were identified. Twenty-five patients with Legionnaires' disease were associated with an engineering plant, 4 with Bolton town centre, and 8 with both the plant and town centre. Twenty-two people with non-pneumonic legionellosis were linked with the engineering plant and one with the plant and the town centre. A case-control study carried out among 37 employees with legionellosis and 109 control subjects at the plant showed that infection was associated with one of the 15 cooling towers on the site. Legionella pneumophila indistinguishable by serological and genetic typing methods was isolated from this cooling tower and from sputum samples from two patients. In the town centre, no one tower was linked with infection and L. pneumophila was not cultured from any of the nine towers identified. Control measures were implemented and to date there have been no further cases of legionellosis associated with Bolton Health District.

Abstract:
As the general population ages, so too does the nursing workforce. A recent Queen's Nursing Institute (QNI) report (2019a) stated that 1 in 10 qualified district nurse positions have been lost in the past 10 years—without a trained workforce to replace those who retire. Workforce planning is central to ensure safe and effective care in the community setting. This article aims to provide a narrative account of one student who started her career at the University of Bolton as a trainee assistant practitioner and over time achieved her present position as a district nurse team manager. The account will include her journey from support worker to assistant practitioner, studying at the University of Bolton as a trainee assistant practitioner on a ‘work, earn and learn’ programme. It details the support she received from mentors in the trust, alongside her onward journey to registered nurse and specialist nurse practitioner qualifications.

Abstract:
The article describes the results of studying mycobiota in Olkhovsky district located in the Central part of Volgograd region. The standard protocol included methods of collecting, photographing in natural and laboratory conditions. To identify the material Mikmed-5 binocular microscope, Levenhuk C510 NG camera and a standard set of reagents (alkali solution, Melzer reagent) were used in the laboratory of Experimental biology of Volgograd State University. The works of Russian and foreign mycologists such as M.A. Bondartsev, V.M. Kotkov, L.G. Perevedentseva, T. Niemelä, L. Ryvarden became a base for determining fungi. The specimens were cut in half before drying and dried at a temperature of 38° C. Dried samples placed in paper envelopes are stored in the Mycological herbarium of Volgograd State University. Freezing of aphyllophoroid fungi for a month was a necessary procedure before placing it in a mycological herbarium. As a result of preliminary studies from September to November 2019, we identify nine species of basidiomycetes such as Bovistella utriformis (Bull.) Demoulin et Rebriev, Fomes fomentarius (L.) Fr., Lepista personata (Fr.) Cooke, Marasmius oreades (Bolton) Fr., Phellinus igniarius (L.) Quél., Pholiota populnea (Pers.) Kuyper et Tjall.- Beuk., Pleurotus ostreatus (Jacq.) P. Kumm., Saproamanita vittadinii (Moretti) Redhead, Vizzini, Drehmel et Contu, and Schizophyllum commune Fr. The article presents an annotated list of fungi with the indication of data on the location, substrate, frequency of occurrence and the name of the collector, if the collection was not carried out by the authors, for each type. Species names were accepted according to CABI “Index Fungorum” international electronic database (www.indexfungorum.org) as of December 2019. The trophic analysis of the composition of macromycetes shows that the largest number of species was found on dead branches and trunks, as well as live weakened trees of Fraxinus lanceolata Borkh., Populus nigra L., Salix alba L. This study of mycobiota in Olkhovsky district will be continued.

