Abstract:
TB remains a leading cause of morbidity and mortality in developing countries. The incidence of congenital TB is underestimated. Current recommendations regarding the management of neonates of mothers with tuberculosis are variable and no tangible guidelines have been advised. Congenital TB is fatal if untreated, moreover the mortality and morbidity is increased if the diagnosis and treatment is delayed. Therefore, the treating clinician should be aware of the unusual presentation of congenital TB. A high suspicion and good screening of mothers and neonates is of paramount importance. Congenital tuberculosis is diagnosed by Cantwell criteria. Isoniazid prophylaxis for 6 months is recommended in neonates born to mothers with TB who are infectious. Breastfeeding should be continued, and isolation is recommended only in certain circumstances such as mother is infectious, has multidrug resistant tuberculosis or non-adherent to treatment. BCG vaccine is recommended in all neonates however, the timing of administration varies according to various guidelines. Neonate diagnosed with congenital TB should be treated with anti-tubercular drug regimen.

Inspite of so many available health facilities, humans are unable to prevent preterm deliveries may be because of evolution process for increasing intelligence in Human species. Now a days most of the preterm babies are surviving due to antenatal steroid coverage, with advancement of health care facilities. As authors are unable to replace the nature and create in utero environment, Kangaroo mother care is a simple but most effective way of developmental supportive care. Here baby is exposed to some known and similar environment like in utero. Preterm survival without morbidity and com-promised quality is an important than survival alone. As preterm organs are still in a developing stage particularly central nervous system where organization, synapse formation still occurring hence enriched environment is necessary for optimal development. In kangaroo care baby usually meet enriched environment, here authors are giving a brief review of Kangaroo Care because of more evidence was there but still not practicing fully in many centers where needed particularly in developing countries. That’s why authors are adding from their side.

The enactment of theoretical contribution in research articles (RAs) is a critical academic writing practice for showcasing the scientific progress of a disciplinary field. To decide how novel research strengthens a discipline’s current state of knowledge necessitates evaluating the new empirical results based upon its specific academic norms. Yet the academic values utilized to formulate theoretical advancement have been little studied in the literature. Drawing upon pertinent evaluation frameworks, the purpose of this study is to uncover how RA authors negotiate the theoretical value of novel research by conducting a rhetorical analysis of 60 research article discussion sections taken from four prestigious management journals. The present investigation reveals a tentative taxonomy of six rhetorical appeal types deployed by RA authors to articulate the value of novel research in the knowledge market. The widespread use of these appeals in management research article discussion sections indicates that a strong promotional orientation is prevalent in the rhetoric of the journals examined. The findings of this research have theoretical and pedagogical implications for academic evaluation and promotion, as well as for teaching academic writing

Abstract:
Nodular fasciitis is defined as a benign and reactive fibroblastic growth extending from the superficial fascia into the subcutaneous tissue or muscle. It is a rare benign neoplasm most commonly afflicting the soft tissues of upper extremity followed by trunk, head, and neck. It accounts for 0.025% of all pathologic diagnosis. We report a case of a swelling over the right scapula which on cytology examination was diagnosed as a benign spindle cell lesion. It was subsequently confirmed to be an intramuscular nodular fasciitis on histopathological examination.

Head and neck cancer is the sixth most common cancer worldwide. The single most important factor affecting prognosis for squamous cell carcinoma is the status of the cervical lymph nodes. Metastasis to the regional lymph nodes reduces the 5-year survival rate by 50% compared with that of patients with early-stage disease. The American cancer society reports that 40% of patients with squamous carcinoma of the oral cavity and pharynx present with regional metastases to the cervical lymph nodes. This activity presents the steps for safe and optimum neck dissection. Objectives of the study were to identify the anatomical structures in neck dissection, review the complications of head and neck surgery and summarize the importance of care coordination and to improve outcomes for patients undergoing head and neck surgery.

Abstract:
Phyllodes tumours are rare entities of fibroepithelial diseases. The exact pathogenesis and their relationship with fibroadenomas are oblivious. Women aged between 35 to 55 years are commonly affected, even younger in Asian population. Triple assessment should be applied as a guide to management in any breast pathology. Clinical appearances are typically diagnostic for phyllodes tumours. Even though sometimes inconclusive, mammography and ultrasonography are the main imaging modalities. Although the role of cytology is debatable, presence of both epithelial and stromal elements supports the diagnosis especially in malignant type. Core biopsy is rather favored in view of higher accuracy for the diagnosis. Accurate preoperative assessment and histologic diagnosis allow correct surgical intervention and subsequent avoidance of reoperation. Surgical management can be either wide excision with more than 1 cm margins or mastectomy without axillary surgery. Local recurrence has been associated with inadequate excision of the pseudopod. Adjuvant radiotherapy is recommended for positive surgical margin and for local control of borderline and malignant phyllodes tumors. The role of chemotherapy and endocrine therapy has not been fully studied.

