Abstract:
In Australia, local government plays an essential role in emergency management, although not a provider of emergency services. The role of supporting emergency services and the community both during and after emergencies has been a traditional role. Added to this is an increasing responsibility as the focal point for the conduct of local mitigation using risk analysis, prioritization, and treatment under the methodology of emergency risk management. This role is part of a shift in the emphasis of emergency management in Australia away from the strong focus on emergencies and the emergency services, towards an emphasis on the sustainability of the community and its life in the context of the risk of loss posed by natural and other hazards. Models of municipal emergency risk management planning are presented to assist municipalities to connect or even integrate their emergency management planning processes with other similar community safety activities such as crime and injury prevention.

Cancer and its treatment include many medical emergencies. Hence taking care of these emergencies presents a challenge not only to the clinicians but also to the medical oncologists. Cancer patients may have complex medical problems in addition to cancer such as coronary heart diseases, diabetes mellitus, and respiratory diseases. Such patients require immediate medical assistance and emergency care facilities to improvise their health condition. The present review paper focuses on more commonly confronted emergencies in cancer patients and their related management.

Abstract:
Introduction The aim of the study was to assess the frequency, features and management of dental emergencies at sea in France. Methods A descriptive study was carried out by retrospectively examining medical records of patients who were assisted by the French maritime TeleMedical Assistance Service (TMAS) from 2012 to 2016. Data were ranked in different categories: socio-demographic data, diagnosis, prescription, and monitoring or treatment prescribed. Results The TMAS recorded 9122 medical files for all medical emergencies. Among these medical records 135 concerned oral diseases. The main causes for dental emergencies are dental abscess (51.8%), tooth decay (33.3%), and dental fracture (8.9%). Even where teledentistry is validated for remote screening and oral lesion diagnosis, management of dental emergencies mostly requires a dental procedure. On board, without special equipment and/or specifically trained healthcare workers, this management often results in the prescription of medication. Discussion The International Medical Guide for Ships published by the World Health Organization could be updated to suit the latest recommendations of dental emergency management. This could facilitate the addition of a medical act to dental management, resulting in more effective treatment. Furthermore, simple and specific equipment could be added to the medical supplies.

AbstractRecently, Indonesia has experienced six major provincial, civil, armed conflicts. Underlying causes include the transmigration policy, sectarian disputes, the Asian economic crisis, fall of authoritarian rule, and a backlash against civil and military abuses. The public health impact involves the displacement nationwide of >1.2 million persons. Violence in the Malukus, Timor, and Kalimantan has sparked the greatest population movements such that five provinces in Indonesia each now harbor > 100,000 internally displaced persons. With a background of government instability, hyperinflation, macroeconomic collapse, and elusive political solutions, these civil armed conflicts are ripe for persistence as complex emergencies.Indonesia has made substantial progress in domestic disaster management with the establishment of central administrative authority, strategic planning, and training programs. Nevertheless, the Indonesian experience reveals recurrent issues in international humanitarian health assistance. Clinical care remains complicated by absences of treatment protocols, inappropriate drug use, high procedural complication rates, and variable referral practices. Epidemiological surveillance remains complicated by unsettled clinical case definitions, non-standardized case management of diseases with epidemic potential, variable outbreak management protocols, and inadequate epidemiological analytic capacity. International donor support has been semi-selective, insufficient, and late.The militia murders of three UN staff in West Timor prompted the withdrawal of UN international staff from West Timor for nearly a year to date. Re-establishing rules of engagement for humanitarian health workers must address security, public health, and clinical threats.

Gemelli,” has led to several laws of the Constitution of the Italian Republic (Article 35), the Criminal Code (Articles 437 and 451), and the Civil Code (Article 2087): The law of Feb 12, 1955, #51 (Official Gazette 54, March 7, 1955); Decree 547 of April 27, 1955 “Regulations for the prevention of industrial accidents” and Decree 302 of March 19, 1956.The study of these assistance procedures has led to the formulation of an emergency plan divided into two basic parts: prevention of accidents, and rescue operations.By prevention we mean: a) The training of all staff and Managing Bodies of our Faculty/Staff to take effective action in the case of an intra-hospital accident, b) The need for detailed information on the various procedures to be used depending on the gravity of the event, c) Awareness of the civil and criminal responsibilities relating to the various management and non-management levels in case of emergency.The training plan for the whole staff, approved by the management administrative, technical and medical bodies, consists of theoretical and practical courses. These courses, which started in June 1983 are “modular.” The staff will take part in them as homogeneous groups, for a period of time which will vary, in regard to the number of hours and kind of instruction, and in relation to what each group has to learn about work safety, fire prevention and emergency measures.

