Abstract:
TB remains a leading cause of morbidity and mortality in developing countries. The incidence of congenital TB is underestimated. Current recommendations regarding the management of neonates of mothers with tuberculosis are variable and no tangible guidelines have been advised. Congenital TB is fatal if untreated, moreover the mortality and morbidity is increased if the diagnosis and treatment is delayed. Therefore, the treating clinician should be aware of the unusual presentation of congenital TB. A high suspicion and good screening of mothers and neonates is of paramount importance. Congenital tuberculosis is diagnosed by Cantwell criteria. Isoniazid prophylaxis for 6 months is recommended in neonates born to mothers with TB who are infectious. Breastfeeding should be continued, and isolation is recommended only in certain circumstances such as mother is infectious, has multidrug resistant tuberculosis or non-adherent to treatment. BCG vaccine is recommended in all neonates however, the timing of administration varies according to various guidelines. Neonate diagnosed with congenital TB should be treated with anti-tubercular drug regimen.

Now a days most of the preterm babies are surviving due to antenatal steroid coverage, with advancement of health care facilities. As authors are unable to replace the nature and create in utero environment, Kangaroo mother care is a simple but most effective way of developmental supportive care. Here baby is exposed to some known and similar environment like in utero. Preterm survival without morbidity and com-promised quality is an important than survival alone. As preterm organs are still in a developing stage particularly central nervous system where organization, synapse formation still occurring hence enriched environment is necessary for optimal development. In kangaroo care baby usually meet enriched environment, here authors are giving a brief review of Kangaroo Care because of more evidence was there but still not practicing fully in many centers where needed particularly in developing countries. That’s why authors are adding from their side.

The enactment of theoretical contribution in research articles (RAs) is a critical academic writing practice for showcasing the scientific progress of a disciplinary field. To decide how novel research strengthens a discipline’s current state of knowledge necessitates evaluating the new empirical results based upon its specific academic norms. Yet the academic values utilized to formulate theoretical advancement have been little studied in the literature. Drawing upon pertinent evaluation frameworks, the purpose of this study is to uncover how RA authors negotiate the theoretical value of novel research by conducting a rhetorical analysis of 60 research article discussion sections taken from four prestigious management journals. The present investigation reveals a tentative taxonomy of six rhetorical appeal types deployed by RA authors to articulate the value of novel research in the knowledge market. The widespread use of these appeals in management research article discussion sections indicates that a strong promotional orientation is prevalent in the rhetoric of the journals examined. The findings of this research have theoretical and pedagogical implications for academic evaluation and promotion, as well as for teaching academic writing

Abstract:
Nodular fasciitis is defined as a benign and reactive fibroblastic growth extending from the superficial fascia into the subcutaneous tissue or muscle. It is a rare benign neoplasm most commonly afflicting the soft tissues of upper extremity followed by trunk, head, and neck. It accounts for 0.025% of all pathologic diagnosis. We report a case of a swelling over the right scapula which on cytology examination was diagnosed as a benign spindle cell lesion. It was subsequently confirmed to be an intramuscular nodular fasciitis on histopathological examination.

Head and neck cancer is the sixth most common cancer worldwide. The single most important factor affecting prognosis for squamous cell carcinoma is the status of the cervical lymph nodes. Metastasis to the regional lymph nodes reduces the 5-year survival rate by 50% compared with that of patients with early-stage disease. The American cancer society reports that 40% of patients with squamous carcinoma of the oral cavity and pharynx present with regional metastases to the cervical lymph nodes. This activity presents the steps for safe and optimum neck dissection. Objectives of the study were to identify the anatomical structures in neck dissection, review the complications of head and neck surgery and summarize the importance of care coordination and to improve outcomes for patients undergoing head and neck surgery.

Abstract:
Phyllodes tumours are rare entities of fibroepithelial diseases. The exact pathogenesis and their relationship with fibroadenomas are oblivious. Women aged between 35 to 55 years are commonly affected, even younger in Asian population. Triple assessment should be applied as a guide to management in any breast pathology. Clinical appearances are typically diagnostic for phyllodes tumours. Even though sometimes inconclusive, mammography and ultrasonography are the main imaging modalities. Although the role of cytology is debatable, presence of both epithelial and stromal elements supports the diagnosis especially in malignant type. Core biopsy is rather favored in view of higher accuracy for the diagnosis. Accurate preoperative assessment and histologic diagnosis allow correct surgical intervention and subsequent avoidance of reoperation. Surgical management can be either wide excision with more than 1 cm margins or mastectomy without axillary surgery. Local recurrence has been associated with inadequate excision of the pseudopod. Adjuvant radiotherapy is recommended for positive surgical margin and for local control of borderline and malignant phyllodes tumors. The role of chemotherapy and endocrine therapy has not been fully studied.

