Abstract:
Bakgrund: Anorexia Nervosa är en psykiatrisk sjukdom som främst drabbar unga kvinnor mellan 15 och 23 år med hög mortalitet. Sjukdomen orsakar ätstörning vilket medför kraftig viktnedgång. Syftet: Syftet är att beskriva kvinnors upplevelser av att leva med Anorexia Nervosa. Metod: Denna studie är genomförd av kvalitativ metod med analys av narratives. Datainsamlingen är baserad på fyra självbiografier skrivna av kvinnor som drabbats av Anorexia Nervosa. Resultat: Resultatet visade att samtliga kvinnor har haft olika upplevelser av Anorexia Nervosa. Kvinnornas upplevelser delas in i tre olika teman vilket förtydligades med citat. Teman som beskrivs avspelar kvinnors olika upplevelser. De teman som uppkom var Det svåruppnåeliga kvinnoidealet, Den ångestfyllda tillvaron, Betydelsen av att ha kontroll. Slutsats: Anorexia är en komplex psykiatrisk sjukdom som kräver en optimal omvårdnad med beredskap och förståelse från vårdpersonal. Sjuksköterskor strävar efter att förhindra ett lidande och främja hälsa i vårdandet av patienter. Studiens resultat bidrog med en bredare kunskap och förståelse kring sjukdomen Anorexia Nervosa för en optimal återhämtning hos patient.
Background: Anorexia nervosa is a psychiatric disorder that primarily affects young women between 15 and 23 years with high mortality. The disease causes eating disorder resulting in significant weight loss. Purpose: Purpose of the study was to describe women's experiences of living with anorexia nervosa. Method: This study was conducted by qualitative method of analysis of narratives. Data collection was based on four autobiographies written by women who have lived with Anorexia Nervosa. Results: The results showed that all women have had different experiences of Anorexia Nervosa. This experiences were divided into three different themes which are clarified with quote. Themes described reflects women's different experiences with anorexia nervosa, which is the elusive ideal of women, the anguished existence and the importance of being in control. Conclusion: Anorexia is a complex psychiatric illness that requires an optimum care of preparedness and understanding of health professionals. Nurses strive to prevent suffering and promote health in the care of patients. The study's results contributed to a wider knowledge and understanding of the disease anorexia nervosa for optimal recovery of the patient.

Abstract:
This thesis attempts a sociological and feminist analysis of anorexia nervosa. Anorexia is an illness which affects predominantly women, and its incidence is greatest among middle-class young women in Western countries. Its strong bias along class and gender lines suggests that such an approach to the illness could prove fruitful. The thesis argues that analysis of anorexia demands a clear understanding of the sociology of the body. The sociology of the body sees the body as constructed in social life: understandings of the body vary temporally and culturally, and reflect the categories of their culture. It is suggested that anorexia nervosa represents an attempted transformation of the concept of the feminine body in contemporary culture. Anorexic women aim to transcend appetite, and to allow no intrusions into the body, constructing an anorexic body which is closed, separate and inviolable. Since this transformation is individuated and privatised, however, it cannot ultimately succeed in overturning a system of social meanings. The thesis concludes that individual solutions to anorexia will not lead to the end of the illness as a social phenomenon in the lives of women. Only collective feminist action can reconstruct the degraded contemporary concept of woman. The argument is pursued firstly through a discussion of the initial use of the term `anorexia nervosa' in the late nineteenth century by Gull and Lasegue. The treatment of anorexia as a modern disease is discussed, and the claim that anorexia has always existed but has not been recognised is refuted. Psychiatric and feminist accounts of anorexia are then considered. The former see anorexia as a purely individual phenomenon, and the limitations of this position are discussed. Feminist analyses of anorexia, in seeing it as deeply intertwined with women's social position in a patriarchal culture, are argued to advance understanding of the illness, while still retaining individualist elements. The next section analyses the ways in which anorexic women themselves explain their illness. This leads on to a discussion of the notion of the body as concept. After a theoretical outline, several body-concepts are analysed and placed in their social and historical contexts. Contemporary understandings of the body as an individuated possession are then discussed, with particular focus on the concept of the feminine body as passive object. Objectification, discipline and chaos are argued to be the central meanings of the feminine body in contemporary culture. Analysis of the issues of abortion and rape seek to make this theoretical point clearer. A detailed analysis of anorexic practices looks at how these meanings are transformed in anorexia. It is suggested that anorexic women try to construct an inviolate anorexic body which is completely under their control through a complexly ritualised eating pattern. The precarious nature of this control points to the limitations of individual `solutions' to social problems.

Abstract:
This thesis aimed to examine the functioning of families with a sufferer of Anorexia Nervosa (AN), using self-report measures and a direct observation family discussion task. Researchers and clinicians have long been interested in the interactional patterns of these sufferers within the family unit, although few have furthered our understanding of the interplay between family functioning, cohesion and emotional expression and issues such as control, by directly examining these interactions. The construct of control has been another area of interest in recent research, although how it is constructed and operates within this disorder remains unclear and ambiguous. Thus the purpose of this research was to expand the study of adolescents suffering Anorexia Nervosa who still resided within the family unit, from merely studying self-report measures and retrospective surveys of recovery, to include a direct examination of sufferers and their families at the time of the disorder. Participants included 16 female sufferers of Anorexia Nervosa and their parents, 17 drug and alcohol sufferers, and 18 non-clinic families. Both parents and their daughters completed self-report inventories assessing their clinical status as well as family functioning and marital happiness. In addition, family members participated in a series of family discussion tasks which were coded for the dimensions of autonomy, cohesiveness, avoidance and control. In Study 1 (Chapter 7), the self-report measures of the anorexic families were examined specifically in relation to the construct of control acting as a mediating variable between level of dysfunction and severity of the disorder. Consistent with previous research, levels of marital happiness and family cohesion influenced the severity level of the daughter's Anorexia Nervosa. This study also explored the construct of control within the family system and found that the more moral-religious emphasis in the family, the more concerned the daughter was with her weight. At the same time daughters were reporting higher levels of moral-religious emphasis, they were reporting higher levels of self-control. The daughters in this study appeared to be inconsistent in their behavioural responses. The daughters' weight concern increased with their own reported levels of moral-religious emphasis in the family. However, as this emphasis increased, so too did their control scores. It was supposed that conflict may be created in a family when strong religious and/or strong moral values are emphasized, particularly when one of the family members suffers AN. This issue is discussed in depth. Study 2 sought to examine these variables further by using a direct observation family discussion task to compare an independent observer's ratings to the family's ratings of the discussion, across the three groups. The study examined the daughters' and mothers' perceptions and compared significant results to the observer's ratings. This study highlighted that the mothers of sufferers appeared not to be concerned about their own bodies and weight, and not distressed when discussing with their daughters the issues of control in the areas of family, body, school and friends. Furthermore, the anorexic daughters presented as more sad and anxious than the two other groups and indicated that their parents had more control over their bodies than they did themselves. Finally, for the majority of participating families, the fathers were absent and this issue is explored.

