Abstract:
BackgroundSialosis is a non inflammatory nor neoplastic expansion of the salivary glands. it is a common mimicker for obstructive, inflammatory and malignant salivary gland disorders.(1) It had been described in acromegaly, vitamin A, and Thiamin deficiency and in amylophagia and bulimia.(2-5) Imaging should be normal in Sialosis(6) Here we describe a case of Sialosis due to Vitamin deficiency who was treated for a few months with anti-tuberculous and anti inflammatory drugs.ObjectivesTo raise awareness about this cause of parotid swelling and encourage practice of exclusion prior to treatment.MethodsA 30 year old lady with no significant past medical history presented to us with 2 years of slowly progressive, non tender bilateral facial swellings, with no excess or reduced salivation and no pain upon eating, she had negative history of preceding upper respiratory tract infection. the condition was associated with feeling a lump in her right axilla, and cervical region, there was no fever or systemic manifestations. she consulted a surgeon during that period, an exisional biopsy of the lump revealed the presence of fatty lumps, she was then prescribed antituberculosis medications since her symptoms were vague and because TB is quite common in Iraq. She denied any improvement with these medications. She then was prescribed treatment with hydroxyChloroquin and prednisolone by another physician assuming she had sjogren’s syndrome. When we saw the patient she was extremely distressed and depressed, had bilateral diffused parotid swellings, along with submandibular swelling, no mass could be felt during the exam, her mandibles and neck appeared large, she had normal tear film production and negative eye test, with no articular or muscular abnormalities.ResultsHer Labs were negative, and ultrasound for the parotids revealed enlarged glands with normal texture, MRI showed enlarged both parotids and submandibular glands with no other abnormality, minor salivary gland biopsy showed normal tissue with no inflammatory cell infiltrates. she had normal thyroid, parathyroid and growth hormone levels, normal metabolic panel and denied history of Alcohol consumption of Bulimia, but rarely consumed meat. a trial of Vitamin A, Thiamin and pilocarpine was started, the patient reported regression in the size of the swellings after 2 weeks of therapy but then developed mucosal dryness, afterwards, Vitamin A was stopped, she developed bone pain with normal bone markers and for that reason, pilocarpine was stopped. despite stopping these medicines, the size continued to regress on thiamin supplement alone, and she was prescribed anxiolytics and antidepressants by a Neurologist.Figure 1.Showing the appearance A, before and B after one month of treatment.ConclusionSialosis can be a manifestation of variable metabolic and endocrine conditions and it should be kept in mind when facing a patient with parotid swelling being a potentially treatable cause.References[1]References in Sialosis: 35 cases of persistent parotid swelling from two countries - British Journal of Oral and Maxillofacial Surgery [Internet]. Abstract:
Abstract Background: Carcinogens have been observed in many tattoo inks which can form new carcinogenic compounds when exposed to ultraviolet radiation and can accumulate in the lymph nodes and other organs. Tattooing can result in short- and long-term inflammatory and immune responses. At least 38 cases of melanoma have been reported in tattoos. The incidence of melanoma in Utah is the highest in the U.S., and potentially modifiable risk factors are of public health interest. In this study, we evaluated associations between tattooing and melanoma risk. Methods: We utilized a population-based case-control study of 514 incident in situ (284 men and 230 women) and 534 invasive (285 men and 249 women) melanoma cases diagnosed in Utah between 2020-2021. Controls (n=5,240) were selected from respondents to the Utah Behavioral Risk Factor Surveillance System (BRFSS) survey and frequency matched to cases in a 5:1 ratio on sex, age, race, and ethnicity. We fit logistic regression models to compute odds ratios (ORs) and 95% confidence intervals (CIs) associating ever tattooing with in situ and invasive melanoma risk, adjusting for age, education level, race, and ethnicity, separately for women and men. We then fit logistic models stratified by education level (< four-year college degree/four-year college degree or more) and models stratified by age (< 50 years/50+ years), separately for women and men. Results: Among men, the prevalence of tattooing was 7% for both in situ and invasive cases, and 11% for controls. Among women, tattooing prevalence was 20% for in situ cases, 15% for invasive cases, and 18% for controls. Ever tattooing was associated with decreased invasive melanoma risk among men (OR=0.62 [95% CI: 0.37-1.02]), particularly among those who had less than a four-year college degree (0.52 [0.28-0.99]), and among ages 50+ (0.36 [0.16-0.84]). Among men, the OR for ever