Birth centers are maternal care models that use appropriate technology when providing care to birthing women. This descriptive study aimed to characterize intrapartum care in a freestanding birth center, in light of the practices recommended by the World Health Organization (WHO), with 1,079 assisted births from 2006 to 2009 in the Sapopemba Birth Center, São Paulo, Brazil. Results included the use of intermittent auscultation (mean=7 controls); maternal positions during delivery: semi-sitting (82.3%), side-lying (16.0%), other positions (1.7%), oral intake (95.6%); companionship (93.3%); exposure to up to three vaginal examinations (85.4%), shower bathing (84.0%), walking (68.0%), massage (60.1%), exercising with a Swiss ball (51.7%); amniotomy (53.4%), oxytocin use during the first (31.0%) and second stages of labor (25.8%), bath immersion (29.3%) and episiotomy (14.1%). In this birth center, care providers used practices recommended by the WHO, although some practices might have been applied less frequently.

Independence on the subject has begun to develop, the subject at home was not willing to be bathed again, when the bath time the subject will ask for toiletries and then bathed himself, and after bathing, the subject will use his own clothes, but the clothes still need to be prepared before. This research uses research with qualitative of case study. This research data form of interview result, observation result and written sources. The data were collected using interview and observation techniques. The data analysis activity is done by thematic analysis.

The study of fiqh is indeed a very vital material. Because this is related to how Islamic law is carried out. Especially about the worship of mahdhah, therefore every Muslim is obliged to learn to understand it so that at the level of implementation it can run well by religious orders. This service activity was carried out at the At-Taqwa taklim assembly in the hamlet of Lestari Jaya which was the location of the KKL of STAI Sangatta East Kutai students related to strengthening and mentoring religious fiqh. In its implementation, of course, there is planning, implementation, and evaluation that must be considered in compiling and implementing a program. The result of this activity is an increase in the knowledge of the members of the taklim assembly related to ablution, obligatory bathing, and prayer. The enthusiasm of the assisted participants also showed a positive attitude so in this activity it is necessary to follow up with similar activities by discussing other topics.

Abstract:
The Community Service program activity of the Electricity and Renewable Energy Faculty lecturer team at the PLN Technology Institute for the 2019/2020 fiscal year was carried out in Sukadana village, Ciomas district, Serang region, Banten precisely at RT 011 RW 003 Cilatak hamlet. The selection of Sukadana village as a place to implement Community Service is because this village is still experiencing problems in the construction and development facilities and infrastructure. The problems that are currently being experienced by villagers regarding the clean water distribution for ablution of villagers in the mosque. During this time for ablution in mosque, villagers only rely on water from mountain springs whose distribution relies on gravitational pressure through a plastic hose and accommodated in a large tub located beside the mosque. In addition to being used for ablution, water is also used by villagers to bathing, washing, etc., so that when used for ablution sometimes it is no longer clean because it has been mixed with soapy water used for bathing and washing. Based on those problems, our team provided solution to partner in the form of infrastructure development and clean water supply by reconfiguring the clean water distribution installation for ablution in the mosque by installing solar water pump. The method used in the implementation of this Community Service activity is to approach the villagers during the survey through interviews by providing solution to the problem and conducting questionnaires and socialization in the form of a brief education. The results of the activities have been done by installing 2 units of DC water pump @180 Watt, 4 units of polycrystalline solar modules @100 Wp, 2 units of battery @100 Ah 12 V and 2 units of solar charge controller. With the implementation of Community Service activities in Sukadana village, aside from being able to help overcome the problems faced by partner, besides that it can provide progress and development of infrastructure for partner village and the existence of a partnership that makes that village as IT PLN assisted village.

