Abstract:

Objetivo. Presentar los resultados de la carga de enfermedad en México de 1990 a 2010 para las principales enfermedades, lesiones y factores de riesgo, por sexo. Materiales y métodos. Se realizó un análisis secundario del estudio de la carga mundial de la enfermedad 2010. Resultados. En 2010 se perdieron 26.2 millones de años de vida saludable (AVISA), 56% en hombres y 44% en mujeres. Las principales causas de AVISA en hombres fueron violencia, cardiopatía isquémica y los accidentes de tránsito. En las mujeres fueron la diabetes, la enfermedad renal crónica y la cardiopatía isquémica. Los trastornos mentales y musculoesqueléticos concentran 18% de la carga. Los factores de riesgo que más afectan a los hombres son sobrepeso/obesidad; niveles de glucosa en sangre y de presión arterial elevados; y el consumo de alcohol y tabaco (35.6% de AVISA perdidos). En las mujeres, el sobrepeso y la obesidad; glucosa elevada; hipertensión arterial; baja actividad física; y el consumo de alcohol y tabaco fueron responsables de 40% de los AVISA perdidos; en ambos sexos, la dieta contribuye con 12% de la carga. Conclusiones. El panorama epidemiológico en México demanda una urgente adecuación y modernización del sistema de salud.

Una mujer sola y soltera sale del hospital con su recién nacido en brazos. Como carece de medios económicos lo deja en un lujoso coche con una nota pidiendo su cuidado y amor. Aunque huye desesperada pronto se arrepiente y vuelve, pero el coche ha sido robado. Un vagabundo y vendedor de vidrio pasea por un suburbio y encuentra al bebé. Después de varias peripecias decide acogerlo. Durante los cinco primeros años el chico crece en un hogar y establece un estrecho vínculo afectivo con su cuidador. Además, se hace su compañero, comparte la comida, la pobreza y colabora con curiosos trabajos para sobrevivir. Mientras, la madre del niño se convierte en una mujer rica porque es una célebre cantante de ópera. No obstante, está triste por no tener a su hijo e intenta recuperarlo. Con frecuencia ayuda a los niños pobres. El chico enferma y el vagabundo avisa al médico quien certifica el lugar insalubre donde vive el niño y avisa a la policía para trasladarlo a un albergue infantil. Finalmente, una serie de sucesos logra que madre e hijo se reencuentren.

Abstract:

Objetivos. Describir el proceso de priorización para el desarrollo de guías de práctica clínica (GPC) en el Seguro Social de Salud del Perú (EsSalud). Métodos. El proceso de priorización se realizó mediante un análisis multicriterio en tres pasos: identificar las variables, obtener información de las mismas y asignarles una ponderación. Finalmente, se obtuvo una lista ordenada que fue puesta a consideración de los decisores de política. Resultados. Las cinco enfermedades con mayor número de AVISA (años de vida saludable perdidos) fueron: artrosis, glaucoma, esquizofrenia, osteoporosis y fracturas patológicas, y diabetes mellitus. Las de mayor gasto en medicamentos fueron: hemofilia, el grupo de nefritis, nefrosis (principalmente insuficiencia renal crónica), artritis reumatoide, cáncer de mama y SIDA. Las enfermedades prioritarias desde el punto de vista de la oferta de servicios fueron aquellas con alta mortalidad y alta discapacidad. Conclusiones. Se realizó el primer proceso de priorización de GPC en el Perú. Las 5 primeras enfermedades priorizadas fueron: artrosis, diabetes mellitus, glaucoma, enfermedad renal crónica y lumbago.

A produção ficcional de João Paulo Borges Coelho representa hoje um importante contributo para o repensar das questões identitárias no espaço moçambicano, através da recuperação da memória coletiva, quer pré quer pós-independência. No presente ensaio, proceder-se-á à análise de Água. Uma novela rural (2016), na qual o autor retoma a representação dos rios como uma metáfora central na construção da narrativa, observando os modos de construção de uma reflexão pertinente sobre a construção das identidades, numa relação complexa entre tradição e modernidade, campo e cidade, nativo e estrangeiro, novo e velho. A situação de seca extrema que se vive na aldeia agudiza os conflitos sociais latentes, e a novela desenvolve-se num diálogo pontuado pela perplexidade e pelas contradições, à semelhança da própria água que, avisa o narrador logo no início da novela, não é de fiar. Escassa no início, torrencial depois, a água pontua a vida das gentes e assinala a chegada de um tempo em que as fronteiras se esboroam e todas as mestiçagens se fazem necessárias.

