Abstract:
The effects of gastric autonomic afferent reflexes on cardiovascular autonomic efferent activity are regarded as a direct neural effect of the activation of gastric receptors which send afferent inputs to the central nervous system (CNS) to cause various cardiovascular changes (Van Orshoven et al., 2004; McHugh et al., 2010; Girona et al., 2014). However, the cardiovascular responses to liquid ingestion in humans may be related to gastric distension, volume loading effects, or to its osmotic proprieties. The purpose of this study was to investigate cross autonomic reflex function and to elucidate the effects of the resulting cardiac efferent autonomic activity in resting young healthy subjects. The ingestion of 300 mL of isothermic water increased both the cardiac vagal tone as indicated by increased RMSSD (mean 23.95 ± 20.50 msec increase, p<0.05) and sympathetic activity shown by increased QTc interval (mean 9.86 ± 8.59 msec increase, p< 0.05) during the first 40 minutes post-ingestion. These effects were absent with an identical volume of a physiological (0.9% w/v) saline solution which would increase plasma volume more, indicating that the cardiovascular responses to water drinking are influenced by its hypo-osmotic properties, rather than being related to the volume loading effects. Nevertheless, subjects responded to gastric distension with an ingestion of 300 mL of Fybogel solution with an increase in sympathetic activity during the first 20 minutes post-ingestion, but not in cardiac vagal tone. These results indicate that the mechanisms underlying the cardiovascular responses to water ingestion have additional components to the gastric stretch effect. Contrarily, the cold mediated sympathetic inhibition after drinking the same volume of either cold water or cold Fybogel solution probably happened in the NTS where the two branches of the ANS meet for the first time during their central pathway (Kubin et al., 2006; Thayer and Lane, 2009). In conclusion, the cardiovascular responses to water drinking are influenced by its hypo-osmolality properties and temperature, not by the volume loading effects.

Abstract:
Recent reports suggest that human immunodeficiency virus (HIV), the virus causing AIDS, may cause autonomic nervous system (ANS) dysfunction. ANS abnormalities on cardiovascular reflex tests have been demonstrated in HIV+ persons, persons infected with HIV, who have signs of illness or have used intravenous drugs. In this study the cardiovascular reflex function of 11 HIV+ homosexual or bisexual males meeting the Centers for Disease Control criteria for absence of illness was compared to that of 11 uninfected homosexual or bisexual males of similar ages. Somatic, depression and fatigue differences between groups were assessed using an ANS symptom checklist, the Beck Depression Inventory (BDI) and the Profile of Mood States (POMS). Six of the 11 HIV+ subjects were impaired on the cardiovascular reflex tests. Differences on the BDI and POMS were not attributable to a depressive mood or despair, but rather to presence of mild symptoms of HIV infection and fatigue.

