Abstract:
Abstract Introduction. Evidence suggests that human epidermal growth factor receptor 2-positive (HER2+) metastatic breast cancer (MBC) patients have different clinical characteristics and outcomes according to the hormone receptor (HR) status. We aimed to evaluate the impact of HR status in this population extracted from a large real-world database. Methods. We performed a retrospective analysis of HER2+ MBC patients (pts) included in the real world ESME MBC database (NCT03275311) between 2008 and 2017. Descriptive statistics, the Kaplan-Meier method and Cox proportional hazards models were used to report characteristics, outcomes and prognostic factors. Results. Of 4,145 HER2+ MBC pts eligible for our analysis, 1,449 (35%) had HR-negative (HR-), while 2,696 (65%) had HR-positive (HR+) tumors. Pts with HR- tumors had metastases earlier (median 9.3 vs 28.0 mo from primary cancer), had more often de novo MBC (47.6% vs 38.1%), grade III tumors (50.9% vs 33.6%), visceral metastases (70.9% vs 60.8%) and less often bone only disease (8.4% vs 21.1%) compared to those with HR+ tumors, all p<0.001. Pts with HR+ MBC received less frequently an anti-HER2+ targeted therapy during first line of treatment than pts with HR- tumors (74.9% vs 90.8%, p<0.001). Pts with HR- MBC had significantly worse outcomes than those with HER2+/HR+ MBC, with median overall survival (OS) 42.0 mo [95% CI 38.8-45.2] vs 55.9 mo [95% CI 53.7-59.4], p<0.001, and median progression-free survival (PFS) 9.8 mo [95%CI 9.2-11] vs 12.2 mo [95%CI 11.5-12.9], p=0.012, respectively. The independent prognostic value of HR status was confirmed in a multivariable analysis for OS (HR- versus HR+, hazard ratio=1.25 [1.13-1.39], p<0.001) but not for PFS (hazard ratio=1.03 [95%CI 0.95-1.13], p=0.8) (Table 1 for OS). The multivariable analysis also included tumor grade, age at and time to MBC diagnosis, presence of visceral metastases, number of metastatic sites and performance status as prognostic factors. In the HR+ population, ER and PR statuses were not prognostic. Conclusions. These data confirm the remaining strong and independent adverse prognostic effect of negative hormone receptors among pts with HER2+ MBC treated in the past 12 years. Abstract:
Abstract Context: Polymerase epsilon (POLE) gene missense hotspot mutations can generate pathogenic (p) proofreading defects resulting in hypermutated genomic profiles. Aim: Determine the prevalence, genomic consequences and immunotherapy sensitivity of advanced POLE mutated tumors according to mutation site, primary tumor and tumor mutational burden (TMB). Results: Pan-Cancer TCGA & MSKCC databases genomic analyses found a prevalence of non-pathogenic POLE mutations (POLEnp) of 3.4% with median TMB of 11 mutations/Megabase (mt/Mb, IQR 3-34). Pathogenic POLE mutations (POLEp) prevalence was 0.4% with median TMB of 215 mt/Mb (IQR 107-324), predominantly in colorectal and endometrial cancers. Prevalence dropped to 0.1% in metastatic cancers. We assessed prospectively the efficacy of PD-1 blockade in mismatch repair proficient advanced solid tumors harboring POLE missense mutations (phase II ASCe Nivolumab trial; NCT03012581). Variants were categorized prospectively by a molecular board as POLEp, POLEnp or Variants with Unknown significance (VUS). The primary endpoint was the Overall Response Rate (ORR) at 12 weeks according to RECIST 1.1, and secondary endpoints included survival analyses according to POLE variants pathogenicity. Among 61 screened patients, 21 were eligible and 20 received Nivolumab and 19 were assessable for response (table 1). The 12-week ORR was 37% for patients harboring POLEp and VUS and resulted in major survival improvement compared to POLEnp patients (HR=0.1 ; CI95% 0.02-0.7); see results in Table 1. Among patients POLEp tumors, while higher TMB was not predictive of response, higher proportion of POLE-related mutational signature correlated with improved benefit. In silico exonucleasic POLE domain analyses confirmed that all POLEp and 2 VUS clustered in the DNA binding or the Catalytic site. Recategorizing the VUS according to the location within the exonucleasic domain improved the prediction of survival outcomes. Impact: This study gives new insights on how DNA repair defects, mutational burden and signatures sensitize to PD-1 blockade and may offer emerging tumor agnostic biomarkers for benefit to checkpoint blockade. Abstract:
