Abstract:
Agrarian pruning and plantation removal (APPR) is a common residue from pomo-technical maintenance of orchards, vineyards, and olive groves, frequently overlooked by the energy sector. This paper sheds new light on the socio-economic role of APPR as solid biofuel. The paper suggests business models that could both alleviate energy poverty in rural areas and increase the competitiveness of perennial crop (fruits, olives, vine) production. The effects of mobilizing APPR potential via business models are estimated at the micro (households’ income) and macro (number of households affected, size of the investment, tax effect, CO2eq savings) levels in the Croatian setting. Contrary to classical energy planning, the paper provides an approach that fosters local use of bioenergy by linking rural development with renewable energy end-use. The approach linked pruning energy potential with 47% of Croatian rural households which allows creating an evidence-based policy for rural development utilizing APPR as a sustainable solid biofuel. The most attractive business model for Croatian rural areas is utilization of APPR as a solid biofuel. It could be used as an energy poverty alleviation measure, allowing savings per heating season equivalent to 5%–11% of the average disposable income per household. Net APPR influence at VAT tax is estimated at 5.5 million euro·y−1. The paper proves that using average values for APPR in energy planning could be misleading.

Abstract:
In patients with Alzheimer’s disease (AD), synaptic plasticity seems to be involved in cognitive improvement induced by cognitive training. The platelet amyloid precursor protein (APP) ratio (APPr), i.e., the ratio between two APP isoforms, may be a useful peripheral biomarker to investigate synaptic plasticity pathways. This study evaluates the changes in neuropsychological/cognitive performance and APPr induced by cognitive training in AD patients participating in the “My Mind Project”. Neuropsychological/cognitive variables and APPr were evaluated in the trained group (n = 28) before a two-month experimental protocol, immediately after its termination at follow-up 1 (FU1), after 6 months at follow-up 2 (FU2), and after 24 months at follow-up 3 (FU3). The control group (n = 31) received general psychoeducational training for two months. Some memory and attention parameters were significantly improved in trained vs. control patients at FU1 and FU2 compared to baseline (Δ values). At FU3, APPr and Mini Mental State Examination (MMSE) scores decreased in trained patients. Δ APPr correlated significantly with the Δ scores of (i) MMSE at FU1, (ii) the prose memory test at FU2, and (iii) Instrumental Activities of Daily Living (IADL), the semantic word fluency test, Clinical Dementia Rating (CDR), and the attentive matrices test at FU3. Our data demonstrate that the platelet APPr correlates with key clinical variables, thereby proving that it may be a reliable biomarker of brain function in AD patients.

Abstract:
Reconfigurable antenna systems have gained much attention for potential use in the next generation wireless systems. However, conventional direction-of-arrival (DoA) estimation algorithms for antenna arrays cannot be used directly in reconfigurable antennas due to different design of the antennas. In this paper, we present an adjacent pattern power ratio (APPR) algorithm for two-port composite right/left-handed (CRLH) reconfigurable leaky-wave antennas (LWAs). Additionally, we compare the performances of the APPR algorithm and LWA-based MUSIC algorithms. We study how the computational complexity and the performance of the algorithms depend on number of selected radiation patterns. In addition, we evaluate the performance of the APPR and MUSIC algorithms with numerical simulations as well as with real world indoor measurements having both line-of-sight and non-line-of-sight components. Our performance evaluations show that the DoA estimates are in a considerably good agreement with the real DoAs, especially with the APPR algorithm. In summary, the APPR and MUSIC algorithms for DoA estimation along with the planar and compact LWA layout can be a valuable solution to enhance the performance of the wireless communication in the next generation systems.