Abstract:
Heart Rate Variability describes the beat-to-beat variation in heart rate arising from activity of the sympathetic and parasympathetic branches of the Autonomic Nervous System (ANS). Reduced ANS tone measured by reduced heart rate variability (HRV) is a powerful predictor of adverse diagnosis in patients and is associated with increased mortality. Published research suggests that inflammation has a deleterious effect on Autonomic Nervous System tone. This study aimed to: establish if mild inflammatory conditions are associated with changes in autonomic tone as defined by heart rate variability studies in the following conditions: a. Influenza vaccination b. Reduction in oesophageal inflammation c. Reduction in weight The aim of the first study was to assess the link between inflammation resulting from the influenza vaccination and the associated changes on heart rate variability. 71 healthy volunteers opting to have a routine influenza vaccination were investigated for potential changes in cardiovascular autonomic tone associated with the temporary inflammatory effects of an Influenza vaccination. A number of temporal and frequency domain parameters of heart rate and breathing were assessed 2-5 days prior to vaccination and 1-4 days post vaccination. A sub-group of 15 volunteers who reported significant symptomatic reaction to the vaccination for at least 24 hours following vaccination displayed a statistically significant (p=<0.02) reduction in five of the six HRV parameters obtained during metronome-guided breathing. There was no evidence of significant reduction in autonomic tone following vaccination in the full sample of 71 volunteers. The aim of the second study was to establish whether inflammation resulting from erosive or non-erosive oesophagitis caused by gastro-oesophageal reflux disease had any association with changes in heart rate variability. 12 volunteers with non-erosive oesophageal reflux disease (NERD) and 8 with erosive oesophageal reflux disease (ERD) were investigated for HRV after initial diagnosis under gastroscopy. HRV assessment was repeated following 8 weeks of treatment with a proton-pump inhibitor (PPI). Initial reflux symptoms and response to PPI treatment were assessed using the GERD Impact Scale questionnaire. All participants had effective symptom response to treatment and there was no significant difference insymptoms score between NERD and ERD groups. There was a small but statistically significant increase in HRV detected following PPI treatment in the ERD group (p=0.05). The aim of the third study was to assess the link between obesity / pro-inflammatory adiposity, weight loss and the associated changes in heart rate variability. 38 clinically obese volunteers (BMI 30-39) with a family history of diabetes were reviewed for HRV prior to and following a lifestyle intervention designed to reduce body weight and BMI. Volunteers underwent repeated HRV studies after 4 months and 8 months of treatment. Volunteers on average achieved a weight loss of 11.5% (±6.0). There were statistically significant changes in HRV parameters in sub-group A (BMI ≥36) and correlation of biochemical measures with weight loss. These results further elucidate the effect of mild inflammatory triggers on autonomic tone as measured by HRV. These effects and their significance are discussed in detail in this document. The significance of the ‘cholinergic anti-inflammatory pathway’ is discussed with respect to the inflammatory conditions investigated. Suggestions for further work are proposed. In conclusion it is entirely possible to measure subtle changes in heart rate variability associated with mild inflammation and that on the evidence presented here these changes in heart rate variability are hypothesised to be reversible. My original contribution to knowledge is: 1. Changes in heart rate variability are associated with low grade inflammation resulting from the Influenza vaccination, erosive oesophagitis and increased adiposity. 2. Measurement of subtle changes in autonomic tone, associated with inflammatory challenges is possible and concurs with other published research. 3. The level of HRV attenuation does appear to be linked to those with a higher level of inflammation. In each study the most significant results came from subgroups of volunteers either demonstrating: a higher level of symptom severity, erosive oesophagitis or were in a subgroup of participants with the highest BMI / adipose tissue. 4. In the early stages of reduced heart rate variability we see that concurrent reduction in inflammation is associated with an increase in autonomic tone.

Abstract:
Acute inhalation of airborne pollutants alters cardiovascular function and has been shown to have its greatest affects on individuals with pre-existing cardiovascular disease. Evidence suggests that pollutant-induced activation of airway sensory nerves via the gating of ion channels is critical to these systemic responses. Here, we have investigated the cardiovascular responses evoked by inhalation of AITC (TRPA1 agonist) and capsaicin (TRPV1 agonist) in healthy Sprague Dawley (SD) and Wistar Kyoto (WKY) rats, and cardiovascular diseased Spontaneously Hypertensive (SH) rats. Inhalation of the agonists by healthy SD and WKY rats caused significant bradycardia, atrio-ventricular (AV) block and prolonged PR-Intervals. Inhalation of TRP agonists caused differential cardiovascular responses in the cardiovascular diseased SH rats, such that the TRP agonists evoked brady-tachy with AV block and premature ventricular contractions (PVCs). Bradycardic responses to AITC were inhibited by the TRP channel blocker ruthenium red and the muscarinic antagonist atropine, but atropine did not prevent the tachycardic responses seen in the SH rats. Adrenergic inhibition with atenolol prevented the tachycardic responses, but did not prevent the bradycardic responses evoked by AITC in the SH rats. In healthy rats, AITC inhalation also caused a biphasic blood pressure response: a brief hypertensive phase followed by a hypotensive phase, while evoking hypertension in the SH rats. Atropine accentuated the hypertensive phase in all animals, while preventing the hypotension in the healthy animals. In all animals, AITC-evoked heart rate responses were not abolished by terazosin, the [U+F061]1 adrenoceptor inhibitor, which prevented the hypertensive responses. Anesthetics had profound effects on AITC-evoked bradycardia and AV block, which was abolished by urethane, ketamine and isoflurane. Nevertheless, AITC inhalation caused bradycardia and AV block in paralyzed and ventilated rats following pre-collicular decerebration. In conclusion, we provide evidence that activation of TRP channels expressed on nociceptive airway sensory nerves causes significant cardiovascular effects in healthy rats via reflex modulation of the autonomic nervous system (ANS), and that these effects are exacerbated in cardiovascular diseased rats.

