The effects of oxytocin and vasopressin on the brain and behavior can be sexually dimorphic, especially during the course of development (Bales, Kim, et al., 2004; Bales, Pfeifer, et al., 2004; Bales, Plotsky, et al., 2007; Bielsky et al., 2005a; Carter, 2003; Thompson et al., 2006; Yamamoto et al., 2005; Yamamoto et al., 2004). Given the sexual discrepancy observed in autism spectrum disorders (ASDs), these two neuropeptides, oxytocin (OT) and arginine vasopressin (AVP), have received attention for their potential role in ASDs (Green and Hollander, 2010; Insel et al., 1999; Leckman & Herman, 2002; Welch et al., 2005; Winslow, 2005; Young et al., 2002). Changes in either OT or AVP and their receptors could be capable of influencing symptom domains or behaviors associated with ASDs. Arginine vasopressin is androgen dependent in some brain regions (De Vries & Panzica, 2006), and males are more sensitive to AVP, especially during development. We hypothesize here that AVP, which has a unique role in males, must be present in optimal levels to be protective against ASDs. Either excess AVP or disruptions in the AVP system could play a role in development of the traits found in ASDs. In contrast, OT may possibly be secreted in response to adversity, especially in females, serving as a protective factor.

This chapter focuses on the transformation of the Turkish media field as a result of the shifts in media ownership, the cultivation of AKP-friendly media conglomerates, the consequent upsurge in partisanship, and the decline in press freedoms. It traces the connections between these developments and the broader political economic forces, such as the economic crisis of 2001, the AKP's electoral hegemony, the decline of military tutelage, the entrenchment of Muslim bourgeoisie, and the new Islamist cadres in governmental and administrative structures. It argues that in Turkey's contemporary media landscape, commercial outlets have been simultaneously independent of the state and dependent on it. They are not formally owned, operated, or dominated by the state, yet their survival depends on their informal ties with the ruling elite, high bureaucracy, and judiciary. While this dependency on the state is not a new development, it has nonetheless revealed itself in astounding ways under the AKP's single-party rule.

Apolipoprotein (apo) A-I is the key structural protein of high-density lipoprotein (HDL) and is necessary for sustaining the circulating level of HDL. It has also been studied extensively for its role in mediating many of the antiatherosclerotic and antithrombotic properties of HDL. More than 50 naturally occurring mutations and variants have been described, and they usually result in marked HDL deficiency in a gene-dose dependent manner. However, the propensity to develop accelerated coronary heart disease (CHD) is highly heterogeneous. Mutations resulting in inability to synthesize apo A-I tend to be associated with early CHD, while mutations resulting in structurally altered apo A-I are generally not associated. Furthermore, a number of apo A-I variants, for example apo A-I Iowa or apo A-I Helsinki, have been linked to amyloidosis, resulting in potentially serious morbid complications.

The study of gender in American political development (APD) challenges the efficacy for advancing women’s political inclusion of a liberal tradition valorizing principles of individual equality and positing a separation of the family and the state. Masked are ways in which gender roles and the family are integral to governance and state-building. Gender is both a dependent and an independent variable in APD. Shaped by institutions and policies of the state, it also shapes institutions and policies that promote women’s political citizenship and expand the state’s capacity for social provision—by asserting not only liberal claims of women’s equality with men, but also by invoking maternalist claims based on women’s difference from men, thereby challenging and altering relationships between public and private spheres.

While early works in American political development (APD) incorporated congressional actors in accounts of state-building, policymaking, and social reform, there is a growing body of historically oriented scholarship that places the institution of Congress front and center. We highlight three major streams of contemporary congressional research that engage with APD. The first analyzes the development of congressional institutions, often drawing upon concepts of path dependence and layering to understand the presence or absence of change in legislative operations. Second, several important studies of state-building and policy development highlight the role of congressional actors in driving—or blocking—critical political and social reforms. Finally, new datasets that track congressional elections and roll call voting over long time spans have given rise to a growing literature that uses historical evidence to test contemporary theories of legislative behavior. We close with a discussion of the contributions and pitfalls of using historical evidence in this way.

Iodine is a key component of thyroid hormones. Development of the fetal brain and nervous system are dependent on thyroid hormones supplied by the mother via the placenta, increasing the maternal demand for iodine throughout pregnancy. Women with adequate iodine intake before conception (~150 #amp;#x03BC;g/day) can adapt to the increased demand for thyroid hormones during pregnancy, because the thyroid gland adjusts its hormonal output; but this depends on sufficient availability of dietary iodine and the integrity of the thyroid gland. Iodine deficiency causes congenital hypothyroidism, and in severe form, the irreversible brain damage associated with cretinism. Moderate iodine deficiency in pregnancy is associated with lower learning capacity, reduced IQ, hearing impairment, and increased risk of attention deficit disorder. Pregnant women should take a daily multivitamin that contains 150 #amp;#x00B5;g of iodine, unless they regularly consume concentrated food sources of iodine.

