Abstract:
The aims of this paper are to present the findings of a process evaluation exploring the experiences and opinions of clinicians who have been involved in the HealthPathways Barwon clinical workgroups and discuss implications for further development of the program, as well as regional health service initiatives more broadly. HealthPathways Barwon is a web-based program comprising locally agreed-upon evidence-based clinical pathways that assist with assessment, management and region-specific referral for various clinical conditions. Clinical workgroup members participated in focus groups. Coding and thematic analysis were performed and findings were compared with similar evaluations of HealthPathways in other jurisdictions. Five broad themes emerged from the focus group, each with several subthemes: (1) purpose of HealthPathways; (2) workgroup process; (3) barriers and facilitators to HealthPathways use; (4) impact of HealthPathways on clinical practice; and (5) measuring performance. Findings of particular interest were that the perceived drivers for implementation of HealthPathways Barwon are broad, HealthPathways Barwon is viewed positively by clinicians, the workgroup process itself has a positive impact on relationships between primary and secondary care clinicians, existing habits of clinicians are a major barrier to adoption of HealthPathways Barwon, the sustainability of HealthPathways Barwon is a concern and it is difficult to measure the outcomes of HealthPathways. Although HealthPathways Barwon is viewed positively by clinicians and is seen to have the potential to address many issues at the primary–secondary care interface, successful implementation and uptake will depend on buy-in from clinicians, as well as continuous evaluation to inform improved development and implementation. More broadly, health service initiatives like HealthPathways Barwon require longer-term certainty of funding and administration to become established and produce meaningful outcomes. What is known about this topic? HealthPathways is a program that has been implemented in Canterbury, New Zealand, and several regions in Australia. Early evaluations in these jurisdictions have found that measuring the impact of the program is challenging, and there is little evidence of the program’s influence on health system performance indicators such as waiting times. However they have found some evidence of improved collaboration between primary and secondary care clinicians and improved clinician experience in providing patient care. What does the paper add? This case study outlines a potential method of evaluation of HealthPathways, as well as some early evidence regarding the experiences of those developing, implementing and using the program in the Barwon region in South-West Victoria. What are the implications for practitioners? HealthPathways Barwon may impact positively on clinician-clinician relationships and confidence, however getting more clinicians to use the program may require identification of ways to better incorporate HealthPathways use into their existing clinical information sourcing and referral routines. Additionally, HealthPathways Barwon’s future depends on recruitment of more clinicians to develop and update pathways.

IntroductionLittle is known about the bone health of adults with bipolar disorder, aside from evidence purporting bone deficits among individuals with other mental illnesses, or those taking medications commonly used in bipolar disorder. In this paper, we present the methodology of a case–control study which aims to examine the role of bipolar disorder as a risk factor for bone fragility.Methods and analysisMen and women with bipolar disorder (~200 cases) will be recruited and compared with participants with no history of bipolar disorder (~1500 controls) from the Geelong Osteoporosis Study. Both cases and controls will be drawn from the Barwon Statistical Division, south-eastern Australia. The Structured Clinical Interview for DSM-IV-TR Research Version, Non-patient edition is the primary diagnostic instrument, and psychiatric symptomatology will be assessed using validated rating scales. Demographic information and detailed lifestyle data and medical history will be collected via comprehensive questionnaires. Participants will undergo dual energy X-ray absorptiometry scans and other clinical measures to determine bone and body composition. Blood samples will be provided after an overnight fast and stored for batch analysis.Ethics and disseminationEthics approval has been granted from Barwon Health Research Ethics Committee. Participation in the study is voluntary. The study findings will be disseminated via peer-reviewed publications, conference presentations and reports to the funding body.