O SUS necessita do envolvimento dos usuários nas ações de controle social com vistas a minimizar as fragilidades do sistema. Trabalhadores e gestores partilham da responsabilidade de fortalecer esses sujeitos para o exercício de seus direitos em saúde. Assim, este estudo teve como objetivo construir um espaço de ação-reflexãoação com os usuários da rede básica, que permitisse refletir sua condição de autonomia e participação no controle social. Desenvolveu-se um estudo de caráter qualitativo, apoiada nos preceitos metodológicos de Paulo Freire, fundamentados na percepção crítica da realidade. Os sujeitos envolvidos foram usuários atendidos por uma Estratégia de Saúde da Família do Bairro Subuski na Cidade de Santo Ângelo, interior do Estado do Rio Grande do Sul. Os participantes foram sete usuários do SUS com idades variando entre 17 e 72 anos. O Círculo de Cultura foi desenvolvido entre os meses de fevereiro a abril de 2010. Esta pesquisa foi submetida ao Comitê de Ética em Pesquisa na Área da Saúde da Universidade Federal do Rio Grande-(CEPAS – FURG), sob o parecer n° 129/2009, permitindo contemplar todos os preceitos éticos que regem a pesquisa com seres humanos. A análise e a interrelação dos dados ocorreu a partir das falas emergidas durante os encontros registrados em áudio e transcritos, emergindo duas categorias: Direitos do usuário: existência ou negação e Participação dos usuários no controle social: possibilidades e impossibilidades. Os usuários expressaram angústias relacionadas ao desrespeito aos seus direitos, afirmando que a falta de informação e de comprometimento ético por parte de trabalhadores de saúde e da gestão local os torna vulneráveis. Em relação a sua participação no controle social, evidenciaram possibilidades e impossibilidades relacionadas à sua falta de credibilidade no sistema político e na gestão local, contribuindo para a fragilidade dos usuários e para a falta de acesso efetivo a seus direitos e à participação democrática. Foi possível evidenciar a necessidade de investir na aplicação de estratégias envolvendo a contribuição de gestores e trabalhadores da saúde na valorização da autonomia dos usuários. Assim, perceber estes sujeitos como co-participes para a melhoria da qualidade do setor, bem como fomentar o exercício da cidadania e da justiça social precisam ser impulsionados de forma a integrar ambientes de atuação e formação profissional, competindo à profissão de enfermagem assumir sua responsabilidade na promoção da saúde e no protagonismo social.
SUS needs the commitment of its users in the actions of social control with the aim to minimize the fragilities of the system. Workers and management share the responsibility of strengthening these individuals to exercise their health rights. The present study aimed to build a space of action-thought-action with the users of public health, which allowed them to reflect their status of autonomy and participation in social control. A study of qualitative feature was developed supported in the methodological concepts of Paulo Freire, reasoned in the critic perception of reality. The individuals involved were users served by a ESF in the Subuski neighbourhood, city of Santo Angelo, in the countryside of the state of Rio Grande do Sul. There were seven participants of the SUS aged between 17 and 72 years. The Culture Circle was developed between the months of February and April 2010, shortly after the approval of research by the Committee of Ethics Research in Heathcare under the no. 129/2009, allowing the contemplation of all ethical principles governing research with humans. Analysis and interrelationships of the data was based on the contents and meanings emerged during the meetings, which were recorded in audio and transcribed, enabling their understanding and grouping according to their affinities. From these analyses, two categories emerged: user’s rights, existence or denial; and the user’s participation in social control: possibilities and impossibilities. In relation to the perception of their rights, the users expressed fears related to the failure committed by health workers, claiming that the lack of information and ethical commitment of the workers and management makes them vulnerable. Regarding their participation in the social control, their perception that highlight the possibilities and impossibilities are related to the lack of credibility in the political system and in local management, which enables the fragility of these individuals with no guarantee concerning the effective access to their rights and to the democratic participation. The study shows the need to invest in the application of strategies involving the contribution of managers and healthcare workers in the recovery of users’ autonomy. Considering these individuals as co-participants in order to improve the quality of the sector as well as foster the exercise of citizenship and social justice needs to be improved so as to integrate environments of work and vocational training, giving the nursing profession the chance to take responsibility in the promotion of health and social role.
EL SUS necesita del envolvimiento de los usuarios en las acciones de control social con vistas a minimizar las fragilidades del sistema. Trabajadores y administradores dividen de la responsabilidad de fortalecer estos sujetos para el ejercicio de sus derechos en salud. Este estudio tuvo como objetivo construir un espacio de acciónreflexión-acción con los usuarios de la red básica, que permitiese reflejar su condición de autonomía y participación en el control social. Se desarrolló un estudio de carácter cualitativo, apoyada en los preceptos metodológicos de Paulo Freire, fundamentados en la percepción crítica de la realidad. Los sujetos envueltos fueron usuarios atendidos por una ESF del Barrio Subuski en la Ciudad de Santo Ângelo, interior del Estado de Rio Grande do Sul. Los participantes fueron siete usuarios del SUS con edades variando entre 17 y 72 años. El Círculo de Cultura fue desarrollado entre los meses de febrero a abril de 2010, justo después de la aprobación de la investigación por el Comité de ética en Investigación en el Área de la Salud bajo el parecer n° 129/2009, permitiendo contemplar todos los preceptos éticos que rigen la investigación con seres humanos. El análisis y las interrelaciones de los dados se dieron a partir del contenido y sus significados emergidos durante los encuentros registrados en audio y transcritos, viabilizando su entendimiento y agrupación conforme sus afinidades. De estos análisis emergieron dos categorías: derechos del usuario: existencia o negación y participación de los usuarios en el control social: posibilidades e imposibilidades. Acerca de la percepción de sus derechos, los usuarios expresaban angustias relacionadas a la falta de respeto de estos por parte de los trabajadores de la salud, afirmando que la falta de información y de compromiso ético por parte de trabajadores y de la gestión local los vuelve vulnerables. En relación a su participación en el control social, sus percepciones que evidencian posibilidades e imposibilidades se relacionan a la falta de credibilidad en el sistema político y en la gestión local, viabilizando la fragilidad de estos sujetos, sin garantizarles acceso efectivo a sus derechos y la participación democrática. Fue posible evidenciar la necesidad de invertir en la aplicación de estrategias envolviendo la contribución de administradores y trabajadores de la salud en la valorización de la autonomía de los usuarios. Percibir estos sujetos como coparticipes para la mejoría de la calidad del sector, bien como fomentar el ejercicio de la ciudadanía y de la justicia social necesita ser impulsado de forma a integrar ambientes de actuación y formación profesional, compitiendo a la profesión de enfermería asumir su responsabilidad en la promoción de la salud y en el protagonismo social.