Abstract:
Raw meats are sometimes contaminated with Campylobacter species from animal faeces, and meats have repeatedly been implicated in foodborne infections. This study evaluated the prevalence, virulence genes, antimicrobial susceptibility patterns, and resistance gene determinants in Campylobacter species isolated from retailed meat carcasses. A total of 248 raw meat samples were collected from butcheries, supermarkets, and open markets; processed for enrichment in Bolton broth; and incubated at 42 °C for 48 h in 10% CO2. Thereafter, the broths were streaked on modified charcoal cefoperazone deoxycholate agar (mCCDA) plates and incubated at the same conditions and for the same amount of time. After incubation, colonies were isolated and confirmed by Polymerase chain reaction using specific oligonucleotide sequences used for the identification of the genus Campylobacter, species, and their virulence markers. The patterns of antimicrobial resistance profiles of the identified isolates were studied by disk diffusion method against 12 antibiotics, and relevant resistance genes were assessed by PCR. From culture, 845 presumptive Campylobacter isolates were obtained, of which 240 (28.4%) were identified as genus Campylobacter. These were then characterised into four species, of which C. coli had the highest prevalence rate (22.08%), followed by C. jejuni (16.66%) and C. fetus (3.73%). The virulence genes detected included iam (43.14%), cadF (37.25%), cdtB (23.53%), flgR (18.63%), and flaA (1.96%), and some of the isolates co-harboured two to four virulence genes. Of the 12 antibiotics tested, the highest phenotypic resistance displayed by Campylobacter isolates was against clindamycin (100%), and the lowest level of resistance was observed against imipenem (23.33%). The frequency of resistance genes detected included catll (91.78%), tetA (68.82%), gyra (61.76%), ampC (55%), aac(3)-IIa (aacC2)a (40.98%), tetM (38.71%), ermB (18.29%), tetB (12.90%), and tetK (2.15%). There is a high incidence of Campylobacter species in meat carcasses, suggesting these to be a reservoir of campylobacteriosis agents in this community, and as such, consumption of undercooked meats in this community is a potential health risk to consumers.

Abstract:
The following is an account of glottalization and gemination in the traditional vernacular of the Greater Bolton area. Bolton is a town situated 12 miles north-west of Manchester in northwestern England. The Greater Bolton area consists of the county borough of Bolton, Farnworth municipal borough, and the urban districts of Horwich, Turton, Little Lever and Kearsley. Its population is approximately 230,000.From a consideration of various historical, industrial, economic, social and administrative factors, the Greater Bolton area can be shown to be a relatively homogeneous cultural unit within the urban field of influence of Bolton. This cultural homogeneity is reflected in the speech of the area, which is also relatively homogeneous in character, and distinctive when compared to the speech of other parts of Greater Manchester County, South Lancashire and Merseyside (cf. Shorrocks 1980:1–27).

Abstract:
Campylobacter species are among the major bacteria implicated in human gastrointestinal infections and are majorly found in faeces of domestic animals, sewage discharges and agricultural runoff. These pathogens have been implicated in diseases outbreaks through consumption of contaminated milk and water in some parts of the globe and reports on this is very scanty in the Eastern Cape Province. Hence, this study evaluated the occurrence as well as virulence and antimicrobial-associated makers of Campylobacter species recovered from milk and water samples. A total of 56 water samples and 72 raw milk samples were collected and the samples were processed for enrichment in Bolton broth and incubated for 48 h in 10% CO2 at 42 °C under microaerobic condition. Thereafter, the enriched cultures were further processed and purified. After which, presumptive Campylobacter colonies were isolated and later confirmed by PCR using specific primers for the detection of the genus Campylobacter, target species and virulence associated genes. Antimicrobial resistance profiles of the isolates were determined by disk diffusion method against a panel of 12 antibiotics and relevant genotypic resistance genes were assessed by PCR assay. A total of 438 presumptive Campylobacter isolates were obtained; from which, 162 were identified as belonging to the genus Campylobacter of which 36.92% were obtained from water samples and 37.11% from milk samples. The 162 confirmed isolates were further delineated into four species, of which, 7.41%, 27.16% and 8.64% were identified as C. fetus, C. jejuni and C. coli respectively. Among the virulence genes screened for, the iam (32.88%) was most prevalent, followed by flgR (26.87%) gene and cdtB and cadF (5.71% each) genes. Of the 12 antibiotics tested, the highest phenotypic resistance displayed by Campylobacter isolates was against clindamycin (95.68%), while the lowest was observed against imipenem (21.47%). Other high phenotypic resistance displayed by the isolates were against erythromycin (95.06%), followed by ceftriaxone (93.21%), doxycycline (87.65%), azithromycin and ampicillin (87.04% each), tetracycline (83.33%), chloramphenicol (78.27%), ciprofloxacin (77.78%), levofloxacin (59.88%) and gentamicin (56.17%). Relevant resistance genes were assessed in the isolates that showed high phenotypic resistance, and the highest resistance gene harbored by the isolates was catII (95%) gene while VIM, KPC, Ges, bla-OXA-48-like, tetC, tetD, tetK, IMI and catI genes were not detected. The occurrence of this pathogen and the detection of virulence and antimicrobial resistance-associated genes in Campylobacter isolates recovered from milk/water samples position them a risk to human health.