tattooing and in situ melanoma was in the same direction as invasive melanoma, but the estimate was statistically imprecise (0.73 [0.45-1.19]). Among women, we did not observe an association between ever tattooing and invasive melanoma (0.83 [0.56-1.20]), while the association between ever tattooing and in situ melanoma was statistically imprecise (1.25 [0.87-1.80]). However, ever tattooing was associated with a nearly two-fold increased risk of in situ melanoma among women with a four-year college degree (1.98 [1.21-3.26]), but not for those without a college degree (0.89 [0.52-1.50]). Conclusion: These findings do not suggest that there is an association between tattooing and melanoma. Associations between tattooing and health-seeking behaviors, which may vary by sex and other demographic factors, may influence associations with melanoma diagnosis, particularly for in situ melanoma. This study is the first step in investigating whether reported cases of melanoma arising in tattoos are coincidental or whether tattoos may be associated with an increased risk of melanoma. Abstract:
Показано, что фазовое превращение палладия в собственную фазу при селективном растворении сплава Ag15Pd протекает в режиме мгновенной нуклеации и лимитируется поверхностной диффузией ад-атомов Pd к растущему трехмерному зародышу новой фазы. С применением нестационарных электрохимических методов установлены кинетические закономерности процесса электроокисления муравьиной кислоты на сплаве Ag15Pd, подвергнутом предварительному селективному растворению. Найдено, что процесс анодной деструкции НСООН в кислом сульфатном растворе протекает с более высокой скоростью на анодно-модифицированном сплаве Ag15Pd, поверхность которого морфологически развита и обогащена палладием в результате потенциостатическогоселективного растворения при закритических условиях поляризации. Процесс электроокисления НСООН является нестационарным, протекает в смешанно-кинетическом режиме и ускоряется с ростом анодного потенциала. С применением метода хроноамперометрии найдены кинетические токи анодного окисления муравьиной кислоты. Обнаружена корреляция между значением электрического заряда, пропущенного при предварительной анодной модификации сплава Ag15Pd и скоростью кинетической стадии электроокисления НСООН. ЛИТЕРАТУРА 1. Christiaan Barnard is famous for his legendary operation of first ever successful human heart transplantation performed on December 3, 1967 at the Groote Schuur hospital Cape Town, South Africa. The operation captured the World public imagination as no surgical procedure had ever before or since. Christian Barnard, a little known surgeon from apartheid South Africa suddenly became the focus of media attention. The Groote Schuur team captured the headlines of the world’s leading newspapers. However, forgotten are the names of those innumerable surgeons, cardiologists, basic scientists and researchers whose research works paved the pathway to transplant success. It may be difficult to estimate the level of world’s interest about the action of Christiaan Barnard and his team. One example is a commemorative postal stamp published to honor him by countries from different parts of the world. They include Bhutan, St. Helena, Romania, Commonwealth of Dominica, Mozambique and Republic of Congo and others. Yemen Arab Republic printed a series of stamps honoring Dr. Christiaan Barnard. African country Guine Bissau printed a beautiful stamp and first day coverportraying Barnard and a beautiful image of a little girl on a hospital bed with the teddy bear in her arms.Latin American country Paraguay gave Dr. Barnard a distinct honor by portraying him on a series along with Reverend Martin Luther King & President John F Kennedy. Christiaan Barnard was among the most celebrity surgeons of the world. Many countries have printed commemorative stamps to respect and celebrate his achievement. Abstract:
Abstract Background: Approximately 50% of high-grade serous ovarian carcinomas (HGSOC) are homologous recombination deficient (HRD). HRD is associated with increased T-cell infiltration and improved survival. This study aimed to describe the distribution of HRD features and cytotoxic T-cell infiltration by gene expression subtype among Black HGSOC cases. Methods: We included Black HGSOC cases from the African American Cancer Epidemiology Study and the North Carolina Ovarian Cancer Study. We used RNASeq data to assign gene expression subtypes (n=160), and whole exome sequencing data from matched blood and tumor formalin-fixed paraffin-embedded specimens (n=206) to characterize HRD features. We defined HRD features as (1) germline variants and somatic mutations in HRD genes, and (2) HRD-associated signatures (SBS3 and ID6) that we obtained from de novo mutational signature analysis for single-base pair substitution, insertion, and deletion. We estimated the proportion of intratumoral cytotoxic T cells using multiplex immunofluorescence (n=125). Results: The subtype distribution in our study was 39% immunoreactive, 29% mesenchymal 