This volume presents the Proceedings of the International Conference of the MEDCORE project held in Florence, Italy, in November 2005. The MEDCORE project "From river catchment areas to the sea: a comparative and integrated approach to the ecology of Mediterranean coastal zones for sustainable management", funded by the European Commission, was carried out from 2002 to 2005 by a consortium of researchers from European and Mediterranean partner countries. It focused on coastal areas, with particular attention to the interactions and links between the elements. The approach was characterised by multidisciplinary research and integration of expertise. This book contributes to the information flow originated by the project and the conference. Despite the variety of the contributions, they have been gathered into this single volume to construct a baseline for an innovative interdisciplinary perspective.

Neurocognitive outcome studies are a potentially rich source of information for evidence-based approaches to neuroprotection. This chapter provides a critical examination of assessment methods for studying postoperative cognitive decline (POCD), with regard to definition of the phenomenon, the role of hypotheses regarding mechanisms and localization of neural injury, and psychometric considerations for the design of POCD assessment protocols. It also describes an approach to conducting clinical neuropsychological assessments outside of research contexts for patients who present with signs or symptoms of POCD when there are no preoperative baseline cognitive test data. The authors propose a set of guidelines for designing, conducting, interpreting, and evaluating cognitive outcome studies in surgical patients and performing clinical POCD assessments.

Psychophysiological measures provide useful tools for investigating neurobiological mechanisms of trauma-related sequalae. In addition, they can serve as objective biological assessments of symptom severity in clinical research. This chapter describes the methods for collection of psychophysiological measures. These include muscle contractions (startle), electrodermal skin conductance, heart rate, and heart rate variability (HRV) at baseline, under stress, and following Pavlovian fear conditioning. These approaches are important both for understanding biology as well as for providing objective biomarkers that can be compared translationally from animals to humans. It also reviews the literature that has used these measures in PTSD. The evidence to date strongly suggests that these data provide robust correlates of PTSD severity.

Like every animal, human offspring evolved to fit into their communities, but social fittedness for mammals requires a supportive early nest that fosters socio-emotional intelligence, self-regulation, and sympathy. Within a supportive environment, children naturally develop orientations that facilitate prosocial behaviours within the community. We use the evolved developmental niche (EDN), apparent in 95% of human history as small-band hunter-gatherers, for a baseline representative of human evolution. In these societies, children grow into cooperative, agile moral actors. We compare the EDN with five modern approaches to young child group care and make suggestions to early caregivers on how to provide, in the modern world, what children evolved to need.

The Great Recession of 2008/09 and its aftermath present a major challenge to macroeconomics. Many researchers think that prevailing models fail to grasp essential aspects of recent developments, including unprecedented monetary policies and interest rates at the zero lower bound. Approaches that focus on steady states, rational expectations, and individuals planning over infinite horizons are not suitable for analyzing such abnormal situations. This text does not criticize the traditional approach but aims at improvement. The study’s distinctive feature is a rich institutional structure that includes elements such as credit money, external finance, borrowing constraints, net worth, real estate, and commercial banks. To cope with such a complex setting, the text reduces rationality requirements but adheres to the method of dynamic general equilibrium (DGE) with optimizing agents and fully specified models. Results are derived from mathematical reasoning and simulations. Starting with a simple baseline model, the argument is developed step by step in a unified framework that covers almost everything of interest for monetary macroeconomists. The topics discussed include the superneutrality of money, the Tobin effect, monetary policy under sticky prices and wages, but also liquidity traps with borrowing constraints, Fisherian debt-deflations, housing cycles, and environments with excess bank reserves. The text addresses researchers worldwide and may prove useful for teaching postgraduate and advanced graduate courses. The principle objective is to demonstrate that a “not-too-rational” DGE approach makes it possible to develop clean models that work outside steady states and are appropriate for answering macroeconomic questions of actual interest.

