Abstract:
Fourteen percent of the Swedish population report subjective hearing loss. The number of persons suffering from hearing loss is expected to increase in accordance with the increased length of the average life span, causing an associated increase in the demand for hearing health care services as new patient groups who expect a higher quality of life begin to request hearing care. The main goal of this thesis was to develop new approaches in audiological rehabilitation to meet these demands and achieve user satisfaction. Two randomized controlled trials including 39 and 38 subjects, respectively were performed that evaluated two interventions, user-controlled adjustment and sound awareness training, which were performed prior to a hearing aid fitting. The new approaches focused on increasing user participation and activity. To evaluate the goals of audiological rehabilitation, e.g., reducing auditory impairment, optimizing auditory activities and minimizing participation restrictions, several standardized self-reporting instruments were used to assess activity limitations, participation restriction, satisfaction and psychosocial well-being. Several of the instruments were validated for a Swedish population in a postal survey including 162 subjects. Furthermore, an interview instrument that was appropriate for telephone interviews and a categorization rating scale were developed for assessing the global clinical impression of the audiological rehabilitation. Few significant differences in outcomes were found between the treatment and control groups in the short term, and the interventions did not achieve additional or more successful hearing aid users in the long term. Thus, it was concluded that the hearing aid rehabilitation was effective in and of itself, as both the treatment and control groups showed significant improvements in psychosocial well-being and reduced activity limitation and participation restriction. The self-report instruments were found to be valid, and a factor analysis indicated that the number of questionnaires could be reduced with a recommendation for further clinical use. The telephone interviews evaluating the clinical global impression of the audiological rehabilitation were found to be effective and showed success in a vast majority of the users. Advantages such as simpler administration and less time consumption warrant their continued use in additional audiological settings. The pre-interventions in these studies need to be further investigated before they could be recommended for clinical use also in a Swedish context. The international standardized self reports, however, can already be recommended for clinical use. A first attempt to evaluate global clinical impression by telephone interviews was found to be effective and further validations are suggested.

Abstract:
This thesis describes the data mining of a large set of patient records from the hearing aid clinic at James Cook University Hospital in Middlesbrough, UK. As typical of medical data in general, these audiology records are heterogeneous, containing the following three different types of data: Audiograms (graphs of hearing ability at different frequencies) Structured tabular data (such as gender, date of birth and diagnosis) Unstructured text (specific observations made about each patient in a free- text or comment field) This audiology data set is unique, as it contains records of patients prescribed with both ITE and BTE hearing aids. ITE hearing aids are not generally available on the British National Health Service in England, as they are more expensive than BTE hearing aids. However, both types of aids are prescribed at James Cook University Hospital in Middlesbrough, UK, which is also an important feature of this data. There are two research questions for this research: Which factors influence the choice of ITE (in the ear) as opposed to BTE (behind the ear) hearing aids? For patients diagnosed with tinnitus (ringing in the ear), which factors influence the decision whether to fit a tinnitus masker (a gentle sound source, worn like a hearing aid, designed to drown out tinnitus)? A number of data mining techniques, such as clustering of audiograms, association analysis of variables (such as, age, gender, diagnosis, masker, mould and free text keywords) using contingency tables and principal component analysis on audiograms were used to find candidate variables to be combined into a decision support system (OSS) where unseen patient records are presented to the system, and the relative likelihood that a patient should be fitted with an ITE as opposed to a BTE aid or a tinnitus with masker as opposed to tinnitus not with masker is returned. The DSS was created using the techniques of logistic regression, Nalve Bayesian analysis and Bayesian network, and these systems were tested using 5 fold cross validations to see which of the techniques produced the better results. The advantage of these techniques for the combination of evidence is that it is easy to see which variables contributed to the final d~~Jpion. The constructed models and the data behind them were validated by"presenting them to the Principal audiologist, Dr. Robertshaw at James Cook University Hospital in Middlesbrough for comments and suggestions for improvements. The techniques developed in this thesis for the construction of prediction models were also used successfully on a different audiology data set from Malaysia. These decisions are typically made by audiology technicians working in the out- patient clinics, on the basis of audiogram results and in consultation with the patients. In many cases, the choice is clear cut, but at other times the technicians might benefit from a second opinion given by an automatic system with an explanation of how that second opinion was arrived at.