Abstract:
Over the course of our careers we will see many thousands of patients, but there are one or two who we remember vividly; it is often these patients who shape and change our practice most. The problem-based review written by Kevin Jones’ team in Bolton in this edition reminded me of one of the first patients I clerked as a medical Senior House Off icer, while working in a District General Hospital back in the early 1990s. I was revising for the written part of the MRCP Part 2 examination at the time, so when the general practitioner described the young man’s fatigue, low grade fever and rash as ‘probable viraema’, my thoughts immediately turned to far more esoteric diagnoses. For once my suspicions were confirmed. The ‘fatigue’ was clearly proximal myopathy with marked tenderness suggesting a myositis; his rash had the typical ‘heliotrope’ distribution accompanied by nail-fold oedema; the diagnosis of dermatomyositis was sealed long before the consultant ward round when the laboratory phoned through his Creatinine Kinase result of over 15,000. I remember the feeling of elation at having made the diagnosis, the metaphorical pat on the back from my consultant and the look of relief in the eyes of the patient and his mother when I told them that we had the answer. I had no doubt in my mind that the hydrocortisone I had prescribed would rapidly improve his symptoms allowing him to resume his usual activity within a few weeks – I had read all about it in Kumar and Clark or some other erudite medical text book. For a few hours medicine was wonderful – until my cardiac arrest bleep went off. CPR was already in progress when I arrived on the ward; I had attended resuscitation calls before, but never for an 18 year old. A nurse reported that he had complained of dyspnoea and became drowsy shortly before he stopped breathing. There was a cardiac output, but no respiratory effort – he was intubated, ventilated, transferred to intensive care and then to a regional specialist unit, but sadly never regained consciousness before he died 2 weeks later. This was a tragic story which affected many people, but which taught me a really important lesson: making a diagnosis is where our job starts – not where it ends. We will never know whether we could have made any difference to this poor young man by anticipating a different outcome, closer monitoring of his respiratory function or earlier involvement of our critical care team. The speed of his deterioration was such that even today’s focus on early warning scoring and prompt recognition of changes in physiology, which has developed over the past 2 decades, would probably not have changed his outcome. However the key is to expect the unexpected; there are formes frustes of many conditions which behave differently to those which appear in the text books. Making a diagnosis should never be a cause for celebration.

Abstract:
Girsang Sipangan Bolon is one of the sub-districts under the administration of Simalungun Regency whose territory is partly directly adjacent to the shores of Lake Toba. The development concept of a waterfront culture for sustainable tourism is an importhing to do in this area. Tourism development that only relies on scenic beauty of the landscape will have difficult time to sustain for the future when there is degradation on environmental quality and boredom of visitors to the scenery presented. Identification of the spatial structure of Girsang Sipangan Bolon, morphology and circulation of the waterfront area and the typical type of development of the waterfront area are the objectives to be sought in this paper by using observation and geographic imaging of the area, whether are supports the concept of waterfront culture. Lack of open space to develop waterfront cultural activities, the morphology of the coastal area is not well planned where buildings with dense masses and heights block the view, and the area is more characterized by a recretional waterfront that relies more on the private sector. These characteristics are expected to provide more understanding and consideration for future development to emphasizing the concept of waterfront culture as an attraction and improving the morphology and circulation of the area that support sustainable waterfront tourism

Abstract:
Uganda, in common with many countries in sub-Saharan Africa, has many population risk factors predictive of high levels of mental disorder but poor coverage of mental healthcare (Kigozi, 2005). Recent population studies conducted in Uganda have shown rates of disorder in excess of 20% (Kasoro et al, 2002; Bolton et al, 2004; Ovuga et al, 2005) and the survey by Kasoro et al (2002) showed a high prevalence of patients with severe mental illness and poor access to services. There are 19 psychiatrists for 24.8 million people in Uganda, all but one of whom is based in the capital city, Kampala (Kigozi, 2005). The provision of mental health services relies on the use of psychiatric clinical officers (a cadre of trained mental health workers, similar to community psychiatric nurses, who currently cover 18 of the 56 districts in Uganda), primary care personnel, non-governmental organisations and members of the community. Liaison with traditional healers is encouraged (Ovuga et al, 1999).

