Abstract:
Poisoning with agricultural chemicals is common in rural Asia-Pacific with up to 300,000 annual deaths from pesticide self-poisoning. The pharmacokinetics and pharmacodynamics of pesticides can vary markedly depending on the chemicals ingested, the pesticide’s lipid solubility, enzyme reactivation, co-ingested toxicants, and extent of decontamination and organ dysfunction. Diagnosis and management is based on clinical signs and standard investigations. Staff should wear standard universal precaution attire for examining and treating patients; nosocomial poisoning is rare. Management of poisonings should include careful airway intervention and administration of oxygen, except in suspected paraquat poisoning. Organophosphorus insecticide poisoning causes a cholinergic crisis with excess airway secretions and acute respiratory failure. Patients should be treated with intravenous atropine and observed for the neuromuscular disorder ‘intermediate syndrome’, which can cause further paralysis and respiratory failure after 24 hours. Few antidotes exist for other agricultural chemical poisonings with the mainstay of treatment being supportive standard ICU care.

Abstract:
Anaphylaxis is a severe life-threatening systemic hypersensitivity reaction, which occurs in approximately 10–20 per 100,000 population per annum, and accounts for 0.3% of adult critical care admissions. Anaphylaxis most commonly results from an exaggerated immune reaction to foreign antigens, prompting release of vasoactive substances from mast cells. A broad range of agents including foods, insect stings, latex, and drugs can trigger anaphylaxis. Common food allergens include nuts, shellfish, milk, and eggs. The most frequently implicated drugs include neuromuscular blocking agents (NMBAs) and antibiotics. The clinical features of anaphylaxis are variable and may occur together or in isolation. Epinephrine is the primary treatment for anaphylaxis, administered via the intramuscular route. Measurement of mast cell tryptase is essential for the diagnosis of anaphylaxis. Samples should be taken immediately, at 1–2 hours, and at 24 hours after the onset of symptoms. Investigations should be directed at identification of the trigger agent, and patients should be provided with information to enable them to avoid probable triggers and recognize future episodes.

Exploring London's literary identity during the 1930s Anna Cottrell shows how vital writing was to the capital’s booming leisure scene on the eve of the Second World War. The book explores London and Londoners, with a focus on the way in which London's lower-middle-class citizens became inseparable from central London’s leisure scene in the period’s imagination. In contrast with Modernism’s flâneurs and flâneuses, the key figures of 1930s London literature were shop girls, clerks, dance hostesses, and financially insecure journalists whose leisure hours were spent in London’s cinemas, bars, and glittering teashops. Writing about this type of Londoner and her milieus was at the heart of the decade’s experiments in revitalising the British novel, which to many of the period’s writers and intellectuals appeared to lack energy and authenticity. Meticulous description was central to this project of re-energising British writing, and it is in passages describing London milieus such as the teashop and the Soho nightclub that this book locates the decade’s most original and astute meditations on modernity, mass culture, and the value of ordinary lives.

Abstract:
Sudden cardiac death (SCD) is defined as unexpected death due to a cardiac disease, in a patient with or without known cardiac disease and which occurs within 1 hour from the appearance of the first clinical symptoms. The sudden cessation of cardiac activity leads to haemodynamic collapse, typically due to sustained ventricular tachyarrhythmias. The event is described as an aborted SCD (or sudden cardiac arrest) when an intervention (e.g. defibrillation) or spontaneous reversion restores circulation. The lack of uniformity with this definition complicates SCD statistics. By convention, the use of SCD to describe both fatal and non-fatal cardiac arrests persists. SCD continues to be a leading cause of death in Western countries, and accounts for 15%–20% of all natural deaths in adults in the US and Western Europe, and up to 50% of all cardiovascular deaths. In the US, estimates of SCDs from retrospective death certificate analyses range from 300 000 to 350 000 annually, giving an incidence of 0.1%–0.2% per year amongst the population above the age of 35 years. Event rates are said to be similar in Europe, although worldwide incidence is difficult to estimate and varies in accordance to the prevalence of CHD. The incidence of SCD increases with age and underlying cardiac disease. There is also a male preponderance, with men 2–3 times more likely to experience SCD than women, and this reflects the higher incidence of CHD in men.