Abstract Background The everolimus (EVE)-exemestane combination has been included in the International guidelines for metastatic HR+/HER2- breast cancer (mBC) since the results of the Bolero-2 trial. Marketing authorization has been granted in France in July 2012. A first report of the UNICANCER Epidemiological Strategy and Medical Economics (ESME) Research program described the real world use of everolimus therapy (ESMO 2017, #266). We report here updated data with long-term overall survival (OS) analyses, focusing on patients treated from 2012 onwards. Methods All patients (pts) who initiated treatment for a newly diagnosed mBC between 2008 and 2017 in all 18 French Comprehensive Cancer Centers have been included in the real life ESME database, which collects retrospective data using a clinical trial-like methodology. The primary endpoint of the present report was overall survival in pts who received everolimus. In order to adjust for differences between groups (EVE treated vs non EVE treated patients), we analyzed the impact of everolimus on OS and PFS by using a propensity score and inverse probability of treatment weighting (IPTW) sensitivity analyses, built with relevant cancer-related clinical variables in relation to survival and allocation of treatment. Those analyses were performed by line 1, 2 or 3 of treatment for mBC. Results The ESME program included 23,698 pts of whom 1897 with HR+/HER2- mBC received at least 1 dose of EVE and were diagnosed after 2012. Median age was 63 y (22-103). EVE pts were slightly younger than non-EVE pts (25.9% and 23.7% under 52 y, respectively, p=0.0574). EVE treated pts had more frequent non visceral metastases (52.3% vs 47.9%) and bone only metastases (38.5% vs 31.3%) at metastatic diagnosis, and less symptoms at mBC diagnosis (42.4% vs 47.5%) than non-EVE pts (all p values <.001). 94.3% of EVE pts had received an EVE-exemestane combination. EVE was administered either in the 1st, 2nd or 3rd line+ setting, in 3.3%, 14.7% and 25.3% of all cases respectively. At each line, treatments in the non-EVE treated pts included various endocrine therapy or chemotherapy regimen. Median follow-up was 47.9 m (0-98.7), and 61.4 m (2.1-98.7) for the overall- and EVE treated populations, respectively. Crude and landmark (6 and 12 months) OS analyses all suggested a longer OS with EVE based therapy. IPTW analyses by lines of treatment, comparing EVE-treated and non-EVE-treated patients, suggested a longer progression free survival with EVE when administered in the 3rd line setting (HR=0.82 CI95% [0.745-0.903], p<.0001). IPTW analyses consistently showed a significantly longer OS with EVE based treatment when administered in lines 1, 2 and 3 with respective HR values of 0.34, 0.34 and 0.23 (Table). Causes for EVE discontinuation were mainly disease progression (54%) and toxicity (26.6%). An exploratory analysis showed that 998 EVE treated patients (52.6%) also received a CDK4/6 inhibitor based treatment, mostly after EVE therapy (n=826, 82.8%). Median duration of CDK4/6 inhibitor therapy for those patients was 4.6 months (IQR 2.9-8.7). Conclusions These results are limited by the retrospective nature of the study and the lack of performance status, comorbidity or nutritional data. However, the long-term follow up of the ESME database suggests a favorable association between everolimus therapy and overall survival in HR+, HER2- mBC, persistent after IPTW adjusted analyses. Abstract:
This thesis compares and contrasts the historiography of Quebec and Flanders during the first half of the twentieth century. The main argument is that the philosophy of neo-Thomism was influential to the conceptualization and writing of history by prominent nationalist historians in both Quebec and Flanders during the period leading up to the Second World War. By extensively comparing the life and works of prominent nationalist historians that played an active role in the nationalist movements of Quebec and Flanders, it has been found that the Catholic University of Leuven was influential in the development of nationalist historiography in Quebec and Flanders during the first decades of the twentieth century. In this sense, this thesis argues that the nationalist historians of Quebec and Flanders be considered as part of a shared historiographical tradition that was influenced by the neo- Thomist philosophy which played an essential role at the Catholic University of Leuven during this period, and