MR lymphangiography using dynamic contrast-enhanced images is useful in providing high quality images to diagnose many clinical conditions. However, this procedure cannot be used for traumatic patients or patients with severe side effect of using contrast media. Thus the only option available is to develop a protocol for lymphedema without contrast agents in order to reduce the contra indication of the procedure and to extract diagnostic information without contrast medium. The objectives of this study are to evaluate the role of MR- lymphographic for the assessment of lymphedema before and after lymph node self-transplantation. This study was conducted at IRCCS Policlinico San Matteo (PV), Pavia, Italy. A total of 17 patients were investigated for lymphedema evaluation due to primary or secondary lymphedema. All procedures were performed due to justified clinical conditions according to the ethical guidelines. All procedures were performed using two MRI machines: Siemens Magnetom Aera 1.5T and Philips ingenia 1.5 T. Early Lymphedema stages diagnosis represents great challenges. Non contrast MRL is used to diagnose accurately the lymphatic system disorder. From studies, the researchers have found non contrast MRL is as a promising methodology in the diagnosis of lymphatic system disorders with accuracy up to 90%. The study revealed that non-contrast MRL imaging technique can increase the accuracy of lymphedema diagnosis, improve disease prognostication, and provide a more robust marker of treatment response.

Abstract:
Severe traumatic injuries and surgical procedures like tumor resection often create peripheral nerve gaps, accounting for over 250,000 injuries in the US annually. The clinical "gold standard" for bridging peripheral nerve gaps is autografts, with which 40-50% of patients regain useful function. However, issues including their limited availability and collateral damage at the donor site limit the effectiveness and use of autografts. Therefore, it is critical to develop alternative bioengineered approaches that match or exceed autograft performance. With the use of guidance channels, the endogenous regeneration process spontaneously occurs when successful bridging of short gaps (< 10mm) occurs, but fails to occur in the bridging of longer gaps (≥15mm). Several bioengineered strategies are currently being explored to bridge these critical size nerve gaps. Other labs and ours have shown how filler materials that provide topographical cues within the nerve guides are able to enhance nerve growth and bridge critical length gaps in rats. However, the mechanism by which intra-luminal fillers enhance nerve regeneration has not been explored. The main goal of this dissertation was to explore the interplay between intra-luminal scaffolds and orchestrated events of provisional fibrin matrix formation, glial cell infiltration, ECM deposition and remodeling, and axonal infiltration - a sequence we term the 'regenerative' sequence. We hypothesized that the mechanism by which thin films with topographical cues enhance regeneration is by serving as physical 'organizing templates' for Schwann cell infiltration, Schwann cell orientation, extra-cellular matrix deposition/organization and axon infiltration. We demonstrate that aligned topographical cues mediate their effects to the neuronal cells through optimizing fibronectin adsorption in vitro. We also demonstrate that aligned electrospun thin films are able to enhance bridging of a critical length nerve gap in vivo by stabilizing the provisional matrix, creating a pro-inflammatory environment and influencing the maturation of the regenerating cable leading to faster functional recovery compared to smooth films and random fibers. This research will advance our understanding of the mechanisms of peripheral nerve regeneration, and help develops technologies that are likely to improve clinical outcomes after peripheral nerve injury.

