Abstract:
<p><strong>Abstract.</strong> This paper discusses the relationship between advanced geomatics and Conservation Management Plan (CMP), by deepening the case of a CMP dedicated to the conservation of the 20th century architectural heritage. A number of issues have already been discussed on how the advanced survey techniques contribute to the conservation field for the last decades (e.g. Laser Scanner, HDR, GIS, intelligence vs. abundance, B.I.M, VT/IM etc.). The authors analyse pros and cons of each technique with respect to the main purposes of a CMP: 1.knowledge, 2.value assessment, 3.data sharing and dissemination of results, 4.support for conservation and restoration activities, 5.support for the planned conservation of buildings / facility management over time. With respect to the research on the CMP for the National Art Schools of Havana, the conclusions focus on the need to share results to non-specialist stakeholders, and on the possibility to combine different scales of analysis and a plurality of buildings with various levels of interest and conservation needs.</p>

ABSTRACTThe effects of darunavir-ritonavir at 600 and 100 mg twice daily (b.i.d.) alone, 200 mg of etravirine b.i.d. alone, or 600 and 100 mg of darunavir-ritonavir b.i.d. with 200 mg etravirine b.i.d. at steady state on the steady-state pharmacokinetics of maraviroc, and vice versa, in healthy volunteers were investigated in two phase I, randomized, two-period crossover studies. Safety and tolerability were also assessed. Coadministration of 150 mg maraviroc b.i.d. with darunavir-ritonavir increased the area under the plasma concentration-time curve from 0 to 12 h (AUC12) for maraviroc 4.05-fold relative to 150 mg of maraviroc b.i.d. alone. Coadministration of 300 mg maraviroc b.i.d. with etravirine decreased the maraviroc AUC12by 53% relative to 300 mg maraviroc b.i.d. alone. Coadministration of 150 mg maraviroc b.i.d. with etravirine-darunavir-ritonavir increased the maraviroc AUC123.10-fold relative to 150 mg maraviroc b.i.d. alone. Maraviroc did not significantly affect the pharmacokinetics of etravirine, darunavir, or ritonavir. Short-term coadministration of maraviroc with darunavir-ritonavir, etravirine, or both was generally well tolerated, with no safety issues reported in either trial. Maraviroc can be coadministered with darunavir-ritonavir, etravirine, or etravirine-darunavir-ritonavir. Maraviroc should be dosed at 600 mg b.i.d. with etravirine in the absence of a potent inhibitor of cytochrome P450 3A (CYP3A) (i.e., a boosted protease inhibitor) or at 150 mg b.i.d. when coadministered with darunavir-ritonavir with or without etravirine.

L'atto del rilievo rappresenta l'operazione preliminare volta alla comprensione e conoscenza approfondita di una struttura. Un rilievo tridimensionale effettuato con le moderne tecniche geomatiche della scansione laser o della fotogrammetria digitale può fornire una rappresentazione metrica completa e rigorosa dell'oggetto a partire dalle nuvole di punti generate, opportunamente gestite. Questo dato, in sé essenziale per una corretta documentazione geometrica a molteplice valenza, può essere utilizzato anche per l'applicazione di un approccio di Building Information Modeling (B.I.M.); tale metodologia, che trova oggi adozione soprattutto in sede di progettazione, può essere infatti efficacemente adottata anche per l'esistente attraverso software di B.I.M. authoring che consentono l'utilizzo di nuvole di punti come guida per una precisa modellazione. Il B.I.M. è basato su un modello condiviso contenente informazioni, e può fornire agli utenti la possibilità di arricchire costantemente il modello parametrico e di interrogarlo per svolgere simulazioni strutturali, energetiche, ecc. Attraverso il caso di studio relativo all'edificio che ospiterà in Bologna una nuova sede della catena olandese The Student Hotel, è stato possibile creare un accurato modello B.I.M. grazie alle scansioni che hanno interessato l'intera struttura. Sono stati modellati tutti gli oggetti parametrici strutturali (travi, pilastri, setti murari, solai) con precisione millimetrica, garantita dalla tipologia di rilievo, per poi essere posizionati nello spazio virtuale. Il modello è stato sfruttato per svolgere l'analisi statica della struttura all'interno dello stesso software di modellazione B.I.M. e, grazie all'interoperabilità e alla condivisione tra i diversi ambiti concorrenti alla costruzione, si può considerare come base per l'integrazione di informazioni necessarie per compiere anche la progettazione architettonica, l'integrazione di dati impiantistici, l'analisi di efficientamento energetico, ecc.