Abstract:
The recognition by societies of the problems caused by deafness and hearing impairment has led to a significant increase in the need for audiologists. Additionally, the rapid development of audiology in the last few decades, a wide diversity of audiology education programs, and increased globalisation highlighted the need to evaluate current audiology education and practice from an international perspective. A major aim of this thesis was therefore to study comparative audiology, to allow systematic analysis of what hearing health care is needed worldwide, and the strategies required to achieve future improvements. Rather than remaining theoretical, this thesis also aimed to develop an affordable and sustainable seeding model for audiology education, which would build capacity in countries with inadequate audiology education. An international study surveyed major audiology organisations, and completed surveys were received from 62 countries (representing 78% of the world population). Eighty-six percent of countries needed more audiologists, and cited a lack of government funding, low public awareness of deafness and inadequate audiology education as major reasons for the shortages. There was a wide range of professionals providing hearing health care and a large overlap in their scope of practices. To discuss these complex issues, a conceptual model for hearing health care was developed which formalised the interacting issues, and highlighted the feedback loops between them. The conceptual model presented a framework that was used to discuss individual countries in more detail, and provided a basis for the design of strategies aimed at improving hearing health care and education for groups of countries. Although five distinct groups were described, in practice, most countries may benefit to some extent from hearing health care systems that range from decentralised community-based models (providing basic care), to more institutionally-based models (offering advanced services and technologies). Evolution of hearing health care systems from basic community-based, to more advanced institutionally-based services, would presumably come with strengthening economies and health and education infrastructures, and concurrent growth in audiology education and private and public hearing health care markets. The rate at which this occurs depends in part on the strength of a country’s economy, but also on its other health priorities, the equality of its wealth distribution, the diversity and geographic spread of its population, and the adequacy and efficiency of its hearing health care education programs.

Posttraumatic stress disorder, or PTSD, is associated with enduring and profound impairments affecting emotional stability and the ability to perform fundamental activities. In our VA tinnitus clinic population, more than 35% of the patients carry the diagnosis. Perhaps of greater concern, PTSD is generally acknowledged to be substantially underreported among children (Herman, 1997). Individuals with PTSD experience disruptions to a variety of life functions, intrusive memories, and other powerful symptoms. Functional and emotional consequences of trauma are measured using a variety of handicap scales, and several physiological measures such as EEG and EKG are altered by the effects of trauma. Additionally, fMRI and PET scans reveal enduring changes to neural structures such as the hippocampus following exposure to traumatic events (Bremner, 2002). In this eAudiology Web seminar, we will stress that PTSD should be viewed as a psychological injury; the trauma-provoked physiologic changes that influence not only an individual's to function, but also their ability to obtain benefit from medical care. This presentation will also review the history of the PTSD diagnosis and identify specific elements of the injury that are of significance to audiologists.

Outside of major metropolitan areas, Arizona has limited options for hearing healthcare. The Arizona Affordable Hearing Aid Task Force proposed the development of a statewide program to provide low-cost hearing aids and audiologic care to low-income residents. The purpose of this review was to determine the status of the literature on the clinical outcomes of teleaudiology services for evaluations and hearing aid fittings which could be used to serve inhabitants of rural counties and address the goals of the Task Force. A literature search was performed to identify articles with original research in teleaudiology in the areas of evaluation and treatment and yielded 234 results. After exclusion criteria were applied, there were 15 articles for review; 10 articles focused on audiological evaluation and 5 articles focused on verification, validation, and counseling for hearing aid fittings. Evaluation-related articles showed that pure-tone air conduction testing was generally within the ± 5 dB acceptable range of variability, with little evidence on bone conduction or speech testing reliability. Studies reporting real-ear measurements with probe microphones were shown to yield similar results in traditional and teleaudiology fitting sessions and validation measures documented similar or better outcomes from teleaudiology fittings. Based on the literature, it is concluded that the use of teleaudiology is feasible for hearing aid fittings and counseling in rural areas, if associated barriers related to costs and limitations related to the availability of technology are overcome.