IntroductionAlmost 78 000 women gave birth in the state of Victoria, Australia, in 2019. While most births occurred in metropolitan Melbourne and large regional centres, a significant proportion of women birthed in rural services. In late 2016, to support clinicians to recognise and respond to clinical deterioration, the Victorian government mandated provision of an emergency training programme, called Maternity and Newborn Emergencies (MANE), to rural and regional maternity services across the state. This paper describes the evaluation of MANE.Design and settingA quasi-experimental study design was used; the Kirkpatrick Evaluation Model provided the framework.ParticipantsParticipants came from the 17 rural and regional Victorian maternity services who received MANE in 2018 and/or 2019.Outcome measuresBaseline data were collected from MANE attendees before MANE delivery, and at four time points up to 12 months post-delivery. Clinicians’ knowledge of the MANE learning objectives, and confidence ratings regarding the emergencies covered in MANE were evaluated. The Safety Attitudes Questionnaire (SAQ) assessed safety climate pre-MANE and 6 months post-MANE among all maternity providers at the sites.ResultsImmediately post-MANE, most attendees reported increased confidence to escalate clinical concerns (n=251/259). Knowledge in the non-technical and practical aspects of the programme increased. Management of perinatal emergencies was viewed as equally stressful pre-MANE and post-MANE, but confidence to manage these emergencies increased post-delivery. Pre-MANE SAQ scores showed consistently strong and poor performing services. Six months post-MANE, some services showed improvements in SAQ scores indicative of improved safety climate.ConclusionMANE delivery resulted in both short-term and sustained improvements in knowledge of, and confidence in, maternity emergencies. Further investigation of the SAQ across Victoria may facilitate identification of services with a poor safety climate who could benefit from frequent targeted interventions (such as the MANE programme) at these sites.

This study aims to explore how the Riau Provincial Government manages unexpected expenses in the face of the Covid-19 pandemic. The method in this study is a qualitative method with the type of case study. Data collection techniques used are interviews, observation, and documentation. The results showed that the management of unexpected costs was carried out through the planning stage by reallocating and centralizing the budget. At the administrative and accountability stage, there are problems in recording Unexpected Expenditures, namely the absence of clear technical guidelines regarding the implementation of Unexpected Expenditures, setting spending limits for emergencies. There is no valid data for the distribution of aid funds for MSMEs affected by Covid-19 as well as valid documents for recording third party grant assistance. This research contributes to the government in making policies in financial management in disaster emergencies. Abstract:
The combination of modern, technological hazards and overlapping government jurisdictions requires coordinated, multijurisdictional emergency management. The Three Mile Island incident clearly demonstrated the impact of technical hazards and the importance of intergovernmental cooperation. A method is required to understand intergovernmental considerations in emergency management. This thesis derives such a method by proposing a three component model. The first component considers that all intergovernmental relationships are dynamic. Efforts to describe intergovernmental systems in terms of fixed relationships are not accurate. Rather intergovernmental systems are better described by the concept of movement along a scale between relationships that are separate and distinct and relationships that overlap and are interdependent. Relationships change along the scale depending on the strength of case-specific influencing variables. Identification and use of windows of opportunity describes the second component of the model. Institutional opportunists in favor of cooperative, intergovernmental programs must be able to identify and act when opportunities exist. Understanding this second component improves the chances of implementing lasting, cooperative intergovernmental results. The final component of the model emphasizes that by taking advantage of system change at the optimal time, linkages can be established between multiple jurisdictions. In multi-jurisdictional emergency management these linkages are made by integrating emergency plans and procedures. Applying the model by utilizing a case study in multijurisdictional emergency management completes this thesis. The case study documented is an intergovernmental cooperative planning effort between the Department of Energys Savannah River Operations Office and the states of South Carolina and Georgia.

