Abstract:
Bromodomain testis (BRDT), a member of the bromodomain and extraterminal (BET) subfamily that includes the cancer targets BRD2, BRD3, and BRD4, is a validated contraceptive target. All BET subfamily members have two tandem bromodomains (BD1 and BD2). Knockout mice lacking BRDT-BD1 or both bromodomains are infertile. Treatment of mice with JQ1, a BET BD1/BD2 nonselective inhibitor with the highest affinity for BRD4, disrupts spermatogenesis and reduces sperm number and motility. To assess the contribution of each BRDT bromodomain, we screened our collection of DNA-encoded chemical libraries for BRDT-BD1 and BRDT-BD2 binders. High-enrichment hits were identified and resynthesized off-DNA and examined for their ability to compete with JQ1 in BRDT and BRD4 bromodomain AlphaScreen assays. These studies identified CDD-1102 as a selective BRDT-BD2 inhibitor with low nanomolar potency and >1,000-fold selectivity over BRDT-BD1. Structure–activity relationship studies of CDD-1102 produced a series of additional BRDT-BD2/BRD4-BD2 selective inhibitors, including CDD-1302, a truncated analog of CDD-1102 with similar activity, and CDD-1349, an analog with sixfold selectivity for BRDT-BD2 versus BRD4-BD2. BROMOscan bromodomain profiling confirmed the great affinity and selectivity of CDD-1102 and CDD-1302 on all BET BD2 versus BD1 with the highest affinity for BRDT-BD2. Cocrystals of BRDT-BD2 with CDD-1102 and CDD-1302 were determined at 2.27 and 1.90 Å resolution, respectively, and revealed BRDT-BD2 specific contacts that explain the high affinity and selectivity of these compounds. These BD2-specific compounds and their binding to BRDT-BD2 are unique compared with recent reports and enable further evaluation of their nonhormonal contraceptive potential in vitro and in vivo.

Sudden sensorineural hearing loss is a hearing loss of >30 dB in at least three consecutive frequencies that occurs in 3 days. The aim of this study was to investigate anxiety and depression caused by sudden, idiopathic, one-sided hearing loss. The levels of anxiety and depression in patients with this type of hearing loss were determined using the Beck Anxiety Scale (BAS) and the Beck Depression Inventory (BDI) at the time of the patient's first visit. In total, 56 patients (32 men and 24 women) with a mean age of 32.8 ± 9.9 years (range: 20 to 58 years) were selected as the patient group and 45 individuals without symptoms of anxiety and depression were selected as the control group. The mean pretreatment air-conduction threshold and bone-conduction threshold were 61.1 ± 26.1 and 49.4 ± 13.8, respectively. In the patient group, the pretreatment mean anxiety, depression, and hopelessness scores were 19.5 ± 10.7, 11.6 ± 8.4, and 6.2 ± 4.7, respectively. The control group's mean anxiety, depression, and hopelessness scores were 4.1 ± 3.0, 3.8 ± 2.1, and 1.8 ± 1.0, respectively. For all the tests, the difference between the patient group and the control group was statistically significant (p < 0.001 for all). Hearing levels were not correlated with scores on the BAS, BDI, and Beck Hopelessness Scale (p = 0.1, p = 0.6, and p = 0.4, respectively). In conclusion, the results of this study show that sudden hearing loss can cause anxiety and depression. Questioning patients with sudden hearing loss about symptoms associated with anxiety and depression might be useful, and a psychiatric consultation should be requested if necessary.

ABSTRACT Objective: Aim of the present study was to evaluate the retreatability and reestablishment of apical patency of two calcium silicate-based sealers, TotalFill BC Sealer (BCS) and mineral trioxide aggregate Fillapex (MTA F), versus AH Plus, when used in combination with Gutta-percha (GP). Materials and Methods: The canals of 54 single-rooted anterior teeth were instrumented and filled with GP/AH Plus (Group A), GP/MTA F (Group B), or GP/BCS (Group C) using continuous wave obturation technique. The groups were subdivided into subgroups with the master-GP cone placed to the working length (WL) or intentionally 2 mm short. The retreatment procedures were performed using ultrasonics, chloroform, rotary, and hand files. The ability to establish the patency and reach WL was determined as well as the time taken to reach WL was calculated in minutes. Furthermore, the samples were observed under a dental, optical microscope, after vertically splitting them. Results: The WL and patency were reestablished in 100% of specimens in all groups. The Mann–Whitney U-test indicated that there was a significant difference in the amount of time required to reach WL between the groups (P < 0.05) with group GP/BCS short of the WL showing the most amount of time to be retreated. Conclusion: The novel calcium silicate-based sealers are negotiable under simple root canal anatomy. However, the conventional retreatment techniques are not able to fully remove them.