Inledning: Hörapparatsutprovning är en stor del av audiologisk rehabilitering. Flera intresseorganisationer inom hörselområdet har dock påtalat att det förekommer skillnader rörande utbildningsnivån hos hörselvårdpersonal inom Europa samt att tillhandahållen hörselvård i vissa fall är bristande. Syfte: Att jämföra hörapparatsutprovningsprocessen i ett urval av Europeiska länder. Fokus ligger på att belysa vilka yrkesgrupper som är involverade i hörseldiagnostik och hörapparatsutprovning samt vilken utbildningsnivå dessa har. Utöver detta kommer ländernas finansiering av hörapparater undersökas. Eventuella likheter och skillnader mellan länderna kommer sedan diskuteras ur ett patient perspektiv.Metod: En integrativ litteraturstudie har genomförts. Resultat: En jämförande beskrivning av de yrkesgrupper som patienter träffar under hörapparatsutprovningsprocessen i de studerade länderna samt vad hörapparaterna där kostar patienterna. Resultaten visar vidare att audiologiutbildningar varierar i omfattning mellan länderna, från 11-månaders yrkesutbildningar till 5-åriga universitetsutbildningar. Finansieringen av hörapparater i respektive land varierar också från gratis offentlig hörselvård till att patienten själv får betala hela kostnaden hos privata hörselkliniker. Slutsats: De skillnader i audiologiutbildning och finansiering av hörapparater som observerats motiverar fortsatt utvecklingsarbete inom området för att uppnå en likvärdigare hörselvård inom hela Europa.

Abstract:

King-Kopetzkys syndrom (KKS) beskrivs som när en person upplever svårigheter med att uppfatta tal, speciellt i bullriga miljöer, men har normala tontrösklar. Orsaken till syndromet är fortfarande oviss och detta gör att de som jobbar inom hörselvården ställs inför ett val då de möter dessa patienter. Patienten kan antingen avfärdas då denne anses höra normalt, eller så tas problemet på allvar och vidare utredning utförs.

Genom att göra en systematisk litteraturstudie belyses och förklaras syndromet och syftet har varit att göra en sammanställning och ge en översikt över KKS utifrån de vetenskapliga artiklar som berör ämnet. Målet med detta arbete är att öka förståelsen för KKS.

Resultatet pekar på att det är fler kvinnor än män som drabbas och att medelåldern för de med syndromet är ganska låg, runt 32 år. Det finns många olika teorier till orsaken och ingen är i dagsläget mer rätt än den andra. Detsamma gäller mätningar som använts för att försöka utreda syndromet, här framkommer många olika förslag på vägen till diagnostisering. Slutsatsen som dras är att mer forskning kring ämnet är nödvändigt för att få en förklaring och ett tillvägagångssätt för diagnostisering av syndromet.

Abstract:
Optimising outcomes for children with hearing impairment (HI) requires a family centred approach that prioritises parent involvement. Families must be provided with information to encourage participation; and meet their need for emotional support and knowledge. Diagnostic audiology reports can help provide this information, but their delivery alone is insufficient. If these reports are not readable and comprehendible they cannot meet national and international legal standards, nor can they support the health literacy of parents. The majority of New Zealand adults have insufficient health literacy skills, a concerning fact given the strong association between poor health literacy and negative health outcomes. The aim of this study was to evaluate a paediatric diagnostic audiology report, revise it and verify the revision. A mock audiology report was evaluated via a readability analysis and semi-structured interviews with parent participants. Results confirmed that the report was difficult to read and understand. Next, the report was revised using best practice guidelines and parental recommendations. Verification of the revision process with 32 participants revealed that parents who read the revised report had significantly greater comprehension, self-efficacy and perception ratings than parents who read the unrevised report. Additionally, the report’s readability was markedly improved. These results may have critical implications for parents and their children with HI. Incomprehensible audiology reports fail to support parental health literacy, promote understanding, encourage participation or offer emotional support. Because knowledge is power for these families, it is hoped that the findings of this study will be recognised and implemented into clinical practice.

The purpose of this study was to identify counseling competencies considered important for audiologists to possess to provide effective counseling in practice.To achieve this, researchers and clinicians with expertise in audiologic counseling, were recruited to participate in a three-round consensus survey. These experts were asked to generate and rate a list of counseling competency items they believed were important for audiologists to possess. In the final round of the study, final consensus was met on 64 counseling competency items (16 knowledge, 35 skills, and 13 attitudinal items) deemed important for audiologists to possess. The results of this study revealed that current practice guidelines in the field lack the clarity and detail necessary for audiologists to provide effective counseling in practice. Items that met consensus can inform counseling competencies students can acquire during pre-professional training. Future research is proposed to explore what is needed to move these competencies forward into training/practice and to help improve audiologic patient/family outcomes.