which can be traced back in the writings and practices of nationalist historians in both Quebec and Flanders. Out of this shared influence of the neo-Thomist philosophy then, this thesis argues for a reevaluation of the traditional portrayal of nationalist historiography in the first half of the twentieth century, and a reconsideration of the influence neo-Thomism has had on the conceptualization of nationalist history in Quebec and Flanders. It is argued that the nationalist historians of both Quebec and Flanders have traditionally been characterized as unscientific due to their convergence of science and politics, and portrayed the nation as deterministic, meaning that the nation's essence and development was unaffected by the historical circumstances. By analysing the historical works of nationalist historians that either attended the Catholic University of Leuven, or were part of a network that was influenced by the writings of the neo-Thomists that taught at Leuven, this thesis will make three general arguments that will nuance this traditional portrayal of nationalist historiography during the first half of the twentieth century. First, it will be argued that the neo-Thomist emphasis on the interdependence of essential and existential characteristics nuances the essentialist portrayal of the nation. Using the case of neo- Thomist chemistry as a counterexample, it will be shown how nationalist historians in Quebec and Flanders ascribed an important role to the existentiality and historicity of the nation, and as such, compels us to reconsider the essentialist paradigm of nationalist historiography. Secondly, the neo- Thomist notion of science which legitimated the convergence of subjectivity and objectivity sheds new light on the practice and theory of what constituted scientific history in the first half of the twentieth century. Moreover, it will be argued that Quebec and Flanders shared a similar theoretical concept of what constituted scientific history, but represented their historical works differently due to the differentiating political and academic context. Finally, the thesis will highlight how the notions of ambiguity and human freedom, which figured prominently in neo-Thomism, influenced the notion of teleology in Quebec and Flemish nationalist historiography, as is illustrated by the notion of coincidence in Flemish, and providence in Quebec historiography. In addition, using the cases of nationalist historians Lionel Groulx and Hendrik Elias, it will be argued that the different political contexts influenced the political actions of the two nationalist historians, which helps to shed new light on the motives of Flemish nationalist historians to collaborate during the Second World War. By comparing and contrasting the two cases then, this thesis is able to show how the neo- Thomist framework and crucial concepts were not only instrumental to the nationalist historiographies in Quebec and Flanders, but were also malleable to differing historical contexts, and, as such, provides new insight in the intricate relationship between religion, nationalism and historiography that underpinned nationalist historiography in Quebec and Flanders during the first half of the twentieth century.

Abstract:
Augered Cast-In-Place (ACIP) Piles are used in areas were the loading from a superstructure exceeds the soil bearing capacity for usage of a shallow foundation. In Northwest Florida and along the Gulf Coast, ACIP piles are often utilized as foundation alternatives for multi-story condominium projects. Data from 25 compression load tests at 13 different project sites in Florida and Alabama were analyzed to determine their individual relationships between anticipated and determined compression load capacity. The anticipated capacity of the ACIP pile is routinely overestimated due to uncertainties involved with the process of estimating the compressive capacity and procedures of placing the piles; therefore, larger diameter and deeper piles are often used to offset this lack of understanding. The findings established in this study will provide a better empirical relationship between predicted behaviors and actual behaviors of ACIP piles in cohesionless soils. These conclusions will provide the engineer with a better understanding of ACIP pile behaviors and provide a more feasible approach to more accurately determine the pile-soil interaction in mostly cohesionless soils.