AbstractA sensitive, specific and cost-effective HPLC method was developed for quantitative determination of carbinoxamine in human plasma using liquid chromatography with ultraviolet detection. A simple liquid–liquid extraction by ethyl acetate was used to extract carbinoxamine from human plasma. Satisfactory separation was achieved by a mobile phase consisting of 20 mM ammonium dihydrogen orthophosphate containing 0.01% triethyl amine (pH adjusted to 3 by using phosphoric acid) and acetonitrile in ratio of (75:25, v/v) at a flow rate of 0.9 mL/min on Hypersil® BDS C18 column (250×4.6 mm, 5 μm) column. The UV detector was set at 260 nm. The developed method was validated in human plasma with a lower limit of quantitation of 5 ng/mL for carbinoxamine. Linearity was demonstrated over the concentration range 5–200 ng/mL. The observed within- and between-day assay precision ranged from 1.902 to 14.90%; accuracy varied between 98.87 and 108.0%. This method can be used suitably for pharmacokinetic studies and in therapeutic drug monitoring in patients treated with carbinoxamine.

Pengembangan perangkat pembelajaran menggunakan model 4-D yang direduksi menjadi 3-D dan diujicobakan di tiga kelas XI SMAN 4 Surabaya tahun pelajaran 2015/2016 dengan one group pretest posttest design. Pengumpulan data menggunakan observasi, tes, dan angket.Data dianalisis menggunakan deskriptif kualitatif dan kuantitatif. Temuan penelitian yaitu: (1) Validitas: (a) Validasi RPP kategori baik, BAS kategori cukup baik, LKS kategori baik, Tes Uji Miskonsepsi umumnya valid; (b) Rata-rata keterbacaan BAS sebesar 59,8% dan LKS sebesar 71,3%; (c) Rata-rata tingkat kesulitan BAS sebesar 32,5% dan LKS sebesar 34,3%; (2) Kepraktisan meliputi: (a) Perangkat pembelajaran ditinjau dari keterlaksanaan RPP berkategori baik, dan (b) Tingkat aktivitas siswa dikategorikan baik (3) Keefektifan yang meliputi: (a) hasil tes uji miskonsepsi dengan N(g) 0,73; dan hasil analisis uji statistik one-way ANOVA, yaitu : 1) tidak ada perbedaan yang signifikan penurunan miskonsepsi dari ketiga kelas, 2) N-Gain penurunan miskonsesi siswa dialami pada masing-masing kelas (b) Respon siswa terhadap KBM adalah positif. Berdasarkan hasil temuan dalam penelitian ini, disimulkan bahwa perangkat pembelajaran listrik dinamis dengan model inkuiri terbimbing layak digunakan oleh siswa SMA untuk menurunkan miskonsepsi siswa.

The sintering temperature of 8YSZ (8mol% yttrium stabilized zirconia) is very high, usually above 1500°C. BAS (BaO-Al2O3-SiO2) microcrystalline glass can be used as sintering aids to reduce the sintering temperature of 8YSZ. In this research, large proportion (30-50wt%) BAS was added in 8YSZ to observe the influence of the sintering aids. The change rules of mechanical properties such as density test, bending strength and toughness in different material component and sintering process were researched. The results show that physical and mechanical performance improvement with the increase of sintering temperature and sintering time, density and bending strength decreases with the increase of BAS mass fraction. The optimal mechanical properties are obtained by sintering temperature 1300°C, sintering time 0.5h and 30wt% BAS.

Thrombophilic genetic factors PAI-1, MTHFRC677T, V Leiden 506Q, and Prothrombin 20210A were studied as risk factors in 235 Caucasian subjects: 85 patients with abdominal thrombosis (54 with portal vein thrombosis (PVT) and 31 with Budd-Chiari syndrome (BCS) without liver cirrhosis or hepatocellular carcinoma) and 150 blood bank donors. Seventy-five patients with PVT/BCS showed associated disease or particular clinical status (46 PVT/29 BCS): 37 myeloproliferative neoplasm (20 PVT/17 BCS), 12 abdominal surgery (10 PVT/2 BCS), 10 contraception or pregnancy (6 PVT/4 BCS), 7 abdominal acute disease (6 PVT/1 BCS), and 9 chronic disease (4 PVT/5 BCS); ten patients did not present any association (8 PVT/2 BCS). PAI-14G-4G, MTHFR677TT, and V Leiden 506Q were significantly frequent (OR 95% CI andχ2test withPvalue) in abdominal thrombosis; in these patients PAI-14G-4G and MTHFR677TT distributions deviated from that expected from a population in the Hardy-Weinberg equilibrium (PAI-1:χ2=13.8,P<0.001; MTHFR677:χ2=7.1,P<0.01), whereas the equilibrium was respected in healthy controls. V Leiden Q506 and Prothrombin 20210A were in the Hardy-Weinberg equilibrium both in patients with abdominal thrombosis and healthy controls. Our study shows an important role of PAI-14G-4G and MTHFR677TT in abdominal thrombosis without liver cirrhosis or hepatocellular carcinoma.