Abstract:
Background & Purpose This article focuses on the current educational reform movement in New York State resulting from the state's receipt of $700 million in Race to the Top (RTTT) money. Specifically, the article will focus on one aspect of the RTTT requirement, which requires that school districts develop teacher accountability systems that are based in part on test data, i.e., the Annual Professional Performance Review (APPR). We will provide an account of how the New York State Education Department's implementation of RTTT has changed the role of educators, eroded autonomy in publicly controlled schools, promoted a culture of mistrust, diverted funds from the classroom to meet governmental directives, and paved the way for corporate vendors to profit from taxpayer money. Finally, we will examine whether the APPR policy developed to hold teachers accountable using an objective metric is a reliable and valid one. Research Design We examine the APPR legislation passed by both the legislative and executive bodies of New York State by focusing on field guidance documents and legislation released by the State Education Department (SED) as well as memos we received from SED. We also review how school districts have decided to implement APPR in their local environment. Finally, articles appearing in the press about the APPR have also been surveyed to ascertain key themes about the question whether teacher effectiveness can be objectively measured by those standards set forth by the SED. Conclusions The APPR policy as it is currently implemented is an unreliable tool in measuring teacher performance. Its subjectivity and inconsistent implementation calls into question the core purpose of this reform, i.e. to rid schools of poor performing teachers, while identifying those that are excellent. The implementation of RTTT and APPR has deteriorated the quality of public education in New York State by creating confusion through untested policies, creating a culture of distrust, diverting money from the classroom to for profit vendors, and developing a pedagogical methodology of teaching to the test.

We provide one of the first formal accounts of apprehension: In a structure [p [q=sa'ne]], =sa'ne 'APPR' encodes a modal semantics where the goal worlds of the actor responsible for p avoid a salient situation r=>q. Finally, we reveal two inherent asymmetries among apprehensional functions (precautioning asymmetry and timitive asymmetry), thus making substantial predictions with regards to typological patterns in apprehensional morphology.

Abstract:
For a long time, the equivalent ionospheric slab thickness τ has remained in the shadow of ionospheric main parameters: the maximum density, NmF2 (or the critical frequency, foF2), and the total electron content. Empirical global models have been developed for these two parameters. Recently, several global models of τ have appeared concurrently. This paper compares τ of the Neustrelitz equivalent slab thickness model (NSTM), with τ(IRI-Plas) of the IRI-Plas model, and τ(Appr) of the approximation model, constructed along the 30° E meridian using data from several ionosondes. The choice of the model of the best conformity with observational data was made, which was used to study the effects of space weather during several magnetic storms in March 2012. The effects included: (1) a transition from negative disturbances at high latitudes to positive ones at low latitudes, (2) the super-fountain effect, which had been revealed and explained in previous papers, (3) a deepening of the main ionospheric trough. The efficiency of using τ(Appr) and τ(IRI-Plas) models for studying the effects of space weather has been confirmed. The advantage of the τ(Appr) model is its closeness to real data. The advantage of the τ(IRI-Plas) model is the ability to determine foF2 without ionosondes. The efficiency of the NSTM model is insufficient for a role of a global τ model due to the accuracy decreasing with the increasing latitude.

Abstract:
ABSTRACT Replication of the ∼30-kb plus-strand RNA genome of coronaviruses and synthesis of an extensive set of subgenome-length RNAs are mediated by the replicase-transcriptase, a membrane-bound protein complex containing several cellular proteins and up to 16 viral nonstructural proteins (nsps) with multiple enzymatic activities, including protease, polymerase, helicase, methyltransferase, and RNase activities. To get further insight into the replicase gene-encoded functions, we characterized the coronavirus X domain, which is part of nsp3 and has been predicted to be an ADP-ribose-1"-monophosphate (Appr-1"-p) processing enzyme. Bacterially expressed forms of human coronavirus 229E (HCoV-229E) and severe acute respiratory syndrome-coronavirus X domains were shown to dephosphorylate Appr-1"-p, a side product of cellular tRNA splicing, to ADP-ribose in a highly specific manner. The enzyme had no detectable activity on several other nucleoside phosphates. Guided by the crystal structure of AF1521, an X domain homolog from Archaeoglobus fulgidus, potential active-site residues of the HCoV-229E X domain were targeted by site-directed mutagenesis. The data suggest that the HCoV-229E replicase polyprotein residues, Asn 1302, Asn 1305, His 1310, Gly 1312, and Gly 1313, are part of the enzyme's active site. Characterization of an Appr-1"-pase-deficient HCoV-229E mutant revealed no significant effects on viral RNA synthesis and virus titer, and no reversion to the wild-type sequence was observed when the mutant virus was passaged in cell culture. The apparent dispensability of the conserved X domain activity in vitro indicates that coronavirus replicase polyproteins have evolved to include nonessential functions. The biological significance of the novel enzymatic activity in vivo remains to be investigated.

