Full textIntroduction: Stroke is a serious public health problem in Portugal and the entire world. High levels of glucose in the blood are common in the early phase of stroke. The prevalence of hyperglycemia, defined as a level of blood glucose >6.0mmol/L (108mg/dl) was observed in more than 50% of patients, regardless of the subtype of stroke. The main aim of this work was to evaluate the relationship between blood glucose levels on admission and prognosis on discharge in patients with diagnosis of stroke. The secondary aim was to study the relationship between the prognoses of these patients with other variables. Methods: We conducted a retrospective study of patients admitted to the CHCB’s U-Stroke with the diagnosis of stroke. The study took place during the period from June 1, 2009 and May 30, 2010. To collect data (level of blood glucose, HbA1c, CRP, fibrinogen and D-dimmers measured on admission, the score of NIHSS on admission and also, the scores of Barthel Index and Rankin Scale on discharge), we proceeded to query clinical processes. Results: We studied 297 patients. The 31.3% were diabetic and the 73.7% had blood glucose levels above the values considered normal. Nineteen patients with hyperglycemia on admission died and 114 were dependent. However 2 patients without hyperglycemia on admission died and 34 were dependent. The relationship between hyperglycemia on admission with dependence and mortality was not statistically significant (p=0.71 and p=0.199, respectively). Conclusion: These results suggest that hyperglycemia on admission and Diabetes are a common finding in stroke patients. Patients with hyperglycemia on admission had a higher mortality and a worse prognosis (higher number of patients classified as dependent) compared with patients without hyperglycemia on admission.

Full textIn this thesis we study the use of money by a destitute category of the population in its cultural and social aspects. However, taking into account the diversity and complexity of the destitute part of the population we classified these individuals (persons or customers) in three categories : ones with great financial difficulty, those in less difficulty, and those on the verge of having financial difficulty. While giving the subject of money a relative autonomy, we stress the symbolic logic occurring in its use. To do this; after combining several methods in sociology and anthropology (in particular through participatory observation) we interpret the behavior of the destitute clients in their relations with +la poste; as a condensed picture of their view of the financial world. In the first part, it stands out that despite the compartmentalization of the means of payment and the partitioning of social life, each cultural group works out additions try to find a symbolic unit to represent its way of seeing and living in the world. In the second part, we show that the customers in trouble draw their answers from their cultural and social background in their relation to +la poste;. Their way of exchange with the financial institutions, beyond their ability to improvise is one +of telling; of doing; beliefs and actions which come from their lifetime experience and the handling of their individual and collective entity. In the third part, we show that even the sacred character of money plays the role of a social link. Indeed, the syncretic management to which these individuals devote themselves is a manifestation of their language, their relations in respect to time andpower ; in a word, of their system of reference. Depending on the cultural and social background, the rangr of possibilities allows, the authorized sincretic work or not. As a conclusion, we point out that the relation of customers in trouble with +la poste;, is more or less determined by the search for a social answer to an economic crisis, while the search for money is often the desire of and economic answer to social crisis

Full textArtificial Life researchers have, for decades, created a plethora of creatures using numerous encoding schemes, motile capabilities and cognitive capacities. One recurring pattern, however, is that focus is solely put on the evolved individuals, with very limited environmental variations. In this work, we argue that more complex abiotic constraints could drive an evolutionary process towards more robust and diverse regions of the genetic space. We started with a complex morphogenetic model, based on K. Sims’ directed graphs, which relied on the Bullet physics engine for accuracy and used 6Degrees of Freedom constraints to connect pairs of organs. We evolved a panel of natural-looking plants which had to cope with varying resource levels thanks to a mobile light source and seasonal rain patterns. In addition to this experiment, we also obtained improved vertical growth by adding an artificialbiotic constraint in the form of static grass blades. However, the computational cost of this model precluded scaling to a population-level evolution and was reduced in the successive experiment, notably by removing the physical engine. This led to the exploration of co-evolution on single-species populations which, thanks to our Bail-Out Crossover (BOC) algorithm, were able to self-reproduce. The resulting populations provided valuable insight into the mechanisms of self-sustainability. These were put to action in an even more straightforward morphogenetic model inspired by the work of Bornhofen. Due to its light weightness, this allowed for both larger populations (up to thousands of individuals) and longer evolutionary periods (100 years, roughly 5K generations). Our first experiment on this model tested whether text-book cases of speciation could be reproduced in our framework. Such positive results were observed thanks to the species monitoring capacities of APOGeT, a novel clustering procedure we designed for online extraction of species from a genealogic tree. This drove us to a final experiment in which the environment was controlled through Cartesian Genetic Programming thus allowing the automated evolution of both the population and abiotic constraints it is subjected to. Through a variation of the traditional1 + λ algorithm, we obtained 10 populations (evolved group e) which had endured in harsh and unpredictable environments. These were confronted to a control group c, in which the constraints were kept mild and constant, on two types of colonization evaluation. Results showed that the evolved group was heterogeneous with half of e consistently outperforming members of c and the other half exhibiting worse performances than the baseline. We also found a very strong positive correlation between catastrophic drops in population level during evolution with the robustness of their final representatives. From this work, two conclusions can be drawn. First, though the need to fight on both the abiotic and biotic fronts can lead to worse performances, more robust individuals can be found in reasonable time-frames. Second, the automated co-evolution of populations and their environments is essential in exploring counter-intuitive, yet fundamental, dynamics both in biological and artificial life