Abstract:
Abstract Background CHG bathing is used to suppress patients’ microbial skin colonization, in order to prevent infections and transmission of multidrug-resistant organisms. Prior work has suggested that microbial growth is inhibited when CHG skin concentrations exceed threshold levels. Methods We conducted 6 single-day surveys from January 2018 to February 2019 in 7 academic hospital MICUs with established CHG patient bathing. Adult patients were eligible to have skin swabbed from adjacent 25 cm2 areas on the neck, axilla, and inguinal region for culture and CHG concentration determination. CHG skin concentrations were measured by a semi-quantitative colorimetric assay. Selective media were used to isolate targeted microorganisms (Table 1). Species were confirmed by matrix-assisted laser desorption ionization time-of-flight mass spectrometry; antibiotic susceptibility was determined by MicroScan (Beckman Coulter). We modeled the relationship between CHG skin concentrations (log2-transformed) and microorganism recovery (yes/no as primary outcome) using multilevel models controlling for clustering of body sites within patients and within ICUs, assessing slope and threshold effects. Results We enrolled 736/759 (97%) patients and sampled 2176 skin sites. Gram-positive bacteria were detected most frequently (Table 1). The adjusted odds of identifying gram-positive organisms decreased linearly as CHG skin levels increased (Figure 1a), without evidence of a threshold effect. We also found significant negative linear slopes without evidence of threshold effects for other pathogens tested (Table 2; Figure 1), with the exception of gram-negative bacteria and vancomycin-resistant enterococci. When modeling quantitative culture results (colony-forming units) for gram-positive organisms as a continuous outcome variable, a similar relationship was found. Conclusion Higher concentrations of CHG were associated with less frequent recovery of gram-positive bacteria and Candida species on the skin of MICU patients who were bathed routinely with CHG. For microbial inhibition, we did not identify a threshold concentration of CHG on the skin; rather, increasing CHG skin concentrations led to additional gains in inhibition. For infection prevention, aiming for high CHG skin levels may be beneficial. Disclosures All authors: No reported disclosures.

Abstract:
The objective of this thesis was to examine the best available evidence concerning how to minimise agitated behaviours in older adults with dementia who are being assisted with their bathing. Specifically, the goal was to develop a clinical guideline based on the identified evidence for formal and informal caregivers and nurses who supervise non-qualified caregivers. The comprehensive systematic review method was used to determine the best available evidence on this topic. To identify multi-dimensional evidence that can be used to develop useful and context-specific practices, textual opinion papers were considered as alternative sources of “knowing how” type of knowledge to augment the qualitative and quantitative evidence. A search of various databases and hand-searching identified 299 papers. Twenty-eight papers were assessed for methodological quality and from this 15 papers were included for the review: five experimental and quasi-experimental studies, two qualitative studies and eight textual opinion papers. In the quantitative component, statistical pooling was not possible due to the clinical and methodological heterogeneities of the studies, and consequently findings were analysed in a narrative format. The identified quantitative evidence supports the person-centred showering approach and towel bath/thermal bath both for reducing agitated behaviours of people with dementia and improving caregivers’ psychological status. Playing the preferred music of older adults who have dementia is also recommended for reducing the frequency of aggressive behaviours during bathing. Regarding the qualitative component, 13 findings from the two included studies were aggregated into five categories and two meta-syntheses: 1) There should be a strategy to facilitate residents’ sense of control and caregivers should have relevant assessment and communication skills; and 2) There should be a strategy to promote safe assisted bathing while the patient’s dignity is respected. In the textual component, the eight included papers were aggregated into 63 findings and 14 categories, and from these four meta-syntheses were developed: 1) Assisted bathing should be considered an opportunity for positive and respectful human interaction with a resident in order to minimise aggressive behaviour; 2) A creative and flexible care plan that is implemented with organisational support should be in place to provide effective assistance in minimising aggressive behaviours during bathing; 3) Practice should change from a rigid custodial task-oriented approach with a management perspective to a therapeutic person-centred approach with a prevention focus; 4) There should be a strategy to provide a safe, comfortable and private bathing environment with effective and appropriate management of orthopaedic pain. Finally, a Best Practice Information Sheet was developed based on the evidence from the three components of the review to inform and support caregivers who provide direct assistance to people with dementia. The project successfully demonstrated that textual evidence can contribute to the identification of ‘knowing how’ type of evidence where there is a lack of qualitative evidence. The evidence-based healthcare movement has been criticised for failing to meet the needs of clinicians in the reality of everyday practice. In order to fill the evidence and practice gap, healthcare practice should be informed by multiple types of evidence, namely quantitative evidence, qualitative evidence and textual evidence.