The changes that took place in the last decades in Brazil impact all sectors of the society. Those reformations act on all the systems, reverberating in significative changes on the work content. The autonomy is prioritized on the profile of the worker. Education as a social phenomenon couldn t stay out of this revolution. The aforementioned changes on educational politics ended as an intensification of the teacher s work, new work demands and, consequently, a higher fret and dissatisfaction from the workers. The former research is based on a analysis about the teachers engagement in the process of conception and development of autonomy in the school. So, we had as an objective the analysis of the professional autonomy degree related to the working conditions of the teachers in the public education of the city of Mossoró (RN), correlating those to the IDEB (Basic Education Development Index). This is a descriptive, analytical and exploratory study, based in a quantitative and qualitative methodology. To materialize the quantitative procedure, we applied the socialdemographic questionary, the scale of teachers professional autonomy and the scale of teachers occupational stressors, with a sample of 404 teachers. On the qualitative procedure we used a pseudo structured interview with 7 teachers of different schools, through which we wanted to know their perceptions about autonomy and their working conditions. The obtained results point that the autonomy of the teacher depends on the offered working conditions, as the autonomy general index correlates positively with the occupational stressors general index (r = 0,221; p< 0,01). It was identified that the schools with best IDEB are judged by the teachers with the best working conditions to develop their activities (r = 0,177; p< 0,01), and those teachers fell like they have more autonomy (r = 0,304; p < 0,01). Factors such as relationship with the students and parents, the overcharge of work specially in function of the actual moment in education due to the goals to be achieved are triggers of the stress. The teachers discourse makes it evident that there are possibilities and limits to develop their autonomy, the class is the space where teachers feel they re more autonomous. The participation in councils or organizations is pointed as a means of resistance to the impositions of the scholar setting. The study contributed to give consistency to the idea expressed on the official document that there is autonomy, on the teacher, however the schools reveal the loss of some space for reflection, discussion and construction of an autonomy, alerting about the unfavorable working conditions offered to the teachers, therefore affecting the teachers performance. As mudanças ocorridas nas últimas décadas no Brasil têm refletido em todos os setores da sociedade. São reformas que atuam em todos os sistemas, repercutindo em mudanças significativas no conteúdo do trabalho. A autonomia é priorizada no perfil do trabalhador. A educação como um fenômeno social não ficou isenta dessa revolução. Tais mudanças significativas nas políticas educacionais tem resultado em intensificação do trabalho docente, novas demandas de trabalho e, consequentemente, em maiores desgastes e insatisfação por parte desses trabalhadores. A presente pesquisa baseia-se numa análise sobre o envolvimento dos professores no processo de concepção e desenvolvimento da autonomia na escola. Desse, modo, objetivou-se analisar o grau de autonomia profissional relacionando-a com as condições de trabalho dos professores da rede pública de ensino da cidade de Mossoró (RN), correlacionando-as ao IDEB (Índice de Desenvolvimento da Educação Básica). Trata-se de um estudo de caráter descritivo, exploratório e analítico, baseado numa metodologia de cunho quantitativa e qualitativa. Para a concretização do percurso quantitativo, foram aplicados o questionário sociodemográfico, a Escala de Autonomia Profissional dos Professores e a Escala Estressores Ocupacionais dos Professores, numa amostra de 404 professores. No percurso qualitativo foi realizado a entrevista semiestruturada com 7 professores de diferentes escolas, através dos quais procurou-se conhecer suas percepções acerca da autonomia e suas condições de trabalho. Os resultados obtidos apontaram que a autonomia do professor depende das condições de trabalho oferecidas, visto que o índice geral de autonomia se correlaciona positivamente com o índice geral de estressores ocupacionais (r= 0,221; p< 0,01). Identificou-se que as escolas que apresentam melhor IDEB são julgadas pelos professores com melhores condições de trabalho para desenvolver suas atividades (r= 0,177; p< 0,01), bem como, nessas escolas os professores sentem-se mais autônomos (r=0,304; p<0,01). Fatores como relacionamento com alunos e responsáveis, a sobrecarga de trabalho em função especialmente do momento na qual se vive na educação em função de metas que devem ser atingidas são desencadeadores de estresse. No discurso dos professores evidenciam-se possibilidades e limites para desenvolver sua autonomia, sendo o espaço da sala de aula o local onde esses profissionais sentem-se mais autônomos. A participação em conselhos ou organizações foi colocada como uma forma de resistência às imposições ocorridas no espaço escolar. O estudo contribui para dar consistência à ideia expressa nos documentos oficiais de que há autonomia, no professor, no entanto as escolas ainda revelam uma perda de espaço para uma reflexão, discussão e construção de uma autonomia, alertando para as condições de trabalho desfavoráveis que são oferecidas aos professores, que consequentemente afeta o desempenho discente. A dissertação está inserida na linha de pesquisa de Filosofia da Educação, no Programa de Pós-Graduação em Educação da Universidade de Caxias do Sul (PPGEd-UCS). Por meio de uma análise interpretativa dos referenciais teóricos, reflete-se, no primeiro capítulo, sobre os aspectos gerais e históricos do Iluminismo, pois fazem parte do ideário iluminista os conceitos de educação e autonomia. Posteriormente, busca-se a aproximação do referido movimento com o pensamento de Immanuel Kant, no intuito de corroborar o entendimento acerca das questõeschave do presente trabalho, utilizando-se, em especial, do texto O que é o Iluminismo? No terceiro capítulo, num primeiro momento, elabora-se uma análise interpretativa da importância da filosofia kantiana e das conseqüências da idéia de autonomia na pedagogia contemporânea e, num segundo momento, retomam-se os conceitos de autonomia, heteronomia e educação, procurando explicitar alguns elementos teóricos que permitam refletir sobre as condições e as possibilidades de se ter um processo pedagógico que possa estabelecer uma educação para autonomia. Para tanto, buscou-se referências em autores como, Foucault, Höffe, Piaget e Adorno. É possível educar para a autonomia? No intuito de responder a essa pergunta duas observações são importantes: a) ter presente, como fundamento da consciência dos agentes promotores da educação, a ideia de estabelecer uma ação educativa que prepare o educando para que ele seja capaz de pensar criticamente e de formular os seus próprios juízos, de modo que ele possa decidir como agir nas diferentes circunstâncias da vida; b) refletir sobre a possibilidade da ação educativa para a autonomia, de modo que o professor, por um lado, assuma tal projeto e, por outro, oportunize-o aos alunos a fim de que eles se tornem autônomos.

