Il tema, di grande respiro, prende come punto di partenza il concetto braudeliano di Mediterraneo. La sua visione di un mare chiuso come una opportunità geografica per una integrazione economica fra popolazioni diverse per religioni, linguaggi ed entità etniche e politiche continua a funzionare come modello per studi applicati ad un ampio raggio di contesti. L’obiettivo che si è posta la 50ª Settimana di studi è stato quello di andare oltre lo studio dei singoli sistemi visti in modo isolato per combinare diverse analisi di mari aperti e chiusi o aree costiere, allo scopo di comprendere il ruolo di integrazione giocato in Europa dalle connessioni marittime. Poiché nelle civiltà preindustriali il trasporto per via d’acqua era più facile di quello via terra, è sembrato giunto il momento di richiamare l’attenzione sul modo in cui queste reti di relazione operavano a livello europeo e con i partner commerciali asiatici e nordafricani. Il volume prende le mosse dalle grandi tradizioni di ricerca su base regionale o tematica, che però sono state raramente integrate su una più ampia scala continentale. Immanuel Wallerstein ha elaborato il concetto braudeliano concettualizzandone le dimensioni interculturali e transnazionali e il ruolo nel sistema di divisione del lavoro. Egli lo chiamò un “sistema mondo”, non perché coinvolgesse il mondo intero, ma perché è più vasto di qualunque unità politica giuridicamente definita. E si tratta di una “economia mondo” perché il legame di base tra le varie parti del sistema è economico. I vari aspetti e le tradizioni regionali di ricerca sono stati collegati tra loro in un approccio coerente che si posto l'obiettivo di valutare: - Sulla base di quali elementi geografici, nautici, tecnici, economici, giuridici, sociali e culturali siano emerse le varie reti regionali, e come funzionavano, - Il carattere e il ruolo dei porti marittimi come punti nodali delle rotte marine e del loro hinterland, attraverso fiumi, canali e strade, - I legami commerciali e personali tra mercanti e armatori in vari porti, - In quale modo le reti regionali si collegavano tra di loro e come, nel corso del tempo, finirono per integrarsi in unità più ampie, - In quale modo le reti private, inizialmente costituite da organizzazioni di mercanti e navigatori, finirono per trattare con le autorità locali e, una volta cresciute, con gli stati e gli imperi, per proteggere i propri interessi

Abstract:
The monograph is devoted to the generalization of the factors of railway transportation in Western Siberia and their influence on the formation of the experience of saving transportation resources in 1965-1991. The basic factors are considered such as the natural conditions and the production apparatus of the region, the development of a program-oriented approach to the development of the territory and the formation of the material base of railway transport in its space. These components are considered as historical challenges for the design of labor initiatives "from below". These initiatives are classified into three groups: speeding up the turnover of wagons, using the locomotive fleet, and saving fuel, energy, and other" variable " resources of the railway transportation process. The issues are considered taking into account the trends of fading opportunities for the Soviet model of system-wide development. This chapter introduces the topic of metasemantics, developing a distinction between a metasemantic base and a metasemantic superstructure. Conceptual engineering is concerned with the meanings of our representational devices. Representational devices have meanings in virtue of some facts; there is some fact that makes it the case that ‘snow’ means snow, for example. The metasemantic base consists of those make-it-the-case facts: the grounding facts for meaning and reference. The metasemantic superstructure consists in our beliefs, hopes, preferences, and so on about our meanings. Most theorists have attempted to practice conceptual engineering at the superstructure level: they have attempted to get us to think differently about our meanings. This approach is mistaken. To change meanings we need to change the grounding facts: we need to change the metasemantic base.

This paper was one of several that cemented the key role of nerve growth factor (NGF) in sensory neurons in inflammatory pain processing. It used a novel way of biologically ‘removing’ NGF using a sequestration molecule that could then investigate the effect of the absence of NGF in pain paradigms. These data contributed to the robust evidence base indicating the importance of NGF and, although there had been issues in developing usable anti-NGF moieties in human pain conditions, this study could be considered part of the journey that has led to the introduction of antagonists to NGF actions for the treatment of pain in bone.

The most important principle in understanding disorders of water balance is that sodium balance is determined by the adequacy of the effective circulating volume, while water balance is determined by osmoregulation and the interplay between vasopressin activity, renal concentrating and diluting ability, and thirst. Disorders of sodium balance can be determined only by clinical examination. Orthostatic hypotension implies volume depletion and sodium deficiency. Edema implies volume excess and sodium excess. Potassium is predominantly an intracellular cation. The intracellular balance of potassium is regulated by endogenous factors such as acidemia, sodium, adenosine triphosphatase, insulin, catecholamines, and aldosterone. Clinically, it is absolutely critical to follow a stepwise approach to acid-base disorders. Metabolic acidosis is defined as a primary disturbance in which the retention of acid consumes endogenous alkali stores. This is reflected by a decrease in bicarbonate. Metabolic alkalosis is defined as a primary disturbance in which plasma bicarbonate is increased. The signs and symptoms of metabolic alkalosis include weakness, muscle cramps, hyperreflexia, alveolar hypoventilation, and arrhythmias.

This chapter proposes an analysis that derives the word order variation in dependent clauses in OHG within a universal VO base order, plus additional cyclic leftward movement operations that target different information-structural projections in the complex left periphery of the clause. More precisely, it is argued that categories conveying contrastive information land in [Spec,FocP], with the finite verb targeting Foc° and marking the left edge of the new-information focus domain, while background information is placed further left, between ForceP and FocP. This positional realization of the verb and phrases expressing different semantic types of focus is considered a special strategy of disambiguating broad from narrow focus, as well as of avoiding the clash of two focus phrases in the middle field of clauses with multiple foci.

Abstract:
Advice for pregnant women on food avoidance, dangerous exposures, and inappropriate behaviours abounds on the internet and through various information sources. This chapter reviews the evidence base for such advice and clarifies issues where common advice is not supported by credible data. Foods containing potential teratogens, mutagens, or toxicants that need consideration include liver (high vitamin A), some herbal teas, contaminated grains, predatory fish, caffeine-containing foods, and various sources of foodborne infections. Exposure to environmental toxicants such as lead, pesticides, herbicides, polycyclic aromatic hydrocarbons, bisphenol-A, and other endocrine-disrupting chemicals should be avoided, as should alcohol consumption and cigarette smoking. Restrictive diets and unusual dietary cravings (pica) need to be properly managed. Evidence for harm from personal care products is generally weak, but pregnant women may choose to avoid some unnecessary exposures.

Abstract:
Metabolic alkalosis occurs in up 51% of abnormal acid-base samples in the hospital. It is characterized by a primary increase in bicarbonate concentration and is always associated with chloride depletion. In critically-ill patients, it is most often generated by diuretic administration, digestive losses, alkali administration, or rapid correction of hypercapnia. Even after all causal factor are removed, it can be maintained by blood volume depletion and potassium depletion. Metabolic alkalosis results in hypercapnia, hypoxaemia, cardiac arrhythmias, altered consciousness, and neuromuscular hyperexcitability. It is first treated by removing the causal factors, whenever possible. Maintaining factors must be reversed by sodium chloride and/or potassium chloride administration. Acetazolamide and renal replacement therapy, when given for specific indications, can also correct the alkalosis. Lysine and arginine chloride are no longer used. If metabolic alkalosis is severe or when other treatments are contraindicated or ineffective, hydrochloric acid infusion is useful. Dilute hydrochloric acid can be infused safely, provided adequate precautions are taken to prevent extravascular leakage, vessel damage, and tissue necrosis.