Violence against children is widespread in the Region of the Americas and it takes many different, equally unacceptable forms. This report is a major milestone for the Region, because it is the first of its kind. It specifically builds on and is informed by the momentum of INSPIRE: Seven Strategies to End Violence Against Children, a technical package of seven strategies based on the best available evidence and with the highest potential to end violence against all children. The report provides a more in-depth analysis of strategies and approaches to prevent and respond to violence against children in the Americas. Being in line with the commitment made by Member States to the Sustainable Development Goals (SDGs) and multiple regional and global strategies, attention to this topic is timely. Its importance is further underlined by the fact that it is the first time that governments are self-reporting on their work to address violence against children in line with the evidence-based strategies of INSPIRE. The report comes at unprecedented times, as the COVID-19 pandemic has created a new urgency for action on violence against children. This report offers a baseline of effort in the Region to prevent and respond to violence against all children everywhere.

This chapter first explores the logical imperative that if autonomy is valuable for all, there must be rights for each person to have at least some baseline of autonomy. This baseline imposes duties on each person in society to do her part, including giving up some of her own autonomy to make a minimal amount of autonomy available to all. But once one accepts these duties, one accepts some kind of theory of bounded rather than unfettered individual liberty. The theory of bounded autonomy is more of a justification for the principle of distributive autonomy rather than a principle itself. This chapter articulates a first approximation of the principle of distributive autonomy. This first approximation provides a context for discussing the campaign finance cases in subsequent chapters. To further sketch the initial contours of the principle, the discussion shifts to how key autonomy and liberty theorists have approached this problem.

Ecosystem management is an integrative, systems-based approach developed in response to the inadequacy of land management strategies centered on single species or resources such as timber. Contemporary ecosystem management acknowledges the dynamism of natural systems, need for ongoing adaptive learning, and importance of citizen engagement, especially in managing public lands. However, ecosystem management faces both conceptual and ethical challenges. Core concepts—such as ecosystem, stability, health, and resilience—remain difficult to define and operationalize. In addition, rapid directional changes in ecological systems have destabilized the use of historical baselines for management. One response to this challenge is to seek new moorings in an “ecosystem services” approach, focused on the role of ecological systems in supporting human interests and needs. Although ecosystem management began as a way to broaden beyond a focus on maximizing yields of particular resources, ecosystem services approaches may—ironically—reintroduce reductionistic tendencies that thwart this original goal.

Genetic diversity in modern Arctic communities provides a baseline from which to assess population history. This is augmented by documenting patterns of genetic variation in prehistoric populations using ancient DNA methods, and inferring dietary resource information and adaptive strategies derived from stable isotope analyses. This chapter uses this multidisciplinary approach to examine population history and colonization events in the Aleutians of South Alaska, and the origin and population history of Paleoeskimo and Neoeskimo populations of the North American Arctic. The power to identify past demographic events relies on knowledge of both genetic and isotopic signatures of demographic events, and on acquisition of securely dated and well provenienced samples for analysis.

Cancer therapeutics may induce cardiac damage in the left and the right ventricle. Radiotherapy most frequently induces valvular damage, carotid stenosis, and coronary artery disease. Pericardial disease may be due to both chemo- and radiotherapy. The manifestations of both chemo- and radiotherapy can develop acutely but also become overt years after their performance, in particular after radiotherapy. The main cardiac damage of cancer therapeutics-related cardiac dysfunction (CTRCD) corresponds to the reduction of left ventricular (LV) systolic function. The Expert Consensus document from ASE and EACVI has defined CTRCD as a decrease in LV ejection fraction (LVEF) of greater than 10 percentage points, to a value less than 53%. The accurate calculation of LVEF at baseline and during follow-up is extremely important. The assessment of LV longitudinal function, in particular of speckle tracking-derived global longitudinal strain (GLS), can provide additional information, allowing early, subclinical detection of CTRCD. The ideal strategy could be to compare the measurements of GLS obtained during chemotherapy, with the one obtained at baseline. An integrated approach with the use of echocardiography at standardized, clinical preselected intervals with biomarker (ultrasensitive troponin) assessment prior to each chemotherapy cycle could be suggested in patients at high risk of CTRCD. Follow-up after therapy should depend on the type of chemotherapy/radiotherapy and the presence/absence of on-therapy CTRCD. Long-term follow-up should be planned after radiotherapy.