Abstract:
This thesis analyses clinical knowledge surrounding anorexia. What clinicians say and write about anorexia constitutes a key area of clinical knowledge. This knowledge is complex and has many dimensions. It identifies typical "manifestations" of the illness, such as "fear of fatness". It also identifies the type of woman (e.g. her age or social class) likely to suffer from the disorder. Most importantly, clinical knowledge suggests that anorexia has specific "causes". Anorexia, clinicians inform us, represents a flight from adulthood on the part of girls who are intensely afraid of becoming fat. This thesis analyses this complex psychiatric model, using a number of different methods. The study argues that psychiatry constructs a stereotype of the anorexic. This stereotype is visible in two areas. Firstly an archetype of the sufferer's cognitive symptoms is visible. This suggests that anorexics fear fatness and are afraid to grow up. Secondly a stereotype of the anorexic's demographic profile is apparent. This constitutes the sufferer as young, middle class, educated, and urban. The study examines the accuracy of this stereotype among a sample of 104 anorexics. Using open and closed ended-data the study suggests that the "anorexic stereotype" may have limited empirical support. In the current study, patients suggested that anorexia was a strategy of control. The concept of control, while validly describing patients' experiences may partly have been acquired by patients from clinicians. The thesis appraises patients' testimonies to examine the extent to which patients have acquired such "medicalised" language. It also analyses 24 doctor-patient interviews and concludes that doctors may encourage patients to view their disorder in clinical terms. Using feminist theory, and theory from the sociology of chronic illness, the findings of the thesis are analysed from a sociological perspective.

The objective of the present research was to analyze a pro-anorexia blog indentifying the control relationships between the blog manager and the users of it. In order to do so, at first all posted messages by the manager and the commentaries by the readers were categorized and later the relationships between the verbal responses were analyzed. For each theme introduced by the manager s messages and by the blog users commentaries it was verified whether there was agreement/repetition or disagreement by the other readers. Forty four themes in seven messages and 605 commentaries were identified. The majority of the themes could be classified in pro-anorexia and anti-anorexia themes. The pro-anorexia theme more frequently observed on the manager s messages was the reported negative feelings related to being fat . However the most commented by the readers was the fact that anorexia is a sickness which causes pain and death. The manager s messages accompanied by photos were followed by the major number of commentaries. Among the most used persuasion resources by the manager are the artifices of strength as caps lock, italics and bold. The majority (77, 4%) of the readers identified themselves with female nicknames. One may come to the conclusion that mutual verbal control occurred, once the readers questioned the manager, asked more information and extra blog contact and the manager replied directly some readers
O objetivo da presente pesquisa foi analisar um blog pró-anorexia identificando as relações de controle entre a gerenciadora do mesmo e seus leitores. Para isso foram categorizadas todas as mensagens postadas pela gerenciadora e os comentários dos leitores, o que permitiu uma análise das relações entre essas respostas verbais. Para cada tema introduzido nas mensagens da gerenciadora e nos comentários dos leitores do blog, verificou-se se houve concordância/repetição por outros leitores ou discordância. Foram identificados 44 temas em sete mensagens e em 605 comentários. A maioria dos temas pode ser classificada em temas pró-anorexia e anti-anorexia. O tema pró-anorexia mais freqüentemente observado nas mensagens da gerenciadora foi o relato de sentimentos negativos relacionados a estar gorda . Contudo o tema mais comentado pelos leitores foi o fato de que a anorexia é uma doença que causa sofrimento e morte. As mensagens da gerenciadora que foram acompanhadas de fotos receberam o maior número de comentários. Entre os recursos de persuasão mais utilizados pela gerenciadora estão os artifícios de força como o uso de caixa alta, itálico e negrito. A maioria (77,4%) dos leitores identificou-se com apelidos femininos. Pode-se concluir que ocorreu controle verbal mútuo, uma vez que os leitores questionaram a gerenciadora, pediram informações e contato extra-blog e a gerenciadora respondeu diretamente a alguns leitores

Esta dissertação propõe investigar o sintoma anoréxico na estrutura neurótica como um sintoma contemporâneo em consequência da baixa operatividade da função paterna e da incidência avassaladora do Desejo Materno. A pesquisa foi iniciada por um levantamento histórico-bibliográfico do jejum e da recusa alimentar ao longo da história, até o estabelecimento da anorexia como uma psicopatologia. Para dar sustentação ao eixo central desta Dissertação, a pesquisa explora o conceito de pai na Obra de Freud e no ensino de Lacan, investigando a baixa operatividade desta função e seus efeitos na formação do sintoma. O conceito de pulsão é explorado com o objetivo de compreender a anorexia como uma invasão pulsional no corpo decorrente da precária sustentação da armadura simbólica que a lei paterna insere. Deste modo, esta Dissertação expõe duas hipóteses centrais: a anorexia como uma manobra de separação, operação que a função paterna não inscreve devidamente e desta forma um recurso diante da hegemonia materna; e a anorexia como estragos que apontam a invasão da mortífera pulsão no corpo, decorrentes da presença avassaladora do Outro materno e da inconsistência da função paterna
The purpose of this thesis is to investigate the anorexic symptom in the neurotic structure as a contemporary symptom resulting from the low operatibility of the paternal role and the overwhelming incidence of the Maternal Desire. The research was initiated by a historical-bibliographical survey of fasting and food refusing throughout history, until the establishment of anorexia as a psychopathology. Giving support to the control axis of this thesis, the research explores the father concept in the work of Freud and Lacans teaching, investigating the low operability of this function and its effects on symptom formation. The drive concept is explored in order to understand anorexia as an encroachment drive in the body due to the precarious support of the symbolic armor that the paternal law inserts. Thus, this thesis exposes two central hypotheses: anorexia as a separation ploy, an operation that the paternal function does not fulfil properly and, therefore, is not an appeal in the face of maternal hegemony, and anorexia as damages that point the invasion of deadly drive in the body, arising from the overwhelming presence of the Maternal Other and of the inconsistency of the paternal function

Abstract:
Anorexia Nervosa (AN) is a psychiatric disorder characterised by persistent self-starvation, which has high mortality rates, and low rates of recovery. In conjunction with a lack of effective treatments, the serious nature of the disorder emphasises the need for research investigating the underlying processes contributing the persistence of the disorder, and the identification of novel avenues for treatment development. In individuals with AN, dietary restriction often takes on a driven and compulsive quality, with increasing suggestion of parallels with other disorders featuring compulsive behaviour, such as Obsessive-Compulsive Disorder (OCD) and substance dependence. The commonalities and comorbidities in compulsive behaviours across a range of disorders have been highlighted by Robbins et al (2012), who propose a transdiagnostic approach to compulsivity, arguing that this behavioural construct has cross-diagnostic significance. Conceptualising compulsivity in this way may aid in the development of novel research avenues investigating the learning processes and neurobiological mechanisms underpinning this behaviour. In light of this, the aim of this thesis was to investigate the compulsive nature of behaviour in AN, and the behavioural and neural underpinnings of this behaviour guided by research carried out in other compulsive disorders. Chapter 1 reviews models of the neural and behavioural underpinnings of compulsive behaviour, and discusses these in relation to compulsive behaviour in AN. Chapter 2 presents qualitative data supporting parallels between the subjective experience of compulsive behaviour in AN and substance use disorders (SUDs). These parallels are further examined in Chapter 3, in which two novel measures of compulsive weight-loss behaviour were developed and validated by adapting a questionnaire assessing addictive behaviour. Chapter 4 presents data suggesting that these parallels in behaviour do not reflect a common tendency to develop habits in learning, as found in substance dependence and OCD. The electrophysiological data presented in Chapter 5 was unable to provide evidence that AN may be underpinned by dysfunction of the neurocircuitry seen in other compulsive disorders. Whilst these studies support suggestion of parallels with substance dependence, at least at the level of self-report, the hypothesised parallels in the underlying behavioural and neural mechanisms were not found.