A tese procura relacionar os conceitos de critica e autonomia na filosofia de Immanuel Kant por meio do seguinte recorte: a noção de forma legislativa, analisada à luz dos juízos determinante e reflexionante, permite vislumbrar um caráter positivo para a critica, para além do costumeiro caráter negativo a ela sempre atrelado. Com isso, é a própria idéia de autonomia que, extrapolando o seu \"lugar de origem\" na arquitetônica da razão, passa a ser a chave de interpretação de tal caráter positivo. Para isso, é necessário a este trabalho três passos argumentativos: analisar, em primeiro lugar, o próprio conceito kantiano de filosofia a partir da relação entre os conceitos acadêmico e mundano, definidos por Kant na Lógica. Em segundo lugar, trata-se de analisar uma possível vinculação entre o conceito de finalidade e as relações entre forma e reflexão, inerentes a possíveis representações da natureza. Por fim, através dos conceitos de fundamentação e aplicação, procura-se redimensionar o conceito de autonomia que se encontra na Critica da razão pratica.
This work aims to establish a relationship between the concepts of critique and autonomy in Immanuel Kant\'s philosophy, through the notion of legislative form. In regard to determining and reflecting judgments, that notion makes it possible to notice a positive aspect of the critique, going beyond the usually recognized negative aspect. Therewith, it is the idea of autonomy that, beyond its \"original place\" in the architectonic of reason, becomes the interpretive key to such a positive aspect. In order to show it, this work is divided into three steps: firstly, it must analyse Kant\'s concept of philosophy having in view the relationship between the academic and the wordly concepts of philosophy as defined by Kant in his Logic. Secondly, it must discuss a possible bond between the concept of finality and those of form and reflection which are inherent to possible representations of nature. Thirdly, it must, through the concepts of foundation and application, reconsider the concept of autonomy from the Critique of practical reason.