Background: Heart failure is associated with albuminuria, frequently microalbuminuria and it has shown to correlate with the severity of heart failure. Methods: Prospective observational study. Hundred patients were studied, with albuminuria being measured before and after correction of heart failure. Results: One third of patients with heart failure had microalbuminuria. No correlation of albuminuria with severity of heart failure was found. The decrease in albuminuria following treatment of heart failure was statistically significant. Conclusions: This study shows association of albuminuria to be statistically associated with heart failure and its reversibility with treatment of heart failure.

This article examines the use of ‘enhanced interrogation techniques’ in the context of international legal obligations under the Convention against Torture and Other Cruel, Inhuman or Degrading Treatment or Punishment 1984 (CAT) and the domestic implementation of the international prohibition of torture into United States (US) law under 18 United States Code Sections 2340-2340A. The legal basis for the interrogation programme was a series of contentious legal memoranda written by Department of Justice Office of Legal Counsel lawyers.1 This article examines whether the memo drafters ought to be investigated for incurring criminal liability for the consequences of their memoranda, namely under CAT and Sections 2340-2340A and what has unfolded under President Obama’s administration.

We will pay special attention to everything that affects the supervisory body of the employment pension funds, which is the control commission. Non-clear cell renal cell carcinoma (nccRCC) is not as common as clear cell renal cell carcinoma (ccRcc). But then it is not uncommon in regular urological practice. It usually carries a grave prognosis. Authors need to converse ourselves with National comprehensive cancer network (NCCN) categories for evidence and we need to converse as well with NCCN categories for preferences. ‘Preferred intervention’ is based on superior efficacy, safety and evidence and when appropriate, affordability. ‘Other recommended interventions’ are somewhat less efficacious, more toxic, or based on less mature data or significantly less affordable for similar outcomes. ‘Useful in certain circumstances’ are interventions that may be used for a selected patient population. Clinical trials for targeted agents are mainly directed at ccRCC because of its high prevalence. According to the NCCN panel, enrolment in clinical trials is the preferred strategy for nccRCC. Outcomes of patients with nccRCC have improved with the introduction of targeted therapy. Precise pathological diagnosis of the types of nccRCC by immunohistochemical analysis is mandatory. This enables specific treatments for individual nccRCC. Currently TKIs are the drug of choice (both first and second line) for metastatic papillary RCC. Both TKIs and mTOR inhibitors are effective against chromophobe RCC. Platinum based chemotherapy should be used for metastatic CDC. Further evidence is required for management of nccRCC.

This article presents results of studies of the effect of movement surface slope on tractive and tow performance of wheeled forestry machine’s mover. Dependencies, which the mathematical model is based on, take into account the impact of direction deviations of the resultant load on soils from the gear from the normal, which reduces bearing capacity of the base and increases the deformation of soil and its resistance to machine movement. As a consequence, the model considers reductions of the mover’s tractive and tow performance.

Abstract:
This article explores the value of attending to the emotional side of the doctoral journey by focusing on the use of a ‘secret’ Facebook group amongst a cohort of EdD (Professional Doctorate in Education) students at one English university. Presented as a piece of action research in which the participants created an intervention to address a perceived problem and then reflected on its effectiveness, it is co-authored by the cohort of six students and their tutor. The stresses and loneliness of the doctoral journey have been well documented and constitute the ‘problem’ addressed by this cohort of students. Their inception and use of a Facebook group was a response to challenges experienced in their studies, with the expectation of facilitating peer support. As will be shown this aim was successfully met with enhancements in academic, social, and emotional support. However, unexpected benefits arose from the interactions within the group including a normalization of the challenges of the doctoral quest and the advantage of being able to follow the ‘breadcrumb trail’ found in the group postings as group journal and aid to reflection. Further, both tutors and students have noted the development of a strong sense of ‘cohortness’ and inclination to work collaboratively. Through a process of individual and group reflection on experiences of the intervention, combined with analysis of the content of the postings, this article examines the characteristics of the Facebook intervention and considers some ethical implications. We suggest that key characteristics that have contributed to its success include the student ownership, the protection of the secret format, and the combination of emotionally supportive, academic, and irreverent exchanges between group members. It is hoped that these insights may be useful to future doctoral candidates and their tutors as they negotiate their own way through the doctoral woods.