Abstract:

This essay maps out a constellation of early modern English feminine gatekeeper tropes that represent female sexual consent and imagine a gendered Cartesian dualism. This trope’s inherent mind–body divide grants the female subject’s mind a greater measure of rationality and autonomy from the body than other early modern discourses of feminine virtue, such as humoralism. However, it can also undercut feminine agency in self-regulation by placing all the responsibility and blame on the woman’s mind in cases of sexual harrassment and assault. Hadrian Dorrell’s Avisa, Shakespeare’s Lucrece, Thomas Heywood’s Jane Shore, and Christopher Marlowe’s Hero represent a spectrum of feminine mental complicity in extramarital sex, yet their mental “gatekeepers” are all suspected of failure. Shakespeare’s Juliet and Cressida literalize this gatekeeper trope and render it a material allegory when they negotiate with male suitors at literal portals on stage, a window and a chamber door. Examining the extraordinary pressures put on feminine “gatekeeper” minds in early modern texts allows us to discern contemporary willingness to blame the victims of sexual assault.

Abstract:

Esta investigación estimó la carga de enfermedad por tuberculosis en Pereira, en el periodo 2010-2015. Se incluyeron los pacientes del programa de tuberculosis. Se calcularon incidencias e indicadores de carga de enfermedad. Se utilizaron tarjetas de manejo, seguimiento y tratamiento, libros de pacientes, historias clínicas y registros de defunciones del DANE; el procesamiento se hizo con la plantilla de la Organización Mundial de la Salud, y en hojas de cálculo de Excel. En el periodo ingresaron al programa 1796 casos; Los AVPP (años de vida potencialmente perdidos), 3753,61. Comparando 2010 y 2015, los casos aumentaron 4,2 veces en hombres y 3,2 veces en mujeres. Los AVAD (años de vida ajustados por discapacidad) fueron mayores en hombres, el 67% entre los 30 y 69 años, la mayor diferencia entre sexos se dio en el 2015. Se perdieron 8711,2 AVISA (años de vida saludable perdidos). En conclusión, la carga de enfermedad por tuberculosis en Pereira es mayor por AVPP en hombres, en edades entre 15 y 69 años, y aumentó considerablemente entre 2010 y 2015.

Abstract:

Este trabajo informa el proceso y resultados iniciales de la automatización del test de diagnóstico médico de atención primaria en salud, estandarizado y reconocido a nivel mundial para la detección temprana del deterioro cognitivo en pacientes geriátricos denominado Test MiniMental. Es un cuestionario que evalúa la memoria, orientación, concentración y lenguaje; se basa en el otorgamiento de puntuaciones según las respuestas del paciente, luego las puntuaciones se promedian y se comparan con los parámetros estandarizados en el test determinando de esta manera el nivel de deterioro cognitivo que puede sugerir o no el inicio de patologías psiquiátricas como el Alzheimer. La aplicación creada permite seguir de forma secuencial y gráfica la evolución clínica del paciente; incluye también un componente de recordatorios que avisa al paciente la medicación a tomar según prescripción médica indicada. Para validar el trabajo se contó con la colaboración del médico ocupacional de la Universidad Técnica de Cotopaxi y la psicóloga clínica del Hogar de Ancianos Instituto Estupiñán de Latacunga – Ecuador, institución donde se aplicó la herramienta a 12 pacientes de 75 a 93 años, 5 varones y 7 mujeres, como resultado se identificó deterioro cognitivo grave en 4 pacientes, deterioro cognitivo entre discreto y moderado en 6 pacientes.

