Abstract:
Anomali arteri koroner kongenital merupakan kasus yang jarang. Secara klinis, anomali arteri koroner bisa jinak atau mengancam jiwa tergantung pada prognosisnya. Laporan kasus ini bertujuan untuk menggambarkan pasien yang mengalami keluhan nyeri dada atipikal sebagai gejala awal yang kemudian dilakukan CT. Gambaran CT scan menunjukkan adanya kelainan anomali arteri koroner berupa tidak terbentuknya arteri koroner utama kiri dan sumber left anterior descendent artery (LAD) berasal dari sinus aorta kanan dan tidak ada bukti plak atau stenosis di arteri koroner utama, sumber left anterior descendent artery (LAD), Left Circumflex Artery (LCX) dan right coronary artery (RCA). Anomali ini termasuk dalam klasifikasi anomali asal arteri (Anomalous origin). LAD berasal dari sinus aorta kanan, dengan jalur pre-pulmonic sehingga hemodinamiknya masih baik dan tergolong dalam anomali tanpa iskemia. Kata kunci: Anomali arteri koroner, Tidak terbentuknya LMCA, Anomali origin.

Abstrak. Trauma tumpul toraks memiliki insiden yang sangat tinggi pada populasi dewasa, 20 – 50 % dari kasus dapat mengakibatkan kematian. Cedera pada struktur pembuluh darah dari bagian dada atas, terutama pembuluh darah arteri subklavia kiri, sangat jarang ditemukan dan biasanya disertai dengan “hematotoraks massif”. Observasi ketat dari tanda- tanda vital dan foto toraks serial sangatlah penting. Kami paparkan sebuah kasus jarang dari seorang pasien yang mengalami cedera traumatik arteri subklavia kiri. Tidak ditemukan gambaran klinis dari hematoma di supra klavikula, pulsasi arteri radialis kiri tidak teraba, dan tidak ditemukan pelebaran dari mediastinum pada foto toraks. Tetapi didapatkan hematotoraks massif kiri. Dilakukan posterolateral torakotomi kiri, dijumpai sumber perdarahan dari bagian apex paru kiri, dicurigai terdapatnya robekan dari pembuluh darah arteri subklavia kiri. Tindakan dilanjutkan dengan insisi median sternotomy yang diperluas ke arah supraklavikula kiri, ditemukan rupture total dari arteri subklavia kiri dengan jarak 1,5 cm proksimal dari arkus aorta. Tindakan repair (perbaikan) arteri secara primer (end to end anastomosis) dilakukan dengan hasil baik. Blunt thoracic trauma highest incidence is in adult, 20% to 50 % of the trauma cause death. Injuries to the vascular structures of the thoracic outlet, especially left subclavian artery, are rare and typically accompanied by massive hemorrhage. Close observation of vital sign and serial chest x-ray are very important. We describe an unusual presentation of a patient who suffered traumatic rupture of left subclavian artery. No clinical presentation of supraclavicular hematoma, unilateral absence of radial artery pulse and mediastinal widening in chest x-ray was found, but there is a massive hemothorax. Left posterolateral thoracotomy was performed, the source of bleeding was found in the apex of the lung, suspected a rupture of the left subclavian artery. The procedure continued with median sternotomy extended to left supraclavicular incision, a rupture was found in the left subclavian artery, 1,5 cm proximal to aortic arch. Introduction Angiosome formed by Anterior tibial artery and dorsalis pedis artery supply the ankle and the dorsum of foot. Both the vessels shows variation in termination and branching pattern. Materials and Methods 50 free formalin preserved limbs were dissected for the study. Results and Conclusion Anterior tibial artery showed variation in termination in 16%, most common of them was formation of loop with perforating branch of peroneal artery in 10%, bifurcation in to medial and lateral tarsal artery in 4% and trifutcation in 2%. Branching pattern of dorsalis pedis artey was variable in 18%, medial and lateral tarsal arteries were not branches of dorsalis pedis artery in 14% and 16% respectively. Arcuate artery was absent in 10%, when present it was branch of dorsalis pedis artery in 96%, perforating branch of peroneal artery in 2% and loop of medial and lateral tarsal artery in 2%. First dorsal metatarsal artery was not branch of dorsalis pedis artery in 4%. Lateral tarsal artery was constantly found in all the dissected limbs. Thorough knowledge of variation about branching pattern of arterial tree is clinically important for accuracy in interpretation of angiographs, Doppler study of lower limb vessels and optimum use of vascular pedicle for myocutaneous flaps. Importance of palpation of peripheral pulse in lateral tarsal artery is discussed in the study.

This publication presents the results of studies on biomineralization of “atherosclerotic plaque” forming in the coronary arteries of the heart. Obtained results were compared to studies of an artery unaffected by the process of atherosclerosis. Different types of artery biomineralization were marked, in terms of both mineral and chemical composition. Theories on the formation of crystallization centers where “atherosclerotic plaque” develops were also presented. Results of preliminary in vitro studies on dissolving cholesterol and phosphate mineralization in arteries were presented. The results were interpreted, and conclusions were offered.

Abstract:
In 1997, the first ambulatory off-pump left internal mammary artery (LIMA) to left anterior descending artery (LAD) anastomosis was performed via xiphoid approach, an intervention known today as the MINI-OPCAB operation. This operation has been performed in more than 150 patients around the world for the management of single and multiple vessel coronary artery disease, and 70 of those cases underwent the procedure in our Foundation from 1997 to 2021, showing no operative mortality or reintervention rates, low conversion rates, early hospital discharge, and 100% graft patency at the follow-ups. The minimally invasive direct coronary artery bypass (MIDCAB) procedure was introduced in 1994 by Benetti from Argentina, and was developed based on our previous experience performing off-pump coronary surgery by manipulating the ascending aorta through a left-sided small thoracotomy. Although it represents one of the most common minimally invasive procedures done worldwide, the original technique is not easy to reproduce, and in most cases the pleura is opened, which increases patients pain, morbidity, hospital length of stay, and hinders the realization of immediate hybrid revascularization after the procedure. The latest minimally invasive approaches for CABG include robotic-assisted procedures, in which the IMAs are harvested using robotic technology or the whole procedure is done using robotic technology. This approach has shown adequate short-term clinical results, however, there are no studies available reporting the procedure’s long-term results. Conclusion: The MINI-OPCAB operation is a friendly approach to perform Minimally Invasive Coronary Bypass Surgery in old, high-risk, multivessel coronary artery disease patients, and it represents a good intervention alternative for the majority of cardiac surgeons and cardiac specialized centers. We also consider this approach as ideal for patients who further require Hybrid Revascularization; however, more experience is needed to validate this initials results.

Coronary artery ectasy is a rare coronary disease often revealed by an acute myocardial infarction.The management of this pathology is not well established.We are reporting two cases of ST-elevation myocardial infarction associated with coronary artery ectasy.These cases were reported using electrocardiography and coronary angiography data and the modalities of treatment.Then a discussion was made on the basis of the literature concerning the topic of coronary artey ectasy and myocardial infarction.Our work adds to the literature by providing additional data on the link between artery coronary ectasy and acute myocardial infarction as well as the management modalities.