Abstract In Budd-Chiari syndrome (BCS), thrombosis develops in the hepatic veins or inferior vena cava. To study the relationship between hypofibrinolysis and BCS, we measured plasma levels of fibrinolysis proteins in 101 BCS patients and 101 healthy controls and performed a plasma-based clot lysis assay. In BCS patients, plasminogen activator inhibitor 1 (PAI-1) levels were significantly higher than in controls (median, 6.3 vs 1.4 IU/mL, P < .001). Thrombin-activatable fibrinolysis inhibitor and plasmin inhibitor levels were lower than in controls (13.8 vs 16.9 μg/mL and 0.91 vs 1.02 U/L, both P < .001). Median plasma clot lysis time (CLT) was 73.9 minutes in cases and 73.0 minutes in controls (P = .329). A subgroup of cases displayed clearly elevated CLTs. A CLT above the 90th or 95th percentile of controls was associated with an increased risk of BCS, with odds ratios of 2.4 (95% confidence interval, 1.1-5.5) and 3.4 (95% confidence interval, 1.2-9.7), respectively. In controls, only PAI-1 activity was significantly associated with CLT. Analysis of single nucleotide polymorphisms of fibrinolysis proteins revealed no significant differences between cases and controls. This case-control study provides the first evidence that an impaired fibrinolytic potential, at least partially caused by elevated PAI-1 levels, is related to the presence of BCS.

O azevém é uma gramínea com elevado potencial de infestação em lavouras e pomares, e seu controle com glyphosate tem sido limitado devido à existência de biótipos resistentes a esse herbicida. Objetivou-se comparar quatro biótipos de azevém coletados em diferentes regiões do Rio Grande do Sul, com o intuito de testar a hipótese de que aspectos morfofisiológicos e fenológicos estejam vinculados à resistência. O experimento foi conduzido em casa de vegetação, onde um biótipo suscetível (B1S, Passo Fundo) e três resistentes ao glyphosate (B2R, Santa Maria; B3R, Júlio de Castilhos; e B4R, Vacaria) foram colhidos aos 126, 147, 168 e 189 dias de idade. O biótipo B1S mostrou 21 dias de antecipação no florescimento em relação aos demais biótipos. O biótipo B4R produziu significativamente mais folhas (43 por planta), afilhos (14 por planta), espigas (14 por planta), sementes (3.484 por planta) e matéria seca total (raízes + parte aérea = 13,8 g por planta). Conclui-se que biótipos sensíveis não podem ser diferidos de biótipos resistentes apenas por aspectos morfológicos relacionados ao vigor.

Methodik: Die CDRS-R wurde aus dem Amerikanischen übersetzt. 60 Kinder und Jugendliche (26 Mädchen, 34 Jungen) im Alter von 7.5 bis 17.9 Jahren (Mittelwert = 13.8, Standardabweichung = 2.9), die sich in kinder- und jugendpsychiatrischer Behandlung befanden, wurden mit der CDRS-R innerhalb der ersten Woche der Behandlung interviewt. Zur Validierung wurde das K-SADS-PL d auf der Selbstbeurteilungsebene das BDI–II bzw. die DIKJ eingesetzt. Ergebnisse: Psychometrische Analysen zeigen eine hohe interne Konsistenz der Skala (α = 0.90) und gute Trennschärfekoeffizienten für die einzelnen Items. Die Korrelationen zu BDI–II und DIKJ sind hoch. Der CDRS-R-Mittelwert für die acht Jugendlichen mit einer majoren depressiven Störung entspricht den international gefundenen. Schlussfolgerungen: Die deutsche Übersetzung der CDRS-R weist insgesamt gute psychometrische Eigenschaften auf und kann zur Diagnostik depressiver Störungen bei Kindern und Jugendlichen verwendet werden.

Hasil penelitian menunjukkan: (1) Validitas perangkat pembelajaran yang meliputi: (a) Validitas komponen RPP dengan skor rata-rata 4,09 (sangat baik), (b) Rerata skor BAS yang meliputi sub komponen kelayakan isi 3,53 (sangat baik), kebahasaan 3,43 (sangat baik), penyajian 3,42 (sangat baik), (c) Rata-rata skor validasi LKS 4,32 (sangat baik), (d) Soal pretest-posttest yang valid. (2) Keterlaksanaan KBM rata-rata 3,86 (sangat baik), aktivitas siswa mempunyai indeks reliabilitas rata-rata 90% dengan kegiatan paling dominan berdiskusi dalam merumuskan kesimpulan. Analisis N-Gain hasil belajar siswa menunjukkan: 42% siswa mengalami peningkatan sedang dan 58% siswa mengalami peningkatan yang tinggi. Respon siswa menunjukkan tertarik, mudah dan berminat dalam mengikuti KBM. Berdasarkan hasil-hasil analisis data dalam penelitian ini dapat disimpulkan bahwa perangkat pembelajaran yang dikembangkan layak digunakan dalam kegiatan pembelajaran untuk meningkatkan hasil belajar Fisika siswa SMA.