Abstract:
Objective HealthPathways (HPW) is an international web-based information portal that provides health practitioners with guidelines and referral pathways to specialists and services. The present study explored usage of HPW by general practitioners (GPs) in the Barwon region, south-west Victoria, and any benefits and barriers to its use. Methods Approximately 421 GPs provide services in 86 clinics across the Barwon region, south-west Victoria and Barwon Health is the public health service providing acute, subacute, residential aged care and community health services to approximately 350000 people. The present study was a mixed-methods analysis of all GPs in the region, who were invited to complete a survey in two waves (2014 and 2016) and participate in focus groups. Data were survey/questionnaire (fixed response and free text options, analysed with descriptive statistics and content analysis), and focus groups (interview and facilitated group discussion, analysed using the principles of thematic analysis). Results Most GPs surveyed used HPW and usage increased over time from 2014 to 2016 (67% vs 77% respectively). Junior GPs used HPW more often than the more experienced practitioners. GPs reported that HPW was easy to access and navigate, improved their knowledge of local services, improved their confidence, changed their clinical management and saved them time. Main barriers to use of HPW were: GPs did not think to look at HPW, or simply did not know about it. Conclusions HPW has the potential to improve patient management and health outcomes, and use of HPW is increasing over time. What is known about the topic? Initiatives such as HPW are viewed positively by clinicians and have the potential to address challenges at the primary–secondary care interface, specifically, referral to secondary specialists. What does this paper add? Proof of concept that the ongoing availability of localised HPW for common clinical conditions improves GP usage of HPW and has increased access to HPW as the first source of clinical information. Benefits and barriers to use have been identified. What are the implications for practitioners? HPW improves knowledge of local services and provides GPs with easy access to referral pathways.

Abstract:
The Jean Hailes Foundation has a distinct approach to the health care of women. The integration of a team of clinicians, educators and researchers enables the Foundation to give women access to relevant, timely and clear information about their health and health care issues to assist them in making informed health choices. The Rural Women's Education Program has been developed to meet the increasing demand for more information by rural Australians. The isolation of rural women, their difficulties in service access and transport availability are some of the problems addressed in the planning process for each respective area. To date each program has consisted of large public seminars, interactive information sessions and professional training. The response to each program has been overwhelming. Local ownership of the event has been part of its success. The 'Regional Women's Health Plan, 1998 - 2000' for the Barwon-South Western Region outlined certain health needs in the area. The Foundation responded to this need and, in collaboration with local network leaders, devised a program to meet the specific needs of that community. The planning process, delivery and evaluation of this successful program are discussed.

PurposeThe Vitamin D in Pregnancy Study is a long-term ongoing cohort study. It was conceived to explore relationships between maternal vitamin D status in pregnancy and offspring growth and development, and has since diversified to include a wide range of physical and mental health exposures and outcomes.ParticipantsRecruitment was from the University Hospital Geelong (Barwon Health) antenatal clinic, Geelong, Victoria, Australia, between 2002 and 2004. 475 women were initially recruited, which resulted in 400 eligible mother–child pairs at birth.Findings to dateThe cohort has been followed up twice in pregnancy, at birth, and 1 year, 6 years and 11 years post birth. The study has reported an association between vitamin D in pregnancy and musculoskeletal health and body composition in the children.Future plansSubject to funding, there will be a prospective young adult follow-up. This profile aims to foster both cross-national and international collaborations with both existing and future data collection.

Abstract:
Abstract This thesis set out to achieve the following objectives: (1) To identify the priorities and expectations that the Geelong community has of its public health care system. (2) To determine if there is a common view on the attributes of a just health system. (3) To consider a method of utilising the data in the determination of health care priority setting in Barwon Health. (4) To determine a model of community participation which enables ongoing input into the decision making processes of Barwon Health. The methodology involved a combination of qualitative and quantitative research. The qualitative work involved the use of focus groups that were conducted with 64 members of the Geelong community. The issues raised informed the development of the interview schedule that was the basis of the quantitative study, which surveyed a representative sample of 400 members of the Geelong community. Prior to reporting on this work, the areas of distributive justice, scarcity and community participation in health care were considered. The research found that timely access to public hospitals, emergency care and aged care services were the major priorities; for many people, the cost was less relevant than a quality service. Shorter waiting times and increased staffing levels were strongly supported. Increased taxes were nominated as the best means of financing the health system they sought. Community based services were less relevant than hospital services but health education was supported. An egalitarian approach to resource distribution was favoured although the community was prepared to discriminate in favour of younger people and against older people. There was strong support for the community to be involved in decision making in the public health care system through surveys or focus groups but very little support was given to priorities being determined by politicians, administrators and to a lesser extent, medical professionals.