27% proliferative, and 4% differentiated. Germline variants and somatic mutations in any HRD gene were observed in 27% and 23% of cases, respectively, and at least one of these was observed in 42% of cases. Pathogenic germline variants and somatic mutations were observed in 7% and 3% of cases, respectively. The prevalence of germline or somatic variants by gene was 16% for BRCA1, 14% for BRCA2, 11% for ATM, 5% for RAD51, and 2% each for BRIP1, PALB2, and BARD1. We observed a COSMIC HRD signature in 43% of cases, with 25% SBS3 only, 5% ID6 only, and 13% with both signatures. The prevalence of any HRD gene variant or signature was higher in immunoreactive cases (52% and 59%, respectively) than in mesenchymal (28% and 43%) or proliferative (44% and 28%) cases. Germline HRD variants were more common in the proliferative subtype (44%) compared with immunoreactive (25%) and mesenchymal (17%). In contrast, somatic HRD mutations were more common in the immunoreactive subtype (33%), compared with proliferative (16%) and mesenchymal (15%). This was due to a higher occurrence of BRCA1 somatic mutations in the immunoreactive subtype (25%) compared with proliferative (9%) and mesenchymal (8%). A higher proportion of cytotoxic T-cells (≥1% vs <1%) was also observed in the immunoreactive subtype (44%) compared with mesenchymal (31%) and proliferative (11%). Conclusion: We observed differences in the distribution of HRD features and cytotoxic T-cells by gene expression subtype. In particular, in the immunoreactive subtype, the most common gene expression subtype in Black HGSOC cases, we observed a higher occurrence of any HRD feature, somatic BRCA1 mutations, and cytotoxic T cell infiltration than in any of the other gene expression subtypes. Abstract:
Cardiac transplantation is one of the greatest medical marvels of the twentieth century. Performing this miraculous operation on 3rd December 1967, Dr. Christiaan Barnard, an unknown surgeon from the then apartheid state of South Africa suddenly became an international celebrity. Probably no single procedure in the history of medicine had attracted so much media and public attention. But there were many who thought that he didn’t deserve much of this glory. A lion share of this should have gone to somebody else. Although Barnard completed the final step in the road to transplant, it was the end product of serious research work carried out in many centers around the World. Most important was Stanford University Medical Center, Palo Alto, California USA, where Dr. Norman Edward Shumway was engaged in transplantation related research work along with his junior colleague Dr. Richard Lower. The most of the techniques used in cardiac transplantation today were actually developed by Dr. Shumway and his team. Barnard worked in the same unit with Shumway at University of Minnesota when he came to USA. He visited USA again in 1966 when he observed the works of Shumway’s research partner Dr. Richard Lower. During both of his visits he had adopted many techniques from the research work of his American counterparts and later used in his unique accomplishment. Barnard succeeded utilizing techniques developed through Shumway’s painstaking work over the years depriving Shumway much of the glory he deserved. Shumway excelled the technical details and Barnard drew media and public attention to the importance of this procedure. After almost five decades the name of Barnard is still well known by the common people around the World; whereas Shumway remains unknown even to most of the cardiac surgeons as well. Abstract This essay demonstrates that Athenagoras’ theology is primarily concerned, not with the creative activity of God, as L.W. Barnard has argued, but rather with the immateriality of the divine nature and the unity of the Father and the Son. It is this two-fold basis of distinction and unity that makes the apprehension of God possible only by mind and reason. Since the divine nature is heavenly and immaterial, such apprehension cannot occur in the physical realm as promoted in pagan worship, but must take place in the mind through the Son, who is the Logos or Mind, the Reason and Wisdom of the Father. Athenagoras’ assertion that the immaterial God can only be apprehended by reason emphasizes the distinction between God and matter, while the unity of the Father and Son in God’s acts and teachings highlights the role of reason in the soul’s apprehension of the divine. One must be conformed to the Son, who is the Reason of God, in order to apprehend God the Father, and Athenagoras evokes the ethical dimension of reason in the soul’s apprehension of the divine. As the soul follows the Son in obeying his teachings, it is conformed to the Son, thereby becoming rational and engaging in rational worship, focusing on the heavenly rather than the earthly. Thus it is in ethical conduct that Christians are essentially pure in spirit and rational in worship, as they are directed by the Son, who is unified with the Father, to apprehend the immaterial God.