Background: Heart failure is associated with albuminuria, frequently microalbuminuria and it has shown to correlate with the severity of heart failure. Methods: Prospective observational study. Hundred patients were studied, with albuminuria being measured before and after correction of heart failure. Results: One third of patients with heart failure had microalbuminuria. No correlation of albuminuria with severity of heart failure was found. The decrease in albuminuria following treatment of heart failure was statistically significant. Conclusions: This study shows association of albuminuria to be statistically associated with heart failure and its reversibility with treatment of heart failure.

Abstract:
The purpose of this paper is to analyse which navigational rights are applicable to foreign commercial vessels performing trans-arctic shipping through the Northwest Passage and how this affects the potential for Canada, through article 234 of the United Nations Convention on the Law of the Sea (UNCLOS), to issue national environmental legislation regulating pollution from ships. This is done through the method of legal dogmatics, characterised by analysis of the sources of law and the hierarchy between them. In this paper, the main sources of law employed are UNCLOS and relevant case law from the International Court of Justice. The main conclusions presented in the paper are that the Northwest Passage consists of a combination of territorial sea and exclusive economic zone, which generally means that commercial vessels are allowed innocent passage through the parts of the passage forming the territorial sea and are largely subject to freedom of navigation through those parts that form the exclusive economic zone. However, it seems likely that the Northwest Passage is a strait used for international navigation, in which case, the vessels passing through it are instead subject to the regime of transit passage. This would negatively affect the possibility for Canada to issue national legislation to regulate pollution from foreign vessels. However, as long as the Northwest Passage is covered by ice for most of the year, article 234 UNCLOS allows Canada far more leeway in regard to issuing anti-pollution legislation, both in the territorial sea and the exclusive economic zone, regardless of whether the Northwest Passage is found to be a strait used for international navigation. The passage regime applicable to foreign commercial vessels under article 234 could be characterised as a sui generis passage.

The thesis deals with the preliminary reference procedure. There is outlined not only the characteristic of this procedure or its evolution, but also the interpretation of the term "court or tribunal of a Member State" or the Exceptions to the Duty to Refer. The following chapters are concerned with the statistical curiosities of the preliminary reference procedure or the ways of how to make this procedure more efficient. The last chapter is analysing the preliminary reference procedure from the Czech law's point of view. It deals with some Czech institutions as courts of the Member State, procedural aspects of this procedure or study of the Czech preliminary references. It also contains some cases of The Constitutional Court of the Czech Republic regarding with this procedure.

This chapter discusses Article 267 of the Treaty on the Functioning of the European Union (TFEU). Article 267 TFEU (ex Article 234 EC) gives the Court of Justice jurisdiction to deliver preliminary rulings on the validity and interpretation of EU law. The primary purpose of Article 267 is to ensure that EU law has the same meaning and effect in all the Member States. Where it considers a decision on a question of EU law is necessary to enable it to give judgment, any court may refer that question to the Court of Justice (the discretion to refer). Where a question of EU law is raised before a national court of last resort, that court must refer it to the Court of Justice (the obligation to refer).

In a regular free market economy, the prices of goods and services occur at the point where the quantities of supply and demand get up with one another. On the other hand, it can be in question the distortion of this equilibration by various purposes and practices. In this case, optimal goods and services cannot be received under optimum conditions. As a conclusion of “ultima ratio” principle in the criminal law, every illegal conduct in this context does not constitute a crime but some fraudulent ones are imposed a sanction. The crimes of “fraud” in articles 157-158, “fraudulent bankruptcy” in article 161, “bid rigging” in article 235, “rigging during the discharge of contractual obligation” in article 236 and “manipulation on prices” in article 237 of Turkish Penal Code are some of the examples which can be given in this context. In this study, “crime of manipulation on prices” which is not dwelled on adequately in the doctrine is examined by featuring the controversial points.

Abstract:
The article deals with objective and optional features of crimes with material and formal composition, with an emphasis on crimes that are more or less associated with the spread of infection. Attention is drawn to the fact that crimes with a formal composition do not require the establishment of a causal relationship. Various points of view on “danger” are considered as one of the fundamental criteria used in the qualification of crimes. The author emphasizes the ambiguity of the content of this concept, as well as the concepts of “threat”, “significance”, “mass character”, etc. It is concluded that there are gaps, ambiguities, and contradictions in the legislation and normative legal acts on terminology and conceptual apparatus in relation to the above-mentioned concepts of danger, threat, and its magnitude (significance, mass character). Legislative amendments adopted in connection with the pandemic of coronavirus infection, which led to the emergence of a formal crime under part 1 of article 236 of the criminal code of the Russian Federation, are considered. Attention is drawn to the careless form of guilt under this article, in contrast to other articles, the use of which is also not excluded in the case of infection (article 125 and article 237 of the criminal code), but provide for the intentional nature of the crime.