Abstract:
INTRODUÇÃO: A criopreservação de tecido paratireóideo é empregada no tratamento cirúrgico do hiperparatireoidismo secundário nos pacientes com doença renal crônica. Entre a captação do tecido e a criopreservação, realizada em laboratório especializado, o tecido é preservado em solução para cultura de células a 4°C (solução para transporte). Não há dados que demonstrem por quanto tempo o tecido paratireóideo humano pode permanecer viável nesta solução, antes de ser criopreservado. Este estudo objetiva avaliar o período de tempo que o tecido da glândula paratireóide hiperplásica de humanos pode permanecer na solução para transporte, sem apresentar danos ultra-estruturais. MÉTODOS: Estudo prospectivo que incluiu 11 pacientes submetidos a paratireoidectomia total com autoimplante heterotópico e criopreservação de fragmentos de tecido paratireóideo. Parte do tecido destinado para exame anatomopatológico foi selecionado para preservação em solução para transporte. Foram definidos 5 períodos relacionados ao tempo de permanência dos fragmentos de paratireóide na solução para transporte. No tempo 1, o material foi fixado a fresco, sem contato com a solução para transporte, este tempo serviu para controle. No tempo 2, os fragmentos de tecido permaneceram imersos na solução para transporte por 2 horas, no tempo 3, este período foi de 6 horas, e os tempos 4 e 5, corresponderam a preservação dos fragmento de paratireóide na solução para transporte por 12 e 24 horas respectivamente. Ao final de cada período os fragmentos foram removidos da solução de transporte e fixados com glutaraldeído a 2%, seguido por preparo do material para cortes ultrafinos. A análise por microscopia eletrônica avaliou a adesão celular e a integridade das membranas plasmáticas, dos núcleos e das mitocôndrias, além da presença de edema celular e de vacúolos. RESULTADOS: Dos 11 casos estudados, 10 apresentaram achados ultraestruturais compatíveis com a normalidade nos fragmentos de tecido que permaneceram na solução para transporte por até 12 horas. Em apenas um destes casos, houve preservação das características morfológicas do tecido por 24 horas, na solução para transporte. Em um caso os achados caracterizaram sinais de dano celular irreversível em todos os períodos, inclusive no tempo inicial, em que o tecido foi fixado a fresco, sem contato com a solução para transporte. As alterações das mitocôndrias representaram os danos ultra-estruturais mais constantes nos casos estudados. CONCLUSÃO: A análise da ultra-estrutura do tecido da glândula paratireoide hiperplásica de humanos permite concluir que ocorre manutenção adequada da integridade estrutural do tecido que permanece na solução com meio de cultura de células a 4°C.até cerca de 12 horas após sua retirada do organismo, na maioria dos casos.
BACKGROUND: The cryopreservation of parathyroid tissue is employed in the surgical treatment of secondary hyperparathyroidism in patients with chronic kidney disease. During the period between surgical resection and cryopreservation of tissue, which requires a specialized laboratory, the tissue is stored in a cell culture solution, at 4 °C (solution for transport from the operating room to the laboratory). There is no data showing for how long the human parathyroid tissue can remain viable in this solution, before being cryopreserved. The present study evaluates the time that the tissue of human hyperplastic parathyroid gland could remain in solution for transportation, without showing ultrastructural damages. METHODS: This prospective study included 11 patients, who underwent total parathyroidectomy with heterotopic autotransplantation and cryopreservation of parathyroid tissue fragments. Part of the tissue intended for pathological examination was selected for storage at solution for transportation. Five periods were defined, related to the storage time of parathyroid fragments at solution for transportation. At time 1, the material was fixed at the time of surgical resection, without contact with the solution for transport, this time was used as control. At time 2, the fragments of tissue remained stored at the solution for transportation for 2 hours, at time 3, this period was 6 hours, and Times 4 and 5, corresponded to the parathyroid fragments stored in the transport solution for 12 and 24 hours, respectively. At the end of each period the fragments were removed from the transport solution and fixed with 2% glutaraldehyde, followed by preparation of material for ultrathin sections. The analysis by electron microscopy was used to evaluate cell adhesion and integrity of plasma membranes, nuclei and mitochondria, and the presence of edema and cell vacuoles. RESULTS: Of the 11 cases studied, 10 showed ultrastructural findings consistent with the normal tissue fragments that remained in the solution to transport up to 12 hours. In only one of these cases, there was preservation of the morphological characteristics of the tissue for 24 hours, at the solution for transportation. In one case, there were findings of marked signs of irreversible cell damage in all periods, including the initial time in which the tissue was fixed at the time of surgical resection, without contact with the solution for transportation. Changes of mitochondria represented the ultrastructural damage more constant in the cases studied. CONCLUSION: The analysis of the ultrastructure of human hyperplastic parathyroid gland tissue shows that, in most cases, ultrastructural integrity is properly maintained in fragments stored up to 12 hours in a solution of cell culture, at 4° C.