In the last years smart buildings topic has received much attention as well as Building Information Modelling (BIM) and interoperability as independent fields. Linking these topics is an essential research target to help designers and stakeholders to run processes more efficiently. Working on a smart building requires the use of Innovation and Communication Technology (ICT) to optimize design, construction and management. In these terms, several technologies such as sensors for remote monitoring and control, building equipment, management software, etc. are available in the market. As BIM provides an enormous amount of information in its database and theoretically it is able to work with all kind of data sources using interoperability, it is essential to define standards for both data contents and format exchange. In this way, a possibility to align research activity with Horizon 2020 is the investigation of energy saving using ICT. Unfortunately, comparing the Architecture Engineering and Construction (AEC) Industry with other sectors it is clear how in the building field advanced information technology applications have not been adopted yet. However in the last years, the adoption of new methods for the data management has been investigated by many researchers. So, basing on the above considerations, the main purpose of this thesis is investigate the use of BIM methodology relating to existing buildings concerning on three main topics: • Smart data management for architectural heritage preservation; • District data management for energy reduction; • The maintenance of highrises. For these reasons, data management acquires a very important value relating to the optimization of the building process and it is considered the most important goal for this research. Taking into account different kinds of architectural heritage, the attention is focused on the existing and historical buildings that usually have characterized by several constraints. Starting from data collection, a BIM model was developed and customized in function of its objectives, and providing information for different simulation tests. Finally, data visualization was investigated through the Virtual Reality(VR) and Augmented Reality (AR). Certainly, the creation of a 3D parametric model implies that data is organized according to the use of individual users that are involved in the building process. This means that each 3D model can be developed with different Levels of Detail/Development (LODs) basing on the goal of the data source. Along this thesis the importance of LODs is taken into account related to the kind of information filled in a BIM model. In fact, basing on the objectives of each project a BIM model can be developed in a different way to facilitate the querying data for the simulations tests. The three topics were compared considering each step of the building process workflow, highlighting the main differences, evaluating the strengths and weaknesses of BIM methodology. In these terms, the importance to set a BIM template before the modelling step was pointed out, because it provides the possibility to manage information in order to be collected and extracted for different purposes and by specific users. Moreover, basing on the results obtained in terms of the 3D parametric model and in terms of process, a proper BIM maturity level was determined for each topic. Finally, the value of interoperability was arisen from these tests considering that it provided the opportunity to develop a framework for collaboration, involving all parties of the building industry.