Abstract:
This paper aims to discuss the Lacanian clinic of anorexia. Therefore, a qualitative methodology that included a literature review and a field survey was used. We consulted the main authors who study anorexia and refer to the Lacanian theory. Nine professionals working in this area were interviewed. Throughout the work, we sought to interconnect field/literature. The work was divided into four chapters. The first one discusses the arrival of the anorexic patient to the clinic. In general, Anorexic girls do not seek psychoanalysts by themselves. Most of the times, they are led to the clinic by their parents, more specifically, by their mothers. There is no clear demand. Demand usually comes from a person seeking knowledge about himself. Such fact does not occur in anorexia since food refusal is not seen as a problem. What don´t these girls demand? What do they not want to know? What, in fact, are they refusing? Eating nothing in anorexia means anesthetizing the suffocating mother who suffocates through porridge the structural distortion of body image, sex and femininity in all its implications. Second and third chapters discusses the interruption of this demand circle, almost inexistent for these people. The deadly anorexic circle only finds limit when life is at risk. At this time also, it is not possible to talk of demand in the psychoanalytic sense. There is, rather, a call for help. The psychoanalyst can not neglect such request. It is necessary, through transfer, to twist this call in order to imply the patient in her symptom. This subjective rectification is only possible because it is grounded in the following clinical assumptions: the construction of the clinical case, the interpretation of the refusal, the desire of the analyst and the analytic act. Having said that, we studied two experiences of psychoanalytic treatment in severe cases in Italy. The fourth chapter discusses anorexia contemporary Lacanian clinic. We propose a different view of the anorexia understanding and treatment. The view is not about the interpretation of the symptom any longer. Anorexia is seen as a possible link to be built from the real body image. This is where the symptom does not dive into the interpretation. There is a permanence of the symptom. The analyst's role is to bear all that in order to prevent it from overflowing and endangering individuals lives. The interviews transcripts were enclosed in the annexes.
Este trabalho tem como objetivo discutir a clínica lacaniana da anorexia. Para tanto, foi utilizada como parâmetro de pesquisa a metodologia qualitativa, que contou com uma pesquisa bibliográfica e com uma pesquisa de campo. Foram consultados os principais autores que discutem a anorexia e que têm como referência a teoria lacaniana. Foram entrevistados nove profissionais que atuam nessa clínica. Ao longo do trabalho, buscou-se estabelecer uma interconexão campo/literatura. O trabalho foi dividido em quatro capítulos. O primeiro discute a chegada da anoréxica na clínica. Essas meninas, em geral, não procuram os psicanalistas por vontade própria. Quase sempre, elas chegam a um hospital carregadas pelos pais, mais especificamente, pela mãe. Não há uma demanda clara. A demanda supõe um sujeito que busca um saber sobre si, o que não ocorre na anorexia, uma vez que a recusa não é colocada como um problema. O que essas meninas não demandam? Do que elas não querem saber? O que, na verdade, elas estão recusando? Comer nada, na anorexia, significa anestesiar a mãe que sufoca com a papinha, a distorção estrutural da imagem corporal, do sexo e da feminilidade com todas as suas implicações. O segundo e terceiro capítulos discutem como se dá rompimento deste circuito de demanda, quase nula, nesses sujeitos. O circuito mortífero anoréxico só encontra um limite quando a vida é colocada em risco. Neste momento, também, ainda não é possível falar de uma demanda no sentido psicanalítico. Há, sim, um pedido de socorro. O psicanalista não pode ficar avesso a esse pedido. Faz-se necessário, via transferência, provocar uma torção nesse pedido de ajuda, implicando a anoréxica em seu sintoma. Essa retificação subjetiva só é possível porque está embasada nos seguintes pressupostos clínicos: a construção do caso clínico, a interpretação da recusa, o desejo do psicanalista e o ato analítico. Posto isto, foram discutidas duas experiências de tratamento psicanalítico com casos graves na Itália. O quarto capítulo, por sua vez, discute a clínica lacaniana contemporânea da anorexia. Propõe-se um outro olhar sobre a clínica da anorexia. O olhar não é mais sobre a interpretação do sintoma - a anorexia é vista como um enlace possível de ser feito frente ao real do corpo. Trata-se de casos em que o sintoma não cede à interpretação. Há uma permanência do sintoma. O papel do analista é o de suportá-lo a fim de evitar que ele transborde e coloque a vida desses sujeitos em risco. No apêndice final, foi disponibilizada a transcrição das entrevistas.

Bakgrund: Eftersom det blivit allt vanligare med ätstörningar som anorexia nervosa finns det en stor möjlighet att vårdpersonal träffar på dessa patienter inom vården oavsett vårdområde. För att som sjuksköterska kunna bemötta dem på bästa sätt och erbjuda en god vård, krävs det kunskap kring upplevelser av att leva med denna sjukdom.

Syfte: Syftet med litteratur studien var att undersöka upplevelsen av anorexia nervosa.

Metod: Metoden som användes var en litteraturstudie med en kvalitativ analys. Fem självbiografier utgjorde studiens datamaterial och de analyserades med en kvalitativ innehållsanalys.

Resultat: I analysen framkom två huvudkategorier som ligger till grund för resultatet. Dessa var upplevelsen av lidande och upplevelsen av trygghet. Resultatet visade att individen med anorexia nervosa går igenom ett lidande som genomtränger hela deras liv. Lidande kunde yttra sig på olika sätt och i samband med olika upplevelser. Framförallt upplevdes lidandet ihop med maktlöshet över sin situation och tvångsåtgärder som de utsattes för. Och ett kontrollbehov som skapade en negativ trygghet och ett lidande då individen upplevde sig fängslad i rutiner och regler. Det fanns även ljus i mörkret där människan kunde uppleva en trygghet i sin livsvärld med hjälp utav familj och vänner.

Slutsats: Människor med anorexi går ständigt igenom ett lidande i kampen emot sjukdomen och en inre röst som begränsar dem i deras livsvärld. Individer med anorexisjukdom har ett stort kontrollbehov och lever i en maktlöshet som genomsyrar deras livsvärld. Relationen till familj och vänner var mycket betydelsefullt för kvinnornas livsvilja. Det var familjens och vännernas kärlek som fick kvinnorna börja känna tro och hoppas att någon dag bli frisk.

Det är av stor vikt att sjukdomen upptäcks i tid så att behandling snabbt kan sättas in. Forskning har visat att sjuksköterskor upplevde bristande kunskaper inom psykisk ohälsa och hade därmed svårigheter att vårda patienter med anorexia. Syfte: Syftet var att beskriva familjens erfarenheter av att leva med en familjemedlem med anorexia. Metod: Litteraturstudien baserades på tio vetenskapliga artiklar, fem med kvantitativ ansats och fem med kvalitativ ansats. En integrerad analys utfördes utifrån Kristenssons (2014) beskrivning. Resultat: De kategorier som framkom i analysen var Familjedysfunktion, Psykisk belastning, Familjens hanteringsstrategier, Familjens erfarenheter av bemötande från vårdpersonal och Erfarenheter av att vårda. Familjen upplevde att anorexia påverkade deras psykiska hälsa negativt och de oroade sig för den sjuka familjemedlemmen. De upplevde att de fått otillräcklig information och stöd från sjukvården. Sjukdomen hade trots detta även givit dem värdefulla erfarenheter. Slutsats: Familjer upplevde både psykisk och praktisk belastning av att leva med en familjemedlem med anorexia och de använde olika strategier för att hantera detta. De uttryckte ett behov av hjälp från sjukvården. Sjuksköterskans roll är att se hela familjen och inte enbart den enskilda individen vilket kan möjliggöra att lidande lindras och välbefinnande främjas.