Abstract:

Grain legumes are nutritionally important components of smallholder farming systems in sub-Saharan Africa and Asia. Unfortunately, limited access to quality seed of improved varieties at affordable prices due to inadequate seed systems has reduced their contribution to improving nutrition and reducing poverty in these regions. This paper analyses four seed systems case studies: chickpea in Ethiopia and Myanmar; cowpea in Nigeria; and tropical grain legumes in Nigeria, Tanzania and Uganda highlighting outcomes, lessons learned, and the enabling factors which supported the successful innovations. All four case studies highlighted at least some of the following outcomes: increased adoption of improved varieties and area planted; increased productivity and income to farmers; improved market access and growth; and significant national economic benefits. Important lessons were learned including the value of small seed packets to reach many farmers; the value of innovative partnerships; capacity building of value chain actors; and continuity and coherence of funding through Tropical Legumes projects II and III and the recently funded Accelerated Varietal Improvement and Seed Delivery of Legumes and Cereals in Africa (AVISA) project. Successful adoption of innovations depends not just on the right technologies but also on the enabling environment. The case studies clearly showed that market demand was correctly identified, establishment of successful partners and institutional linkages overcame constraints in production and delivery of improved seed to smallholders, and fostered conducive policies supported national seed systems. All were integral to seed system viability and sustainability. It is hoped that these examples will provide potential models for future grain legume seed systems efforts. In addition, the analysis identified a number of areas that require further research.

O negócio ganhou um prêmio de empreendedorismo na Poli e bombou.</p><p>Depois de se formar, Ariel trabalhou por quatro anos no Google na Irlanda e aproveitou o que aprendeu para montar uma consultoria e ajudar as empresas a melhorar seu posicionamento no <em>site</em> de busca. Foi durante viagens que vieram as ideias para os seus negócios mais conhecidos, a 99 (criada com Renato e Paulo Veras) e a Yellow Bike (com Renato e Eduardo Musa). No primeiro caso, precisava pegar um táxi na Alemanha quando soube que existia um aplicativo para isso. No segundo, viajava para a China para negociar a venda da 99 quando conheceu empresas com um modelo de compartilhamento de bicicletas sem estações físicas.</p><p>Em seu cérebro, que não desliga, surgem inúmeras formas de expandir a Yellow, que iniciou suas atividades neste ano em São Paulo. Bicicletas elétricas, patinetes na versão sentada... Várias alternativas de mobilidade estão sendo estudadas. Unir ao transporte soluções para pagamentos e alimentação, por que não? Um usuário da Yellow poderia usar a carteira virtual do aplicativo para pagar o metrô. Depois de sair da estação e pegar a bicicleta, pararia no caminho do escritório para pegar uma entrega de comida. “Acho que dá para fazer com que todo esse sistema converse”, diz Ariel.</p><p>Nesta entrevista à<em> GV-executivo</em>, o empreendedor mostra como, além daquele clique, é preciso muito mais para um negócio vingar, como foco no usuário, conservadorismo na expansão e perseverança para enfrentar as incertezas e os reveses. “Se você está empreendendo só porque viu que a 99 deu muito certo e virou um unicórnio, não vai funcionar”, avisa.</p>

BackgroundThe AVISE Connective Tissue Disease (CTD) test uses autoantibody, erythrocyte-bound C4d (EC4d) and B-cell-bound C4d (BC4d) levels to aid in diagnoses of SLE, other CTDs and fibromyalgia. We evaluated the utility of the AVISE CTD test in predicting SLE disease development and damage progression.MethodsPatients who had undergone AVISE CTD testing were assessed for SLE diagnosis by the Systemic Lupus International Collaborating Clinics (SLICC) and American College of Rheumatology criteria and for SLE damage by the Systemic Lupus International Collaborating Clinics Damage Index (SDI) at the time of AVISE testing (t=0) and 2 years later (t=2).ResultsAmong 117 patients without a previous diagnosis of SLE, 65% of patients who tested positive developed SLE at t=2, compared with 10.3% of patients who tested non-positive (p<0.0001). AVISE-positive patients fulfilled significantly more SLICC diagnostic criteria than AVISE-non-positive patients at both t=0 (3.8±2.1 vs 1.9±1.1, p=0.001) and t=2 (4.5±2.2 vs 2.1±1.2, p<0.0001). AVISE-positive patients also had had significantly higher SDI at t=2 (1.9±1.3 vs 1.03±1.3, p=0.01). BC4d levels correlated with the number of SLICC criteria at t=0 (r=0.33, p<0.0001) and t=2 (r=0.34, p<0.0001), as well as SDI at t=0 (r=0.25, p=0.003) and t=2 (r=0.26, p=0.002).ConclusionsThe AVISE CTD test can aid in SLE evaluation by predicting SLE disease development and future damage progression.

