Abstract:
An anomalous origin of the left coronary artery from the pulmonary artery, or Blund – White – Garland syndrome, is a rare congenital heart disease in which an anatomically correctly formed left coronary artery extends away from the pulmonary artery. Most patients with this defect die from progressive left ventricular failure during the first year of life, and the rest may suddenly die in adolescence or adulthood from acute coronary insufficiency. Several surgical methods of correction of the defect have been proposed, but a small number of observations do not allow one to express unequivocally in favor of one of the methods of performing the operation. We present our own observation of a surgical correction of a defect in a teenager. We performed off-pump aortocoronary bypass surgery of the anterior interventricular artery, and separation of the left coronary artery from the pulmonary artery by ligation, with the stitching of the left coronary artery. The issue of the expediency of tying hypertrophied collaterals between right coronary artery and left coronary artery systems was discussed with the purpose of reducing competitive blood flow after performing left coronary artery shunting. Experience in performing coronary artery bypass surgery without artificial circulation made it possible to perform an off-pump operation.

Abstract:
Coffee is consumed worldwide, but there are different types of espresso blends, each with its unique concentration of caffeine, which can have different effects on the human being. The aim of this study was to understand the effect of the impact of caffeine on the autonomic nervous system, evaluating the physiological changes and subjective responses due to different levels of caffeine intake. A double-blind tasting task consisting of one within-subject factor design (caffeine level: high / double caffeine mixture (blend A) vs single-charge caffeine mixture (blend B) vs low-caffeine mixture (blend c) allowed us to assess participants’ autonomic responses using Heart Rate Variability (HRV) and Pupillary Reactivity (PR). Arousal was also assessed through the Self-Assessment Manikin (SAM). Results revealed statistically significant differences in HRV and PR between coffee blends, showing the blend A,a more pronounced autonomic response that blend C. However, no significant differences were found in arousal level among coffee blends. These results are similar to previous research that pointed out to a discordance between subjective and objective measures when caffeine is consumed.

Abstract:
Transoral thyroidectomy using the vestibular approach has achieved excellent cosmetic outcomes without leaving any visible scars. However, concerns have been raised on how to prevent complications, such as skin perforation and anterior jugular vein injury, especially in the learning period. This is because working space is generally created via blind and blunt dissection using a blunt dilator through small vestibular incisions unfamiliar to beginners. Herein, we explain an intraoperative real-time ultrasound technique employed in transoral thyroidectomy to safely elevate a skin flap while avoiding any damage to the flap and anterior jugular vein in the learning period.

La ricerca è circoscritta all’opera di un maestro delle arti, Giovanni Blund, vissuto dal 1175 al 1240. L’opera su cui converge l’attenzione dell’intera ricerca è il Tractatus de anima, scoperto da Padre Callus che ne ha dato una completa edizione critica nel 1970. Nel tentativo di definizione di una nozione ambigua com’è quella di ‘anima’, si è rivolta l’attenzione anche ad Alfredo di Sareshel, a Domenico Gundisalvi, ad Avicenna e alle cognizioni mediche arabe e greche, indiscussa fonte per i maestri attivi tra i secoli XII e XIII. Blund era stato uno dei primi a tenere lezioni sul nuovo Aristotele, sia ad Oxford che a Parigi, e in breve tempo aveva acquisito una celebrità notevole. Tuttavia nel suo Tractatus non troviamo tanto Aristotele quanto i suoi interpreti arabi: al-Kindi, Avicenna, al-Farabi e Costa ben luca. Il suo trattato sull’anima è dunque testimonianza della penetrazione nelle scuole, all’inizio del secolo XIII, delle teorie del filosofo arabo Avicenna, guida preziosa nell’esegesi di un testo difficile e intricato qual era l’opera aristotelica. L’opera si articola, in base a quanto indicato dall’indice dei contenuti, in 27 capitoli ma l’ultimo, il De divina providentia, manca e non ci è possibile stabilire se l’autore abbia lasciato l’opera incompiuta o se l’ultimo capitolo sia stato deliberatamente omesso. Il piano del Tractatus è approssimativamente quello del testo avicenniano. Fatta eccezione per il capitolo xxvi sul libero arbitrio, significativamente ispirato al De libero arbitrio di Anselmo d’Aosta, Blund segue da vicino il Liber Sextus Naturalium, organizzando però la materia liberamente. Il Tractatus non è l’opera di un teologo ma di un maestro delle arti; è quanto indubbiamente suggerisce il metodo della trattazione. Persino argomentando questioni come l’immortalità dell’anima o il libero arbitrio, Blund non spodesta il teologo dal suo compito, non ne invade il territorio ma tratta le varie questioni in modo puramente filosofico. Il Tractatus de anima segna l’ingresso, nell’Occidente latino, di un nuovo approccio alla trattazione dell’anima perché il piano metafisico e teologico incrocia abilmente considerazioni fisiche e psicologiche. Sömnstörningar hos ungdomar anses vara ett växande problem. Det finns misstankar om att ungdomarnas ökade användning av Informations- och Kommunikationsteknik (IKT) är boven i dramat. Ungdomar upplevs spendera en allt större del av sin tid framför datorn, samtidigt som mobilen går varm av samtal och SMS. Det saknas dock utförlig forskning kring sambanden mellan IKT-användande och sömn. Syfte: Att undersöka huruvida ungdomar lider av sömnstörningar och om det i så fall kan kopplas till deras IKT-användande. De frågeställningar som behandlas är hur ungdomars sömnmönster ser ut och om det finns några kopplingar mellan eventuella negativa sömnmönster och deras IKT-användande. Dessutom undersöker vi om det finns några könsmässiga skillnader i sömn och IKT-användandet. Metod: Den metod som används är kvantitativ och datainsamlingen har skett genom en enkät. Urvalet består av 392 elever i årskurs ett från en gymnasieskola i västra Sverige. Enkäten besvarades och samlades in vid ett och samma tillfälle. Datan har bearbetats och analyserats i statistikprogrammet SPSS. Resultat: Analysen av resultaten visar att deltagarna har sömnproblem och att de upplever negativa effekter av dessa sömnproblem. Resultatet visar även att det finns ett samband mellan deltagarnas IKT-användande och dålig sömnkvalité, ju högre användande desto sämre sömn. Det finns inga direkta könsmässiga skillnader gällande sömnmönstret, däremot finns det skillnader gällande IKT-användandet. Slutsats: Undersökningen visar att deltagarna faktiskt lider av sömnproblem, och att användandet av IKT påverkar sömnen. Samtidigt indikerar resultatet att IKT-användning endast är en av flera påverkansfaktorer. Ytterligare forskning behövs för att till fullo förstå de bakomliggande orsakerna till ungdomarnas sömnproblem.
