Abstract:

Context and objective. Implementation of enhanced recovery programs after hip arthroplasty is critical to therapeutic success. The present study aimed to evaluate the practice of Enhanced Rehabilitation after Surgery in total hip arthroplasty. Methods An observational serial case study of patients was conducted from November 1, 2020 to November 30, 2021 at the Gonesse Hospital for patients anesthetized for total hip arthroplasty. The evaluation of professional practices was based on 14 recommendations (pre, per and postoperative) rated 0 to 2 and the average rating of 2/2 was made to assess the overall level of application of the recommendations. Results One hundred and eight patients were selected. Their average age was 77.4 ± 13.8 years old, predominantly women (65.7 %), American Society of Anesthesiologists (ASA) class 2 predominated (49.1 %), fracture was the main indication (49.1 %) and the prosthesis used was simple (88 %) and not cemented (74.1%). General anesthesia and spinal anesthesia were used at 50% each. Postoperative complications were infection (urinary: 2.8 % and surgical site: 2.8 %). Two patients died after the intervention. The average length of stay was <5 days in 57.4%. The pre and intraoperative variables were managed according to the recommendations (grade 2), unlike the postoperative. The overall level of application of the recommendations was 90.6 %. Conclusion In this series, the practice of improved rehabilitation after total hip arthroplasty is consistent with the recommendations. French Abstract Contexte & objectif. La mise en pratique des programmes de la récupération améliorée après arthroplastie de la hanche est essentielle dans le succès thérapeutique. La présente étude a évalué la pratique de la Réhabilitation Améliorée Après arthroplastie totale de la hanche à la lumière des recommandations de la Société Française d’Anesthésie Réanimation. Méthodes. C’était une étude observationnelle d’une série des patients anesthésiés pour arthroplastie totale de la hanche, menée du 1er novembre 2020 au 30 novembre 2021 ; à l’hôpital de Gonesse en France. L’évaluation des pratiques professionnelles avait porté sur 14 recommandations (en pré, per et postopératoire) cotées de 0 à 2. La côte de 2/2 était considérée pour apprécier le niveau global d’application des recommandations. Résultats. Cent-huit patients étaient enrôlés. Leur âge moyen était de 77,4 ± 13,8 ans avec preponderance féminine (65,7 %), et la classe ASA 2 prédominait (49,1 %). La fracture était l’indication majoritaire (49,1 %) et la prothèse utilisée était simple (88 %) et non cimentée (74,1 %). L'anesthésie générale et la rachianesthésie étaient utilisées à 50 % chacune. Les complications postopératoires étaient l’infection (urinaire : 2,8 % et du site opératoire : 2,8 %). Deux patients sont décédés à distance de l’intervention. La durée moyenne de séjour était < 5 jours dans 57,4 %. Les variables pré et peropératoires étaient gérées selon les recommandations (cote 2), à la différence du postopératoire. Le niveau global d’application des recommandations était de 90,6 %. Conclusion. Dans cette série, la pratique de la réhabilitation améliorée après arthroplastie totale de la hanche est conforme aux recommandations. Abstract:

Ce travail de revue présente le développement et l'état actuel de l'arthroplastie de la prothèse de hanche totale. Les progrès depuis l'introduction de cette opération il y a plus de 40 ans ont consisté avant tout en une diminution importante du taux de complications: infection, insuffisance du matériel utilisé, thromboembolie et instabilité aseptique. Le taux de survie à 10 ans de 97% ne devrait plus être dépassé. Le standard atteint à l'heure actuelle est devenu un grand obstacle pour les progrès futurs. Les facteurs les plus importants pour l'évaluation et le résultat sont les techniques opératoires – en cas d'implantation par ciment, de la technique du ciment, ensuite seulement le design, la qualité de la surface et l'implant lui-même. Le pronostic de la résistance d'une arthroplastie ne peut être établi que sur la base de critères négatifs subjectifs et objectifs bien précis. L'examen radiographique met principalement en évidence des modifications au niveau de la séparation implant – os et l'os périprothétique lui-même.

Abstract:

Background: Ceramics are inorganic nonmetallic materials and are used as bioinert components in joint replacement surgeries. Ceramics are known to be low allergenic. We experienced a ceramic-induced psoriasis. Objective: We report a first case of possible ceramic-induced psoriasis caused by a ceramic insert. Methods: A 55-year-old female received an implanted ceramic-on-ceramic total hip replacement for osteoarthritis of the right hip joint. Following surgery, she developed psoriatic lesions, which continued for 10 years. We suspected that psoriasis was caused by a ceramic insert and removed it surgically. Results: When the ceramic insert was replaced with a polyethylene-on-metal hip joint, the psoriatic lesions completely disappeared. Conclusion: The pathogenesis of psoriasis is still an enigma, although deregulation of nuclear factor κB signaling and resulting abnormal cytokine secretion are speculated to be involved. Ceramics may affect these signaling events and cause the onset of psoriasis. Renseignements de base: Les céramiques sont des matériaux non métalliques inorganiques et sont utilisées comme composants bio-inertes dans les arthroplasties. Les céramiques sont reconnues pour être peu allergisantes. Nous avons observé un psoriasis provoqué par la céramique. Objectif: Nous exposons un premier cas possible de psoriasis provoqué par la céramique causé par un implant en céramique. Méthodes: Une femme de 55 ans a reçu une prothèse céramique-sur-céramique implantée lors d'une arthroplastie totale de la hanche pour traiter l'arthrose de l'articulation de la hanche droite. Après la chirurgie, elle a développé des lésions psoriasiques dont la manifestation s'est poursuivie pendant 10 ans. Nous avons soupçonné que le psoriasis était causé par l'implant en céramique, et nous l'avons enlevé chirurgicalement. Résultats: Une fois l'implant en céramique remplacé par une prothèse articulaire en polyéthylène-sur-métal, les lésions psoriasiques ont disparu complètement. Conclusion: La pathogenèse du psoriasis demeure une énigme, bien qu'on suppose que la dérégulation de la voie de signalisation NF-κB et que la sécrétion anormale de cytokines résultante soient impliquées. Les céramiques peuvent affecter ces événements de signalisation et provoquer l'apparition du psoriasis.