The complexity of fish behaviour and information processing indicates high levels of neural, anatomical and functional organization. Neural cells are conducting neurons and neuroglia with putative support and physiological roles. Neuronal conduction, synaptic transmission, reflexes and neuropils are factors in integrative activity and information processing. Fish nervous systems are organized into central (brain and spinal cord) and peripheral (including autonomic) components. Interestingly the structure and function of the fish optic tectum have been considered comparable to those of the tetrapod cerebral cortex. Also of interest are the bilaterally paired large Mauthner fibres in the teleost central nervous system, which mediate startle responses. The autonomic nervous system in fish occupies a pivotal position amongst vertebrates, including uncertainty about the existence of a posterior parasympathetic component. The trend is to regard it in terms of spinal autonomic (sympathetic) cranial autonomic (parasympathetic) and enteric systems. Accounts of the autonomic control of individual effector systems are included.

Typically polyneuropathy will cause the combination of distal limb muscle weakness, loss of tendon reflexes, and reduced distal limb sensation. There is variable involvement of the autonomic innervation, damage to which causes a dry, vasodilated foot or hand. Loss of tendon reflexes is a cardinal sign of polyneuropathy, often restricted to the ankle jerks in axonal degeneration, but involving more proximal reflexes in acquired demyelinating neuropathies which may involve more proximal segments or the nerve roots. Clinical features suggestive of demyelinating or conduction block polyneuropathy include: a relative lack of muscle wasting in relation to the degree of weakness because no denervation has occurred; weakness of proximal muscles as well as distal, because of nerve root involvement; and disproportionate loss of joint position and vibration sensations compared to relative preservation of pain and temperature sensations which are carried by unmyelinated fibres.

Autonomic testing is an important addition to neurological evaluations. While there are many excellent textbooks on autonomic disorders, only a few texts focus on how to perform and interpret autonomic tests. This manual fills the gap, dealing mainly with the practical aspects of autonomic testing. In accord with the maxim that “a good picture is worth a thousand words,” signal drawings are heavily used throughout the text to explain and illuminate test results. This book has two parts. The first part describes in detail the Brigham protocol of autonomic tests, which includes cardiovascular tests (deep breathing, Valsalva maneuver, tilt tests), sudomotor assessment (quantitative sudomotor axonal reflex test and electrochemical skin conductance), and skin biopsies for assessment of epidermal and sweat gland small fibers. The cardiovascular tests use heart rate, blood pressure, respiratory parameters (respiratory rate and end tidal CO2), and cerebral blood flow velocity. All tests are graded with an updated quantitative scale for cardiovascular reflex tests and transcranial Doppler—the Quantitative Sudomotor Axon Reflex Test (QASAT)—and small fiber (epidermal sensory and sweat gland) densities from skin biopsies. The second part of the book describes 100 cases covering a variety of autonomic disorders. The cases are thematically grouped into orthostatic intolerance syndromes (neurally mediated syncope, orthostatic hypotension, postural tachycardia syndrome, inappropriate sinus tachycardia, orthostatic cerebral hypoperfusion syndrome, hypocapnic cerebral hypoperfusion, and pseudosyncope), dysautonomia in neurodegenerative disorders, small fiber neuropathies (idiopathic, secondary, inflammatory), and autonomic overactivity. The case descriptions are presented in a consistent format featuring pertinent clinical information, autonomic tests results, interpretation of testing, conclusions, and recommendations. This text is intended to be a guide for autonomic fellows, and for residents in neurology, general medicine, and other specialties, and for anyone who is interested in performing and interpreting autonomic tests.

