AbstractAlthough not as prevalent as in Caucasian rhinoplasty, correction of a hump nose also poses a significant problem for East Asian rhinoplasty. The hump noses in East Asians can be classified as generalized humps, isolated humps, and relative humps with a low tip. Appropriate management of the Asian hump nose demands a good understanding of the anatomical characteristics unique to East Asians. As the East Asians generally prefer to have a nose with a prominent tip and well-augmented nasal dorsum, the concept of redistribution is significant in managing convex dorsum of the East Asians. Hump reduction should be done in a conservative and incremental manner, including careful removal of the bony cap and cartilaginous hump. Tip augmentation using a septal extension graft is an essential maneuver to achieve an aesthetically pleasing tip height, which has a tremendous impact on the overall aesthetic outcome. Midvault reconstruction, dorsal augmentation, and camouflage are additional key elements for the success of the operation. Complications such as undercorrection and dorsal irregularities are not uncommon, requiring surgeons to take utmost care to address them.

Abstract:
Objective The authors intended to review our surgical techniques used in Asian tip surgery and to suggest effective techniques, according to the types of the nasal tip that are addressed. Methods 107 patients who had nasal tip surgery from Jan. 2005 to Dec. 2006 in a tertiary hospital were enrolled in the study. Medical records, graphic operation records, and standardized patient photographs were retrospectively reviewed. Initial diagnosis, types of nasal tip surgery, associated operative techniques, surgical results and complications were analyzed. Results Initial diagnosis included deviated nose, hump nose, combine deviated and saddle nose, low profile nose, short nose, and cleft nose deformity, in order of frequency. Nasal tip surgery included augmentation of projection, control of rotation, restoration of symmetry and volume reduction, among which augmentation was used in all cases. External approach was used in 74% and intranasal approach in 26%. For augmentation of the tip projection, onlay grafts (39%), septal extension grafts (25%), and tip modifying suture with onlay grafts (20%) were the most commonly used methods. Infection or extrusion of the grafts was not found. Complications included tip deviation due to the deviation of the septal extension graft in 1 case, visible tip graft in 1 case, and slight upward rotation of the tip in 1 case. Conclusions In Asians, augmentation of the tip projection is the main issue in tip surgery and is best performed with cartilage onlay grafts or septal extension graft rather than suture modification.

e12502 Background: Incidence of breast cancer in Asian countries is about one third to one half of that of the US. Asian immigrants have a higher risk of developing breast cancer in the US presumably by adopting more westernized lifestyles. We conduct a retrospective analysis of breast cancer patients (pts) in an Asian-majority community in Southern California. Methods: Unselected consecutive female breast cancer pts were studied. Clinical data were review and statistical analysis was conducted by SPSS 16.0. Results: 501 pts, 334 Asians (incl. Chinese 84%) and 167 non-Asians (incl. Caucasian 47% and Hispanic 45%) were studied. Conclusions: This study raised several intriguing findings: 1. Asian pts are significantly younger and have lower BMI than non-Asians. 2. Asian pts are more likely to undergo mastectomy than breast conserving surgery. 3. Asian pts tend to have denser breasts which may potentially limit the sensitivity of mammogram. Since the ratio of DCIS to invasive cancer is comparable between Asian and non-Asian pts in this study, mammogram is probably effective in detecting early cancer in Asians. 4. Significantly fewer Asian pts are diagnosed by screening mammogram. It is probably due to lower utilization of mammogram in the Asian population. 5. BRCA1/2 mutation is not significantly different between Asians and non-Asians. Due to inherent limitations of retrospective study, these findings need to be confirmed in a larger prospective study. Abstract:
Purpose: Differences in stage-stratified survival have been reported between Asian and Western populations with gastric cancer. This study examines differences in presentation and outcomes among Asian and non-Asian patients evaluated and treated at a Canadian institution. Patients and Methods: We reviewed 2,043 patients (159 Asians and 1,884 non-Asians) with gastric adenocarcinoma treated between 1978 and 1997. Overall survival was examined by the Kaplan-Meier method, and multivariable analysis by Cox proportional hazards was used to identify whether Asian ethnicity had independent prognostic significance for survival. Results: Median survival was 13.1 months for Asians and 11.1 months for non-Asians (P = .0016). Asian patients were younger and had a greater proportion of signet ring cell histology but were less likely to have proximal disease. Signet ring cell histology did not adversely affect survival. By multivariable analysis, proximal location, poor differentiation, and extent of disease were independently associated with worse survival. Survival was improved with curative resection, palliative resection, and palliative chemotherapy. Asian ethnicity was not independently associated with survival (hazard ratio, 0.89; 95% confidence interval, 0.74 to 1.08). Although a similar proportion of patients underwent curative resection, an interaction was observed between Asian ethnicity and efficacy of resection, with Asians achieving a greater benefit as compared with non-Asians even when adjusted for age and location. Conclusion: The disparity between Eastern and Western gastric cancer survival is not explained by the hypothesis of ethnicity-related differences in tumor biology. Although it is not an independent predictor of survival, Asian ethnicity is associated with distinct characteristics at presentation and more favorable outcomes after curative surgery.