Full textThis thesis propounds and asseses an analytical model of service encounter. Most of the analyses of service encounter concentrate on face to face encounter, a dyadic meeting between customer and employee. At the opposite, in the self-service encounter, the customer is alone to deal with a serving machine. In that case, there is no human interaction and the comunication is digital. Our main argument concerns a third situation besides the face to face and the self service encounter : the triad encounter. In the triad, the employee uses a computer to deliver the service. He has to deal with his machine as well as the customer. The situation can be difficult to master for the service of the computer disturbs the interchange with the customer. The thesis has two parts. The first part deals with concepts and theories used to build the model. It includes three chapters. The second part is devoted to the empirical analysis. It develops the method of survey then presents the results in four chapters. Finaly, the conclusive remarks set the future prospects of face to face encounter, self service and triad encounter

Full textIn a highly competitive environment, one of the most concerns of companies is to fight effectively against customers volatility. More than ever, the development of customers long term relationships is regarded as a potential of resources for companies. The objective of this research is to better understand the factors that maintain the Business-consumer relationship. A literature review was conducted embracing works in economics, psychology and marketing. Then, a socio-economic mediating model of long term relationship maintenance was proposed. This model formalizes the relationship maintenance through two parallel ways, one economic and the other social. It considers, in particular, the influence of interaction components on the answers to specific dissatisfaction. This relationship is hypothesized to be mediated by the global evaluation of the relationship. The model was tested in an empirical study of 1687 french banking customers. Results show the major role played by the social way in the maintenance of long term relationship. They confirm also the moderating effect of the relationship duration, the frequency of internet use and customer involvement. Contributions of this research, on a theoretical level, are related to the enrichment of work on the long term relationship maintenance and, on a practical level, provide the banking leaders with key factors to manage their customers relationship

Full textThis thesis studies several extensions of multi-armed bandit problem, where a learner sequentially selects an action and obtain the reward of the action. Traditionally, the only information the learner acquire is about the obtained reward while information about other actions is hidden from the learner. This limited feedback can be restrictive in some applications like recommender systems, internet advertising, packet routing, etc. Usually, these problems come with structure, similarities between users or actions, additional observations, or any additional assumptions. Therefore, it is natural to incorporate these assumptions to the algorithms to improve their performance. This thesis focuses on multi-armed bandit problem with some underlying structure usually represented by a graph with actions as vertices. First, we study a problem where the graph captures similarities between actions; connected actions tend to grand similar rewards. Second, we study a problem where the learner observes rewards of all the neighbors of the selected action. We study these problems under several additional assumptions on rewards (stochastic, adversarial), side observations (adversarial, stochastic, noisy), actions (one node at the time, several nodes forming a combinatorial structure in the graph). The main contribution of this thesis is to design algorithms for previously mentioned problems together with theoretical and empirical guaranties. We also introduce several novel quantities, to capture the difficulty of some problems, like effective dimension and effective independence number

Full textIn this thesis, production systems facing abandonments are studied. These problems are modeled as stochastic scheduling problems with due dates. In the literature, few results exist concerning the optimal control of such systems. This thesis aims at providing optimal control policies for systems with impatience. We consider a generic system with a single machine, on which jobs have to be processed. Processing times, due dates (or patience time) and release dates are random variables. A weight is associated to each job and the objective is to minimize the expected weighted number of late jobs. In our study, we use different models, taking into account the specific features of real life problems. For example, we make a difference between impatience, when a customer has been waiting for too long, and abandonment, when a customer leaves the system after getting impatient. In the class of static list scheduling policies, we provide optimal schedules for problems with impatience. In the class of preemptive dynamic policies, we specify conditions under which a strict priority rule is optimal and we give a new heuristic, both extending previous results from the literature. We study variants and extensions of these problems, when several machines are available or when preemption is not authorized