Le poids et la taille furent mesurés chez 1 464 personnes vivant dans la communauté et 963 individus en institution. Les sujets institutionnalisés avaient un poids corporel inférieur à ceux vivant en communauté (59,1 ± 13,8 kg. vs. 65,1 ± 14,1 kg.); ils étaient plus petits (160,4 ± 10.1 cm vs. 162,6 ± 10 cm), et leur IMC étaient plus bas (23,3 ± 5,0 vs. 24,6 ± 4,5) (p< .001). Les valeurs anthropométriques furent plus basses chez les femmes, et diminuaient progressivement avec l'âge. Les Québécois avaient une taille plus petite et un poids corporel inférieur à ceux des Canadiens vivant ailleurs. Les mesures anthropométriques ont diminué suivant le diagnostic cognitif, en allant de normal, à «deficit cognitif sans démence» (CIND), à dément; les participants atteints de la démence de type Alzheimer avaient les poids corporels les plus bas. Abstract:
Along a transect from 900 m to 3300m elevation, various ecological parameters concerning epiphytic bryophytes have been studiesd. The most characteristic life form between 900 and 2300 m elevation is the dendroid, which is interpreted as an adaptation to better gas exchange under the unfavourable conditions (low light, high air humidity) of the rain forest. Above 2500 m, cushions are the predominant lif form. The cushion are 2-5 cm high but can reach 50 cm at the forest line. In spectacular cases, moss balls are formed up to 1 m in diameter. PH measurements of the bark of host trees revealed values between 4.1 and 6.2, which is in accordance with result from South America and SE-Asia. The bryophyte cover on soil is very low (<5%) below 2700 m but raises to 90% in the subalpine ericaceous belt, but not in the subalpine Senecio-belt. The percentage cover of bryophytes on bark increases from 5 to 80% in the transect with increasing elevation, and the light intensity increases from <1% to 50%. The mean annual temerature decreases continously from 20.6°C at 900 m elevation to 2.5° at 4500 m elevation. This allows a comparison of the growth conditions of bryohyte species occurring disjunct in the oceanic parts of the temperate regions and the tropical mountains. The phytomass of epiphytic bryophytes per m² and per ha increases synchronously in the transect. There is a sudden increase from 8-44 kg/ha in 900-1300 m to 100-600 kg/ha in elevationis between 1900 and 2900 m. Maximum values of 6 tonnes/ha are found at the forest line , being 750 time higher than the lowest value in the lower part of the transect. The water storing capacity of epiphytic bryophytes is 20-60 1/h at elevations between 900 and 1300 m, 130-2000 1/ha between 1900 and 2900 m and 18000 1/ha at the forest line. The latter is 18% of the estimated rainfall.

Marital adjustment plays a key role in the physical and psychosocial wellbeing. We conducted a cross-sectional study to evaluate marital adjustment and its association with psychological distress, suicidal ideation, sleep problems, and quality of life in patients with cancer. We collected demographic and clinical information using a structured survey. We assessed marital adjustment, quality of life, psychological distress profile, and sleep problems of participants using validated instruments: the Locke and Wallace Marital Adjustment Test (LWMAT), the Short-Form Health Survey-12, the Beck’s Depression Inventory (BDI), the Beck Anxiety Inventory (BAI), and the Insomnia Severity Index (ISI). Suicidal ideation was assessed with item nine of the BDI. Of the 130 patients (52.3% females, mean age 57.9 ± 12.4 years) enrolled, 20 (15%) were classified as experiencing poor marital adjustment. Moderate to severe depression, anxiety, and insomnia were found in 25.4%, 34.6%, and 24.7% of participants, respectively. Positive suicidal ideation was documented in 13.8% of participants. We found a significant association between poor marital adjustment and depression, anxiety, suicidal ideation, and poor sleep. Our study confirms the relevance of marital adjustment in relation to the psychological wellbeing of patients with cancer. Depression, anxiety, and poor sleep were found to be significantly associated with poor marital adjustment.

IntroductionDuring last 10 years, the number of non-suicidal self-injury patients (NSSI patients) in Department of Adolescent Psychiatry of Medical University in Łódź has doubled (from 20 to over 40%). According to DSM-5 criteria, NSSI are deliberate and superficial skin injuries. Such behaviours should be distinguished from suicidal behaviour disorder (SBD). However, the two display the high rate of co-occurrence.Material and methodThe study covered 1300 patients (12–19 years of age) hospitalized during last 6 years. The analysis included various variables, e.g. demographic, familial, problems related to school and to peer relations, and variables describing NSSI.Results and summaryThe analyzed group consisted of 60,4% girls and 39,6% boys. Out of 43% of patients who performed NSSIs, 45% also confirmed BDS. The patients with diagnosed mental retardation and with schizophrenic psychoses performed significantly fewer NSSIs. The instrumental motive was the most frequently declared reason of self-injury (76%). The reactive (65%), and the illness-related motives (only 15%) were less frequent. In comparison to patients without self-injuries, the NSSI patients significantly more frequently (P < 0.05) experienced physical violence, unreciprocated love, played truant and repeated grades. Conflicts, delinquency, divorce, and lack of a sense of support were significantly more frequent in the family systems of NSSI patients. They more often experienced loss of their parent before age 15 and had close relatives suffering from mental disorders.ConclusionsNSSI is a multi-dimensional issue that requires further research. e12625 Background: Pathologic complete response(pCR) after neoadjuvant chemotherapy(NAC) in patients with breast cancer(BC) is associated with decreased recurrence and improved survival. In NSABP B18 and B27, 10-yr locoregional recurrence(LRR) after pCR in patients with Stage I-III BC undergoing breast conserving surgery(BCS) and whole breast RT(WBRT) was 5.2-6.9%. However, LRR may be overestimated as Her2 therapy was not used and only some eligible patients received endocrine therapy. A retrospective study using modern protocols found a 5-yr LRR of up to 2.6%. We hypothesize that LRR in N0 patients is even lower, and de-escalation of therapy should be examined. The study objective is to assess if a prospective trial of omission of WBRT after BCS in patients with N0 BC and pCR after NAC is warranted and to assess feasibility. Methods: Patients with T1-T3 N0 invasive BC diagnosed between Dec 2011-2017 treated with NAC and BCS were identified from a hospital BC registry. Health records were retrospectively reviewed to identify patients with pCR, defined as absence of residual invasive or in-situ disease(ypT0N0). Incidents of locoregional and distant recurrence were recorded. Results: Of 89 patients with T1-3 N0 invasive BC treated with NAC and BCS, 29(32.6%) had pCR. Median follow-up was 61.1 months. Median age was 55 yrs and median tumour size was 2.4cm. Receptor status was 16(55.2%) HR-Her2-, 4(13.8%) HR-Her2+, 7(24.1%) HR+Her2+ and 2(6.9%) HR+Her2-. NAC protocols consisted of an anthracycline and/or a taxane in 27(90%) patients, and 6 patients were treated on NAC trials. All patients with Her2+ disease received Her2 targeted therapy. Adjuvant endocrine therapy was taken by 8 of 9 patients with HR+ disease. All patients received WBRT without nodal RT. RT plan was available for 26(86.7%) patients. RT dose ranged from 40-50Gy, and all but 4 received tumour bed boost. There were no local or regional recurrence events at last follow-up. One patient developed brain metastases at 15.7 months. Conclusions: Over 6 years, 29 patients were identified that would be eligible for a prospective trial evaluating omission of WBRT after pCR in N0 patients treated with NAC and BCS. At median 5-yr follow-up, there were no locoregional recurrences in our cohort, demonstrating that the absolute benefit provided by WBRT is likely small. Our results indicate a prospective trial is warranted and will require multi-institution participation to accrue.