La gestione di una rete stradale rappresenta un metodo operativo volto a garantire la funzionalità dell’infrastruttura, correlata al ciclo di vita, e sicurezza e comfort di marcia per gli utenti. Negli ultimi anni è stata rivolta particolare attenzione agli studi riguardanti i possibili interventi nell’ambito del settore stradale, con la principale finalità di migliorare le condizioni di sicurezza della circolazione. Tale problema è molto complesso e vi sono parecchie variabili in gioco dalle quali dipendono, anche, le cause dei sinistri. La complessità sta nel fatto che la gestione delle infrastrutture stradali e, in particolare, delle pavimentazioni, è un processo che racchiude in sé fasi diverse fra loro e strettamente correlate. In particolare, il processo ingloba la pianificazione, la progettazione, la costruzione, il controllo e il rilevamento, la manutenzione e, infine, lo sviluppo di adeguate ricerche. Ognuna di queste attività richiede continue scelte decisionali spesso in conflitto fra loro e, in ogni caso, strettamente dipendenti dalle risorse economiche disponibili. Tali problematiche, quando non vengono risolte, aggravano il disagio per gli utenti e, con questo, anche i costi che deve sopportare la collettività. Il principale ostacolo ad una corretta gestione del patrimonio viario è la concezione che la strada sia un bene quasi eternamente durevole e che, pertanto, il decadimento delle sue caratteristiche funzionali venga avvertito solo quando si innescano reali pericoli per la circolazione; ma ciò, ovviamente, non deve essere l’obiettivo finale del processo di manutenzione. È, invece, fondamentale tenere sotto controllo i parametri prestazionali delle sovrastrutture, affinché, in fase di esercizio, le caratteristiche funzionali e strutturali siano ad un accettabile livello di efficienza. Esistono metodi di indagine e apparecchiature non invasive, definibili “ad alto rendimento”, che consentono con speditezza e grande precisione la valutazione dello stato di salute della sovrastruttura. In base a queste indagini è possibile effettuare un continuo rilevamento delle caratteristiche prestazionali della pavimentazione, a partire dalle condizioni iniziali, creando una banca dati contenente le informazioni storiche dell’infrastruttura, i dati relativi ai flussi di traffico e agli incidenti. Sulla base di queste considerazioni, l’obiettivo della tesi è, dunque, quello di fornire un supporto di tipo tecnico/applicativo per il monitoraggio dello stato delle pavimentazioni stradali, tramite la definizione di una procedura automatizzata volta all’analisi tridimensionale dei degradi strutturali delle superfici. Definire una metodologia che sia in grado di individuare e caratterizzare dettagliatamente i degradi delle pavimentazioni diventa di fondamentale importanza per poter contribuire al monitoraggio e conseguente riqualificazione delle infrastrutture viarie. Per questo sono state sviluppate metodologie per l’analisi dei degradi, soffermando l’attenzione sulle buche e sulle irregolarità superficiali. In particolare, è stato codificato un algoritmo per la caratterizzazione automatica delle buche stradali, partendo da mappe tridimensionali della superficie, col fine di sviluppare una procedura intesa come valida alternativa alle moderne e ormai diffuse metodologie di pothole detection. La procedura ha permesso un'analisi globale dello stato della pavimentazione, individuando efficacemente le buche ed escludendo falsi positivi o altre forme di irregolarità. L'algoritmo riesce ad elaborare le immagini digitali 3D in modo diretto, senza interruzioni e/o interventi manuali, e con una certa efficienza, poiché fornisce risultati coerenti su vari tipi di immagini, con diversi livelli di illuminazione e rumore, toni, forma delle buche presente, ecc. L'approccio proposto è caratterizzato dall'interconnessione tra le caratteristiche geometriche bidimensionali della buca, quali forma, perimetro e area, e la stima della profondità. I risultati ottenuti possono essere utilizzati per classificare le buche in base alla loro estensione e profondità, per efficaci e utili analisi di manutenzione a livello di progetto o anche per valutazioni di sicurezza per i conducenti dei veicoli a 2 ruote. Pertanto, le potenzialità dell'algoritmo rappresentano un valido supporto ai processi di manutenzione e gestione della sicurezza stradale e consentono di integrarlo perfettamente con altre procedure di rilevamento automatico, come quelle già disponibili per crepe. Al fine di salvaguardare le prestazioni dell’infrastruttura, un ulteriore passaggio di questa ricerca è stato quello di studiare anche altre forme di irregolarità che comportano una riduzione delle adeguate condizioni di sicurezza, ovvero le irregolarità superficiali, siano esse ormaie o depressioni. Per questi motivi, è stato rapportato il codice di rilevamento e analisi delle buche al caso delle irregolarità superficiali. Utilizzando come base l’algoritmo di analisi delle buche, con qualche modifica, risulta possibile sviluppare un nuovo codice per il rilevamento delle irregolarità superficiali, in modo da garantire una valutazione complessiva e maggiormente rappresentativa del reale stato di degrado delle pavimentazioni. Tutto ciò, in un primo approccio, è stato pensato solo in termini teorici, per ragioni sia tempistiche che applicative; un passo avanti potrà essere, dunque, quello di codificare e validare una nuova procedura di analisi degli avvallamenti, sulla base di quanto già realizzato. Comunque, alla luce di ciò, è stata condotta un’analisi sul comportamento delle pavimentazioni in occasione di eventi piovosi, confrontando lo stato fisico delle superfici ammalorate ad analisi idrauliche relative all’accumulo di acqua superficiale. Ci si è chiesti se la presenza di degradi diffusi sulla superficie influisce sulle condizioni di sicurezza di guida e in che misura, rapportando i vari scenari alle condizioni di strada bagnata. È noto come la presenza di acqua influisca in maniera decisamente negativa sulle condizioni di aderenza ruota-superficie, portando a situazioni di pericolo specialmente in corrispondenza di alcune tipologie di degrado. In particolare, gli scenari di minima sicurezza si hanno in corrispondenza delle irregolarità superficiali (ormaie, depressioni) in corrispondenza delle quali si creano veri e propri serbatoi d’acqua che limitano, se non annullano del tutto, l’aderenza tra lo pneumatico e la superficie stradale (condizioni di aquaplaning). È stata, dunque, proposta una metodologia per la valutazione del rischio idraulico tramite la definizione di un nuovo indice prestazionale, cui è stato dato il nome di Hydraulic Condition Index (HCI), volto alla definizione delle condizioni della pavimentazione in presenza di irregolarità del manto stradale (tramite il coefficiente fisico CF) e in occasione di eventi piovosi (tramite il coefficiente idraulico CH). L’obiettivo è fornire un giudizio qualitativo/descrittivo sulle prestazioni della pavimentazione e, quindi, sul livello di rischio idraulico cui la strada è sottoposta.