Abstract This article presents the first results of the neutronics analysis of RBMK-1000 at the 3D assembly level using the OpenMC code as an early stage of the long-term large academic research project. The goal of the large academic research project is to develop a Multi-group Monte Carlo computational module around the OpenMC code using Python programming language. The results of this paper include the criticality calculation and depletion analysis of RBMK-1000. The effective multiplication factor value (keff) has been determined. In depletion calculation, the fission reaction rates of U-235, U-236, U-238, production of fissionable materials, and two fission product poisons Xe-135 and Sm-149 have been investigated. There is ongoing work for benchmarking the first results from OpenMC against Serpent 2 Monte Carlo code. The parametric design data of RBMK-1000 have been taken from the OECD - Nuclear Energy Agency (NEA) SFCOMPO 2.0 (Spent Fuel Isotopic Composition) database that includes measured isotopic concentrations of spent nuclear fuel, with operational histories and design data. The case study was the Leningrad-1 NPP. The ENDF/B-VII.1 data library has been used in this analysis. Moreover, this is the first time to date that the OpenMC code has been used in neutronic studies of the RBMK reactors.

The article is the first in Russia to present sociologically correct (relative to the general population) and simultaneously psychologically profound results of 2 All-Russian Internet surveys: screening-diagnostics of the level of resistance/vulnerability to Internet addiction in Russia and its federal districts (2017: n = 3 007, ages 10-40); identification of words - markers of values for norm and risk groups (2018: n = 144, ages 18-28). Methods: Internet addiction test (A. E. Zichkina), self-reports on the duration of the offline period per year, 16-FLO (R. Kettell, MD self-evaluation scale, B intellect scale), ‘Short portrait questionnaire of the Big Five (B5-10)’ (M.S. Egorova, O.B Parshikova), ‘Existence’ (A. Langle, K. Orgler, S.V. Krivtsova), author’s questionnaire, Deception scale. Results: 3/4 citizens of the Russian Federation fall within a normal range, but only 1/4 have no signs of internet addiction. Contrary to social prejudice and statistics from English-language studies, Internet addiction is least pronounced among 18-21-year-old Russian respondents (when they are virtually active). Normally young people are characterised by the needs for Career, Care, the ability to Manage/Control and Influence events/decisions, anticipate internet escapism when they lose their Meaning, Wisdom or Interest. The risk group includes 8.6 % males, and 23.6 % females. Internet addicts 2.3 % (coinciding with global statistics): twice as many women (different from global statistics). Girls under 14, teenagers, men aged 22-25 and women aged 30-35 are at risk and among those considered to be Internet addicts. Adults in this group develop existential indecisiveness, have unmet status-related claims (specifically Respect) and a strongly overestimated willingness to use coping strategies in reality instead of virtually. Internet addicts are most numerous in the Central Federal District (4.6 %), with the highest risk group in the Far East (37.8 %).

Our PCs and mobile smartphones include several metals obtained through mining operations, numerous household items have been made by using chemical reactions. If we are ill, we may need medication, which is made by using chemical reactions and biological experiments. Autonomous vehicles and flying devices are gaining increasing popularity and extended application in both commercial and non-commercial use. Some more radical members of society may say that we do not need these things and may withdraw from their use. However, it would be more realistic to contend that the society would never give up existing comfort, even if it is gained at least partly by activities creating increased danger. Therefore, the question is, how to regulate the liability for damages, caused by the activities incurring increased or abnormal danger to ensure that these regulations provide sufficient protection for injured parties and, at the same time, do not discourage potential operators from carrying out these activities, which create significant positive effect, despite of their danger. Various jurisdictions are dealing with this question in different manner. The most important Latvian regulations are included mainly in the Art. 2347 para. 2 of Latvian Civil Law, adopted in 1992 and mainly mirroring the legal provisions of the earlier 20th century, with slight adjustments, introduced in 2012. The author of the present paper holds an opinion that this Latvian regulation should be revised on the basis of more recent examples from other jurisdictions, such as Netherlands, and partially taking into account the findings and proposals, made in such splendid example of international academic cooperation as Principles of European Tort Law (hereinafter – PETL). The author of the current article makes such comparative analysis, as well as examines case law, discusses findings of some other authors and, consequently, proposes several amendments, which should be considered in order to improve the Latvian regulations, dealing with liability for damages caused by abnormally dangerous activity.