I dagens samhälle pågår en "hälsohets" för många kvinnor. Att leva upp till samhällets normer och vad som anses vara det normala idealet kan i vissa fall skapa enormt mycket stress och strävan efter det perfekta utseendet blir för stor. Denna strävan bidrar till att tillfrisknandeprocessen från Anorexia Nervosa (AN) blir mer komplex hos kvinnor. Syftet med studien var att undersöka tillfrisknande hos kvinnor med AN. Studien är utförd som en allmän litteraturstudie med vetenskapliga artiklar som grund. Resultat påvisar att stöd är en betydelsefull omvårdnadsåtgärd sjuksköterskan kan ge i denna process. För kvinnornas del är det av största vikt att beslutet om tillfrisknande är självvalt och inte för att tillfredsställa någon annan. Det finns ett samband mellan kropp och själ, vilket är en utmaning för sjuksköterskan i omvårdnaden kring kvinnor med AN och att viktuppgång inte är korrelerat till ett fullt tillfrisknande belyses tydligt. För sjuksköterskan krävs fördjupade kunskaper om kvinnornas upplevelser kring tillfrisknandet för att få en helhet om sjukdomen. Det krävs ytterligare forskning om kvinnornas upplevelser efter avslutad behandling och efter tillfrisknande från sjukdomen. Även ytterligare komplimenterande forskning behövs kring kvinnornas syn på huruvida ett tillfrisknande är möjligt.
In today’s society many women have to endure health hazard. To live up to society's norms and what is seen as the ideal appearance can put a lot of pressure as well as force the women to strive for this ideal. This constant endeavor can make the recovery from Anorexia Nervosa more complex. The purpose of the literature study was to examine women's recovery from AN. The study is performed as a general literature study with scientific articles as a base. Results have proven that support is the most important caretaking method the nurse can provide throughout this process. For the women’s part, it’s of highest importance that the decision of recovery is self-decided. That consideration is taken to both body and soul is a challenge in the caretaking for women with AN and the fact that gaining weight is not related with full recovery is distinctly highlighted. It’s required for the nurse to possess in depth knowledge of women’s experiences surrounding the recovery to gain an entirety. Additional research of the experiences after finished treatment and after recovery from the disease is required. Even further complementary research surrounding women’s view on a recovery may be required.

Aspects of the social networks of anorexic (N=34) and non-anorexic (N=35) women were examined according to hypotheses derived from social network theory and research and from the literature pertaining to anorexia nervosa. The nature of the social network was discussed from the perspective of Pattison's (1977a) psychodynamic psychosocial systems theory. Subjects were compared on selected social network variables using the Pattison Psychosocial Inventory (PPI). The California Psychological Inventory (CPI) and the Family Environment Scale (FES) were used to investigate the contribution of certain personality and environmental variables to social network variation. Statistical analyses of the difference between means were tested using the Hotelling's T² procedure followed by univariate t-tests. Analyses of proportions were performed using z-tests. The Bonferroni inequality was employed in order to reduce the probability of Type I error when determining the statistical significance of the univariate t-tests and z-tests. The null hypothesis was accepted for the majority of the results. Only one social network variable, total network size, significantly differentiated anorexic and control subjects, although several other variables were approaching statistical significance. Various contrasting explanations of the results were discussed. For example, it is possible that anorexia nervosa is not a homogeneous or singular nosological entity and does not inevitably result in predictable and largely invariant social impairment. It was proposed that recent typologies of anorexia nervosa may yield significant between-group variation in social network variables. Furthermore, social networks may vary with the degree of severity and/or chronicity of the anorexic condition. The correlational analyses produced several statistically significant results. Regarding environmental (FES) variables, both cohesion and independence were positively correlated with support from family network members. Contrary to hypotheses, however, interpersonal effectiveness (CPI) achieved only weak and non-significant correlations with social network size and support.

A anorexia tem-se despontado como um dos grandes sintomas da atualidade. Especialmente entre jovens, a recusa radical da alimentação, associada a uma busca por um ideal de corpo esbelto, pode levar a situações extremas de emagrecimento, chegando, em alguns casos, a causar a própria morte. O aumento da incidência de anorexia fez com que as ciências da saúde e os serviços públicos a tomassem como uma de suas grandes preocupações. Diversas pesquisas surgiram, tentando compreender melhor a anorexia e aprimorar as técnicas de intervenção. Da mesma forma, os serviços públicos começaram a tratá-la com maior prioridade, em alguns casos, até criando ambulatórios especializados. Esta pesquisa visa a analisar os sentidos da anorexia produzidos, principalmente, pela literatura científica sobre o assunto e expressos em algumas narrativas de pacientes com esse diagnóstico. Para tanto, fizemos um levantamento do conceito e histórico da anorexia, detendo-nos, primeiramente, em Freud e psicanalistas que abordam diretamente o assunto, considerando ainda a descrição psiquiátrica dessa problemática. Posteriormente, buscamos autores da atualidade que tratam do problema, não apenas da ótica estrita da Psicanálise, mas também daqueles que analisam a anorexia como sintoma do nosso tempo. Ao lado das leituras, realizamos um trabalho de campo, mediante entrevistas com anoréxicas atendidas em um Programa de Transtornos Alimentares ligado a uma Faculdade de Medicina. Ao final, dividimos o material em temas, os quais possibilitaram uma aproximação a essa problemática, tanto do ponto de vista teórico, quanto por meio dessas experiências empíricas. Anorexia has become one of the great symptoms of the present time. Especially among the youth, the radical refusal to eat, associated with the search for an ideal slim body, which may lead to extreme situations of weight loss, in some cases, resulting in death. The increased rate of anorexia has prompted the health sciences and public services to make it one their great concerns. Multiple research works have been conducted to better understand anorexia and improve the intervention techniques. In the same way, the public services began to treat it with greater priority, in some cases, to the extent of creating specialized outpatient facilities. This research has the purpose of analyzing the significance of anorexia, mainly produced by scientific literature on the subject and expressed in some narrations of patients with this diagnostic. Therefore, we made a survey of the concept and historical study of anorexia, beginning with Freud and psychoanalysts who directly address the subject, also considering the psychiatric description of this issue. Subsequently, we searched current authors on the problem, not only from the strict view of Psychoanalysis but also those who analyze anorexia as a symptom of our time. Simultaneously, we executed a field work through interviews with anorexics treated in a Program of Eating Disorders connected to a Faculty of Medicine. Lastly, we divided the material into topics, which made possible a close approach this problematic issue, from the theoretical point of view as well as these empirical experiments. We observed that the point of view of the researched authors is different, especially regarding the treatment of anorexia. Adolescence is the most common period for the onset of anorexia nervosa. A growing body of quantitative evidence exists for treatment efficacy for adolescents. However, it is important, given the large dropout rates and high relapse rates that qualitative research attempts to understand how treatment and recovery is experienced by adolescents. Similarly, an appreciation for mothers' experience of treatment is vital given the evidence for high carer burden in this population and that families' involvement in treatment for anorexia nervosa is crucial. The first paper provides a review of the qualitative evidence looking at adolescents' experience of treatment and recovery for anorexia nervosa. The review made tentative yet important conclusions regarding crucial aspects of treatment and recovery for adolescents including the role of peers, families and professionals, physical and psychological nature of treatment and the adolescents' own concept of recovery. Methodological limitations of the studies and subsequent recommendations for future research were discussed in addition to clinical implications for future service delivery. The second paper adopts an Interpretative Phenomenological Analysis (IPA) methodology in exploring mothers' experiences of home treatment for their adolescent daughters with anorexia nervosa. Nine mothers who had experience of home treatment for at least two weeks participated in the study. IPA revealed two key themes which provided an important context to home treatment: 'becoming enmeshed' and 'reaching rock bottom.' These themes described the mother-daughter relationships and maternal stress and distress prior to home treatment. The third theme 'experience of help' described participants' experience of when help is needed, the need for containment, gaining strength and skills and finally how home treatment fitted into the families own philosophy and values. The results provide important rich accounts on how home treatment is experienced and outline both the helpful and unhelpful aspects. Further research is required to assess its effectiveness.