Full textThis research analyses the relationship of Samuel Beckett (1906-1989) to dance and the relationship of two contemporary choreographers Dominique Dupuy (1930-) and Maguy Marin (1951-) to his works. The first part shows that Samuel Beckett was influenced by the modern dance he discovered between the two World Wars. His writing became more gestural and his discourse more political. Thus, to the healthy and glorious bodies promoted by the Nazi arts, he opposed in his first theatre the vulnerable bodies weakened by old age and disabilities, those of men whom the Third Reich wanted to make superfluous. In 1953, Lucky's dance in Waiting for Godot is a way of telling the unnamable of this violence. In 1981, the dance of Quad brings back the ghosts of the past to prevent the return of the worst. How to say? Dance, an art of the "non word", is part of this research that runs through Samuel Beckett's work. What one can't talk about is what one has to dance about. Secondly, this study shows how Samuel Beckett's works induce dances in which vulnerable beings resist and call for care. Dancing with Happy days, Act Without Words I and Worstward Ho allows Dominique Dupuy to question the possibilities of old age and transform its representations. Starting with Endgame, Maguy Marin wrote May B in 1981, a choreography miles apart from high-performance dance, which makes disabilities visible. Her work offers an updated reading of Samuel Beckett's work that takes into consideration the condition of disabled persons and allows us to envision the possibility of a more inclusive society. The last part shows that an ethical reading of Samuel Beckett's works and their choreographic projections can become the vector of inclusion practices and emancipation at school. This is the vocation of the Meeting Beckett project conducted in 2016-2017 in partnership with choreographer K Goldstein, with disabled pupils from an Ulis (Unité localisée pour l'inclusion scolaire) and 5th graders. Finally, we need to ask ourselves to what extent artistic practices can encourage new professional gestures at school and initiate an inclusive dynamic, capable of giving a place for everyone in society

Full textLocalSolver is a mathematical programming solver.Originally designed to solve large scale combinatorial optimization problems such as those found in the industry, it mainly relies on local search heuristics.This pragmatic solution approach, coupled with expressive nonlinear and set-based modeling techniques, has allowed LocalSolver to establish itself as a successful commercial solver.The purpose of this thesis is to implement a complementary dual approach for the computation of lower bounds within LocalSolver.The main stake is to qualify the solutions found by the solver and to potentially prove their optimality, thus allowing an early stop of the search.Furthermore, lower bounds have many other applications, such as the detection of inconsistent problems.This is useful in the development phase where modeling errors are frequent.We face three major challenges.First, the problems we address are generic and can be combinatorial, nonlinear or even nonsmooth.Then, the integration to an industrial software requires to produce reliable and high-quality code that can scale in time and memory.Finally, any reformulation need must be managed in-house, to allow LocalSolver's users to model their problems in the easiest way possible.The dual module provided to LocalSolver starts by reformulating the given optimization problem into a mixed-integer nonlinear program (MINLP).This program is stored under a standard form that facilitates the implementation of various techniques aiming at computing lower bounds.Examples of such techniques are the generation of convex relaxations, bound tightening techniques and presolve actions.These building blocks are then integrated into a partitioning scheme called the branch-and-reduce algorithm, and interact with the primal modules thanks to concurrent computing techniques.While this approach remains traditional, several choices and implementation features vary from the state of the art.The operators we support and the reformulation technique we use allow us to compute lower bounds on more problems than most global optimization solvers.These solvers also mainly use linear relaxations, whereas our goal is to show that nonlinear relaxations can be competitive.For this purpose, we implement a nonlinear solver dedicated to the computation of lower bounds to our convex relaxations.At last, we establish a duality result under bound constraints that allow us to improve the performance of our custom nonlinear solver.It is also exploited to certify the validity of the lower bounds computed by LocalSolver and to obtain robust inconsistency certificates