The intensive care unit (ICU) can be defined as an area reserved for patients with potential or established organ failure and has the facilities for the diagnosis, prevention, and treatment of multi-organ failure. Usually, the ICU is located in close proximity to A & E, the radiology department, and the operating theatres, as it is between these areas that patient flows are greatest. In large urban hospitals, there may be more than one ICU, some of which serve specific patient populations, such as paediatrics, neurosurgery, cardiothoracic surgery, liver failure, and burns. Many hospitals also have high-dependency units (HDUs) that offer higher nurse-to-patient ratios and more advanced monitoring than a general wards does, as well as limited organ support. In the UK, the distinctions between ICU, HDU, and general ward have been abandoned in favour of a classification based on the patient’s needs rather than their location.

Research has shown that ketosis is neuroprotective against ischemic insults in rodents. This review focuses on investigating the mechanistic links to neuroprotection by ketosis in the aged. Recovery from stroke and other pathophysiological conditions in the aged is challenging. Cerebral metabolic rate for glucose, cerebral blood flow, and the defenses against oxidative stress are known to decline with age, suggesting dysfunction of the neurovascular unit. One mechanism of neuroprotection by ketosis involves succinate-induced stabilization of hypoxic inducible factor-1alpha (HIF1α‎) and its downstream effects on intermediary metabolism. The chapter hypothesizes that ketone bodies play a role in the restoration of energy balance (stabilization of ATP supply) and act as signaling molecules through the up-regulation of salvation pathways targeted by HIF1α‎.

A mortgage is a proprietary interest that can be legal or equitable in status. The equity of redemption encapsulates the rights of a mortgagor and includes the equitable right to redeem and the ability to have certain clauses struck out from a mortgage agreement. Inorganic phosphate ions (H2PO4−/ HPO42−) (abbreviated as Pi) are involved in formation of bone and generation of high-energy bonds (e.g. ATP), metabolic pathways, and regulation of cellular functions. In addition, Pi is a component of biological membranes and nucleic acids. Only about 1% of total body Pi content is present in extracellular fluids, at a plasma concentration in adults within the range 0.8–1.4 mMol/L (at pH 7.4 mostly as HPO42−), with diurnal variations of approximately 0.2 mM. A small amount of plasma Pi is bound to proteins or forms complexes with calcium. Under normal, balanced conditions, absorption of dietary Pi along the small intestine equals the output of Pi via kidney and faeces. Renal excretion of Pi represents the key determinant for the adjustment of normal Pi plasma concentrations. Renal reabsorption of Pi occurs along the proximal tubules by sodium-dependent Pi cotransporters that are strictly localized at the apical brush border membrane. Parathyroid hormone (PTH) and FGF23 are key regulators amongst a myriad of factors controlling excretion of Pi in urine, mostly by changes of the apical abundance of Na/Pi cotransporters. Hypophosphataemia may result in osteomalacia, rickets, muscle weakness, and haemolysis. Hyperphosphataemia can lead to hyperparathyroidism and severe calcifications in different tissues.

The Augustinian vision of humankind, on which so much Christian thinking about war is based, is false. Thanks to Darwinian evolutionary biology we know there was no original couple, Adam and Eve; there was no eating of the apple; there is no original sin. We are not innately depraved in this way. Morbid fatalism is inappropriate. The killer-ape vision of humankind, on which so much Darwinian thinking about war is based, is equally false. Thanks to updated Darwinian evolutionary biology, we know that we did not evolve in the violent ways often presumed, and that in major respects we are designed to avoid war. Culture, particularly agriculture, changed much of that and war became common. Changing this is not to go against our nature. Naïve optimism is no more in place. There is hope of more constructive engagement between Christians and Darwinians. On the Christian side, there are alternative theologies to Augustinian Atonement theology, notable Incarnational theology, not dependent on a literal Adam and Eve. On the Darwinian side, there are fresh empirical findings and interpretations, with truer understandings of human history and nature. Perhaps now, together, we can move forward the debate on the nature and causes and possible ending of human warfare.