Abstract:
In July 1980, the Barren River District Health Department (BRDHD), serving eight counties (combined population approximately 204,000) in Southcentral Kentucky, was selected as a demonstration site under the auspices of the federal Health Education Risk Reduction (HERR) Program. With continued HERR funding for eight years, the BRDHD developed several successful health promotion projects. Major components of these projects include: 1) community health promotion, which serves to identify high -risk groups in the community and provide them with health education-health promotion services, 2) school health education which included the development of a preschool health education curriculum, 3) teacher education workshop, which instructs primary and secondary public school teachers in health education methods, 4) smoking cessation. and 5) a large industrial wellness program. This study reports on an eight year program evaluation of the HERR demonstration. Overall, the program evaluation suggests an increase in health knowledge and some attitude and behavior change for many of the participants ii BRDHD programs.

Övervikt och fetma är ett angeläget och ökande problem bland barn och ungdomar både globalt och nationellt. Det finns många faktorer som bidrar till att utveckla övervikt och fetma exempelvis stress, låg social status, föräldrarnas BMI, mindre fysisk aktivitet och alltför energirik kost. Det har visat sig att de omgivnade faktorerna har en starkare påverkan på utvecklandet av övervikt och fetma än de genetiska. Det hälsofrämjande arbetet skall påbörjas inom barnhälsovården för att sedan följa med barnet till skolhälsovården. Calgary familjemodell är ett bra redskap som hjälper distriktssköterskan att se familjen i sitt rätta sammanhang och kan därigenom ge familjen rätt stöd och hjälp utifrån deras förutsättningar för att skapa ett hälsosamt liv.Syftet med studien är att beskriva distriktssköterskans hälsofrämjande arbete mot övervikt och fetma inom barn- och skolhälsovården. Genom kvalitativ innehållsanalys har tolv vårdvetenskapliga artiklar analyserats för att få en god förståelse i ämnet och uppnå syftet med studien. Det framkommer att övervikt och fetma etablerar sig i allt yngre åldrar, därför är det viktigt att det hälsofrämjande arbetet påbörjas tidigt i barndomen. Det förutsätts att hela familjen involveras, eftersom det är föräldrarnas ansvar att grundlägga ett hälsosamt synsätt hos sina barn. För att motivera familjen bör distriktssköterskan ha god kännedom om familjen och dess livssituation. Distriktssköterskan beskriver att det hälsofrämjande arbetet är frustrerande och komplext. De saknar tid för familjerna och uppdaterat arbetsmaterial för att handha ämnet på ett bra sätt. Distriktssköterskan har ett stort ansvar i att anpassa det hälsofrämjande arbetet mot övervikt och fetma hos barn både inom barn- och skolhälsovården utifrån varje familjs förutsättningar. Familjens involvering skall genomsyra arbetet med barnen, men det blir ett etiskt dilemma när distriktssköterskan har en större ambition än familjen i livsstilsförändringen. För att distriktssköterskan skall känna sig trygg och kunna möta barnet och dess familj på ett tillfredställande sätt anser vi att det behövs mer tid och resurser i form av ett uppdaterat arbetsmaterial och en förstärkt personalstyrka.