The Community Service Program (PKM) entitled "CLEAN ENERGY EDUCATION AND SOLAR PANEL APPLICATION AT THE HKBP AEK BOLON CHURCH" in the Aek Bolon Julu Village, Balige District, Toba Regency, demonstrated the commitment of the PKM team to raise awareness and utilize solar energy. Cooperation with the Pomparan Opung Jaenna Siahaan indigenous community provided significant financial and logistical support, including funding for the construction of solar panel lamp foundation poles, location permits, as well as transportation and accommodation for the PKM implementation team. The impact was not only seen in cost savings for the Church and its congregation but also in optimizing the Church hall asset, which could then be used more effectively at night. Before the solar panel lighting, fear and limited access to the Church at night were the main obstacles. Through educational activities such as lectures during worship days, the PKM team succeeded in increasing the Church community's awareness of sustainable solutions. This program not only provided improvements in infrastructure but also created a safe and comfortable environment, reflecting the positive impact directly felt by the community around the HKBP AEK BOLON Church.

Abstract:
Currently, ecotourism has become an important industry because of its rapid development. Many tourism practices have adverse environmental impacts. Due to the increasingly destructive commercialization of the natural resources on which we depend, there are several negative impacts. Aek Nauli Research Forest (ANRF), with an area of 1,900 hectares, is one of the natural tourist destinations around the Lake Toba Tourism area managed by the Aek Nauli Research Institute for Environmental and Forestry Development (BP2LHK). The location of the study is in Girsang District, Sipangan Bolon, Simalungun Regency, North Sumatera Province. The tourist objects are natural panorama, elephant conservation education tour, and siamang animal ape park. On average, the number of visitors of ANRF on regular days is 100-300 visitors/day and on holidays reaches 300-1,700 visitors/day. The increase in the number of visitors is perceived to have an impact on environmental sustainability. This study aims to determine the capacity of the ANRF ecotourism area to accommodate the number of tourists per day simultaneously. The method used is the effective carrying capacity by Cifuentes method based on several stages of analysis, namely Physical Capacity (PCC), Real Capacity (RCC), Management Capacity (MC), and Effective Capacity (ECC). The environmental carrying capacity analysis results showed that the PCC, RCC, ECC were 26,106 visitors/day, 3,007 visitors/day, 2,505 visitors/day respectively while MC was 0.83. This value can be used to advise managers to limit visitors, particularly during peak season, in order to preserve objects and the quality of visits.

Abstract:
Cultural heritage as factors of cooperation and interaction is one of the challenges facing Southeast Europe, more specifically the Western Balkans. The historical past and cultural diversity of each country in this region is closely related to its past, with monuments of construction heritage, movable, immovable, sacred and secular monuments, of different faiths, with social, religious character etc. Cultural diversity in heritage is the most complex and challenging area in terms of our past as a region, so regional cooperation is more than necessary to channel the facts and equate historical evidence to mitigate the irritations and territorial claims of neighbors to the detriment. of others. Kosovo in particular has a number of problems related to the historical past closely linked to cultural heritage as a result of non-recognition by UNESCO leaving room for others to make claims. Special areas protected by the Ahtisaari Package were erected around some Orthodox cult monuments to usurp the surrounding properties under the pretext of security, while the properties were private (the case is with the Boletin church in Mitrovica district which usurped the lands of Isa's family Bulletin). All this situation would have ended if UNESCO had recognized the cultural heritage of Kosovo as the heritage of Kosovo regardless of which ethnicity or cult it belongs to, aiming at regional cooperation in the cultural diversity that surrounds us.