Abstract:
Em órgãos potencialmente importantes na resposta imune, como o baço, alternativas como o autoimplante de segmentos esplênicos, quando a esplenectomia total torna-se necessária, e a utilização de nutrientes com funcionalidade imunomoduladora vêm sendo estudadas, objetivando minimizar o efeito pró-inflamatório persistente da sepse abdominal. O objetivo deste trabalho foi avaliar o efeito do consumo de óleo de peixe na modulação da resposta inflamatória em animais submetidos a esplenectomia total isolada ou combinada com autoimplante esplênico e à indução de sepse abdominal, verificando a possível otimização na resposta pró-inflamatória e a regeneração funcional do autoimplante. Utilizamos 64 ratos machos da linhagem Wistar, com peso variando entre 140-200 g, aleatoriamente distribuídos em oito grupos: quatro grupos-controle (100% óleo de soja) e quatro grupos-intervenção (35% de óleo de peixe), cada um com oito animais. Os dos grupos-controle (animais alimentados com ração purificada, segundo AIN-93, com conteúdo lipídico constituído por 100% óleo de soja) foram: I sem intervenção cirúrgica e, 16 semanas após, submetidos à indução de sepse abdominal; II esplenectomia total isolada e, 16 semanas após, submetidos à indução de sepse abdominal; III esplenectomia total combinada com autoimplante esplênico e, 16 semanas após, submetidos à indução de sepse abdominal; e IV esplenectomia total combinada com autoimplante esplênico e, oito semanas após, submetidos à indução de sepse abdominal. Os dos grupos-intervenção (V a VIII) foram submetidos a procedimentos similares aos executados nos grupos I a IV, respectivamente, sendo a única modificação fundamentada na substituição de 35% do conteúdo lipídico da alimentação dos animais por óleo de peixe. Todos os animais foram submetidos a sepse induzida por ligadura e perfuração cecal (CLP). Coletamos amostras sanguíneas de todos os animais antes da indução da sepse (período 1) e 2 e 4 horas (períodos 2 e 3) após a indução da sepse abdominal. Verificou-se, a cada três dias, massa corporal (MC) e ingestão alimentar (IA). Analisamos as citocinas INF-γ, IL-6 e IL-10 por meio da tecnologia Luminex. Utilizamos o teste T de Student para análise estatística, considerando significativo com p≤0,05. Os dos grupos V, VI e VIII apresentaram maior consumo alimentar que seus controles. Os do grupo V apresentaram menores concentrações de IFN-γ em todos os períodos e maior IL-10 nos períodos 2 e 3. Os do grupo VI apresentaram menores concentrações de todas as citocinas: IFN-γ nos períodos 2 e 3; IL-6 nos períodos 1 e 2; e maior IL-10 nos períodos 1 e 2. Os do grupo VIII apresentaram menor IFN-γ no período 3, IL-6 no período 2, e maior IL-10 no período 1. Não observou-se diferenças nos do grupo VII em nenhuma das citocinas estudadas. Este estudo demonstrou que a utilização do óleo de peixe em pequena dose, consumido cronicamente, como parte do teor lipídico total da dieta e não de forma suplementar, é capaz de manter a massa corporal adequada e reduzir a resposta inflamatória à sepse abdominal induzida por CLP, aumentando a IL-10 plasmática em ratos que não sofreram intervenção cirúrgica, e parece favorecer a regeneração funcional precoce do autoimplante esplênico.