AbstractWhile digital games are used for health intervention, language training, and science learning, there are few instances of games being used to teach archaeology. This is a missed opportunity, as narratives of exploration and discovery make archaeology well suited to digital games. This article discusses Red Land/Black Land, a prototype Civilization V mod (modification of an existing game) that allows players to explore both the process of cultural change in ancient Egypt and the construction of archaeological knowledge about ancient Egypt. As a case study of scholarly game design, the article explores the project’s development. Included is a discussion of play mechanics, the merits and process of modding, and the resources required to build games in academic environments. The article also discusses preliminary efforts to test the game in an undergraduate classroom. Ultimately, the article argues that digital games are powerful tools for communicating archaeology. Instead of falling under the glamour of commercial games, however, scholars should adopt cost effective approaches to game design, looking to mobile play, alternate reality games, modding, and browser based games to realize their vision. The article also argues that the actual process of designing a game is an excellent tool to teach archaeology.

Abstract:
The article deals with the informative content of spectral analysis of heart rate variability in the assessment of the regulatory impacts on the systemic hemodynamics during orthostatic test. It was observed that the patients who suffer from neurogenic syncope already at a young age had had a decrease in low frequency oscillations, as well as a decrease in peripheral vascular resistance during the test. It allows us to make a conclusion about the sympathetic vasomotor regulation dysfunction, even before the symptoms of orthostatic hypotension were evident. The decrease in the tonic vagal effect which follows from the depression of high-frequency oscillations, makes for increasing the chronotropic function of the heart and keeps a relative sympathetic predominance in order to maintain adequate level of blood pressure

Studies of doctoral education have included an interest not only in processes, structures, and outcomes, but also in students’ experiences. There are often useful recommendations for practice within individual examinations of the doctoral experience, yet there remains a need to strengthen the application of lessons from research to the behaviors of students and others engaged in the doctoral process. This paper is the first to synthesize research about doctoral education with the particular aim of informing practical strategies for multiple stakeholders. In this article, we summarize findings from a literature review of the scholarship about doctoral education from the past 15 years in a stage-based overview of the challenges of doctoral education. Our aim is to apply theory to practice through the systematic consideration of how research about doctoral education can best inform students and those who support them in the doctoral journey. We first present an overview of the major stages of doctoral education and related challenges identified in the research. We then consider key findings of that research to offer recommendations for doctoral students, faculty members, and administrators within and across stages.

Venous stroke being “relatively unknown cerebrovascular disease” occurs in 0.55% of all strokes. Specific diagnostic concerns to venous stroke and the expansion of the volume of radiologic examinations from routinely used non-contrast CT to angiographic and perfusion CT- and MRI, diffusion MRI allowed us to increase the number of diagnosed and verified venous ischemic stroke from 0.4% of all strokes treated in our center to 2.4%, resulting in a 6-fold increase within the 5-year period. Symptoms of cerebral venous sinus thrombosis depend on the size and the growth rate of thrombus. In addition, focal neurological symptoms are diverse and largely dependent on thrombus localization and safety of collateral blood flow, as well as patients’ age and severity of cerebral edema. Cerebral symptoms are nonspecific and may occur in varying degrees of severity related to the localization of the pathological process. The routinely used assessment scales such as the National Institutes of Health Stroke Scale (NIHSS), Bartel Index, Rankin Scale do not reliably identify this type of acute ischemic stroke. The leading symptom is headache with the mean visual analogue scale of 7.3±1.6. The tendency towards a less pronounced neurologic deficit at admission and mild disability status at discharge have been found among patients with venous stroke. The clinical course of venous stroke mostly demonstrated a tendency towards rapid regression of focal symptomatology and neurological deficit in comparison with arterial ischemic stroke.