The employment of audiology assistants to relieve masters and doctoral level audiologists of routine tasks is a timely and controversial topic in our field. Berardino (2000) examined the roles of audiology assistants within Veteran’s Administration (VA) Hospitals using an e-mail survey that was sent out to VA audiologists. The results of that survey suggested that the majority of VA audiologists were in favor of the participation of audiology assistants in the clinic to varying degrees. The purpose of this survey was to determine the current attitudes of audiologists and otolaryngologists toward the role of audiology assistants in the hearing health care profession. The attitudes and opinions of otolaryngologists were of particular interest because this population had not been included in earlier surveys despite the fact that they often employ both audiologists and audiology assistants. The survey was e-mailed to a randomly selected group of audiologists and otolaryngologists. In addition to general opinion and demographic questions, participants were asked to rate specific audiology tasks on a six-point scale ranging from very appropriate to very inappropriate. Results indicate that audiologists and otolaryngologists generally agree on which tasks are appropriate for audiology assistants; however, audiologists feel audiology assistants may be a threat to the profession of audiology whereas otolaryngologists do not.

Eligibility reform in the VA and the resulting increased caseloads have brought about unacceptable wait times for new audiology appointments. Mandates to decrease wait times without increased resources have brought new demands on audiology clinics. One proposed solution is the addition of audiology technicians to VA audiology clinic practice. The first purpose of this study was to survey the attitudes of VA audiologists and service chiefs regarding the use of technicians. The second purpose was to ask the survey respondents to assign as generally appropriate or inappropriate, job duties which might be accomplished by an audiology technician. Following a pre-survey to pilot the questionnaire, the final survey was e-mailed to all members of a national e-mail group. Ninety-three acceptable responses were analyzed. The survey responses indicate that a large majority of the respondents hold positive attitudes concerning the use of technicians, believe technicians can accomplish some duties now done by audiologists and believe technicians can help reduce current appointment backlogs. The respondents assigned a number of clerical, assistive, minor hearing aid repair and other duties as appropriate for technicians and many testing, evaluation, programming, and perceived professional duties as inappropriate for technicians. There were several items which were not clearly assigned as either appropriate or inappropriate technician duties. Open-ended responses indicated that strong feelings, both positive and negative, exist within the audiology and service chief community. The survey results will be used to develop an audiology technician job description and to identify specific training needs for establishing audiology technician positions throughout the VA healthcare system.

Bakgrund: Buller är benämningen på icke önskvärda ljud och blir skadliga när ljudnivåerna blir tillräckligt höga. Effekterna av bullerexponering är många. Höga ljudnivåer förekommer bl.a. inom förskolor. Trots att detta är allmänt känt saknas det en del forskning kring hur just förskolepersonal upplever och påverkas av den höga ljudnivån på arbetet. Syfte: Syftet med studien är att undersöka ljudnivån på en förskola samt hur förskolepersonalen upplever och påverkas av ljudmiljön på arbetet. Metod: En egen tillverkad enkätundersökning riktad till förskolepersonal samt dosmätning genomfördes på en småbarnsavdelning och en äldrebarnsavdelning. Enkäten berörde frågor gällande ljudnivån på förskolan samt förskolepersonalens upplevelse och påverkan. Resultat: De ekvivalenta ljudnivåerna på småbarnsavdelningen låg mellan 73,2 – 78,5 dB(A) och på äldrebarnsavdelningen 71,7 – 78,1 dB(A). Impulstoppvärdena på småbarnsavdelningen låg mellan 110,6 – 135,7 dB(C) och på äldrebarnsavdelningen 116,5 – 143,4 dB(C). Majoriteten av förskolepersonalen upplever hög ljudnivå på arbetet dagligen. Slutsatser: Resultatet visar att förskolepersonalen inte utsätts för högre ekvivalenta ljudnivåer än tillåtet. Trots detta upplever majoriteten av förskolepersonalen att de dagligen utsätts för höga ljudnivåer på arbetet. En orsak till detta kan vara att impulstoppvärdena i vissa fall överskrider insatsvärdena. Denna studie är dock inte representativ för populationen, utan är enbart en deskriptiv studie för denna specifika förskola.