Errors and violations are major contributors to iatrogenic harm in patients. Anesthesia is a complex process within the complex system that is healthcare. In complex systems, decisions need to be reviewed frequently and iteratively with reset baseline data. Errors are unintentional and cannot be avoided simply by trying harder or deterred by the threat of punishment; violations, on the other hand, are deliberate deviations from safe practice. Types of error are statistically predictable, and can be understood in relation to the cognitive processes involved in their generation: either System I thinking, or System II thinking. Safety depends on doing the right things correctly the first time, using a systems-based approach (drawing lessons from aviation and other high-reliability industries) within a just culture.

In this chapter, compulsion is presented in a broader context of ‘treatment pressures’. A hierarchy of pressures is presented, each increment moving in a more coercive direction. It comprises persuasion; interpersonal leverage; inducements; threats; and compulsion. The last has been dealt with in previous chapters. The distinction between inducements and threats turns on whether rejecting a conditional proposal—if you do X, I will do Y; if you don’t, I will do Z—results in the subject being ‘worse off’ or not according to a ‘moral baseline’. Threats involve proposals making the person worse off and represent ‘coercion’; inducements, where rejection does not make the person worse off, do not. However, in the context of mental health care, inducements can be problematic. While threats, often covert, are very common in mental health care, they are considered unethical. Perhaps, if regulated, they could have a place. Justifications, across the board, can follow a ‘capacity–will and preferences’ approach.

The prognosis of acute kidney injury (AKI) depends on early diagnosis and therapy. A multitude of causes are classified according to their origin as prerenal, intrinsic (intrarenal), and post-renal.Prerenal AKI means a loss of renal function despite intact nephrons, for example, because of volume depletion and/or hypotension.There is a broad spectrum of intrinsic causes of AKI including acute tubular necrosis (ATN), interstitial nephritis, glomerulonephritis, and vasculitis. Evaluation includes careful review of the patient’s history, physical examination, urinalysis, selected urine chemistries, imaging of the urinary tree, and eventual kidney biopsy. The history should focus on the tempo of loss of function (if known), associated systemic diseases, and symptoms related to the urinary tract (especially those that suggest obstruction). In addition, a review of the medications looking for potentially nephrotoxic drugs is essential. The physical examination is directed towards the identification of findings of a systemic disease and a detailed assessment of the patient’s haemodynamic status. This latter goal may require invasive monitoring, especially in the oliguric patient with conflicting clinical findings, where the physical examination has limited accuracy.Excluding urinary tract obstruction is necessary in all cases and may be established easily by renal ultrasound.Distinction between the two most common causes of AKI (prerenal AKI and ATN) is sometimes difficult, especially because the clinical examination is often misleading in the setting of mild volume depletion or overload. Urinary chemistries, like calculation of the fractional excretion of sodium (FENa), may be used to help in this distinction. In contrast to FENa, the fractional excretion of urea has the advantage of being rather independent of diuretic therapy. Response to fluid repletion is still regarded as the gold standard in the differentiation between prerenal and intrinsic AKI. Return of renal function to baseline or resuming of diuresis within 24 to 72 hours is considered to indicate ‘transient, mostly prerenal AKI’, whereas persistent renal failure usually indicates intrinsic disease. Transient AKI may, however, also occur in short-lived ATN. Furthermore, rapid fluid application is contraindicated in a substantial number of patients, such as those with congestive heart failure.‘Muddy brown’ casts and/or tubular epithelial cell casts in the urine sediment are typically seen in patients with ATN. Their presence is an important tool in the distinction between ATN and prerenal AKI, which is characterized by a normal sediment, or by occasional hyaline casts. There is a possible role for new serum and/or urinary biomarkers in the diagnosis and prognosis of the patient with AKI, including the differential diagnosis between pre-renal AKI and ATN. Further studies are needed before their routine determination can be recommended.When a diagnosis cannot be made with reasonable certainty through this evaluation, renal biopsy should be considered; when intrarenal causes such as crescentic glomerulonephritis or vasculitis are suspected, immediate biopsy to avoid delay in the initiation of therapy is mandatory.