The article describes the educational and scientific university environment in which Keynes's talent was formed, as well as the Bloomsbury Circle of Intellectuals, in which Keynes occupied a prominent place. The formation of Keynes's specific approach to economic problems, which combine the analysis of real problems, theory and formulation of the practical proposals, is considered on the example of the early work of Keynes in the 1910s. The article argues that Keynesianism is the real engine of the modern economy. Keynes developed methods and apparatus by which the conceptual vision of economics is transformed into concrete representations of the economic theory. After a long journey, shown in his works ("General Theory of Employment, Interest and Money"), Keynes developed his vision of economic and social phenomena as an analytically functional analysis, and finally destroyed the logical capacity of people's faith in the ability of a free market economy to automatically maintain full employment, identified the possibility of influencing the regulation of money circulation on prices, exports, imports, production process and employment. Despite criticism of his theoretical evidence, even his opponents agreed that Keynes's ideas had been confirmed in practice. At the end of the article are the assessments of Keynes' research by authors and scientists with different views.

The right to freedom of expression and information is guaranteed by Article 10 of the European Convention for the Protection of Human Rights and Fundamental Freedoms (ECHR) in all member states of the Council of Europe. The case law of the European Court of Human Rights (ECHR) applying Article 10 must be considered an international standard of authority on the protection of this human right, including the right to express, transmit and receive opinions and information without the interference of public authorities. Freedom of expression is one of the most cherished constitutional rights in democracies. Freedom of expression affects every aspect of our social and political system and receives explicit and implicit protection in every modern democratic constitution. Moreover, it is frequently mentioned in public discourse and has inspired an impressive volume of legal and philosophical literature. Since its inclusion in Article 19 of the Universal Declaration of Human Rights, the right to freedom of opinion and expression has been protected in all relevant international human rights treaties. In international law, the freedom to express opinions and ideas is considered essential both at the individual level, insofar as it contributes to the full development of a person, and being a cornerstone of a democratic society.

Встанов-лено, що аерозольне застосування «Чемі-спрею» 1 раз на добу після виконання первинної хірургічної об-робки рани у поєднанні з одноразовим парентераль-ним введенням 15 % амоксициліну сприяє очищенню ран від некротичних тканин до 6,8±0,64 доби. Серед-ній термін загоєння становив 23,6±0,64 діб, що свід-чить про доцільність застосування під час лікування поранених тварин як локальних, так і загальних ме-тодів терапії. The article presents data on the spread of purulent wounds of horses, clinical, planimetric research and treatment of sick animals. We found that spray application of «Chemi-spray» 1 time per day, after primary surgical processing of a wound in combination with a disposable parenteral doses of 15 % of the amoxicillin helps cleanse wounds of necrotic tissue within 6,8±0,64 days. The average healing period amounted 23,6±0,64 days which indicates the feasibility of use in the treatment of injured animals both local and general treatment methods.

Abstract:In recent years, the Islamic State in Iraq and the Levant (ISIL)1 as well as several Al-Qaida affiliates have used the systematic and large-scale looting of antiquities as one of their income streams. Due to the large-scale and organized looting activities of these groups, in particular, in Iraq and the Syrian Arab Republic, the United Nations Security Council (UNSC), following various reports and recommendations by the ISIL, Al-Qaida and Taliban Monitoring Team has adopted a range of measures, chiefly among them the landmark UNSC Resolution 2347 (2017) to counter this threat. These measures demand that both member states’ regulators as well as private sector stakeholders take specific action to ensure that the art and antiquity trading industry is capable of defending itself against the misuse of their services to finance terrorism. This article outlines the various challenges member states and private industry are facing in this regard and explains how the various new UNSC provisions, including the measures outlined in UNSC Resolution 2347 (2017), could be employed effectively to counter this threat.

The article explores the interaction between legal and political strategy in producing social change. It centres on a long-running dispute in Australia over whether charities can have a dominant political purpose. The focus is on the strategising of the small activist charity that successfully pursued the case over a five-year period. As an 'insider' account, the article charts the in-practice process of translating activisms across legal and political fields. With a stress on contingency and agency, the account affirms a 'politics of rights' approach to legal activism. It shows how the case opened-up new grounds for political contestation, and as such offered prospects for 'non-reformist reform'. It also demonstrates how this occurred more by strategic engagement with unintended effects, than necessarily by design.