Abstract:
The biggest challenge in moving toward a safer healthcare system is patient safety culture—that is, the prevention of harm to patients. Safe medical practices can prevent doing harm to the patients. For this, healthcare professionals must have good attitudes toward patient safety. Medical education plays an important role in promoting patient safety and patient safety attitudes. A study was conducted among medical students in Malaysia to assess their perceptions toward patient safety, using the 26-items Attitudes Toward Patient Safety Questionnaire (APSQ-III). In the analysis, the average percentage of positive responses (APPR) were computed for each domain, and APPR values of ≥75 were used as an indicator of positive perception. Out of the nine domains of APSQ, the students’ attitude was positive in six—Safety Training (85.2%), Error Reporting (76.3%), Working Hours (89.5%), Error Inevitability (86.1%), Team Functioning (94.6%), and Patient Involvement (80.1%). The desired level of positive attitude was not met in Disclosure Responsibility (68.5%), Professional Incompetence (70.0%), and Safety Curriculum (71.1%). APRR for disclosure responsibility was high among the first-year students, but, generally, the effect wore off over the years of study. The results support the need to enhance perception on Disclosure Responsibility, Professional Incompetence, and Safety Curriculum among the medical students in Malaysia.

Straipsnyje pateikiami pedagoginio eksperimento, atlikto Vilniaus Gedimino technikos universitete, rezultatai.