One of the common lessons learned from the 11 September Terrorist Attacks in 2001, Southeast Asia Tsunami in 2004 and Hurricane Katrina in 2005, was there were major command and control (C2) and information challenges during the crisis response efforts. The Department of Defense (DoD) is currently transitioning to face these global threats of terrorism and natural disasters, as well as support the goals of the new National Strategy, by developing new plans and procedures to improve the coordination, communications and operations between DoD and other entities when responding simultaneously to such complex humanitarian disasters (CHD). In searching for a mobile and adoptable communication solution for military operations, the DoD should consider a C2 system that utilizes advanced commercial-off-the-shelf (COTS) technology. Hastilyformed networks (HFN) could provide a global broadband network node with internet, voice, video and data capability in a rapidly deployable manner, which offer significant advantages to military and other crisis response activities. The focus of this thesis concentrates on the financial aspects of HFNs in support of humanitarian assistance and/or disaster relief (HA/DR) efforts by U.S. armed forces. This research and analysis of HFNs could present prospective benefits to DoD, which include cost-savings, enhanced emergency response capabilities and improved interagency/international relations. Additionally, this study will provide a recommended model methodology and iterations for future military-use of HFNs in support of the DoD's vision of "transformation."

The thesis develops a coherent and comprehensive supply chain management framework for managing health care goods provided as humanitarian assistance in complex political emergencies. The objective of the research is improving the effectiveness and efficiency of logistics services which enable humanitarian organizations to reduce suffering, morbidity, disability and mortality of populations affected by armed conflicts. The framework is deduced from models and concepts developed for commercial logistics and supply chain management by considering the objectives and constraints of humanitarian organizations, the context in which they work as well as the characteristics of health care goods. For the logistics processes of item selection, sourcing, storage, transport and customer service frameworks are developed at the levels of supply network design, supply chain planning and supply chain operations. For each category, decision criteria are proposed according to the phase of the crisis, the distance from the crisis area as well as the criticality of health care goods. The framework allows humanitarian organizations to develop strategic, tactical as well as operational plans for providing logistical support to specific humanitarian assistance programmes within their constraints and in the context of the respective complex political emergency. A more universal approach to an understanding of menopause can offer health professionals a broader view of the phenomenon. Menopause is an important time in a woman's life. As her body is going through changes that can affect her social life, her feelings about herself and her ability to work. In the past, menopause was often surrounded by misconceptions and myths. Now, it is recognized that menopause is a natural step in the process of aging. However, different ethnic cultures perceive and experience menopause differently. Women in some Asian cultures, such as Chinese, Indian and Laotian cultures hold more esteemed positions in their culture when they are older and menopausal, while western societies may devalue the role of women as they age. The purpose of this thesis was to examine the perceptions and the experiences of women with Laotian cultural backgrounds living in Victoria, Australia, in regards to how they manage menopause, and to examine the Laotian cultural influence on the perceptions of menopause. The research methodology was decided to use surveys or structured interviews as the means of collecting data. The survey was used to collect demographic data, and record social demographic and cultural factors, menopausal symptoms and menopausal management. Data was collected from 55 Laotian women residing in Victoria. Participating women were recruited by word of mouth, and through a Laotian community representative of Victoria. One of the three methods, such as interviewer-administered questionnaire, self-administered questionnaire and telephone interviewing was used to survey Laotian women who were aged between 45 and 65years old.

AbstractThe emergency department of any institution is an entry point for a significant number of patients to any health care organization. The department caters to various trauma and medical emergencies in both adults and in children round the clock and is adequately staffed with emergency physicians, and nursing to handle such emergencies at all times and days. The department also oversees operations of the prehospital emergency medical services (ambulance) and coordinates their services.The emergency department (ED) is considered particularly high risk for adverse events (AE): 60% of ED patients experienced Medication Error (Patanwala et al., Ann Emerg Med 55:522–526, 2010). From a systematically review about AE related to ED, appears that the prevalence of AE among hospitalized patients ranging from 2.9% to 16.6%, with 36.9% to 51% of events considered preventable (Stang et al., PLoS One 8:e74214, 2013).Maintaining quality and developing error-free systems have been the focus of engineering over the last few decades.Consider the degree of variability of every individual human being compared to machine and also wisdoms from engineering field, for error-free system that guarantees good quality assistance should be defined a program reasonably simple, locally relevant, easily implementable, not be resource intense and have tangible outcomes which can be measured.