Amid the global efforts surrounding United Nations' Millennium Development Goal Target 7c to improve access to safe and sustainable drinking water among populations who lack this resource, it has become essential to monitor and evaluate progress. Development initiatives working to achieve improved drinking water access often introduce appropriate technologies designed to be sustainably owned and operated by populations in rural areas suffering from water related hardships. It is valuable to thoroughly examine the degree to which these technologies satisfy intended objectives and affect user experienced water access. The accurate reflection of impact and progress can be complex, as the evaluation of water supplies can be made based on a variety of indicators that range from "improved' or "unimproved" water source definitions to measurements of the capacity of a source to satisfy desirable conditions related to water quality, quantity, reliability, or user's preference. The goals of this research are to conduct a comprehensive analysis of the effects of two appropriate technologies on local water access using an assortment of methods including: water quality analysis, visual and manual inspection, user interviews, and an overall sustainability analysis. In Panama, the indigenous Ng[oumlaut]be people in the [Ntilde][oumlaut]Kribo coastal area are a group disproportionately affected by a lack of improved access to drinking water and challenges to the feasibility of piped gravity fed water systems that typically serve the rest of the country. An NGO aiming to ameliorate this situation introduced two improved groundwater supply technologies to the region: bailers and EMAS hand pumps. This study assesses the comparative performance of these systems and evaluates the respective performances of existing water sources, using the wide variety of quantitative and qualitative data obtained. The data collected in this investigation suggested that bailers and EMAS pumps yielded a mixed level of performance based on physical, chemical, and bacteriological water quality measurements in the shallow wells of the study environment. The technologies generally satisfied international guidelines and expected ranges based on chemical and physical parameters such as conductivity, TDS, and turbidity (with 57% of samples under 5 NTU). EMAS hand pumps demonstrated excellent bacteriological water quality with all samples indicating undetectable levels of E.coli, while bailers had a fair performance with 83% of samples falling into a range signifying intermediate to no associated health risk. When comparing the overall water quality performance between the two hand augered well systems and with existing sources, the results indicated that bailers and EMAS pumps performed similarly in all aspects except for bacteriological quality. Overall, analysis based on groupings of "improved" and "unimproved" sources yielded very little distinction between the two categories when considering chemical, physical, and bacteriological parameters. This highlights the added value of using alternative indicators such as WHO guidelines to assess water sources, despite the challenges associated with field water quality sampling. Interview data demonstrated that hand augered wells significantly improved household water access in the study area based on user considerations by providing a reliable water drinking water alternative with adequate quantities of water perceived to be clean. Accordingly, the improved water systems were integrated as a resilient water source into a socio-cultural context noting variable dependence on multiple water sources with categorized, appropriate related water uses set informally by Ng[oumlaut]be families. The overall sustainability analysis found EMAS hand pump and bailer technologies to be effective and appropriate; featuring low costs, few materials, and simple designs. Bailer systems were considered to be especially promising for applications in similar remote areas with high groundwater tables. However, the ultimate sustainability of both systems in the local context was found to be largely dependent on factors related to the development strategy adopted while implementing these systems in the [Ntilde][oumlaut]Kribo area.

In primo luogo attraverso un’analisi statistica e strategica della costruzione, e del successivo impiego, dell’istituzione militare del Regno d’Italia durante gli anni della sua esistenza (1805-14); successivamente, è stato scelto un caso di studi particolarmente significativo, come la campagna di Catalogna (1808-11, nel contesto della guerra di Indipendenza spagnola), per poter valutare il contributo operazionale e tattico dei corpi inviati dal governo di Milano e la loro integrazione con l’apparato militare complessivo del Primo Impero. La tesi ha voluto rispondere alla mancanza di studi sul comportamento in guerra dell’esercito italiano e, allo stesso tempo, introdurre nella storiografia militare italiana la metodologia di studi, d’origine anglosassone e ormai di tradizione trentennale, di Military Effectiveness. La ricerca si è primariamente basata, oltre che sulla copiosa memorialistica a stampa italiana e francese, sulla documentazione d’archivio della Secrétairerie d’état impériale (Archives Nationales di Pierrefitte-sur-Seine, Parigi), del Ministère de la Guerre francese (Service historique de la Défence, di Vincennes, Parigi) e del Ministero della Guerra del Regno d’Italia (Archivio di Stato di Milano). Dal punto di vista dei risultati è stato possibile verificare come l’esercito italiano abbia rappresentato, per Bonaparte, uno strumento duttile e di facile impiego, pur in un contesto di sostanziale marginalità numerica complessiva di fronte alle altre (e cospicue) forze messe in campo da parte dell’Impero e dei suoi altri Stati satellite e alleati. Per quanto riguarda la campagna di conquista della Catalogna è stato invece possibile appurare il fondamentale contributo dato dal contingente italiano, sotto i punti di vista operazionale e tattico, per la buona riuscita dell’invasione; questo primariamente grazie alle elevate caratteristiche generali mostrate dallo stesso, ma anche per peculiarità disciplinari e organizzative che resero i corpi italiani adatti a operazioni particolarmente aggressive.