In questo lavoro si presentano i risultati ottenuti attraverso l’applicazione di due metodologie di gestione delle informazioni finalizzate al progetto di conoscenza di un qualsivoglia manufatto architettonico, proponendo per entrambi i casi la creazione di un grande archivio, di facile consultazione, progressivamente aggiornabile ed incrementabile, utilizzabile in ogni momento della vita futura del manufatto stesso, consentendone un livello di conoscenza che risulti al tempo stesso dinamica ed approfondita. I modelli sviluppati mettono a disposizione, oltre ai dati oggettivi, anche le deduzioni e le ipotesi possibili caratterizzanti le parti inaccessibili e non rilevabili del manufatto che nei modelli sul nuovo non esistono; tutti i dati relativi al materiale storico, documenti, immagini e materiale diagnostico prodotto per l’occasione. L’indagine storica ed archivistica ha costituito il momento primario del processo di indagine conoscitiva; la molteplicità e l’eterogeneità dei prodotti conservati conferiscono agli archivi un’eredità preziosa e insostituibile. Essi si sono configurati come i veri e propri testimoni di una storia complessa, che ha ospitato il susseguirsi dei cambiamenti e delle “vicissitudini” della vita dei manufatti in esame. Grazie a questa esperienza ho potuto conoscere ed affrontare dal punto di vista metodologico due realtà profondamente diverse tra loro, caratterizzate da connotazioni diametralmente opposte ma ambedue rappresentative di una cultura radicata nei secoli che, in taluni casi, ha mostrato punti di contatto inaspettati. Le Mura urbiche di Firenze da una parte e l’Ex Monastero di Totolapan, Messico dall’altra si configurano come testimoni di un’epoca di rivoluzioni culturali. La ricerca qui presentata vuole quindi essere un contributo al servizio proprio della resilienza, proponendosi come un supporto al controllo ed alla testimonianza del processo di cambiamento del manufatto architettonico, finalizzati da un lato alla salvaguardarne in termini di conservazione dei sui elementi architettonici originali durante il susseguirsi, forse inevitabile, del suo adattamento funzionale alle esigenze future, dall’altro alla gestione corretta e razionale del suo piano manutentivo . La nostra realtà è lo specchio dell’animo umano ed è quindi caratterizzata da una profonda necessità di rinnovamento e cambiamento del modo di concepire e gestire l’architettura, esistente e non. Il termine “Resilienza architettonica” ha attualmente un significato molto legato all’ecologia, inteso come la capacità intrinseca di un sistema di tornare alle condizioni di equilibrio dopo un intervento esterno perturbante. Il concetto di manutenzione, in tal senso, anche se per i due casi analizzati costituisce solo uno tra i molteplici aspetti caratterizzanti, è parte intrinseca del concetto di resilienza.