Full textThis thesis provides methods for constructing martingales with specified marginals. The first collection of methods proceeds by quantization. This means that a measure is approximated by another measure whose support consits in a finite number of points. We introduce a quantization method which preserves the convex order. The convex order is a partial order on the space of measures which compares measures in terms of their relative dispersion. This new quantization method has the advantage that if two measures admit a martingale transition, then the quantized measures admit a martingale transition as well. This is not the case for the commonly used quantization method, the L2 quantization. For the quantized measures we present several methods to construct martingale transitions. The first method proceeds by linear programming. The second method proceeds by constructing matrices with specified diagonal and spectrum. The third method uses the Chacon Walsh algorithm. In a second part this thesis presents a new solution to the Skorokhod embedding problem. In a third part, this thesis studies the construction of continuous time martingales with specified marginals. Constructions are given using the Brownian Sheet. Other constructions are given by modifying a method developed by Albin. Martingales constructed in such a way have a scaling property. Full textBackground: Given the role of physical activity (PA) in recovering and maintaining functional autonomy after a stroke, many studies have highlighted the need to develop approaches that encourage stroke patients to physical activity. Despite these recommendations, studies have shown limited increase in PA during stroke rehabilitation, leading to lower level of PA in stroke survivors compared to age-matched healthy subjects. PA in the post-stroke rehabilitation phase is far from optimal, with many recent studies reporting that stroke patients do not get enough physical activity. Objective: Characterize the evolution of walking activity in stroke patients and explore factors that influence regular walking. Methodology: A descriptive correlation study was conducted with two assessment times: first at admission in the study (time 1) and second at discharge functional rehabilitation in center (time 2). Data were collected from a sample of eight stroke patients admitted to Institut de réadaptation en déficience physique de Québec (IRDPQ). To meet the study’s first objective, Wilcoxon tests were carried out to determine how walking activity progressed during the intensive rehabilitation phase at the institute. To meet the second objective, Spearman correlations were used to determine the relationship between different variables (walking endurance, comfortable walking speed, balanced, cardiorespiratory endurance, functionnal independence, self-efficcacy) and the level of walking activity after a stroke (number of step/day and active time). Results/conclusion: Except for the number of steps and minutes of walking, all variables significantly improved during the intensive rehabilitation phase at the institute. Although no correlation was significant, age seems to be the factor most closely linked to minutes of walking (r=0.57 p=0.14) and number of steps (r=0.43 p=0.29) at time 1. At time 2, balance was most closely linked to the number of steps (r=0.66 p=0.08) and walking time (r=0.51 p=0.20). Considerable education and awareness-raising are still needed to encourage stroke survivors to be more active on a daily basis.

Full textMy thesis aims at providing an explanation for the Epicurean theory of the living being by accounting for the relationship body and soul entertain in psycho-biological phenomena. In other words, the aim of this thesis is to investigate in which manner and to what extent both the body and the soul are involved in vital phenomena such as sleep, breathing, digestion etc. To this purpose, sections 1 and 2 will deal more closely with Epicurus’ psychological doctrine, while sections 3 and 4 will integrate the discussion with Epicurus’ account of (psycho-)biological phenomena. Accordingly, section 1 will provide a reconstruction of Epicurus’ psychological doctrine through an analysis of Epicurus’ own writings (Letter to Herodotus; Book XXV of the Peri Physeos), Epicurean testimonies (including Lucretius but also Demetrius Lacon and Diogenes of OEnoanda) and anti-Epicureans accounts (Cicero, Plutarch, etc.). Section 2 will offer a dialectical explanation for the development of Epicurus’ psychology (as reconstructed in section 1) in its relationship with the previous philosophical background, Democritus and Aristotle specifically. Section 3 will consider the physio-biological theory from book IV of the De rerum natura and will acknowledge the links this entertains with Epicurus’ psychological theory. In this context, Epicurean explanations of psycho-biological phenomena will be compared with those offered by the atomist, Peripatetic and medical milieu. Lastly, section 4 will offer a comparison between Epicurus’ psycho-biological theories and those of Asclepiad of Bithynia and will examine their respective application in the context of explaining diseases

Full textThe stroke is the main cause of mortality and disability in Portugal, being the disability poor valorized mainly in the oldest people. Thus, the first chapter of this work is about the stroke and the aim of this study is to determinate if there are diferences in functional disability after the ischemic stroke by age and sex. For this end, different clinical patients profiles from a regional hospital (533 patients) were analyzed in order to perform a statistic analysis of the three scales: NIH scale stroke, Barthel índex and Rakin scale. Significant results demonstrate that after ischemic stroke the male gender in contrast with female gender, has less dificulties to perform the ADL (Activitie Daily Life), and therefore, this gender has less necessity of help from another people to survive of simple daily routin. This could be consequence of the fact that in women the stroke in generally occurs in older ages than in men. Another cause is the lack of estrogenic levels during and after menopause. In addition, there is not significant results in term of functional disability by age, however the results demonstrate that the oldest people (≥75 years) have a litle tendency to have more functional disability, probably due to the decrease of capabilitie to superate the damages caused by the stroke. In summary, in last of this capter it is possible to conclude that oldest females are the most affect by ischemic stroke in terms of functional cabilities. In the second and third chapters, it was described how the Communitarian Pharmacy and hospital pharmacy worlds work. It was possible to verify that even more people (patients, physians and nurses) came to the pharmaceutical to ask some advices about health problems, drugs, and medical devices.