Esta tese de doutoramento apresenta trabalho desenvolvido no domínio de Ambient Assisted Living, mais especificamente no cuidado de pessoas acamadas. Numa visão global foram definidas as áreas dos cuidados requeridos pelos acamados que devem ser tidas em consideração, nomeadamente: higiene, conforto, segurança, mobilidade e assistência médica. Normalmente são necessários dois cuidadores, para executar o cuidado de um acamado. Nesta tese, apresenta-se uma solução para que um único cuidador seja capaz de realizar os cuidados de higiene de um acamado. Portanto, todo o estudo está direcionado à perspetiva do utilizador principal, o cuidador, que é o responsável pelas tarefas, sofrendo desgaste físico e psicológico ao longo do tempo. Tendo em conta o elevado espectro para possível atuação e desenvolvimento de uma tese de doutoramento em Ambient Assisted Living, houve necessidade de focar o trabalho numa área específica. Para isso, foram efetuados estudos que são apresentados neste documento, respeitantes a opiniões de vários cuidadores, que os próprios materializaram num questionário que lhes foi direcionado. Com base nos estudos efetuados, concluiu-se que a área mais importante para desenvolver uma solução de equipamento, para ser utilizado pelos cuidadores, é o banho. Sendo assim, esta tese de doutoramento tem como objetivo principal apresentar uma nova metodologia para auxiliar os cuidadores na atividade do banho em ambiente domiciliário. No contexto referido, procedeu-se assim ao projeto de um sistema de banho composto por dois módulos: um dos módulos serve para aquecimento e fornecimento de água à temperatura e caudal necessários para o banho e o outro, que consiste num colchão insuflável, serve para permitir dar o banho ao acamado utilizando a própria cama como suporte físico para esse efeito. Com base nos estudos sistemáticos que foram feitos – testes do protótipo construído em instituições que prestam serviços de cuidados domiciliares – procedeu-se ao desenvolvimento e proposta de uma metodologia para dar banho a acamados. Esta metodologia é sistemática, genérica e não depende do sistema de banho utilizado. É, por isso, um dos principais contributos desta tese de doutoramento.
This doctoral thesis presents the work developed in the field of Ambient Assisted Living, more specifically in the caregiving of bedridden people. From a global viewpoint caregiving of bedridden comprises the following areas: hygiene, comfort, safety, mobility and medical care. Usually two caregivers are required for nursing a bedridden patient. In the framework of this thesis it is aimed to develop applications, which enable one person to perform all care tasks of a bedridden. Therefore, the entire study takes the main user’s perspective into account, the caregiver, who is responsible for a wide range of tasks, suffering physical and psychological exhaustion over time. Considering the high spectrum of possible operational fields and the elaboration of a doctoral thesis in Ambient Assisted Living, it was necessary to focus the work in one specific area. Preliminary studies were carried out by means of questionnaires to collect caregiver’s opinions. Based on these studies, it was concluded that the most important area to develop a novel equipment solution being used solely by one caregiver is the bath. Thus, the main objective of this doctoral thesis is the development of a methodology to assist caregivers during bathing in a home environment. In this respect, the new design of a bathing system is made up of two modules: i) heating and water supply at a defined temperature and flowrate required for the bath, and ii) an inflatable mattress, which enables to bath a bedridden while using the patient’s bed as physical support. Based on subsequent systematic studies that have been conducted by to test the prototype in institutions that provide home care services, the development also provided a new procedure that intended to assist the bathing bedridden patients. This methodology is systematic, generic and does not depend on the bathing system used. It is therefore one of the main contributions of this doctoral thesis.

Self-care skills are the everyday tasks undertaken so children are ready to participate in life activities. While they are typically supported by parents or carers in young children, it is expected that children develop independence while getting older and maturing. Self-care skills are assumed as precursors for most school-related tasks, are expected to be performed independently, and usually it becomes inappropriate for others to assist for tasks related to these. Assistive technology enhances to strengthen children's ability to participate in eating, dressing, bathing, and personal hygiene routines. As a self-management tool, assistive technology helps children with pervasive developmental disorders or mild intellectual disabilities promote to perform daily tasks involved in activities related to self-care skills. In this chapter, first, information about self-care problems with a focus on children with pervasive developmental disorders is given. Then, opportunities, challenges, and assistive tools are presented. Finally, future research directions in this domain are stated.
