Abstract:
This chapter provides an introduction to advance care planning (ACP) and an introductory overview of the book. It affirms the importance of ACP, the strong evidence base and experience in practice. It cites the current and evolving models of ACP in the UK, linked with the Mental Capacity Act, and the use of advance statements (AS), refusals of treatment (ADRT) and nominated spokesperson (LPOA). There is encouragement that most can initiate such discussions and suggested ways to do this. It describes the evolving ACP model balancing both medical (transactional) and personal (transformational) aspects of it, with a description of the different conversations and suggested five steps in ACP to raise public awareness. There is a reflection of the deeper significance of ACP including hope and expectations, concluding with a call for ACP to become a mainstream part of care for all people nearing the end of life.

This chapter describes the infrastructure and strategies that compose the ARC model. The first strategy embeds ARC’s five principles. The second strategy introduces component tools to establish key processes and mechanisms needed for service improvement. The third strategy introduces mental models to align assumptions, mindsets, and reasoning with the application of ARC principles and component tools in the improvement process. The authors describe the individuals and teams needed to implement ARC, along with their roles and responsibilities in the service improvement effort. This information includes the initial training of participants and additional features of ARC to build effective teams, as well as the use of planned materials and guides. Lastly, the chapter describes the mechanisms to ensure sustainability of the improvement effort.

Must causal models distinguish default from deviant events? Yes, say Menzies (2004, 2007), Hitchcock (2007), Hall (2007), and Halpern (2008), inter alia. No, argues this chapter. It argues that adding defaults into causal models (1) generates complicating and under-constrained unclarities, (2) fails to solve the problems it has been claimed to solve, and (3) fails to fit the most psychologically plausible accounts of how norms influence cognition generally. Instead of adding defaults into causal models, it recommends clarifying the background constraints on what counts as an apt causal model, and attending to background cognitive biases about the availability of alternatives.

Abstract:
This chapter describes empirical support for the ARC model from multiple randomized controlled trials. These trials describe the effects of ARC on organizational culture and climate, organizational priorities, clinicians’ work attitudes, clinicians’ evidence-based practice behaviors, and most importantly, client outcomes. Both the ARC model and the validity of the OSC measurement system are also supported by a number of non-experimental and quasi-experimental studies. Collectively, these studies describe linkages with OSC and related criteria using regional and nationwide samples that are fundamental to explaining the value of the ARC model. Much of this work is referenced throughout the book. In this chapter, the authors focus on the randomized controlled trials to support causal inferences about the effects of ARC, including impact on client outcomes, staff outcomes such as turnover and engagement, as well as impact on evidence-based practices. The evidence also highlights the implications for using ARC strategies in planned efforts to improve service effectiveness.

This chapter describes the ARC model of three core strategies for developing effective human service organizations. These include (1) embedding guiding organizational principles, (2) providing organizational component tools for identifying and addressing service barriers, and (3) developing shared mental models. ARC’s strategies provide the tools and the reasoning to guide behaviors and processes among organizational members that ensure improved service quality and outcomes. These strategies are reviewed as part of ARC’s orchestrated and structured process to improve OSC (i.e., the cultures and climates that influence attitudes, decision making and behavior in organizations). The chapter identifies mechanisms of change that highlight the alignment of organizational priorities with the ARC principles, fostering relationships that provide availability, responsiveness, and continuity, as well as developing innovation capacity to adopt new technologies and approaches.

Traditionally, attentional selection has been thought of as arising naturally from resource limitations, with a focus on what might be the most apt metaphor, e.g. whether it is a ‘bottleneck’ or ‘spotlight’. However, these simple metaphors cannot account for the specificity, flexibility, and heterogeneity of the way attentional selection manifests itself in different behavioural contexts. A recent body of theoretical work has taken a different approach, focusing on the computational needs of selective processing, relative to environmental constraints and behavioural goals. They typically adopt a normative computational framework, incorporating Bayes-optimal algorithms for information processing and action selection. This chapter reviews some of this recent modelling work, specifically in the context of attention for learning, covert spatial attention, and overt spatial attention.

Advance care planning (ACP) in New Zealand has grown as a people’s movement resourced by passionate individuals across the country. This people’s movement is a whole of systems approach led by the national ACP Cooperative. The approach has provided a permissive platform for the national evolution of ACP and has allowed ACP to quickly gain momentum. It has facilitated the collaboration of multiple interest groups with consumers at the centre and has provided an environment for innovation. The ACP Cooperative used a deployment model to drive the work. The model insured that the movement focused on engaging healthcare leadership and the community, educating clinicians, and the public, whilst keeping the patient and their family/whānau values at the centre of the process. The Cooperative is acutely aware that providing patient value-based care for people as they approach the end of their lives sets the precedent for how all healthcare could be delivered.

When under time or procedure pressure, people change their decision-making strategies. They may accelerate information processing and filter the information they will process. In this chapter, the author presents several models for decision making under pressure and compares them to more traditional models. The naturalistic decision-making models are proposed as more appropriate for decision making when working with high-risk patients under emergency conditions. Given that it is often stressful for clinicians to evaluate and manage patients or clients who are considered at acute risk to themselves or others, the author presents a model for training to reduce stress that is based on Meichenbaum’s stress inoculation training. The chapter concludes with a discussion of the training for skill development and competence in dealing with behavioral emergencies that is consistent with the recommendations of the APA Task Force on the Assessment of Competence in Professional Psychology.