Este trabalho consiste em um estudo sobre a anorexia nervosa sob a ótica psicanalítica. Apresentaremos primeiramente um histórico do conceito, enfocando a trajetória desta patologia, que foi inicialmente rara e atualmente é encontrada na clínica com freqüência. Discutimos as contribuições trazidas pela psicanálise ao estudo e à clínica da anorexia nervosa através das diversas tentativas de explicá-la nessa perspectiva, embora este não seja um conceito psicanalítico. Abordamos ainda a relação entre a anorexia nervosa e a magreza como ideal de beleza contemporâneo sobretudo a ênfase que é atribuída ao corpo magro pela sociedade contemporânea e suas conseqüências psíquicas.
This work consists of a study about anorexia nervosa from a psychoanalytical perspective. Firstly, we will present a historical view of the concept, focusing the trajectory of such pathology, which was originally rare and has become frequent in current clinical practice. We discuss the contributions brought by psychoanalysis to the study and clinical management of anorexia nervosa throughout its various attempts to explain it, even though anorexia nervosa is not a psychoanalytical concept. We also approach the relationship between anorexia nervosa and thinness as a contemporary beauty ideal, especially the emphasis assigned to the thin body in actual society and its consequences.

This is a cross-sectional study whose objectives are to investigate the occurrence of possible eating disorders, anorexia and/or bulimia in adolescents of both sexes, and to identify the epidemiologic profile of both. To identify eating disorders, 500 adolescents from a private school in São Luís, Maranhão, Brazil, completed self-report questionnaires, the BITE (Bulimic Investigatory Test Edinburgh) and the EAT-26 (Eating Attitudes Test summarized). The findings indicate that 70 (14%) adolescents possibly present with diagnoses of anorexia, 4 (0,8%) of bulimia, and 11(2,2%) with anorexia and bulimia combined. It was observed that 78 (15,6%) adolescents presented with non-usual eating habits, placing them at risk for the development of eating disorders. This investigation shows that there is a high occurrence of possible eating disorders in the sample studied and a heterogeneous epidemiologic profile, occurring in both sexes and within diverse ethnic groups and social-economic classes. The dissatisfaction with their own bodies and the wish to modify them are feelings present in adolescents who probably exhibit eating disorders (p<0.001).
Este é um estudo transversal que tem como objetivos investigar a ocorrência de possíveis transtornos alimentares, anorexia e/ou bulimia, em adolescentes de ambos os sexos e identificar o perfil epidemiológico dos mesmos. Para o rastreamento dos transtornos alimentares foram aplicados os questionários autopreenchíveis BITE (Teste de Investigação Bulímica de Edinburg) e EAT-26 (Teste de Atitudes Alimentares resumido) em 500 adolescentes de uma escola particular na cidade de São Luís, Maranhão, Brasil. Os achados mostraram que 70(14%) adolescentes possivelmente apresentam o diagnóstico de anorexia, 4(0,8%), bulimia e 11(2,2%), anorexia combinada com bulimia. Verificou-se que 78(15,6%) adolescentes apresentam uma conduta alimentar não usual, comportamento de risco para o desenvolvimento de transtornos alimentares. Este trabalho mostra uma alta ocorrência de possíveis transtornos alimentares na amostra estudada e um perfil epidemiológico heterogêneo, ocorrendo, em ambos os sexos, em diversas etnias e camadas sócio-econômicas. A insatisfação com o próprio corpo e o desejo de modificá-lo são sentimentos presentes nos adolescentes que provavelmente têm transtorno alimentar (p<0,001).

Introdução: A procura de um ideal de beleza e a obsessão pela estética imposta pela sociedade moderna em que se vive têm promovido o aumento do número de pessoas com transtornos alimentares, como por exemplo, a anorexia nervosa e a bulimia nervosa. Esses transtornos contribuem para o aumento de pacientes com prejuízos, diretos e indiretos, na saúde oral. Objetivos: Esta tese tem como objetivo a revisão da literatura sobre “anorexia” e “bulimia” nas crianças e adolescentes, correlacionando estas patologias com complicações na cavidade oral, nomeadamente, a erosão dentária, a xerostomia, as cáries, a hipertrofia das glândulas salivares, as restaurações proeminentes, o bruxismo, entre outras. Metodologia: Foi realizada uma revisão bibliográfica com base em artigos publicados em revistas, nos últimos dez anos, disponíveis em bases de dados on line. Conclusão: Preconiza-se que haja uma maior atenção e conhecimento por parte do médico dentista sobre estes transtornos alimentares, bem como das complicações associadas, uma vez que é um dos primeiros profissionais de saúde a ter o contacto e a oportunidade de detetar a doença. Com o diagnóstico precoce, pode-se evitar o desenvolvimento da doença.
Introduction: The search for an ideal of beauty and obsession with aesthetics imposed by modern society in which we live have promoted the increase in the number of people with eating disorders , such as anorexia nervosa and bulimia nervosa. These disorders contribute to the increase of patients with damage, direct and indirect, in oral health. Objective: This study aims to review the literature on "anorexia" and "bulimia" in children and adolescents, correlating these conditions complications in the oral cavity, for example, tooth erosion, dry mouth, cavities, hypertrophy of the salivary glands, prominent restorations and bruxism. Methodology: This literature review from the last ten years was on the basic of articles published in journals available on line database. The keywords and conjugation used was: child and adolescents; anorexia; bulemia; eating disorders and health. Conclusion: It is envisioned to be greater attention and knowlwdge of the dentist these eating disorder, as well as the associated complications, a time that is hum of first health profissionals have the contact and opportunity detectig a disease. With early diagnosis can prevent the development of disease.

Abstract:
Loss of appetite is together with fever and malaise hallmarks of infection. Loosing appetite during an acute infection such as influenza does not result in any longlasting effects, but loosing appetite during chronic diseases such as cancer or AIDS constitutes a risk factor for mortality. Food intake regulation during inflammation is orchestrated by the brain in response to peripheral inflammatory signals. It is known that expression of the prostaglandin synthesizing enzyme cyclooxygenase 2 (COX-2) is crucial for the mechanisms underlying inflammation-induced anorexia, and that prostaglandin E2 (PGE2) is involved in anorexia induced by interleukin-1 beta (IL-1β). In this thesis I examined the prostaglandin-pathways proposed to be involved in anorexia. We show that acute anorexia is dependent on COX-2 expression, while cancer-induced anorexia is mediated by cyclooxygenase 1 (COX-1), at least in the initial stages, suggesting that the signaling pathways for chronic- and acute anorexia are distinct. We were able to demonstrate that the pathway underlying acute anorexia is distinct from that of fever, and that taste aversion is prostaglandin independent. We could also show that both acute and chronic anorexia-cachexia is dependent on expression of myeloid differentiation primary response gene (MyD88) in hematopoietic/myeloid cells. In summary, the findings presented in this thesis suggest that anorexia is a result of many different signaling pathways, as opposed to what is the case for several other inflammatory symptoms such as fever and malaise, where the pathways have been shown to be very exclusive. This provides new insight into the diversity of the pathways underlying inflammatory symptoms, which is fundamental for the ability to present potential, symptom-specific drug targets.