In this thesis, A low-order potential based panel method is presented for the analysis of flow past a two dimensional super-cavitating hydrofoil or a three dimensional partially-cavitatin-g hydrofoil. A closure model must be employed in order to make the mathemati-cal formulation close and the solution unique. The cavity shape is determined by two methods. In the first, the cavity length is given and the cavitation number is unknow-n. We employ this method to analyze the two dimensional super-cavitating hydrofoil flows by the total velocity potential. In the second, the cavitation number is given and the cavity leng-th is unknown. We employ this method to analyze three dimensio-nal partially-cavitating hydrofoil flows by the perturbation po-tential. In the present study, the cavity shape is unknown and is a part of the solution. It makes the problem nonlinear, and we employ a nonlinear iteration method to analyze this problem. The results are compared with those obtained by different metho-ds. Careful in-vestigation shows that the present methods are robust and reliable as far as engineering applications are conc-erned.

This critical assessment is a study of B.K.M. Mtombeni's literary achievements in the Tsonga literature. The literary trend which persists throughout his novels Mibya ya nyekanyeka and Ndzi tshikeni, his volumes of short stories Ndzhaka ya vusiwana and Mavala ya yingwe and his plays Malangavi ya mbilu, Vuhlangi bya vuhlangi and Mihizo ya kayivela, was identified. Mtombeni has been a successful writer, who through his thematic patterns, plot, characterization, language and style, was able to bring out his ideas and thoughts to his readers effectively. Each chapter of this study dealt with special literary issues which contributed to the accomplishment of the research. The first chapter is a general introduction setting out the aim of the research: a biographical sketch; definitions of key terms of the topic and the literary approaches; the motivation behind the selection of the topic; the method of research; and the scope and composition of subsequent chapters. The thematic patterns of Mtombeni's works and his attitude to life are handled in chapter two, whilst the focus of the third chapter is on conflict, which is a major element of plot. Reference is also made to the other elements of plot such as climax and denouement. Chapter four addresses characterization with the aim of determining the lifelikeness, plausibility and. credibility of characters in Mtombeni's works. Chapter five analyses Mtombeni's language and style, the focus being on diction, whilst syntactic patterns, preambles or introductory paragraphs, imagery, proverbs and idioms, tone and dialogue form chapter six. Mtombeni's use of patterned language in his plays is treated in the seventh chapter, whilst the last chapter is the general conclusion of the study, with recommendations for future research.

Abstract We validate, using a coupled finite element/boundary element computer code, a recently-developed series solution for the structural acoustics problem of scattering from two submerged spherical elastic shells. Although the general purpose computational tools for acoustic scattering have never been restricted to single scatterers, the availability of the series solution provides, for the first time, the mutual validation of both exact and numerical approaches for a multiple elastic scatterer problem. The excellent agreement between the two solutions presented thus allows this problem to be added to the short list of existing benchmark structural acoustics problems possessing an analytic solution. For the purposes of this comparison, the direction of incidence is taken as parallel with the axis joining the two shells. The numerical solution uses the NASHUA code, which couples a finite element shell model of the two shells with a boundary element model of the surrounding fluid. The exact (series) solution is found by expanding in terms of classical modal series and uses the addition theorem for the spherical wave functions. The exact solution requires coupling coefficients that are expressed in terms of sums of products of Wigner 3-j symbols (or Clebsch-Gordan coefficients).