Coking wastewater is characterized by poor biodegradability and high microorganism toxicity. Thus, it is difficult to meet Grade I of Integrated Wastewater Discharge Standard of China by biological treatment technology; specifically, COD cannot meet above standard due to containing refractory organics. A novel coupling reactor, electrochemical oxidation using BDD anodes and biological aerated filter (BAF), has been developed for carbon and nitrogen removal from biotreated coking wastewater, focusing on COD, BOD5,NH4+-N, andNO3--N removal on operation over 90 days with average effluent value of 91.3, 9.73, 0.62, and 13.34 mgL−1, respectively. Average value of BOD5/COD and BOD5/NO3--N was enhanced from 0.05 to 0.27 and from 0.45 to 1.21 by electrochemical oxidation, respectively, with average energy consumption of 67.9 kWh kg−1COD. In addition, the refractory organics also were evidently mineralized in the unit based on the data of the three-dimensional fluorescence spectra. Meanwhile, its effluent provided excellent substrate for biological denitrification in BAF. At hydraulic retention time (HRT) of 13.08 h, about 12 mgL−1 NO3--N was depleted through denitrification, and it mainly occurred at top of 0.25 m height of BAF. Therefore, it is feasible to apply the coupling reactor for biotreated coking wastewater treatment and achieve desirable effluent quality.

Hereditary breast and ovarian cancers are mainly linked to variants in BRCA1/2 genes. Recently, data has shown that identification of BRCA variants has an immediate impact not only in cancer prevention but also in targeted therapeutic approaches. This prospective observational study characterized the overall germline BRCA variant and variant of uncertain significance (VUS) frequency and spectrum in individuals affected by breast (BC) or ovarian cancer (OC) and in healthy individuals at risk by sequencing the entire BRCA genes. Of the 363 probands analyzed, 50 (13.8%) were BRCA1/2 mutated, 28 (7.7%) at BRCA1 and 23 (6.3%) at BRCA2 gene. The variant c.5266dupC p.(Gln1756Profs) was the most frequent alteration, representing 21.4% of the BRCA1 variants and 12.0% of all variants identified. The variant c.6313delA p.(Ile2105Tyrfs) of BRCA2 was the most frequent alteration observed in 6 patients. Interestingly, two new variants were identified in BRCA2. In addition, 25 different VUS were identified; two were reported for the first time in BRCA1 and two in BRCA2. The number of triple-negative BCs was significantly higher in patients with the pathogenic BRCA1/2-variant (36.4%) than in BRCA1/2 VUS (16.0%) and BRCA1/2 wild-type patients (10.7%) (p < 0.001). Our study reveals that the overall frequency of BRCA germline variants in the selected high-risk Italian population is about 13.8%. We believe that our results could have significant implications for preventive strategies for unaffected BRCA-carriers and effective targeted treatments such as PARP inhibitors for patients with BC or OC.