Hard coal mining is characterised by features that pose numerous challenges to its current operations and cause strategic and operational problems in planning its development. The most important of these include the high capital intensity of mining investment projects and the dynamically changing environment in which the sector operates, while the long-term role of the sector is dependent on factors originating at both national and international level. At the same time, the conditions for coal mining are deteriorating, the resources more readily available in active mines are being exhausted, mining depths are increasing, temperature levels in pits are rising, transport routes for staff and materials are getting longer, effective working time is decreasing, natural hazards are increasing, and seams with an increasing content of waste rock are being mined. The mining industry is currently in a very difficult situation, both in technical (mining) and economic terms. It cannot be ignored, however, that the difficult financial situation of Polish mining companies is largely exacerbated by their high operating costs. The cost of obtaining coal and its price are two key elements that determine the level of efficiency of Polish mines. This situation could be improved by streamlining the planning processes. This would involve striving for production planning that is as predictable as possible and, on the other hand, economically efficient. In this respect, it is helpful to plan the production from operating longwalls with full awareness of the complexity of geological and mining conditions and the resulting economic consequences. The constraints on increasing the efficiency of the mining process are due to the technical potential of the mining process, organisational factors and, above all, geological and mining conditions. The main objective of the monograph is to identify relations between geological and mining parameters and the level of longwall mining costs, and their daily output. In view of the above, it was assumed that it was possible to present the relationship between the costs of longwall mining and the daily coal output from a longwall as a function of onerous geological and mining factors. The monograph presents two models of onerous geological and mining conditions, including natural hazards, deposit (seam) parameters, mining (technical) parameters and environmental factors. The models were used to calculate two onerousness indicators, Wue and WUt, which synthetically define the level of impact of onerous geological and mining conditions on the mining process in relation to: —— operating costs at longwall faces – indicator WUe, —— daily longwall mining output – indicator WUt. In the next research step, the analysis of direct relationships of selected geological and mining factors with longwall costs and the mining output level was conducted. For this purpose, two statistical models were built for the following dependent variables: unit operating cost (Model 1) and daily longwall mining output (Model 2). The models served two additional sub-objectives: interpretation of the influence of independent variables on dependent variables and point forecasting. The models were also used for forecasting purposes. Statistical models were built on the basis of historical production results of selected seven Polish mines. On the basis of variability of geological and mining conditions at 120 longwalls, the influence of individual parameters on longwall mining between 2010 and 2019 was determined. The identified relationships made it possible to formulate numerical forecast of unit production cost and daily longwall mining output in relation to the level of expected onerousness. The projection period was assumed to be 2020–2030. On this basis, an opinion was formulated on the forecast of the expected unit production costs and the output of the 259 longwalls planned to be mined at these mines. A procedure scheme was developed using the following methods: 1) Analytic Hierarchy Process (AHP) – mathematical multi-criteria decision-making method, 2) comparative multivariate analysis, 3) regression analysis, 4) Monte Carlo simulation. The utilitarian purpose of the monograph is to provide the research community with the concept of building models that can be used to solve real decision-making problems during longwall planning in hard coal mines. The layout of the monograph, consisting of an introduction, eight main sections and a conclusion, follows the objectives set out above. Section One presents the methodology used to assess the impact of onerous geological and mining conditions on the mining process. Multi-Criteria Decision Analysis (MCDA) is reviewed and basic definitions used in the following part of the paper are introduced. The section includes a description of AHP which was used in the presented analysis. Individual factors resulting from natural hazards, from the geological structure of the deposit (seam), from limitations caused by technical requirements, from the impact of mining on the environment, which affect the mining process, are described exhaustively in Section Two. Sections Three and Four present the construction of two hierarchical models of geological and mining conditions onerousness: the first in the context of extraction costs and the second in relation to daily longwall mining. The procedure for valuing the importance of their components by a group of experts (pairwise comparison of criteria and sub-criteria on the basis of Saaty’s 9-point comparison scale) is presented. The AHP method is very sensitive to even small changes in the value of the comparison matrix. In order to determine the stability of the valuation of both onerousness models, a sensitivity analysis was carried out, which is described in detail in Section Five. Section Six is devoted to the issue of constructing aggregate indices, WUe and WUt, which synthetically measure the impact of onerous geological and mining conditions on the mining process in individual longwalls and allow for a linear ordering of longwalls according to increasing levels of onerousness. Section Seven opens the research part of the work, which analyses the results of the developed models and indicators in individual mines. A detailed analysis is presented of the assessment of the impact of onerous mining conditions on mining costs in selected seams of the analysed mines, and in the case of the impact of onerous mining on daily longwall mining output, the variability of this process in individual fields (lots) of the mines is characterised. Section Eight presents the regression equations for the dependence of the costs and level of extraction on the aggregated onerousness indicators, WUe and WUt. The regression models f(KJC_N) and f(W) developed in this way are used to forecast the unit mining costs and daily output of the designed longwalls in the context of diversified geological and mining conditions. The use of regression models is of great practical importance. It makes it possible to approximate unit costs and daily output for newly designed longwall workings. The use of this knowledge may significantly improve the quality of planning processes and the effectiveness of the mining process.