Abstract:
Ticlopidine (TI), an anti-aggregating agent which inhibits the ADP-pathway has been tested in patients with intermittent claudication (IC) in 11 randomized clinical trials (RTCs).As expected, a significant reduction of cardio-vascular events (CVE) due to systemic thrombo-embolism was observed in the 2 larger. Reduction in the number of CVE due to systemic thromboembolism in any arterial bed was observed. This prompted us to confirm the hypothesis that TI was beneficial in preventing systemic thrombo-embolism in patients with IC. Four RCTs from the 11 were blindly selected on the basis on pre-set selection criteria : placebo controlled, more than 1 month duration, , less than 5 % lost-to-follow-up (index of quality), parallel groups, proven atherosclerotic disease. Meta-analysis was performed with 5 statistical methods which gave consistent findings : as compared to 311 patients on placebo, the 301 patients on TI have had a 66 % reduction in the number of CVE during the 6 months of follow-up (9.0 % to 3 % , p = 0.002). Walking distance, a secondary objective of meta-analysis, doubled in 42 % of the patients on TI as against 27 % (p = 0.0005).It was concluded that TI 250 mg b.i.d. prevents CVE in patients with atherosclerotic IC.

Deep venous thrombosis (DVT) remains a serious and frequent complication after fracture of the hip, and even the mostefficacious prophylactic agents, e.g., warfarin, may fail to prevent DVT in up to 2056 of cases. There is evidence that low molecular weight heparin or heparin-like agents may have advantages in antithrombotic prophylaxis with reduced hemorrhagic sideeffects in patients at risk of DVT. We are engaged in a randomized prospective trial comparing the antithrombotic effect ofwarfarin (PT 1.5x control)with that of Organon 10172, a mixture ofsulfated low molecular weight glycosamioglycans (750 anti-Xa u b.i.d. sc., begunpreop and continued 9 days, followe by warfarin). Diagnosis is by 125-1 fibrinogen scan and impedence plethysmography with confirmatory phlebography. At present71 patients have been admitted, and patient groups are comparable in age, sex, type of fracture, and all other significant respects. DVT has been diagnosed in 7of 36 patients given warfarin an in 1 of35 patients who received Organon10172. Pulmonary embolism has not beenencountered. GI bleeding has occurred twice on warfarin and once on Organon 10172 There has been no difference in estimated operative blood loss, transfusion requirements, or other major bleeding complications.One patient on warfarin died ofmyocardial infarction and pneumonia. There were no other adverse reactions.The study is still in progress. The present trend in the results suggests that the heparinoid Organon 10172 may be a promising new agent to prevent DVT in high risk patients, such as those with fractures of the hip.

Fibrinogen (Fgn) and haemorheologic parameters were serially measured in 44 pts (38 males, age 59+/−7) with claudicatio intermittens due to peripheral arterial obliterative disease (PAOD) treated for 24 months with Ticlopidine (T group, 250 mg b.i.d. p.os), or placebo (P group), in a double blind randomized controlled study, part of a larger clinical trial. Fgn (immunodiffusion), haematocrit (Ht, micromethod), whole blood viscosity (BV, Contraves LS 30, at 37 C) at high (94.5) and low (0.2 sec-1) shear rates (s.r.) and plasma viscosity (PV, Contraves LS 30) were measured three-monthly. The data were evaluated by means of ANOVA and Student's t test. The values in groups T and P did not differ at baseline except for greater BV at high s.r. in group P (p< 0.05). During the observation period circannual variations appeared especially in group T, where summer-time values for most parameters (Fgn, Ht, PV, low s.r. BV) were lower (significance from p<0.05 to <0.01) than the correspondent winter-time values, while a similar pattern was observed in group P only for PV (p<0.01). Fgn (p<0.05), Ht (p<0.01), and low s.r. BV (p<0.001) were significantly lowered in group T versus group P but only in the summer months (after about 1 year of treatment). In conclusion, this study proves that long-term Ticlopidine treatment in PAOD is associated with significant lowering of fibrinogen and “improvement” in haemoreologic tests although limited to the summer observations. Ticlopidine while positively interfering with haemorheology in PAOD may not counteract other mechanisms of rheologic deterioration probably occurring during winter. Long-term evaluation of drug effects should therefore take into account spontaneous or drug-enhanced seasonal changes of the investigated values.