Abstract. We use a recently developed Lagrangian transport tool, Lagrangian descriptors, to compare the transport properties of data distributed by AVISO and numerical simulations obtained from the HYCOM model in the Yucatán–Florida current system. Our data correspond to the months from June through August 2010. Structures obtained from HYCOM are noisier than those from AVISO; however, both AVISO and HYCOM succeed in identifying Lagrangian structures that influence the paths of drifters, such as eddies, currents, lobes, etc. We find evidence in which AVISO gives the positions of important hyperbolic trajectories in a manner that is inconsistent with the trajectories of the drifters, while for the same examples HYCOM succeeds to this end.

Purpose Precision active vibration isolation system (AVIS) is crucial for the mechanical processing equipment in the field of precision manufacturing. Working reliability and efficiency of the system directly influence operating condition of the equipment and the quality of work pieces. Design/methodology/approach A complete structure of the AVIS includes two parts: the excitation part and the passive vibration isolation system (PVIS). The excitation part consists of voice coil motors (VCMs). In this paper, the working process of AVIS is studied particularly via linear simplification on the decoupling model and the mechanical dynamic equations to solve the vibration problem, and they are validated by the experiments. Findings According to dynamic analysis and experiment on an AVIS on different reference points, the VCMs are used as actuators in the AVIS to excite the PVIS, and the performance characteristics of the whole AVIS is well reflected by the amplitude–frequency curves, the bode diagrams and the power spectral density curves. Originality/value This study has provided a way for obtaining the inner structure and working condition of the AVIS, which are essential to better control of the AVIS and to further study it in precision manufacturing application.

Abstract:

Purpose: a study of the current state and prospects of avia passenger transportation in international tourism as prerequisites for strategic planning of the tourism business. Methodology: collection, analysis, systematization and synthesis of the latest statistical data of the World Tourism Organization and the International Civil Aviation Organization on avia transportation in tourism. Results: data on the dynamics of avia transportation in tourism in the current century were obtained, the socio-economic role of avia transportation in tourism was clarified, and the structure of avia passenger transportation by tourist regions was determined. Scientific novelty: the current trends and prospects for the further use of avia transportation means in the tourist services market are identified. Practical significance: the results of the study can be used both in the educational process in the development of experts in tourism, and in the tactical and strategic planning of the tourism business.

Zusammenfassung: Für den Fragebogen zur Erfassung aggressiver Verhaltensweisen im Straßenverkehr (AViS) werden Befunde zur Konstrukt- und Kriterienvalidität (N = 841) berichtet. Die Studien zur Konstruktvalidität des AViS belegen eine theoretisch sinnvolle Einbettung verkehrsspezifischer Aggressionen in das untersuchte nomologische Netzwerk aus allgemeiner Aggression, psychologischer Reaktanz, Konfliktbewältigungsstrategien und generellen Persönlichkeitseigenschaften, wie sie mit dem EPP-D erhoben wurden. Zusammenhänge mit Indikatoren des Verkehrsverhaltens, bspw. zur Unfallbeteiligung, Verkehrsverstößen und Führerscheinentzug, belegen die Kriterienvalidität der Punktwerte des AViS. Dabei zeigte sich die Überlegenheit des AViS gegenüber allgemeinen Aggressionsmaßen.

Penelitian ini dilakukan dengan tujuan untuk mengetahui pengaruh produk, harga dan promosi secara parsial dan secara simultan terhadap keputusan pembelian rumah pada Green Avina Birem Puntong Langsa Baro. Penelitian ini menggunakan data primer yang bersumber dari 53 orang responden. Data dianalisis dengan menggunakan persamaan regresi linier berganda, koefisien determinasi (R2), uji t dan uji F. Hasil penelitian menunjukkan bahwa hasil uji koefisien determinasi (R2) diperoleh Ajusted R Square sebesar 42,3% variabel produk, promosi dan harga memberikan pengaruh terhadap keputusan pembelian rumah pada Green Avina di Birem Puntong Langsa Baro dan sisanya sebesar 57,7% dipengaruhi variabel lain yang tidak masuk dalam penelitian ini seperti lokasi dan fasilitas. Hasil uji t menunjukkan bahwa secara parsial variabel produk, harga dan promosi berpengaruh signifikan terhadap keputusan pembelian rumah pada Green Avina Birem Puntong Langsa Baro. Hasil uji F menunjukkan bahwa secara simultan variabel produk, harga dan promosi berpengaruh signifikan terhadap keputusan pembelian rumah pada Green Avina Birem Puntong Langsa Baro