Modern sjukvård har blivit fragmenterad vilket medför nackdelar för patienter och personal. Teamarbete över professionsgränserna har identifierats som ett sätt att motverka fragmentering och erbjuda god heltäckande vård. Detta har relevans för hörselvården där många patienter behöver insatser från olika professioner. Arbetets syfte var att göra en kartläggning av teamarbete inom svensk offentlig hörselvård samt att undersöka hur teamarbetet ser ut. En totalundersökning har gjorts av Sveriges 21 landsting/regioner. Strukturerade intervjuer har genomförts med personer med kunskap om teamarbete inom varje landsting/region. Resultatet visar att teamarbete förekommer inom samtliga landsting/regioner men att både antalet team och deras arbetssätt varierar. De vanligaste är barnteam och team för hörselnedsättning och ytterligare funktionsnedsättning. De flesta team gör målsättning, planering och utvärdering gemensamt. I två av tre team uppges teammedlemmarna delvis kunna ersätta varandra. Olika professioner förekommer i hörselvårdens team. De som förekommer i störst utsträckning är audionomer, pedagoger och kuratorer. 69 % av teamen har en formell teamledare. Audionomer, läkare, kuratorer respektive chefer är teamledare i ungefär samma utsträckning. Interprofessionella aspekter dominerar men det går inte att kategorisera teamen som multi-, inter- eller tvärprofessionella då deras arbetssätt överlappar. Teamarbete upplevs som väsentligt i den offentliga hörselvården och anses fördelaktigt för både patienter och personal.

The aim of this study was to examine the effectiveness of different types of feedback on learning pure-tone audiometry using a simulator. Participants were 51 undergraduate students in the Department of Communication Disorders. Participants were randomly allocated to one of two groups whilst undertaking pure-tone audiometry training with the Clinical Audiology Simulator (CAS). One group received summative feedback during the learning task while the second group received formative feedback. Probes were administered to examine participants’ knowledge of pure-tone audiometry following training. In addition, a subjective workload analysis was used to measure perceived cognitive load during training and assessment. Between-groups analysis was conducted to establish the effect of feedback on learning and cognitive load. Data regarding how much time each student spent training on the simulator was also collected, and a regression analysis was conducted to evaluate the relationship between time and learning. Formative feedback was found to have a large positive effect on learning in comparison to summative feedback. Cognitive load was perceived as being higher for students receiving formative feedback during training compared to those receiving summative feedback. In subsequent assessment, the formative feedback group reported a lower cognitive load than the summative feedback group. No relationship was observed between time spent training on the simulator and probes outcome. The formative feedback training mode of the CAS proved to be effective in supporting learning and cognitive load in novice students. The findings suggest that the type of feedback employed when using simulators affects learning outcomes for users.

Purpose: Hearing loss is a common problem worldwide, and there is an ever- increasing need for more audiologists to be trained. Unfortunately, audiology students cannot always get the clinical experience they need during training. Virtual reality involving computer-based simulation of real-life training experiences is one way of compensating for this. While there are several virtual audiometers available for student use, few of these include the vital case history component. This study sought to develop an interactive virtual patient that includes this component, and to objectively measure the effect of training with this software on student performance. Method: Development of the Patient Simulator Program (PSP) took place in two phases – Phase One involved development of audiometric information and a brief case history summary for 25 patient cases, and Phase Two involved development of comprehensive case histories for these and identification of triggering phrases and keywords for eliciting each piece of information from the virtual patient. Twelve first year audiology students were recruited from the University of Canterbury and divided into matched groups based on their pre-test scores. An alternating treatment design across groups was used to evaluate participants on their verbal and written accuracy, experience, confidence, and efficiency scores on case history tasks. Results: A significant difference was found in verbal accuracy scores between groups at the mid-way assessment point (following simulator training), but not for written accuracy. Differences between groups were not significant at all assessment points for efficiency and experience measures. Confidence gains were greater for the second group to train with the simulator than the first, while performance gains were greater for the first group. Conclusion: These findings support the evidence that simulation training can enhance student’s skills, and provide the first objective evidence for the benefits of training for case history tasks with an interactive virtual patient. While the effect size was small, these findings are a promising springboard for future research into this area. While the PSP is not adequate to replace real clinical encounters, it has potential as an adjunct to the current training program.