Abstract:
Many rural communities in South Africa do not have running water or electricity. The fetching and heating of water is therefore a time consuming and expensive daily ritual. The use of energy sources such as wood or coal are not readily available and cause environmental pollution. Although solar water heaters are commonly available in South Africa, they are very seldom used in rural areas. Whilst this can mostly be attributed to a high system cost, current designs also do not cater for specific rural problems such as the transporting or purification of water. A prototype model, designed with such an approach in mind, has already been constructed by TEMM International (Pty.) Ltd. The Solar Heat Barrow (SHB) was developed in the 1992 to 2003 period with the intention of combining a number of functions at low cost. Specific requirements were: low cost, a suitable design and materials for manufacture in large volume, sufficiently durable taking into account the harsh conditions of use. suitability for cases where no in-house piped water supply was available, the use of appropriate technology and the improvement of quality of life. The unit combines the absorption of solar radiation, the heating of a relatively small volume of water, the transport of the water from the point of supply and the storage of the hot water until it is used. Untreated water sources such as surface waters (streams, rivers, lakes, etc.) or unprotected open wells are the vehicles for waterborne bacterial diseases such as cholera and typhoid fevers. In the case where water is collected from these sources, the SHB has a build-in Purification Dispenser that purifies the water in the collector against waterborne bacterial diseases. Certain research questions need to be answered. They will be answered by demonstrating the SHB in two communities where no in-house piped water supply is available and by establishing the socio-economic response of the users. The research questions are as follow: What are the responses of the users concerning the SHB, in comparison to those of a control group, regarding its operation, durability, utility and satisfaction of needs? What is the daily use of hot water and the reduction in energy use and cost? To what extent will the target community purchase the SHB at the full or subsidised commercial price? Is there a business case that can be developed for the large scale production, marketing. financing and Small, Medium and Micro Enterprises (SMME) development of the SHB? It was decided to choose a community in the Valley of Thousand Hills in KwaZulu Natal as the demonstration site. The name of the community is Mabedlane. It is a remote rural area 20 km's north of Botha's Hill and is situated along the Umgeni River. The community is dependent on the river for domestic water. Most families are headed by women. The area has low levels of infrastructure, poor roads, a high unemployment rate and poor health facilities. The first survey, which was conducted before the test period started, showed a very positive response from potential users. From 112 questionnaires that were given to the people of Mabedlane, all indicated that they were interested in a product that can transport and heat water for domestic use. The socio-economic study has shown that the price per unit needs to be adapted as 85% of the people who participated in the survey indicated that they would only pay less than R100 for the product. 15% indicated that they would pay between RlOO and R200. From the second and third questionnaires it was clear that the users were satisfied with the heating performances of the SHB. The community was very interested in purchasing a SHB. They have realised that a SHB will improve their standard of living and regard it as a necessity in their day to day activities. It is apparent that people, who will benefit from a SHB most, are those who will not be able to pay the full retail price. Therefore, new business strategies have to be researched when implementing the SHB to the target market in South Africa. The idea of considering ways to sponsorlfund the SHB must also be investigated.

Abstract:
Based on the Migration Board's statistics report, the number of children that has gone missing within a time interval of 4 years (2010-2014) comprises 1331 children in total, of whom 146 are girls and 1185 are boys. A rising interest about what has happened to those children created a fundamental platform for this thesis. By reading Åkerman’s report from 2012, and the book De förlorade barnen: en rapport by Mikkelsen and Wagner (2013) the curiosity grew deeper. The research questions that were established were divided into three parts. Their aim is to examine which organization or public authority has the main responsibility for asylum seeking children that arrives to Sweden by themselves, if the quality of the reception has changed since the municipalities took over the responsibility, and what is/has been publicly done to strengthen the protection concerning the individual child. The aim of this study is to authentically answer the research questions, using a qualitative text analysis and an implement model. The qualitative textual analysis is primarily based on reports from Gunnarsson (2008), the Cabinet Office (2011), and a report from the National Swedish Board of Health and Welfare published in 2012. Further reports and books are used in order to authenticate the legitimacy of the arguments and facts that is accounted for in this thesis. The implementation model that is used is an implement problem shaped by Pressman and Willdawsky. The model is used in order to force the problems that solemnly appear in this thesis. The conclusion establishes that within the distinctive reports it exists different views, but also similarities. The various texts establish that despite a set of rules and regulations there has been a considerable ambiguity among the involved parties, in who holds the greatest responsibility for the asylum-seeking children. Åkerman claims that when the children arrive in Sweden they stand without any form of social safety net, whereas Gunnarsson (2008), the Cabinet Office report (2011) and the National Swedish Board report (2012) refer to the laws (LMA, SoL).

Generally, plastic is made up of non-biodegradable chemicals i.e. polyolefin, polyester, and polyurethanes. This is hardly surprising since the use of polythene bags has infiltrated all sectors of economy, agriculture, consumer goods, and especially in packaging, where polythene is increasingly replacing jute, paper, wood, and glass. Polythene bags can release harmful chemicals into the surrounding agricultural soil, which can then seep into ground water or other surrounding water sources. This can cause serious harm to the species that drink this water. Unlike natural organic material they do not disintegrate with time but keep accumulating in the environment. So it is very difficult to dispose of the polythene bags. If the polythene bags burned in agriculture land, they release harmful chemicals like Cd, Pb, CO, NOX and SO2, which causes many diseases in animals as well as in human beings. The accumulation of polythene bags also makes agricultural soil sterile/barren as it stops the gaseous exchange. In normal agricultural land, the concentration of oxygen is 3 ppm. Where polythene is present, the concentration of oxygen in soil decreases to 0.001 ppm. In addition, the polythene pollution can adversely affect agricultural lands, waterways, and oceans.