Prolonged high-mortality crises common in the Cold War era have become rare, corresponding with an increase in international spending, a healthier world, and the politicization of humanitarian assistance. This has created a completely new environment for relief workers in the twenty-first century. This new environment requires an emphasis on chronic diseases, urban settings, and people displaced within their own country. The relative importance of natural disasters has also resulted in a new set of skills deemed essential for humanitarian relief. The concepts of disaster preparedness, risk management, and disaster recovery have become central to the humanitarian endeavour and often these responsibilities fall on the shoulders of host governments. This chapter reviews the new environment in which humanitarian relief exists, the dominant emerging themes, and some of the promising technical advances.

The coexistence of psychiatric disorders in patients with medical illnesses may influence both the diagnosis and the course of the illness by their effects on pathophysiological, diagnostic, and therapeutic processes. There may also be effects on patients’ collaboration with treatment and on their relationships with health care staff. Several factors change the management of, medical illnesses and psychiatric disorders, and their inter-relation ♦ increased life-expectancy and increasing survival of people with-severe illness alter the risk of other medical and psychiatric disorders; ♦ social changes affecting family structure can affect care giving. Other social factors include changes in the role of women (work, delayed maternity); increased immigration with consequent cultural diversity including different concepts of medical and psychiatric disorders (see Chapter 1.3.2); ♦ increased use of medication in medical and in psychiatric treatment, and changes in the organization of health care and social assistance from hospital-based to community-based. This chapter describes how to recognize, treat and manage psychiatric disorders in medical illnesses.

The difficult airway can be challenging to deal with in any setting. In resource-poor environments pathologies are often advanced, specialist equipment may be absent and trained assistance may be lacking, adding to the challenge. The chapter offers practical advice on assessment, planning, and management of difficult airways under these circumstances. Useful techniques considered include spontaneously breathing ketamine anaesthesia, airway topicalization for awake airway management techniques, blind nasal intubation, and awake tracheostomy. Common airway pathologies that may be encountered are described. Practical considerations regarding decision making for tracheostomy, care of tracheostomy on the ward, and management of tracheostomy emergencies are also discussed.

This research in Phase II of its development aims to review, explore, identify and map the PLANCON Contingency Plans for Social Assistance prepared in the COVID-19 pandemic between 2020 and 2021 in the capitals of 26 States and DF to identify and explore the logical questions, techniques and policies for planning and managing preventive and mitigating measures for protection and civil defense in response to an emergency situation. Considered a natural disaster, the pandemic was inserted in a biological subgroup, with a sudden, significant and transitory increase in the occurrence of infectious diseases generated by viruses in the Brazilian Classification and Codification of COBRADE Disasters. The capillarity of the Single System of Social Assistance (SUAS) in the Brazilian social protection policy, in connection with the Single Health System (SUS) makes it possible to promote the development of two PLANCONs of Social Assistance for action on Covid-19 in 27 states and 5,570 municipalities. In Brazil, the Contingency Plans are foreseen in the National Civil Protection and Defense Policy to define procedures, actions and decisions in response to emergencies. As a preponderantly qualitative research and type of study based on a literature review, it is expected, with the support of the digital tool of the webQDA software, to optimize the systematization of two data in a collaborative and distributed environment, essential in the interaction of geographically distant teams. As a propaedeutic result, it will be verified different levels of obstacles not accessible to the planes in the surveyed capitals. The global unpreparedness not confronting contingencies results in different variables that affect the results not planning and management in the areas of health, economy, education, social assistance, among others. As a result, after the completion of all the phases (I, II, III and IV), it is expected that the research could contribute to support the reflection and debate of new working methodologies in the management of social protection public policies.

Relief, rehabilitation, and recovery from climate emergencies require getting the governance of disaster and crisis management right. In Pakistan, there are five actions where response actors can either contribute directly, or facilitate action to enable effective interventions: Support the collection, coordination, and dissemination of data relating to the crisis; Help regenerate multi-sectoral cooperation and partnerships; Assist the vertical integration of institutions at the sub-national level; Urge coordinated resources across response actors: donors, government officials, and civil society; Strengthen social protection systems in the longer term. Most of these are familiar to those that work on humanitarian crises in Pakistan, but they represent unresolved bottlenecks in responding effectively to a crisis. Getting these areas of action right is critical for the current crisis and to prepare for other expected and accelerating climate emergencies. To expand on these five elements, this brief draws on the authors' experiences of national and international responses to previous disasters and their aftermaths in Pakistan.