Genitourinary cancers examines the malignancies arising in the kidney, ureter, bladder, prostate, testis, and penis. Renal cancer has high propensity for systemic spread, largely mediated by overexpression of vascular endothelial growth factor (VEGF). Treatments include surgery, immunotherapy, and targeted therapy. Wilms tumour, a childhood malignancy of the kidney, warrants specialist paediatric oncology management to provide expertise in its unique pathology, staging, and treatment, often with surgery and chemotherapy. Cancer of the bladder and ureters, another tobacco related cancer, may present as either superficial or invasive disease. The former is managed by transurethral resection and intravesical therapy. The latter may require radical surgery, preoperative chemotherapy, or radiotherapy. Prostate cancer, the commonest male cancer, is an androgen dependent malignancy. It has attracted controversy with regards to PSA screening, and potential over treatment with radical prostatectomy. Division into low, intermediate, and high risk disease according to tumour grade, stage, and PSA helps in deciding best treatment, antiandrogen therapy for metastatic disease, radiotherapy and adjuvant hormone therapy for locally advanced disease, either surgery or radiotherapy for early intermediate risk disease, and active monitoring for low risk cases. Testicular cancer divides according to pathology into seminoma, nonseminomatous germ cell tumours (NSGCT), and mixed tumours, the latter two frequently producing tumour markers, alpha-fetoprotein (AFP) and/or human chorionic gonadotrophin (HCG). Stage I disease is managed by inguinal orchidectomy and surveillance or adjuvant chemotherapy. More advanced disease is managed by chemotherapy, with high probability of cure in the majority. Penile cancer, often HPV related, can be excised when it presents early, but delay in presentation may lead to regional and systemic spread with poor prognosis.

The use of non-prescription drugs is widespread and has a major impact on the health of the individual user and society. In 2006, the British Crime Survey reported that 10% of adults had used one or more illicit drugs in the preceding year, with 3% reporting using a Class A drug. Over 11 million people in the UK are estimated to have used an illicit drug at least once in their lifetime (35%). Drugs abused vary in their intrinsic potential to cause addiction and, with it, more regular and harmful use. Drug users are influenced by trends and fashions, adopting new compounds such as crack cocaine and experimenting with routes of ingestion. Some drugs may become less popular over time, such as LSD, while others, such as cannabis, experience a revival as more potent strains (e.g. Skunk) are developed. A problem drug user is best defined as a person whose drug taking is no longer controlled or undertaken for recreational purposes and where drugs have become a more essential element of the individual’s life. The true economic and social cost of drug use is likely to be substantially greater than the published figures, which are derived from a variety of health and crime surveys which may overlook vulnerable groups such as the homeless. The majority of non-prescription drugs used in the UK are illegal and covered by the Misuse of Drugs Act 1971. The drugs most commonly abused gave rise in 2003–4 to an estimated financial cost in England and Wales of 15.4 billion pounds to the economy, with Class A drugs such as heroin and cocaine accounting for the majority of this. Some 90% of the cost is due to drug-related crime, with only 3% (£488 million) due to health service expenditure, which is mainly spent on inpatient care episodes. This still represents a major health pressure, which in 2006–7 amounted to 38 000 admissions, in England, for primary and secondary drug-related mental or behavioural problems, and over 10 000 admissions recorded for drug poisoning. Clinicians in all specialities can expect to encounter harmful drug use, especially those working in primary care, A & E, and psychiatric services. Presenting problems are protean, ranging from mood disorders, delirium, and psychosis to sepsis, malnutrition, and hepatitis. Blood-borne infections such as hepatitis C and HIV are widespread, as contaminated needles and syringes are shared by up to a quarter of problem drug users. Even smoking drugs such as crack cocaine can lead to increased transmission of hepatitis C through oral ulceration and contact with hot contaminated smoking pipes. Amongst the UK population, over half of IV drug users have hepatitis C, a quarter have antibodies to hepatitis B, and, by 2006, 4662 had been diagnosed with HIV. Non-prescription drug abuse is a leading cause of death and morbidity amongst the young adult population (those aged 16–35). In 2006 there were 1573 deaths where the underlying cause was poisoning, drug abuse, or dependence on substances controlled under the Misuse of Drugs Act. The vast majority (79%) were male. Young men, in particular, are at greater risk of violent death through associated criminal activity such as drug supplying and from deliberate and accidental overdose. The male-to-female ratio for deaths associated with mental and behavioural disorder is 6:1.