Background: The intensive training associated with health care education has been suggested to have unintended negative consequences on students' mental or emotional health that may interfere with the development of qualities deemed essential for proficient health care professionals. This longitudinal study examined the prevalence and severity of depressive symptoms among students at a chiropractic educational institution. Methods: Chiropractic students at all levels of training were surveyed at Canadian Memorial Chiropractic College during the academic years of 2000/2001, 2001/2002, and 2002/2003. The measurement tool employed was the Beck Depression Inventory, 2nd edition (BDI-II). Previously established BDI-II cutoff scores were used to assess the severity of reported depression symptoms, and these were compared by sex and year of training. Results: The survey was completed by 1303 students (70%) over the 3 years of the study. The prevalence of depressive symptoms was nearly 25%, with 13.7% of respondents indicating a rating of mild depression, 7.1% indicating moderate depressive symptoms, and 2.8% indicating severe symptoms. Significant differences were found between years of training, with 2nd-year students having the highest prevalence of depressive symptoms, and sex, with females having a higher rate of symptoms. Conclusions: Chiropractic students surveyed at Canadian Memorial Chiropractic College had high rates of depression similar to those measured in other health care profession students. Chiropractic educational institutions should be aware of this situation and are encouraged to emphasize students' awareness of their own personal health and well-being and their access to appropriate care, in addition to the same concerns for their future patients.

Background: The Educational Environment (EE) has an extremely important role in effective student learning. The Dundee Ready Education Environment Measure (DREEM) is a validated and widely tool to assess the EE in health and particularly in dental professional education programs. Objective: This study aimed to compare male and female dental students' perception regarding DREEM inventory of EE in Jazan University. A cross-sectional questionnaire included questions on demographic information and the DREEM of EEs. Methods: DREEM was used to gather information from our BDS students’ program about the environment in our institution. The data were selected from 3rd, 4th, 5th, 6th year students. Data from 330 participants were distributed manually. Other factors such as type of graduated school, level of education, student’s GPA, and monthly income of family were assessed. All the data were analyzed. Results: 286 (86%) of the students completed and returned the DREEM questionnaires. There was no statistically significant difference between genders with respect to mean scores of perceptions of learning. Also, there was a significant difference in terms of educational level, but no significant differences were observed in the DREEM subscales. The average overall DREEM score of the study subjects indicated a positive learning perception of 130.8 for males and 130.2 for females. The subscale for both genders involved students’ perception of learning as 32.1, the students’ perception of teaching as 29.1, while the students’ academic self-perception was 21.7. The students’ self-perception of atmosphere was 30.0 while students’ social self-perception accounted for 17.5. Conclusion: Both gender students recorded positive values of the learning environment. Further, a qualitative investigation is recommended to be done on every single course to evaluate the changes and make necessary alterations.

Abstract:
Clonazepam (CLO) plays a significant role in treating seizures, myoclonic seizures, and other clinical infections, while metronidazole (MTZ) is an antiprotozoal, antibacterial drug. The reversed-phase high performance liquid chromatographic (RP-HPLC) technique has provided greater precision and sensitivity over other methods, especially the colorimetric and spectrophotometric. This study aimed to implement a simple method in pure and pharmaceutical tablets for the simultaneous determination of CLO and MTZ. The RPHPLC method's development and optimization for validating a separation method for the simultaneous estimation of both drugs in pharmaceutical formulation involved studying the optimum mobile phase, buffer concentration, and the pH value. Under the chromatographic conditions, RP-HPLC system achieved excellent separation on a Phenomenex HyperClone BDS (250 x 4.60 mm, 130A, and 5μ) at a temperature of 45°C and the following conditions: 65:35% acetonitrile:acetic acid and sodium acetate buffer solution (NaOAc/HAc) as mobile phase (pH 3.5); 20 μL injection volume at a flow rate of 0.75 mL/min and detection wavelength of 310 nm. The proposed RP-HPLC procedure demonstrated high precision (RSD% 1%), as well as the good linear relationship of the calibration graph at concentration ranges of 50-160, 35-100 ppb with a coefficient of determination (r2) of the regression line of 0.9996 and 0.9997 for metronidazole and clonazepam respectively. The proposed method offered excellent validated values for both LOD (4.24 and 3.06 ppb) and LOQ (14.15– 10.21 ppb) for both metronidazole and clonazepam drugs, respectively. Háttér és célkitűzések: vizsgálatunk célja magyar mintán ellenőrizni a meddőség pszichés vonatkozásait feltáró eddigi, nemzetközi eredményeket. Módszer: 75 fős (63 nő, 12 férfi), magyar, kényelmi, keresztmetszeti minta felmérése történt önkitöltős, részben sztenderdizált kérdőívekkel. Eredmények: a vizsgált nők depresszióértékei szignifikánsan magasabbak a Hungarostudy Epidemiológiai Panelvizsgálat országos reprezentatív vizsgálatban talált, nőkre vonatkozó, 2006-os pontszámoknál (BDI-S; átlag (M)=13,9; szórás(SD)=13,4; vs. M=9,7; SD=11,9; p<0,05). Az élet értelmességének mértéke szignifikánsan alacsonyabb az átlagnépesség sztenderdjeinél (BSCI-LM-7, M=8,9; SD=3,2; vs. M=9,9; SD=2,8; p<0,05). Állapot- és vonásszorongásszintjük azonban nem különbözik ezektől (rendre p=0,205 és p=0,477). A férfiak semmilyen mutató mentén nem tértek el az országos átlagoktól. Mintánkon belül sem a nem, sem az életkor, egy gyermek léte vagy a meddőség hossza, diagnózisa, a kezelések mennyisége és minősége nem látszott befolyásolni a lelkiállapotot. Szignifikánsan magasabb volt mind az állapot-, mind a vonásszorongása azoknak a nőknek, akik spontán vetélést éltek át, szemben azokkal, akiknek nem volt vetélésük, vagy terhesség-megszakításuk (is) volt (STAI-S; M=49,5; SD=13,8; vs. M=41,4; SD=11,9; p<0,05; / STAI-T; M=47,4; SD=11,6; vs. M=41,5; SD=10,4; p<0,05). Azok a med-dők, akik nem hisznek egy transzcendens hatalom létében vagy bizonytalanok, szignifikánsan magasabb depresszió-pontszámot értek el azoknál, akik hisznek (BDI-S, M=17,1; SD=15,1; vs. M=10,0; SD=10,1; p<0,05). A válaszadók 44%-a hiányolja a meddőségi centrumokban a pszicho-lógiai segítségnyújtást. Következtetések: a pilot-vizsgálat alátámasztja a nemzetközi eredményeket, miszerint a termékenységre várók lényegesen lehangoltabbak és életüket kevésbé érzik értelmesnek a ter-mékenységi nehézségekkel nem küzdőknél. Ez megfontolandóvá teszi a meddőségi kezelések során nyújtott pszichés támogatás beépítését a szakmai protokollba.