Resumo: A anorexia tem-se despontado como um dos grandes sintomas da atualidade. Especialmente entre jovens, a recusa radical da alimentação, associada a uma busca por um ideal de corpo esbelto, pode levar a situações extremas de emagrecimento, chegando, em alguns casos, a causar a própria morte. O aumento da incidência de anorexia fez com que as ciências da saúde e os serviços públicos a tomassem como uma de suas grandes preocupações. Diversas pesquisas surgiram, tentando compreender melhor a anorexia e aprimorar as técnicas de intervenção. Da mesma forma, os serviços públicos começaram a tratá-la com maior prioridade, em alguns casos, até criando ambulatórios especializados. Esta pesquisa visa a analisar os sentidos da anorexia produzidos, principalmente, pela literatura científica sobre o assunto e expressos em algumas narrativas de pacientes com esse diagnóstico. Para tanto, fizemos um levantamento do conceito e histórico da anorexia, detendo-nos, primeiramente, em Freud e psicanalistas que abordam diretamente o assunto, considerando ainda a descrição psiquiátrica dessa problemática. Posteriormente, buscamos autores da atualidade que tratam do problema, não apenas da ótica estrita da Psicanálise, mas também daqueles que analisam a anorexia como sintoma do nosso tempo. Ao lado das leituras, realizamos um trabalho de campo, mediante entrevistas com anoréxicas atendidas em um Programa de Transtornos Alimentares ligado a uma Faculdade de Medicina. Ao final, dividimos o material em temas, os quais possibilitaram uma aproximação a essa problemática, tanto do ponto de vista teórico, quanto por meio dessas experiências empíricas. Abstract: Anorexia has become one of the great symptoms of the present time. Especially among the youth, the radical refusal to eat, associated with the search for an ideal slim body, which may lead to extreme situations of weight loss, in some cases, resulting in death. The increased rate of anorexia has prompted the health sciences and public services to make it one their great concerns. Multiple research works have been conducted to better understand anorexia and improve the intervention techniques. In the same way, the public services began to treat it with greater priority, in some cases, to the extent of creating specialized outpatient facilities. This research has the purpose of analyzing the significance of anorexia, mainly produced by scientific literature on the subject and expressed in some narrations of patients with this diagnostic. Therefore, we made a survey of the concept and historical study of anorexia, beginning with Freud and psychoanalysts who directly address the subject, also considering the psychiatric description of this issue. Subsequently, we searched current authors on the problem, not only from the strict view of Psychoanalysis but also those who analyze anorexia as a symptom of our time. Simultaneously, we executed a field work through interviews with anorexics treated in a Program of Eating Disorders connected to a Faculty of Medicine. Lastly, we divided the material into topics, which made possible a close approach this problematic issue, from the theoretical point of view as well as these empirical experiments. We observed that the point of view of the researched authors is different, especially regarding the treatment of anorexia. Abstract:
This thesis explores anorexia nervosa in young people. It comprises two components: a play script, "Frames", about 16-year-old Elizabeth who is combating the disorder, her troubled friend Ben, and their families, and a critical essay that reviews qualitative and quantitative literature about anorexia and contextualises the play in relation to selected narratives, plays and films. In my field research, I interviewed a number of professionals working in the field of anorexia and the essay also discusses their views and insights. Few plays deal with anorexia in an in-depth manner to explore the onset and development of the illness. "Frames" addresses a gap in the literature by focusing on some predisposing factors that may lead to anorexia. It depicts the emergence of the condition in a middle-class Australian teenager living in affluent circumstances because research suggests a higher-than-average incidence of anorexia in this cohort. The play's structure is based on the concept of frames of experience and of perception to suggest that these influence people's lives, often without their awareness and to their detriment. Throughout the play, a series of frames is used to represent different times and situations, with each one encapsulating a significant event or moment in Elizabeth's life. For instance, the first frame represents Elizabeth at age 10, while the second frame signifies her family's perspective. The play suggests that changing our perceptual frame might help us change our lives. I argue that theatre, given its performative aspect, is an appropriate vehicle to illustrate the identity conflicts that characterise anorexia. Different aspects of Elizabeth's identity are presented through the many derivatives of her name, including Lizzie, Beth and Eliza. The theatrical form also allows the dramatisation of significant relationships and their impact upon her life. The primary focus of the thesis is the aetiology of anorexia and, in particular, the multi-factorial approaches suggested by the research literature. Many theories have been put forward to explain the disorder and are discussed in the essay, including medical, psychological, sociocultural, feminist and philosophical theories. The primary approach is psychological, as "Frames" focuses on family and personality factors precipitating Elizabeth’s crisis, together with a therapeutic intervention. However, sociocultural and philosophical values clearly impact on anorexia. The slender beauty ideal, the importance of appearance for societal success, and dualistic thinking that privileges mind over body and figures woman as body all play significant parts in the proliferation of eating disorders.

This thesis explores pro-anorexia, a desire amongst some anorexics to maintain or even enhance their existing illness. Through ethnographic fieldwork in an English eating disorders unit and with participants to pro-anorexia websites this anthropological research centrally explores relationships between personhood and illness as they are articulated and experienced by anorexic informants. Without assuming pro-anorexic desire to be either fully agential of just a product of the illness itself, the thesis traces the ebb and flow of informants’ desires for anorexia and the selves, bodies and anorexia(s) that such processes enact. Moving across these two field sites reveals not only how each illuminates pro-anorexia in the other, but also that both tangibly impact on ways in which informants experience and desire anorexia in the other. This thesis therefore suggests that in both spaces relationships between anorexia and personhood, agency and desire, bodies and eating, are perhaps both ‘virtual’ and ‘actual.’ Importantly, an analytic emphasis on exploring experiences and subjectivities, rather than on corporeal emaciation, seeks to listen to informants’ desires for anorexia whilst also recognising the realities of the illness. This reveals how desire and ambivalence are often conjoined as pro-anorexia signifies both a response to, and reconfiguration of, compromised conditions of possibility. By exploring ways in which seeming antitheses such as pro-anorexia and biomedicine or desire and ambivalence are not only related, but are mutually productive, in informants’ accounts, this thesis not only addresses the largely unexplored topic of pro-anorexia; it also reflects on the practices of anthropology itself, its ethics, ways of looking – or perhaps listening – and its modes of representation.