The Italian Army and the Conquest of Catalonia (1808-1811) A Study of Military Effectiveness in Napoleonic Europe Academic Fields and Disciplines SPS/03 – M-STO/02 The research has the purpose of reconstruct and evaluate the military effectiveness of the Italian Army existed under the reign of Napoleon I. Firstly through a statistic and strategic analysis of the development, and the following deployment, of the military institution of the Kingdom of Italy in the years of its existence (1805-14). Afterwards, a particularly significant case study was chosen, as the campaign of Catalonia (1808-11, in the context of the Peninsular War), in order to assess the operational and tactical contribution of the regiments sent by the Government of Milan and their integration in the overall military apparatus of the First Empire. The thesis wanted to respond to the lack of studies on the Italian army’s behavior in war and, at the same time, to introduce the methodology of the Military Effectiveness Studies (of British and American origin and, by now, enriched by a thirty-year old tradition) in the Italian historiography. The research is primarily based, besides the numerous memoirs of the Italian and French veterans, on the archive documentation of the Secrétairerie d’état impériale (Archives Nationales of Pierrefitte-sur-Seine, Paris), of the French Ministère de la Guerre (Service historique de la Défence, of Vincennes, Paris) and of the Italian Ministero della Guerra (Archivio di Stato di Milano). About the results, it has been verified how the Italian army has become a flexible and suitable instrument for Bonaparte, albeit in a context of substantial overall numerical marginality in comparison to the heterogeneous forces available to the Empire and its others satellites and allied states. Regarding the campaign of Catalonia, instead, it was possible to ascertain the fundamental contribution of the Italian regiments, in an operational and tactical perspective, for the success of the invasion. This was primarily due to the excellent general characteristics shown by the expeditionary force, but also to disciplinary and organizational peculiarities that have made the Italian corps suitable for particularly aggressive operations.

Abstract:
This thesis presents the energy spectrum of cosmic rays deduced from Cherenkov-dominated data measured by the fluorescence detector of the Pierre Auger Observatory. Cherenkov-dominated events, used in the energy spectrum analysis at the Observatory for the first time, enable to decrease the energy threshold for the spectrum measurement down to 1015.5 eV. This energy is more than one order of magnitude lower than in preceding studies. The fluorescence detector was originally designed to detect the fluorescence light generated by extensive air showers in a hybrid mode with the surface detector of the Pierre Auger Observatory. The reconstruction of events dom- inated by Cherenkov light is available due to a newly developed reconstruc- tion technique, the profile constrained geometry fit. Its implementation in the Pierre Auger Observatory software is documented. Aspects of the energy spectrum analysis are described. They consist of exposure calculations done with the use of extensive Monte Carlo simulations, unfolding of the detec- tor effects, and inferring the invisible energy correction in the energy region below 1017 eV. Systematic uncertainties of the measurement are estimated. 1

Abstract:
This diploma thesis examines the possibilities of a new method of astronomical calibration at the Pierre Auger Observatory in Argentina. Its goal is to use stars as objects with known brightness to calibrate the fluorescence detectors and compare these results with the existing calibration. The analysis was done using computer programs which use the star catalog and experimental background data to compare the current calibration with our method. Results that are presented conclude that the calibration using stars is consistent with the existing calibration, however, the error of the new method is much higher. Nevertheless, there is a good potential for scale - it could be used for all past and future data. Potential improvements to this method and its error are suggested.

Firstly, a short introduction of various de- scriptions of the extensive air showers was given. Then a systematic analysis was performed on approx. 7 × 105 simulated showers with three different tech- niques. A brief explanation of the functionality of the PAO was given, then the importance of monitoring the immediate state of the atmosphere using the Shoot- the-Shower program was elucidated and the FRAM telescope was described in detail. FRAM enabled an identification of showers with a clear atmospheric back- ground, this sample of showers was then analyzed and several interesting events warranting a further study were chosen.