TPS589 Background: This phase III post-NC trial evaluates if CWRNRT post-Mx or whole breast irradiation (WBI) with RNRT after BCS significantly reduces the IBCR-FI rate in pts with PAx nodes that are negative after NC. Secondary aims are OS, LRR-FI, DR-FI, DFS-DCIS, second primary cancer, and comparison of RT effect on cosmesis in reconstructed Mx pts. Correlative science examines RT effect by tumor subtype, molecular outcome predictors for residual disease pts, and predictors for the degree of reduction in locoregional recurrence. Methods: Clinical T1-3, N1 IBC PAx nodes (FNA or core needle biopsy) pts complete ≥12 weeks of NC (anthracycline and/or taxane). HER2+ pts receive anti-HER2 therapy. Following NC BCS or Mx, sentinel node biopsy (≥3 nodes) and/or Ax dissection with histologically negative nodes is performed. ER/PR and HER-2neu status before NC is required. Pts receive required systemic therapy. Radiation credentialing with a facility questionnaire/case benchmark is required. Random assignment for Mx pts is to no CWRNRT or CWRNRT and for BCS pts to WBI or WBI RNRT. Statistics: 1636 pts to be enrolled over 5 yrs (definitive analysis at 7.5 yrs). Study is powered at 80% to test that RT reduces the annual hazard rate of events for IBCR-FI by 35% for an absolute risk reduction of 4.6% (5-yr cumulative rate). Intent-to-treat analysis with 3 interim analyses (43, 86, and 129 events) and a 4th/final analysis at 172 events. Pt-reported outcomes focusing on RT effect will be provided by 736 pts before random assignment and at 3, 6, 12, and 24 mos. Accrual as of 2-2-17 is 534 (32.64%). Background: During chemotherapy for hematological malignancies, febrile neutropenia (FN) is a frequent and serious complication. The causative organisms of FN vary from era to era and depending on bacterial resistance in each country. Therefore, epidemiological data are required for each era and country from prospective large clinical trials. We previously reported a prospective clinical trial indicating the importance of the D-index, a measure of neutropenia severity (CEDMIC trial, J Clin Oncol. 2020 10; 38: 815). We hypothesized that analyzing these data would provide accurate contemporary epidemiological data for hematological malignancies. Furthermore, as encountered in practice, we suspected that gram-negative (GN) bloodstream infections (BSIs) are more common at high body temperatures (BTs) than at low BTs and examined the relationship between causative bacteria and BT. Patients and Methods: Between June 2013 and April 2017, we enrolled patients with hematological malignancies aged 16-79 years post-chemotherapy or -stem cell transplantation (SCT) with expected neutropenia durations &gt;7 days. Neutrophil counts &lt;500/mL reflected neutropenia, and all fevers had axillary temperatures ≥37.5°C. Prophylactic antibiotics (Abx) were drugs administered post-chemotherapy and pre-FN onset. The FN incidence and blood culture (BC) positivity rate were examined in 4 treatment groups: allogeneic SCT (Allo-SCT), autologous SCT (Auto-SCT), induction chemotherapy for acute leukemia, and other treatments. Results: In total, 423 patients were enrolled, and 413 (median age: 56 years) were included in the intention-to-treat analyses (Table 1). The treatments included induction chemotherapy (n=67), Allo-SCT (n=106), Auto-SCT (n=158), and others (n=82). Prophylactic Abx were administered in 281 (68.5%) patients (only quinolone, n=222; with β-lactam, n=59). Prophylaxis rates were significantly higher in the Allo-SCT (n=94, 88.7%, p&lt;0.001) and induction chemotherapy (n=49, 73.1%, p&lt;0.017) groups. FN occurred in 356/413 patients (86.4%, Table 1). The Allo-SCT group had a higher FN incidence than that in the other treatments group (93.4% vs. 72%, p&lt;0.001). D-indices ≥5500 and &lt;5500 were associated with FN incidences of 100% and 81.8%, respectively (p&lt;0.001). Prophylaxis did not reduce the FN incidence. In total, 1272 BC sets were collected from 356 patients with FN. At FN onset, BCs were collected from 346 patients; 72 (20.8%) were positive (Table 2). Among initial BCs, 46 (63.9%) harbored gram-positive (GP) and 28 (38.9%) harbored GN bacteria. Three