Abstract:
Parapat is a village in Girsang Sipangan Bolon District, Simalungun Regency. In the Master Plan of Priority Tourism Destinations (RIDPP) of Lake Toba 2020-2045, Parapat became one of the Key Tourism Area (KTA) in supporting Lake Toba Tourism Priority Destination (DPP). DPP Lake Toba is one of ten DPP that the government has set in achieving tourism development goals. So that the government is aggressively developing tourism in Indonesia to be increasingly known both nationally and internationally, tourism development is more directed towards sustainable tourism development. The indicators assessed in this study are on seasonal tourism issues, which refers to the Baseline Indicators of Sustainable Tourism Development for Tourism Destinations by WTO. This research uses qualitative research with a descriptive approach. Award chose qualitative descriptive research because this study explores the assessment of seasonal tourism indicators in Parapat that will be presented in descriptive form. The results of the study analysis found that there was an increase in the number of tourist visits to Parapat. This is due to the many meeting activities and MICE held at the hotel both by various government agencies and the local agency area. Regarding tourist attractions, the Lake Toba Festival cannot be held because of the enactment of PPKM rules in Parapat. In response to the COVID-19 pandemic situation in 2020, all lodging establishments, restaurants, and attractions implement strict health protocols. Some hotels will have to take a policy to close their businesses until June temporarily. However, in July, they reopened their business with a low room occupancy rate. In addition, there are several implementations of health protocols in the dining establishment that must be applied to every dining business in Parapat, such as maintaining distance and providing a sink for visitors.

Abstract:
Background: The freshwater ichthyofauna of Wallacea is diverse and understudied. A baseline survey of Bolano Sau Lake in Parigi Moutong District, Central Sulawesi Province, Indonesia in 2019 found an eleotrid goby (local name payangka) with characters conforming to the genus Giuris, long considered monophyletic as G. margaritacea/G. margaritaceus but recently found to comprise at least eight species. This study focused on the molecular (DNA barcoding) identification and phenotypic characters of the payangka. Methods: Payangka samples were collected from August to December 2019 in collaboration with local fishermen, weighed and measured, and preserved in 75% ethanol. Length, weight, sex (n=111) and seventeen morphometric characters/six meristic counts (n=42) were recorded. DNA barcoding was performed on a fin clipping preserved in 96% ethanol. Homologous nucleotide sequences were obtained from public (GenBank and BOLD) databases, analysis conducted in MEGA X, and phylogenetic trees edited in the Interactive Tree of Life (iToL). Results: Within the polyphyletic Giuris clade, the payangka sequence resolved into a sub-clade identified as Giuris laglaizei (Sauvage 1880), a recently resurrected taxon, based on a sequence provided by Philippe Keith. The length-weight relationship (L = 0.0087∙W3.162) indicated mildly allometric positive growth. Size distribution differed significantly between male and female fish with significantly larger mean size of males (13.56 cm) than females (11.62 cm). The meristic formula was: D VI-I,8 A I,8 P 13 V I,5 C15. Phylogenic analysis indicated four Giuris species in wetlands around Tomini Bay and five in Sulawesi. Conclusions: This first record of G. laglaizei in Indonesia advances knowledge of Wallacean and Indo-Pacific gobioid biogeography and highlights the need for a revision of the conservation status of the taxa currently grouped under Giuris margaritacea/G. margaritaceus in the IUCN Red List and FishBase databases. The data will inform biodiversity and fisheries management at local and regional levels.

Conclusions: This first record of G. laglaizei in Indonesia advances knowledge of Wallacean and Indo-Pacific Gobiiformes biogeography and highlights the need for a revision of the conservation status of the taxa currently grouped under Giuris margaritacea/G. margaritaceus in the IUCN Red List and FishBase databases. The data will inform biodiversity and fisheries management at local and regional levels.