Abstract:
BackgroundAnkylosing Spondylitis (AS) is an inflammatory rheumatic disease affecting the axial skeleton (1), with a prevalence of up to 0.5% in Europe (2; 3), and occurs in men twice as often than in women (4). 80% develop AS symptoms before the age of 30 (4). Nonsteroidal Anti-Inflammatory Drugs (NSAIDs) and biological Disease-Modifying Antirheumatic Drugs (DMARDs) such as Tumour Necrosis Factor inhibitors (TNFi) are the first choice in pharmacological treatments for AS patients (5; 6). Data on disease activity and disease control in AS patients treated in real life settings are limited.ObjectivesTo evaluate the average disease activity status in the study population and assess the percentage of AS patients with a suboptimal treatment response following at least 12 weeks of treatment with TNFi and/or NSAIDs.MethodsINVISIBLE was a real-world evidence study in Germany, Austria and Belgium. Adult patients with confirmed diagnosis of AS for at least 6 months and ongoing treatment with TNFi and/or NSAIDs for at least 12 weeks prior to enrolment were included. Primary data were collected at a single visit and medical history data from available patient files.Results747 adult AS patients were included. 67% of patients were male, mean symptom duration 18.2 years. 77% of 696 patients were HLA-B27 positive. Appr. 23% of patients received NSAIDs, 42% received TNFi, 30% received both NSAIDs and TNFi, 5% received other treatment for at least 12 weeks prior to enrolment. Mean Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) and Ankylosing Spondylitis Disease Activity Score (ASDAS) scores were 3.4 and 2.2, respectively. Appr. 40% of patients achieved a BASDAI score ≥4 and appr. 50% an ASDAS ≥2.1. Patients with a BASDAI score ≥4 achieved higher scores in most physician and patient reported outcomes. Overall appr. 35% of patients were not satisfied with their current symptom state according to Patient Acceptable Symptom State (PASS). Of patients with a BASDAI score ≥4 appr. 61% were not satisfied according to PASS.ConclusionResults from the non-interventional study INVISIBLE showed that a high proportion of AS patients receiving NSAIDs and/or TNFi have high disease activity according to BASDAI and ASDAS in real life settings. These results suggest that in some patients, disease control and treatment response are suboptimal under current treatment strategy.Table 1.Demographics and Disease CharacteristicsTotal (N=747)BASDAI <4 (N=445)BASDAI ≥4 (N=292)NResultNResultNResultAge in years, mean (±SD)74647.9 (±13.1)44546.7 (±13.4)29249.6 (±12.3)*Male, n (%)746500 (67.0)445318 (71.5)292176 (60.3)Age at AS diagnosis in years, mean (±SD)74535.2 (±12.3)44534.1 (±12.0)29236.8 (±12.5)*Treatment with NSAIDs, n (%)744171 (23.0)44585 (19.1)29085 (29.3)Treatment with TNFi, n (%)744311 (41.8)445208 (46.7)290100 (34.5)Treatment with NSAIDs and TNFi, n (%)744222 (29.8)445126 (28.3)29092 (31.7)Treatment with other, n (%)74440 (5.4)44526 (5.8)29013 (4.5)Patient disease activity assessment (VAS 0-100), mean (±SD)73535.8 (±27.5)44420.7 (±19.7)28659.1 (±20.7)***BASDAI Score, mean (±SD)7373.4 (±2.3)4451.8 (±1.1)2925.8 (±1.3)***ASDAS Score, mean (±SD)5992.2 (±1.0)3631.7 (±0.6)2363.1 (±0.7)***ASDAS ≥2.1, n (%)599297 (49.6)36373 (20.1)236224 (94.9)***PASS: current state not satisfactory, n (%)679240 (35.3)40372 (17.9)271166 (61.3)***SD – Standard Deviation; Level of significance: *< 0.05; ** < 0.01 *** < 0.001 (analyses refer to differences between BASDAI <4 and ≥4 and to respective available patient number).References[[1] Braun, J and J, Sieper.Lancet. 2007, Vol. 369, pp. 1379–90.[2]Braun, J, et al.Arthritis Rheum. 1998, Vol. 41, 1, pp. 58-67.[3]Dean, LE, et al.Rheumatology. 2014, Vol. 53, pp. 650-657.[4]Feldtkeller, E, et al.Rheumatol Int. 2003, Vol. 23, pp. 61–66.[5]Dougados, M, et al.Ann Rheum Dis. 2002, Vol. 61, suppl 3, pp. 40–50.[6]Braun, J, et al.Ann Rheum Dis. 2006, Vol. 65, pp. 316–20.Disclosure of InterestsJan Brandt-Juergens Consultant of: Abbvie, Affibody, BMS, Gilead, Janssen, Lilly, Medac, MSD, Novartis, Pfizer, Roche, Sanofi-Aventis, UCB, Judith Haschka Speakers bureau: Eli Lilly, Amgen, Consultant of: Eli Lilly, Richard Finsterwalder Employee of: Novartis Pharma GmbH, Aurélie Casier Employee of: N.V. Abstract:
Introduction. Currently there is insufficient data regarding routine use of various topical treatments for plaque psoriasis outside from randomized clinical trials, their efficacy, cost-effectiveness and perception by dermatologists and patients. The primary rationale for this study was local data generation for the long-term (up to 52 weeks) treatment of body psoriasis in Russian patients’ population with fixed-dose combination of calcipotriol and betamethasone dipropionate in the formulation of gel to evaluate patient’s adherence to the therapy and their satisfaction level.Materials and methods. Primary study objectives were evaluation of the long-term (up to 52 weeks) treatment success of body psoriasis with fixed-dose combination of calcipotriol and betamethasone dipropionate gel by Physician (Physician Psoriasis Global Assessment (PsGA, 5-point scale)) and by patient (Patient’s Global Psoriasis Assessment (PaGA, 5-point scale) and overall patient’s satisfaction using 4-point scale), patient’s satisfaction with the treatment via Patient Preference Questionnaire (PPQ), patients’ adherence to the treatment using Questionnaire of Adherence, impact of treatment on patients’ quality of life using DLQI questionnaire, and patients’ readiness to buy topical therapy product out of pocket based on consumption of the study medication during follow up period. Patients received treatment with topical calcipotriol/betamethasone gel which was prescribed as a routine medication without connection to the study purposes for up to 8 weeks. After the treatment patients were followed up for up to 52 weeks; during this period they were allowed to continue treatment with either maintenance or intermittent schedule. 128 dermatologists participated in the study, 612 patients with body plaque psoriasis (mean age 42.0 ± 13.30 years) were included; from this sample 544 (88.7 %) patients underwent follow up.Results. All 612 included patients received initial therapy with calcipotriol/betamethasone gel; during follow-up period 410 (76.1 %) patients received treatment by any schedule (out of 539 assessed). Overall treatment success based on average PsGA score: initially 3.0 ± 0.54, after appr.8 weeks — 1.3 ± 0.80 and after appr.52 week 1.1 ± 0.96; based on average PaGA score — initially 3.0 ± 0.54, after appr.8 weeks — 1.5 ± 0.88 and after appr.52 week 1.1 ± 0.99. The proportion of patients which described their overall satisfaction with the treatment as “satisfied” or “absolutely satisfied” equaled 92.3 % at visit at the end of active treatment and 93.1 % at visit up to 52 weeks afterwards. Also at visit at the end of active treatment 93.8 % of patients considered their current treatment as “more effective” than previous topical treatment they received. Patients’ adherence to the therapy was satisfactory — average application rate of calcipotriol/betamethasone (should be applied daily) gel at the last 7 days of treatment at visit at the end of active treatment comprised 5.6 ± 2.6 days, which means that 6 out of 7 days of the treatment were not missed. Initial DLQI score was 13.6 ± 5.95 and it reduced down to 3.89 ± 5.95 at visit at the end of active treatment (p < 0.001) reflecting positive impact of successful therapy on patients’ psychosocial wellbeing.Discussion. Results of this study provides additional information regarding routine topical therapy in patients with plaque psoriasis. These findings demonstrate that the therapy of body plaque psoriasis with calcipotriol/betamethasone gel is effective both in short and long term according to the evaluation by dermatologists and by patients themselves. This therapy provides good rate of adherence and patients’ satisfaction as well as affects positively at patients’ psychosocial wellbeing assessed by DLQI score.