En los resultados, los factores “Forma de Estudio”, “Autoevaluación” y “Emociones y Sentimientos” mostraron mayor consistencia en la relación a la muestra estudiada, lo cual refleja un desarrollo más alto de las habilidades autorreguladoras en los estudiantes en entornos virtuales en cuanto al proceso de evaluar su propio desempeño, comparando su desempeño real con sus objetivos y los procesos motivacionales para el logro de estos.

Carlotta Maury (1874-1938) was a paleontologist who specialized in Tertiary mollusks. She was involved in confidential explorations for the oil industry as well as teaching and museum work. This paper—the first of a two-part biographical narrative—traces her educational background and the earlier part of her career. She was born into a family with significant scientific interests and accomplishments. Influenced by the educator Elizabeth Agassiz, Maury attended Cornell University, where despite different academic programs designed to prepare women for occupations considered suitable for them, and the prejudice of male faculty, she obtained a PhD. Her mentor was the paleontologist Gilbert Harris. Women's difficulties in gaining acceptance in fieldwork with men and academic advancement are explored. Maury's failure to be promoted at Barnard College led her to accept a position at a college in South Africa. By 1918, expeditions there, in New York, Louisiana, Trinidad, and one that she led to Santo Domingo had resulted in several publications.

Julmarichardia gutui, n. sp. is described from specimens collected in depths of 78–83 m from the Northwest Continental Shelf of Australia. Of the four previously known species of Julmarichardia Guţu, 1989, J. gutui is distinguished from J. deltoides (K. H. Barnard, 1914) and J. thomassini Guţu, 1989 by having the outer flagella of the antennule with fewer than 9 articles and its distinctive armature on anterodorsal region of the carapace. It can be separated from J. alinati Guţu, 1989 and J. bajau Bamber and Sheader, 2005 by a combination of characters, including (1) a spatulate rostrum fringed with 20 or more blunt marginal teeth interspersed with 5–8 small plumose setae; (2) distinct ridges or tubercles on the carapace, (3) a well-developed, expanded lateral lobe on article 1 of the antenna (absent in J. bajau), and (4) 3–5 blunt spines (instead of 1 or 2) on the anterior margin of the basis of pereopod 2. A key to the five nominal species now comprising the genus Julmarichardia is presented. Julmarichardia gutui represents the first record of its genus from Australian waters and the second for the genus in the eastern Indian Ocean.

Resumen Este texto analiza los fundamentos epistemológicos del poder en el management clásico-racionalista, los describe con base en planteamientos de varios de sus principales exponentes-Barnard, Simon, Drucker, Crozier y Mintzberg- y, además, pone de relieve las racionalidades y los valores que sustentan este management, sobre todo, teniendo en cuenta que las reflexiones sobre el poder en relación con objetivos organizacionales han sido poco elaboradas por los teóricos del management, especialmente los norteamericanos. Para ello, se realiza la revisión de la corriente filosófica del vitalismo, cuyo pensador más influyente ha sido Vilfredo Pareto. El análisis sobre la racionalidad instrumental como base epistemológica del management estratégico permite mostrar cómo se establecen las interacciones entre lo humano y lo no humano y la presencia del arte, la técnica y la ciencia, incluyendo, en su etapa posterior, la cibernética social en términos de la oscilación equilibrio/desequilibrio social y organizacional. De esta manera, se observa que el discurso managerial supone unas formas de racionalidad humana y por ello construye sistemas de saberes que son transversales en una lógica técnico-instrumental. Más que el cuestionarse por las agudas asimetrías del poder organizacional, el management se ha encaminado a orientar y capacitar a los gerentes, empresas y organizaciones en el uso de tecnologías de dominación, bajo emblemas de autoridad funcional con apoyo del aparato comunicacional y, en un período reciente, en redes inter y transorganizacionales. Allí reside su fuerza y legitimidad, más que en su fundamentación en un plano explícitamente político.