patients had multiple bacterial infections. The 2 most frequent GP bacteria were coagulase-negative Staphylococcus (n=21, 44.7%) and viridans streptococci (n=14, 29.8%). The 3 most frequent GN bacteria were Escherichia coli (n=13, 44.9%), Klebsiella pneumoniae (n=4, 13.8%), and Pseudomonas aeruginosa (n=4, 13.8%). Among initial BCs, prophylactic Abx use did not reduce BC positivity. However, the GN BSI incidence was significantly lower in the prevention group vs. non-prevention group (23.8% vs. 56.7%, p=0.007). Among patients who received prophylactic β-lactam, GN BSIs did not occur. Maximum BTs (maxBTs) were significantly higher in patients with positive BCs than negative BCs (38.4°C vs. 38.0°C, p&lt;0.001). The BTs associated with GN BSIs were significantly higher than those associated with negative BCs and GP BSIs (38.7°C vs. 38.0°C vs. 38.2°C, p&lt;0.001, Figure 1). The maxBTs and BC positivity rates were strongly related (Table 3). The BC positivity rates were significantly higher in patients with BTs ≥38°C vs. &lt;38°C (26.5% vs. 14.5%, p=0.008), 38.5°C vs. &gt;38.5°C/&lt;38.5°C (39.7% vs. 15.3%, p&lt;0.001), and ≥39°C vs. &lt;39°C (52.6% vs. 16.9%, p&lt;0.001). Among patients who did not receive prophylactic Abx (n=106), the GN BSI positivity rate was significantly lower in patients with maxBTs &lt;38°C vs. ≥38°C (14.3% vs. 76.2%, p=0.007). Conclusion: In FN patients with high-risk hematological malignancies in Japan, the frequency of GP BSIs is high. Prophylactic Abx use reduces the GN BSI risk but not the FN or BSI risk. Fever severity and BC positivity at FN onset are strongly associated. The BTs of patients with GN BSIs are high. Abstract:
Abstract This study compared physiological and productive parameters in ¾ Holstein × ¼ Gir dairy cows diagnosed or not with subclinical hypocalcemia (SCH) during early lactation. Nonlactating, multiparous cows (n = 32) were enrolled in this experiment 21 d prior to expected date of calving. Cows were maintained in a single pen with ad libitum access to corn silage before calving and received a limit-fed prepartum concentrate. Cow body weight (BW) and body condition score (BCS) were recorded weekly, and blood samples were collected on days −21, −14, −9, −6, and −3 relative to expected calving. After calving (day 0), cows were managed in a single pen with ad libitum access to a total mixed ration, and were milked twice daily. Cow BW and BCS were recorded upon calving and then weekly. Milk production was recorded daily and milk samples collected weekly until 30 d in milk (DIM). Blood was collected during the first 5 DIM, and at 6, 9, 16, 23, and 30 DIM. Cows were classified with SCH when mean total serum Ca during the first 5 DIM was ≤2.125 mmol/L. Cows diagnosed with SCH (n = 11) had less (P ≤ 0.04) mean BCS (2.85 vs. 3.07; SEM = 0.07) and less concentrations of serum insulin (0.396 vs. 0.738 ppmol/L; SEM = 0.115) and insulin-like growth factor I (35.9 vs. 57.9 ng/mL; SEM = 4.2), and these outcomes were noted since 21 d prior to expected calving. Cows diagnosed with SCH had greater (P &lt; 0.01) serum concentrations of cortisol at calving (30.2 vs. 22.4 ng/mL; SEM = 2.0), serum haptoglobin at 3 and 6 DIM (0.453 vs. 0.280 mg/mL on day 3 and 0.352 vs. 0.142 mg/mL on day 6; SEM = 0.046), and tended (P = 0.09) to have greater mean concentrations of nonesterified fatty acids from calving to 30 DIM (0.368 vs. 0.304 μEq/L; SEM = 0.026). No differences were detected (P ≥ 0.41) for cow BW and milk production. Cows diagnosed with SCH had less (P = 0.05) mean concentrations of milk total solids (13.2 vs. 13.8 %; SEM = 0.21), tended to have less (P ≤ 0.10) mean concentrations of milk fat (4.34 vs. 4.81 %; SEM = 0.20), protein (3.31 vs. 3.45 %; SEM = 0.05), and lactose (4.45 vs. 4.55 %; SEM = 0.04), and had greater (P = 0.02) milk somatic cell count during the initial 14 DIM (504 vs. 140 cells/μL; SEM = 90). Collectively, Holstein × Gir cows diagnosed with SCH upon calving had altered periparturient physiological parameters denoting reduced energy nutritional, increased milk somatic cell count, and less concentration of milk components during early lactation compared with normocalcemic cows.