This study is an examination of the integration of information, telecommunications, mobile technology, and artificial intelligence (AI) technology in New York City's (NYC) infrastructure to maximize development and to improve the services provided to residents and their quality of life. Efficiency in service delivery is enhanced through smart technologies, and embracing innovation makes city systems efficient. The study is based on Barlow and Levy-Bencheton's smart cities theory. The research questions concern how the integration of smart technology improves the quality of life for residents and provides economic benefits for the city. The researcher surveyed 425 New York City residents to analyze the impact of smart technology on the city's systems. The findings indicated that 96% of residents experienced positive effects from smart technology on their quality of life. Smart cities have digitalized systems to enhance water supply, transportation, waste management, safety, public awareness, and health service delivery, among other essential services.

While it has been described as ‘a paean to a canine friend’ and ‘a meditation on love and loss’, Laurie Anderson’s Heart of a Dog (2016) can also be understood as a network of ghost stories. Drawing on Anderson’s idiosyncratic multimedia technique (foregrounding technology) and conceptualizing of the future, this chapter explores the ways in which the figures of 9/11, Lou Reed, David Foster Wallace, Gordon Matta-Clark, and the Bardo course through Heart of a Dog. Exploring the implications of the juxtaposition of these themes and Anderson’s oeuvre, Williams positions the film in relation to a confluence of network theory and hauntology as a particular rendering of 21st-century subjectivity.

The debate over climate change, both from natural causes and human activity, is not new. Although the Baron C.-L. de Montesquieu is undoubtedly the best known Enlightenment thinker on the topic of climatic determinism, others, notably the Abbé Du Bos, David Hume, and Thomas Jefferson, observed that climatic changes exerted a direct influence on individuals and society and that human agency was directly involved in changing the climate. Climate—from the Greek term klima, meaning slope or inclination—was originally thought to depend only on the height of the Sun above the horizon, a function of the latitude. A second tradition, traceable to Aristotle, linked the quality of the air (and thus the climate) to the vapors and exhalations of a country. The Hippocratic tradition further linked climate to health and national character. As late as 1779, the Encyclopdédie of Denis Diderot and Jean le Rond D’Alembert defined “climat” geographically, as a “portion or zone of the surface of the Earth, enclosed within two circles parallel to the equator,” in which the longest day of the year differs in length on its northern and southern boundaries by some quantity of time, for example one-half hour. The article goes on to mention Montesquieu’s position on “l’influence du climat sur les mœurs, le charactère, et les loix des peuples.” The second definition of climate provided by the Encyclopdédie was medical, identified primarily as the temperature of a region and explicated through its effects on the health and well-being of the inhabitants. The idea that climate influenced culture was derived in part from the writings of ancient and medieval philosophers, geographers, and historians, including the works of Hippocrates, Albertus Magnus, and Jean Bodin. With no established science of climatology, Enlightenment thinkers apprehended climate and its changes primarily in a literary way. They compared the ancient writings to recent weather conditions, linked the rise and fall of creative historical eras to changes in climate, and promoted a brand of climatic determinism based on geographic location and the quality of the air.

Whatever their particular causes, environmental problems all share one fundamental trait: with rare exceptions they are unintended, unforeseen, and sometimes ironic side effects of actions arising from other intentions. We intend one thing and sooner or later get something very different. We intended merely to be prosperous and healthy but have inadvertently triggered a mass extinction of other species, spread pollution throughout the world, and triggered climatic change—all of which undermines our prosperity and health. Environmental problems, then, are mostly the result of a miscalibration between human intentions and ecological results, which is to say that they are a kind of design failure. The possibility that ecological problems are design failures is perhaps bad news because it may signal inherent flaws in our perceptual and mental abilities. On the other hand, it may be good news. If our problems are, to a great extent, the result of design failures, the obvious solution is better design, by which I mean a closer fit between human intentions and the ecological systems where the results of our intentions are ultimately played out. The perennial problem of human ecology is how different cultures provision themselves with food, shelter, energy, and the means of livelihood by extracting energy and materials from their surroundings (Smil 1994). Ecological design describes the ensemble of technologies and strategies by which societies use the natural world to construct culture and meet their needs. Because the natural world is continually modified by human actions, culture and ecology are shifting parts of an equation that can never be solved. Nor can there be one correct design strategy. Hunter-gatherers lived on current solar income. Feudal barons extracted wealth from sunlight by exploiting serfs who farmed the land. We provision ourselves by mining ancient sunlight stored as fossil fuels. The choice is not whether or not human societies have a design strategy, but whether that strategy works ecologically and can be sustained within the regenerative capacity of the particular ecosystem. The problem of ecological design has become more difficult as the human population has grown and technology has multiplied. It is now the overriding problem of our time, affecting virtually all other issues on the human agenda.