Abstract:
The relationship between the stress protein response and the acute phase response (APPR) was studied in human hepatoma cells to investigate the hierarchy of regulation of these survival responses. Huh-7 cells were subjected to heat treatment (febrile-range temperature 40°C or heat shock 43°C) followed by recovery at 37°C in the presence or absence of IL-6 given either before or after heat treatment. The effects on total, fractional, and acute phase protein synthesis were then analyzed by metabolic labeling, ELISA, real-time PCR, Northern blot analysis, and activation of an α1-antitrypsin reporter plasmid. Cell energetics were studied under the same conditions using an index of mitochondrial activity and measurement of cellular ATP levels. Febrile-range temperature (40°C) augmented acute phase protein production when cells had been pretreated with IL-6. Pretreatment of cells with IL-6 also prevented heat shock-induced suppression of α1-antichymotrypsin (ACT) but not transferrin. mRNA expression of ACT and α1-antitrypsin reporter activation studies was consistent with transcriptional regulation of these proteins. Expression of mRNA transcripts for transferrin was increased despite protein expression being reduced by heat shock. The effects of heat shock on acute phase protein synthesis can be modified by preincubation with IL-6, whereas addition of this ligand after heat treatment has no effect on the suppressive effect of heat on the APPR. The mechanism of this action appears to be transcriptionally regulated in the case of ACT, but in the case of transferrin, it may be mediated by another process such as posttranslational modification.

AbstractIn order to test the validity of the usual neglect of pressure line shifts in investigations of Fe I line asymmetries and shifts, we calculated synthetic line bisectors and shifts of 96 moderate and weak photospheric lines, taking into account both hydrogen- and electron-impact shifts and neglecting the velocity fields. Absolute pressure line shifts obtained are less than or appr. equal to 50 m/s and the largest relative shifts along the bisectors are about 40 m/s. We found pronounced dependences of both shifts and bisectors on the energy of the upper level in transitions and on the transition array.