This chapter describes the appropriate clinical approach to take when presented with a patient reporting a neurological symptom. Just under 10 per cent of the population consult their general practitioner about a neurological symptom each year in the United Kingdom. About 10 per cent of these are referred for a specialist opinion, usually to a neurologist. Nine conditions account for roughly 75 per cent of general neurological referrals and are diagnosed initially on purely clinical grounds, with the other 25 per cent representing the full range of other, potentially very rare, neurological disorders.This chapter underlines the importance of a thorough and informative history to achieve successful diagnosis. Crucial facets for a good history include information on the time course of symptom development, whether symptoms are negative or positive, previous neurological history (both personal and familial), as well as other potentially contributory general medical disorders. The general neurological examination is also described, as are specific examination manoeuvres that may be added to the general neurological examination in specific clinical circumstances.Reflexes play an important role in diagnostic neurology because they reflect the integrity of, or alterations in, the neural structures responsible for their arc. Loss of a reflex may be due to interruption of the afferent path by a lesion involving the first sensory neurone in the peripheral nerves, plexuses, spinal nerves, or dorsal roots, by damage to the central paths of the arc in the brainstem or spinal cord, by lesions of the lower motor neurone at any point between the anterior horn cells and the muscles, of the muscles themselves, or by the neural depression produced by neural shock. In clinical practice, the most useful and oft-elicited reflexes are the tendon reflexes of the limbs, the jaw jerk, the plantar response, the superficial abdominal reflexes, the pupil-light response, and in infants, the Moro reflex. The place of these particular reflexes in the routine neurological examination is outlined, and the elicitation and significance of these reflexes and of a wide variety of others which are used occasionally are described.Examinations that allow localization lesions that are responsible for muscle weaknesses and the assessment of somatosensory abnormalities are described, as are neurological disorders that result in identifiable gait disorders. The clinical signs and examinations relevant to autonomic disorders are also discussed.Intensive care may be required for patients critically ill either as a result of primary neurological disease, or in those in whom a neurological disorder is a component of, or secondary to, a general medical disorder. Indications for admission to neurological intensive care have been defined (Howard et al. 2003): impaired consciousness, bulbar muscle failure, severe ventilatory respiratory failure, uncontrolled seizures, severely raised intracranial pressure, some monitoring and interventional treatments, and unforeseen general medical complications. Naturally specific treatments indicated for the particular diagnosis should be instituted along with general intensive care measures.Finally, the discussion of diagnoses of chronic or terminal conditions with patients is discussed, with particular focus on the best way to present the diagnosis to the patient.

It is prevalent at 3.4 to 4.9 cases per 100,000 population, and is most common after the age of 60. It is called Striatonigral Degeneration, with a predominance of parkinsonian symptoms; it is known as Olivopontocerebellar Atrophy, with cerebellar symptoms, and Shy-Drager syndrome with autonomic symptoms. Physiotherapy plays an important role in the rehabilitation of kinetic- functional impairments. Objective: To report the case of a volunteer with SMA in order to highlight the neurological findings for neurofunctional rehabilitation. Methods: This is an observational, cross-sectional case report study that was conducted at the Clínica Escola de Fisioterapia da Universidade de Franca (UNIFRAN), with the approval of the CEP (CAAE 83164918.2.0000.5495), of a 55-year-old male individual, referred to physiotherapy, diagnosed with SMA. Neurological evaluation was performed in the sector, of sensitivity, movement, tone, reflexes, motor coordination, balance, gait and function. Results: The individual reports that 4 years ago he started having difficulty walking and talking. He was diagnosed with Parkinson’s disease. He underwent a new evaluation, arriving at the current diagnosis. In the physical therapy evaluation dysautonomia, dysarthria, dysdiadochokinesia, dysmetria, hypertonia by rigidity, akinesia and bradykinesia, trunk flexion posture, balance deficit, tetraparesis and fetinated gait were observed. Conclusion: The impairments found impair the patient’s ability to perform several ADLs. This study emphasizes the value of neurofunctional physiotherapeutic semiology for an effective therapeutic treatment that promotes independence and quality of life possible for the individual with SMA.