Abstract:

Recent reforms in K-16 science education advocate for the integration of science content and practice. However, engaging students in authentic science practices can be particularly challenging for certain subjects such as evolution. We describe Avida-ED, a research-based platform for digital evolution that overcomes many of the challenges associated with using biological model organisms in the classroom. We then report the findings of a nationwide, multiple-case study on classroom implementation of Avida-ED and its influence on student understanding and acceptance of evolution. We found that engagement in lessons with Avida-ED both supported student learning of fundamental evolution concepts and was associated with an increase in student acceptance of evolution as evidence-based science. In addition, we found a significant, positive association between increased understanding and acceptance. We discuss the implications of supporting reform-based pedagogical practices with tools such as Avida-ED that integrate science content with authentic science practice.

A central poem in Van Wyk Louw’s Tristia (1962), is “H. Teresa van Avila flap uit” (literally translated: “Saint Teresa of Avila talks too much/babbles uncontrollably”. This article illustrates how intertextual reading helped to clarify the poem. Teresa of Avila’s The way of perfection (a translation of the Spanish work El Camino de la Perfección, 1573) is the intertext of the Van Wyk Louw poem. In the last section of the article it is shown how the figure of St. Teresa of Avila is central not only to Van Wyk Louw’s Tristia (1962), but also to the oeuvre of Sheila Cussons, which underlines a strong intertextuality between these two Afrikaans oeuvres.

Abstract:

Recent advances in the real-time simulation of electric machines are linked with the increase in the operation speed of the numerical models retaining the calculation accuracy. We propose utilizing the method of average voltages at the integration step (AVIS) for the design of a three-phase induction machine’s model in its natural abc reference frame. The method allows for avoiding rotational e.m.f. calculation at every step; in turn, the electromagnetic energy conversion is accounted by the change of flux-linkage. The model is integrated into the object-oriented environment in C++ for designing the computer models of electromechanical systems. The design of the model of an electromechanical system utilizing the proposed approach is explained in an example. The behavior of the numerical models of a three-phase IM has been compared for the set of conventional numerical methods as well as first- and second-order AVIS. It has been demonstrated that both first- and second-order AVIS methods are suitable tools for high-speed applications, namely, AVIS provides higher maximum possible integration step (e.g., first-order AVIS provides 4 times higher than the second-order Runge–Kutta method, and the second-order AVIS provides 2.5 times higher than the first-order method). Therefore, we consider the most preferable order of the AVIS method for the high-speed applications is the second order, while the first order may be a suitable alternative to increase the calculation speed by 30% with the acceptable decrease in the accuracy.

Abstract:

The study compared performance indicators of four major international rugby union competitions, namely Super Rugby, Aviva Premiership, Guinness Pro 12 and French Top 14. Most performance indicators of Aviva Premiership and Guinness Pro 12 rugby competitions were similar but distinctive from French Top 14 and Super Rugby competitions, which in turn were generally similar. The Aviva Premiership and Guinness Pro 12 had similar numbers of ball carries, tries, offloads, kicks, tackles, line-outs and scrums that were significantly higher than French Top 14 and Super Rugby. Offloads ( p = .0015) and tries ( p = .0368) were significantly higher for Super Rugby than the Guinness Pro 12, but the Guinness Pro 12 had significantly higher counts for carries ( p ≤ .0001), passes ( p = .0010) and kicks ( p = .0054). In contrast, counts of tackle breaks ( p ≤ .0001) and line breaks were significantly higher for Super Rugby in comparison to the Aviva Premiership ( p = .0057), Guinness Pro 12 ( p = .0064) and French Top 14 ( p ≤ .0001). All competitions showed areas of weakness and strength, which could be due to specific game plans followed in the unique pitch and weather conditions during competition.