Abstract:
Background and Method: This study compared the effects of self-management mHealth (mobile health) apps and conventional intervention (brochure) on the quality of life (QoL) and exercise engagement of patients with breast cancer. Moreover, the current research aimed to investigate how usability improvement in mHealth leads to increased exercise engagement and QoL. To select apps with different level of usability, a heuristic usability testing using was conducted. Three apps with varying scores of usability - low, average, and high- were selected to enter in a randomized control trial (RCT) experiment. We formed 4 groups of intervention including the app with the highest usability (App5), the app with average usability (App4), the app with the lowest usability (App3), and the brochure. In each group, 5 patients, randomly selected from 20 patients, entered to the experiment and started to use the assigned mHealth app for 6 weeks. Results: The results showed that QoL scores were significantly higher for all participants in app groups compared to the broacher group. Further analysis of the results showed that the patients in App5 group reported higher QoL compared to patients in App4, App3, and brochure groups. In addition, patients in App5 and App4 groups reported more engagement in physical activities. Self-reported engagement in exercise data was not wholly consistent with engagement data logged by phones. Usability evaluation of apps from the patients’ perspective showed that, similar to heuristic usability, the patients rated App5 with higher usability score. The correlation analysis showed that user usability score was positively correlated with heuristic usability score. Moreover, other correlation analysis revealed that patients who rated the app with higher usability score, reported more commitment to physical activities. These results may imply that, compared to conventional intervention (brochure), mHealth apps engaged the patients in physical activities with a higher degree, and consequently improved their QoL.

Abstract:

Abstract The present analysis discusses the narratological structures of the Danish folk ballad on ‘Germand Gladensvend’ (‘Happy Germand’, DgF 33). The text is preserved in six different manuscript versions, all of them written before appr. 1630, and in a printed version from 1591. Although the story is strongly related in all of them, including stanzas that are identical almost word for word, the different versions nevertheless have rather different narratological structures, depending on the strong variation between the conclusions of the different versions. In order to make these differences clear, the analysis uses classical narratological concepts, like movements of objects of value and the semiotic square. Scholarship frequently applies the terms centre and periphery to different parts of Western Christendom, but there is no consensus on exactly which lands can be characterized by these terms. This paper aims at appr oaching the centre periphery problem in Western Christendom through two case studies: the archbishoprics of Uppsala and Spalato, both lying on the rim of the Latin West, were chosen as the objects of the analysis. On the basis of papal letters from the tim e of Pope Alexander III (1159 1181) the intensity and nature of contacts between the Holy See and these “faraway places” were studied. The main question addressed was what perceptions the Roman Curia had of these territories in the second half of the twelf th century.

Abstract:
Abstract Background: Maternal depression has been linked to adverse outcome in the offspring. Existing literature is mainly based on parental reports, which can be fallible when the parent suffers from depression. Aim: The aim of this study was to explore if maternal depression was associated with daily health complaints and low self-assessed health (SAH) in the offspring. Design and setting: Participants were 45,727 children from the Danish National Birth Cohort. Maternal depression was categorised into: no depression, first-time treatment, continued treatment, post treatment and relapse. Methods: Binomial regression was used to estimate the adjusted prevalence proportion ratio (aPPR) of frequent health complaints and low SAH in children of depressed mothers compared to children of non-depressed mothers. Results: The prevalence of any frequent health complaint was 10.8%, frequent somatic complaints was 3.2%, frequent mental complaints 8.7%, both frequent mental and somatic complaints 1.2%, and low SAH 5.3%. Children of mothers with depression (any category) were more likely to report a frequent health complaint; first-time treatment: aPPR=1.35 (95% CI: 0.96-1.85), continued treatment: aPPR=1.59 (1.37-1.85), post treatment: aPPR=1.30 (1.20-1.41), and relapse: aPPR=1.56 (1.35-1.79). Children of mothers with depression were also more likely to report low SAH; first-time treatment: aPPR=1.58 (0.99-2.54), continued treatment: aPPR=1.86 (1.51-2.28), post treatment: aPPR=1.34 (1.19-1.50), and relapse: aPPR=1.56 (1.26-1.93). Conclusions: Treatment of maternal depression was associated with higher prevalence of frequent health complaints and low SAH in the offspring at age 11 years. The association was strongest for children of mothers with continued depression or relapse.