Abstract:
Earlier studies indicate an increased platelet reactivity in patients with hypercholesterolaemia. Physical and mental stress have also been reported to cause increased reactivity. The present study was undertaken to evaluate the effect of gemfibrozil (G) a new lipid lowering drug on platelet reactivity during physical stress. Ten otherwise healthy male subjects with serum cholesterol levels above 7 mmol/l were involved in a double-blind study. It consisted of two treatment periods of 8 weeks during which the patients were given either G (600 mg b.i.d.) or placebo (PI) and an 8 weeks wash-out period before the cross-over. At the end of the treatment periods an exercise test was carried out and platelet reactivity tested. Adrenaline, ADP and collagen were used to induce aggregation and 5-HT and T×B2 release measured. Plasma beta-TG and fibrinogen were also determined.The treshold concentration of adrenaline necessary to evoke secondary aggregation was increased in 8/10 patients during exercise after G treatment and in 2/10 after PI. When the lowest ADP concentration to cause secondary aggregation (2-4 uM) was used there was a significant decrease in the 5-HT (− 44%) and T×B2 (− 48%) secretion and a significant decrease in the area under the aggregation curve (− 28%). A decrease in 5-HT secretion was also seen after G treatment when a fixed ADP concentration of 10 uM was used. During collagen stimulation no changes were seen between the two groups. Beta-TG remained unchanged irrespective of treatment and fibrinogen showed a modest increase during exercise in both treatment groups. These results indicate a new anti-platelet effect of gemfibrozil which might be of importance in prevention of acute thrombotic events in hypercho1estero1aemic patients.

Abstract:
The aim of this study was to reassess the pattern of aggregation and to investigate the platelet-fibrinogen interaction, in response to various agonists, following T. treatment: 250 mg b.i.d. for at least 1 week. Platelets were obtained from healthy volunteers (n=8) as well as from patients with cerebro-vascular disease (n=5). The regimen was able to induce the well-known and expected alterations of platelet behaviour in citrated PRP. Washed platelets were prepared according to MUSTARD et al. (1972). (1) In a first set of experiments it was shown that these alterations persisted after resuspension of twice-washed platelets, in the presence of fibrinogen (3.7 μM); with ADP there was a marked impairment of aggregation, quantified either as maximal change in light transmission (DTLmax), or as maximal velocity; after stimulation with 100 μM ADP there was still a 40% decrease in DTL max and an about 50-fold faster de-aggregation as compared to values obtained without T. treatment. PAF-acether (0.1 to 3 μM): decreased DTLmax mainly with the higher doses, persistent de-aggregation but unimpaired velocity. Collagen (from Stago, μg.ml−1 ): DTLmax was decreased in response to low concentrations only (0.3 and 0.6). Arachidonic acid (20, 50, 100 μM) -induced aggregation remained unchanged, but the concentrations of U 46,619 able to induce an at least 50% DTLmax was shifted from 0.05 to 0.1 or 1.0 μM. (2) The association of human fibrinogen with stimulated platelets was quantified by plotting velocity against concentration of exogenous fibrinogen (0.06, 0.23, 0.92, 3.7 μM). The dose-response curves were modified in the presence of ADP (5 μM) but not PAF (0.5 μM) . With selected low concentrations of collagen or arachidonic acid addition of fibrinogen is a prerequisite for aggregation; under these experimental conditions T. treatment only induced slight modifications of the responses. Fibrinogen-dependent aggregation (DTLmax) of platelets from T. treated patients in response to ADP (10 μM) or after pretreatment with alpha-chymotrypsin was 1/3 and 2/3 of control values respectively. Taken together our results do not support the induction of a functionally thrombasthenic-like state, but rather suggest a selective impairment of ADP-mediated platelet activation. Accordingly, the pattern of platelet behaviour observed under T. treatment can be mimicked by adding ATP (as ADP-antagonist) to normal platelets.