In organs potentially important in the immune response, like the spleen, alternatives such as autotransplantation of splenic segments, when the total splenectomy becomes necessary, and the use of nutrients with immunomodulatory function have been studied, trying to minimize the effect of pro-inflammatory persistent abdominal sepsis. The aim of this study was to evaluate the effect of consumption of fish oil in modulating the inflammatory response in animals submitted to total splenectomy alone or combined with spleen autotransplantation and the induction of sepsis, verifying the possible optimization in the proinflammatory response and the functional regeneration of the autotransplant. We used 64 male Wistar rats, weighing between 140-200 g, were randomly distributed into eight groups: four control-groups (100% soybean oil) and four intervention-groups (35% fish oil), each one with ten animals. The rats in control groups (animals fed with purified according to the AIN-93 with lipid content consisting of 100% soybean oil) : I without surgical intervention, and 16 weeks after, submitted to the induction of abdominal sepsis II total spleenectomy alone, and 16 weeks after, submitted to the induction of abdominal sepsis; III total splenectomy combined with spleen autotransplantation, and 16 weeks after, submitted to the induction of abdominal sepsis; and IV total splenectomy combined with spleen autotransplantation, and 8 weeks after, submitted to the induction of abdominal sepsis. The rats in intervention groups (V to VIII) were subjected to similar procedures performed in groups I to IV, respectively, being the only modification based on the substitution of 35% of the lipid content of animal feed for fish oil. All animals were subjected to sepsis induced by cecal ligation and puncture (CLP). We collected blood samples from all animals before the induction of sepsis (period 1) and 2 and 4 hours (periods 2 and 3) after the induction of abdominal sepsis. We checked every 3 days corporal mass (MC) and alimentary ingestion (AI). We analyzed the cytokines IFN-γ, IL-6 and IL-10 by Luminex technology. We used the T-test of Student for statistical analysis, considering significant with a p ≤ 0.05. The animals in groups V, VI, and VIII showed higher food intake than their controls. The animals in group V showed lower IFN-γ in all, IL-6 in periods 1 and 2; and higher IL-10 in periods 2 e 3. The animals in group VI showed lower concentration of all cytokines: IFN-γ in periods 2 and 3; IL-6 in periods 1 and 2 and higher IL-10 in periods 1 e 2. The animals in group VIII showed lower IFN-γ in period 3, IL-6 in period 2, and higher IL-10 in period 1. No differences were observed with the concentration of cytokines in those of group VII. This study demonstrated that the use of fish oil in small doses, chronically consumed as part of total dietary fat and not as a supplement, is able to maintain the adequate corporal mass and to reduce the inflammatory response to abdominal sepsis induced by CLP, increasing IL-10 concentration in health rats and seems to promote early functional regeneration of the spleen autotransplants.

En appliquant ces principes, nous souhaitons créer un "espace parodontal" dans lequel pourront migrer les cellules progénitrices issues du ligament desmodontal et créer un environnement biologique favorable. La membrane assure la contention et la stabilisation du transplant tout en autorisant la stimulation fonctionnelle. Elle permet une protection du coagulum, du sac folliculaire et des cellules desmodontales. Elle maintient à distance l'épithélium pour permettre l'expression sélective des cellules du ligament alvéolodentaire qui facilitera la réfection du procès alvéolaire et l'édification radiculaire physiologique du transplant. Le but de ce travail est d'expérimenter cette nouvelle approche thérapeutique. Six autotraansplantations de prémolaires avec membranes Résolut* et six autotransplantations de prémolaires sans membrane sont réalisées chez le porc "Large White". Les résultats histologiques obtenus ont été influencés par le choix de l'espèce animale. Avec ce modèle, la cicatrisation des transplants a été compliquée et altérée. Le potentiel de réponse thérapeutique des membranes a été insuffisant sans remettre toutefois en cause le principe thérapeutique de cette nouvelle technique

Dental implant and autotransplantation of tooth are the only two practical ways to increase the number of abutment tooth over the edentulous region. High successful rate and friendly technical procedures are the well known advantage of dental implant. However, high price and long healing period are its major problems. Comparing to dental implant, autotransplantaion is cheaper and has shorter healing period (about 3 weeks), there are still many clinical limitations of autotransplantation. Besides, high technical sensitivity is the another problem of it. In order to overcome the limitation of autotransplantation, the concept of tissue engineering has been proposed. In this study, we developed a transplanted the single-root-teeth of beagle dog translaterally as the animal model. The enamel matrix derivative (Emdogain®) was applied onto the root surface and the recipient socket before the teeth were transplanted. Consequently, the junction of the tooth and socket were covered with the biodegradable membrane (Atrisorb®). No special treatment had been done in the control group. After 4 and 8 weeks, the dogs were sacrificed. The teeth and their surrounding periodontal tissues were prepared for histological observation. All teeth were clinical success after transplantation by showing normal mobility, no periodontal pocket and no metallic sound. Root resorption, however, was noted in every root from both control and test group under microscope examination. The control group was obviously more serious. Comparing between the control and test group, the benefit of application of tissue engineering are summarized as following: (1) less root resorption, (2) increased the rate of healing process and production of PDL, (3)increased the rate of remodeling of cementum and alveolar bone,(4) Sharpey’s fibers formation noted in the most root surface in test group, (5)increase new attachment formation and (6)more bone support

Objetivos: O objetivo desta investigação foi avaliar as evidências científicas atualmente disponíveis sobre o autotransplante dentário. Materiais e métodos: realizaram-se pesquisas electrónicas na Pubmed, LILACS e JEBDP, para identificar os ensaios clínicos randomizados controlados e prospectivos. Uma seleção manual foi executada de acordo com o nível de evidência de cada artigo sobre o tema escolhido nesta dissertação. Tema principal: Em Medicina Dentaria, nas opções de tratamentos que podem ser propostas a um paciente para substituir um dente perdido, o auto transplante dentário é muitas vezes esquecido em favor da prótese fixa ou removível, ou do implante. Esta técnica tem muitas vantagens para o paciente. Torna-se assim numa solução economicamente e psicologicamente mais aceitável, com duas grandes vantagens: adaptar-se ao crescimento do paciente e uma osteointegração quase comparável ao implante.