Grammatical gender is famously the most puzzling of the grammatical categories. Despite our solid knowledge about the typology of gender systems, exciting and unexpected patterns keep turning up which defy easy classification and straightforward analysis. Some of these question, stretch, or even threaten to cross the outer boundaries of the category. These regions are largely unexplored, yet are essential for our understanding of gender, besides being interesting in their own right. The purpose of this book is to explore the outer boundaries of the category of gender and discuss their theoretical significance. Canonical Typology, a cutting-edge approach already successfully applied to a range of linguistic phenomena, provides the ideal framework for this endeavour. In this approach, a linguistic phenomenon—for example, a morphosyntactic feature like gender—is established in terms of a canonical ideal: the clearest instance of the phenomenon. The canonical ideal is a clustering of properties that serves as a baseline from which to measure the actual examples that are found. This approach allows us to analyse any gender system and determine for each of its component properties whether it is more or less canonical. The languages discussed in this volume all diverge from the canonical ideal in interesting ways. Each language is assessed by international experts, who approach their work from a typological perspective. The book explores a wide range of typologically different languages drawn from all over the world, from South America to Melanesia, from an Italo-Romance dialect of Central Italy to Mawng of Northern Australia.

Adaptive governance is defined by a focus on decentralized decision-making structures and procedurally rational policy, supported by intensive natural and social science. Decentralized decision-making structures allow a large, complex problem like global climate change to be factored into many smaller problems, each more tractable for policy and scientific purposes. Many smaller problems can be addressed separately and concurrently by smaller communities. Procedurally rational policy in each community is an adaptation to profound uncertainties, inherent in complex systems and cognitive constraints, that limit predictability. Hence planning to meet projected targets and timetables is secondary to continuing appraisal of incremental steps toward long-term goals: What has and hasn’t worked compared to a historical baseline, and why? Each step in such trial-and-error processes depends on politics to balance, if not integrate, the interests of multiple participants to advance their common interest—the point of governance in a free society. Intensive science recognizes that each community is unique because the interests, interactions, and environmental responses of its participants are multiple and coevolve. Hence, inquiry focuses on case studies of particular contexts considered comprehensively and in some detail.Varieties of adaptive governance emerged in response to the limitations of scientific management, the dominant pattern of governance in the 20th century. In scientific management, central authorities sought technically rational policies supported by predictive science to rise above politics and thereby realize policy goals more efficiently from the top down. This approach was manifest in the framing of climate change as an “irreducibly global” problem in the years around 1990. The Intergovernmental Panel on Climate Change (IPCC) was established to assess science for the Conference of the Parties (COP) to the U.N. Framework Convention on Climate Change (UNFCCC). The parties negotiated the Kyoto Protocol that attempted to prescribe legally binding targets and timetables for national reductions in greenhouse gas emissions. But progress under the protocol fell far short of realizing the ultimate objective in Article 1 of the UNFCCC, “stabilization of greenhouse gas concentrations in the atmosphere at a level that would prevent dangerous anthropogenic interference in the climate system.” As concentrations continued to increase, the COP recognized the limitations of this approach in Copenhagen in 2009 and authorized nationally determined contributions to greenhouse gas reductions in the Paris Agreement in 2015.Adaptive governance is a promising but underutilized approach to advancing common interests in response to climate impacts. The interests affected by climate, and their relative priorities, differ from one community to the next, but typically they include protecting life and limb, property and prosperity, other human artifacts, and ecosystem services, while minimizing costs. Adaptive governance is promising because some communities have made significant progress in reducing their losses and vulnerability to climate impacts in the course of advancing their common interests. In doing so, they provide field-tested models for similar communities to consider. Policies that have worked anywhere in a network tend to be diffused for possible adaptation elsewhere in that network. Policies that have worked consistently intensify and justify collective action from the bottom up to reallocate supporting resources from the top down. Researchers can help realize the potential of adaptive governance on larger scales by recognizing it as a complementary approach in climate policy—not a substitute for scientific management, the historical baseline.