Denna experimentella studie syftar till att undersöka huruvida en eventuell träningseffekt i 

frekvensdiskrimination kvarstår efter en tids träningsuppehåll. Tidigare forskning har visat 

tydliga samband mellan frekvensdiskriminering och talutveckling, kortikal signalbehandling 

och dyslexi. Det har klargjorts genom tidigare studier att förmågan att diskriminera frekvenser 

är möjligt att träna upp. Därmed är träning i frekvensdiskrimination en lämplig del i en 

behandlingsmetod med syfte att utveckla den auditiva perceptionen. Denna studie undersöker 

de praktiska förutsättningarna till denna behandlingsmetod genom att utreda varaktigheten av 

den eventuella träningseffekten 14 dagar efter träningen. 24 normalhörande personer deltog i 

studien där försökspersonernas förmåga att frekvensdiskriminera undersöktes med hjälp av ett 

egenutvecklat mjukvaruprogram. Först uppmättes försökspersonernas förmåga när de var 

otränade (mätning 1). Sedan genomfördes ett träningspass som följdes av en mätning 

(mätning 2). Efter 14 dagar genomfördes ytterligare en mätning för att undersöka den 

bestående effekten (mätning 3). 

Resultatet visade (när tre outliers exkluderats) en signifikant skillnad mellan mätning 1 och 2 

(p=0,034) vilket innebär att en omedelbar träningseffekt erhölls. Ingen signifikant skillnad 

kunde påvisas mellan mätning 2 och 3 (p=0,952) men en signifikant skillnad erhölls mellan 

mätning 1 och 3 (p=0,031) vilket påvisar att den omedelbara träningseffekten kvarstod efter 

de gångna 14 dagarna.

Bakgrund: Få barn föds varje år med dövblindhet som varierar i grader av syn- samt hörselnedsättning. Information om kommunikationsstrategier till pedagoger och närstående krävs och audionomen bör vara inläst på hur interaktion skapas mellan barnet, pedagogerna och närstående. Audionomen bör veta vilken roll pedagoger och närstående spelar i samverkan med barnet. Syfte: Att utifrån litteraturen belysa interaktionen mellan pedagoger och närstående och barnet med dövblindhet samt se på de karaktäristiska kommunikationssätten för barnet med dövblindhet. Metod: Litteraturstudie där kvalitativa och kvantitativa studier använts.                                Resultat: Två olika utbildningsmodeller hittades. En återkom i flera studier och byggde på lämpliga samt olämpliga beteenden, en beskrev tillvägagångssätten i interaktionen. Ett varierat resultat påvisades hos pedagogerna/närstående, mestadels positivt och ett fåtal mindre optimala resultat. En fallstudie beskrev interaktionen mellan 6 barn och deras föräldrar. Studien visar fungerande, om än i varierande grad, interaktion mellan samtliga barn och föräldrar. En studie behandlade mor- och farföräldrars roll i kommunikationen med barnen där oro inför umgänge med, och framtiden för, barnbarnen utrycktes. Två studier studerade interaktion mellan barnen och lärare. Den visade att interaktion fanns mellan parterna, med få initiativ från eleverna. Slutsatser: Pedagoger och närstående bör få kontinuerlig utbildning för utveckling av interaktion med barnet med dövblindhet

Bakgrund: Hörselskador är ofta förkommande hos personer med utvecklingsstörning, autism och inom autismliknande tillstånd, både medfödda och åldersrelaterade hörselskador. Ofta förblir de oupptäckta då dessa personer kan ha flera funktionsnedsättningar eller svårigheter att uttrycka sig. Dessa personer kan bo på en bostad med särskild service, och personal på dessa boenden behöver ha kunskap i hörselrelaterade ämnen för att de boendes behov ska tillgodoses. Syfte: Att belysa praktisk och teoretisk kunskap i ämnen som hörsel, hörselnedsättning, vissa hörhjälpmedel och kommunikation hos personal som arbetar på bostad med särskild service. Metod: För att belysa kunskapen hos personal i Karlskoga och Lindesbergs kommun användes en postenkät som mätinstrument i denna kvantitativa undersökning. Resultat: Personalen hade störst kunskap inom området kommunikation och vilken påverkan hörselnedsättningar kan ge. Sämre kunskap fanns att se inom området hörseltekniska hjälpmedel och hörapparater och om hur vanligt förekommande hörselnedsättningar är. Slutsats: Resultatet tyder på att det finns vissa brister i personalens kunskap, främst inom de områden som handlar om hörsel, hörselnedsättning och vissa hörhjälpmedel. Även om majoriteten av personalen hade bättre kunskap om kommunikation fanns det vissa brister även där. Utifrån detta resultat kan slutsatsen dras att personalen kan vara i behov av information och utbildning inom hörselrelaterade ämnen.