Abstract:
Palabras clave: parroquia, advocacionesTopónimos: Lemos, Lugo, GaliciaPeríodo: siglo vi, siglo xv ABSTRACTThe county of Lemos is a district located in the south of the province of Lugo. Its origins go back to protohistory when it was occupied by the Lemavi tribe, and it subsequently became a territorium and a terra in the Middle Ages. The county’s history is marked by continuity, which allows us to study the spiritual evolution of its parishes. The saints chosen by the community as its protectors are historic testimonials of the time. In this paper, we study the list of church parish devotions in Lemos through medieval documentation. Our objective is to improve our knowledge about this medieval county by studying both the stability and changes in its parish saints. Abstract:
THE KITCHEN AS A PEDAGOGICAL STRATEGY. Addressing Celiac Disease from the encounter with “others” is to build a “we”, which leads to accessing a Good Living. María de los Ángeles Cignoli, Universidad Católica de Córdoba, Argentina. Artículo de investigación y reflexión generativa. This work arises within the framework of the Pehuajó Solidarity Network (RSP), where a voluntary group, independent since 1997, meets alternating kitchen/s to discuss some issues of interest related to signs and symptoms that a member of their family suffered, without know the condition of Celiac disease. Currently, the fact of being celiac in the social context where one lives and/or in the tourist places that they visit, do not always meet the nutritional needs of children, adolescents, young people, adults, people in general who they suffer from Celiac disease. That is to say, the achievement of Municipal Ordinance 94/2019 in district 079 of the province of Buenos Aires and the Laws: National and Provincial in Argentina are still known and are gradually being complied with, being the Gastronomic offer, sale of food, services of confectionery, among other insufficient. They do not have at least one menu for celiacs, with few exceptions. In other words, the group of aware independent volunteers not only incorporates knowledge to self-manage celiac disease, but as an informal entity it is managing to transpose in formal institutions, the community of the context-territory, non-profit activities with the objective of: disseminate, raise awareness and make celiac disease visible as a pedagogical strategy. That is, to make visible the advances developed in the 21st century, the very concept of this chronic disease, which went from being defined as a mere food intolerance, to being recognized as a systemic pathology with an autoimmune basis. O ENSINO MÉDIO BRASILEIRO NO CONTEXTO DE CRISES DO CAPITALISMO: ANÁLISE DA LEI Nº 13415/2017. João Paulo Pereira Coelho, Jani Alves da Silva Moreira, Universidade Estadual do Mato Grosso do Sul (UEMS), Cassilândia, Brasil. Artículo de investigación. Analisa a Lei nº 13415 a partir do processo histórico-político e econômico brasileiro. DIAGNOSIS ON SOCIAL IMPACT MEASUREMENT IN THE MATANZAS SCIENCE AND TECHNOLOGY PARK. Yoandy Sergio Villalonga Arencibia, Lourdes Tarifa Lozano, María de Lourdes Artola Pimentel, Edith Adolfina González Palmira, Yudith Acosta González, Leyda Finalé de la Cruz, Universidad de Matanzas, Cuba. The current Cuban context evidences a boom in research projects involving science, technology and innovation. Our National Innovation Management System is focused on the solution of the problems presented by our socialist social development economic model, with the use of knowledge and innovation. As part of the Informatization Policy of the country, the Scientific and Technological Parks (PCT) arise in Cuba, with which an accelerated development of R+D+i projects is propitiated in order to solve the fundamental problems of this model. At the express request of the country’s management, the projects generated by the PCTs in order to fulfill their mission and social transformation, it is essential to measure their social impact; however, the review of their design and a diagnosis (using methods such as surveys, interviews, documentary analysis) of the skills of the project managers working in the PCTCs, show marked weaknesses in measuring the social impact of their projects, which is the main objective of the present work: to show the results of the diagnosis carried out from the definition of indicators and measurement criteria for this purpose, as well as the authors’ analysis of the possible ways of solution.