Abstract:
Abstract Background Pregnancy losses remain a neglected issue and it will be taking more than a century before a pregnant woman in Sub Sahara has the same chance of her baby being born alive as a woman in a high-income country. Pregnancy loss data are limited and not universal in Sub Saharan countries. This study was aimed to assess the magnitude and determinants of pregnancy loss in eastern Ethiopia. Methods This study was conducted in, open continues and dynamic cohort of population, Kersa Health and Demographic Survillance site (HDSS) in Eastern Ethiopia in 2008–2019. All mothers who had known pregnancy outcomes during the period and reside in Kersa HDSS were considered. The prevalence proportions were calculated as the sum of all pregnancy loss divided by the number births in the specified year. Log-Binomial regression was used to determine factors associated with pregnancy loss. Prevalence Proportion Ratio (PPR) was used to report the magnitude and strength of association. A p-value of less than 0.05 was considered statistically significant. Results From 39,153 included pregnancies, 810 (20.7; 95%CI:19.32, 22.15 per 1000 births) experienced pregnancy loss. Stillbirth was higher than abortion (11.14 Vs. 9.55 per 1000 births). Lacking own income (aPPR:1.26; 95%CI: 1.01, 1.58), being daily laborer (aPPR:1.44; 95%:1.08, 306) history of previous pregnancy loss (aPPR:2.26, 95%CI:1.69, 3.03), unwanted pregnancy (aPPR:1.26; 95%CI:1.01, 1.80), not receiving antenatal care (aPPR:1.59; 95%CI: 1.19, 2.13) and not receive the TT-vaccine during pregnancy (aPPR:1.33; 95%CI: 1.08, 1.80) were positively associated with pregnancy loss. Conclusions The overall rate pregnancy loss was ranged between 19.32, 22.15 per 1000 births with higher still births than miscarriage or abortion. Pregnancy loss was positively associated with social factors reproductive health factors, and maternal health service utilization.

Abstract:
Abstract Background The latest AHA/ACC cholesterol guidelines on primary prevention of coronary artery disease (CAD) recommend the use of the CAC-score to help in decision making for not using or using statins: If the CAC-score is 0, it is reasonable to withhold statin therapy (as long as higher conditions are absent). If CAC-score is 1 - 99, it is reasonable to initate statin therapy for patients ≥55 years of age. If the CAC-score is ≥100, it is reasonable to initiate statin therapy. Therefore, in the present analysis, we assessed the impact of these guidelines in everyday cardiology practice. Methods We analysed our data base with 16083 assessments of the CAC-score in persons with no known coronary or other cardiovascular disease and no exercise-dependent chest pain or shortness of breath. The CAC-score was determined with a multi-slice CT. Using the “step-and-shoot” acquisition protocol, the average dose was around 1 mSv. Results In the total group, a CAC-score of 0 was found in 35%, a CAC-score of >0 up to <100 in 36% and ≥100 in 29%. The percentage of the above mentioned 3 CAC-score groups depending on age and gender are listed in table 1. Conclusion With the support of the CAC-score, a prescription of statins can be avoided in up to appr. 60% of middle-aged male and up to appr. 80% of middle-aged female persons. On the other hand, the use of statin is reasonable in appr. two thirds of higher-aged male and one third of higher-aged female persons for primary prevention. Abstract:
The considerable portion of kaolin used in Russia was imported from Ukraine. There is urgent necessity to assume the measures for the import substitution in consideration of the presence of suitable deposits. The Zhuravliny Log kaolin deposit (Chelyabinsky district, Russia) is the largest one (more than sixty million tons of assured resources of the primary kaolin) in Russia. The chem-ical and phase composition of the concentrated kaolin was determined. The SiO2/Al2O3ratio made 1.30. The free quartz quantity was equal to 4.4%. CaO and mica were not revealed. Kaolin powders were fine-dispersed (mainly up to 2 μm). In his paper, the thermal behavior of this kaolin was studied by the complex thermal analysis, X-ray diffractometry, and Fourier transform infrared spectroscopy. It was shown that the dehydroxylation occurred at appr. 500 °С. Further, at 910 °Сmetakaolinite probably turned into silica spinel. The absence of a peak at appr. 250–300 °C implies the absence of the free gibbsite Al(OH)3 or goethite FeOOH. By size of reflexes in the range of 2θ20–22°it was estimated the Hinckley index (HI) as the structure order indicator: HI made 1.76 that indicated rather high degree of order. After a heat treatment at 400 °С index reduced to 1.69. Crystallite size along the c-axis amounted 61.5 nm. Mullite was the main phase at 1200 °С.