Objectives : The aim of this investigation was to assess the currently available evidence concerning tooth auto transplantation. Materials and methods : Electronic searches were conducted on Pubmed, LILACS and JEBDP, to identify randomized controlled and prospective clinical trials. After that, manual selection was operated in relation to the evidence level of every article about the topic of this dissertation. Principal topic : In the panel of solutions which can be propose to a patient to replace a missing tooth in dentistry, dental transplant is a lot of time forgotten in favor of fixed or removable prosthesis, or implant. Nonetheless, this technic have a lot of advantages for the patient : this an economical and psychological attractive solution with two majors advantages : adapt to the growing patient and osteointegration comparable to implant.

Трансплантація органів та інших анатомічних матеріалів людини та правове регулювання в цій сфері – є одними із головних показників, за якими відслідковується рівень розвитку медицини в загалом в кожній країні, тому це і зумовлює актуальність обраної теми та проведення порівняльного аналізу сутності та правового регулювання трансплантації як в нашій державі, так і в країнах ЄС та вироблення на основі цього рекомендацій та пропозицій щодо вдосконалення українського законодавства. Метою кваліфікаційної роботи є визначення поняття, сутності, проблем в сфері трансплантології України та комплексному порівняльному аналізі трансплантації органів та інших анатомічних матеріалів людини в України та країнах ЄС. Об’єктом дослідження даної кваліфікаційної роботи є суспільні відносини та їх правове регулювання в сфері трансплантації органів та інших анатомічних матеріалів людини. Предметом дослідження виступають конституційні основи трансплантації органів та інших анатомічних матеріалів людини. Методологічну основу кваліфікаційної роботи складають сукупність принципів, підходів та методів пізнання. Для проведення дослідження в роботі застосовуються такі методи як: метод критичного мислення, порівняльно-правовий метод, метод співставлення та співвідношення, метод структурного аналізу тощо.
EN : Qualifying work consists of 100 pages, contains 71 sources of used information. Today in the world transplantation of organs and other anatomic materials of man is examined as a non-alternative method of treatment of irreversible diseases and damages of vital and became widespread enough as medical service that helps to save greater part of patients and recover the functions of organism of sick man. In present time the question of transplantation of organs in Ukraine, regulation of this sphere and financing of development of the system of medical service are distant from perfection. From it greater part of Ukrainians annually must drive out foreign country for the receipt of such medical service. In activity of the establishments of health care, related to transplantation and to activity that related to transplantation an important place occupies a procedural aspect - terms, order, features and limitations. Transplantation of organs and other anatomic materials of man and legal adjusting in this sphere are one of main indexes on which the level of development of medicine is watched in general in every country, therefore it stipulates actuality of the chosen theme and realization of comparative analysis of essence and legal adjusting of transplantation as in our country so in European Union and making on the basis of it recommendations and suggestions concerning the improvement of the Ukrainian legislation. The aim of qualifying work is determination of concept, essence, problems in the field of transplanting in Ukraine and complex comparative analysis of transplantation of organs and other anatomic materials of man in Ukraine and European Union. The object of research of this qualifying work are public relations and their legal adjusting in the field of transplantation of organs and other anatomic materials of man. Constitutional bases of transplantation of organs and other anatomic materials of man come forward the article of research. Methodological basis of qualifying work is made by totality of principles, approaches and methods of study. For realization of research different methods were used, such as: method of the critical thinking, comparatively-legal method, method of comparison, method of structural analysis, etc.