The article is devoted to the assessment of the actual possibility of citizens' realization of the constitutional right to access to justice in the conditions of the current model of criminal proceedings in the Russian Federation. The author investigates the problems of the application of certain provisions of the criminal procedure legislation of the Russian Federation defining the reasons, grounds and timing of the initiation of a criminal case, combined into the stage of initiation of a criminal case (Chapters 19-20 of the Code of Criminal Procedure of the Russian Federation). The question of the need to reform domestic legislation in this part in order to increase the effectiveness of the criminal procedural function of the state is being investigated. The study is based on the perspective of topical issues of initiating criminal cases for crimes under the jurisdiction of the FSSP of the Russian Federation, including those provided for in Article 177 of the Criminal Code of the Russian Federation, taking into account official statistics for 2020 and judicial practice on citizens' complaints. The relevance is based on disappointing data on the execution of judicial acts in the Russian Federation. In particular, attention is drawn to the presence in 2020 of more than 110.5 million enforcement proceedings in the FSSP of the Russian Federation, the generalized level of enforcement for which amounted to no more than 50%, totaling 696 billion rubles, which is a serious threat to economic stability and requires timely response. The relevant data are considered as a consequence of the existing barrier at the stage of initiation of criminal proceedings in the implementation of criminal legislation, including determining responsibility for crimes against the authorities, malicious non-execution of judicial acts, which is also confirmed by unjustified indicators of the initiation of criminal cases under Articles 177, 311, 312, 315 of the Criminal Code of the Russian Federation. In order to ensure the implementation of the criminal procedural function of the state, a comprehensive reform of this stage is proposed by transforming it into the institution of the beginning of criminal proceedings, which will restore the preventive function of the norms of the criminal legislation of the Russian Federation and ensure the effective operation of state authorities.

Methods: The article considered approaches to achieve the research goal through the creation of a backend on Django Rest Framework, as well as the application of the simplex method to perform treatment optimization. Results: A server-side software product was obtained, where a special database was implemented, which takes into account the requirements for the data structure of the organization of medical information, and a module for selecting optimal treatment strategies for patients was created. Discussion: The conducted research allowed us to obtain a prototype of medical information system, which will be useful due to its versatility in the data structure of the organization of medical data and the use of machine learning methods to optimize treatment strategies.

In this article, authors analyze different models of codified acts. In carrying out this classification, several features of the codified documents emerged and were presented in a light conducive to study. The authors provide a practical example of doctrinal concepts, based on their empirical observations, with the relevant example for our legal system. Despite the multitude of scientific proposals, the classifications presented in the article are far from exhaustive, but are a theoretical-doctrinal exercise, aimed at systematizing knowledge about the nature and features of codified acts. Finally, the authors conclude that codes are the only form of codified act recognized by national law, although other forms are known in foreign legal systems. Without prejudice to the legal nature of codes and other classified acts, they may be doctrinally classified according to various criteria, which simplifies the understanding of their essence and contributes to their practical application.

Despite significant advancements in neuroprosthetic control strategies, current peripheral nerve interfacing techniques are limited in their ability to facilitate accurate and reliable long-term control. The regenerative peripheral nerve interface (RPNI) is a biologically stable bioamplifier of efferent motor action potentials with demonstrated long-term stability. This innovative, straightforward, and reproducible surgical technique has shown enormous potential in improving prosthetic control for individuals with upper limb amputations. The RPNI consists of an autologous free muscle graft secured around the end of a transected peripheral nerve or individual fascicles within a residual limb. This construct facilitates EMG signal transduction from the residual peripheral nerve to a neuroprosthetic device using indwelling bipolar electrodes on the muscle surface. This review article focuses on the development of the RPNI and its use for intuitive and enhanced prosthetic control and sensory feedback. In addition, this article also highlights the use of RPNIs for the prevention and treatment of postamputation pain.

The article emphasizes the relevance of drawing attention to the increasing role of administrative and legal provision of information security. The current administrative legislation is analyzed, and proposals are formulated aimed at finding administrative and legal means and methods, problems of ensuring information security. Based on the current administrative legislation, the article presents the author's approach to the problem of legal responsibility in the field of security in the global cybernetic space. The article examines the legal and organizational aspects of the implementation of administrative responsibility measures in the mechanism of ensuring information security, the importance of administrative coercion in the mechanism of building an information society is noted in the work. The main conclusions of the study are that over the last period of time a number of changes have been made to the administrative legislation in terms of legal regulation of information security. These changes indicate that in the Russian Federation, state control is being strengthened to prevent information threats and risks, while the legislator responds in a timely manner to changes in society in the context of the digitalization of law and geopolitical instability in the world. To date, measures of administrative responsibility for offenses in the field of information security are constantly being developed and improved.With the advent of new information technologies, means of communication and objects of critical information infrastructure in the Russian Federation, it is necessary on an ongoing basis to conduct scientific research on issues of legal liability for unlawful influence on such information objects with the active participation of public authorities in order to introduce scientific and technical developments into practical activities.