Aims and Objectives. This study aimed to describe auditory characteristics and balance function in patients with diabetes between 18-55 years of age as well as determine the association between patients' auditory and balance function with diabetes characteristics (type, duration and control). Background. Diabetes is one of the most prevalent non-communicable diseases worldwide with approximately 422 million people diagnosed globally. This number is projected to rise to 642 million by 2040 if no appropriate interventions are implemented to reverse the rise in the number of people with diabetes. South Africa has the second highest diabetes prevalence in Africa (after Nigeria) with 2.6 million cases. A rise in diabetes prevalence should be a concern for audiologists with increasing literature linking diabetes with the risk of acquiring hearing and balance disorders. However, there is currently a lack of research done in South Africa to investigate auditory and balance disorders in patients with diabetes. Therefore, the current study sought to investigate auditory characteristics and balance function in South African patients diagnosed with diabetes. It is anticipated that the study findings will yield evidence that will highlight the role of an audiologist in the clinical management of patients with diabetes. Research Design. The study utilised an observational cross-sectional matched groups design with a cohort (patients with diabetes) and control (volunteers without diabetes) group of participants. Participants were recruited from a Primary Health Care clinic in Polokwane, Limpopo using purposive and convenience sampling for the cohort and control group respectively. Methodology. Several methods were used to collect data pertinent to this study. These included case history interview and a medical folder review to obtain information related to participants' diabetes status. Furthermore, all participants underwent the following assessments: otoscopy, pure tone audiometry, diagnostic distortion product otoacoustic emissions (DPOAE), vision screening, peripheral neuropathy screening, Dynamic Gait Index test (DGI) and the Modified Clinical Test of Sensory Integration (MCTSIB). Data were analyzed using both descriptive and inferential statistical tests. Results. A total of 192 participants took part in this study; 110 in the cohort and 82 in the control group. There were similar distributions of gender in both groups with the following age distributions (in years) for each group; cohort; median =46, range =20-55, control; median =43, range =21-55. Pure tone audiometry assessments showed a significantly higher prevalence of hearing loss in the cohort (55%) when compared to the control (20%) group (p < .001). Participant age, gender and diabetes duration were associated with the likelihood of having hearing loss (age: odds ratio=2.90, 95% CI: 1.19-7.08, p=0.019; gender (male): odds ratio=.266, 95% CI: .104-.677, p=0.005; diabetes duration: odds ratio=1.12, 95% CI: 1.02-1.22, p=0.013). DPOAE assessments showed significantly higher percentages of abnormalities with signal to noise ratio (p < 0.01) and DPOAE level (p < 0.01) in the cohort compared to the control group. A significantly higher proportion (38%) of participants in the cohort group reported tinnitus when compared to 15% in the control (p < .001). Balance screening assessments with the DGI and the MCTSIB, showed significantly poorer performance in the cohort group than the control (DGI:, p < .001; MCTSIB: p < .001). Conclusion. Overall findings of this study showed that participants who were diagnosed with diabetes had a higher proportion of auditory and balance abnormalities when compared to those in the control group. Older age, male gender and longer duration since diabetes diagnosis were associated with a higher likelihood of having hearing loss. The findings of this study therefore suggest that auditory and balance dysfunction should be considered as comorbidities associated with diabetes. This study also highlighted the role of an audiologist in the managment of patients with diabetes with respect to early identification and management of auditory and balance dysfunctions amongst these patients.

The impact of Central Auditory Processing Disorders (CAPD) on language and communication of children and adults is well documented in research. Several tests and normative data exist to assess and diagnose for CAPD, but these tests are not valid for use in South Africa due mismatch in the language or accent of the test recording and the language spoken in South Africa as well as normative data that were collected in the United States of America. As such CAPD may remain undiagnosed, misdiagnosed or designed management plans may be inappropriate as they arose from invalid tests. This study employed a developmental study design in order to adapt an existing 2-pair dichotic digit test (2-pair DDT) to a South African English accent to better suit the South African context. The study further investigated the effect of accent on dichotic digit test performance by comparing the performance of South African adults on the newly adapted South African accent 2-pair DDT and the existing 2-pair DDT, specified by the Department of Veteran Affairs (DVA), using a Wilcoxon-signed ranked test. Lastly, the study collected normative data for the 2-pair DDT that can be used in South Africa. The study successfully used a developmental study design to produce a South African accent 2-pair DDT that meets the international specifications as outlined by the DVA. A Wilcoxon signed ranked test showed that South Africans performed better on the South African accent 2- pair DDT than on the DVA specified test. The latter finding was supported by qualitative reports from the participants who found the DVA specified 2-pair DDT stimuli to be more difficult to listen to as compared to the South African accent test. Furthermore, the study produced preliminary normative data for the South African accent 2-pair DDT that met international standards and may be used to score 2-pair DDT performance. The successful use of the developmental study design to adapt the 2-pair DDT shows promising possibilities as a systematic process for adapting or developing tests that are more contextually appropriate for various contexts.