Abstract:
Context: Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by reversible narrowing of brain vessels. It affects young adults, predominantly female, with acute and self-limited course, initiated with “Thunderclap” headaches associated or not with neurological symptoms, such as seizures and focal deficits. Pathophysiology is the dysfunction in the control of the sympathetic autonomic nervous system through a multifactorial mechanism with a strong association with hormones, immunological, biochemical and other predisposing factors, including drug addiction, with emphasis on marijuana, ether and chloroform. Case report: A 19-year-old female started with an insidious acute clinical condition of severe holocranial headache after using marijuana and “loló” (chloroform and ether narcotic), evolved with behavioral and level of consciousness alterations, admitted in status epilepticus, being orotracheal intubation (OTI) is required. No comorbidities. Neurological examination: OTI and sedated, at RASS-2, slowed trunk reflexes and global weakness (Grade 3). Skull tomography: Hyperdense foci in splenium of the corpus callosum and cingulate gyrus. Cerebral angiography: mild to moderate cerebral vasospasm evident in the P1 and P2 segments of the posterior cerebral arteries, suggestive of RCVS. Conclusion:The diagnosis is clinical and radiological, with cerebral angiography being the gold standard. Vascular involvement such as intraparenchymal hemorrhage, non-aneurysmatic subarachnoid hemorrhage and cerebral infarction are common. Toxicological tests help in the investigation, and the exclusion of the precipitating cause is important, in addition to drug therapy that prevents recurrence of vasospasm. The improvement can occur in up to three months, in our case the evolution with neurological improvement was evident after one month.

Abstract:
This paper describes the simulated environment that was prepared for validation of Cognitive Function Synthesis, or CFS, Autonomy. The environment was developed with the V-REP simulator and assembled around the ‘Eric Rohmer’ Pioneer robot. The simulated robot was configured to act as a Behaviour-Based Robot that is constrained to move about the test area, as defined by a state diagram, and based on pre-configured reflexes, provided by the Braitenberg algorithm. Vision sensors, i.e. simulated cameras, were found to be better suited to configure these behaviours than light sensor cues. Initial results suggest that the simulated environment may be used for CFS autonomy validation. This research marks the first simulated environment produced for investigating CFS Autonomy and consequently needs to be further validated.

This paper analyses results from experiments performed using a previously-described, simulated environment that was developed for validation of Cognitive Function Synthesis, or CFS, Autonomy. Navigation performance of the Pioneer robot platform used the following metrics: Average Cycle Time per simulation run; Average Wall Contact per cycle; and Average Shock Treatment Activation per simulation run. Two ultrasound, or US, configurations were used while the robot navigated in either the ‘preconfigured-reflexes only’ mode or the ‘Braitenberg Obstacle Avoidance’ mode. Results from the “16 sensor” US configuration was generally found to be significantly different from that of the “8 sensor” configuration, independently of obstacle avoidance considerations. Robot performance, when subject to the Braitenberg Obstacle Avoidance algorithm, was also found to be significantly different from ‘preconfigured-reflexes only’ performance, regardless of US configuration. The difference in Shock Treatment and the Average Wall Contact, observed between the “16 sensor” US setting and the “8 sensor” configuration for the ‘Braitenberg Obstacle Avoidance’, are likely to be due to the coefficient values adopted for the rear US sensors together with robot position at experiment start. The use of this environment to enable statistical analysis of results, to determine significant difference in obstacle avoidance performance, validates its usefulness as a tool for CFS Autonomy validation.

Abstract:
Rural economic transformations in Africa are generating new opportunities to engage with agricultural value chains. However, many young people are said to be locked out of such opportunities because of limited access to farmland, which pushes them out of agriculture and rural areas, and/or hinders their autonomy. This framing of the ‘land problem’ imperfectly reflects rural young people’s livelihoods in much of sub-Saharan Africa, and therefore does not provide a solid basis for policy. Policy-relevant discussions must consider the diversity of rural contexts, broader land dynamics and more nuanced depictions of youth engagement with the rural economy.