Abstract:
This thesis sought to develop and assess a novel scale to measure the experience of an anorexic voice in anorexia nervosa. It consists of three standalone papers. Papers one and two have been prepared for submission to two separate journals relevant to the methodology and research area for each, and comply with the journal guidelines. Paper one is a thematic synthesis of qualitative literature exploring "inner speech" for individuals with a diagnosed psychological disorder. Following a standardised procedure for thematic synthesis, six databases were searched and 10 studies were included. In total, three analytical themes were identified within the data: 1) Withdrawing to an inner world, 2) A fragmented and conflicted self and 3) Re-connecting with the outside world. The results found several similarities regarding the nature, function and negative consequences of engaging with critical inner dialogues across clinical samples. IS as both a coping mechanism and a maintaining factor of psychological distress was a key finding. The results indicated therapeutic approaches facilitating acceptance of internal experiences and disengagement from critical internal dialogues may be helpful. Paper two is a two-stage quantitative study that describes the development and assessment of a novel scale, the Experience of an Anorexic VoicE Questionnaire (EAVE-Q). Using data collected in an earlier qualitative study, Stage 1 generated scale items and tested for face and content validity using cognitive interviews with seven individuals with a diagnosis of anorexia nervosa. In Stage 2, 148 individuals with a diagnosis of anorexia nervosa were recruited from mental health services, eating disorder charities, the community and online to complete the EAVE-Q and measures of eating disorder symptoms, mood and quality of life. The dimensionality of the scale was assessed using principle axis factoring and associations between the EAVE-Q domains and clinical outcomes were evaluated. Factor analysis derived an 18 item scale with five domains. Two domains related to negative consequences of the AV, two to positive functions and one to the externalisation of the AV from the self. There were significant associations between individual EAVE-Q domains and eating disorder symptoms, mood and quality of life. The results highlighted the value in further research to refine the EAVE-Q, and to use this in future research to assess if changes in AV experiences are associated with clinical change over time. Paper three is a critical appraisal of this research. It includes reflections on the research process, discussion of the methodological limitations, and consideration of the implications of the findings for existing theory, future research and clinical practice.

L’anorexie mentale (AN), un trouble du comportement alimentaire multifactoriel, se traduit par une perte de poids. La sévère dénutrition retrouvée dans l’AN est associée à des altérations métaboliques induisant une dérégulation de l’axe intestin cerveau. Les mécanismes physiopathologiques sont encore mal connus. Le travail de cette thèse était de mieux appréhender les dysfonctions de l’axe intestin cerveau en évaluant le métabolisme protéique de divers tissus (hypothalamus, côlon et estomac) dans un modèle murin d’anorexie par une approche protéomique. Le premier travail a permis de mieux caractériser le modèle d’anorexie nommé activity-based anorexia (ABA) en fonction du sexe. Puis les différentes analyses protéomiques ont permis de constater une adaptation tissu dépendant des mécanismes régulant l’équilibre énergétique, avec une activité cérébrale potentiellement augmentée au détriment des fonctions digestives. Chez les souris femelles ABA, il a été constaté une augmentation d’expression de protéines mitochondriales au niveau de l’hypothalamus et à l’inverse, une diminution du métabolisme protéino-énergétique au niveau colique avec un rôle de la voie de signalisation mTOR. L’autophagie était augmentée dans ces deux tissus. Ensuite, nous avons démontré un ralentissement de la vidange gastrique secondaire à la dénutrition, et l’analyse protéomique a permis de constater une augmentation du stress oxydant au niveau de l’antre des souris ABA femelles. Ces altérations peuvent contribuer aux troubles fonctionnels gastro intestinaux. En conclusion, nos études soulignent des mécanismes d’adaptation tissu dépendants dans l’anorexie, qui devront être ultérieurement approfondis
Anorexia nervosa, a multifactorial eating disorder, is a major public health problem and results in a severe body weight loss. The severe malnutrition observed in anorectic patients is associated with metabolic alterations inducing disturbance of the gut-brain axis. However, involved mechanisms remained poorly understood. The aim of the present thesis was to better understand the alterations of the gut-brain axis in the activity-based anorexia (ABA) model by evaluating the protein metabolism of various tissues (hypothalamus, colon and stomach) by proteomic approach. Firstly, we have better characterized the response to ABA model according to sex. Then, different proteomic analyses were performed using female C57BL/6 mice. Our results revealed a tissue-dependent adaptation of protein and energy metabolism with an increased hypothalamic activity and a decrease in the gastrointestinal tract. Indeed, ABA mice exhibited an increased expression of proteins involved in mitochondrial metabolism at the level of the hypothalamus, and conversely a decrease of proteins involved in protein and energy metabolism in colonic mucosa with a key role of the mTOR signaling pathway. Both in hypothalamus and colon, autophagy was increased. We were also able to show that gastric emptying was delayed in ABA mice that is mainly due to malnutrition. In addition, proteomic analysis revealed an increase in gastric oxidative stress in female ABA mice. These alterations may contribute to the gastrointestinal functional disorders frequently described in anorexia nervosa. In conclusions, our study underlined tissue-dependent adaptive metabolic process during anorexia that should be further explored

Bakgrund: Anorexia nervosa blir allt vanligare bland unga kvinnor. Med rätt behandling och stöd tillfrisknar ändå de flesta från sin ätstörning. Syfte: Syftet med litteraturstudien var att ur en ung kvinnas perspektiv som har haft anorexianervosa, beskriva faktorer som har betydelse för tillfrisknandet från ätstörningen, för att sjuksköterskan ska få en ökad förståelse för unga kvinnor med anorexia nervosa och därmed kunna hjälpa dem. Metod: Litteraturstudien baseras på tio vetenskapliga artiklar, både kvalitativa och kvantitativa, som analyserades och utformades till studiens resultat. Resultat: De faktorer som hade betydelse för tillfrisknandet var att den unga kvinnan själv kom till insikt om sin sjukdom och lärde sig begripa och hantera sina känslor, sin självbild och självkänsla. Tillfrisknandet påverkades även av den unga kvinnans egen motivation till att vilja tillfriskna och ändra sitt matbeteende. Resultatet visade att sjuksköterskan hade en viktig roll i att vägleda den unga kvinnan med anorexia nervosa genom tillfrisknandeprocessen. Andra personer som familj, vänner, partner, terapeuter och sjukvårdspersonal hade också en betydelsefull roll, men även aktiviteter för att sysselsätta sig var viktigt. Slutsats: För att en ung kvinna med anorexia nervosa ska kunna tillfriskna krävs rätt stöd från familj och vänner men även sjuksköterskor med rätt utbildning har en betydelsefull roll i att stödja kvinnan till ett liv utan anorexia nervosa.

 

 

 


Background: Anorexia nervosa is increasing more and more among young women, although with accurate treatment and support most of them recover from their eating disorder. Aim: The aim of the literature study is to describe factors of importance concerning recovery from an eating disorder from a young woman’s perspective, in order for nurses to receive an increased understanding of young women with anorexia nervosa. Method: The literature study is based on ten scientific articles, qualitative as well as quantitative, which were analysed to obtain the result of the study. Result: Factors of importance for recovery were that the young woman herself obtains an awareness of her disease and learns to understand and handle her feelings, her selfimage, self-esteem and the young woman’s motivation and will to get well and to change her eating habits. In addition the result also shows that nurses have an important role in guiding the young woman in her process of recovery, as well as family, friends, partners, therapists and remaining medical staff. Finally occupational activitites are also vital for the recovery process. Conclusion: In order for a young woman suffering from anorexia nervosa to recover she needs support from family and friends but especially from nurses with suitable training and education. They have an important role in supporting the young woman on her way to recovery to a life without anorexia nervosa.