Abstract:
Urban heat islands (UHI) are areas with elevated temperatures occurring in cities compared to surrounding rural areas. This study realizes the lack of research regarding the trends of UHIs in desert countries and focuses on Doha. The research includes twelve months of two-time periods; 2000-2019. ArcGIS software was used to compute the land surface temperature (LST) of the city using Landsat images. Land use/land cover (LULC) maps were computed to show how the city has evolved in 19 years. 30 field samples were used to verify the accuracy of the LULC. Results showed UHI in Doha did not display similar pattern to that of cities in subtropical and temperate regions. Higher temperatures were prevalent in out-skirts comprising of barren and built-up areas with high population and no vegetation. Comparatively, the main downtown with artificially planted vegetation and shade from skyscrapers created cooler microclimates. The overall LST of greater Doha has increased by 0.7°C from 2000 to 2019. Furthermore %LULC of built up, vegetation, barren land, marsh land and water body were 29%, 4.5%, 58.6%, 2.8% and 5% in 2000 and 56.5 %, 8.2%, 33.2 %, 0% and 2.1% in 2019 respectively. Overall, there was an increase in built-up and vegetation decrease in water and barren areas and complete loss of marshland. Highest temperatures were recorded for marshland area in year 2000 and barren and built in year 2019. Transect profiles showed positive correlation between NDBI and LST and a negative correlation between NDVI and LST.

Investigations were carried out at the pig breeding farm located in Šalčininkai district. There are 9 hulls in the pig farm, two of which are dedicated to pig breeding. One barn was reconstructed: installed heated-cooled floor with the heat pump and a manure-slurry cooling system. The second barn was installed more than 50 years ago and has not been modernized yet. This hull doesn’t have mechanical cooling system, is heated by a gas heater, infrared rays are installed in the pig holders. Both barns have a mechanical air exhaust system. The research data were collected during the winter season. Also was collected statistical data on the birth rate, weight gain, morbidity and piglets’ death. The parameters of the old barn microclimate depended directly on outdoor air parameters. The average temperature in the old housing was 21 ° C, in the modernized 27 °C; humidity was 79% and 54% respectively. In the winter period, in the old barn, due to low air temperatures and high humidity, the piglets’ death was 7.8% upper and the weight gain of 35% lower than in the reconstructed barn. It has been determined that the microclimate parameters influence the growth of the weight of piglets, their fall, and morbidity. The installed heated-cooled floor and manure-slurry cooling systems allow the farm to not use other types of fuel, facilitate the maintenance of the microclimate parameters of the barn (maintenance of the design temperature all the year), reduce emissions of CO2 and NH3 into the environment, and receive higher revenues from growing and selling products.

The very primary step of physiological needs, eating habits and constraints, of such a wide audience who are supposed to be at the top of self-realization is an issue that is carefully be focused on. The aim of this study which tries to investigate Bartın Vocational School students’ tendency and constraints on nutrition is to search and identify daily main meals and snacks of students; to determine body-mass index on age basis;; and to evaluate the arguments whether interdependent variables such as economic level, place of residence and gender which are allegedly affect nutritional care have influence on food choice. This study is considered important because it assesses dietary habits and tendencies of consumers with different levels of opportunity and limited incomes. In this context, secondary data have been given in theoretical framework. Primary data, on the other hand, have been obtained through a questionnaire that is compiled from various sources. Research problem and hypotheses have been tested by parametric test Independent-Samples T Test and frequency analysis. As results of the research, the findings that students do not have adequate nutrition awareness though their body-mass index is in normal range; that they consume fast food particularly during the day; and that this situation leads to the so-called morbid obesity health problems have been discussed. In addition, policy recommendations and suggestions have been made in this regard.