Abstract:
A linearized, global spectral model with eight levels was used to determine whether the nonlinear interaction between a planetary-scale wave (wavenumber four) and a rapidly growing synoptic-scale wave (wavenumber seven) could produce a northeasterly wind, characteristic of East Asian cold surges. The amplitude of the synoptic-scale wave, or generic cyclone, was produced by a nonlinear Eady model of the atmosphere that included friction. The resulting nonlinear forcing was applied to either the first law of thermodynamics, the vorticity equation, or both. The thermal forcing did not produce a significant cold surge response. The vorticity forcing produced a respectable cold surge within 48 hours. The results of this study indicate the planetary-synoptic wave interaction is a possible method for initiating East Asian cold surges. Keyword: Theses

Abstract:
Large-scale atmospheric disturbances play important roles in determining the general circulation of the atmosphere during the North Pacific boreal winter. A number of scientific questions have been raised due to these disturbances’ spatial and temporal complexity as well as the hydrological implication associated with them. In this dissertation, the principal goal is to further improve our understanding of the atmospheric high frequency (HF) and intermediate frequency (IF) disturbances active over the North Pacific. The study focuses on their energetics, intraseasonal and interannual variability, and the resulting hydrological impact over the eastern North Pacific and Western U.S. including extreme events. To delineate the characteristics of HF and IF disturbances in the troposphere, we first derive a new set of equations governing the local eddy kinetic energy (EKE), and assess the critical processes maintaining local budgets of the HF and IF EKE. The diagnosis assesses the 3-D patterns of energy flux convergence (EFC), barotropic conversion (BT), baroclinic conversion (BC), and cross-frequency eddy-eddy interaction (CFEI). The local EKE budget analysis is followed by an investigation of the modulation of HF and IF eddy activity by different modes of low frequency climate variability. On interannual timescales, the response of various local energetic processes to El Niño-Southern Oscillation (ENSO) determines the HF and IF EKE anomalies and the role of CFEI process is important in producing these anomalies. Also on interannual timescales, winter precipitation deficits associated with suppressed cyclonic activity, i.e., negative HF EKE anomalies, are linked to severe droughts over the U.S. Southern Great Plain (SGP) region. The suppressed cyclonic activity is, in turn, tied to phase changes in the West Pacific (WP) teleconnection pattern. On intraseasonal timescales, variations in HF disturbances (a.k.a. storm tracks) over the North Pacific are closely coupled with tropical convection anomalies induced by the Madden-Julian Oscillation (MJO), and partly drive larger scale intraseasonal flow anomalies in this region through eddy-eddy interactions. Anomalous HF eddy activity induces subseasonal transitions between “wet” and “dry” regimes over the west coast of North America. Also on intraseasonal timescales, the East Asian cold surge (EACS) is found to provide a remote forcing of the winter precipitation anomalies in the western U.S. This modulation is achieved through “atmospheric rivers” (ARs), which are narrow channels of concentrated moisture transport in the atmosphere and are responsible for over 70% of the extreme precipitation events in the western U.S.. EACS effectively modulates the IF disturbance activity over the North Pacific, and the anomalous IF disturbances lead to the formation of an AR over the eastern North Pacific that ultimately induces precipitation anomalies in the western U.S. Analyses of the simulations from the NCAR Community Climate System Model version 4 (CCSM4) demonstrate that the connections among the EACS, AR and western U.S. precipitation are better captured by a model with higher spatial resolutions. The improved simulation of these connections is achieved mainly through a better representation of the IF disturbances, and the associated scale-interaction processes in the higher resolution model.

The surge of the coronavirus disease (COVID-19) pandemic has driven countries worldwide to launch substantial stimulus packages to support economic recovery. This paper estimates effects of fiscal measures on output using data from 2000 to 2019 for a panel of nine developing Asian economies and a vector autoregression model. Results show that (i) the 4-quarter and 8-quarter cumulative fiscal multipliers for general government spending range between 0.73 and 0.88 in baselines, in line with recent estimates for developed countries but larger than those for developing countries; (ii) government spending is more effective than tax cuts in boosting the economy; and (iii) an accommodative monetary policy regime can make fiscal measures more effective.

Abstract:
Choledochal cysts are a core pathology in pediatric surgery, affecting 1/100,000 live births in the western world, and 1/13,000 in eastern asia. These cysts are classified by the Todani classification, types I-V, in respect to their location and underlying pathophysiology. Infants and children presenting with stigmata of biliary disease should undergo evaluation for choledocal cyst. Workup includes axial imaging, ultrasonography, and laboratory investigation. A liver biopsy is necessary in neonates and newborns to rule out cystic biliary atresia, which would require further evaluation and management. Large choledochal cysts may be visualized on prenatal ultrasound, and warrant referral to a fetal care center for postnatal work up and monitoring. Management of choledochal cysts is dependent on the anatomic variant and spans from ERCP with sphincterotomy, to cyst excision with ductal and alimentary tract reconstruction. Type V choledochal cysts may require liver transplantation. Long term follow up is required due to an enhanced risk of future malignancy, even after resection.