Relatively few studies have investigated the strategies, policies, programmes, and projects concerning rural NEETs (namely, young people living in rural areas neither in employment nor in education or training). This special issue of Quality of Life Jour- nal is dedicated to fulfilling this gap in the literature. It contributes case studies from European Union (EU) member states. The primary purpose of this special issue is to approach and discuss the various strategies, policies, programmes, and projects appli- cable to youth rural NEETs. This special issue edition has sought to gather research papers (or critical, theoretical analysis on selected topics) covering NEETs strategies, policies, measures, programmes, and projects. These highlight the success factors and scope for transferability of good (or, how to avoid bad) practices. To that end, they examine pertinent labour market and NEET situations which can help to explain the context, reasons, and baseline for the various strategies and policy measures being implemented. They also cover assessment of NEET strategies by focusing on formal education, training and skills development, and active employment policies, in particular the role of Youth Guarantee in local/rural employment. Various other relevant factors are considered/examined, including: assessment of the NEET policy measures and results; objectives, target groups, geographical and sectoral scope in different economic activities, financial, legal and institutional frameworks; results and lessons learned, including adaptations due to crisis; domestic and international programmes, as well as numerous projects and instruments; difficulties and constraints; and, finally, assessment of the success factors and transferability of projects and strategies thereof

Liberalism—and its promise of market-led prosperity—was in crisis well before Covid-19. Recent decades have seen a rise in concentrated unemployment, and a long-term stagnation in real wages, in many of the world’s leading economies. At the same time, the world has witnessed a dramatic rise of corporate power, and the wealth of the top 1%. Alongside this has been the failure of liberal societies to address some of the most pressing challenges of our time, including climate change. Covid-19 has only exacerbated the fragility of work, and the effects of corporate power and inequality. When Covid-19 is over, liberalism will therefore be badly in need of renovation. Indeed, to survive, liberalism will need a radical reboot—to find new ways of tackling the current challenges posed by corporate power, inequality, and climate change. This also means moving beyond recent “neoliberal” versions of liberalism toward a more truly democratic form of liberalism, or from the idea of free markets to a vision of fair markets. Fair market policies are not democratic socialist: they hold on to the idea of markets as promoting growth and freedom. But they insist that markets must be subject to wide-ranging democratic regulation. This book offers a new vision of a “fair markets” approach–and the concrete policies that could make this ideal a reality. It proposes: (1) a universal “green” jobs guarantee; (2) a significant increase in the minimum wage and government support for wages; (3) universal healthcare based on a two-track model of public and private provision, and (4) a similar public baseline for childcare and basic leave benefits for all workers; (5) a new critical infrastructure policy for nation states to sit alongside a commitment to global free trade; and (6) universal pollution taxes, with all proceeds returned directly to citizens by way of a green dividend. The common theme of all the policies is that they combine a commitment to markets with democratic commitments to equal dignity for all citizens, and the regulation of markets in line with majority interests and understandings—or the idea that markets should be both free and fair, and well-functioning, as opposed to simply “free.” Because of this, they are also policies that are “blue,” “pink,” and “green.” The book also explains how to pay for these ideas, and the kind of democratic politics needed to make them a reality.