This research study stemmed from my experience as a community-based Audiologist in the Mitchell's Plain sub-district in Cape Town, South Africa. During this time, I worked closely with the local school nurses and witnessed first-hand the many challenges that these health workers are faced with. Through my collaborative work with the Mitchell's Plain school health nurses I identified the urgent need for a contextually relevant hearing screening protocol to assist the school nurses in improving the current state of the school-based hearing screening service. I therefore embarked on this research process taking on the role of a 'clinician-researcher' who had prior relationships with the primary research participants i.e. the local school nurses.

Abstract:
Aims and Objectives. This study aimed to describe auditory characteristics and balance function in patients with diabetes between 18-55 years of age as well as determine the association between patients' auditory and balance function with diabetes characteristics (type, duration and control). Background. Diabetes is one of the most prevalent non-communicable diseases worldwide with approximately 422 million people diagnosed globally. This number is projected to rise to 642 million by 2040 if no appropriate interventions are implemented to reverse the rise in the number of people with diabetes. South Africa has the second highest diabetes prevalence in Africa (after Nigeria) with 2.6 million cases. A rise in diabetes prevalence should be a concern for audiologists with increasing literature linking diabetes with the risk of acquiring hearing and balance disorders. However, there is currently a lack of research done in South Africa to investigate auditory and balance disorders in patients with diabetes. Therefore, the current study sought to investigate auditory characteristics and balance function in South African patients diagnosed with diabetes. It is anticipated that the study findings will yield evidence that will highlight the role of an audiologist in the clinical management of patients with diabetes. Research Design. The study utilised an observational cross-sectional matched groups design with a cohort (patients with diabetes) and control (volunteers without diabetes) group of participants. Participants were recruited from a Primary Health Care clinic in Polokwane, Limpopo using purposive and convenience sampling for the cohort and control group respectively. Methodology. Several methods were used to collect data pertinent to this study. These included case history interview and a medical folder review to obtain information related to participants' diabetes status. Furthermore, all participants underwent the following assessments: otoscopy, pure tone audiometry, diagnostic distortion product otoacoustic emissions (DPOAE), vision screening, peripheral neuropathy screening, Dynamic Gait Index test (DGI) and the Modified Clinical Test of Sensory Integration (MCTSIB). Data were analyzed using both descriptive and inferential statistical tests. Results. A total of 192 participants took part in this study; 110 in the cohort and 82 in the control group. There were similar distributions of gender in both groups with the following age distributions (in years) for each group; cohort; median =46, range =20-55, control; median =43, range =21-55. Pure tone audiometry assessments showed a significantly higher prevalence of hearing loss in the cohort (55%) when compared to the control (20%) group (p < .001). Participant age, gender and diabetes duration were associated with the likelihood of having hearing loss (age: odds ratio=2.90, 95% CI: 1.19-7.08, p=0.019; gender (male): odds ratio=.266, 95% CI: .104-.677, p=0.005; diabetes duration: odds ratio=1.12, 95% CI: 1.02-1.22, p=0.013). DPOAE assessments showed significantly higher percentages of abnormalities with signal to noise ratio (p < 0.01) and DPOAE level (p < 0.01) in the cohort compared to the control group. A significantly higher proportion (38%) of participants in the cohort group reported tinnitus when compared to 15% in the control (p < .001). Balance screening assessments with the DGI and the MCTSIB, showed significantly poorer performance in the cohort group than the control (DGI:, p < .001; MCTSIB: p < .001). Conclusion. Overall findings of this study showed that participants who were diagnosed with diabetes had a higher proportion of auditory and balance abnormalities when compared to those in the control group. Older age, male gender and longer duration since diabetes diagnosis were associated with a higher likelihood of having hearing loss. The findings of this study therefore suggest that auditory and balance dysfunction should be considered as comorbidities associated with diabetes. This study also highlighted the role of an audiologist in the managment of patients with diabetes with respect to early identification and management of auditory and balance dysfunctions amongst these patients.