Man har genom forskning inte kunnat dra någon generell slutsats angående orsaksfaktorer bakom anorexia nervosa. Alla drabbade har egna erfarenheter och tankar om varför just hon eller han blivit sjuk. Vårt syfte var att i möjligaste mån återspegla de i studien utvalda kvinnornas beskrivningar av tankar och erfarenheter kring sitt sjukdomsförlopp, utifrån familjeförhållanden och individuella faktorer. Vi ville veta vad de ansåg vara orsaken till just deras insjuknande, men också vad som var anledningen till att de slutligen bestämde sig för att bli friska. Detta syfte ledde oss till frågeställningar som: Hur kan familjen påverka ett insjuknande i anorexia nervosa? Vilka individuella faktorer kan influera? Vad ansåg de vara vändpunkten för sitt tillfrisknande? För att få en så tydlig bild som möjligt av kvinnornas tankar och erfarenheter behövde vi se en helhet, varför vi valde att göra en kvalitativ studie med djupintervjuer. I arbetet framförs tidigare forskning om familjeförhållanden och individuella faktorer som kan ha en inverkan på insjuknandet, samt om vändpunktsprocesser. Vår granskning gick ut på att tolka kvinnornas berättelser i jämförelser med den forskning vi använt oss av. I vårt resultat framkommer att berättelserna till viss del liknar varandra, och att vissa jämförelser kan ses med forskningen, men att erfarenheterna trots allt är unika och därmed skiljer sig för de fyra kvinnorna.

Anorexia nervosa (AN) is a serious disorder with long-term consequences for those afflicted. No evidence-based care is available for adults with full or subthreshold AN. The thesis research investigated aspects of emotion and cognition relevant to the maintenance of AN that might inform psychological treatment. In addition, the effectiveness of a recent psychotherapy model of AN was investigated. Study I investigated alexithymia and emotional awareness and their associations with depression, anxiety, and perfectionism among patients with AN compared with a control group. The AN group exhibited the same level of emotional awareness as did the control group and the same level of alexithymia when controlling for depression and anxiety. Alexithymia and emotional awareness were not associated, despite representing an overlapping construct. The results of the present study indicate that those with AN can trust their emotional awareness. Study II explored implicit pro-thin and anti-fat attitudes (towards the self and others), striving for thinness (loosely corresponding to positive reinforcement), and avoidance of fatness (loosely corresponding to negative reinforcement). The AN and the control groups were found to have equally strong implicit pro-thin and striving for thinness attitudes. The AN group exhibited stronger implicit anti-fat and avoidance of fatness attitudes (loosely corresponding to negative reinforcement) than did the control group. There was no association between implicit and explicit measures. The results are in line with the over-evaluation of weight and shape as a core feature of eating disorders. Study III compared the effectiveness of Acceptance and Commitment Therapy (ACT) and treatment as usual (TAU) for adults with AN after day-care. Follow-up measures indicated no difference in improvement or deterioration between the two groups. The level of perfectionism was reduced in the ACT group relative to the TAU group. The study was compromised by a lower inflow of patients than anticipated and by a high drop-out rate, and thus fails to provide evidence of a difference between the two groups. The present thesis demonstrates that emotional awareness is intact in those with AN and that implicit attitudes concerning weight and shape reflect the explicit attitudes, although without association. The treatment study indicates that, when designing treatment, it is important to consider the ambivalence to treatment among those suffering from AN, which is reflected in the high drop-out rate in the present study.

Abstract:
This study compared cognitive test performance between matched controls and participants with anorexia (N=30) and bulimia (N=30). The results revealed that both of the eating disorder groups demonstrated a considerable number of neuropsychological deficits. However, between-group significance was not established and the deficits were accounted for by a small subgroup of poorer test performers. On analysing group results on a wide range of cognitive tests, both clinical groups performed more poorly than controls on attentional, learning and visuo-spatial/constructional tasks. There was however, no evidence of executive difficulties in either group. On partialling-out the effects of anxiety and depression, between-group significance was removed on a considerable proportion of the test results. However, significance remained for the anorexia group on a digit coding task, two auditory-verbal learning tests, a measure of incidental visual learning and a measure of visuo-spatial/constructional functioning. These results suggested mild difficulties in cognitive functioning across neuropsychological domains which were not fully explained by the influence of emotional and other psychological factors. In the bulimia group, significance remained on a digit coding task and an omnibus attentional measure together with two dependent measures of auditory-verbal learning. There was also a trend for poorer performance on tests of visuo-spatial/constructional functioning. However, the results were largely accountable by the effects of emotional variables on attentional processes. The results of cognitive test performance in anorexia and bulimia were discussed with reference to previous studies and with reference to clinical practice.

Abstract:
Anorexia Nervosa (AN) is a well-documented but poorly understood psychological disorder. There is controversy over the conceptual clarity and efficacy of therapies for the condition and it continues to have one of the highest mortality rates of all psychiatric disorders. Recent psychological conceptualisations of AN have viewed anorexic behaviour as a coping strategy to deal with underlying general psychopathology. The search for predictors of AN has been a frequent topic in eating disorders research and anorexics have often been identified as perfectionistic and valuing achievement as a means to self-worth. The eating disturbance associated with AN has also been viewed as a means to enhancing self-perception. Research into coping has also achieved much attention. Antonovsky (1979; 1987) has posited the construct of Sense of Coherence which allows individuals to cope successfully with stress and avoid psychological ill-health. Part of this construct posits educational and vocational achievement as indicators of a strong Sense of Coherence. This study hypothesised that anorexics, with their high achievement orientation, would exhibit different patterns of scores on the Sense of Coherence questionnaire when compared to a group of controls and a group of depressed individuals. Results indicated that anorexics' SOC scores were significantly lower than controls' but significantly higher than depressed individuals'. Concern was expressed regarding the validity of the clinically orientated SOC research literature given the tendency to use this salutogenic measure in pathogenic research designs.

Abstract:
Anorexia nervosa (AN) is a chronic and complex psychosomatic condition, characterised by a primary drive to be thin and a refusal to maintain normal body weight. Only a minority of people diagnosed with AN ever become asymptomatic and more research has been called for to address high drop-out rates and lack of engagement in AN treatment services, in particular psychotherapeutic treatment. Prior studies have generally examined this problem in terms of patient mediated variables, such as attitudes and behaviours, with little focus on contextual factors. Research that has studied therapeutic engagement in the area of AN has yet to examine psychotherapeutic treatments-in-practice. Guided by this gap in the literature this thesis examines ways in which therapists engage with adolescents diagnosed with AN in naturally occurring psychotherapeutic interactions. A secondary and concurrent focus is to look at how the therapists’ underlying theoretical models are reflected in in situ practice. The data corpus comprises twenty-four therapy sessions recorded in an eating disorders programme based in a children’s hospital. In contrast to eating disorders treatment statistics reported in the literature, the programme has a low drop-out rate, zero mortality rate and good long-term patient outcomes, making it an especially suitable setting to examine engagement. Drawing on methods from discursive psychology (DP) and conversation analysis (CA), a number of interactional practices are found which show how the key principles of engagement and neutrality are brought off, or achieved as such in turn-by-turn interaction. Central to the analysis, is the recurrent production of patients’ bodily states and conduct as delicate items. As these topics are also the primary focus of the institutional setting, the analysis shows how practices such as perspective display series and dispositional management allow delicately marked institutional tasks to be carried out. The analysis also examines how patients’ bodies and conduct are embedded in, and constituted as problematic in the interactions. Regularities, such as agentic repositioning in accounts, demonstrate the co-production of patients as psychologically compliant with treatment while physically non-compliant. This thesis contributes to work in applied CA concerning links between theoretical models and interactional practices by demonstrating naturally occurring regularities that describe key guiding principles of the eating disorders programme. It also builds on work in DP concerning examinations of the body and embodiment, by showing how patients’ physical bodies are an integrated feature of the interactions. Finally, this thesis has implications for a clinical audience in terms of extending therapists’ awareness of how engagement with patients is constituted interactionally, which also contributes to wider AN literature on ‘resistance’ to therapy.