Abstract:
Abstract Background/Aims Hydroxychloroquine (HCQ) is used widely in the management of rheumatic diseases and is considered to be generally well tolerated. HCQ ocular toxicity is a dose- and time-related reaction. The Royal College of Ophthalmologists (RCOphth) noted a low rate of drug discontinuation as a result of baseline testing (< 4%) and that prevalence data for HCQ ocular toxicity was based on treatment duration >5 years. The RCOphth updated their ocular toxicity screening guideline in 2020. A major change was removal of requirement for baseline screening, with screening commenced at 5 years of treatment unless risk factors are present. Though the number of patients starting HCQ treatment and the UK prescribing data could be estimated, the proportion of patients remaining on treatment for >5 years was stated to be unknown. We reviewed departmental HCQ prescribing, adverse event and attrition data over 5 years from Bolton NHS Foundation Trust, a district general hospital which serves a population of nearly 300,000. Methods Patients were identified as newly commencing HCQ if they had a shared care agreement in the calendar year 2016. Adverse events, attrition rates and reasons for cessation were obtained through review of hospital records over 5 years. Results In the 2016 calendar year, 100 patients commenced HCQ. The mean age was 58, and 70% patients were female. The most common indication was rheumatoid arthritis (75%). At 1 year, 30% of patients had stopped taking HCQ. At 5 years 57% of patients had stopped taking HCQ. The most common reason for cessation at 1 year was gastrointestinal symptoms (30%), and remained the most common reason at 5 years (19%). Eye symptoms were documented as reason for cessation in 7% of patients at 1 year, and 5% of patients at 5 years. On review of records, none of these were thought to be attributable to HCQ, though it was not recommenced. There were no patient deaths directly attributable to HCQ use. Conclusion A significant proportion of patients (57%) stopped taking HCQ after 5 years. The rate of attrition was highest within 12 months, which then almost doubled by 5 years. Centres wishing to estimate the workload related to HCQ retinopathy screening can capture their prescribing for patients newly starting HCQ and/or their catchment population numbers to estimate the Ophthalmology activity related to this 5 years' hence. This may help to fund and staff adequately to successfully carry out this drug safety activity. Disclosure D. Barahman: None. C. Saleh: None. A. Paul: None.

Abstract:
Abstract Background/Aims Who is responsible for the blood monitoring of disease modifying anti-rheumatic drugs (DMARDs)? It may be Primary Care, Specialist Rheumatology teams or a combination. In Greater Manchester, it was deemed good practice to initiate ‘amber’ conventional synthetic DMARDs (cs-DMARDs) by Rheumatology and then transfer to Primary Care under shared care after 3 months or when blood monitoring was stable. ‘Red’ drug prescribing and monitoring for targeted synthetic DMARDs (ts-DMARDs) and biologics (b-DMARDs) would remain under Specialist care long-term. In 2013, the process began for the Rheumatology team at Bolton to take on the ‘in house’ initiation of cs-DMARDs and the implementation of formal shared care. It would not be possible to manage this with paper slips of pathology results and shared care booklets, as the workload and safety implications would be un-manageable for our small team. The Rheumatology department purchased blood monitoring software, allowing only abnormal results or concerning trends to filter through. The implementation of this new way of working required collaborative working with the Informatics, Laboratory, Software and Rheumatology teams, as well as Primary care. At the time, the likely numbers of patients monitored and the workload of abnormal results/trends was unknown. Here we share our experience of software-based rheumatology DMARD monitoring in a district general hospital setting, serving a population of nearly 300 000. Methods Roles and responsibilities were allocated across the multidisciplinary, administration/clerical and informatics teams. A workflow and weekly timetable of activity and cross cover for leave was implemented. Two ‘super-users’ (AP/LB) undertook the initial testing of the software with informatics support (DE) and still work closely to address any data flow issues, software updates and testing related to emerging DMARDs. The patient cohort included those starting ‘amber’ DMARDs (to the point of shared care), stabilization following a dose increase and those remaining on specialist only ‘red’ DMARDs. Since 2019, a monthly monitoring report has allowed a more detailed documentation of workload. Results There were 518 patient licenses in use December 2019, 589 in 2020 and 635 in 2021. In the pre-pandemic year of 2019, 203 patients were newly registered or re-registered. Abnormal results and concerning trends were ‘flagged’ by the software for 719 patients. 263 shared care agreements were generated for monitored DMARDs. Conclusion This workload data relates to the use of software-based DMARD monitoring in a Rheumatology department, where patient flows relate to 1 whole time equivalent Rheumatology Consultant per 100 000 population. Centres considering taking on a similar form of DMARD monitoring can extrapolate this data for their population and patient flow to guide regarding staffing needs. Disclosure A. Paul: None. L. Abstract:
